Expression of Biofilm-Related Genes in Extensively Drug-Resistant Acinetobacter baumannii

Background: Acinetobacter baumannii is an important cause of nosocomial pneumonia in patients requiring long-term mechanical ventilation. Besides, extensively drug-resistant (XDR) strains cause infection in intensive care unit (ICU) patients. Chronic infections of A. baumannii and antimicrobial resistance are associated with biofilm formation. Several virulence genes, such as blaPER-1, pgaA, and bap, are involved in biofilm formation. Objectives: The current study examines the expression levels of biofilm formation-related genes in pneumonia patients. Methods: The sputum samples were collected from patients hospitalized in the ICU, and A. baumannii ATCC 19606, the reference strain, was isolated and cultured on blood agar, eosin methylene blue agar, and chocolate agar medium. The media were then incubated at 37°C for 18 - 24 hours. Next, Gram-Thirty XDR A. baumannii isolates were collected from the sputum samples of ICU patients at Besat Hospital in Tehran, Iran. Bacterial isolates were characterized for antibiotic resistance patterns and biofilm-forming ability. Subsequently, RNA was extracted from the biofilm-forming isolates. A real-time polymerase chain reaction (PCR) assay was performed to evaluate the expression levels of the blaPER-1, pgaA, and bap genes. Transcripts were normalized to 16S rRNA as an internal control, and gene expression fold changes were calculated. Statistical analysis was performed using an unpaired two-tailed t-test (P < 0.05) with SPSS (V. 16). Results: The disk diffusion susceptibility test revealed that all 30 (100%) isolates were resistant to piperacillin-tazobactam, cefepime, ceftriaxone, ceftazidime, gentamicin, imipenem, meropenem, levofloxacin, and ciprofloxacin. All 30 isolates from ICU-admitted patients (100%) were classified as XDR, and 27 (90%) isolates demonstrated the ability to form biofilms. The obtained results indicated a significant difference in gene expression levels. The fold change in expression for blaPER-1, bap, and pgaA was 7.473, 11.964, and 5.277, respectively. Conclusions: In our study, XDR A. baumannii primarily caused ventilator-associated pneumonia, and an observed increase in the expression of biofilm-related genes was noted in these strains. Healthcare centers should implement appropriate infection control programs to manage nosocomial infections, particularly in the ICU.

A recalibrated prediction model can identify level-1 trauma patients at risk of nosocomial pneumonia

IntroductionNosocomial pneumonia has poor prognosis in hospitalized trauma patients. Croce et al. published a model to predict post-traumatic ventilator-associated pneumonia, which achieved high discrimination and reasonable sensitivity. We aimed to externally validate Croce’s model to predict nosocomial pneumonia in patients admitted to a Dutch level-1 trauma center.Materials and methodsThis retrospective study included all trauma patients (≥ 16y) admitted for > 24 h to our level-1 trauma center in 2017. Exclusion criteria were pneumonia or antibiotic treatment upon hospital admission, treatment elsewhere > 24 h, or death < 48 h. Croce’s model used eight clinical variables—on trauma severity and treatment, available in the emergency department—to predict nosocomial pneumonia risk. The model’s predictive performance was assessed through discrimination and calibration before and after re-estimating the model’s coefficients. In sensitivity analysis, the model was updated using Ridge regression.Results809 Patients were included (median age 51y, 67% male, 97% blunt trauma), of whom 86 (11%) developed nosocomial pneumonia. Pneumonia patients were older, more severely injured, and underwent more emergent interventions. Croce’s model showed good discrimination (AUC 0.83, 95% CI 0.79–0.87), yet predicted probabilities were too low (mean predicted risk 6.4%), and calibration was suboptimal (calibration slope 0.63). After full model recalibration, discrimination (AUC 0.84, 95% CI 0.80–0.88) and calibration improved. Adding age to the model increased the AUC to 0.87 (95% CI 0.84–0.91). Prediction parameters were similar after the models were updated using Ridge regression.ConclusionThe externally validated and intercept-recalibrated models show good discrimination and have the potential to predict nosocomial pneumonia. At this time, clinicians could apply these models to identify high-risk patients, increase patient monitoring, and initiate preventative measures. Recalibration of Croce’s model improved the predictive performance (discrimination and calibration). The recalibrated model provides a further basis for nosocomial pneumonia prediction in level-1 trauma patients. Preventive oral hygiene and ventilator-associated pneumonia in paediatric intensive care unit.

Data indicate a tendency towards an increased incidence through the last decades of various forms of pneumonia. Among these, nosocomial pneumonia in patients who have been on mechanical ventilation support (increased after the Covid-19 pandemic) is a condition that must be addressed as soon as possible to avoid complications. Current literature lacks an in-depth analysis of the potential correlation between ventilator-associated pneumonia and poor oral hygiene, especially in children. The aim of this literature review is to investigate if improving oral hygiene could affect the onset of nosocomial pneumonia in children. A search was performed in Pubmed, Medline, and Scopus for the keywords: oral care, children, neonates, ventilator-associated pneumonia, combined with 'AND' or 'OR' Boolean Operators. The relevant papers retrieved in the scientific literature emphasised the importante of good oral care bundles to mitigate the bacteria proliferation in the bloodstream, and to prevent the development of ventilator-associated pneumonia.

Risk factors and prognosis of nosocomial pneumonia in patients undergoing extracorporeal membrane oxygenation: a retrospective study.

ObjectiveWe aimed to examine the risk factors and prognosis of nosocomial pneumonia (NP) during extracorporeal membrane oxygenation (ECMO).MethodsWe retrospectively analyzed data of patients who received ECMO at the Affiliated Hangzhou Hospital of Nanjing Medical University between January 2013 and August 2019. The primary outcome was the survival-to-discharge rate.ResultsSixty-nine patients who received ECMO were enrolled, median age 42 years and 26 (37.7%) women; 14 (20.3%) patients developed NP. The NP incidence was 24.7/1000 ECMO days. Patients with NP had a higher proportion receiving veno-venous (VV) ECMO (50% vs. 7.3%); longer ECMO support duration (276 vs. 140 hours), longer ventilator support duration before ECMO weaning (14.5 vs. 6 days), lower ECMO weaning success rate (50.0% vs. 81.8%), and lower survival-to-discharge rate (28.6% vs. 72.7%) than patients without NP. Multivariable analysis showed independent risk factors that predicted NP during ECMO were ventilator support duration before ECMO weaning (odds ratio [OR] = 1.288; 95% confidence interval [CI]: 1.111–1.494) and VV ECMO mode (OR = 10.970; 95% CI: 1.758–68.467).ConclusionNP during ECMO was associated with ventilator support duration before ECMO weaning and VV ECMO mode. Clinicians should shorten the respiratory support duration for patients undergoing ECMO to prevent NP.

Neumonía intrahospitalaria asociada a ventilación mecánica en un hospital del estado de Tabasco, México

Objective: To evaluate the cases of nosocomial pneumonia in patients with mechanical ventilation (NAVM), from the regional hospital of high specialty Gustavo Adolfo Rovirosa Pérez in the emergency area, from January 2016 to December 2017. Methods: A quantitative, observational, retrospective and transversal study was carried out. The variables of interest were taken from critical patients admitted to the emergency department, with mechanical ventilation requirements that developed pneumonia 48 hours after the start of ventilatory support. 97 clinical files were reviewed, 30 met the inclusion criteria, a format for the collection and elaboration of the database was elaborated, for the analysis of the variables, the statistical software Spss version 22 was used. Results: The distribution of VAP by gender was higher in men (62.9%) than in women (37.1%) and the ages ranged between 18 and 86 years. The main cause of admission diagnoses was severe TBI (71.4%), followed by cranial-facial trauma (20%) and moderate TBI with poor secretion management (8.6%). Of the total of analyzed files, 35 corresponded to NAVM, with an average evolution of 2 to 5 days and up to more than 20 days. 88.6% presented fever and all patients with leukocytosis with a minimum value of 15,000. On chest radiographs, infiltrates and condensation zones were observed in 48.6% of the cases; in the case of the affected hemithorax, the most frequent was the right (62.9%), followed by bilateral (22.9%) and left (14.3%) injuries. Conclusions: The development of VAP, is related to the time of exposure to orointubation. Clinical signs and radiographic changes are related to the suspicion of this type of pneumonia and practically in all cases, they presented admission diagnoses related to some type of trauma.

Early enteral nutrition in patients with severe traumatic brain injury: a propensity score–matched analysis using a nationwide inpatient database in Japan

Whether enteral nutrition (EN) should be administered early in severe traumatic brain injury (TBI) patients has not been fully addressed. The present study aimed to evaluate whether early EN can reduce mortality or nosocomial pneumonia among severe TBI patients. Using the Japanese Diagnosis Procedure Combination inpatient database from April 2014 to March 2017 linked with the Survey for Medical Institutions, we identified patients admitted for intracranial injury with Japan Coma Scale scores≥30 (corresponding to Glasgow Coma Scale scores≤8) at admission. We designated patients who started EN within 2 d of admission as the early EN group, and those who started EN at 3-5 d after admission as the delayed EN group. The primary outcome was in-hospital mortality. The secondary outcome was nosocomial pneumonia. Propensity score-matched analyses were performed to compare the outcomes between the 2 groups. We identified 3080 eligible patients during the 36-mo study period, comprising 1100 (36%) in the early EN group and 1980 (64%) in the delayed EN group. After propensity score matching, there was no significant difference in in-hospital mortality (difference: -0.3%; 95% CI: -3.7%, 3.1%) between the 2 groups. The proportion of nosocomial pneumonia was significantly lower in the early EN group than in the delayed EN group (difference: -3.2%; 95% CI: -5.9%, -0.4%). Early EN may not reduce mortality, but may reduce nosocomial pneumonia in patients with severe TBI.

Frequency of Ventilator Associated Pneumonias in Patients in the Intensive Care Unit.

Introduction:Ventilator associated pneumonia (VAP) is defined as nosocomial pneumonia in patients who have mechanical ventilation (MV) for more than 48 hours. The diagnosis of VAP is based on radiological-microbiological examinations. In the United States, the Centers for Disease Control and Prevention (CDC) and the National Health Care Network (NHSN) have an incidence of VAP of 5.8% per 1,000 days on mechanical ventilator.Aim:In this study, we had an aim to determine the occurrence of ventilator-associated pneumonia (VAP) in patients with MV who were hospitalized in the intensive care unit.Method:The study was retrospective, clinical, conducted in the period from January 1, 2016 until December 31, 2016. In a one-year period, 719 patients of both sex, aged 14 to 91, were hospitalized in the intensive care unit of the Clinic for Anesthesia and Resuscitation of the University Clinical Center in Sarajevo. The study included 250 patients of both sex who had respiratory support with mechanical ventilator. No patient was excluded from the study. As a confirmation of VAP, we used microbiological reports from the patient history documentation. The results were presented statistically through tables and graphs, numerically, by a percentage, and by a mean value with standard deviation.Results:Out of the 719 hospitalized patients, 250 or 34.8% underwent controlled ventilation. In 103 or 41.2% of patients some form of pneumonia was confirmed microbiologically. An average patient age on controlled ventilation was 60.4 ± 16.8 years. The mean age of a female patients who were on controlled ventilation was 63.2 ± 16.7, higher than that of male patients, which was 57.8 ± 16.6 years. The most frequent patients were over 60 years of age (52.8%). The shortest hospitalization of patients on controlled mechanical ventilation was 1 day and the longest was 120 days. Average duration of mechanical ventilation was 6.9 ± 10.5 days.Conclusion:VAP is a relatively common complication in patients with MV that can increase morbidity and mortality, as well as treatment costs. It is more frequent in females and in the elderly. Medical staff should provide normal maintenance of respiratory functions to a patient who is on MV, which will reduce the risk of VAP.

Extremely High-frequency Therapy in the Complex Treatment of Pneumonia in Patients with Burn and Inhalation Trauma

Background. The improvement of treatment tactics for nosocomial pneumonia in patients with burn and inhalation trauma is a relevant issue due to the frequency of this complication, which develops at any stage of treatment and aggravates the course of the underlying disease.Aim of study. To evaluate the effectiveness of extremely high frequency therapy (EHFT) in the complex treatment of pneumonia in patients with burn and inhalation injury.Material and methods. The results of extremely high-frequency therapy (EHFT) was compared, studying 23 burn patients with complex EHFT and 30 patients without EHFT.Results. EHFT reduces the signs of endogenous toxemia, as evidenced by a statistically significant decrease in elevated concentrations of dead leukocytes in venous blood from 147-106/L (98; 363-106/L) to 81.8-106/L (72; 150-106/L) (p=0.041, Wilcoxon test) on day 6-8 (after 5-6 procedures), whereas in the comparison group, on the contrary, an increase in the dead cells from 121-106/L (66; 210-106/L) up to 137.4-106/L (116; 207-106/L) was observed. When comparing this indicator in two groups on day 6-8 from the onset of pneumonia, a statistically significant difference was found (p=0.021, Mann-Whitney). The term of managing pneumonia in patients receiving EHFT was significantly shorter than in the comparison group, 18 (13; 23) versus 21 (18; 27) days (p=0.020, Mann-Whitney).Conclusion. EHFT reduced intensity of endogenous toxicosis and inflammation, approaching pneumonia relief for 3 days.

Subdistribution hazard models for competing risks in discrete time.

A popular modeling approach for competing risks analysis in longitudinal studies is the proportional subdistribution hazards model by Fine and Gray (1999. A proportional hazards model for the subdistribution of a competing risk. Journal of the American Statistical Association94, 496-509). This model is widely used for the analysis of continuous event times in clinical and epidemiological studies. However, it does not apply when event times are measured on a discrete time scale, which is a likely scenario when events occur between pairs of consecutive points in time (e.g., between two follow-up visits of an epidemiological study) and when the exact lengths of the continuous time spans are not known. To adapt the Fine and Gray approach to this situation, we propose a technique for modeling subdistribution hazards in discrete time. Our method, which results in consistent and asymptotically normal estimators of the model parameters, is based on a weighted ML estimation scheme for binary regression. We illustrate the modeling approach by an analysis of nosocomial pneumonia in patients treated in hospitals.

Nosocomial Pneumonia in Patients Admitted to the Intensive Care Unit of a Tertiary Care Center in Mashhad, Northeast of Iran; an Etiologic Survey

Background: Hospital-acquired pneumonia (HAP) prevalence is related to patients’ population and diagnostic methods. HAP incidence is reported in different studies from 9% to 46%. The current study aimed at evaluating the etiology of HAP in patients admitted to intensive care units (ICUs). Methods: The patients admitted to ICU of Imam Reza hospital (a tertiary care and teaching hospital in Mashhad, Northeast of Iran) were evaluated for HAP based on the following criteria: fever, leukocytosis, purulent discharge, new radiologic findings, changes in O2 saturation, and ventilator setting. Blood and endotracheal aspirates cultures were performed for all patients. Demographic characteristics were recorded in a checklist. Results: Among the 88 adult patients enrolled in the current study, the most frequent radiologic finding was patchy infiltration in chest X-ray (71.6%). Mean age of the patients was 58 ± 20.1 years; mean hospital stay was 63 ± 40.8 days; mean interval between hospitalization and pneumonia development was 20.9 ± 16.8 days; and 41 patients (46.6%) died. Acinetobacter spp. were the most frequent microorganisms in purulent discharge and methicillin-resistant Staphylococcus aureus (MRSA) were the main bacteria isolated from blood culture. Conclusions: Early diagnosis and appropriate antibiotic therapy can decrease HAP mortality and morbidity. The current study findings revealed that Acinetobacter app. were the most frequent cause of HAP in ICU patients in the studied center, which should be considered at the time of diagnosis and empirical antibiotics administration. Appropriate infection control and preventive measures should also be taken in ICUs to prevent HAP, especially against those caused by Acinetobacter spp.

Pseudomonas aeruginosa PcrV and Psl, the Molecular Targets of Bispecific Antibody MEDI3902, Are Conserved Among Diverse Global Clinical Isolates

Bispecific antibody MEDI3902, targeting the Pseudomonas aeruginosa type 3 secretion system (PcrV) and Psl exopolysaccharide, is currently in phase 2b development for prevention of nosocomial pneumonia in patients undergoing mechanical ventilation. We surveyed a diverse collection of isolates to study MEDI3902 epitope conservation and protective activity. P. aeruginosa clinical isolates (n = 913) were collected from diverse patients and geographic locations during 2003-2014. We conducted whole-genome sequencing; performed PcrV and Psl expression analyses via immunoblotting and enzyme-linked immunosorbent assay, respectively; performed crystallography to determine the MEDI3902 PcrV epitope, using anti-PcrV Fab and PcrV components (resolved at 2.8 Å); and evaluated MEDI3902 protective activity against select isolates in vitro and in vivo. Intact psl operon and pcrV genes were present in 94% and 99% of isolates, respectively, and 99.9% of isolates contained at least one of the genetic elements. Anti-Psl binding was confirmed in tested isolates harboring a complete Psl operon or lacking nonessential psl genes. We identified 46 PcrV variant sequences, and MEDI3902-PcrV contact residues were preserved. MEDI3902 maintained potent in vivo activity against various strains, including strains expressing only a single target. Psl and PcrV are highly prevalent in global clinical isolates, suggesting MEDI3902 can mediate broad coverage against P. aeruginosa.

Characteristics and factors associated with nosocomial pneumonia among patients undergoing continuous renal replacement therapy (CRRT): A case–control study

ObjectivesContinuous renal replacement therapy (CRRT) is a specialized type of dialysis. However, the characteristics and factors associated with nosocomial pneumonia in patients undergoing CRRT have received little attention to date. Therefore, this study investigated the characteristics of and factors contributing to nosocomial pneumonia in patients receiving CRRT. MethodsThe clinical data of 1160 patients undergoing CRRT during the period January 2008 to December 2015 were analyzed retrospectively. Of these 1160 cases, 145 (12.5%) were included in the nosocomial pneumonia group, while 1015 were included in the control group. ResultsThe primary pathogen in the 145 cases of nosocomial pneumonia in the CRRT patients was Staphylococcus aureus (58.57%); the morbidity rate was 12.5%. Multivariate logistic regression analysis revealed that age (odds ratio (OR) 2.209), initial curative time (OR 1.960), underlying diseases (OR 1.820), consciousness disorder (OR 1.616), organ failure (OR 2.154), the Acute Physiology and Chronic Health Evaluation II score (APACHE II) (OR 1.186), and the Charlson Comorbidity Index score (CCI) (OR 1.278) were risk factors for nosocomial pneumonia (all p<0.05). Conversely, the serum white blood cell count (OR 0.585), albumin (OR 0.673), and hemoglobin (OR 0.712) levels were protective factors (all p<0.05). ConclusionsResults from this study indicate that by modifying risk factors, such as providing adequate nutrition, earlier treatment of underlying diseases, and controlling organ failure, the risks associated with nosocomial pneumonia may be reduced.

Th17 cells are associated with protection from ventilator associated pneumonia.

BackgroundCD4+ T-helper 17 (Th17) cells and Interleukin (IL)-17A play an important role in clearing pathogens in mouse models of pneumonia. We hypothesized that numbers of Th17 cells and levels of IL-17A are associated with risk for nosocomial pneumonia in humans.MethodsWe collected bronchoalveolar lavage (BAL) fluid from mechanically ventilated (n = 25) patients undergoing quantitative bacterial culture to evaluate for ventilator associated pneumonia (VAP). We identified Th17 cells by positive selection of CD4+ cells, stimulation with ionomycin and PMA, then staining for CD4, CD45, CCR6, IL-17A, and IFN-γ followed by flow cytometric analysis (n = 21). We measured inflammatory cytokine levels, including IL-17A, in BAL fluid by immunoassay.ResultsVAP was detected in 13 of the 25 subjects. We identified a decreased percentage of IL-17A producing Th17 cells in BAL fluid from patients with VAP compared to those without (p = 0.02). However, we found no significant difference in levels of IL-17A in patients with VAP compared to those without (p = 0.07). Interestingly, IL-17A levels did not correlate with Th17 cell numbers. IL-17A levels did show strong positive correlations with alveolar neutrophil numbers and total protein levels.ConclusionsTh17 cells are found at lower percentages in BAL fluid from mechanically ventilated patients with VAP and IL-17A levels correlated with Th17 cell percentages in non-VAP subjects, but not those with VAP. These findings suggest that Th17 cells may be protective against development of nosocomial pneumonia in patients receiving mechanical ventilation and that alveolar IL-17A in VAP may be derived from sources other than alveolar Th17 cells.

Metoclopramide for preventing nosocomial pneumonia in patients fed via nasogastric tubes: a systematic review and meta-analysis of randomized controlled trials.

Metoclopramide, a prokinetic agent, has been recommended to reduce incidence of pneumonia, but its efficacy is controversial. Thus, this systematic review aimed to evaluate the effectiveness of metoclopramide for pneumonia in patients fed via nasogastric tube. Cochrane Central Register of Controlled Trials, PubMed, EMBASE, and OVID were searched from their inception to March 31th 2015. Randomized controlled trials (RCTs) of metoclopramide against placebo in patients fed via nasogastric tube were identified. The Cochrane risk of bias assessment tool was used for quality assessment. Four trials involving 694 patients fed via nasogastric tube were identified. Compared with placebo, metoclopramide showed no significant effects in reducing pneumonia (n=694; risk ratio [RR]: 0.79; 95% CI: 0.45 to 1.38, p=0.40) or mortality (n=694; RR: 0.93; 95% CI: 0.78 to 1.11, p=0.44). In two trials using continuous data, metoclopramide significantly delayed the development of nosocomial pneumonia (n=80; weighted mean difference [WMD]: 1.74 days; 95% CI: 1.03 to 2.46 days, p<0.00001). However, in two other trials using dichotomous data, metoclopramide increased the proportion of cases showing early-onset nosocomial pneumonia (n=103; RR: 1.32; 95% CI: 1.10 to 1.58, p=0.003). Adverse effects monitoring was reported in one included trial, No significant adverse reactions were noted in this study. Because of the poor methodological quality and high risk of bias in the included studies, this systematic review revealed no definite conclusion about the application of metoclopramide for the reduction of nosocomial pneumonia. Therefore, more high-quality studies with larger sample sizes are required.

Efficacy of Blind Tracheal Aspirate in Comparison to Bronchoalveolar Lavage for Microbiological Diagnosis of Nosocomial Pneumonia in Patients on Ventilator

Background and Objectives: For diagnosis of nosocomial pneumonia in patients on ventilator, invasive procedure like bronchoscopy for microscopy and quantitative cultures of lower respiratory tract samples is useful but not always possible for potential risk of the procedure and the associated cost. The non-bronchoscopic sampling of the lower airways and quantitative cultures of tracheal aspirate may offer simple and readily available alternative to bronchoscopy with promising results. This study was done to evaluate the efficacy of blind tracheal aspirate in the microbiological diagnosis of nosocomial pneumonia occurring in intubated patients on mechanical ventilator.&#x0D; Materials &amp; Methods: This cross-sectional study was carried out in the Intensive Care Unit in the Department of Critical Care Medicine, BIRDEM Hospital, Dhaka over a period 16 months starting from January 2010 to April 2011. A total of 54 clinically diagnosed cases of nosocomial (hospital acquired) pneumonia who were on ventilator were consecutively included in the study based on predefined enrolment criteria. All the 54 cases were subjected to blind endotracheal aspirate (BTA) followed by bronchoalveolar lavage (BAL) for quantitative cultures of specimens and isolation of causative microorganisms from them.&#x0D; Result: The present study showed that the mean age of the patients was 61 years (range: 24-86 years). Males were predominant in the series with male to female ratio being 7:3. Majority of the patients was haemodynamically stable as indicated by mean blood pressures, heart rate, temperature and respiratory rate. Most (83.3%) of the cases showed significant growth of microbes on culture of blind tracheal aspirates at cut-off value of 105 colony forming unit/ml (cfu/ml), while 87% of the cases exhibited positive growth on culture of bronchoalveolar lavage at cut-off value of 104 cfu/ml. Acinetobacter baumannii was the predominant organism isolated from BTA (73.3%) followed by Pseudomonas aeruginosa (33.3%). An almost similar pattern of growth was evident in BAL with more than 70% being Acinetobacter baumannii and about 30% Pseudomonas aeruginosa. C. albicans. Kiebsiella sp., E. coli, and Flavobacter were less commonly observed in either group. The Kappa test revealed a good agreement (70.7%) between the two procedures suggesting that the two diagnostic modalities are almost comparable in diagnosing pneumonia in patients admitted in ICU (p &lt; 0.001).&#x0D; Conclusion: The study concluded that the accuracy of blind tracheal aspirate and bronchalveolar lavage in the diagnosis of nosocomial pneumonia was fairly comparable. Risk factors for nosocomial pneumonia in patients with abdominal surgery

Abstract Background The risk of post-operative pneumonia is a latent complication. A study was conducted to determine its risk factors in abdominal surgery. Material and methods A cross-sectional study was performed that included analysing the variables of age and gender, chronic obstructive pulmonary disease and smoking, serum albumin, type of surgery and anaesthesia, emergency or elective surgery, incision site, duration of surgery, length of hospital stay, length of stay in the intensive care unit, and time on mechanical ventilation. The adjusted odds ratio for risk factors was obtained using multivariate logistic regression. Results The study included 91 (9.6%) patients with pneumonia and 851 (90.4%) without pneumonia. Age 60 years or over (OR = 2.34), smoking (OR = 9.48), chronic obstructive pulmonary disease (OR = 3.52), emergency surgery (OR = 2.48), general anaesthesia (OR = 3.18), surgical time 120 min or over (OR = 5.79), time in intensive care unit 7 days or over (OR = 1.23), time on mechanical ventilation greater than or equal to 4 days (OR = 5.93) and length of post-operative hospital stay of 15 days or over (OR = 1.20), were observed as independent predictors for the development of postoperative pneumonia. Conclusions Identifying risk factors for post-operative pneumonia may prevent their occurrence. The length in the intensive care unit of greater than or equal to 7 days (OR = 1.23; 95% CI 1.07–1.42) and a length postoperative hospital stay of 15 days or more (OR = 1.20; 95% CI 1.07–1.34) were the predictive factors most strongly associated with lung infection in this study.

Incidence, risk factors and outcome of nosocomial pneumonia in patients with central nervous system infections

Pneumonia is the most frequent nosocomial infection in intensive care units. The reported frequency varies with definition, the type of hospital or intensive care units and the population of patients. The incidence ranges from 6.8-27%. The objective of this study was to determine the frequency, risk factors and mortality of nosocomial pneumonia in intensive care patients. We analyzed retrospectively and prospectively the collected data of 180 patients with central nervous system infections who needed to stay in the intensive care unit for more than 48 hours. This study was conducted from 2003 to 2009 at the Clinical Centre of Kragujevac. During the study period, 54 (30%) patients developed nosocomial pneumonia. The time to develop pneumonia was 10 +/-6 days. We found that the following risk factors for the development of nosocomial pneumonia were statistically significant: age, Glasgow Coma Scale (GCS) score < 9, mechanical ventilation, duration of mechanical ventilation, tracheostomy, presence of nasogastric tube and enteral feeding. The most commonly isolated pathogens were Klebsiella-Enterobacter spp. (33.3%), Pseudomonas aeruginosa (24.1%), Acinetobacter spp. (16.6%) and Staphylococcus aureus (25.9%). Nosocomial pneumonia is the major cause of morbidity and mortality of patients with central nervous system infections. Patients on mechanical ventilation are particularly at a high risk. The mortality rate of patients with nosocomial pneumonia was 54.4% and it was five times higher than in patients without pneumonia.

Effect of metoclopramide on nosocomial pneumonia in patients with nasogastric feeding in the intensive care unit

This study in the Islamic Republic of Iran aimed to determine whether metoclopramide can prevent nosocomial pneumonia in the intensive care unit (ICU). Of 220 patients admitted to the surgical ICU who had a nasogastric tube for more than 24 hours, 68 case patients received oral metoclopramide (10 mg every 8 hours) and 152 control patients did not. Similar proportions of cases and controls developed nosocomial pneumonia (33.8% versus 33.6%). Endotracheal intubation was a risk factor for nosocomial pneumonia (odds ratio 7.70). There were no significant differences between groups in mortality rate or time of onset of nosocomial pneumonia. Metoclopramide appears to have no effect on the development of nosocomial pneumonia with nasogastric feeding.

Spectrum of practice in the diagnosis of nosocomial pneumonia in patients requiring mechanical ventilation in European intensive care units

Information on clinical practice regarding the diagnosis of pneumonia in European intensive care units is limited. The aim of this study was to describe the spectrum of actual diagnostic practices in a large sample of European intensive care units. Prospective, observational, multicenter study. Twenty-seven intensive care units of nine European countries. Consecutive patients requiring invasive mechanical ventilation for an admission diagnosis of pneumonia or receiving mechanical ventilation for >48 hrs irrespective of admission diagnosis. None. A total of 2,436 patients were evaluated; 827 were admitted with or developed nosocomial pneumonia (hospital-acquired pneumonia [HAP], 27.1%; ventilator-associated pneumonia [VAP], 56.2%; very early onset VAP, 16.7%). Mean age was 59.4 +/- 18.1 yrs, 65.0% were men, and mean admission Simplified Acute Physiology Score II was 46.7 +/- 17.1. Worsening oxygenation (76.8%), purulent/changing respiratory secretions (72.1%), and new temperature elevation (69.2%) were the most frequent clinical signs of nosocomial pneumonia. Etiological diagnosis was based on noninvasive respiratory specimens in 74.8% of episodes. Bronchoscopy was performed in 23.3% of episodes. Bronchoscopy performance, after adjustment by severity of illness, age, and type of hospital, were predicted by worsening oxygenation (odds ratio 2.03; 95% confidence interval, 1.27-3.24) and male sex (odds ratio 1.77; 95% confidence interval, 1.19-2.65). Definite cause was documented in 69.5% of nosocomial pneumonia cases. The most common isolates were Staphylococcus aureus (16.3% methicillin-sensitive S. aureus and 16.0% methicillin-resistant S. aureus), Pseudomonas aeruginosa (23.1%), and Acinetobacter baumannii (19.1%). Presence of nosocomial pneumonia significantly prolonged mean length of mechanical ventilation (10.3 days, p < .05) and mean intensive care unit length of stay (12.2 days, p < .05) in intensive care unit survivors. Mortality rate was 37.7% for nosocomial pneumonia vs. 31.6% for patients without pneumonia (p < .05). Etiological diagnosis of nosocomial pneumonia in a large sample of European intensive care units was based mainly on noninvasive techniques. However, there was high variability in bronchoscopy use between the participating intensive care units.
