Outcomes of iStent inject combined with cataract surgery in Asian eyes: Australian data from the Fight Glaucoma Blindness international registry.

To analyse real-world outcomes in Asian eyes of iStent inject, a second-generation trabecular micro-bypass stent, combined with phacoemulsification. This is a multi-centre, observational study of glaucomatous Asian eyes that have undergone iStent inject implantation combined with cataract surgery. Patient data were extracted from the Fight Glaucoma Blindness! Registry. Outcome measures included those of IOP reduction, glaucoma medication reduction, and adverse events including the need for secondary surgery. 123 eyes of 86 patients with a mean age of 68.4 ± 9.3years underwent iStent inject implantation with phacoemulsification. At baseline, the mean ± SD preoperative intraocular pressure (IOP) was 16.0 ± 4.4mmHg, and the mean preoperative number of topical glaucoma medications was 1.9 ± 1.4. At 12months 30.8% of eyes demonstrated a reduction in IOP greater than 20%, the mean IOP reduction was 12.5% with an additional reduction of 0.7 glaucoma medications. 40% of eyes were using no medications at 12months compared to 16.3% preoperatively. 8.2% of eyes required a subsequent procedure within the 12-month follow-up window. iStent inject implantation combined with phacoemulsification in Asian eyes showed a reduction of IOP and glaucoma medication use in a real-world clinical setting. The safety profile of the device is good with minimal adverse outcomes, however, a subset of patients required secondary procedures within the 12 month follow up.

Intraocular pressure profile following selective laser trabeculoplasty in pigmentary and primary open-angle glaucoma.

To compare the intraocular pressure (IOP) profile and the incidence of IOP spikes following selective laser trabeculoplasty (SLT) between pigmentary glaucoma (PG) and primary open-angle glaucoma (POAG). Retrospective comparative study of 65 PG eyes of 51 patients matched with 65 POAG eyes of 65 patients who received SLT. Matching was done based on age, gender, glaucoma severity, pre-laser IOP, and number of medications. Post-SLT IOP spike was defined as IOP elevation ≥5mmHg, 30-45 min after the laser. In PG and POAG groups, the average age was 62.33 ± 9.18 and 62.58 ± 9.19 years (p = 0.95). The glaucoma severity (p = 0.708), baseline IOP (PG = 21.61 ± 1.34mmHg vs. POAG = 21.13 ± 5.09mmHg, p = 0.943), and number of topical glaucoma medications(PG = 2.34 ± 1.34 vs. POAG = 2.1 ± 1.41, p = 0.342) were comparable. More PG patients were on oral acetazolamide (PG = 26.15% vs. POAG = 1.5%, p < 0.001). Average logMAR visual acuity was significantly higher in the POAG group (0.207 ± 0.3 vs. 0.192 ± 0.37, p = 0.012). Eyes with PG received lower laser energy (POAG = 63.65 ± 22.03 mJ vs. PG = 43.71 ± 25.68 mJ, p < 0.001). IOP spikes were recorded in 5 PG eyes (7.6%) and none in the POAG group (p = 0.058). Failure rates were similar (PG = 50.7% and POAG = 43.1%, p = 0.205). In multivariable analysis, only pre-laser IOP (coefficient = 2.154 [CI: 0.765-3.543], p = 0.003) was a significant predictor of IOP change percentage after 12 months. SLT was comparably effective in both PG and POAG. IOP spikes were observed only in the PG group, though the total laser energy was lower in this group compared with POAG.

Early and Late Complications of Ahmed Glaucoma Valve Implant in a Real-World Situation

Purpose: To determine the early and late complications of Ahmed Glaucoma Valve (AGV) implant surgery. Study Design: Interventional case series. Place and Duration of Study: Ophthalmology department of MTI-Hayatabad Medical Complex Peshawar, from May 2018 to February 2023. Methods: All the patients between May 2018 and February 2023 who underwent AGV implant were included in the study. Data included patient's age, gender, indications for surgery, type of AGV implant used, length of follow-up period, intraocular pressure (IOP), number of topical glaucoma medications used, early and late complications, and the final outcome. Continuous variables (duration of follow-up, best corrected visual acuity, IOP, and number of topical anti-glaucoma medications) were represented in means and standard deviations. Paired t-test was applied for comparison of means between baseline and last follow-up data. A p-value of &lt; 0.05 was considered statistically significant. Results: A total of 28 eyes were included. Mean age of the patients were 28.75 ± 20.5 years. Duration of follow-up was 213.96 ± 169.33 days. There were 53.85% females. Commonest indication was congenital glaucoma 7 (25%). Majority, (n = 18, 64.3%) of the eyes had previous cataract surgeries followed by glaucoma, 13 (46.4%). The commonest early post-operative complication was hyphema (25%) and commonest late complication was hypertensive episode 32.1%. Majority of the eyes 23 (82.1%) did not require any further intervention for these complications. Conclusion: Majority of AGV complications are self-limiting, necessitating just conservative therapy. However, serious complications might happen that could lead to blindness, therefore the surgeons should have strategies for treating or preventing them.

Outcomes of 60-Degree Nasal Goniotomy for Open Angle Glaucoma.

Sixty-degree nasal sectoral juxtacanalicular space goniotomy with the single-use Espaillat juxtacanalicular space slider device significantly reduced intraocular pressure and the number of required glaucoma hypotensive medications, with sustained reductions over 24 months. This study analyzed the safety and efficacy of the Espaillat juxtacanalicular space slider during minimally invasive 60-degree nasal sectoral goniotomy for mild-to-moderate open angle glaucoma (OAG). This prospective, observational case series performed at a private clinical practice and outpatient surgery center assessed the eyes of patients with mild-to-moderate OAG who underwent phacoemulsification cataract surgery with 60-degree nasal sectoral goniotomy using the Espaillat juxtacanalicular space slider. The same surgeon performed all procedures. Intraocular pressure, intraocular pressure-lowering drugs, and adverse effects were assessed over 24 months, and a regression analysis of intraocular pressure reduction was performed. Success was defined as (1) A reduction in the intraocular pressure of at least 20% for at least 12 months, compared with the baseline value, and (2) a decrease in the number of medications by at least one for at least 12 months. Among 38 eyes, 27 (72%) had moderate glaucoma, and 11 (28%) had mild glaucoma. Postoperatively, intraocular pressure was decreased by 38% at 12 months (mean 13.7±1.7mmHg), 35% at 18 months (mean 14.4±1.8mmHg), and 31% at 24 months (mean 15.2±1.9mmHg). In addition, the number of topical glaucoma medications used decreased from a mean of 1.6 to 0.4 ( P <0.001). The main adverse event was temporary focal corneal edema (29 patients; 76%). Using the Espaillat juxtacanalicular space slider during invasive 60-degree nasal sectoral goniotomy for OAG yielded a significant and sustained reduction in intraocular pressure and reduced the need for glaucoma medications with minimal adverse events.

Risk, Prevalence, and Progression of Glaucoma in Eyes With Age-Related Macular Degeneration Treated With Intravitreal Anti–Vascular Endothelial Growth Factor Injections

To examine the risk, prevalence, and progression of glaucoma development in age-related macular degeneration (AMD) eyes receiving intravitreal anti-vascular endothelial growth factor (anti-VEGF) injections compared to controls. Retrospective clinical cohort study. Retrospective review of eyes receiving intravitreal anti-VEGF injections from January 1, 2004, to December 31, 2013, for exudative AMD. Age- and sex-matched control groups of eyes included eyes with nonexudative AMD (NEAMD) and no AMD. Eyes with a diagnosis of glaucoma or glaucoma suspect were reviewed for injection details, type and date of glaucoma diagnosis, glaucoma treatments, standard automated perimetry (SAP), and spectral domain optical coherence tomography (SD-OCT). Qualitative progression was determined by indication of glaucoma progression in provider notes. Quantitative progression was assessed based on change in mean deviation (MD) on SAP, retinal nerve fiber layer thickness on SD-OCT, and intraocular pressure (IOP). There were 707 eyes of 504 patients treated with anti-VEGF injections and 1008 eyes in the NEAMD and no-AMD cohorts. There was no difference in glaucoma or suspect prevalence at initial presentation between eyes treated with injections and NEAMD (6.9% vs 9.7%, P=.22) or no-AMD controls (vs 8.5%, P=.55). There was no difference in cumulative 5-year probability of new glaucoma diagnosis after anti-VEGF injections compared to NEAMD (1.9% vs 1.0%, P=.69) or no-AMD controls (vs 1.6%, P=.88). There was no difference in qualitative progression of glaucoma in the injection cohort vs NEAMD (P=.19) or no-AMD controls (P=.61). The rate of MD change in injection eyes was similar to NEAMD eyes (P=.74) but greater than no-AMD eyes (P=.02). Eyes receiving injections required more topical glaucoma medications compared with NEAMD (P=.03) and more glaucoma laser treatments compared with no-AMD controls (P=.009). Eyes receiving injections did not require more frequent incisional glaucoma surgery compared with NEAMD (21.0% vs 15.0%, P=.95) or no-AMD controls (vs 10.0%, P=.10). Eyes treated with intravitreal anti-VEGF injections for exudative AMD did not have increased risk of developing glaucoma compared with controls. Of those with a glaucoma diagnosis, exudative AMD eyes receiving injections required a greater number of topical glaucoma medications compared with NEAMD eyes and had a greater rate of MD loss than no-AMD controls.

12-Month Outcomes of Combined Phacoemulsification and iStent Inject in Asian Eyes with Normal Tension Glaucoma: A Single-Centre Experience.

To evaluate efficacy and safety of the iStent inject combined with phacoemulsification in Asian eyes with normal-tension glaucoma (NTG) up to 12months after surgery. Retrospective case series. Retrospective case series of eyes undergoing surgery from April 2017 to August 2019. Outcome measures included intraocular pressure (IOP), number of topical glaucoma medications and adverse outcomes. Data were retrieved from the post-operative day 1 (POD1), week 1 (POW1), month 1 (POM1), month 3 (POM3), month 6 (POM6) and month 12 (POM12) timepoints. Data were analysed at each timepoint for eyes with available clinical data at that visit. Subgroup analysis was performed for a consistent cohort which included only eyes with data at all six follow-up timepoints. Data from 91 eyes in the collective subject group was analysed. Majority of subjects were male (53, 58.2%) and Chinese (79, 86.8%). Mean age was 73.2 ± 7.2years. Pre-operatively, mean IOP was 14.3 ± 3.0mmHg and mean number of medications was 1.5 ± 0.8. Significant IOP reduction was observed at all timepoints except POW1 and POM12. The number of medications was reduced at all timepoints (p < 0.001). By POM12, there was reduction of 1.2 ± 1.0 medications (p < 0.001). Both the collective and consistent cohorts demonstrated a trend towards decreasing surgical efficacy with time, in IOP reduction and decrease in medications. None of the eyes experienced sight-threatening complications. Asian eyes with NTG undergoing combined phacoemulsification and iStent inject surgery demonstrated significant and sustained reduction in number of glaucoma medications up to 12months post-operatively.

Micropulse trans-scleral laser therapy outcomes for uncontrolled glaucoma: a prospective 18-month study

ObjectiveTo evaluate the efficacy and safety of micropulse trans-scleral laser therapy (mTLT) in glaucomatous patients. DesignProspective, interventional study in a university hospital setting. ParticipantsFifty-two eyes of 52 adult patients with uncontrolled glaucoma despite maximal tolerated medical treatment, and/or poor candidates for filtering surgery. MethodsParticipants received a 360-degree mTLT diode laser treatment (2000mW, 31.33% duty cycle), with duration adjusted to iris pigmentation and glaucoma severity (160–320 seconds). They were followed for 18 months to assess intraocular pressure (IOP), number of medications, corrected distance visual acuity (CDVA), glaucoma progression based on Humphrey Sita 24-2 perimetry and Cirrus high-definition optical coherence tomography, and complications. The primary outcome measure was the absolute success at 18 months. Absolute success was defined as an IOP 6–21 mm Hg and at least 25% IOP reduction, with equal or less number of IOP medications. Qualified success allowed for an increased number of IOP medications. Failure was defined as an inability to meet the criteria for success or the need for incisional glaucoma surgery. ResultsTreatment absolute success was 61.5% at 12 months and 59.6% at 18 months. Mean IOP was reduced by 35.6% at 18 months (23.6 ± 6.5 mm Hg at baseline; 15.2 ± 4.1 mm Hg at 18 months, p < 0.001). mTLT did not significantly reduce the number of topical glaucoma medications (p = 0.075); however, 15 eyes (29%) had systemic oral glaucoma treatment at baseline and 10 eyes (20%) at 18 months. Eight patients (15%) experienced vision loss of ≥2 lines after the procedure. Three patients (6%) regained their preoperative CDVA by 1 month, and 3 patients (6%) by 3 months, while 2 patients (4%) sustained persistent visual loss. No ocular complications were noted in 84.6%. Incisional surgery was required in 25% of eyes owing to inadequately controlled glaucoma despite mTLT. ConclusionsmTLT is a good therapeutic option for moderate IOP reduction, while being safe and predictable. This improved safety profile makes mTLT a treatment to be considered earlier in the management of glaucoma.

Evaluation of ocular surface disease in elderly patients with glaucoma: expression of matrix metalloproteinase‐9 in tears

PurposeTo evaluate the symptoms and signs of ocular surface disease (OSD) and tear film matrix metalloproteinase‐9 (MMP‐9) overexpression using point‐of‐care testing (InflammaDry test) in patients with primary open‐angle glaucoma (POAG).MethodsThis prospective, case‐control study included sixty‐seven patients diagnosed with POAG and forty‐seven healthy control subjects. The OSD assessment included Schirmer‐I test, the Oxford corneal stain scale, tear breakup time (TBUT), and the 5‐Item Dry Eye Questionnaire (DEQ‐5). Measurement of extracellular MMP‐9 was performed using the InflammaDry test. The OSD parameters and MMP‐9 expression levels were compared between the POAG and control groups. Additional subgroup analysis in the POAG group was performed according to the number of topical glaucoma medications (1, 2, or 3 bottles).ResultsThere were significant differences between the control and POAG groups for all OSD parameters. MMP‐9 overexpression was observed in 71.6% of POAG group, whereas only 31.9% of control group showed MMP‐9 overexpression. The subgroup analysis revealed that DEQ‐5, Oxford stain score, Schirmer‐I, and MMP‐9 overexpression demonstrated no significant difference among the three groups. Abnormal TBUT (≤ 5sec) was observed in 37.5%, 59.1%, and 76.2% of each subgroup according to number of medications (1, 2, and 3), and strong MMP‐9 overexpression was detected in 25.0%, 40.9%, and 61.9%, respectively (p = 0.032, p = 0.043).ConclusionsThe use of preservative‐containing medications may affect the ocular surface in patients with POAG. Graded measurement of tear‐film MMP‐9 could provide more information on OSD and might be a more useful marker for inflammation than conventional results obtained using an MMP‐9 kit.

Assessment of Ocular Surface Disease in Glaucoma Patients in Ghana.

Dry eye disease (DED) observed in this study is associated with ocular surface abnormalities as shown by specific dry eye tests. Changes in tear quality may be responsible for dry eye symptoms among the participants. This study sought to evaluate DED in glaucoma patients in Ghana. This was a hospital-based descriptive cross-sectional study involving glaucoma patients on topical glaucoma treatment for at least 6 months. The medical records of participants were thoroughly reviewed for information on demographics, ocular history, medical history, number of topical antiglaucoma medications used within the last 6 months, and any other ophthalmic medications used by the participants. An Ocular Surface Disease Index (OSDI) questionnaire was administered to each participant, after which Schirmer test and tear break-up time (TBUT) were performed on each participant in a controlled environment. A total of 100 individuals participated in the study. The mean±SD age of participants was 60.44±14.05 years. The majority (60.0%) of the participants were females and 55.0% of participants used 2 or more topical antiglaucoma medication. Of the participants, 81.0% reported subjective dry eye symptoms. Schirmer test and TBUT showed that 55.0% and 87.0% of the participants had decreased tear production and abnormal tear quality, respectively. Multiple logistic regression showed a significant association between age and abnormal OSDI (P=0.037), Schirmer test (P=0.003), and TBUT (P=0.042); the number of topical glaucoma medications and OSDI (P=0.016) and Schirmer test (P=0.009). The prevalence of DED was 23.0%. Increasing age was associated with an ∼7 times higher odds of having DED (P=0.0019), using >2 glaucoma medications with ∼4.5 times higher odds of having DED (P=0.001), 5 or more years of glaucoma medication use with ∼4 times higher odds of DED (P=0.009), and both benzalkonium chloride and sodium chlorite as preservative was associated with ∼3.5 times higher odds of DED (P=0.047). DED is prevalent among glaucoma patients in Ghana and is associated with age, duration of glaucoma medication, type of preservatives, and number of topical glaucoma medications.

Comparison of the Icare ic100 Rebound Tonometer and the Goldmann Applanation Tonometer in 1,000 Eyes

Background: Measurement of intraocular pressure (IOP) forms a crucial component in the diagnosis and management of glaucoma. Many devices have been developed to measure IOP with Goldmann applanation tonometry (GAT) considered the gold standard for IOP measurement. Objectives: The objective was to compare the IOP measured using Icare ic100 and GAT. Methods: This cross-sectional study measured IOP in 1,000 eyes (500 left and 500 right) using the Icare ic100 and GAT. Central corneal thickness (CCT) was measured using a hand-held pachymeter. IOP measurements were investigated in all eyes, by IOP substrata, by CCT group, by number of topical glaucoma medications, and diagnosis. Results: There was moderate agreement between ic100 and GAT IOP measurements (intraclass correlation coefficient 0.73). Mean IOP was significantly lower when measured by ic100 than by GAT (12.1 vs. 16.2 mm Hg, p < 0.0001). Mean ic100 IOPs were also significantly lower than mean GAT IOPs within each IOP strata ≤12 (7.9 vs. 9.7 mm Hg, p < 0.0001), 13–21 (12.1 vs. 16.6 mm Hg, p < 0.0001), and ≥22 (18.4 vs. 25.2 mm Hg, p < 0.0001) and within each subanalysis. Conclusions: The Icare tonometer consistently under estimated IOP compared to GAT, irrespective of CCT ranges and other subgroup analyses. The mean difference of 4.2 mm Hg can have significant clinical implications, particularly in the management of glaucoma patients.

Ab Interno Canaloplasty in Open-angle Glaucoma Patients Combined With In Vivo Trypan Blue Aqueous Venography.

To demonstrate canalogram patterns observed when trypan blue tracer is combined with oculoviscoelastic device during ab interno canaloplasty, and discuss surgical results and potential implications for diagnosis, prognosis, and treatment. This is a retrospective small case series study. The authors performed this procedure on 5 patients with informed consent with 4 to 18 months of follow-up. All procedures have been complication free. The average preoperative intraocular pressure was 16.4 mm Hg with an average number of topical glaucoma medications being equal to 3.6. Postoperatively, the average intraocular pressure for the group was reduced to 13 mm Hg with a concurrent drop in the average number of topical medications used equaling 1.8. Currently, there are limited practical options to evaluate the patency of the aqueous outflow system in our patients. The primary purpose of this paper is to describe canalogram patterns observed during ab interno canaloplasty with trypan blue. This technique may allow us to further our understanding of the aqueous outflow system and its role in the underlying pathophysiology of glaucoma. Further research is required to evaluate the prognostic and diagnostic significance of this technique.

The subgroup analysis revealed that DEQ-5, Oxford stain score, Schirmer-I, and MMP-9 overexpression demonstrated no significant difference among the three groups. Use of iStent as a Standalone Operation in Patients with Open-Angle Glaucoma.

Purpose The iStent provides a direct pathway for aqueous outflow from the anterior chamber to Schlemm's canal in patients with open-angle glaucoma (OAG). We performed a meta-analysis to evaluate the effectiveness of iStent as a standalone operation in patients with OAG in reducing the intraocular pressure (IOP) and the number of topical glaucoma medications. Methods We searched various databases between January 1, 2000, and September 30, 2019, and included only peer-reviewed, prospective, or retrospective clinical studies in our analyses. Details regarding the IOP and the number of medications at baseline and end point were recorded from each study. Standardized mean differences (SMDs) of IOP and medication numbers were calculated. Furthermore, the success rate (the proportion of IOP ≤18 mmHg and IOP reduction ≥20% at end point) and the complication rate were also summarized. Finally, a subgroup analysis was done based on the iStent generation (first and second), follow-up duration (≤6, 6–18, 18–36, and >36 months), and iStent number (one, two, and three). The outcome measures were aggregated SMDs computed from each study. Results A total of 17 studies with 978 eyes were included in this analysis. All studies demonstrated a reduction in IOP after iStent implantation. Aggregated SMDs of IOP revealed a significant reduction (SMD = −2.64, 95% confidence interval (CI): −3.21 to −2.07). The success rate was significantly good, and most of the complication rates were low. The number of medications was also significantly reduced (SMD = −1.71, 95% CI: −2.18 to −1.24). The subgroup analysis revealed a reduction in IOP and medication burden in each category of iStent generation, follow-up duration of up to 42 months, and iStent numbers. Conclusion Use of iStent as a standalone procedure does reduce the IOP and the number of glaucoma medications. The benefit of iStent lasts for at least 42 months.

The Effectiveness and Safety of Micropulse Cyclophotocoagulation in the Treatment of Ocular Hypertension and Glaucoma

To evaluate the effectiveness and safety of primary and adjunctive micropulse cyclophotocoagulation (MPCPC) in the treatment of ocular hypertension (OHT) and glaucoma. Retrospective cohort study. Ocular hypertension and all severities of glaucoma (including treatment-naïve and well-sighted eyes) and all types of glaucoma (including normal-tension glaucoma [NTG]). Consecutive eyes with glaucoma or OHT that underwent MPCPC (Iridex Cyclo G6 Glaucoma Laser System, Mountain View, CA) between 2016 and 2018 were identified. Intraocular pressure (IOP), visual acuity, glaucoma medications, and ocular adverse events. Three hundred ninety-nine MPCPC surgeries, on 342 eyes of 214 patients, were analyzed. Laser power ranged from 900 to 2800 mW. Main diagnoses in descending prevalence were primary open-angle glaucoma (55.9%), chronic angle-closure glaucoma (10.8%), neovascular glaucoma (9.0%), NTG (6.5%), and OHT (5.5%). Mean baseline IOP was 19.8±7.4 mmHg and IOP reduction was 22.7%, 20.2%, 20.7%, and 23.7% at postoperative months (POMs) 1, 3, 6, and 12 (P < 0.0001 for all time points). The end point of 20% or more mean IOP reduction from baseline was achieved by 67.8% of the study cohort at POM 12. Additional mean IOP reduction of 16.4% (P < 0.0001) was achieved with each re-treatment. Analysis based on IOP stratification demonstrated 30.5% mean IOP reduction at POM 12 when baseline IOP was more than 21 mmHg and 20.1% when it was 21 mmHg or less (71% of overall cohort; P < 0.0001). Analysis based on laser power stratification demonstrated mean IOP reduction of 31.5% at POM 12 with laser power of 2500 mW or more and 17.8% with laser power of less than 2500 mW (P < 0.02). Overall, the mean number of topical glaucoma medications was unchanged from baseline to POM 12. Greater baseline IOP and number of baseline topical glaucoma medications were significant predictors of effectiveness in the regression analysis. No patients demonstrated persistent inflammation or hypotony, phthisis bulbi, or sympathetic ophthalmia. In patients with OHT or glaucoma, MPCPC demonstrated effectiveness and safety in IOP reduction sustained to 1 year. Baseline IOP of 21 mmHg or less subgroup demonstrated a more limited response. A dose-response relationship is suggested with respect to laser power and repeat treatments.

Efficacy of the XEN gel stent on intraocular pressure lowering in East Asian eyes.

This study aims to evaluate the efficacy and safety of XEN45 gel stent (Allergan plc, Dublin, Ireland) in East Asian patients with glaucoma. A retrospective study of consecutive XEN45 gel stent implantation surgeries with or without concurrent cataract surgery from May 2017 to August 2018 was performed in a single institution. The clinical charts of all cases were reviewed to examine the safety and efficacy of the implant. Primary outcome measures were intraocular pressure (IOP), number of topical glaucoma medications required, additional procedures done and adverse outcomes. All patients included were required to fulfil a minimum of three follow-up visits by post-operative month 1. Sixty-three eyes were included in the study. The median duration of follow-up was 6months [interquartile range (IQR) 2-9]. Median IOP was significantly lowered from 19mmHg (IQR 16-24) pre-operatively to 15mmHg (IQR 11-18) at the end-of-follow-up (p < 0.001). The median number of medications was reduced from 3 (IQR 2-4) at baseline to 0 (IQR 0-2) at the end-of-follow-up (p < 0.001). 66.7% of patients were medication-free by the end of follow-up. Post-operative bleb intervention was required for 61.9% of eyes. Adverse events included post-operative hypotony requiring intervention (4.8%) and post-operative gross hyphaema (9.6%). The XEN45 gel stent when used as a sole procedure or in combination with cataract surgery in East Asian eyes is effective in the treatment of glaucoma. However, a high rate of post-operative bleb intervention appears to be required to support its efficacy.

Double-session micropulse transscleral laser (CYCLO G6) for the treatment of glaucoma.

This study aims to evaluate the effectiveness and safety of double-session micropulse transscleral (MP3) laser for the treatment of glaucoma. This was a retrospective chart review of all patients who underwent MP3 laser with the standard protocol and were within at least 12months of their follow-up appointments. Eyes were treated at 2000mW with 31.3% duty cycle for 80s per treatment session, with double sessions in each hemifield alternating between upper and lower and upper and lower. Success criterion was defined as a greater than 20% reduction in intraocular pressure (IOP) and an IOP between 6 and 18mmHg at the end of the follow-up period. Eighty-nine eyes of seventy-six glaucoma patients were included; 31 of which were considered primary, i.e., no previous glaucoma surgery. The average pre-treatment IOP was 29.9 ± 6.4mmHg. At around 16.7 ± 3.1months of follow-up, the average IOP was 14.8 ± 4.7mmHg (p < 0.001) (28.8 ± 7.4 to 15.7 ± 5.3mmHg, p < 0.001, at 17.0 ± 3.1months in the primary eyes). The number of topical glaucoma medications was reduced from 3.6 ± 0.5 to 1.9 ± 0.9 at the last evaluation (p < 0.001). The best corrected visual acuity in logMAR ranged from 0.41 ± 0.34 preoperatively to 0.45 ± 0.44 at the last visit (p = 0.2), and also remained stable in primary eyes. Success was obtained in 86.5% of the eyes, with 1.5 ± 0.8 procedures, and in 90.3% with 1.2 ± 0.5 MP3 laser procedures in the primary eye group. MP3 laser in two consecutive 80-s sessions was shown to be safe and effective in the treatment of glaucoma. Primary eyes required lower number of MP3 laser procedures.

The STARflo™ glaucoma implant: a single-centre experience at 24months.

To evaluate the 24-month outcomes following STARflo™ implantation in patients with moderate or advanced open-angle glaucoma. We enrolled 32 patients (40 eyes) with high intraocular pressure (IOP) resistant to topical and systemic medical therapy. After baseline assessments, patients were implanted with STARflo™ implants with the goal of IOP reduction and long-term maintenance. Patients were followed for 24months. Complete success of implantation was defined as a restoration of normal IOP without topical glaucoma medications, while qualified success was defined as a restoration of normal IOP with implantation and topical glaucoma medications. STARflo™ did not satisfactorily reduce IOP at 24months. Twenty-eight eyes (70%) had elevated IOP at least once during the 24months post-implantation period. Five eyes (12.5%) developed corneal decompensation. The average IOP 24months after the surgery was 13.42 ± 6.03mmHg and was not significantly different than IOP at 12months (13.2 ± 5.59mmHg). Moreover, 45% of treated eyes needed additional glaucoma procedures after 1year and 2years to achieve these IOPs. STARflo™ did reduce the average number of topical glaucoma medications from 2.7 pre-implantation to 1.17 at 24months postoperatively in the rest 55% of the eyes. The STARflo™ implantation did not meet success criteria and so appears to be an ineffective alternative to filtering surgical procedures for patients with treatment-refractory open-angle glaucoma.

Efficacy and safety of transscleral cyclophotocoagulation in Swedish glaucoma patients.

To retrospectively evaluate the efficacy and safety of all transscleral cyclophotocoagulation (TCP) treatments performed during a 5-year period. Medical records of all patients, who had undergone TCP treatment between 2010 and 2014 at Umeå University Hospital, Sweden, were evaluated. Clinical data including intraocular pressure (IOP), visual acuity (VA), number of topical glaucoma medications, use of oral acetazolamide, retreatments and complications during a 2-year follow-up were registered. Global success was defined as IOP 6-18mmHg with or without glaucoma medication. Three hundred patients underwent TCP during the time period. Mean IOP at baseline was 29.3±11.0 (mean±standard deviation) mmHg (n=297) with a mean reduction of 11.5 (±12.0) mmHg at 1year (n=258; p<0.001) and 12.6 (±12.0) mmHg at 2-year follow-up (n=245; p<0.001). Global success at 2years was 64%, achieved by a mean of 1.2 treatments (n=257). The number of topical glaucoma medications at baseline was 3.1 (±1.0; n=296) and was reduced by 0.9 (±1.0) medications at 2years (n=244; p<0.001). Use of oral acetazolamide decreased from 30% (n=90) at baseline to 5.3% (n=13) at 2years. In eyes with Snellen VA≥0.1, the mean VA at baseline was 0.55 (±0.3) logarithm of the minimum angle of resolution (logMAR; n=132) and 1.1 (±0.9) logMAR (n=76) at 2years (p<0.001). No cases of phthisis bulbi were found. This study displays a substantial and long-term reduction of IOP following TCP with a decrease in topical and oral glaucoma medications. The treatment appears to be safe but the decrease in VA during follow-up is a concern that needs further evaluation.

Changes in Intraocular Pressure and Angle Status After Phacoemulsification in Primary Angle Closure Hypertension.

To study the change in intraocular pressure (IOP) and angle status after phacoemulsification in a cohort of primary angle closure hypertension (PACHT) patients. Tertiary Eye Care, India. Prospective interventional case-series. Case-series of 18 eyes of 18 patients. Preoperative biometry including axial length, anterior chamber depth (ACD), lens thickness, and central corneal thickness were studied. Preoperative and postoperative IOP, number of topical glaucoma medications, angle swept source optical coherence tomography (SS-OCT) parameters of nasal and temporal angle opening distance, trabecular iris space area, scleral spur angle, lens vault and circumferential iridotrabecular contact (ITC) were studied and their correlations derived. The mean preoperative IOP, 31±6 mm Hg, decreased to 14±1 mm Hg at 6-months postoperative period, P<0.001, a fall by 58±14%. The number of glaucoma medications reduced from 4(3-5) to 2(1-3), P<0.001. All SS-OCT studied parameters denoted significant angle widening postsurgery. The ITC% reduced from 52(16-100) to 19(0-97), P<0.001. The preoperative ITC showed moderate to strong correlation with all preoperative nasal and temporal angle parameters. It also showed moderate positive correlation with percentage fall in IOP at 1-month postoperative period. The preoperative ACD showed significant negative correlation with preoperative and postoperative ITC. PACHT patients benefit significantly from cataract surgery with marked angle widening, IOP reduction and a decrease in the number of glaucoma medications. The SS-OCT derived circumferential iridotrabecular contact index can be used as the single best parameter to indicate the preoperative angle status and predict postoperative change in IOP, as against the numerous single section measured angle parameters.

Long-term outcome of mitomycin C-augmented needle revision of trabeculectomy blebs for late trabeculectomy failure.

To describe the 3-year outcomes of mitomycin C (MMC)-augmented bleb needling procedures following late trabeculectomy failure. A retrospective, observational chart review of 44 eyes that underwent MMC-augmented bleb needling following late trabeculectomy failure. The primary outcome measure was IOP at 3 years post-operatively. Secondary outcome measures were the number of topical glaucoma medications and success rate at 3 years. Success was defined on the basis of IOP (≤21 mmHg and ≥5 mmHg with a reduction of ≥ 20% from pre-operative baseline), need for ocular hypotensive medications, and need for further laser or surgical intervention. Mean IOP was reduced from 19.8 mmHg preoperatively to 13.9 mmHg (29.5% reduction) at 3 years. By the 3-year time point, 26 eyes (59.1%) had met one or more of the criteria for failure. A lower immediate post-operative IOP was found to be associated with greater likelihood of success. Bleb needling augmented with MMC can achieve a significant reduction in IOP in eyes with late trabeculectomy failure. By 3 years however, 59% of eyes were classified as failures, with half requiring further glaucoma surgery or laser. Patients undergoing this procedure should therefore be warned of the likelihood of requiring further intervention, and a repeat antimetabolite-augmented trabeculectomy or glaucoma tube shunt surgery in the first instance can be considered instead.
