Pediatric Primary Care Mental Health Specialist Examination: Job Task Analysis

Developmental, behavioral, and mental health (DBMH) conditions among pediatric populations have increased in prevalence in primary care. Approximately 1 in 5 children have mental health conditions, but only 20% receive care. In October 2021, a national emergency in children’s mental health was declared. The Pediatric Nursing Certification Board offers a pediatric primary care mental health specialist (PMHS) examination that validates the knowledge, skills, and abilities of certified nurse practitioners caring for children, adolescents, and young adults with DBMH conditions. This review describes the methodology, data analysis, and results of the job task analysis that ensures examination quality measuring preparedness to practice as a certified PMHS.

Evaluating the Practice of Pediatric Nurse Practitioners: Is It Time for Dual Primary and Acute Care Preparation?

This survey aimed to evaluate contemporary pediatric nurse practitioner (PNP) practice as it relates to the competencies of both the primary and acute care population focus and settings of practice to guide curriculum revisions. The design of the study was a cross-sectional survey of PNPs certified by the Pediatric Nursing Certification Board. There were 2,265 surveys completed. Regardless of the certification type, PNPs report providing care across settings and integrating the competencies of both the primary and acute care PNP into practice. This warrants further consideration by programs to prepare future PNPs for dual primary and acute care certification.

Development and implementation of a paediatric cardiac intensive care advanced practice provider curriculum.

Education of paediatric advanced practice providers takes a generalist approach which lacks in-depth exposure to subspecialties like paediatric cardiac intensive care. This translates into a knowledge gap related to congenital cardiac physiology and management for APPs transitioning to the paediatric cardiac ICU. A specialised interprofessional peer-reviewed curriculum was created and distributed through the Pediatric Cardiac Intensive Care Society. This curriculum includes a textbook which is complemented by a didactic and simulation review course. Course evaluations were collected following each course, and feedback from participants was incorporated into subsequent courses. Pediatric Cardiac Intensive Care Society partnered with the Pediatric Nursing Certification Board to develop a 200-question post-assessment (exam) bank. From December 2017 to January 2022, 12 review courses were taught at various host sites (n = 314 participants). Feedback revealed that courses improved preparedness for practice, contributed to advanced practice provider empowerment, and emphasised the importance of professional networking. 97% of attendees agreed/strongly agreed that the course improved clinical knowledge, 97% agreed/strongly agreed that the course improved ability to care for patients, and 88% agreed/strongly agreed that the course improved confidence to practice. 49% of participants rated the course as extremely effective, 42% very effective, 6% moderately effective, and 3% as only slightly effective. A standardised subspecialty curriculum dedicated to advanced practice provider practice in cardiac intensive care was needed to improve knowledge, advance practice, and empower APPs managing critically ill patients in the cardiac ICU. The developed curriculum provides standardised learning, increasing advanced practice provider knowledge acquisition, and confidence to practice.

Feasibility of nasal bridge pressure injury prevention using a protective dressing and the Halyard Fluidshield® N95 mask in a COVID-positive environment.

The purpose of this study was to prevent nasal bridge pressure injury among fit‐tested employees, secondary to long‐term wear of the N95 mask during working hours. A prospective, single‐blinded, experimental cohort design. Participants were enrolled using the convenience sampling methods and randomisation was utilised for group assignment. Eligibility was determined by a COVID Anxiety Scale score and non‐COVID clinical assignment. Participants with a history of previous skin injury or related condition were excluded. The experimental group was assigned Mepilex Lite® and the control group used Band‐ Aid®. Formal skin evaluations were done by Nurse Specialists who are certified in wound and ostomy care by the Wound, Ostomy, Continence, Nursing Certification Board (WOCNCB®). Fit test logs were provided to participants to measure subjective user feedback regarding mask fit and level of comfort. The results of this feasibility trial are promising in supporting the use of a thin polyurethane foam dressing as a safe and effective dressing to apply beneath the N95 mask. Additional research is needed to validate results due to limited data on efficacy and safety of the various barrier dressings as a potential intervention to prevent skin breakdown to the nasal bridge.

A White Paper: Perceived Value of Certification for Nurses and Nurse Managers

Since the first RNs earned orthopaedic certification in 1988, over 12,000 nurses have proudly held the ONC credential. Today, nearly 7000 orthopaedic-certified nurses, nurse practitioners, and clinical nurse specialists demonstrate excellence in the care of persons with musculoskeletal health concerns. Why did they choose certification? A representative sample of over 300 orthopaedic-certified nurses responded to a survey from the Orthopaedic Nurses Certification Board (ONCB) to confirm why they value their credentials and maintain specialty certification. Their answers offer compelling reasons for nurses to get, and keep, orthopaedic nursing certification.

Expanding a Foot Care Education Program for Nurses: A Quality Improvement Survey.

Faculty of a foot care continuing education program in the Midwest sought to determine current practice and interest in expanding the current program, both didactic and clinical, to meet expanded requirements in certification criteria. An electronic e-mail survey with both quantitative and qualitative questions was developed and sent to 445 past participants who completed the Foot and Nail Care: Education for Nurses Program hosted by the University of Wisconsin-Eau Claire Continuing Education over the past 5 years. There were 99 responses for a response rate of 22%. We asked several questions about clinical practice specific to the use of equipment, roles and responsibilities, and practice sites. Most participants used a mechanical rotary file for debriding toenails in their practices, with almost half having obtained a more advanced rotary file the longer they practiced. Most participants practiced in a variety of roles and settings including part of their paid employment, an independent business (full- or part-time), or volunteer. Less than half of respondents have become certified in the specialty. Those who have sought certification have chosen either the Wound Ostomy Continence Nursing Certification Board's Certified Foot Care Nurse or the American Foot Care Nurses Association's Certified Foot Care Specialist (CFCS). Sufficient interest (77%; n = 75) was expressed to expand the course to meet the requirements of certification, specifically for clinical hours. Based on the results of this survey, we concluded there is value in offering the program with several modules where participants could complete an introductory course and have the option to participate in continued education should they decide to learn more, seek certification, or need continuing education for recertification.

Keeping Up With Current Orthopaedic Nursing Practice: Results of the ONCB 2016 Role Delineation Study.

The Orthopaedic Nurses Certification Board (ONCB) conducts a role delineation study (RDS), also known as a job task analysis, every 5 years. Results identify tasks performed by orthopaedic registered nurses and nurse practitioners, as well as musculoskeletal health conditions commonly experienced by their patients. The purpose of this study was to define current practice patterns among orthopaedic nurses and nurse practitioners to determine content for future certification examinations. An online survey methodology was used to identify task and knowledge statements representative of orthopaedic nursing practice. Of 6,462 e-mails sent, 904 valid responses were returned (response rate 13.9%). This is lower than results of the Orthopaedic Nurses Certification Board's 2010 RDS (22.7% response rate) but is considered acceptable for an RDS. Survey results were analyzed by ONCB Test Committee members with guidance from psychometric staff at PSI/AMP. PSI/AMP staff were used to review and revise examination specifications for the Orthopaedic Nurse Certified (ONC) and Orthopaedic Nurse Practitioner-Certified (ONP-C) certification programs. New examination specification and passing points were implemented with April 2018 testing.

A Job Task Analysis of the Expanding Role of the Pediatric Mental Health Specialist and the Nurse Practitioner in Pediatric Mental Health

Lack of access to support, services, and providers for children and adolescents with mental health conditions is a serious problem in the United States. Fewer than half of the children who need services obtain them. To meet this demand, the Pediatric Nursing Certification Board launched a new specialty certification in 2011 for advanced practice registered nurses called the pediatric primary care mental health specialist (PMHS). In 2016, a job task analysis of the role of the PMHS and nurse practitioners practicing in pediatric mental health and developmental/behavioral pediatrics was conducted. Surveys were completed by 293 respondents. Demographic characteristics, tasks performed in practice, diagnoses made, assessment and screening tools used, and types of pharmacologic and nonpharmacologic interventions were obtained and are reported. The roles of the PMHS and nurse practitioner in pediatric mental health are well established and key to expanding services and access to children and their families.
