Latent profile analysis of depression in US adults with obstructive sleep apnea hypopnea syndrome.

This study used latent profile analysis to explore the level of depression among US adults with obstructive sleep apnea hypopnea syndrome (OSAHS) symptoms and to identify different latent categories of depression to gain insight into the characteristic differences between these categories. The data of this study were obtained from the National Health and Nutrition Examination Survey (NHANES) database, and the subjects with OSAHS symptoms were aged 18 years and older. The latent profile analysis (LPA) method was used to fit the latent depression categories in subjects with OSAHS symptoms. The chi-square test, rank sum test, and binary logistic regression were used to analyze the influencing factors of depression subgroups in subjects with OSAHS symptoms. Three latent profiles were identified: low-level (83.7%), moderate-level (14.5%) and high-level (1.8%) depression. The scores of 9 items in the high-level depression group were higher than those in the other two groups. Among them, item 4 "feeling tired or lack of energy" had the highest score in all categories. Depression in subjects with OSAHS symptoms can be divided into low-level, moderate-level and high-level depression. There are significant differences among different levels of depression in gender, marital status, PIR, BMI, smoking, general health condition, sleep duration and OSAHS symptom severity.

An OSAHS evaluation method based on multi-features acoustic analysis of snoring sounds

Snoring is the most direct symptom of obstructive sleep apnea hypopnea syndrome (OSAHS) and implies a lot of information about OSAHS symptoms. This paper aimed to identify OSAHS patients by analyzing acoustic features derived from overnight snoring sounds. Mel-frequency cepstral coefficients, 800 Hz power ratio, spectral entropy and other 10 acoustic features were extracted from snores, and Top-6 features were selected from the extracted 10 acoustic features by a feature selection algorithm based on random forest, then 5 kinds of machine learning models were applied to validate the effectiveness of Top-6 features on identifying OSAHS patients. The results showed that when the classification performance and computing efficiency were taken into account, the combination of logistic regression model and Top-6 features performed best and could successfully distinguish OSAHS patients from simple snorers. The proposed method provides a higher accuracy for evaluating OSAHS with lower computational complexity. The method has great potential prospect for the development of a portable sleep snore monitoring device.

Quantifying Long QT Syndrome in Obese Sleep Apnea Patients Undergoing Roux-en-Y Gastric Bypass Surgery.

Obstructive Sleep Apnea/Hypopnea Syndrome (OSAHS) is sleep-disordered breathing distinguished by repetitive cessation or reduction of airflow due to the collapse or narrowing of the upper airway during sleep with continued respiratory effort. A high level of incidence of OSAHS is correlated with obesity. Both severely obese patients and OSAHS patients manifest Long QT Syndrome (LQTS). It is reported that most obese patients undergoing weight reduction surgery positively reverse symptoms of LQST. Also, severely obese OSAHS undergoing the same surgery report alleviation of OSAHS symptoms. In this study, we presented preliminary results of the changes in QT and QTc intervals for obese OSAHS patients undergoing Roux-en-Y Gastric Bypass (RYGB) surgery and had their weight reduced, and were treated from OSAHS post-RYGB surgery. We developed an algorithm to detect the different waveforms in the ECG signal and calculated QT and QTc intervals. Results comparing the changes in the QT and QTc pre- and post-RYGB surgery for four apneic subjects (Aged 37.0 ± 8.9 years, Pre-RYGB BMI 51.7 ± 10.1 kg/m2, Post-RYGB BMI 35.6 ± 7.9 kg/m2) were contrasted with a control group of 3 non-apneic subjects (Aged 32.7 ± 7.0 years, Pre-RYGB BMI 50.8 ± 11.8 kg/m2, Post-RYGB BMI 31.6 ± 2.9 kg/m2) who underwent the same surgery. The results suggest that although the RYGB surgery is successful in weight loss and OSAHS symptoms reduction, apneic patients may continue to have non-reversible LQTS despite long-term weight reduction.

THE DISTRIBUTIONS OF AIRFLOW WITHIN THE UPPER AIRWAY AND POSTURE CHANGES OF SOFT PALATE IN PATIENTS WITH OSAHS AFTER NASAL STRUCTURAL CORRECTION

Nasal obstruction frequently has been associated with obstructive sleep apnea-hypopnea syndrome (OSAHS). Based on the CT data before and after the operation of the three Chinese patients, 3D numerical models involving the upper airway and the soft palate are established. Computational modeling for inspiration and expiration was performed by using fluid–structure interaction (FSI) method. The airflow characteristics such as velocity and pressure drop, and displacement distribution of soft palate are selected for comparison. The distribution of airflow in the upper airway and the motion changes of the soft palate are analyzed quantitively before and after the surgery. From the results, that the inner connection between the both upper and lower part of the upper airway is very complex due to the individual difference of own unique feature of upper airway. Whether can it treat OSAHS only through nasal surgery, which depends primarily on the improvement of ventilation in upper part of upper airway giving a beneficial effect on the lower part (especially in the velopharyngeal region). The simulation results correspond with the monitoring results of polysomnography (PSG) and the patient's presenting complaint. Moreover, the mechanical parameters obtained by numerical simulation can support the improvement of OSAHS symptoms after surgery, which provides quantitative and referential evidence for further study the role of nasal structure in OSAHS and the treatment effect of nasal surgery on OSAHS.

Observation the clinical curative effect of children's laryngopharyngeal reflux and sleep apnea hypopnea syndrome

To observe the clinical curative effect of anti-reflux treatment for obstructive sleep apnea hypopnea syndrome (OSAHS) in children. Twenty children with sleep-disordered breathing were included in this study. There were 15 males and 5 females, aged 3-9 years old, median 6 years old. The electronic laryngoscope, polysomnography (PSG) monitoring, Reflux symptom index (RSI) questionnaire and Reflux finding score (RFS) were used to establish the initial diagnosis of OSAHS with LPRD, preclude adenoid hypertrophy and tonsil hypertrophy and nasal disorders. Oral Domperidone and Omeprazole were given for treatment. For children under 3-year-old, the dosage of Domperidone was 0.6 ml.kg⁻¹.day⁻¹.For children over 3-year-old, Domperidone combined with Omeprazole were given with the dosage of 0.3 mg.kg⁻¹.day⁻¹. After 4 weeks of treatment, 19 patients symptoms of OSAHS include disturbed sleep, dyspnoea and apneic attack improved. After 8 weeks of treatment, 20 cases with OSAHS symptoms improved than before treatment. Under the electronic laryngoscope, the decrease in pharyngeal lymphoid follicles, the epiglottis, aryepiglottic fold and scoop intergenic region erythema shallow, edema lessened. After treatment of 4 weeks and 8 weeks, there was statistically significant (P < 0.05). Before and after treatment, the difference of RFS was statistically significant (P < 0.05); PSG monitoring proved significant effect in 3 cases (15.0%), effective in 11 cases (55.0%) and 6 cases were ineffective (30.0%). Twenty patients with obstructive apnea index change was not obvious (P > 0.05), apnea hypopnea index and lowest artery oxygen saturation better, differences were statistically significant (Z of 2.819 and 2.733 respectively, P < 0.05). The treatment of LPRD can improve the symptoms of OSAHS, these two diseases may coexist in mutual relations.

Obstructive Sleep Apnea‐hypopnea Syndrome Patients with Overweight and Hypertension in a Japanese Workplace

The objective of this study was to determine the relationship between obstructive sleep apnea-hypopnea syndrome (OSAHS) and overweight combined with hypertension and to examine whether OSAHS in conjunction with overweight and hypertension is associated with daytime sleepiness. In a Japanese workplace of 28,636 employees, 368 men (19-62 yr old), who were anxious regarding their OSAHS symptoms, underwent home pulse oximetry. Of these, 153 men subsequently underwent all-night polysomnography (PSG), and OSAHS was diagnosed in 149. We next classified these 149 men into the following groups: A [Overweight (-)/Hypertension (-), n=41], B [Overweight (-)/Hypertension (+), n=15], C [Overweight (+)/Hypertension (-), n=46], and D [Overweight (+)/Hypertension (+), n=47]. The Epworth Sleepiness Scale (ESS) was used to evaluate daytime sleepiness and the apnea-hypopnea index (AHI) was used to evaluate the severity of OSAHS. The averages of the ESS score and the AHI were compared in each group. Both the average ESS scores and the percentage of ESS scores > or =11 were not significantly different among the groups. The average AHI of group D was the highest among all of the groups and that of group C was significantly higher than those of groups A and B. In all the groups, the OSAHS patients with overweight and hypertension in this study had the highest AHI. The level of daytime sleepiness evaluated by the ESS in this study was almost the same in the OSAHS patients regardless of the degree of overweight or hypertension. These observations suggest that it is necessary to positively recommend PSG to men who are suspected of having OSAHS with overweight and hypertension, even if they do not have daytime sleepiness.
