Frequency of thyroid disorders in patients with menstrual irregularities presenting to the OPD in tertiary care hospital.

Objective: To determine the frequency of thyroid disorders in patients with menstrual irregularities presenting to the OPD in tertiary care hospital. Study Design: Cross Sectional study. Setting: Department of Outpatients Obstetrics & Gynaecology, Ward 8, Jinnah Postgraduate Medical Center, Karachi, Pakistan. Period: July 30, 2021 to January 29, 2022. Methods: All patients who met the inclusion criteria and visited JPMC, Karachi were enrolled in the study. After providing a thorough explanation of the procedure, risks, and benefits of the study, informed consent was obtained. The study involved conducting detailed history and examination, as well as recommending relevant laboratory investigations such as thyroid and ultrasound pelvic/TVS profiles. Results: 150 women of thyroid disorders in patients with menstrual irregularities were included with mean age was 33.72±7.62 years, mean height was 1.69±0.07 meter, mean weight was 76.60±12.70 kg, mean BMI was 26.78±4.13 kg/m2, mean parity was 2.84±2.05 and mean duration of menstrual irregularity was 0.82±0.07 days. Positive family history of thyroid disorder was found to be in 15 (10.0%). Positive history of thyroid surgery was noted in 26 (17.3%) patients. Thyroid disorder was noted in 57 (38.0%) cases. In distribution for type of thyroid disorder, subclinical hypothyroidism was noted in 37 (64.9%), 9 (15.8%) overt hypothyroidism, 11 (19.3%) subclinical hyperthyroidism while overt hyperthyroidism was noted in 0 (0.0%). In distribution of menstrual complain, amenorrhea was noted in 3 (5.3%), 3 (5.3%) oligomenorrhea, 7 (12.3%) metrorrhagia, 32 (56.1%) menorrhagia while polymenorrhea were noted in 12 (21.1%) cases. Conclusion: It is to be concluded that thyroid disorder is frequently observed in patients experiencing menstrual irregularities, as indicated by the study. However, further research is required to validate these finding.

Background: Outpatient department is considered a doorway to health services. The outpatient department thus requires a systematic study of its services for its efficient management and functioning. Time-motion study in ARV OPD provides information on how much time it takes for management of animal bite patients at each step. Such a study could prove useful to improve efficiency of health care services. Objective: To assess the average time spent in different service delivery points in ARV OPD Methodology: A hospital based cross0sectional study was conducted in ARV OPD from July 2022 to June 2023 on Patients coming to ARV OPD. A total of 960 patients were included in the study using Systematic Random sampling technique. Data was collected using a pre-designed and pre-tested questionnaire for socio-demographic details and case history of patients and time required for each step in management of animal bite. Stopwatch of smartphone was used to record time at each step of patient management. All the collected data was entered in SPSS software (version 20.0) and was analyzed accordingly. Results: Study results showed that out of 960 patients 613 (63.85%) were male and 347(36.15%) were female. 509 (53.02%) were from urban and 451(46.98%) were from rural area. Out of 960 patients 816 (85 %) were of category III bite and 144 (15%) of category II bite. The most common site for animal bite was lower limb 556 (57.9%) followed by 288 (30%) on upper limb, on trunk 38 (3.96 %) and 78 (8.14%) on head and neck region. Total time required for management of animal bite was 35.15+/- 17.92 minutes. Conclusion: The time spent in transit could be reduced with proper management of logistics. Also reframing of strategies at various steps could help reduced average time needed to attend a patients at ARV OPD.

Embracing Motherhood: Impact of Nurse Led Fear and Apprehension Related to Labour or Childbirth Improving Intervention (FALCI) in Reducing Childbirth Fear Among Pregnant Women.

Childbirth is a complex process with various dimensions including physical, psychological, emotional, social, and cultural aspects. The study intended to assess the effectiveness of Nurse led FALCI (Fear and Apprehension related to Labour or Childbirth Improving) Intervention in reducing childbirth fear among pregnant women. A quasi-experimental study was conducted among 115 pregnant women (selected through purposive sampling) at antenatal OPD of tertiary care hospital of Bhopal. Data was collected using Wijma Delivery Expectancy Questionnaire (DEQ) and self-structured questionnaire on socio-demographic and clinical variables. The intervention (FALCI) was carried out in two phases, each lasting for 30 minutes conducted at week 3-4 and week 5-6 and information booklet was provided for reinforcement. The data was analysed using two-way repeated measures mixed ANOVA (within group and between groups). Findings revealed that most of the pregnant women in both experimental (68.5%) and control (90.2%) group had high level of childbirth fear. Post-intervention data revealed mean of pretest, posttest 1 and posttest 2 were 73.85, 46.94 and 37.90 in experimental group and the mean of pretest, posttest 1 and posttest 2 were 75.49, 72.59 and 72.67 in control group respectively and found that there is significant difference in childbirth fear (within group, F value=114.69, p<0.001 in experimental group and F =0.575, p=0.564 in control group) and between groups (F value= 81.52, p value <0.001). However, no significant association was found between childbirth fear and sociodemographic/clinical variables. The study findings revealed that Nurse led FALCI intervention was effective in reducing childbirth fear among the pregnant women and it is recommended that such intervention must be planned at all levels and implemented as routine protocol in maternity services.

Pattern of pediatric dermatoses in a tertiary hospital in South India

Background: Several cross-sectional studies in the form of school surveys and studies on paediatric patients visiting dermatology OPD in tertiary care hospital has been conducted in the last three decades. This study was conducted to note the pattern of paediatric dermatoses and also to compare the difference in disease pattern with other studies conducted in southern India in last decade. Methods: A retrospective cross-sectional study was conducted at the Dermatology department of MVJ teaching hospital to determine the pattern of pediatric dermatoses from May 2021 to October 2021 after obtaining institutional ethics committee clearance. Cases under the age of 18 years were included in the study. Cases with age above 18 years, incomplete data, dermatoses where diagnosis was uncertain were excluded from the study. A total of 150 children were included in the study. The results were entered and tabulated in MS-excel sheet and descriptive analysis was done. Results: 150 children were included in the study.36 types of dermatoses were noted overall. Eczematous diseases (32.7%) contributed to the maximum followed by infections and infestations (25.3%). The most frequent dermatoses found were Atopic dermatoses (10%), pityriasis alba (8.6%), scabies (8.6%), Acne vulgaris (8.6%). Eczematous diseases and hypersensitivity disorders were noted to have increased frequency when compared to other studies in last one decade Conclusions: Rising trend in atopic dermatitis has been noted which might be due to changing environmental factors and also due to better recognition of condition by physicians in last one decade. A study of psychological impact of acne vulgaris on quality of life using CADI &amp; DLQI scoring in patients attending dermatology OPD in tertiary care hospital

Acne Vulgaris is a chronic inflammatory disease with multifactorial etiology. It is one of the leading reasons for visiting a dermatologist. Acne patients undergo considerable amount of psychological impairment as face is socially and psychologically the most important body region. There is no sufficient data available on the Impact of acne on QoL in Indian patients.To Determine the Impact of Acne Vulgaris on Quality of Life using two different scoring systemsi.e.CADI &amp; DLQIIt is a hospital based cross sectional study done at dermatology Opd in tertiary care. It is a one year study from January 2016 to December 2016. 500 diagnosed cases of acne Vulgaris of the age groups 15-30 were studied. Informed written consent and Photographs was obtained from all patients. A complete history, clinical examination, was done in all patients. A baseline ‘global acne grading system’ was being performed. Quality of life in acne vulagaris patients were studied using pre- structured questionnaire of DLQI (Dermatological life quality index) &amp; CADI (Cardiff acne disability index scoring systems).Based on GAGS, 80% of the patients were mild, 19% moderate and remaining 1% had severe grading of acne. Majority of the patients had very large effect on quality of life (76%) on DLQI scoring and medium impact in quality of life (86%. 12.4%) on CADI scoring.The results of the study show that majority had medium &amp; large effect on QoL. Hence the disability caused by acne must be taken into account when individualizing treatment by health professionals and include QoL measurements to provide better care.

Association of Postpartum Depression with Social Support: A Cross-Sectional Study in A Hospital Setting of Rawalpindi-Islamabad, Pakistan

In Pakistan, the association of factors related to depression needs to be explored a little more because here when a woman discusses or shares her problems, her concerns are dismissed and turned down, the same reason women don’t express their feelings and seek help. Objective: To determine the association between postpartum depression and social support among women presenting at a hospital in Rawalpindi-Islamabad, Pakistan. Methods: This cross-sectional study included 310 women presenting to the obstetrics and gynaecology OPD, vaccination center and psychiatric OPD of Tertiary Care Hospital within postpartum phase. Participants were recruited via convenience sampling. The Structured questionnaire included demographics and gynaecological history. Edinburgh postpartum depression scale (EPDS) was used to classify patients at risk of postnatal depression. Social support was scored on multidimensional scale of perceived social support. Data analysis included descriptive statistics such as frequency &amp; percentages and inferential statistics consisted of Pearson Chi-Square, independent samples t-test and Multiple Linear Regression. Statistical analysis done using IBM SPSS version-26. Results: The results indicated that the social support from significant other and family members is significantly associated with the development of postpartum depression (p-value &lt;0.05). Furthermore, age of the mother, type of delivery, baby feeding habits, and history of being unwell during pregnancy did not present to have significant association with the development of postpartum depression. Conclusions: The findings of the study conclude that the development of postpartum depression is most importantly affected by lack of support from the family and husband after childbirth

EMERGENCY CONTRACEPTION: KNOWLEDGE, ATTITUDE, AND PRACTICES AMONG HEALTH CARE PROVIDERS

Background: After unprotected sexual relations, emergency contraception (EC) is recommended to prevent unplanned pregnancy and its adverse implications. Its proper application can help to avoid unwanted pregnancy or unsafe abortion, which can reduce maternal mortality. As a result, the current study sought to investigate the knowledge, attitudes, and practices of married females attending OPD of tertiary care hospital regarding EC. Methods: The current cross-sectional study was carried out among married females who were willing to participate and visited the OPD of a tertiary care hospital over the course of three months. A questionnaire was used to collect data, and proportions were used to present the ndings. Results: 300 women were ready to give written consent were recruited for study. 90% of those surveyed had heard about EC, with media being the main informational source. 81% correctly identied the time when oral tablets were used as EC, and 91% were aware that this was the case. 73% of people would be willing to use EC themselves in the event of unprotected intercourse, and 85% would recommend it to a friend or relative if necessary. About 70% of those surveyed have ever used EC. Conclusions: High literacy among married women explains their high awareness, positive attitude, and adequate use of EC. It shouldn't make authorities complacent; rather, they should push initiatives to reduce maternal mortality using regular contraceptives besides EC.

Vitamin D status in 5-15 years old children attending pediatric OPD in tertiary care hospital in Jammu and Kashmir, North India: a cross sectional study

Background: Vitamin D is vital in children mainly because of its profound effect on growth, development, immune system and importantly bone health. Recent evidence suggests maintaining 25 (OH) D levels above 20 ng/ml for maximizing health benefits. Objectives were to assess the vitamin D status in children in the age group of 5-15 years attending a tertiary care teaching institute of North India and to analyze the factors which can contribute to vitamin D deficiency in these children. Methods: It was a cross-sectional study. 5-15 years old children attending the outpatient department were included in the study. Children with co morbid conditions that affect vitamin D metabolism and those children on chronic drug treatment and on vitamin D supplementation were excluded from the study. Results: Average age of study population was 8.93±2.02. In the study 170 (18.47%) children among a total of 920 had a normal vitamin D status while the rest 750 children (81.52%) had insufficient vitamin D status (25 hydroxy vitamin D &lt;30 ng/ml). Among these children 190 (26.38%) had suboptimal vitamin D levels, 300 (40%) were deficient, 143 (19.06%) had severe and 117 (15.6%) had very severe deficiency of vitamin D. The average vitamin D level among the study population was 12.33±11.13 ng/ml. There was significant statistical association between vitamin D deficiency and stunting (p=0.003). No statistically significant association was found between outdoor activity. Conclusions: Vitamin D insufficiency was documented in 81.52% of 5-15 years old children and there was a significant association between stunting and vitamin D deficiency.

Chondroblastoma in a Distal Phalanx of the Great Toe &amp;ndash; A Rare Case Report

Chondroblastoma is a rare benign appearing lesion arising in the epiphysis of long bones in skeletally immature individuals. It is a cartilage-producing neoplasm constituting &lt; 1% of all bone tumors. A 16 years old girl presented to orthopedic OPD of tertiary care hospital with swelling of her left great toe for 3 years. Clinicomycological &amp; epidemiological profile of superficial fungal infections caused by dermatophytes in a tertiary care centre, Ahmedabad, India.

Purpose : (i)To determine the prevalence of dermatophytosis in the study population attending the skin OPD of our institute.(ii) To identify host &amp; epidemiological factors that affect the occurrence of disease. (iii)To isolate &amp; identify dermatophytes from clinically suspected cases of tinea &amp; to correlate their occurrence with the epidemiological factors. Materials &amp; Methods : Total 260 scrapings from skin, scalp &amp; nails were taken from clinically suspected cases of tinea attending the OPD of tertiary care hospital of Gujarat,India. Direct examination with KOH mount was done. The scrappings were cultured on SDA &amp; identified based on their morphologies &amp; biochemical reactions. Results :Of the total 260 specimens, KOH positivity was 34.61 % &amp; culture positivity was 28.84%. Females(50.38%) were affected more than males(49.61%) &amp; maximum cases of tinea were present in 31-40 years(21.46%) age group. Housewives(36.15%) &amp; daily wagers(18.84%) were the most inflicted occupational groups. Maximum specimens were from skin(36.15%), followed by nails(33.46%) &amp; hairs(30.38%). T. capitis &amp; T. corporis were commonest clinical types identified, with T. capitis majorly present in paediatric(&lt; 10 years) age group(49.29%). Maximum isolates of dermatophytes were T. violaceum(29.33%), followed by T. mentagrophytes(24%) &amp; T. rubrum(12%). Conclusion : In India, T.mentagrophytes &amp; T.rubrum have been found to be more common but in our study T.violaceum was the commonest dermatophyte isolated; which was due to more number of T. capitis cases. Activities that involve laborious tasks make a person more prone to dermatophytosis. Hence, better practices of self-hygiene can help reduce the incidence alongwith community education.

The clinical study of dermatophytosis in a tertiary care hospital of konkan region, Maharashtra

Dermatophytes are aerobic fungi that produce proteases that digest keratin and allows colonization, invasion and infection of the stratum corneum of the skin, the hair shaft, and the nail. This study was an attempt on the part of the author to find out the incidence and various clinico-epidemiological characteristics of Dermatophytosis in skin OPD of Tertiary care Hospital, B.K.L.W.R.M.C., Dervan. The study was carried on 2444 patients of Dermatophytosis attending the OPD, out of which 49.79% of the cases were Tinea Cruris (TC). The age group of 11-20 years showed maximum prevalence of Dermatophytosis (22.17%). 58% of the patients had diffuse lesions. Body involvement was seen in 87% of cases. Itching was the most common condition associated with Dermatophytosis. 35% of the patients reported with family history of Tinea Cruris.Circumscribed variety of TC was found to be most common variant mostly observed in 2and 4 decade, with a male preponderance. Groin was the most common site with multiple lesions. Nail changes was most commonly associated with TC.

Association of Dental Caries with Body Mass Index in Patients Attending Dental OPD of Tertiary Care Hospital, Pakistan: A Cross-Sectional Study

Background: Dental caries area public health problem. Its prevalence is about 60% in the Pakistani population.&#x0D; Purpose: The purpose of this study was to correlate the association between decayed, missing, and filled teeth (DMFT) score and its body mass index in patients visiting the Dental Department of Liaquat University of Medical and Health Sciences, Jamshoro.&#x0D; Materials and Methods: This cross-sectional study was conducted from January 2021 to May 2021 at the Department of Operative Dentistry Liaquat University of Medical and Health Sciences. Patients with age 16 to 70 years with presence of atleast 15 to 18 permanent teeth were included in te study. Patients having primary teeth were set in exclusion criteria. For Dental caries status, decayed, missing, and filled teeth (DMFT) index was used. Height and weight were measured using digital scales. BMI was calculated according to the formula weight in kilograms/height in centimeters. Data were analyzed for descriptive statistics. One Way ANOVA test was used for association. The statistical significance level was evaluated at p&lt;0.05&#x0D; Results: We were able to obtain the data of a total of 203 patients of which 57% were female.The mean age was 36.7±13.8. ). The mean DMTF score was 5.7±3.7. In association of BMI with DMTF score, females BMI was statistically significant with DMTF score with p=0.039 and in overall was also significant p =0.002&#x0D; Conclusion: DMFT score was higher in overweight and obese individuals and it was significant statistically. However, large community based studies are required to confirm the association.

Quality of Life in Patients with Urinary Incontinence: A Cross Sectional Study

Background: Urinary incontinence in women severely affects the physical, social and psychological aspects of life. Objectives: To find out the Quality of Life in female patients with urinary incontinence in a tertiary care hospital. Materials and Methods: A cross-sectional, observational study was conducted in the Gynecology out-patient department. All non-pregnant women ≥21 years of age, attending the Gynecology OPD of tertiary care hospital for various complaints were included in the study. After obtaining written informed consent, demographics, relevant medical and surgical history was noted, and they were then administered two validated questionnaire 'Incontinence Quality of Life Questionnaire' (IQOL) and SF-36. Result: The overall incidence of incontinence in this population was 17.0%, with a steadily rising incidence by age. The incidence was more in postmenopausal than premenopausal women. Menopausal women compared to non-menopausal women also had significantly higher incontinence QOL scores (median of 5 vs. 3; p=0.001). Avoidance and limiting behavior (ALB), Psychosocial impact (PSI) and social embarrassment (SE) scores were studied improved after 24 weeks. Items determining ALB had high correlation coefficient with total ALB and all of them were statistically significant. Oral Health Status Among Pregnant Women Attending Gynae OPD of Tertiary Care Hospital in Rawalpindi

Objective: &#x0D; The objectives of this study were to determine the oral health status and treatment needs using DMFT &amp; CPITN indices in pregnant women attending Gynae OPD of tertiary care Hospital in Rawalpindi, to Determine Association of Oral health status using DMFT and CPITN with socioeconomic status and to determine oral health status of various trimesters of pregnancy&#x0D; Materials and methods: An analytical cross-sectional study was carried out to assess the oral health status and treatment needs among pregnant women at Tertiary Care Hospital, Rawalpindi from April’ 2020 to September’ 2020. Consecutive sampling was used to select the study participants. An adaptive version of WHO questionnaire was used. Results were presented in the form of frequency tables. Chi square test of statistics was applied to assess the association between categorical DMFT and CPITN with the sociodemographic characteristics of the participating females.&#x0D; Results: The mean DMFT score pregnant females was 2.41 + 2.30 and were belonged to a middle socioeconomic status 43.8% (n=173). About 61.8% (n=244) of participants had DMFT total score 1-7. Whereas, majority of the participants were reported bleeding gums on probing 30.6% (n=121) on assessment by CPITN. The participating pregnant females also depicted the worsening of periodontal tissues (P=0.00) and dentition status (P=0.02). Socioeconomic status was not significantly associated with dentition (P=0.39) and periodontal status (P=0.69).&#x0D; Conclusion: The study revealed that oral health status was deteriorated during pregnancy. Education and gestational period were strong indicator for oral health status among pregnant women. Bleeding gums were reported in majority of participants during second trimester of pregnancy. Socioeconomic status was not significantly associated with oral health status by CPITN and DMFT score.

ASSOCIATION OF ANKLE BRACHIAL INDEX WITH CARDIOVASCULAR AND SOCIODEMOGRAPHIC RISK FACTORS IN DIABETIC POPULATION

Lower-extremity arterial occlusive disease is much more likely to occur in patients with type II diabetes. Due to rapidly increasing prevalence of Diabetes and its dreadful complications like amputation and associated mortality, it is crucial to screen symptomatic as well as asymptomatic diabetic patients for peripheral arterial disease (PAD) at an early stage. There is immense evidence stating increased risk of coronary vascular disease(CVD) and PAD in diabetics. The risk factors being similar to that of CVD, there is less information available regarding association of Ankle brachial index with individual cardiovascular risk factors in diabetic population. This study was done with the objective to evaluate association of Ankle Brachial Index (ABI) with Physical activity level, chronicity of Diabetes Mellitus ,cardiovascular risk factors and sociodemographic factors. Methods:Type 2 diabetic patients coming to diabetic-endocrinology OPD of tertiary care hospital were evaluated for Fasting Blood Sugar, Serum Total cholesterol and Serum Triglycerides, International Physical Activity Questionnaire- long form (IPAQ LF). ABI was evaluated and interpreted as per guidelines of American Heart Association. Data was analyzed using SPSS for windows version 16 software. Results: Total 500 patients (male=271, female=229) with mean age 57.85 (± 9.66) years participated in the study. The association of ABI was statistically signicant with Physical activity level (p= 0.005), chronicity of Diabetes Mellitus (p&lt; 0.001), Glycemic control (p= 0.02), Serum Total Cholesterol (p &lt;0.001) and Serum Triglycerides (p = 0.032). The association of ABI with Waist circumference (p=0.307) and Smoking (p= 0.380) was statistically not signicant. The study also revealed signicant association of ABI with age (&lt;0.001), gender (&lt;0.001), weekly sitting time (p= 0.002), presence of claudication symptom (0.019), annual family income (p= 0.021)and family history of diabetes mellitus (p= &lt;0.001). Conclusions: This study revealed signicant association of Ankle Brachial Index with cardiovascular risk factors and socio-demographic parameters.

Treatment Pattern of dermatophytosis at the outpatient clinic of a tertiary healthcare hospital of Chhattisgarh, Central India

Background: in the recent past dermatophytosis are being highly prevalent, main culprit behind is inappropriate use of antifungal agents and self-medications which is leading to exponential rise in global burden superficial fungal infections, and antifungal drug resistance. Clinician’s management pattern data collection on time to time to have a check on the resistance/sensitivity pattern of the commonly prescribed antifungals in clinical grounds is important. Aims and objectives: The present study was to describe the pattern of antifungal drug prescription and self-medication for common superficial fungal infections of skin in dermatology OPD in tertiary care hospital of Chhattisgarh. Materials and Methods: Current study was a cross sectional unicentric study conducted at Dermatology OPD of Pt J.N.M. Medical College Raipur, the data collection for this observational study was undertaken for a period of six months. All patients attending the dermatology OPD with tinea infections were included in the study. This study was designed to record parameters include information related to demographic, disease profile, medications prescribed, number of drugs, dosage & duration were reported. Self medication pattern was also being observed during this study. Results: A total of 17286 patients attended dermatology OPD and 42% patients were suffering from tinea. The most common fungal infection was Tinea cruris 55.07%, the commonest treatment pattern was Combination of oral T amongst which clotrimazole (67.04%)were the commonest. Commonly prescribed individual antifungal was oral Fluconazole 73.11%. The self-medication prevalence for dermatophytosis was 62.26%. Clinicomycological study of dermatophyte infections in patients of DVL opd in tertiary care hospital at SANGLI

A research was carried out to determine the most common clinical type of dermatophytosis and the species that causes it. It was conducted at DVL OPD with collaboration with the microbiology department. A total number of 130 patients were enrolled who had no history of any treatment whether topical or systemic for the dermatophytosis. All the patients underwent Pottasium Hydroxide (KOH) mount test for fungal element by taking skin scrapings with aseptic precautions from the scaly skin lesions while for hair and nails, clippings were taken and send to central laboratory of microbiology department. The samples were simultaneously sent for culture in Sabouraud agar media. The reports of KOH mount and culture was thoroughly studied, analyzed and correlated with the clinical presentation of each patient. In this study, we found that Tinea corporis was the most common type followed by Tinea cruris and the most common causative organism being the Trichophyton mentagrophytes. This study also proved that KOH mount preparation is a good screening test for Dermatophytosis and hence we can abruptly start treatment based on clinical suspicion and the KOH mount findings which we can procure within few minutes.
