Continuous non-invasive vs. invasive arterial blood pressure monitoring during neuroradiological procedure: a comparative, prospective, monocentric, observational study

BackgroundIn the perioperative setting, the most accurate way to continuously measure arterial blood pressure (ABP) is using an arterial catheter. Surrogate methods such as finger cuff have been developed to allow non-invasive measurements and are increasingly used, but need further evaluation. The aim of this study is to evaluate the accuracy and clinical concordance between two devices for the measurement of ABP during neuroradiological procedure.MethodsThis is a prospective, monocentric, observational study. All consecutive patients undergoing a neuroradiological procedure were eligible. Patients who needed arterial catheter for blood pressure measurement were included. During neuroradiological procedure, ABP (systolic, mean and diatolic blood pressure) was measured with two different technologies: radial artery catheter and Nexfin. Bland-Altman and error grid analyses were performed to evaluate the accuracy and clinical concordance between devices.ResultsFrom March 2022 to November 2022, we included 50 patients, mostly ASA 3 (60%) and required a cerebral embolization (94%) under general anaesthesia (96%). Error grid analysis showed that 99% of non-invasive ABP measures obtained with the Nexfin were located in the risk zone A or B. However, 65.7% of hypertension events and 41% of hypotensive events were respectively not detected by Nexfin. Compared to the artery catheter, a significant relationship was found for SAP (r2 = 0.78) and MAP (r2 = 0.80) with the Nexfin (p < 0.001). Bias and limits of agreement (LOA) were respectively 9.6 mmHg (− 15.6 to 34.8 mmHg) and − 0.8 mmHg (− 17.2 to 15.6 mmHg), for SAP and MAP.ConclusionsNexfin is not strictly interchangeable with artery catheter for ABP measuring. Further studies are needed to define its clinical use during neuroradiological procedure.Trial registrationClinicaltrials.gov, registration number: NCT05283824.

Continuous non-invasive arterial blood pressure monitoring with photoplethysmography via SE-MSResUNet network

ObjectiveArterial blood pressure (ABP) waveforms include physiological parameters closely related to BP values, while only a few studies predict BP values through the monitoring ABP waveforms. Current ABP waveforms monitoring methods are limited by its high computing cost and information redundancy of multi-channel input signals. The aim of our work is to propose a new end-to-end method for monitoring the ABP waveforms using single-channel photoplethysmography (PPG) signals by constructing a lightweight Multi-Scale Residual U-Net fused with Squeeze-and-Excitation module (SE-MSResUNet) model. MethodsFirstly, the PPG and ABP signals are preprocessed with denoising, segmentation, and eliminating phase difference. Subsequently, the SE-MSResUNet model is constructed and trained using training data to achieve the mapping relationship between PPG and ABP signals. Then, the BP values are extracted from the monitored ABP waveforms. Finally, the performance of the SE-MSResUNet in ABP waveform monitoring and BP estimation is investigated. ResultsThe ABP waveforms monitored by our proposed SE-MSResUNet network with PPG signals only are highly consistent with the reference ABP waveforms. The MAE ± SD of estimated diastolic blood pressure (DBP), mean arterial pressure (MAP) and systolic blood pressure (SBP) compared with reference values are 2.16 ± 3.75, 2.29 ± 3.72, and 3.88 ± 6.17 mmHg, respectively. The proposed method meets the Association for the Advancement of Medical Instrumentation (AAMI) standard and reach A level of the British Hypertension Society (BHS) standard. ConclusionsOur proposed method outperforms current state-of-the-art methods for non-invasive blood pressure monitoring from PPG signals. The algorithm provides a potential possibility for continuous non-invasive blood pressure monitoring through portable wearable devices.

Agreement of continuous non-invasive arterial blood pressure monitoring (CNAP) and pulmonary artery catheter thermodilution method in estimating the hemodynamic parameters of post CABG patients

Abstract Background The use of hemodynamic parameters in monitoring patients after coronary artery bypass surgery is an essential element of clinical practice. It helps intensivist and physicians to properly manage patients after surgery where hemodynamic parameters are usually dynamic and complicated. Pulmonary artery guided thermodilution (PACT) has been the gold standard in assessing the adequacy of cardiac output, however, its invasive nature is also associated with more complex procedures and the risk of complications, including infections. With the emergence of new non-invasive hemodynamic monitoring devices such as the CNAP (continuous non-invasive arterial pressure), alternative methods for hemodynamic monitoring is now possible. Compared to invasive methods, it is easier to use with an error-free application, safer, less risk of infection and more cost effective. Purpose To determine the agreement of Continuous non-invasive arterial pressure (CNAP) and pulmonary artery guided thermodilution in monitoring the hemodynamic parameters of patients who underwent coronary artery bypass graft surgery. Methods This is a cross-sectional study conducted at the cardiovascular surgical intensive care unit and included 63 patients with Coronary artery disease who underwent elective coronary artery bypass graft surgery. Cardiac output, Cardiac Index, Stroke volume and Systemic vascular resistance were simultaneously obtained using both the CNAP and PACT. Three consecutive sets of measurements were recorded for each patient with a 3 hour interval; during arrival at the ICU, 3 hours post op, and 6 hours post op. Results The mean difference between PACT-CO and CNAP-CO was -0.81 L/min (SD -1.18 to -0.44) with a 95% limits of agreement of -3.74 to 2.11 L/min on the 0 hour, -0.81L/min (SD -1.22 to -0.41) with a 95% limits of agreement of -1.98 to 1.18 L/min on the 3rd hour, and -0.77L/min (SD -1.08 to -0.46) with a 95% limits of agreement of -3.22 to 1.68 L/min on the 6th hour post op. The results showed a significant difference between the PACT and CNAP. However, a positive correlation was noted between PACT-CO and CNAP-CO, the correlation coefficient on the 0, 3rd and 6th hour were 0.5384, 0.5158, and 0.6265 respectively. Conclusion CNAP derived hemodynamic parameters had a high mean difference compared to the PACT and there was no agreement between the two methods. However, it was able to have a positive correlation with the PACT, making it possible for the CNAP to track the hemodynamic changes during the post operative course.

Comparison of two non-invasive arterial blood pressure monitoring techniques in brown bears (Ursus arctos)

Monitoring arterial blood pressure (BP), represents a more accurate evaluation of hemodynamics than heart rate alone and is essential for preventing and treating intra- and post-operative complications in wildlife chemical immobilization.The objectives of the study were to test the correlation between standard oscillometry and Korotkoff's technique in anesthetized free-ranging brown bears in Croatia and Scandinavia and to assess the blood pressure in both locations.Five bears were snared and darted with xylazine and ketamine in Croatia, and 20 bears were darted from a helicopter with medetomidine and tiletamine-zolazepam in Scandinavia. Blood pressure was simultaneously measured with both techniques every 5 minutes. Correlation between techniques, trends of BP variation, and the factors of the capture which likely influenced BP were assessed.Successful measurements of BP were achieved in 93% of all attempts with the Korotkoff's technique but in only 29% of all attempts with oscillometry. The latter method mostly provided lower values of BP compared to Korotkoff's technique in yearlings. Most bears showed a decreasing trend in systolic and mean BP over time, consistent between the two techniques. All bears were hypertensive: the auscultatory technique detected moderate to severe systolic hypertension in 25% and 84% of bears in Croatia and in Scandinavia, respectively, with significantly higher BP in subadults and adults compared to yearlings. Only Korotkoff's method resulted in a reliable and effective tool for BP assessment in brown bears. The anesthetic protocols used in the present study in association with the capture methods produced hypertension in all animals.

The perfusion index measured by the pulse oximeter affects the agreement between ClearSight and the arterial catheter-based blood pressures: A prospective observational study

BackgroundClearSight is a noninvasive arterial blood pressure monitor, but it remains unknown whether it is affected by the state of perfusion to the fingers. We investigated whether the lower perfusion index (PI) measured with a pulse oximeter, which reflects finger perfusion, would affect the agreement between arterial pressures measured with ClearSight versus those obtained with an arterial catheter.MethodsPaired arterial pressure data (ClearSight and arterial catheter-based pressures) and PI values were prospectively obtained from 30 patients undergoing major abdominal surgery. The primary outcome was standard deviation (SD) of the bias (precision) of blood pressure between ClearSight and arterial catheter. The ratio of the adjusted SD of the bias between PI≤1 and PI>1 was calculated using the linear mixed-effects model. The secondary outcomes were the bias and the limits of agreement (LOA) between the two devices (repeated measures Bland-Altman analysis).ResultsWe analyzed 6312 paired data points. The adjusted SD of bias in PI ≤1 compared with those in PI >1 was 1.4-fold (95% confidence interval: 1.3- to 1.4-fold) for systolic arterial pressure, 1.5-fold (95% confidence interval: 1.3- to 1.6-fold) for diastolic arterial pressure, and 1.3-fold (95% confidence interval: 1.2- to 1.5-fold) for mean arterial pressure. The bias (LOA) were as follows: systolic arterial pressure in the PI ≤1 and PI >1 groups, -3.5 (-35.4 to 28.4) mmHg and 2.2 (-19.9 to 24.3) mmHg, respectively; diastolic arterial pressure in the PI ≤1 and PI >1 groups, 13.1 (-5.1 to 31.3) mmHg and 9.0, (-2.6 to 20.6) mmHg, respectively; and mean arterial pressure in the PI ≤1 and PI >1 groups, 8.7 (-11.3 to 28.7) mmHg and 7.6 (-6.2 to 21.3) mmHg, respectively.ConclusionsPI ≤1 was associated with a large SD of the bias between the devices. The PI value could be a real-time indicator of ClearSight precision.

Efficacy of a continuous non-invasive arterial blood pressure monitor system for bronchoscope diagnosis and treatment under general anesthesia

Objective To compare continuous non-invasive arterial blood pressure (CNAP) monitoring with non-invasive blood pressure (NIBP) monitoring in anesthesia monitoring application value of bronchoscope diagnosis and treatment, and to find whether the CNAP can improve the detection rate of hypotension and detect low blood pressure earlier so that improve the safety of patients. Methods Twenty patients undergoing bronchoscope diagnosis and treatment under general anesthesia from outpatient were included in this study. The NIBP monitoring was put on one side of the upper limb and the CNAP monitoring was connected to the other side upper limb before inducement of general anesthesia. Both connections were kept until the end of operation. The systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP) of the CNAP were recorded one time per minute whereas the SBP, DBP and MAP of NIBP were recorded one time every 3 min. The low blood pressure (SBP<80 mmHg, 1 mmHg=0.133 kPa) frequency of CNAP and NIBP were respectively counted. The low blood pressure detection rate and the hypotension checkout time of NIBP and CNAP were calculated respectively. The consistency and correlation between the two blood pressure measurements were compared. At the same time, the complications of the two methods of monitoring were recorded and compared. Results The low blood pressure detection rate of CNAP and NIBP was 17.2% and 14.7% (P<0.05). The hypotension checkout time of CNAP and NIBP were (1.2±0.4) min and (3.0±0.0) min respectively (P<0.05). The SBP, DBP and MAP bias of CNAP and NIBP measurement were (1.5±8.8), (1.1±8.2) mmHg and (1.3±7.1) mmHg respectively. Consistency limits were -18.70-15.70 mmHg, -17.20-14.90 mmHg and -15.30-12.70 mmHg respectively. The proportion within the scope of the consistency of its corresponding boundary were 96.70%, 97.10% and 97.10% respectively. The correlation coefficients of SBP, DBP and MAP measured by CNAP and NIBP were 0.910, 0.861 and 0.941 respectively (P<0.05). None of the patients had complications. Conclusions CNAP is more sensitive and timely in detecting hypotension. It has higher consistency and more correlation than NIBP. Thus, CNAP can improve the safety of anesthesia monitoring for bronchoscope diagnosis and treatment. Continuous noninvasive arterial blood pressure monitoring using the vascular unloading technology during complex gastrointestinal endoscopy: a prospective observational study.

The innovative vascular unloading technology (VUT) allows continuous noninvasive arterial blood pressure (AP) monitoring. We aimed to investigate whether the VUT enables AP changes to be detected earlier compared with intermittent AP monitoring in patients undergoing gastrointestinal endoscopy. In this prospective observational study, we recorded continuous AP measurements with the VUT (CNAP system; CNSystems Medizintechnik AG, Graz, Austria) and intermittent AP measurements with upper arm cuff oscillometry in 90 patients undergoing complex gastrointestinal endoscopy (Department of Interventional Endoscopy at the University Medical Center Hamburg-Eppendorf, Hamburg, Germany). A "hypotensive phase" was defined as a time period of at least 30s during which ≥ 50% of the VUT-AP values were in a predefined range of hypotension, i.e., AP value a) ≥ 10% below the last oscillometric value and b) ≤ 65mmHg for mean AP or ≤ 90mmHg for systolic AP. In the 5-min-interval between two oscillometric measurements, one or more hypotensive phases were detected in 26 patients (29%) for mean AP and in 27 patients (30%) for systolic AP. Hypotensive phases had a mean duration of 195 ± 99s for mean AP and 197 ± 97s for systolic AP with a mean procedure duration of 36 (± 21) min. Continuous noninvasive AP monitoring using the VUT enables hypotensive phases to be detected earlier compared with intermittent AP monitoring during complex gastrointestinal endoscopy. These hypotensive phases may be missed or only belatedly recognized with intermittent AP monitoring. Continuous noninvasive AP measurement facilitates detecting hemodynamic instability more rapidly and therefore may improve patient safety.

The impact of continuous non-invasive arterial blood pressure monitoring on blood pressure stability during general anaesthesia in orthopaedic patients: A randomised trial.

In patients undergoing general anaesthesia, intraoperative hypotension occurs frequently and is associated with adverse outcomes such as postoperative acute kidney failure, myocardial infarction or stroke. A history of chronic hypertension renders patients more susceptible to a decrease in blood pressure (BP) after induction of general anaesthesia. As a patient's BP is generally monitored intermittently via an upper arm cuff, there may be a delay in the detection of hypotension by the anaesthetist. The current study investigates whether the presence of continuous BP monitoring leads to improved BP stability. Randomised, controlled and single-centre study. A total of 160 orthopaedic patients undergoing general anaesthesia with a history of chronic hypertension. The patients were randomised to either a study group (n = 77) that received continuous non-invasive BP monitoring in addition to oscillometric intermittent monitoring, or a control group (n = 83) whose BP was monitored intermittently only. The interval for oscillometric measurements in both groups was set to 3 min. After induction of general anaesthesia, oscillometric BP values of the two groups were compared for the first hour of the procedure. Anaesthetists were blinded to the purpose of the study. BP stability and hypotensive events. There was no difference in baseline BP between the groups. After adjustment for multiple testing, mean arterial BP in the study group was significantly higher than in the control group at 12 and 15 min. Mean ± SD for study and control group, respectively were: 12 min, 102 ± 24 vs. 90 ± 26 mmHg (P = 0.039) and 15 min, 102 ± 21 vs. 90 ± 23 mmHg (P = 0.023). Hypotensive readings below a mean pressure of 55 mmHg occurred more often in the control group (25 vs. 7, P = 0.047). Continuous monitoring contributes to BP stability in the studied population. Assessing the eligibility of a non-invasive continuous blood pressure measurement technique for application during total intravenous anaesthesia.

To assess the eligibility for replacement of invasive blood pressure as measured "within" the arterial vessel (IBP) with non-invasive continuous arterial blood pressure (cNIP) monitoring during total intravenous anaesthesia (TIVA), the ability of cNiP to track fast blood pressure changes needs to be quantified. A new method of statistical data analysis is developed for this purpose. In a pilot study on patients undergoing neurosurgical anaesthesia, mean arterial pressure MAPIBP measured with IBP was compared to MAPCNP measured by the CNAP Monitor 500 in ten patients (age: 63±13 a). Correlation analysis of changes of device differences ΔeMAP=ΔMAPCNP-ΔMAPIBP with changes of MAPIBP (ΔMAPIBP) during intervals of vasoactivity was conducted. An innovative technique, of linear trend analysis (LTA) applied to two signals, is described to perform this analysis without a priori knowledge of intervals of vasoactivity. Analysis of ΔeMAP during vasoactivity revealed that ΔMAPCNP systematically underestimated ΔMAPIBP by 37%. This was confirmed in the complete data set using LTA technique showing a systematic, yet patient specific, underestimation in tracking ΔMAPIBP (16…120%). The proposed LTA technique is able to detect systematic errors in tracking short-term blood pressure changes otherwise masked by established analysis. LTA may thus be a useful tool to assess the eligibility of cNIP to replace IBP during TIVA.

Radial artery applanation tonometry for continuous noninvasive arterial blood pressure monitoring in the cardiac intensive care unit.

Hemodynamic monitoring plays a pivotal role in the treatment of patients in the cardiac intensive care unit (CICU). The innovative radial artery applanation tonometry technology allows for continuous noninvasive arterial blood pressure (AP) measurement. By closing the gap between continuous invasive AP monitoring (arterial catheter) and intermittent noninvasive AP monitoring (oscillometry) this technology might improve CICU patient monitoring. We therefore aimed to evaluate the measurement performance of radial artery applanation tonometry in comparison with a radial arterial catheter in CICU patients. In this prospective method comparison study, we simultaneously recorded AP noninvasively with radial artery applanation tonometry (T-line 200 pro device; Tensys Medical Inc., San Diego, CA, USA) and invasively with an arterial catheter (criterion standard) in 30 patients treated in the CICU of a German university hospital. We statistically analyzed 7,304 averaged 10-beat epochs of measurements of mean AP, systolic AP, and diastolic AP by using Bland-Altman analysis for repeated measurements. Our study revealed a mean difference ± standard deviation (95% limits of agreement; percentage error) between radial artery applanation tonometry and the criterion standard method (radial arterial catheter) of +2 ± 6 mmHg (-10 to +14 mmHg; 17%) for mean AP, -6 ± 11 mmHg (-28 to +15 mmHg; 20%) for systolic AP, and +4 ± 7 mmHg (-9 to +17 mmHg; 23%) for diastolic AP. In CICU patients, continuous noninvasive measurement of AP using radial artery applanation tonometry is feasible. The technology showed reasonable accuracy and precision in comparison with radial arterial catheter-derived AP values.

The T-Line TL-200 system for continuous non-invasive blood pressure measurement in medical intensive care unit patients

The T-Line TL-200 (Tensys Medical, San Diego, CA, USA) is a non-invasive arterial blood pressure (BP) monitoring system allowing continuous "beat-to-beat" monitoring of systolic arterial pressure (SAP), mean arterial pressure (MAP), and diastolic arterial pressure (DAP). It provides a real-time BP waveform like that obtained using an arterial catheter. The aim of this study was to compare BP measurements obtained using the T-Line TL-200 with simultaneous invasive BP measurements using a femoral arterial catheter in unselected critically ill medical patients. In 28 patients treated in a medical intensive care unit (ICU), BP values were simultaneously obtained using a femoral arterial catheter and the T-Line TL-200. All recorded data were included in the final analysis. For comparison of BP measurements, Bland-Altman analysis accounting for repeated measurements was performed (primary endpoint). A total of 76,826 pairs of BP measurements (each consisting of SAP, MAP, and DAP) were analyzed. For MAP, Bland-Altman analysis revealed a mean difference of +0.47 mmHg (95 % limits of agreement -16.53 to +17.46 mmHg). For SAP and DAP, the bias and 95 % limits of agreement were -9.01 mmHg (-37.47 to +19.45 mmHg) and +5.22 mmHg (-13.50 to +23.94 mmHg), respectively. Non-invasive, continuous, radial BP measurement with the T-Line TL-200 is basically feasible in medical ICU patients (with a low bias for MAP compared to MAP assessed using a femoral arterial catheter). High limits of agreement (particularly of SAP and DAP) preclude the use of the device as a single source of BP information in unstable critically ill patients.

Cerebrovascular response to valsalva maneuver: Methodology, normal values, and retest reliability

To provide reference values of cerebrovascular reactivity measured with transcranial Doppler ultrasonography in response to rapid changes in blood pressure during the late straining phase (IIb) of a Valsalva maneuver (VM). Middle cerebral artery blood flow velocity, arterial blood pressure changes, and end-tidal partial carbon dioxide pressure (pCO(2) ) concentrations were simultaneously and noninvasively measured in 60 healthy subjects. Centro-peripheral Valsalva ratio was calculated by the quotient of cerebrovascular reactivity and peripheral arterial blood pressure response to VM during phase IIb. Age dependency and short- and long-term reliability of indices were evaluated. Correlation between the autonomic response and changes of pCO(2) was calculated. Centro-peripheral Valsalva ratio was higher in younger healthy subjects, whereas peripheral autonomic reactivity appeared age-independent. There was no correlation between dynamic autoregulatory response and pCO(2) . Short- and long-term measurements of autoregulatory indices showed excellent reliability. VM is a suitable maneuver to evaluate dynamic autonomic response using transcranial Doppler ultrasonography and noninvasive arterial blood pressure monitoring. Autonomic response during VM does not depend on pCO(2) . Normal human subjects show age-dependent centro-peripheral autoregulatory response.

A comparison of a continuous noninvasive arterial pressure (CNAP™) monitor with an invasive arterial blood pressure monitor in the cardiac surgical ICU

Accurate measurement and display of arterial blood pressure is essential for rational management of adult cardiac surgical patients. Because of the lower risk of complications, noninvasive monitoring methods gain importance. A newly developed continuous noninvasive arterial blood pressure (CNAP™) monitor is available and has been validated perioperatively. In a prospective study we compared the CNAP™ monitoring device with invasive arterial blood pressure (IAP) measurement in 30 patients in a cardiac surgical Intensive Care Unit (ICU). Patients were either mechanically ventilated or spontaneously breathing, with or without inotropes. CNAP™ was applied on two fingers of the hand contralateral to the IAP monitoring catheter. Systolic, diastolic and mean pressure data were recorded every minute for 2 h simultaneously for both IAP and CNAP™. Statistical analysis included construction of mountain plot and Bland Altman plots for assessing limits of agreement and bias (accuracy) calculation. Three thousand and six hundred pairs of data were analyzed. The CNAP™ systolic arterial pressure bias was 10.415 mmHg and the CNAP™ diastolic arterial pressure bias was -5.3386 mmHg; the mean arterial pressure (MAP) of CNAP™ was close to the MAP of IAP, with a bias of 0.03944 mmHg. The Bland Altman plot showed a uniform distribution and a good agreement of all arterial blood pressure values between CNAP™ and IAP. Percentage within limits of agreement was 94.5%, 95.1% and 99.4% for systolic, diastolic and MAP. Calculated limits of agreement were -4.60 to 25.43, -13.38 to 2.70 and -5.95 to 6.03 mmHg for systolic, diastolic and mean BP, respectively. The mountain plot showed similar results as the Bland Altman plots. We conclude CNAP™ is a reliable, noninvasive, continuous blood pressure monitor that provides real-time estimates of arterial pressure comparable to those generated by an invasive arterial catheter system. CNAP™ can be used as an alternative to IAP.

Data recorded from the devices used to monitor a patient's vital signs are often used in the development of displays, alarms, and information systems, but high-resolution, multiple-parameter datasets of anesthesia monitoring data from patients during anesthesia are often difficult to obtain. Existing databases have typically been collected from patients in intensive care units. However, the physical state of intensive care patients is dissimilar to those undergoing surgery, more frequent and marked changes to cardiovascular and respiratory variables are seen in operating room patients, and additional and highly relevant information to anesthesia (e.g., end-tidal agent monitoring, etc.) is omitted from these intensive care databases. We collected a set of high-quality, high-resolution, multiple-parameter monitoring data suitable for anesthesia monitoring research. Vital signs data were recorded from patients undergoing anesthesia at the Royal Adelaide Hospital. Software was developed to capture, time synchronize, and interpolate vital signs data from Philips IntelliVue MP70 and MP30 patient monitors and Datex-Ohmeda Aestiva/5 anesthesia machines into 10 millisecond resolution samples. The recorded data were saved in a variety of accessible file formats. Monitoring data were recorded from 32 cases (25 general anesthetics, 3 spinal anesthetics, 4 sedations) ranging in duration from 13 minutes to 5 hours (median 105 min). Most cases included data from the electrocardiograph, pulse oximeter, capnograph, noninvasive arterial blood pressure monitor, airway flow, and pressure monitor and, in a few cases, the Y-piece spirometer, electroencephalogram monitor, and arterial blood pressure monitor. Recorded data were processed and saved into 4 file formats: (1) comma-separated values text files with full numerical and waveform data, (2) numerical parameters recorded in comma-separated values files at 1-second intervals, (3) graphical plots of all waveform data in a range of resolutions as Portable Network Graphics image files, and (4) graphical overview plots of numerical data for entire cases as Portable Network Graphics and Scalable Vector Graphics files. The complete dataset is freely available online via doi:102.100.100/6914 and has been listed in the Australian National Data Service Collections Registry. The present dataset provides clinical anesthesia monitoring data from entire surgical cases where patients underwent anesthesia, includes a wide range of vital signs variables that are commonly monitored during surgery, and is published in accessible, user-friendly file formats. The text and image file formats let researchers without engineering or computer science backgrounds easily access the data using standard spreadsheet and image browsing software. In future work, monitoring data should be collected from a wider range and larger number of cases, and software tools are needed to support searching and navigating the database.

Evaluation of a continuous non-invasive arterial blood pressure monitoring device in comparison with an arterial blood pressure measurement in the ICU

Due to a lower risk of complications, non-invasive monitoring methods gain importance. Measuring arterial blood pressure belongs to the standard hemodynamic monitoring. A newly developed continuous non-invasive arterial blood pressure (CNAP) measurement method is available and has been validated perioperatively [1]. We compared the CNAP monitoring device with invasive arterial blood pressure measurement (IBP) as the gold standard in critically ill patients.

Pulsatile Stress in Middle-Aged Patients With Type 1 or Type 2 Diabetes Compared With Nondiabetic Control Subjects

OBJECTIVEArterial pulse pressure is considered to be an independent cardiovascular risk factor. We compared pulse pressure during an active orthostatic test in middle-aged patients with type 1 diabetes and with type 2 diabetes and corresponding nondiabetic control subjects.RESEARCH DESIGN AND METHODSForty patients with type 1 diabetes (mean age 50 years, diabetes duration 23 years, and BMI 23.0 kg/m2) were compared with 40 nonhypertensive patients with type 2 diabetes (respectively, 50 years, 8 years, and 29.7 kg/m2). Patients taking antihypertensive agents or with renal insufficiency were excluded. All patients were evaluated with a continuous noninvasive arterial blood pressure monitoring (Finapres) in standing (1 min), squatting (1 min), and again standing position (1 min). Patients with type 1 or type 2 diabetes were compared with two groups of 40 age-, sex- and BMI-matched healthy subjects.RESULTSPatients with type 1 diabetes and patients with type 2 diabetes showed significantly higher pulse pressure, heart rate, and double product of pulse pressure and heart rate (PP×HR) (type 1: 5,263 vs. 4,121 mmHg/min, P = 0.0004; type 2: 5,359 vs. 4,321 mmHg, P = 0.0023) levels than corresponding control subjects. There were no significant differences between patients with type 1 diabetes and type 2 diabetes regarding pulse pressure (59 vs. 58 mmHg), heart rate (89 vs. 88/min), and PP×HR (5,263 vs. 5,359 mmHg/min).CONCLUSIONSPatients with type 1 diabetes have increased levels of peripheral PP, an indirect marker of arterial stiffness, and PP×HR, an index of pulsatile stress, comparable to those of nonhypertensive patients with type 2 diabetes at similar mean age of 50 years.

The Reliability of Continuous Noninvasive Finger Blood Pressure Measurement in Critically Ill Children

Continuous noninvasive arterial blood pressure can be measured in finger arteries using an inflatable finger cuff (FINAP) with a special device and has proven to be feasible and reliable in adults. We studied prototype pediatric finger cuffs and pediatric software to compare this blood pressure measurement with intraarterially measured blood pressure (IAP) in critically ill children. We included sedated and mechanically ventilated children admitted to our pediatric intensive care unit. We performed simultaneous arterial blood pressure measurements during a relatively stable hemodynamic period and compared FINAP, IAP, and the noninvasive blood pressure oscillometric technique. We also compared IAP to a reconstruction of brachial pressure from finger pressure. Thirty-five children between 2 and 22 kg body weight were included. In total, 152 attempts to record a FINAP pressure were performed of which 4.6% were unsuccessful. When comparing FINAP to IAP, bias was -16.2, -7.7, and -10.2 mm Hg for systolic arterial blood pressure, diastolic arterial blood pressure, and mean arterial blood pressure. Limits of agreement (LOA) were respectively 26.1%, 30.1%, and 22.6%. When reconstruction of brachial pressure from finger pressure was compared to IAP, these results were -11.8, 0.6, and -0.9 mm Hg for bias and 21.7%, 8.9%, and 8.9% for LOA. When noninvasive blood pressure oscillometric technique was compared to IAP, the results were: -6.8, -0.9, and -3.8 mm Hg for bias and 18.2%, 38.6%, and 22.1% for LOA. Beta type continuous noninvasive arterial blood pressure monitoring using a finger cuff with brachial arterial waveform reconstruction seems reliable in hemodynamically stable critically ill children.
