Abstract Within the context of the COVID-19 pandemic, telehealth was seen to be a viable and alternative solution to provide occupational therapy services in the Philippines. This phenomenological study aimed to describe the lived experiences of Filipino parents and carers in receiving occupational therapy through telehealth for their children with disabilities. We interviewed 10 participants who were considered primary carers of a Filipino child with disabilities undergoing telehealth during the COVID-19 pandemic situation. An interpretative phenomenological analysis involving double hermeneutics was employed to analyze the interviews that yielded four themes: “dimensions of telehealth in occupational therapy”, “reinforcing family-centered occupational therapy”, “emphasizing an occupational therapist’s advocacy role”, and “telehealth in occupational therapy–today and tomorrow”. Our findings suggest that in order for telehealth to be a sustainable service, it should be seen not merely as an alternative in the occupational therapy service delivery process. Although telehealth remains to be an evolving concept and practice within health services, telehealth must be practiced within the principles of family-centered care approaches, interprofessional collaboration, and health accessibility and equitability. This study hopes to facilitate intersections between service providers and service users to cultivate a shared goal of bringing together experiences that will inform a more contextualized occupational therapy and telehealth practice at the tail-end of the pandemic. In conclusion, telehealth in occupational therapy shall not be an alternative, but an integrative tool that occupational therapists can maximize to transform occupational therapy access and equity.

Abstract Introduction The actions adopted with the purpose of avoiding community infections, during the COVID-19 pandemic, meant an occupational interruption since these measures prevented or restricted the participation in significant occupations of people. At the same time, the different health institutions and health professionals had to resort to telehealth to provide continuity of care. Objective To analyze from an occupational perspective the use of telehealth in Occupational Therapy in confinement contexts. Methodology Qualitative. Information is produced through focus groups and individual interviews. 9 students, 10 teachers, 5 users and/or family members and 5 members of the care teams participate in this study. An analysis of the thematic content is carried out through the triangulation of actors to integrate the opinions of all the participants, later in order to deepen the use of telehealth from an occupational perspective, theoretical triangulation is included. Results Telehealth as an occupation materializes the possibility of having socio-emotional support and allows collaboration in search of well-being in times of social isolation. It is also an occupation that allowed recognizing the home as a territory, in which the appropriation of material and relational resources occurs in a collaborative way between the treatment team and the users. Conclusions Telehealth is a collective occupation that allows re-existence in times of occupational interruption and new forms of occupational dislocation.

Telehealth interventions in occupational therapy with older adults: Results from a scoping review targeting better health promotion.

Telehealth interventions have the potential to enhance access to care and improve efficiency while reducing the burden on patients. Although telehealth interventions are well accepted and adopted in physical therapy, their usage in occupational therapy for older adults is less common, and limited information exists regarding their setting and context. To provide an inventory and synthesis of telehealth interventions in occupational therapy for older adults. For published studies on telehealth-based occupational therapy interventions in older adults between 2000 and 2022, six databases were reviewed. Data extraction and analysis were guided by the taxonomies developed by Tulu, McColl and Law and informed by the Canadian Model of Occupational Performance and Engagement. Twenty-three studies on telehealth interventions in occupational therapy for older adults were identified, mostly from North American authors (n = 11; 47.8%) and randomised clinical trials (n = 9; 39.1%). Most participants had a health problem (n = 20; 87.0%), mainly stroke (n = 9; 39.1%). Interventions focussed primarily on symptom management education (n = 12; 52.2%) of community-dwelling adults with health conditions, using videoconferencing systems or applications (n = 14; 60.7%). Interventions were delivered from the healthcare centre (n = 6; 26.1%) to the person's home (n = 18; 78.3%) synchronously (n = 19; 82.6%). About one third (n = 8; 34.8%) of the studies specified the therapist's location. Published studies on telehealth interventions in occupational therapy with older adults have mainly focussed on the synchronous training and education of participants using videoconferencing systems or applications. According to these studies, the scope of interventions is limited and could be expanded, for example, through occupational development and environmental modification. To better understand and describe best practices in the use of telehealth in occupational therapy, future studies should provide more details about the interventions performed, the technology used and the environmental settings of the therapist.

Does the Integration of Telehealth into Occupational Therapy Practice Impact Clinical Outcomes for Hand and Upper Limb Rehabilitation? A Matched Case Control Study.

Health services are capitalizing on the rise of telehealth and seeking to develop sustainable models incorporating telehealth into standard care. Further research is required to explore the service and clinical outcomes of telehealth in occupational therapy hand and upper limb practice. This research utilized a case-control study to explore the feasibility and clinical outcomes of case matched patients who received a telehealth hybrid model versus traditional in-person care. One hundred and two patients were recruited (n=51 in the controls and cases) with a mean age of 45 years. Telehealth was not inferior to standard care with no significant increase in therapy time (p=0.441) or length of referral (p=0.047). There was no difference in clinical adverse events (p=0.741). Patients who received telehealth had significantly less withdrawals from the service (p = 0.031). Patient and therapist satisfaction were high, supporting the ongoing use and continued implementation of telehealth in occupational therapy.

Toward successful future use of telehealth in occupational therapy practice: What the COVID-19 rapid shift revealed

BACKGROUND: Prior to the onset of the COVID-19 pandemic, occupational therapy (OT) services delivered through telehealth demonstrated comparative effectiveness to in-person services. At the onset of the pandemic, occupational therapy practitioners (OTPs) needed to continue delivering care to clients without being in-person. Many OT practitioners pivoted rapidly to telehealth, in many instances, with very little training. OBJECTIVE: The objective of this study was to describe the use of telehealth in occupational therapy during the early stages of the pandemic, and to explore how participants traversed the barriers. The perceived benefits and barriers to success with rapid telehealth adoption as experienced by OTPs, along with the specific strategies used to promote favorable outcomes may inform ongoing successful telehealth use in occupational therapy. METHOD: This study used a mixed-methods sequential explanatory design. Data was collected from September to December 2020 with an electronic survey. RESULTS: A total of 193 OTPs completed the survey, representing the delivery of OT services in 13 countries. Three main barriers to the use of telehealth were availability of materials, mastery of technology, and collaboration with caregivers/e-helpers. These barriers were negatively and significantly correlated with the participants’ confidence level in the use of telehealth. Participants overcame barriers by independently obtaining telehealth training, including training within one’s organization, support from social media, self-directed learning, and paid online telehealth webinars. CONCLUSION: Employers and educators can remove barriers to telehealth use by OTPs by providing a variety of learning opportunities and supports to enhance practitioners’ confidence, thus increasing the likelihood of continued use of telehealth as a powerful and gap-bridging delivery model in occupational therapy.

Stimulating Research to Advance Evidence-Based Applications of Telehealth in Occupational Therapy.

Occupational therapy focuses on therapeutic means to address participation in meaningful everyday tasks across the lifespan. No single setting is more conducive to this pursuit than individuals' authentic contexts. Occupational therapists are therefore uniquely suited to lead the charge toward stimulating research and advancing evidence-based application of telehealth. To this end, the American Occupational Therapy Foundation digitally convened their 2020 Planning Grant Collective to focus on the topic of Telehealth. Participants of the interdisciplinary collective collaboratively identified four themes: (1) Using technology to assess and intervene in the everyday context, (2) Partnerships with caregivers, (3) Telehealth delivery, and (4) Uniform data collection. Subgroups explored potential research and funding opportunities in their specialty area while also addressing the centralizing concepts of equity and diversity of telehealth delivery and COVID-19. Here, we provide a summary of the key concepts and recommendations from the 3 days of collaboration.

Was a global pandemic needed to adopt the use of telehealth in occupational therapy?

An analysis of data from an international survey was undertaken to determine the impact of the COVID-19 pandemic on telehealth practice in occupational therapy worldwide, in addition to facilitators and barriers in utilising this form of service delivery. The global online survey was circulated in the occupational therapy community by the World Federation of Occupational Therapists (WFOT) between April and July 2020, collecting responses to closed-ended questions, in addition to free-text comments. Descriptive statistics and bivariate analyses were used to assess relationships between respondent characteristics and the utilisation of telehealth. Thematic statement analysis provided further insight regarding factors impacting telehealth use. Findings revealed a significant increase in the use of telehealth strategies with the onset of the pandemic among survey respondents, with many reported benefits. Bivariate analyses indicated telehealth users were more likely to score higher feelings of safety and positive work morale, as well as perceive employer expectations to be reasonable. Restricted access to technology, limitations of remote practice, funding issues and slow pace of change were identified as barriers for some respondents to utilising telehealth. Facilitators included availability of supportive policy, guidelines and strategies, in addition to education and training. This study advances the understanding of the current scope of occupational therapy telehealth practice in the context of the ongoing COVID-19 pandemic. Although results suggest long-term potential for telehealth use as an adjunct to traditional service provision, important considerations were identified regarding factors influencing integration of such strategies.

University experiences of occupational therapy in Brazil during the Covid-19 pandemic: Contributions and support in mental health for the population

ABSTRACT Brazil stands out in the world for having a president who refers to the coronavirus pandemic (Covid-19) as a ‘little flu’. Social inequalities in the country are worsening and telehealth was only increased after the pandemic was installed. The aim of this article is to reflect and discuss telehealth in occupational therapy, presenting the techniques, themes and research in the initial phase on supporting groups for adults. Online social networks were used to invite and organise interested people. Virtual meetings are happening twice a week by video call and in addition we are also supporting chat through a WhatsApp group. People are reporting improvements in daily routines, personal relationships and anxiety. The most common themes are about everyday life, life history, difficulty in sleeping, eating, socialising with the same people 24 h a day, childhood trauma, politics and gender violence. We understand the important role of occupational therapy in analysis and intervention in people's life to reduce the risk of emotional distress, suffering and mental illness.

Persons With Dementia and Their Caregivers' Perceptions About Occupational Therapy and Telehealth: A Qualitative Descriptive Study.

Occupational therapy (OT) services provided in the home to a person with dementia can decrease caregiver burden by targeting home modifications, functional performance, and safety. One method to deliver these services is through telehealth. The use of telehealth is expanding rapidly, but there is a current lack of evidence to support the use of telehealth to deliver OT services to persons with dementia. The purpose of this study was to identify the perceptions of persons with dementia and their caregivers about the use of telehealth to receive OT services. A qualitative descriptive design was used. Results provided themes in three domains: dementia journey, skills for technology use, and perceptions of OT. Each participant identified a unique journey through awareness, challenges, and adaptive strategies as they adjusted to a life with dementia. The skills for technology focused on current usage and perceptions about technology. Participants had limited knowledge of OT but did appreciate the role it has in dementia care. The themes identified in the study provide support for further exploration of the use of telehealth in OT for persons with dementia and their caregivers.

The Process for the Formulation of the International Telehealth Position Statement for Occupational Therapy.

The World Federation of Occupational Therapists (WFOT) consists of 84 member organizations representing over 420,000 occupational therapists internationally (WFOT, 2014). In 2014, WFOT published the WFOT Telehealth Position Statement on the use of telehealth in occupational therapy. The process for the formulation of the official document involved reviewing WFOT member organizations’ telehealth position statements and data collected from a survey sent to member organizations’ delegates in April 2014. Qualitative data from 39 countries yielded factors to consider in five key areas: licensure/registration requirements, the cost of technology, privacy and security, reimbursement/payment models, and other issues (e.g., need for collaboration/transfer of knowledge, client selection, provider competencies, standard of care). The WFOT Telehealth Position Statement addressed each of these areas. The collaborative effort resulting in the development of the WFOT Telehealth Position Statement serves as a model for other international organizations.

Telehealth and ergonomics: A pilot study

As an emerging model of health care delivery, telehealth has the potential to provide individuals living in remote regions with access to health care services that are otherwise not available. This paper provides a review of the feasibility, comparative effectiveness, cost effectiveness, client and clinician satisfaction, and the barriers to providing telehealth services. A proof-of-concept study that examined the feasibility of telehealth-based ergonomic assessment and intervention is included in this paper to demonstrate the application of telehealth in occupational therapy and ergonomics. Ten computer users received a telehealth ergonomic assessment and intervention for their computer workstations via the Telerehabilitation Computer Ergonomics System (tele-CES) - a platform for computer users to access web-based assessments and to communicate with researchers via tele-conferencing. The tele-CES was used to assess participants' computer workstations and pain and comfort levels at baseline and at 4-month follow-up. Ergonomic recommendations were also provided via the tele-CES. Although there was no significant improvement in pain and comfort for participants, a high rate of compliance with ergonomic recommendations - 88% of all recommendations - was observed. Further research examining the comparative effectiveness of telehealth ergonomic assessment and intervention relative to in-person ergonomic assessment and intervention is warranted.
