Clinical analysis of 2152 cases of abnormal uterine bleeding treated by NovaSure endometrial ablation.

ObjectiveTo evaluate the efficiency, postoperative hysterectomy rate, and influencing factors for therapeutic effect of the NovaSure endometrial ablation procedure in abnormal uterine bleeding (AUB).MethodsWe conducted a retrospective cohort study of 2152 patients from the Department of Gynecology at the Third Xiangya Hospital, CSU from October 2010 to December 2018.ResultsFrom the first year to the eighth year after operation, annual effective rate was above 95.24%, and the differences were not statistically significant. There are statistically significant differences between the effective and ineffective groups with regard to age, intrauterine polyps, total length of the uterus, systemic coagulation disorder, and preoperative hemoglobin. A multivariate logistic regression analysis showed that the risk factors associated with systemic coagulation disorders (P = 0.027) and high total uterine length (P = 0.003) affected NovaSure efficacy in the treatment of AUB. By December 2019, the postoperative hysterectomy rate was 1.86% (40/2152) and the complication rate was 1.67% (36/2152).ConclusionNovaSure is a reliable treatment for AUB and serious medical complications because of its simple operation, low amount of bleeding, quick postoperative recovery, and safe and effective short‐term and long‐term efficacy. However, it should be carefully selected for patients with a total uterus length exceeding 10 cm.

1322 Comparison of Two Endometrial Ablation Techniques

Study Objective To compare the effectiveness of two methods (Novasure and Minerva) of endometrial ablation in a single community private practice. Design Retrospective chart review of all practice patients undergoing Novasure or Minerva endometrial ablation between 2015 and 2018. Minimum follow-up after the procedure was six months. Setting Procedures were all performed at Saddleback Center for Surgery (ambulatory surgery center) under general anesthesia by the physicians of Orange Coast Women's Medical Group. Patients or Participants All ablations performed between 2015 and 2018 were reviewed. 208 total ablations were performed. There were 127 in the Minerva cohort and 81 in the Novasure cohort. Interventions Not Applicable Measurements and Main Results Charts were reviewed for patient age, amenorrhea rates, rate of subsequent hysterectomy, improvement in bleeding, number of procedures performed by each provider, and days of follow up. Key results revealed no significant difference in the rate of amenorrhea in patients undergoing ablation with Minerva 32.3% (41/127) and Novasure 40.7% (33/81), (p=0.21). There was no significant difference in the rate of subsequent hysterectomy, 5.5% in the Minerva group and 9.9% in the Novasure group (p=0.24). There was no significant difference in patients showing improvement in bleeding without amenorrhea, 48.8% in the Minerva group and 44.4% in the Novasure group (p=0.72). There was no significant difference in the total combined amenorrhea and bleeding improvement; 81.1% in the Minerva group and 85.1% in the Novasure group (p= 0.45). Conclusion In our series, there was no significant difference in the Minerva or Novasure endometrial ablation systems in the rate of amenorrhea, subsequent hysterectomy, and overall decrease in bleeding.

Comparison of Combined Bipolar Radiofrequency Impedance-Controlled Endometrial Ablation with Levonorgestrel Intrauterine System versus Bipolar Radiofrequency Endometrial Ablation Alone in Women with Abnormal Uterine Bleeding

Study ObjectiveThis study aims to evaluate the efficacy of the combination of bipolar radiofrequency impedance-controlled endometrial ablation (NovaSure; Hologic Inc., Bedford, MA) and levonorgestrel intrauterine system (LNG-IUS; Mirena; Schering AG, Berlin, Germany) placement in comparison with NovaSure endometrial ablation alone in patients with abnormal uterine bleeding (AUB). DesignA propensity score matching study. SettingBeijing Tiantan Hospital, Capital Medical University, Beijing, China. PatientsA retrospective study was conducted on 246 patients with AUB who underwent NovaSure endometrial ablation with (NovaSure+LNG-IUS group) or without (NovaSure group) LNG-IUS between January 2013 and August 2016. To overcome selection bias, propensity score matching was used to establish a 1:1 match between these 2 groups. Accordingly, 41 patients were included in each group. InterventionNovaSure endometrial ablation, immediately followed by LNG-IUS insertion in the NovaSure+LNG-IUS group, and NovaSure endometrial ablation alone in the control group. Measurements and Main ResultsFollow-up assessments performed at postablation months 6, 12, and 24 revealed the following: The rate of amenorrhea (78.05% vs 46.34%, 85.37% vs 53.65%, and 87.80% vs 58.54%, respectively; p <.005) and the rate of dysmenorrhea remission (100% vs 70.59%, 100% vs 64.70%, and 100% vs 64.70% [p <.05, p <.01, and p <.01], respectively) were significantly higher in the NovaSure+LNG-IUS group than in the NovaSure group. The rate of reinterventions was similar for both groups at postablation month 6. However, at postablation months 12 and 24, these rates were significantly lower in the NovaSure+LNG-IUS group than in the NovaSure group (0 vs 14.63% and 2.44% vs 21.95% [p <.05 and p <.01], respectively). ConclusionFor women with AUB, the combination of NovaSure endometrial ablation and LNG-IUS is more effective than NovaSure alone in achieving amenorrhea, alleviating dysmenorrhea and reducing reinterventions.

A large number of clinical trials, as well as retrospective analysis of clinical and financial data, has shown that endometrial ablation allows for a lower morbidity and mortality rate and significantly lower procedure costs. Worldwide, endometrial ablation is increasingly being adopted by the gynecological community. Aim of the Work: to study the effect of bipolar radiofrequency ablation system on endometrial thickness and bleeding in patients with premenopausal bleeding. Patients and Methods: This study was conducted on: 30 women with premenopausal bleeding not responding to medical treatment. All subjects enrolled in the study were asked to give an informed written consent after explaining the nature, steps and aim of the study. The approval of Medical Ethics Committee was obtained. Results: the net results after treatment, the majority of the patients was improved (83.3%), 19 of them was complete recovery (63.3%) while 6 cases (20.0%) had minor bleeding, 4 cases (13.3%) had bleeding less before treatment, while only one case had no change.Conclusion: Bipolar radiofrequency ablation performed under local anaesthetic in the postmenstrual phase is an effective and efficient method of treating the majority of women who wish conservative surgical treatment for heavy menstrual loss.

Predictors of Long-Term NovaSure Endometrial Ablation Failure

Study ObjectiveEndometrial ablation using the NovaSure system (Hologic Inc., Marlborough, MA) is 1 of the treatment options for heavy menstrual bleeding (HMB), which has a reported success rate of 81% to 90%. We aimed to identify predictors for NovaSure endometrial ablation failure. This will contribute to a more effective and individualized preoperative counseling. DesignA retrospective multicenter cohort study (Canadian Task Force classification II-2). SettingOne university hospital and 1 large teaching hospital. PatientsFour hundred eighty-six patients with HMB who had undergone NovaSure endometrial ablation between 2008 and 2014. InterventionsThe NovaSure endometrial ablation system for patients with HMB. Measurements and Main ResultsIn total, the required characteristics of 486 patients were collected and analyzed. With a median follow-up of 45 months, 19.3% (n = 94) required a secondary treatment and thus were considered NovaSure failures. Multivariate logistic regression analysis showed that younger age (p = .019), a history of sterilization (p = .002), the presence of dysmenorrhea (p < .001) and the presence of an intramural leiomyoma on transvaginal sonography (p = .005) were independent predictors of NovaSure ablation failure. Preoperative hysteroscopy appeared to be a protective factor (p = .001). ConclusionWithin a median follow-up of 45 months, 19.3% of patients required a secondary treatment. Identification of specific predictive factors for endometrial ablation failure is important in preoperative counseling and patient selection. In the current era of personalized medicine, doctors and patients will ideally choose the therapy that results in the highest chance of success in the individual case.

Complete amenorrhea following NovaSure endometrial ablation for resistant menorrhagia: a case report

Heavy menstrual bleeding (HMB) or menorrhagia is the most common form of dysfunctional uterine bleeding (DUB). In spite of medical treatment for DUB, many women will eventually require a hysterectomy, which is an invasive treatment option. NovaSure ablation offers a same day non-invasive alternative to hysterectomy and hysteroscopic ablation. A 38-year-old woman presented with HMB in 2015. The attacks of HMB interrupted her lifestyle, and she refused to continue with medical treatment, which failed to resolve her symptoms. This patient was admitted to the hospital four times over 2015 due to the anemia caused by the DUB. Medroxyprogesterone acetate and oral contraceptive pills failed to control the patient’s bleeding episodes. Hysteroscopic examination of the uterine cavity showed a normal cavity, and the endometrial biopsy showed proliferative endometrium. She was counseled about NovaSure ablation as the last treatment option before hysterectomy. The NovaSure ablation procedure took 90 s, and the patient was discharged from the hospital 6 h after the procedure. At follow-up, the patient is completely amenorrheic, and she is satisfied with her results. This study demonstrated that NovaSure endometrial ablation is a safe, effective, non-invasive alternative to hysteroscopic endometrial ablation for treatment of DUB.

Outcome of Novasure Endometrial Ablation in Women with Heavy Menstrual Bleeding with or without Dysmenorrhoea, Including Those with Uterine Cavity Length More Than 6.5 cm

Heavy Menstrual Bleeding (HMB) is a common health problem and may be associated with iron deficiency anaemia affecting the quality of life of many women. Problems of HMB can be more complicated when it is associated with dysmenorrhea. Traditionally, medical management of HMB has been the first line therapy. Unfortunately, many options can be associated with hormonal side effects or lack of efficacy, leading to discontinuation of medical therapy and requirement of surgical interventions.

The Effect of Uterine Size on the Failure Rate of NovaSure Radiofrequency Endometrial Ablation [11D

INTRODUCTION: Utilization of NovaSure endometrial ablation has been shown to be successful in treating menorrhagia. Little data exists describing NovaSure ablation failure. The purpose of this research is to determine variables most predictive of failure with NovaSure endometrial ablation. METHODS: A retrospective case-control study included 224 women who received NovaSure ablation between 2003-2014 at Memorial University Medical Center. 112 of these patients were identified as failures receiving a subsequent hysterectomy and the other 112 women were identified as control patients without a need for subsequent hysterectomy. Patient charts were accessed and reviewed to determine uterine measurements and the preoperative status of each woman including pre-operative pain and prior bilateral tubal ligation. Using SPSS Statistical software, the data was analyzed. RESULTS: Mean uterine sound (p= 0.043) and mean cervical length (p= 0.049) were predictive of failure. Secondary analyses determined women with prior bilateral tubal ligation (p= 0.009) predicted failure with NovaSure ablation. Women who experienced preoperative pelvic pain trended toward failure requiring subsequent hysterectomy, but no statistically significant difference was noted (p= 0.073). CONCLUSION: Within the limitations of this study, a NovaSure ablation in women with a longer uterus and a longer cervix yields higher failure rates with subsequent hysterectomy. The data also supports the hypothesis that the presence of tubal sterilization is associated with increased risk of subsequent hysterectomy. Determining the significance of pre-operative pain will require further chart investigation. This study can help physicians better identify NovaSure ablation candidates that will be successful thereby improving cost efficacy.

Effectiveness of thermal balloon ablation versus NovaSure endometrial ablation in different age groups

The aim of this study is to compare the frequency of amenorrhoea, overall satisfaction and hysterectomy after thermal balloon ablation (TBA) versus NovaSure endometrial ablation (NA) in women at different ages, being the commonest ablation devices used currently in the UK. It is a retrospective review of women who had endometrial ablation, using TBA or NA, at the Pennine Acute Trust, between 2009 and 2012. Patients were grouped into two cohorts: those aged up to 45 years and those of 46 years or more. A telephone questionnaire was used to assess patient’s pattern of bleeding after the procedure, and if they needed further management including medical or surgical intervention. It also looked at the overall satisfaction. In group A (45 years old or less), the amenorrhoea rate was 44 % after NovaSure and 23 % after TBA. Satisfaction was equal at around 80 % in both groups. Hysterectomy rates were 23 % in the TBA group and 20.8 % in the NovaSure group. In group B (46 years old or more), the amenorrhoea rate was 57 % after TBA and 50 % after NovaSure. Satisfaction rates were similar. However, hysterectomy rates were 10 % after TBA for persistent heavy periods and 8 % after NovaSure. In conclusion, NA ablation is more effective than TBA in achieving amenorrhoea in younger women and should be the preferred treatment option. Failure of ablation and/or hysterectomy rates were higher in younger women, especially those with dysmenorrhoea and after TBA. This highlights the necessity for careful selection of the treatment modality especially for younger women with HMB and for clear patient counselling.

Patient satisfaction and amenorrhea rate after endometrial ablation by ThermaChoice III or NovaSure: a retrospective cohort study

Heavy menstrual bleeding poses an important health problem, which can be managed, besides other treatments, with endometrial ablation. Nowadays, the bipolar radio frequency device (NovaSure) is the most commonly used device for endometrial ablation, followed by the thermal balloon device (ThermaChoice III). Thus far, studies looking at treatment outcomes have mainly been done comparing NovaSure with the older ThermaChoice (I–II) devices. The aim of this study is to compare the effectiveness of the improved ThermaChoice III with NovaSure. Patients treated with ThermaChoice III at the Ziekenhuisgroep Twente hospital and NovaSure at the Medisch Spectrum Twente hospital were included in the study. The primary outcome measure was patient satisfaction after treatment, measured by the condition-specific menorrhagia multi-attribute scale (MMAS). The secondary outcome measure was effectiveness of the treatment, measured by the amenorrhea rate and the hysterectomy rate. Five hundred fourteen patients were included in this study; of these, 216 patients were treated with ThermaChoice III and 289 patients with NovaSure. The score on the condition-specific MMAS was high for both groups, without a significant difference between the groups (88.8 vs 86.5, p = 0.183). The amenorrhea rate was significantly higher in the NovaSure group (45 vs 27 %, p = 0.001). The hysterectomy rate was slightly higher in the ThermaChoice III group, without a significant difference between the groups (19 compared to 13 %, p = 0.066). Effect of Undiagnosed Deep Adenomyosis After Failed NovaSure Endometrial Ablation

Study ObjectiveTo determine the prevalence of adenomyosis and deep adenomyosis after NovaSure (Hologic Inc., Newark, DE) endometrial ablation in hysterectomy specimens after NovaSure endometrial ablation failure. DesignProspective observational study (Canadian Task Force classification II-2). SettingThe TweeSteden Hospital, a teaching hospital in the south of the Netherlands. PatientsAll women who underwent hysterectomy for menorrhagia and/or dysmenorrhea after failure of NovaSure endometrial ablation between November 2007 and January 2011. InterventionsAll patients who underwent hysterectomy between January 2005 and April 2009 for the same indication but did not undergo prior endometrial ablation from choice or the lack of availability of the endometrial ablation procedure. Measurements and Main ResultsOf 213 patients who underwent NovaSure therapy, 22 (10.3%) underwent a hysterectomy because NovaSure failed. Of these, 10 (45.5%) exhibited adenomyosis in their hysterectomy specimens. The control group patients had a similar adenomyosis prevalence (74/173, 42.8%). However, the NovaSure failure group had a significantly higher prevalence of deep adenomyosis (>2.5 mm endometrial penetration) (9/22, 40.9%) than the control group (37/173, 21.4%, p < .05). ConclusionDeep adenomyosis after failed NovaSure endometrial ablation was present in a significant number of patients. It is not clear whether adenomyosis is induced by endometrial ablation or whether it causes endometrial ablation failure.

Postablation Tubal Sterilization Syndrome: A Case Report

Background: Endometrial ablation is a minimally invasive, readily available procedure for the management of persistent heavy uterine bleeding (HUM) with good results. Complications though rare, have included post-ablation tubal sterilization syndrome. Case: A 39-year-old multiparous lady with persistent, unresolved HUM despite conservative therapy and a history of bilateral tubal ligation with filshie clips underwent an uncomplicated hysteroscopy with Novasure endometrial ablation. Subsequently, she had severe recurrent cyclic pelvic pain of over 6months duration, which was relieved after a total laparoscopic hysterectomy with pathology showing thickened and dilated proximal tubes with hematosalpinx. Conclusion: Physicians performing endometrial ablation should have a high index of suspicion for, be able to diagnose and treat post ablation tubal sterilization syndrome.

Comparative Efficacy of NovaSure, the Levonorgestrel-Releasing Intrauterine System, and Hysteroscopic Endometrial Resection in the Treatment of Menorrhagia: A Randomized Clinical Trial

Abstract Objective: The aim of this study was to determine the efficacy of NovaSure, the levonorgestrel-releasing intrauterine system (Mirena), and hysteroscopic endometrial resection in the treatment of menorrhagia. Design: The study was a prospective, randomized, clinical trial. Materials and Methods: A total of 110 consecutive patients with menorrhagia were included and divided randomly into three groups. Forty-eight subjects were in the Mirena group, 30 were in the NovaSure endometrial ablation group, and 32 were in the hysteroscopic endometrial resection group. Successful results (according to decreased blood loss and less interaction between bleeding and normal activity), complications, and patient satisfaction were compared between the three groups. Results: After treatment, there was no hemorrhage in 44.8% of patients in the NovaSure group, 12.2% in the Mirena group, and 43.3% in the hysteroscopic endometrial resection group (p=0.005). Evaluation of the NovaSure Endometrial Ablation Procedure in Women With Uterine Cavity Length Over 10 cm

ObjectiveTo evaluate procedure-related adverse events, post-procedure menstrual bleeding status, and surgical re-intervention in women with a sounded uterine length>10cm compared to women with a sounded uterine length≤10cm who underwent the NovaSure endometrial ablation procedure. MethodsWe conducted a retrospective cohort study of 188 women from a Canadian community-based gynaecology practice. Eighty-seven women had a sounded uterine length>10cm, and 101 patients had a sounded length≤10cm. Procedure-related adverse events, post-procedure menstrual bleeding status, and surgical re-interventions were compared between groups. ResultsMean uterine sounding lengths were 11.0±0.6cm and 8.9±0.8cm in the>10cm and≤10cm groups, respectively. There were no differences between the groups in demographic characteristics or gynaecologic history, with the exception of higher BMI in the>10cm group and a greater prevalence of dysmenorrhea in the≤10cm group. Overall, 44.1% of all participants had been unsuccessfully treated with hormonal therapy, and 20.7% had failed non-hormonal treatment before the ablation procedure. The remaining 35.2% of participants had declined alternative therapy and proceeded directly to endometrial ablation. No serious procedure-related adverse events occurred in either group. Menstrual bleeding status at follow-up at 30.4±15.3months (>10cm group) and 30.5±15.5months (≤10cm group) was not different between the groups (P=0.85). In women who did not undergo surgical re-intervention after the initial ablation, amenorrhea was reported by 51.9% in the>10cm group and 65.9% in the≤10cm group. ConclusionThe NovaSure endometrial ablation procedure was associated with successful menstrual bleeding outcomes in women with sounded uterine length>10cm. No serious procedure-related adverse events occurred, and the need for surgical re-intervention was low. There was no significant difference in bleeding rates between the>10cm and≤10cm uterine length cohorts.

Women's satisfaction and symptoms following NovaSure endometrial ablation: A postal questionnaire survey in Lothian, Scotland

Impedance-controlled endometrial ablation (NovaSure) is commonly used to treat premenopausal heavy menstrual bleeding in Lothian, Scotland. Using postal questionnaires, we assessed patient satisfaction, treatment success and post-procedure symptoms in a cohort of women who had NovaSure treatment between January 2007 and May 2009. The response rate was 61.2% (115/188): 90.4% (104/115) of women were satisfied and would recommend the procedure; 36.8% (42/114) of women were amenorrhoeic at follow-up (mean = 497 days) and NovaSure significantly improved levels of both menstrual bleeding and pain (p < 0.01). Of the women, 67.0% (77/115) reported symptoms such as discomfort, vaginal discharge and bleeding/spotting within 6 weeks following NovaSure. A total of 13.9% (16/115) required further treatment following NovaSure, including hysterectomy. Patient satisfaction with NovaSure was high. Preoperative counselling needs to include information on character and duration of postoperative symptoms. Future research could compare patient satisfaction and clinical outcomes between NovaSure and the levonorgestrel-releasing intrauterine system.

Post-Ablation Tubal Sterilization Syndrome (PATSS) Following Novasure Endometrial Ablation: Two Case Reports and Review of Literature

PATSS is a complication that potentially occurs following a global endometrial ablation in women with previous tubal sterilization. PATSS presents as cyclic pelvic pain caused by tubal distention from occult bleeding into the obstructed tubes. We report two cases of PATSS after NovaSure radiofrequency endometrial ablation.
