Relationship between concentrations of microbiological agents in the air of agricultural settings and occurrence of work-related symptoms in exposed persons.

For assessment of the dose-response relationship between concentrations of microbial agents in the air of various agricultural settings and occurrence of work-related symptoms in exposed workers, a meta-analysis of the results obtained in 1994-2007 on the territory of eastern Poland was performed. The studies on the airborne concentrations of total culturable microorganisms, mesophilic bacteria, Gram-negative bacteria, thermophilic actinomycetes, fungi, and bacterial endotoxins, as well as on the frequency of work-related respiratory and general symptoms in the exposed workers, were carried out at grain, thyme, valerian, flax, and hop handling on farms, in cow barns, piggeries, horse stables and in a modern hatchery. The airborne concentrations of the total microorganisms were in the range of 9.2-1236.5 × 10(3) CFU/m(3) , of the total mesophilic bacteria 3.5-1225.8 × 10(3) CFU/m(3) , of Gram-negative bacteria 0.0- 46.2 × 10(3) CFU/m(3) , of thermophilic actinomycetes 0.0-7.1 × 10(3) CFU/m(3) , of fungi 2.1-77.9 × 10(3) CFU/m(3) , and of bacterial endotoxin 0.00925-429.55 µg/m(3) . The frequency of work-related symptoms ranged between 21.7-63.8%. In a meta-analysis for assessment of the correlations between the log-transformed concentrations of airborne microbial agents and the occurrence of work-related symptoms, the multiple regression test was applied. Statistically significant correlations were found between the occurrence of work-related symptoms and the concentration of total airborne microorganisms (R=0.748555; P=0.020317), mesophilic bacteria (R=0.7573; P=0.029548), Gram-negative bacteria (R=0.835938; P=0.019129), and endotoxins (R=0.705356; P=0.03378). The correlations between the concentrations of thermophilic actinomycetes and fungi, on one side, and frequency of work-related symptoms on the other, did not attain the threshold of significance (P=0.087049 and P=0.062963, respectively). Results of the meta-analysis confirm harmful health effects of the total airborne microorganisms, total airborne mesophilic bacteria, airborne Gram-negative bacteria, and airborne bacterial endotoxin on the occupationally-exposed agricultural workers, and indicate a need for the establishment of internationally recognized occupational exposure limits for these microbial agents.

The effects of long-term occupational exposure to dust from herbs

Herbs are a heterogeneous group of many species with several thousand plants, which are used in large quantities in the pharmaceutical and food industries. The aim of the study was to analyse the health effects of long-term occupational exposure to dust from herbs. A group of 150 people occupationally exposed to dust from herbs, consisting of farmers and workers from herbs processing industry, was examined. As a reference group, 50 urban dwellers not exposed to any kind of organic dust were examined. Examined people were interviewed with the help of the ATS questionnaire compiled by Ferris and by the questionnaire developed in the Institute of Agricultural Medicine in Lublin, Poland for examination of work-related symptoms caused by organic dust. The lung function examination (vital capacity (VC), forced expiratory volume in the first second (FEV(1)), and FEV(1)/VC (%) of normal ranges) and allergological tests (skin prick test, precipitin test and inhibition of leukocyte migration (MIF) test) with microbial antigens were conducted. 71.3% (95% CI 64.1-78.6%) of the exposed people reported occurrence of work-related symptoms. A post-shift decrease of spirometric values (VC, VC%) was observed in the exposed group (mean decrease 2.6%, P<0.01). A significant relationship was found between the number of work-related symptoms and decrease of FEV(1) values, both before (Spearman correlation coefficient r=-0.21, P<0.05) and after work (r=-0.31, P<0.01). In allergological tests, the frequencies of positive reactions in the exposed group were significantly higher than in the reference group. Precipitins specific to Pantoea agglomerans were found in 30.6% (95% CI 23.2-38.1%) of exposed, compared to 12.0% among unexposed (P=0.01). The frequency of positive results in the migration inhibition test was significantly higher among exposed workers for all antigens tested. Long-term exposure to dust from herbs causes work-related symptoms and decrease of lung function parameters, which, finally, may lead to occupational disease.

Occupational Exposure Among Bakery Workers: Impact on the Occurrence of Work-Related Symptoms as Compared With Allergic Characteristics

The aim of the study is to examine the relationship between exposure to occupational allergens, sensitization, and atopy and several outcome variables and the independent associations of these interrelated risk factors to work-related symptoms in particular. Data were collected by questionnaire, skin prick tests, lung function tests, and exposure measurements among 246 workers from 74 bakeries and analyzed using logistic and linear regression analyses. Exposure levels were associated with respiratory symptoms, sensitization to baker's allergens, and lung function. High exposure and sensitization to common and bakers' allergens were independent risk factors for work-related symptoms. However, allergy determinants performed best in predicting work-related symptoms. Current exposure to occupational allergens is a risk factor for work-related symptoms, but sensitization to occupational allergens and atopy are more adequate and easier obtainable predictors of work-related symptoms.

Subjective and objective evaluation of an office environment

An office environment has been evaluated objectively by means of measurements of air quality and climate and subjectively by means of self-administered questionnaires. The main problem reported by the employees was dry air. However, relative air humidity was mainly in the range of 36–39% due to humidification. Air quality with respect to measured dust and vapours was good and the mean temperature in the office areas was 22.8 °C. The sensation of dry air and the occurrence of work-related symptoms were significantly influenced by sex and position level. When adjusted for these background factors there was a statistically significant relation between the sensation of high temperature and the sensation of dryness. The implications of the findings are discussed.

A search for discomfort-inducing factors in carbonless copying paper.

Connections between various types of carbonless copying paper and the occurrence of work-related symptoms of the skin and mucous membranes were studied. The analysis started from the exposure factor, i.e., from the carbonless copying papers and not from the exposed persons. In total, 276 paper samples were investigated. One hundred ninety samples were carbonless copying papers for which information was obtained concerning both the handling of the papers and the symptoms occurring among those handling the papers. To distinguish between carbonless copying papers of different makes, gas chromatography of paper extracts was performed, sometimes combined with thin-layer chromatography of the color formers. By coding the paper samples, the analyses of the papers and the interviews of the exposed persons could be performed "blindly." In the carbonless copying paper most frequently associated with complaints, mono-iso-propylbiphenyl (MIPB) was used as solvent for the color formers. It is probable, however, that the symptoms connected to this paper were induced by some component of the paper other than MIPB. A covariation between papers associated with skin symptoms and mucous membrane symptoms was observed. Exposure to carbonless copying papers treated with desensitizing ink (D-ink) covariated statistically with work-related skin irritation. The causative factor was probably two specific D-inks available on the Swedish market at the period of the investigation.
