The Moment Criterion of Anthropomorphicity of Prosthetic Feet as a Potential Predictor of Their Functionality for Transtibial Amputees.

The purpose of this paper is to discuss a new quantitative mechanical parameter of prosthetic feet called the Index of Anthropomorphicity (IA), which has the potential to be adopted as an objective predictor of their functionality. The objectives are to present the research findings supporting the introduction of IA and unify previous results into a coherent theory. The IA is founded on the moment criterion of the anthropomorphicity of prosthetic feet. The term "anthropomorphicity" is defined for this application. Studies with a small number of human subjects and prostheses have shown that the value of the parameter is positively correlated with patient comfort and with the restoration of certain normal gait characteristics. Confirmatory studies with controlled human trials and mechanical tests with a wider selection of prosthesis types can give prosthesis manufacturers a new criterion to follow in the design process, and prosthetists may use the IA for selecting more suitable prostheses for a patient's comfort and health.

Design and Preliminary Testing of a Novel Variable-Damping Prosthetic Knee

This article proposes a variable-damping prosthetic knee (ReKnee) with a novel hydraulic damper, and the intelligent control system is proposed. The LSTM network is proposed to identify the locomotion pattern under five road conditions. Predictive control is first applied to the hydraulic damping control for prosthetic knee. Prototype experiments are carried out to verify the performance of the control system. The experiment results show that LSTM possesses better accuracy and robustness than the traditional RNN network and other shallow machine learning algorithms, and NNPC damping control improves the gait symmetry compared to fuzzy logic control with decreasing in the range of 5.75–19.27% of the SI values when walking speed varies. It is demonstrated that the ReKnee can make a good performance on enhancing the accuracy of angle tracking and the approximation of normal gait characteristics.

Bi-unicondylar arthroplasty : a biomechanics and clinical outcomes study.

AimsBi-unicondylar arthroplasty (Bi-UKA) is a bone and anterior cruciate ligament (ACL)-preserving alternative to total knee arthroplasty (TKA) when the patellofemoral joint is preserved. The aim of this study is to investigate the clinical outcomes and biomechanics of Bi-UKA.MethodsBi-UKA subjects (n = 22) were measured on an instrumented treadmill, using standard gait metrics, at top walking speeds. Age-, sex-, and BMI-matched healthy (n = 24) and primary TKA (n = 22) subjects formed control groups. TKA subjects with preoperative patellofemoral or tricompartmental arthritis or ACL dysfunction were excluded. The Oxford Knee Score (OKS) and EuroQol five-dimension questionnaire (EQ-5D) were compared. Bi-UKA, then TKA, were performed on eight fresh frozen cadaveric knees, to investigate knee extensor efficiency under controlled laboratory conditions, using a repeated measures study design.ResultsBi-UKA walked 20% faster than TKA (Bi-UKA mean top walking speed 6.7 km/h (SD 0.9),TKA 5.6 km/h (SD 0.7), p < 0.001), exhibiting nearer-normal vertical ground reaction forces in maximum weight acceptance and mid-stance, with longer step and stride lengths compared to TKA (p < 0.048). Bi-UKA subjects reported higher OKS (p = 0.004) and EQ-5D (p < 0.001). In vitro, Bi-UKA generated the same extensor moment as native knees at low flexion angles, while reduced extensor moment was measured following TKA (p < 0.003). Conversely, at higher flexion angles, the extensor moment of TKA was normal. Over the full range, the extensor mechanism was more efficient following Bi-UKA than TKA (p < 0.028).ConclusionBi-UKA had more normal gait characteristics and improved patient-reported outcomes, compared to matched TKA subjects. Maximum permissible load for Yonaguni ponies (Japanese landrace horses) trotting over a short, straight course

AbstractThis study aimed to determine the load-bearing capacity of trotting Yonaguni ponies using gait analysis. The Yonaguni pony is one of the Japanese landrace horses, and has normal gait characteristics when trotting. As a breed they are small in stature and hence susceptible to the effect of the rider's weight. It is therefore important to determine their load-bearing capacity as regards to their welfare. Ten Yonaguni ponies with a (mean ± SD) height at withers of 122 (± 2.9) cm had a marker attached to their chests, and their unridden gait was recorded using two high-resolution hybrid cameras while they trotted along a short (40 m), straight course. In total, nine tests were performed for each horse: the first with a 0-kg load; seven with randomly loaded weights weighing 10-70 kg; and then a final test again with a 0-kg load. Three-dimensional movement of the marker was analysed using a motion capture system. The time series of vertical displacement of the marker underwent spectrum analysis, and the autocorrelation coefficient was calculated. The first two peaks of the autocorrelation were defined as symmetry and gait regularity, and their sum was defined as stability. Symmetry (no unit) in the 70-kg test (0.53) was lower than that in the first 0-kg test (0.68), and stability (no unit) in the 70-kg test (1.16) was lower than that in the first 0-kg test (1.41). We concluded that the maximum permissible load for a trotting Yonaguni pony is &lt; 70 kg, which represents 33% of its bodyweight. To promote welfare, it is important to determine the load-bearing capacity for individual types of horse.

The way we walk determines the energetic investment needed. Humans spontaneously alter their walking style to exploit energetic opportunities. New research demonstrates the sensitivity and timing of this optimization and opens the door to discovering the underlying mechanisms.

Gait kinematics of people with Multiple Sclerosis and the acute application of Functional Electrical Stimulation

This study aimed to (i) compare the gait characteristics of people with Multiple Sclerosis (pwMS) to those of healthy controls walking at the same average speed, and (ii) assess the effects of the acute application of Functional Electrical Stimulation (FES) to the dorsiflexors.Twenty-two people with pwMS (mean age 49 years), prescribed FES, and 11 age matched healthy controls participated. Three dimensional gait kinematics were assessed whilst (i) pwMS and healthy controls walked at self-selected speeds (SSWS), (ii) healthy controls also walked at the average walking speed of the pwMS group, and (iii) people with MS walked using FES.Compared to healthy controls walking at their SSWS, pwMS walked slower and showed differences in nearly all gait characteristics (p<0.001). Compared to healthy controls walking at the same average speed, pwMS still exhibited significantly shorter stride length (p=0.007), reduced dorsiflexion at initial contact (p=0.002), reduced plantar flexion at terminal stance (p=0.008) and reduced knee flexion in swing (p=0.002). However, no significant differences were seen between groups in double support duration (p=0.617), or hip range of motion (p=0.291). Acute application of FES resulted in a shift towards more normal gait characteristics, except for plantar flexion at terminal stance which decreased.In conclusion, compared to healthy controls, pwMS exhibit impairment of several characteristics that appear to be independent of the slower walking speed of pwMS. The acute application of FES improved most impaired gait kinematics. A speed matched control group is warranted in future studies of gait kinematics of pwMS.

Waterloo Vision and Mobility Study: Normal gait characteristics during dark and light adaptation in individuals with age-related maculopathy

The purpose of this study was to evaluate the gait responses of individuals with low vision compared to those of normal-visioned individuals when their vision is challenged by extreme levels of light. Twenty subjects with age-related maculopathy (ARM) and 20 subjects with normal vision first walked along a flat, unobstructed path immediately after the ambient light level was changed from low (5 lux) to high (2500 lux). The procedure was repeated after the light was reduced from the high to the low level. Muscle activity, temporal and kinematic variables, and ground reaction forces were used to detect gait characteristics because of ambient light level changes. Data suggested that ARM subjects walked slower and with more caution than normal subjects but that these differences were not related to ambient light level. Head angle, an estimate of gaze direction, was lower for ARM subjects during high light, but the gaze direction for both groups was low during low light. Among these ARM subjects, extreme levels of ambient light did not affect gait; subjects made adaptations that were reasonable to encourage safe ambulation, despite the direction of light change. Normal-visioned individuals in this study experienced more difficulty in low light than high light situations.

Normal gait characteristics under temporal and distance constraints

Walking patterns of 53 males and 39 females, all in good health, were studied at slow, free, and fast speeds using a walkway system developed by the author. Three males and three females, also in good health, were then studied under constrained walking conditions such as rhythm constraint, speed coupled with constraint, walking up or down a slope, line stepping contraint, stepping onto a marked square, and starting/stopping of walking. In the first set of experiments, the following results were obtained. When increasing speed, the male had a tendency to increase step length and the female had a tendency to increase cadence. The relationships between the speed and the statistics of gait parameters, i.e. the coefficient of variation and the symmetry were examined. The data in this experiment were also applied to Grieve's gait equations which formulated the relationships between step frequency and speed, or between swing time and cycle time. In the second set of experiments the following results were obtained. Although rhythm constraint (induced by a metronome) resulted in no difference of gait between males and females, a difference did appear in the case of speed coupled with constraint. When walking up and down a slope, the ascent case showed a longer step length and a lower cadence compared with the descent. The idea of functional asymmetry, a supporting function of the left leg and a moving function of the right leg, is well accepted. However, in this study of the effect of line stepping constraints predominant right-left functional differences were found. The perturbation of gait when the subjects stepped onto a marked square resembling a force-plate was recorded quantitatively. With regard to the starting and stopping characteristics of walking, it was concluded that the two steps from starting and the three steps before stopping should be excluded from ordinary data due to their acceleration and deceleration properties.

Assessing the reliability of measurements from the Krusen limb load monitor to analyze temporal and loading characteristics of normal gait.

This study assessed the reliability of measurements made by four physical therapists on healthy subject gait data recorded from the Krusen limb load monitor. The five components of step (stance time, time up, time to second peak, and force at the first and second peaks) were analyzed. Six components contributing to gait (ambulation time; velocity; cadence; average swing phase duration, left lower extremity; average swing phase duration, right lower extremity; and ratio of unilateral weight bearing, right lower extremity to left lower extremity) were also analyzed. Intraclass correlation coefficients for the five step components and the gait measures of ambulation time, velocity, and cadence showed high measurement reliability. The other measures of gait showed low intraclass correlation coefficients. The limb load monitor can, therefore, be used by clinicians to measure the five step components and three gait measures (ambulation time, velocity, and cadence) with high measurement reliability.
