Application of enhanced recovery after surgery in perioperative nursing care of elderly patients with hip and knee arthroplasty: A systematic review and meta-analysis.

To collect data from randomized controlled trials (RCTs) to evaluate the effects of enhanced recovery after surgery on postoperative recovery of elderly patients who underwent hip or knee arthroplasty. The search was limited to studies published prior to January 1, 2023, in the electronic databases of Cochrane, Embase, Ovid Medline, Proquest, PubMed, Scopus, Web of Science, and Chinese databases, including China National Knowledge Internet (CNKI) and SinoMed. All relevant data were collected from the studies that met the inclusion criteria. The outcome variables were recovery of joint function and incidence of complications. STATA software (version 14.0) was used for the meta-analysis. A total of 44 published studies met the inclusion criteria. The cumulative data included 2203 cases receiving enhanced recovery after surgery (ERAS), and 2173 cases receiving traditional recovery after surgery (non-ERAS). The meta-analysis showed that the VAS score was significantly lower in the ERAS group than in the non-ERAS group (P < .01), and there were fewer incidences of complications in the ERAS group than in the control group (P < .01). ERAS significantly reduced pain and the incidence of complications in elderly patients who had undergone joint replacement surgery.

Advancing Sustainable Healthcare through Enhanced Therapeutic Communication with Elderly Patients in the Kingdom of Saudi Arabia

Effective communication in nursing, particularly with older patients, is critical to providing high-quality care. The purpose of this research is to fill key gaps in the existing literature by emphasizing the importance of therapeutic communication in the setting of mental nursing care for elderly patients in Saudi Arabia. Building on the study’s foundation, which recognizes the various issues faced by cultural, religious, and linguistic diversity, this research adopted a rigorous research methodology incorporating a broad group of senior healthcare professionals as experts. We analyze various therapeutic communication approaches used by mental health nurses using extensive surveys and observations. This empirical study’s findings are likely to make a significant addition to the field by throwing light on the most efficient methods for improving nurse–elderly-patient communication. The study identifies Simulation-Based Training as the most viable technique, with potentially far-reaching implications for improving care for older patients in Saudi Arabia. This study paves the way for significant advances in healthcare practices, with a focus on mental health nursing, ultimately helping both nurses and elderly patients by developing trust, understanding, and increased communication.

Application of three-dimensional quality assessment model of structure-process-outcome in continuous nursing of elderly patients with lung cancer after operation

Objective To explore the application of three-dimensional quality assessment model of structure-process-outcome in continued nursing care for elderly patients with lung cancer after operation. Methods By purposive sampling, 48 elderly patients discharged after radical resection of lung cancer from January to December 2016 were taken as control group and received routine continuous nursing care. Another 51 elderly patients discharged after radical resection of lung cancer from June 2017 to June 2018 were taken as the observation group. Continuous nursing program based on structure-process-outcome quality assessment model was applied for the observation group. The effect of intervention was assessed by using Strategies Used by People to Promote Health (SUPPH) and Quality of Life Questionnaire-30 (QLQ-30) . Results At 6 months after discharge, the scores of each dimension of SUPPH scale in the observation group were statistically higher than those in the control group (P<0.05) . In the observation group the scores of physical function, role function, social function and overall health of QLQ-30 scale were higher than the control group, the scores of fatigue, pain, insomnia, lack of appetite, constipation, and diarrhea were all lower than the control group with statistical significance (P<0.05) . Conclusions The continuous nursing care based on the three-dimensional quality assessment model of structure-process-outcome for elderly patients with lung cancer can help to improve their sense of self-efficacy and quality of life. Observation of the effect of overall nursing mode applied to the emergency nursing care of elderly patients with acute myocardial infarction

Objective To explore the effect of overall nursing mode on the emergency nursing care of elderly patients with acute myocardial infarction. Methods A total of 96 patients with acute myocardial infarction treated in our hospital from March 2014 to November 2015 were selected prospectively and were divided into 2 groups according to odd and even number, 48 cases in each group.After admission, the control group were treated with conventional emergency care, while the observation group received the overall nursing on the basis of the control group.The treatment effect, rescue effect and the patients' satisfaction with nursing service of the two groups were compared. Results The explicit efficiency and total effective rate of the observation group was 54.17% and 95.84% respectively, which was significantly higher than that of 31.25% and 77.08% of the control group of (P<0.05). The rescue effect, hospitalization time, bed time, emergency balloon expansion time, time out of the emergency room and costs of the observation group was significantly better than that of the control group (P<0.05), and AMI recurrence rate was significantly lower than that of the control group (P<0.05). The satisfaction rate and overall degree of satisfaction of the observation group was 50.00% and 97.92% , which was significantly higher than that of 27.08% and 82.25% of the control group (P<0.05). Conclusions To implement the overall nursing mode on emergency nursing care of elderly patients with acute myocardial infarction is conducive to reduce the hospital stay, reduce the recurrence rate and improve nursing satisfaction, and the effect is significant, it is worthy of clinical application and promotion. Application of comprehensive nursing care in elderly patients with hip replacement

Objective To explore the comprehensive nursing care in elderly patients with hip replacement. Methods A total of 68 elderly patients with hip replacement was randomly divided into experimental group (routine nursing and comprehensive nursing care) and control group (routine nursing). After intervention, we compared the satisfaction of patients and the incidence rate of complication such as the score of hip Harris score, infection, constipation and electrolyte disturbances. Results The scores of joint motion range, function, pain and deformity were (4.35±0.44), (40.91±2.30), (40.58±1.18) and (3.60±0.23) in the experimental group, compared with (3.88±0.18), (39.32±2.48), (39.00±1.37) and (3.41±0.36) in the control group (t=5.689, 2.379, 5.114, 2.512, respectively; P<0.05). While the incidence rate of infection, electrolyte disturbances, constipation and other other complication were 2.9%, 2.9%, 5.9%, 5.9% in the experimental group, these of the control group were 17.6%, 17.6%, 23.5%, 23.5% (χ2=3.981, 3.981, 4.221, 4.221, respectively; P<0.05). After intervention, the scores of fundamental nursing, specific nursing, attitude of service, health education and total satisfaction degree were (28.88±0.91), (29.06±0.81), (18.50±1.19), (18.65±1.04), (95.18±2.89), all that were higher than those in the control group (P<0.05). Conclusions The comprehensive nursing care for elderly patients with hip replacement can contribute to rehabilitative quality after operation, minimize complication incidence and promote elderly satisfaction. Influence of integrated nursing intervention on clinical effect for elderly patients with coronary heart disease and chronic kidney disease

Objective To evaluate the effect of integrated nursing on elderly patients with coronary heart disease (CHD) and chronic kidney disease (CKD).Methods The study was conducted among 86 hospitalized patients aged≥60 years with CHD and CKD.The patients were randomly divided into control group (n =42) that received conventional medications and routine nursing intervention,and observation group (n =44) that received integrated nursing interventions on the basis of the control group treatment.Patients of two groups were followed up for 1 year.Integrated nursing measures including emergency care,general care,psychological care,risk factors care were taken.Results After the intervention,hospitalized duration of the observation group was significantly lower than that of the control group [(13.5 ± 1.81) vs (17.3 ± 2.11) d; t =4.655,P ＜0.01].Indicators of integrated care intervention were significantly prior in the observation group than the control group:lower incidence of gastrointestinal symptom(15.9％ vs 38.1％),constipation,deterioration of cardiac function (6.8％ vs 31.0％)and renal function (11.4％ vs 47.6％),and lower incidence of rehospitalization (6.8％ vs 26.2％) and mortality (2.3％ vs 14.3％) ; while higher levels of albumin,hemoglobin,higher smoking cessation (70.5％ vs 35.7％),higher incidence of glucose(59.1％ vs 28.6％)and blood pressure control (61.4％ vs 35.7％),higher life quality score [(64.47 ± 20.59) vs (55.37 ±18.62)] and the differences were statistically significant(x2/t =5.399,8.265,13.699,5.917,4.147,10.424,8.116,5.658,2.430 ; P ＜ 0.05).Conclusions It plays an important role to take comprehensive nursing care in elderly patients with CHD and CKD,which can improve the nursing level,promote the rehabilitation,delay the progression of the disease,improve quality of life and reduce mortality. Long-Term Nursing Care of Elderly People: Identifying ethically problematic experiences among patients, relatives and nurses in Finland

The aim of this study was to explore ethically problematic situations in the long-term nursing care of elderly people. It was assumed that greater awareness of ethical problems in caring for elderly people helps to ensure ethically high standards of nursing care. To obtain a broad perspective on the current situation, the data for this study were collected among elderly patients, their relatives and nurses in one long-term care institution in Finland. The patients (n = 10) were interviewed, while the relatives, (n = 17) and nurses (n = 9) wrote an essay. Interpretation of the data was based on qualitative content analysis. Problematic experiences were divided into three categories concerning patients' psychological, physical and social integrity. In the case of psychological integrity, the problems were seen as being related to treatment, self-determination and obtaining information; for physical integrity, they were related to physical abuse and lack of individualized care; and for social integrity, to loneliness and social isolation. This study provided no information on the prevalence of ethical problems. However, it is clear from the results that patient integrity warrants more attention in the nursing care of elderly patients.
