Prevalence of prolonged otitis media with effusion among 2 to 3 years old Cameroonian children in the era of 13-valent pneumococcal conjugate vaccines

ObjectivesThere is data scarcity on the overall effects of pneumococcal conjugate vaccines (PCVs) on otitis media (OM) in low- and middle-income countries. The impact of the 13-valent PCV (PCV13) program on OM was evaluated in Cameroon where infant vaccination was implemented in July 2011 using a 3-dose primary series at 6, 10 and 14 weeks of age. MethodsThrough community-based surveillance, we used a retrospective cohort study design to assess OM prevalence among PCV13-vaccinated children aged 24 to 36 months in 2015. This was compared with a 2013 age-matched cohort of PCV13-unvaccinated children. OM was diagnosed by clinical inspection for chronic suppurative OM (CSOM) and tympanometry for OM with effusion (OME). CSOM was defined as draining of the middle ear with duration of more than 2 weeks and prolonged OME was defined as a flat ‘type B’ tympanogram. PCV13-vaccinated and PCV13-unvaccinated cohorts were compared by calculating prevalence odds ratios for OM and baseline characteristics. ResultsAltogether, 111 OM cases were identified; 42/433 (9.7%) in the PCV13-unvaccinated in 2013 and 69/413 (16.7%) in the PCV13-vaccinated cohort in 2015. In the 2013 baseline survey, 3/433 (0.7%) children were identified with unilateral CSOM compared to 9/413 (2.2%) in the PCV13-vaccinated cohort in 2015. Bilateral prolonged OME was diagnosed in 7/433 (1.6%) PCV13-unvaccinated children and in 12/413 (2.9%) in PCV13-vaccinated children. Proportions of children with unilateral prolonged OME were 31/433 (7.2%) in the PCV13-unvaccinated group compared with 48/413 (11.6%) in the PCV13-vaccinated group. Multivariate logistic regression analysis showed evidence that PCV13-vaccinated children in 2015 had 40% less risk of contracting OM compared to PCV13-unvaccinated children in 2013 (adjusted prevalence odds ratios = 0.60 [95% confidence interval: 0.38 to 0.94], P = 0.025). Additionally, attributable proportion estimates show that, 58% of OM infections among the PCV13-vaccinated group would still have occurred despite PCV13 vaccination. ConclusionOur findings provide significant evidence on the effect of PCV13 in decreasing OM or OME among children in this age group. It also supports justification for government's continuation of PCV13 immunization program in the absence of GAVI's funding. Further research is needed to assess the long-term impact of the PCV13 program on in OM Cameroon.

Effect of Adenoid Hypertrophy on Otitis Media with Effusion (OME): A Study of 120 Pediatric Cases

The adenoids are lymphoid tissue, located behind the nose and soft palate; normally present in all children. They are susceptible to frequent infections of their nose and throat, resulting enlarged adenoids which cause obstructing nasal breathing. Since the adenoids are close to the Eustachian tube, their enlargement or infection may contribute to recurring diseases such as acute or chronic otitis media, Eustachian tube dysfunction, otitis media with effusion (OME) etc. OME is a very common disease; approximately 90% of children (80% of individual ears) have it at some point before reaching school age, often from 6 months to 4 years of age. Spontaneous resolution occurs usually within 3 months, but 30% to 40% of children develop recurrent OME, and 5% to 10% of the episodes last 1 year or longer [1,2].

ObjectiveTo summarize recent advances in knowledge on otitis media (OM) and quality of life (QoL) and development by synthesizing relevant research in this field published between June 1., 2015 until June 1., 2019. Data sourcesSystematic searches of PubMed, Embase and the Cochrane Library using predefined database-specific syntaxes. Review methodsArticles selected were randomized controlled trials and observational studies with an adequate control group estimating treatment effects of OM including acute OM (AOM), recurrent AOM (RAOM), OM with effusion (OME), chronic OM (COM) and chronic suppurative OM (CSOM). Items included were Health Status, Health Status Indicators, Quality of Life, Functional Status, Specific Learning Disorder, Developmental Disabilities, Language Development Disorders, and Problem Behavior. ResultsThe electronic database searches yielded a total of 699 records. After screening titles and abstracts, we identified 34 potentially eligible articles. Of these, 18 were excluded. This left 15 articles suitable for inclusion. ConclusionsAlthough evidence is accumulating that OM may significantly impair children's QoL and development as well as caregiver's QoL, studies on this topic are relatively scarce and vary substantially in terms of methodological quality and outcome measurement instruments (OMI) used. In this review, studies have used 10 different OMIs capturing a wide range of OM symptoms as well as generic and disease-specific QoL outcomes. OM was associated with negative effects on auditory processing, language and speech development, school readiness, social competence, psychosocial wellbeing, and sleep. We found only four relevant randomized controlled trials, which mostly failed to demonstrate superiority of interventions in terms of QoL improvement and reports on reversibility are lacking. This underpins the urgent need for high quality studies in this field using validated and uniform OMIs. To facilitate interpretation and harmonization of study findings, we suggest and support the development of a core outcome set for the various OM entities that should include the most reliable and meaningful QoL and developmental OMIs.

Relation between adenoid size and otitis media with effusion

AimsThis study compares the efficacy of adenoidectomy on otitis media with effusion (OME) in patients with different size of adenoids and the connection between differently sized adenoids and middle ear effusion. Material and methodsChildren with a history of at least 3 months’ OME underwent adenoidectomy and myringotomy without the insertion of a tympanostomy tube. Treatment assignment was stratified by adenoids’ size causing choanal obstruction (grade I-III) and according to Eustachian tube ostium obstruction (grade A–C). The subjects were followed for 12 months. ResultsAdenoidectomy was significantly more effective in children with adenoids in contact with torus tubarius (grade B, C) compared to those with small adenoids without contact (P<0.001). The volume of the adenoids was irrelevant (P=0.146). The size of adenoids did not affect the viscosity of the middle ear secretion. The distribution of mucous and serous secretion was not dependent on the size of adenoids; the efficacy of adenoidectomy was 82% in mucous as well as serous secretion. ConclusionThe relation between adenoids and torus tubarius is more important than the volume of the adenoids. The viscosity of middle ear fluids (serous or mucous) did not influence the rate of treatment efficacy.

This is an update of the 1994 clinical practice guideline “Otitis Media With Effusion in Young Children,” which was developed by the Agency for Healthcare Policy and Research (now the Agency for Healthcare Research and Quality). In contrast to the earlier guideline, which was limited to children aged 1 to 3 years with no craniofacial or neurologic abnormalities or sensory deficits, the updated guideline applies to children aged 2 months through 12 years with or without developmental disabilities or underlying conditions that predispose to OME and its sequelae. The Role of Fibrinolysis in Adhesive Otitis Media(AOM)

ObjectiveTo study the effects of Batroxobin on otitis media with effusion (OME), an early stage of Adhesive Otitis Media (AOM), for the purpose of expanding our understanding of the role of fibrinolysis in the pathogenesis of AOM. MethodForty cases of OME (45 ears) were randomly selected to receive intratympanic administration of Dexamethasone at 5 mg/ml (Group 1 or G1), Batroxobin at 1 BU/ml (Group 2 or G2) or Batroxobin at 2 BU/ml (Group 3 or G3). Pre– and post–treatment changes in clinical symptoms, the Air conduction Hearing Threshold (AHT) in pure tone audiometry and average AHT over 0.25 to 2kHz were compared. ResultsData from 31 cases (33 ears) were available for analysis. AHTs among three groups were similar prior to treatment (P> 0.05). The rate of normal hearing following treatment in G3 was 70% or 7/10, higher than in G1 (41.7% or 5/12) and G2(54.5% or 6/11) (P < 0.05). The rate of improvement following treatment was 81.8% or 9/11 and 80.0% or 8/10 in G2 and G3, respectively, higher than that in G1 (50.0% or 6/12) (P < 0.05). ConclusionsTherapeutic effects of intratympanic injection of Batroxobin on OME is superior to traditionally used Dexamethasone. In addition, higher concentration of Batroxobin appears to be superior to lower concentrations. These findings confirm that fibrinolysis plays a pivotal role in the pathogenesis of middle ear adhesion and that fibrinolytic medicine can prevent or reduce adhesion development in the middle ear.

Effect of pneumococcal vaccination on otitis media with effusion in children older than 1 year

Pneumococcal vaccines may be effective in preventing or decreasing the burden of disease related to otitis media. In study reported here, we investigated the effectiveness of pneumococcal vaccination on otitis media with effusion (OME) in children (n = 383 children aged 1-7 years) with a history of recurrent otitis media within the framework of a randomized double blind placebo controlled trial. Children were randomized to be immunized with either a 7-valent pneumococcal conjugate vaccine followed by the 23-valent pneumococcal polysaccharide vaccine, or by hepatitis A or B vaccines. The proportion of children diagnosed with OME at the scheduled follow-up visits--7, 14, 20 and 26 months after randomization--was then calculated. OME was diagnosed according to an algorithm combining tympanometry and otoscopy. The percentage of children diagnosed with OME was similar in the pneumococcal vaccination group and control group at both baseline and the follow-up visits at 7, 14, 20 and 26 months--52.9 versus 52.7, 44.9 versus 44.2, 34.9 versus 31.5, 40.8 versus 32.2 and 31.4 versus 26.1, respectively (corresponding to p-values of 0.96, 0.89, 0.51, 0.13 and 0.36, respectively). We conclude that the combined pneumococcal conjugate and the polysaccharide vaccination have no beneficial effect on OME in children aged 1 year or older with a history of recurrent otitis media. Therefore, these vaccinations are not indicated in the prevention of OME in these children.

Adenoid and otitis media with effusion—mini review

Although favorable effect of adenoidectomy on otitis media with effusion (OME) has been already established by several randomized clinical trials, the mechanism how OME is improved by adenoidectomy has not yet been fully clarified. In this paper, the mechanism was discussed from the viewpoints of Eustachian tube function (ETF) and sinusitis by reviewing our clinical studies carried out over 15 years. Comparison of ETFs between children with and without adenoid hypertrophy revealed no significant differences. Passive opening pressure (POP) of the Eustachian tube (ET) showed no significant improvement immediately after adenoidectomy compared to that before surgery. Comparison of ETFs between adenoidectomized and non-adenoidectomized children revealed significant improvement only in the active ventilatory function in adenoidectomized group 6 months after surgery. Improvement of sinusitis was observed more frequently in adenoidectomized children than in non-adenoidectomized children 6 months after surgery. ETF (active ventilatory function) was significantly better in those whose sinusitis was improved by adenoidectomy than in those whose sinusitis was not improved. These results indicated that OME might be improved after adenoidectomy as the inflammatory environment in the nasopharynx is improved owing to elimination of an infectious focus.

Evaluation of the Effect of Passive Smoking on Otitis Media in Children by an Objective Method: Urinary Cotinine Analysis

We aimed to determine objectively the effect of the passive smoking on otitis media with effusion (OME) and recurrent otitis media (ROM) by using the method of cotinine urinalysis. We designed a prospective case-control study with follow-up of the case group for 1 year after insertion of tympanostomy tubes to evaluate postoperative complications such as otorrhea and early extrusion (<6 months), in case a significant risk factor was found. One hundred fourteen children between 3 and 8 years of age requiring tympanostomy tubes because of OME and ROM were chosen and compared with 40 age-matched children. Exposure to environmental tobacco smoke was assessed by cotinine urinalysis, which was performed by means of the radioimmunoassay method. In this study, 73.7% (84 of 114) of the children in the case group and 55.0% (22 of 40) of the children in the control group were found to be "exposed" (P = .0461). This difference was statistically significant. Comparing the cotinine urinalysis results with parental smoking histories, 23.1% (9 of 39) of the children without parental smoking histories were "exposed" to tobacco smoke versus 84.3% (97 of 115) of the children with parental smoking histories (at least one person smoking). Our results indicate that sidestream smoking increases the risk of OME and ROM. Legal regulations and guidelines must be established to protect children from passive smoking. Because cotinine urinalysis is a noninvasive and reliable method for the determination of passive smoking, it can be used for that purpose.

Antibiotics for Otitis Media: A Clarification

To the Editor. —Although the review by Williams et al 1 provides additional evidence for a significant short-term impact of antibiotics on otitis media with effusion (OME), caution is necessary when generalizing their results. The brouhaha alluded to in the article's subtitle refers to the value of a therapeutic course of antimicrobials for OME; however, the authors included four trials of prophylactic antimicrobials 2-5 —once-daily administration of a subtherapeutic dose of medication—in their meta-analyses of short-term and long-term OME outcomes. This confounds meaningful interpretation of the results by underestimating the impact of therapeutic antibiotics on OME control. Another caution in generalizing the results concerns the OME meta-analysis of intermediate- to long-term outcomes. The hypothesis tested was whether a single course of therapeutic or prophylactic antibiotic conferred benefits lasting up to 11 months; OME relapse was untreated when it occurred in each of the individual studies. The etiology of middle ear disease in Nijmegen, The Netherlands was studied on the basis of a data set collected in a prospective epidemiological study on otitis media with effusion (OME) in a cohort of 1439 preschool children. A factor analysis was used to evaluate two hypotheses: (1) that OME, acute otitis media (AOM), common cold and tonsillitis are manifestations of the same pathological entity, and (2) that a group of children can be distinguished who develop these conditions more frequently than average. The results only partly supported these hypotheses. The correlation between OME, AOM, common cold and tonsillitis was lower than expected from a review of the literature. Common cold appeared to be the ubiquitous ENT disease in childhood and, depending on the child's predisposition, could be accompanied by OME, AOM or tonsillitis. The course of middle ear and upper airway disease showed a gradual scale from "healthy" to "ill" with most of the children suffering from these conditions at an average frequency.

Analysis and Presentation of Data on the Natural Course of otitis media with effusion in Children

Methods of analysing and presenting longitudinal data on health characteristics that have a fluctuating course are by no means standard. Several possibilities are discussed and tried out on longitudinal data on Otitis Media with Effusion (OME) in Dutch children. If the purpose of a study is descriptive, a graphical presentation based on an idea from Fiellau-Nikolajsen is preferred. For estimation purposes on the other hand, hazard functions are the most suitable. Further reduction of the data does not lead to a clear view into the complex nature of the course of a disease such as OME.

Effect of adenoidectomy on otitis media with effusion, tubal function, and sinusitis

Three clinical studies were performed to investigate the effects of adenoidectomy on otitis media with effusion (OME), especially with regard to eustachian tube (ET) disfunction and sinusitis, which often accompanies OME. In the first study, the audiograms, tympanograms, and ET ventilatory functions of 78 adenoidectomized patients (121 ears) and 54 non-adenoidectomized patients (63 ears) were compared over 6 months. The audiograms and tympanograms of the adenoidectomized group showed significant improvement; however, no difference in passive tubal opening pressure was noted despite an improvement in positive pressure equalizing function observed in the adenoidectomized group at 6 months after the operation. In the second study, tubal passive resistance (PR) and the ratio of passive resistance to active resistance (PR/AR) were compared before and 1 month after adenoidectomy using the forced response test (12 subjects, 12 ears). Neither PR nor PR/AR had significantly improved after the operation. In the third study, sinusitis improvement in 45 adenoidectomized patients 6 months after the operation was evaluated in comparison with 33 non-adenoidectomized patients. This condition was found to have improved significantly in the adenoidectomized group. Overall, adenoidectomy appeared effective in reducing the incidence of OME and sinusitis, and in improving the active ventilatory function of the ET without causing changes in the tubal passage. It is conceivable that tubal active ventilatory function was improved due to a reduction of inflammation and pollution around the nasopharynx by adenoidectomy, and that the effect of adenoid mass on the ET is minimal.

Transtympanic iontophoresis of dexamethasone and fosfomycin.

The effects of transtympanic iontophoresis of corticosteroids and antibiotics on otitis media with effusion (OME) were investigated. An experimental study using guinea pigs revealed that transtympanic passage of dexamethasone sodium phosphate and fosfomycin sodium by iontophoresis had sufficient anti-inflammatory and antimicrobial effects. Twenty children with OME (33 ears) who were treated by iontophoresis were compared with 28 untreated children with OME (35 ears) with respect to improvement assessed by tympanoaudiometry. After one month, tympanoaudiometry revealed improvement in 21 (63.6%) of the treated ears but in only 13 (37.1%) of the untreated ears, indicating the efficacy of this treatment for OME.

Clinical and Experimental Studies on the Pathogenesis of Otitis Media with Effusion

Otitis media is the most common pediatric disease as a complication of the upper respiratory infection and at least 10% of the otitis prone children will suffer from prolonged episodes of minor conductive hearing loss and communication disorder due to the presence of middle ear effusion (MEE). In spite of exhaustive studies presented in the past three international symposia on otitis media with effusion (OME), there still remain wide gaps in our knowledge regarding the etio-pathogenesis and appropriate treatments of the disease. A couple of years ago, a new project of clinical and laboratory researches on OME were initiated in the Department of Otolaryngology, Tohoku University School of Medicine and some results of the studies have been reported in recent Japanese academy meetings as well as in the 2nd and 3rd International Symposium of Recent Advances in OME. The purpose of this paper is to summarize the results of our recent studies performed by several investigators in our department and related hospitals.
