The Impact of Assessment of Nurses' Experiences in Thoracic Surgery in Onco-Hematological Patients.

Background: Nowadays, Thoracic Surgery is technologically advanced; therefore, it also focuses its attention on nursing care. The aim of the study is to evaluate the effect of the assessment of a dedicated team of nurses (DTN) in all onco-hematological patients undergoing VATS lobectomy for lung cancer on the outcome of the patient, preventing pressure injuries, reducing perioperative stress, duration of operations, complications, and hospital stay times. Methods: We performed a single-center observational retrospective study, including 31 DTN and 760 onco-hematological patients who underwent thoracic surgery between 30 October 2018 and 30 June 2023 at "Vanvitelli" University of Naples. Results: DTN ensures good nursing care before, during, and after surgery. Operative time was reduced by approximately 20 min, decreasing hospital infections in the DNT period and reducing intraoperative complications such as bleeding and hospital costs (p < 0.05). Conclusions: Thoracic surgery nurses require more specialized training to adapt to the development of sophisticated.

CARING OF INPATIENT NURSES WITH THE FAMILY'S READINESS TO RECEIVE MEDICAL INFORMATION ABOUT ADVANCED THERAPY

Background. Caring is the basis for nurses to be able to analyze nursing actions carried out well and appropriately for clients which becomes a reference in further care. In other words, caring is the moral ideal of the nursing profession which refers to the willingness to provide sincere care with seriousness in carrying out actions to care for clients' health. Research Method. A literature search in this research was carried out by identifying all types of national and international articles regarding caring for community nurses and the readiness of families to receive further therapy information The electronic databases used were PubMed, Springer, and Google Scholar with 30 articles then filtered into 3 articles with a search strategy using the PICO method (patient, intervention, comparison and outcome). Findings. Participants expressed that caring is an attitude of empathy, caring about patient needs, providing nursing care not only when patients come to the health center but also when nurses provide home care. Participants expressed caring behavior such as empathy, being friendly to patients who came to the treatment center, caring about patient needs, making home visits to patients, paying attention to the patient's health needs. Conclusion. Caring for inpatient nurses is an effort to improve the performance of nurses in providing nursing services to patients so as to create integrated and comprehensive care for patients. By increasing professional patient-nurse relationships, participants said they were helped by getting to know the many patients who came for treatment at the health center.

The Relationship Between The Accuracy of Filling Out The Early Warning Score System Form and Nurse Decision Making

&lt;p&gt;&lt;em&gt;Early Warning Score System plays an important role in monitoring and tracking patient deterioration through vital signs and physical condition. Nursing decision making is a complex process that has the potential to influence the quality of care provided. Clinical decision making requires critical thinking skills for nurses. This study aims to analyze the relationship between the accuracy of filling out the Early Warning Score System form and nurses' decision making. The type of research used is quantitative research using a retrospective descriptive research design with a cross sectional descriptive research design. The sampling technique is purposive sampling. The respondents of this study were nurses in integrated inpatient rooms A and B at the Haji Adam Malik Central General Hospital, Medan. This research was carried out in the integrated inpatient ward at the Haji Adam Malik Central General Hospital, Medan. The sample consisted of 148 respondents in the inpatient ward. The instruments used were the standardized Early Warning Score System monitoring form at Haji Adam Malik Hospital and nurse decision making adopted from the Ekwantoro questionnaire. The data were analyzed using the Spearman rank test to obtain a correlation coefficient of +0.470 with a significant p-value = 0.000. The conclusion is that there is a relationship between the accuracy of filling in the Early Warning Score System formula and nurse decision making which is quite strong and in the same direction. It is recommended that nurses increase their in-depth understanding of filling out the Early Warning Score System form in order to make appropriate decisions in providing nursing care.&lt;/em&gt;&lt;em&gt;&lt;/em&gt;&lt;/p&gt;&lt;p&gt; &lt;/p&gt;&lt;p&gt;&lt;strong&gt;&lt;em&gt;Keywords: Accuracy of filling, Decision making, ERWSS&lt;/em&gt;&lt;/strong&gt;&lt;/p&gt;

The Effect of Family-Centered Empowerment Program on Husbands' Understanding of Illness, Perceived Stress and Self-care Behaviors of Type 2 Diabetes Patients

Background: Comprehensive support from individuals with whom one has a deep emotional connection can significantly improve the management of life's stresses. Objectives: This study aims to investigate the impact of a family-oriented empowerment program for husbands on the illness understanding, perceived stress, and self-care behaviors of patients with type 2 diabetes. Methods: In this semi-experimental study, 72 spouses of diabetic patients who met the inclusion criteria were randomly selected and divided into intervention and control groups. The intervention group received an educational program based on the family-centered empowerment model, which consisted of six 60-minute training sessions over six consecutive weeks. The control group received usual care. Data were collected using questionnaires on self-care behaviors, disease understanding, and perceived stress, administered before, immediately after, and three months after the intervention. Data were analyzed using independent t-tests, paired t-tests, and repeated measures ANOVA. Results: There were no significant differences between the intervention and control groups in terms of demographic characteristics and baseline variables (P = 0.11). The Mann-Whitney test revealed significant differences between the two groups in terms of disease understanding, perceived stress, and self-care behaviors after the intervention (P &lt; 0.001). The independent t-test confirmed significant changes in the mean scores for disease perception, perceived stress, and self-care behaviors between the two groups (P &lt; 0.001). Conclusions: The results indicate that a family-centered empowerment program can enhance illness understanding, reduce perceived stress, and improve self-care behaviors among the spouses of diabetic patients. Given the increasing prevalence of diabetes, incorporating such programs into nursing care plans is recommended.

Exploring Job Satisfaction and Caring Behaviors among Critical Care Nurses: A Descriptive Analytic Study

Introduction: Caring behaviors are actions that prioritize the welfare of patients. Job satisfaction is one factor that can influence nurses' caring behaviors, as nurses with higher job satisfaction tend to exhibit more positive caring behaviors.Objective: The study aimed to assess critical care nurses' job satisfaction levels, explore the dimensions of their caring behaviors, investigate the relationship between their caring behaviors and job satisfaction, and identify the significant independent predictors of nurses' job satisfaction and caring behavior in critical care units.Method: The study used a descriptive-analytic, correlational, cross-sectional research design. It was conducted at the critical care units of King Fahd Military Medical Complex in Saudi Arabia. A convenience sample of 112 registered nurses completed an online survey that included the Job Satisfaction Survey and the Caring Behavior Inventory.Results: The majority of respondents fall under the category of Ambivalent job satisfaction, constituting 67.9% of the total nurses. The analysis indicated a moderate job satisfaction level and a high caring behavior among critical care nurses. The study found a positive correlation between nurses' job satisfaction and caring behaviors. Factors such as working hours, work unit, and years of experience were significantly associated with nurses' job satisfaction. However, factors like operational procedures, co-worker relationships, nature of work, and working hours were significant predictors of nurses' caring behavior.Conclusion: The study highlights the importance of addressing job satisfaction factors to enhance critical care nurses' caring behaviors, which can ultimately improve patient outcomes. Strategies to enhance nurses' job satisfaction and support their caring practices should be a priority for healthcare organizations

Hospital organisational health as a mediator between positive nursing organisational culture, caring behaviour, and quality of nursing care

Hospital organisations aim to restore patients’ health and improve their comfort. Furthermore, nurses are obligated to provide quality nursing care to patients. A hospital’s positive culture, interpersonal caring behaviour, and organisational health are important factors related to nursing quality. This study aimed to identify the factors that affected the quality of nursing care and verify the mediating effect of hospital organisational health on the relationship between positive nursing organisational culture, caring behaviour, and quality of nursing care. Data were collected from 190 clinical nurses who had worked for over 6 months in a tertiary hospital located in C city, South Korea. Descriptive statistics, independent t-test and one-way analysis of variance, Scheffe’s post-hoc test, Pearson’s correlation coefficient, hierarchical regression, and Baron and Kenny’s 3-stage procedures were calculated via SPSS version 25.0. Mediating effects were verified via bootstrapping. Hospital organisational health (β = 0.47, p < 0.001), ‘satisfied’ in departmental satisfaction (β = 0.26, p = 0.034), ‘surgical ward’ (β = 0.15, p = 0.017), ‘operating room/delivery room’ (β = 0.13, p = 0.023), ‘neonatal intensive care unit/nursery’ (β = 0.13, p = 0.039), and ‘outpatient department’ (β = 0.14, p = 0.018) in department of work were all factors that affected the quality of nursing care. Furthermore, these had significant effects, with an explanatory power of 57.2% (F = 10.70, p < 0.001). Hospital organisational health had a full mediating effect on the relationship between positive nursing organisational culture and quality of nursing care (B = 0.27, 95% BootCI = 0.18–0.37), as well as between caring behaviours among nurses and quality of nursing care (B = 0.21, 95% BootCI = 0.14–0.28). Practical measures to enhance nursing quality based on these findings should improve work and environmental conditions at the organisational level. In addition, measures to foster and manage a positive nursing organisational culture and promote various caring behaviour enhancement programs to improve hospital organisational health are recommended.

Presenteeism and missed nursing care: a descriptive, correlational and observational study

BackgroundMissed nursing care poses a significant challenge for healthcare staff in terms of patient safety and care quality.ObjectiveTo evaluate presenteeism and missed care attitudes of nurses and to determine the correlation between presenteeism and missed care.MethodsThis descriptive, correlational, and observational study was conducted between February and August 2023. The Stanford Presenteeism Scale-Short Form and the MISSCARE Survey were used to collect the data among nurses at two public hospitals in a city in Turkey. The study was completed with 229 nurses representing 27.4% of the total number of nurses who met the inclusion criteria. The data was analyzed using a comprehensive analytical approach, including Cronbach’s alpha analysis, frequency and percentage distribution, the Shapiro–Wilk test of normality, correlation coefficient analysis, Pearson correlation coefficient, and the Bonferroni test.ResultsThe participants’ mean age was 30.22 ± 7.14 years, and 74.2% of them were female. 53.3% of the participants reported difficulty providing patient care due to material shortages, and 62.9% experienced challenges delivering care due to the intensity of paperwork in the clinic. Nurses who felt that paperwork intensity affected patient care and were not confident in their care provision had higher levels of presenteeism (p = 0.041) and a significantly higher frequency of missed care instances (p < 0.001).ConclusionsMaterial shortages and high paperwork intensity are contributing factors to the difficulties experienced by nurses in their practice. These difficulties may lead to an increase in presenteeism and instances of missed nursing care. It is important to address these challenges to ensure adequate care provision and reduce the likelihood of presenteeism among nurses. The correlation between presenteeism and instances of missed nursing care highlights the impact of presenteeism behaviors on the quality of patient care.

Relieving effect of jiao-tune of 5-element music on negative emotions in patients with decompensated hepatitis B virus cirrhosis: A randomized and controlled experiment study.

Depression and anxiety are common in patients with decompensated hepatitis B virus (HBV) cirrhosis. This study aimed to evaluate the relieving effects of the jiao-tune of 5-element music on negative emotions in patients with decompensated HBV cirrhosis. The patients were randomly allocated into the control group (standard nursing care) and the jiao-tune group (standard nursing care plus a 2-month course of music therapy with the jiao-tune of 5-element music). The negative emotions of patients were assessed before intervention treatment and at the end of the 2-month follow-up using the Zung Self-Rating Depression Scale (SDS) and Zung Self-Rating Anxiety Scale (SAS). The analysis included 209 patients, with 102 in the control group and 107 in the jiao-tune group, all of whom returned their completed questionnaires. Baseline clinical characteristics and length of hospital stay were comparable between 2 groups. Before intervention treatment, there were no significant differences in SAS score (55.78 ± 5.64 vs 56.47 ± 3.28) and SDS score (65.13 ± 3.12 vs 64.48 ± 4.47) between the jiao-tune group and control group. After 2-month follow-up, the jiao-tune group had a significantly lower SAS score (53.17 ± 5.61) and SDS score (61.28 ± 1.52) compared with the control group (55.49 ± 3.37 and 63.08 ± 2.76), there were significant differences between 2 groups (P < .001). The jiao-tune of 5-element music can relieve the negative emotions in patients with decompensated HBV cirrhosis.

Asuhan Keperawatan pada Nn. M dengan Gangguan Sistem Endokrin: Post Op Isthmolobektomi dengan Indikasi Struma Nodusa Non Toksik di Ruang Mawar 2 RSUD dr. Soeselo Kabupaten Tegal

Non-toxic goiter nodusa is an enlargement of the thyroid gland in which one or more nodules are clinically detected without hyperthyroidism (Sari et al., 2021). The purpose of this writing is to find outand provide nursing care to Ms. M with endocrine system disorders: post op isthmolobectomy with indications of non-toxic nodal goiter in mawar ward 2 RSUD dr. Soeselo Tegal Regency in accordance with nursing standards. The method used is a descriptive method in the form of interviews, observation, documentation studies, library and literature studies. From the case review, it was found that the patient’s main comlaint was pain in the surgical wound on the left side to the neck, P:pain worsened when mobilizing, speaking and swallowing, Q: pain like being cut, R: pain on the left side of the neck, S: scale 4, T: disappears. There were three diagnoses found, namely acute pain, knowledge deficit and risk of infection. Intervetions are prepared based on the theory of SDKI, SLKI and SIKI as well as on the client’s condition and can be implemented.

Exploring nurse mentors' job crafting: A longitudinal study on missed nursing care across student supervision

AimTo compare the job crafting strategies nurse mentors use when mentoring students versus during periods of respite and to assess the effectiveness of these strategies in reducing instances of missed nursing care (MNC). BackgroundNurses who serve as mentors may have to train a group of nursing students on top of their routine nursing duties. The corresponding high workloads and limited resources may force them to decide which nursing care to delay or omit (i.e., MNC). The mentors’ initiative and the actions they take to address the multiple job requirements which do not align with the organization's resources are referred to as job crafting strategies. Mentors can use these strategies to shape their role as nurses as well as their role as mentors, depending on their motives and personality. DesignA longitudinal study with data collected at two time points. MethodsOne hundred nurse mentors completed validated questionnaires assessing job crafting strategies, MNC and work overload while actively mentoring students. Eighty returned for a follow-up during a non-mentoring period three months later. Data analysis included paired t-tests and hierarchical multivariable linear regressions. ResultsNo significant differences were found in MNC between the two points (1.83 SD 0.6 vs. 1.82 SD 0.75; p=0.942). A decrease in hindering demands was noted during active mentoring compared with respite (2.6 SD 0.97 vs. 2.84 SD 0.96; p=0.038). Enhancing structural job resources was significantly negatively correlated with MNC during active mentoring, while enhancing challenging job demands was positively correlated with MNC during these periods (β=0.48, p=0.18 and β=-0.35, p=0.014, respectively). ConclusionNurse mentors can effectively reduce MNC by focusing on enhancing structural resources and limiting challenging demands during mentoring periods. It is essential for healthcare organizations to support nurse mentors with manageable workloads and necessary resources to maintain high-quality care.

Beneficial effect of integrated nutritional interventions in patients with hematological diseases undergoing hematopoietic stem cell transplant.

The nutritional status of patients undergoing hematopoietic stem cell transplantation (HSCT) is critically important. This study was aimed to assess the impact of comprehensive nutritional interventions on the well-being of individuals with hematological diseases who underwent HSCT. A total of 175 patients with hematological diseases who underwent HSCT were included, with 94 in the control group and 81 in the research group. Patients in the control group received standard nursing care, while those in the research group underwent integrated nutritional interventions. Nutritional status was evaluated using the mini nutritional assessment (MNA) and subjective global assessment (SGA), along with body measurements and serum levels of albumin, prealbumin, and hemoglobin. There were no significant differences in the proportion of malnourished patients evaluated by MNA or SGA between the control and research groups at admission. However, at discharge and 3 months post-discharge, fewer patients in the research group were malnourished compared to the control group, as assessed by both MNA and SGA. At admission, there were no significant differences in albumin, prealbumin, hemoglobin levels, weight, calf circumference (CC), triceps skinfold thickness (TSF), or subscapular skinfold thickness (SSF) between groups. However, at discharge and 3 months post-discharge, the levels of these indicators significantly decreased compared to those upon admission. Levels of albumin, prealbumin, and hemoglobin, as well as weight, CC, TSF, and SSF, were significantly higher in the research group than in the control group at both discharge and 3 months post-discharge. HSCT led to a decline in nutritional status among patients with hematological diseases, but integrated nutritional interventions effectively improved their nutritional status.

Exploring determinants influencing the selection of geriatric nursing care services in Thailand Bangkok metropolitan area

The global population structure is undergoing a significant transformation, marked by an increasing number of elderly individuals. Thailand, in particular, is transitioning into an aging society and exhibits the third-highest growth rate in Asia, following Japan and Singapore. This phenomenon is driven by various factors, including economic and social conditions, technological advancements, family dynamics, and evolving lifestyles. Consequently, a growing number of elderly individuals are either living alone or require caregivers and elderly care facilities if they are dependent. This study is prompted by a keen interest in the elderly care service industry, which demonstrates significant potential both currently and in the future. The primary objective of this research is to explore the determinants influencing the selection of Geriatric nursing care and to identify the challenges encountered in delivering these services. The study investigated demographic and social factors, the necessity for elderly care, and the decision-making process in selecting geriatric nursing care. Data was collected through online questionnaires and analyzed, employing both descriptive and inferential statistics, including factor analysis, reliability analysis, and correlations between variables. The target population comprises Thai individuals aged 50 and above residing in Bangkok and its metropolitan areas with a sample size of 426 participants. Findings suggest that demographic factors, excluding occupation, along with social factors, the necessity for dependency, and the decision-making process, affect the selection of Geriatric nursing care in the studied regions.

Experiences and Fundamental Care Needs in Patients With Small Intestinal Neuroendocrine Tumours-An Interview Study in a Surgical Context.

The study aimed to describe patients' fundamental care needs and their experiences of nursing care, throughout surgical treatment of small intestinal neuroendocrine tumours. A qualitative descriptive study was performed. Patients' interviews (n = 19) were conducted in Sweden from May 2021 to January 2022 and analysed using directed qualitative content analysis guided by the Fundamentals of Care framework. The results are presented in three descriptive categories chronologically throughout the care chain. In the preoperative phase of care, the category was 'Feeling safe but lonely and frightened, and struggling with existential thoughts'; experiences in the postoperative phase of care resulted in the category 'Feeling cared for but suffering from physical symptoms and feelings of loneliness'; and the category in the discharge phase was 'Lacking self-care information and feeling worried about the future'. There were deficiencies in the delivery of fundamental care for patients with a rare tumour diagnosis throughout surgical treatment. Nursing care is mostly task focused and fragmented, and there is a lack of psychosocial and relational care across the care chain. Registered nurses and nursing managers need to take responsibility for their leadership in nursing care to fulfil patients' fundamental care needs. The Fundamentals of Care framework could be used for work improvements to include all aspects of nursing care. High-quality nursing care is needed throughout the care chain, including self-care after discharge, for patients with this rare tumour diagnosis. A higher awareness of patients' experiences and the importance of psychosocial support is warranted. Registered nurses and nursing managers must revise and improve routines to support patients' psychosocial needs. Registered nurses need to take responsibility for their leadership in nursing care to fulfil patients' fundamental care needs. What problem did the study address? This study highlights patients' fundamental care needs and experiences of nursing care throughout surgical treatment of small intestinal neuroendocrine tumours. What were the main findings? There are deficiencies in fulfilling patients' fundamental care needs across the care chain and in all dimensions of the Fundamental of Care framework throughout surgical treatment of small intestinal neuroendocrine tumours. Patients struggled with loneliness and existential thoughts, as well as worries about the future. Patients experienced a lack of information about plans for the day, self-care, and follow-ups. Where and on whom will the research have an impact? For clinicians to develop an understanding of, and improve, fundamental care needs for patients with small intestinal neuroendocrine tumours in a surgical context. For registered nurses to understand the importance of their leadership and nursing responsibility to fulfil fundamental care needs. The consolidated criteria for reporting qualitative research (COREQ): a 32-item checklist for interviews and focus groups. The patients shared their experiences during the interviews, which has contributed to a deeper knowledge and understanding of the phenomena under study.

Perception of the quality of nursing care and user satisfaction in hospitals in Chile

Introduction: the quality of health depends on whether the service offers the desired results. This is related to health services since users base their preferences and experiences to express satisfaction. Nursing is a central being in the perception of quality. Objective: Describe the perception of the quality of nursing care and user satisfaction in hospitals in Chile. Methodology: a study was carried out with a descriptive-cross-sectional, quantitative design. The sample was made up of 111 users over 18 years of age who had been hospitalized for at least one night in a public hospital in Chile. The instrument used was SERVQHOS-E, which measures the perception of users in relation to the quality of nursing care from corporate aspects and a survey that provides opinion data on perceived quality. Results: an average of 3.28±0.81 could be observed in the users' expectations regarding the quality of nursing care and 75.67 % of the users stated that they were very satisfied or satisfied with the nursing care received. Conclusions: the research revealed positive perception of users about the quality of nursing care in Chilean hospitals. Areas for improvement were identified such as communication and waiting times. The data obtained can collaborate in decision-making and improvements in user care in health facilities, guiding possible strategies to guarantee quality and satisfaction

Differentiating Between Home Care Types to Identify Older Adults at Risk of Adverse Health Outcomes in the Community

ObjectivesAcute hospitalization, recurrent admissions, institutionalization, and death are important adverse health outcomes. Older adults receiving home care are especially at risk of these outcomes, yet it remains unclear if this risk differs between older adults receiving different types of home care and older adults not receiving home care. DesignRetrospective cohort study using national claims data from 2019. Setting and ParticipantsCommunity-dwelling Dutch individuals aged ≥ 65 years (N = 3,174,953). MethodsParticipants were categorized: no home care, household help, personal care, household help combined with personal care, or nursing home care at home. The primary outcomes were the number of people experiencing acute hospitalization, recurrent admissions, institutionalization, or death. Logistic regression models were applied. ResultsIn total, 2,758,093 adults were included in the no home care group, 131,260 in the household help group, 154,462 in the personal care group, 96,526 in the household help combined with personal care group, and 34,612 in the nursing home care at home group. The risk of adverse outcomes differed between home care groups, with all showing higher odds compared with the no home care group. Individuals receiving household help combined with personal care had the highest odds for acute hospitalization [odds ratio (OR), 2.60; 95% CI, 2.55–2.64] and recurrent admissions (OR, 2.60; 95% CI, 2.55–2.65), while those receiving nursing home care at home had the highest odds for death (OR, 7.59; 95% CI, 7.35–7.85) and institutionalization (OR, 63.22; 95% CI, 60.94–65.58). Conclusions and ImplicationsDifferentiating between the type of home care older adults receive identifies subpopulations with different risks for adverse health outcomes compared with older adults not receiving home care. Older adults receiving personal care (nurse based) are at high risk for these outcomes and represent a substantial population with prevention potential. Future research should focus on developing effective interventions for this group.

Bottleneck factors impacting nurses’ workflow and the opportunity to prioritize improvement efforts: factor analysis

PurposeMinimizing delays in delivering nursing care is paramount for enhancing the overall quality of care. Certain bottleneck variables restrict the workflow of nurses, resulting in extended shift times. This study is designed to pinpoint and analyze the principal factors contributing to bottleneck issues in nursing workflow, to direct improvement endeavors. This study seeks to provide insights into the key variables contributing to nurses’ extended shift times, with the ultimate goal of prioritizing efforts for improvement.MethodsA descriptive multicenter cross-sectional study was conducted. A scale was developed for this study by the authors after conducting a literature review, subsequently validated, and its reliability was assessed.ResultsAmong the 31 bottleneck variables, 29 were retained under three persistent bottleneck factors: (1) Nurse staffing— This pertains to the availability of sufficient nursing staff at all times across the continuum of care; (2) Working environment and quality of care—This refers to the availability of necessary skills and resources for nurses to perform their duties effectively and; (3) Medical devices— This factor concerns the availability of fully functional medical devices required for providing care.ConclusionEfforts aimed at enhancing the overall healthcare system should concentrate on addressing persistent bottleneck factors. This may involve the implementation of a healthcare workforce management system, the establishment of standards for a conducive and supportive working environment, and the utilization of a standardized system for the management of medical equipment. The outcomes of this study can be utilized by nurses and policymakers to devise comprehensive strategies for improvement.

Inguinal hernia is a condition where an organ protrudes into a cavity through a thin or weak part of the wall. The small intestine is usually the organ that enters the cavity, causing the protrusion. In 2018, hernia ranked 8th in Indonesia with a total of 18,145 cases or around 1.05%. The number of hernia cases in Central Java is estimated to be over 500, or approximately 1.01% of cases. If an inguinal hernia is not treated promptly, it will continue to grow and exert significant pressure on the surrounding tissues. A common consequence of untreated hernia is that it will enlarge and interfere with the patient's daily activities. One of the complications of hernia is swelling around the lump. One of the treatments for hernia is surgery. The writer is expected to provide optimal nursing care for patient. Association of personal and professional factors, resilience and quality of life of registered nurses in a university medical city in the Kingdom of Saudi Arabia.

This study assessed the resilience of nurses in Saudi Arabia during the corona virus 2019 (COVID-19) pandemic and examined its influence on their quality of life (QOL). A sample of 356 nurses was surveyed in this quantitative, cross-sectional study using the Resilience Scale for Nurses and the World Health Organization Quality of Life (WHOQOL-BREF) from October 2020 to March 2021. The nurses reported the highest resilience score on "situational pattern", while the lowest score was on "relational pattern." The nurses had good perceptions on their overall QOL and health and rated their "social relationship" as having the highest quality, while their "environmental" domain as having the least quality. Gender, marital status, provision of direct nursing care to COVID-19 patients, "philosophical pattern", "situational pattern" and "dispositional pattern" had multivariate impacts on the QOL dimensions. The study concluded that being resilient can positively impact the nurses' QOL during stressful situations, such as the COVID-19 pandemic.

Designing and psychometric evaluation of safe nursing care instrument in intensive care units

BackgroundProviding safe care in a sensitive and high-risk unit such as the ICU is one of the most crucial tasks for nurses. One way to establish the criteria for safe care is by creating a instrument to assess it. Therefore, this study was conducted with the aim of designing and psychometrically evaluating an instrument for safe nursing care in the ICU.MethodsThe current study employed a sequential-exploratory mixed-method approach with two qualitative and quantitative phases. Based on the results of qualitative phase and the literature review, the primary instrument was designed. In the quantitative phase, the designed instrument underwent psychometric evaluation. Face, content and construct validity were assessed. Face validity was assessed by 20 nurses, and content validity was assessed by 26 experts. In the construct validity stage, the sample size for the exploratory factor analysis (EFA) included 300 nurses, and for the confirmatory factor analysis (CFA) included 200 nurses who work full-time in the ICUs of hospitals affiliated with Kermanshah University of Medical Sciences in western Iran. EFA sampling was conducted in three hospitals, encompassing six ICUs, while CFA sampling was carried out in two hospitals, covering four ICUs. Sampling was done using the convenience method. The reliability of the instrument was also assessed. Finally, the interpretability, feasibility, weighting, and scoring of the instrument were evaluated.ResultsThe qualitative phase identified three themes, including professional behavior (with categories: Implementation of policies, organizing communication, professional ethics), holistic care (with categories: systematic care, comprehensive care of all systems), and safety-oriented organization (with categories: human resource management and safe environment). The primary instrument was designed with 107 items rated on a five-point Likert scale. In the quantitative phase, the psychometrics of the instrument were conducted. First, the face and content validity were assessed, and the average scale content validity index (S-CVI) was 0.94. Then, a preliminary test was conducted to assess the initial reliability (α = 0.92) and the correlation of each item with the total score. After completing these steps, the number of items in the instrument was reduced to 52. The results of the EFA explained 58% of the total variance, with 4 factors identified: professional behavior by following guidelines, comprehensive care, accurate documentation, and pressure ulcer care. At the CFA stage, the results of the calculation of indices and goodness of fit showed that the model had a good fit. The reliability of the relative stability by examining the intraclass correlation coefficient (ICC) for the whole instrument in 20 samples was 0.92 with a confidence interval of 0.97 − 0.81. To measure absolute stability and determine the responsiveness of the instrument, the standard error of measurement (SEM) was 4.39 and the minimum detectable change (MDC) was 12.13.ConclusionThe instrument for safe nursing care in the ICU has favorable psychometric properties.

Nursing intervention in acute or critically ill patients with thirst: an integrated review

Aim: Analyze the existing nursing interventions and their effectiveness in managing thirst and promoting the person’s comfort. Background: Thirst is a symptom often experienced by people hospitalized in an acute and/or critical situation, but it is still frequently under-evaluated and under-treated in clinical practice, one of the main reasons being the difficulty in assessing thirst when it comes to people with an altered state of consciousness. Methods: To analyze the existing nursing interventions in managing thirst and promoting the person’s comfort, an integrative review was elaborated through electronic databases MEDLINE, CINAHL, PubMed, and other sources from March 2023 to August 2023. The review protocol is registered (PROSPERO CRD42023413584) and this review integrates a Project in a Higher School of Nursing. Results: After assessing eligibility, 16 final documents were obtained for data extraction and analysis. The results obtained show that several effective strategies were identified to thirst relief, such as drinking cold liquids, ingestion of carbohydrate drinks when fasting is really necessary, the use of ice, performing oral hygiene, applying lip moisturizer, using mentholated substances, controlling the ambient temperature, use of salivary stimulants and artificial saliva. Conclusions: This is a sensitive area of nursing care. Thus, a more significant investment in the scope of nursing research and the development of innovative technology that can improve nursing interventions for the management and thirst relief, would be beneficial to improve the quality of the nursing care provided for hospitalized patients in critical care.
