The Safety Implications of Information Technology in Nursing: Japanese Incident Data Analysis.

This study delves into the impact of Information Technology (IT) on nursing practice in Japan, focusing on patient safety within the 2021-2022 Japanese Medical Accident Report Data. The research aims to understand how IT factors contribute to nursing-related medical incidents in a healthcare landscape rapidly integrating IT. The study identifies IT-related incidents through a retrospective analysis of medical incident reports, primarily in nursing, by analyzing categorized data and free-text descriptions for IT-related keywords. The findings indicate significant IT-related issues, with 'Other EHR Related' problems (36%) and 'EHR Reporting' errors (25%) being the most prevalent. These incidents often involve challenges in patient identification and medication management. The study suggests improvements like enhanced verification processes and automated systems to mitigate these risks. Conclusively, it underscores the dual nature of IT in nursing: while it holds the potential to enhance patient care, it also introduces challenges that necessitate specialized informatics expertise to ensure its beneficial integration into nursing practices.

Analysis of the information ability of nurses in county hospitals in northern Henan province: a survey

ObjectiveTo investigate the status quo of nursing information ability of nurses in county-level hospitals and analyze its influencing factors.MethodsIn June 2022, a total of 303 on-the-job clinical nurses from 3 county-level hospitals in Hebi City, Henan Province were selected as subjects by convenience sampling method. General data questionnaire and self-rating nursing information ability scale were used to investigate them.ResultsThe total score of nursing information ability of 303 nurses in county hospitals of Hebi City, Henan Province was (77.72 ± 18.76). There were statistically significant differences in the scores of nursing information ability among different ages, working years, positions, education, marriage, monthly income, whether they had learned computer-related knowledge and skills, and whether they had participated in the learning or training of nursing information system (all p < 0.05).Multiple linear regression analysis showed that age, years, position, monthly income and whether they had learned computer-related knowledge and skills were the main influencing factors of nursing information ability of county-level nurses (all p < 0.05).ConclusionsThe nursing information ability of nurses in county-level hospitals in northern Henan is at a medium level. The government or society should provide training and guidance on nursing information ability, so as to provide more opportunities for nurses in county-level hospitals to participate in and learn nursing information technology to improve their ability.

We also handsearched: Computers, Informatics, Nursing (Computers in Nursing); Information Technology in Nursing; and the Journal of Nursing Administration.For this update, searches can be considered complete until the end of 2007. We checked reference lists of retrieved articles and other related reviews. Randomised controlled trials (RCTs), controlled before and after studies, and interrupted time series comparing one kind of nursing record system with another in hospital, community or primary care settings. The participants were qualified nurses, students or healthcare assistants working under the direction of a qualified nurse, and patients receiving care recorded or planned using nursing record systems. Two review authors (in two pairs) independently assessed trial quality and extracted data. We included nine trials (eight RCTs, one controlled before and after study) involving 1846 people. The studies that evaluated nursing record systems focusing on relatively discrete and focused problems, for example effective pain management in children, empowering pregnant women and parents, reducing loss of notes, reducing time spent on data entry of test results, reducing transcription errors, and reducing the number of pieces of paper in a record, all demonstrated some degree of success in achieving the desired results.Studies of nursing care planning systems and total nurse records demonstrated uncertain or equivocal results. We found some limited evidence of effects on practice attributable to changes in record systems. It is clear from the literature that it is possible to set up the randomised trials or other quasi-experimental designs needed to produce evidence for practice. Qualitative nursing research to explore the relationship between practice and information use could be used as a precursor to the design and testing of nursing information systems.

NURSING ETHICAL DILEMMA WITH USING INFORMATICS TECHNOLOGY

An ethical dilemma or ethical paradox is a decision-making problem between two possible moral imperatives, neither of which is unambiguously acceptable or preferable. The complexity arises out of the situational conflict in which obeying one would result in transgressing another. Sometimes called ethical paradoxes in moral philosophy, ethical dilemmas may be invoked to refute an ethical system or moral code, or to improve it so as to resolve the paradox. Nurses face ethical dilemmas on a daily basis, no matter where they practice. Nursing information technology and healthcare are going hand in hand these days. Technology in nursing practice is not new. The universal impact of technology in health care has created a new role for nurses. There are many ethical issues nurses can face in the workplace. These include quality versus quantity of life, pro-choice vs. pro-life, freedom versus control, truth versus cheating, resource allocation, and empirical knowledge versus personal beliefs. Information is a source of authority and, increasingly, the key to prosperity among those who have access to it. Thus, developments in information systems also involve social and political relations and therefore ethical considerations in how information is used are more important.

Applications of Information Technology in Nursing During 2005-15: Evidence from Iran.

In this ever-changing health care environment, nurses employ technologies and information systems to accomplish the intentions of the practice of nursing. Information technology supports the basic and advanced nursing practices in all settings. This review provides evidence about applications of information technology in Iranian nursing. We systematically searched all papers about applications of information technology in nursing in Iran that were indexed in SID, Magiran, Iran medex, PubMed and scopus databases. This study indicated that 12 (%52) studies used information technologies in the nursing education domain. Also, in 6 (%26) studies telenursing was used for patient care. 3 (13%) of the articles were related to the impact of the use of computer-based information system on nursing practice. In 2 (%9) papers the researchers developed computerized software for nursing processes. The results of this study indicate the use of information technology in nearly every aspect of nursing in Iran.

Description of Family Satisfaction toward Information Technology Based Family Nursing

Information technology based family nursing care is an important aspect to provide high quality nursing care. Family centered nursing care is the forefront of the community health enhancement. Nursing Information System (NIS) in community health center include development of technology based nursing documentation, and enhancement of communication between health team and family. The aim of this study was to evaluate family satisfaction toward information technology based family nursing care. Family satisfaction was measured by identify family opinion regarding the role of family nurses in providing qualified nursing care as well as their opinion regarding the needs of family nurses. Measurement was performed to 35 families who have received family nursing care based information technology as intervention group and 35 families who have not received as control group. The responses to overall satisfaction in intervention group were very in 14.29% and satisfied 80%. Result of the research showed family satisfaction who have received information technology based family nursing care was higher than control group (p=0.000). However, nursing information technology supported nursing care documentation, which affected the quality and continuity of family centered nursing care, and then affected the level family satisfaction.

The impact of personality on nurses' bias towards automated decision aid acceptance

In medical environments, it is imperative that patients' needs be assessed quickly and accurately to expedite patient care and flow through the system. One way to accelerate this process is to utilise automated decision aids (ADAs). To increase the rate of ADA acceptance, managers and information system (IS) developers must understand the elements that influence adoption. The purpose of this study is to evaluate factors that impact the adoption of a decision support system in a specific domain - nursing. This study investigates the role that personality plays in nursing information technology acceptance. We conducted an experiment with novice and experienced nurses to assess the role of personality on positive bias toward ADA utilisation. The results of the Myers-Briggs type indicator indicate that there is a significant difference in ADA bias for judging and perceiving personalities. Implications for research and practice are discussed.

Global Use of the Practice Environment Scale of the Nursing Work Index

Although the Practice Environment Scale of the Nursing Work Index has been endorsed as a gauge of the quality of the nursing practice environment by several organizations in the United States promoting healthcare quality, there is no literature describing its use in different practice settings and countries. The purpose of this study was to inform research by describing the modifications and use of the scale in a variety of practice settings and countries. The Cumulative Index to Nursing and Allied Health Literature and the PubMed databases were searched for the years 2002-2010 to identify 37 research reports published since 2002 describing use, modification, and scoring variations in different practice settings and countries. The scale was modified for 10 practice settings in five countries and translated into three languages. Composite scores ranged from 2.48 to 3.17 (on a 1-4 scale). The Staffing and Resource Adequacy subscale most often scored lowest. A new Nursing Information Technology subscale has been developed. New scoring methods to identify the favorability of practice environments are described. Over time, the nature of the research conducted using the measure has changed. Overall, most publications report significant associations between scale scores and multiple nurse, patient, and organizational outcomes. Scale use is growing across different clinical settings and countries. Recommendations for future research use include reducing scale length, using consistent scoring methods, considering the impact of various modifications on the basis of cultural and clinical setting nuances, and using the measure in longitudinal and intervention research designs.

Adding a nursing information technology subscale to the practice environment scale of the Nursing Work Index

In the past decade, the use of information technology (IT) to support healthcare delivery has greatly expanded. Introducing new clinical information systems and updating existing systems require continued learning and training among registered nurses (RNs) and other clinicians. Consequently, a 5-item measure was designed as a subscale to the Practice Environment Scale of the Nursing Work Index. This new subscale, the Nursing Information Technology Subscale (NITS) measures RNs' perception of the extent to which IT in their practice environment supports patient care delivery. A psychometric evaluation of the NITS was conducted at 8 Veterans Affairs hospitals. The findings demonstrated that this brief subscale has acceptable reliability as well as convergent and discriminant validity.

Curriculum Strategies to Improve Baccalaureate Nursing Information Technology Outcomes

Improving information technology (IT) outcomes is a top nursing education priority. Improving care access, quality, and cost effectiveness, IT skills are vital for professional development and advancement. Nursing programs have embraced distance learning and added informatics content, courses, and specific technologies; however, undergraduates' and educators' skills are still considered inadequate. Meanwhile, the Nursing Informatics specialty has moved beyond IT competency articulation and measurement. It is promoting information literacy to support evidence-based practice and the cultivation of clinical wisdom. The Technology Informatics Guiding Education Reform (TIGER) Initiative, a coalition aiming to advance IT outcomes in nursing education, has cited benchmarking and sharing best practices as key to achieving its goals. Thus, this article reports on the process, results, and implications of a project using curriculum strategies to enhance IT outcomes. A 3-year action plan directed faculty, student, and agency evaluation, curriculum mapping, model learning module, and documentation development.

Enhancing Baccalaureate Nursing Information Technology Outcomes: Faculty Perspectives

Baccalaureate nurses must be prepared to meet information technology expectations for practice and future professional development. Therefore, educational programs must evaluate curriculum and student outcomes and address areas for improvement. Nursing information technology knowledge, skills, and preparation of student nurses, nursing faculty, and clinicians: a U.S. survey.

Because health care delivery increasingly requires timely information for effective decision making, information technology must be integrated into nursing education curricula for all future nurse clinicians and educators. This article reports findings from an online survey of deans and directors of 266 baccalaureate and higher nursing programs in the United States. Approximately half of the programs reported requiring word processing and e-mail skill competency for students entering nursing undergraduate programs. Less than one third of the programs addressed the use of standardized languages or terminologies in nursing and telehealth applications of nursing. One third of the programs cited inclusion of evidence-based practice as part of graduate curricula. Program faculty, who were rated at the "novice" or "advanced beginner" level for teaching information technology content and using information technology tools, are teaching information literacy skills. The southeastern central and Pacific regions of the United States projected the greatest future need for information technology-prepared nurses. Implications for nurse educators and program directors are discussed.

Nursing record systems may be an effective way of influencing nurse practice. To assess the effects of nursing record systems on nursing practice and patient outcomes. We searched The Cochrane Library, the EPOC trial register (October 2002), MEDLINE, Cinahl, Sigle, and databases of the Royal College of Nursing, King's Fund, the NHS Centre for Reviews and Dissemination, and the Institute of Electrical Engineers up to August 1999; and OCLC First Search, Department of Health database, NHS Register of Computer Applications and the Health Visitors' Association database up to the end of 1995. We hand searched the Journal of Nursing Administration (1971-1999), Computers in Nursing (1984-1999), Information Technology in Nursing (1989-1999) and reference lists of articles. We also hand searched the major health informatics conference proceedings. We contacted experts in the field of nursing informatics, suppliers of nursing computer systems, and relevant Internet groups. To update the review the Medline, Cinahl, British Nursing Index, Aslib Index to Theses databases were all searched from 1998 to 2002. The Journal of Nursing Administration, Computers in Nursing, Information Technology in Nursing were all hand searched up to 2002. The searches of the other databases and grey literature included in the original review, were not updated (except for Health Care Computing Conference and Med Info) as the original searches produced little relevant material. Randomised trials, controlled before and after studies and interrupted time series comparing one kind of nursing record system with another, in hospital, community or primary care settings. The participants were qualified nurses, students or health care assistants working under the direction of a qualified nurse and patients receiving care recorded and/or planned using nursing record systems. Two reviewers independently assessed trial quality and extracted data. Eight trials involving 1497 people were included. In three studies of client held records, there were no overall positive or negative effects, although some administrative benefits through fewer missing notes were suggested. A paediatric pain management sheet study showed a positive effect on the children's pain intensity. A computerised nursing care planning study showed a negative effect on documented nursing care planning, although two other computerised nursing information studies showed an increase in recording but no change in patient outcomes. Care planning took longer with these computerised systems, but the numbers of patients and nurses included in these studies was small. A controlled before-and-after study of two paper nursing record systems showed improvement in meeting documentation standards. No evidence was found of effects on practice attributable to changes in record systems. Although there is a paucity of studies of sufficient methodological rigour to yield reliable results in this area, it is clear from the literature that it is possible to set up randomised trials or other quasi-experimental designs needed to produce evidence for practice. The research undertaken so far may have suffered both from methodological problems and faulty hypotheses.
