Evolving consolidation patterns and outcomes for a large cohort of patients with primary CNS lymphoma

AbstractConsolidation strategies for primary central nervous system lymphoma (PCNSL) after induction chemoimmunotherapy include whole-brain radiotherapy (WBRT; ≤24 Gy reduced-dose [RD], >24 Gy standard-dose) and cytarabine, nonmyeloablative chemotherapy (NMC), or autologous hematopoietic cell transplantation (AHCT); however, comparative outcomes are lacking. PCNSL outcomes from 1983 to 2020 were stratified by decade and Memorial Sloan Kettering Cancer Center recursive partitioning analysis (RPA) class. Clinicodemographic associations with consolidation were analyzed by multinomial logistic regression. Progression-free survival (PFS) and overall survival (OS) were analyzed by proportional hazards from consolidation. Of 559 patients, 385 (69%) were consolidated. Median follow-up and OS were 7.4 and 5.7 years, respectively. WBRT use declined (61% in 1990s vs 12% in 2010s), whereas AHCT (4% in 1990s vs 32% in 2010s) and NMC (27% in 1990s vs 52% in 2010s) rose. Compared with RPA 1 (age of <50 years), RPA 2 (age of ≥50 years, Karnofsky performance status [KPS] ≥70) was more likely to receive NMC. Those with partial response to induction were less likely to receive AHCT (odds ratio, 0.36; P = .02). Among 351 with complete response to consolidation, only receipt of rituximab, methotrexate, procarbazine, and vincristine induction was associated with improved PFS (hazard ratio, 0.5; P = .006). Among RPA 1, median PFS and OS were not reached for AHCT or RD-WBRT, vs 2.5 and 13.0 years, respectively, after NMC. Among RPA class 3 (KPS <70), median PFS and OS after RD-WBRT were 4.6 and 10 years, respectively, vs 1.7 and 4.4 years after NMC. No significant adjusted survival differences were seen across consolidation strategies. NMC is increasingly used in lieu of RD-WBRT despite a trend toward less favorable PFS. RD-WBRT can be considered in patients ineligible for AHCT.

Tumor-infiltrating lymphocytes: A new hope

Tumor-infiltrating lymphocytes (TILs) can be massively expanded from resected tumors and used as a cellular treatment for advanced malignancies. TILs require a preparative non-myeloablative chemotherapy followed by an abbreviated course of interleukin-2. Here, we review the historical development of TIL therapy and discuss potential solutions to ongoing roadblocks that may result in broader and improved efficacy for patients afflicted with treatment-refractory, advanced cancer.

Outcomes in Routine Clinical Practice with Methotrexate-Temozolomide-Rituximab Induction and Modified Etoposide-Cytarabine Consolidation in Newly Diagnosed Primary CNS Lymphoma

Background: Recent studies have reported improved outcomes for patients with Primary CNS lymphoma (PCNSL) with addition of agents in induction (for e.g. cytarabine, thiotepa and rituximab in IELSG studies), intense consolidation (Autologous stem cell transplant (ASCT) as in IELSG 43) and intense non-myeloablative chemotherapy with etoposide/cytarabine (EA) as in CALGB 50202). Delivery of such an intense therapy is often difficult in clinical practice due to advanced age, comorbidities, poor performance status and resource constraints. We started using induction with methotrexate, temozolomide and rituximab (MTR) as per CALGB 50202 study however the intense non-myeloablative EA consolidation was not feasible. We used a strategy of reduced dose EA consolidation for 2 cycles with etoposide at 100 mg/m 2 over 2 hours and cytarabine 2 gm/m 2 twice daily for 2 or 3 days based on clinical judgement of the treating physician. Here we report outcomes with this approach in newly diagnosed PCNSL patients treated at our centre. Method: We did a retrospective analysis of patients with newly diagnosed PCNSL treated with MTR induction followed by modified EA consolidation. Kaplan-Meier was used to estimate progression free (PFS) and overall survival (OS). Variables influencing the outcomes with P less than 0.05 were considered significant. Results: Between January 2015 to December 2022, 72 patients were treated with above protocol with a median age of 46.5 years (range, 17 to 68 years) and 50 (69.4%) were males. Median ECOG performance score at presentation was 1 with 16 (22%) patients having ECOG performance score of 3 or more. IELSG scores were high and intermediate risk in 26 (36.1%) and 27 (37.5%) respectively while only 7 (9.7%) were in the low risk (data was not available for remaining 12 (17%) patients). MSKCC scores were available for all patients with 43 (59%) patients in class 1, 14 (19%) patients in class 2 and 15 (20%) patients in class 3. MTR induction with a dose of 8 gm/m2 could be delivered for all 4 cycles in 64 (88%) patients. Dose reduction was required in 14 (19.4%) patients; reasons were renal dysfunction in 8 (11%), infections in 3 (4%), liver dysfunction in 2 (3%). Nine patients (12.5%) had disease progression during induction. Total of 54 patients received consolidation with systemic therapy. Dose reduced EA consolidation was delivered in 46 (63%) patients while 8 patients received single agent high dose cytarabine consolidation and only 3 patients underwent ASCT. Seven patients received WBRT consolidation. EA consolidation was given for 2 days in 30 (65%) patients while 16 (35%) patients received 3 days of EA consolidation. Twenty six patients received maintenance lenalidomide. With a median follow up of 39.2 months (range, 2.1 to 91), median PFS is 46.9 months and median overall OS has not reached. Three years PFS &amp; OS for all patients is 59.5% and 78% respectively ( Fig 1A &amp;B). Median PFS and OS of patients receiving EA consolidation has not been reached with a 3-year PFS and OS of 77% and 79% respectively. MSKCC score was predictive of OS with 3 year OS in class 1, 2 and 3 of 89.5%, 68% and 58% respectively. Consolidation with split dose EA was associated with better OS and PFS as compared to single agent cytarabine alone. None of the 3 patients receiving ASCT have relapsed. Patients receiving full dose methotrexate had better OS compared to patients requiring dose reduction. Two versus 3 day EA consolidation had no impact on OS. MTR induction was well tolerated with grade 3 or more toxicity as follows: renal dysfunction in 8 (11%) patients, transaminitis in 4 and febrile neutropenia in 4 patients with no treatment related deaths. EA consolidation resulted in 18 episodes of febrile neutropenia with two deaths. Grade 3 or more hematologic toxicities were common with grade 3 or more anaemia, neutropenia and thrombocytopenia in 15 (32%), 40 (86%), 33 (71%) patients respectively. There were two deaths in the consolidation phase, one due to intracranial bleeding and another due to febrile neutropenia. Conclusion: MTR induction with modified EA consolidation is feasible and results in optimal outcomes in real world setting. Higher cumulative methotrexate dose and use of EA consolidation were associated with better OS. These outcomes compare favourably to CALGB 50202 study and represent a feasible option in resource limited setting.

Impact of Thiotepa-Based Autologous Hematopoietic Cell Transplantation in Primary Central Nervous System Lymphoma in First Complete Remission: A University of California Hematologic Malignancies Consortium Retrospective Analysis

BackgroundThe choice between nonmyeloablative chemotherapy (NMA-C) or autologous hematopoietic cell transplantation (autoHCT) as consolidation in primary central nervous system lymphoma (PCNSL), and timing of autoHCT differs among centers. We aimed to clarify these points. MethodsWe retrospectively analyzed PCNSL adult patients who received consolidation in CR1 or underwent autoHCT during their treatment course. Cohort A included those who underwent autoHCT in CR1, cohort B included those who underwent NMA-C in CR1, and cohort C included patients who underwent autoHCT in CR2+. We compared cohorts A and B, and cohorts A and C. The primary endpoint was overall survival (OS), and secondary endpoints were progression-free survival (PFS), treatment-related mortality (TRM) and cumulative incidence of relapse (CIR). Results36 patients were included in cohort A, 30 in cohort B, and 14 in cohort C. The 5-year OS for cohorts A vs B and vs C were 90.7% vs 62.8% (P = .045) and vs 77.9% (P = .32), respectively. The 5-year PFS from diagnosis for cohorts A vs B was 87.8% vs 37.3% (P < .001). The 5-year PFS from autoHCT for cohorts A vs C was 87.6% vs 58.4% (P = .023). The 5-year TRM and CIR in cohorts A vs B was 9.4% vs 9.5% (P = .674), and 2.9% vs 53.2% (P < .001), respectively. The 5-year TRM and CIR in cohorts A vs C from the time of autoHCT was 9.5% vs 22.1% (P = .188), and 2.9% vs 19.5% (P = .104), respectively. ConclusionDespite the limitations, thiotepa-based autoHCT in CR1 appears to improve outcomes in eligible patients with PCNSL.

Abstract OT2-10-04: Treatment of advanced or metastatic triple-negative breast cancer with adoptive therapy of PD1+ tumor-infiltrating lymphocytes (TILS001 trial)

Abstract Background Metastatic triple-negative breast cancer (mTNBC) exhibits a particularly poor clinical outcome, generally with rapid progression and worse overall survival (OS) than other BC subtypes. Among the few therapeutic options, chemotherapy-based combinations are associated with increased toxicity and limited survival benefit, being treatment with sequential single agents, such as paclitaxel considered an appropriate first-line regimen for the metastatic setting for PDL-1 negative patients. Herein, there is an urgent need for clinically active agents for the mTNBC. Adoptive cell transfer (ACT)-based immunotherapy using ex vivo activated and expanded tumor-infiltrating lymphocytes (TILs) has shown promising therapeutic outcomes in some patients with metastatic tumors. The identification, selection, and enrichment of tumor-reactive lymphocytes at the early stages of the ACT generation could enhance their clinical activity. Thus, the selection of reactive T cells, such as PD1-positive (PD1+) TILs, could improve the responses achieved in those settings. TILS001 trial aims to explore the safety, tolerability and efficacy of selected PD1+ T-cell infusion with a previous pre-selection of mRNA PD1-high expression in patients with mTNBC. Study design: TILs001 trial is an open-label, single-arm, multicenter phase I/II prospective study with a two-stage design evaluating treatment with PD1+ TILs infusion in advanced or mTNBC, defined as HER2 negative and Hormonal Receptor &amp;lt; 10%. The study involves three different parts before PD1+TILs treatment. Tumor samples evaluable for PD1 mRNA expression and life expectancy ≥6 months are mandatory for part 1. Patients with high levels of PD1 mRNA, defined by the pre-specified cutoff, candidates for receiving a first-line taxane-based containing regimen and with at least 1 accessible target lesion to generate TILs are eligible for part 2. Finally, once the complete expansion of PD1+TILs is reached, patients will receive the non-myeloablative lymphodepleting chemotherapy regimen followed by PD1+TIL infusion and IL-2 treatment. Allogeneic hematopoietic stem cell transplantation, immune system-related disease or clinically active cerebral metastasis are not allowed. The primary objectives are to evaluate the safety and tolerability of the PD1+ TIL product, as per incidence of grade 3-5 adverse events (AE) or any grade AE that leads to treatment discontinuation and to assess the efficacy of ACT therapy with selected PD1+ TILs in mTNBC in terms of progression-free survival (PFS) at 6 months. The secondary endpoints are clinical benefit rate at 6 months (CBR6), overall response rate, duration of response (DoR), PFS and OS. Further translational research including immunophenotyping, TCR sequencing and mutational analysis will also be performed. The first 3 patients will be included in a safety run-in phase where safety will be evaluated 24h after PD1+/TILs infusion (before IL-2 treatment) and a phase II stage where efficacy will be evaluated, which will include up to 20 patients. Patients will be enrolled in 4 sites in Spain. Recruitment is expected to start by July 2022 and to be completed within 24 months. Impact of Timing of Autologous Hematopoietic Cell Transplantation in Primary Central Nervous System Lymphoma: A University of California Hematologic Malignancies Consortium Retrospective Analysis

Background: Primary central nervous system lymphoma (PCNSL) is potentially curable when high-dose methotrexate-based induction is followed with intensive consolidation, which can consist of non-myeloablative chemotherapy (NMA-C) or myeloablative chemotherapy (MAC) followed by autologous hematopoietic cell transplantation (autoHCT). Although the efficacy of autoHCT has been demonstrated, very limited data is available regarding the optimal timing of autoHCT, i.e., to transplant in first complete remission (CR1) or second or later CR (CR2+). Moreover, the choice of consolidation differs between centers. Recently, the CALGB 51101 trial showed that MAC leads to improved progression-free survival (PFS) compared to NMA-C, but limiting this finding are that more patients who were randomized to receive NMA-C went off treatment prior to consolidation, either as a result of progression or death. To clarify the optimal timing of autoHCT in PCNSL and to support the findings of the CALGB 51101 trial, we conducted a multi-center retrospective analysis. Methods: We retrospectively analyzed patients ≥18 years treated at the University of California campuses in Davis, Fresno, Irvine, and Los Angeles who were diagnosed between January 2005-July 2021. Patients were divided into three cohorts: cohort A included those who underwent autoHCT in CR1, cohort B included those who underwent autoHCT in CR2+, and cohort C included those who underwent NMA-C in CR1 (irrespective of whether or not they underwent autoHCT in CR2+). In the first set of analyses, outcomes were compared between cohort A and B, and in the second set, outcomes were compared between cohort A and C. CR was defined by the lack of enhancement on magnetic resonance imaging. Patients in cohort A and B who were not in CR at the time of autoHCT, and those in cohort C not in CR at the time of NMA-C, were excluded. The primary endpoint was 2-year overall survival (OS), and secondary endpoints were 2-year progression-free survival (PFS), non-relapse mortality (NRM) and cumulative incidence of relapse (CIR). Time-to-event analyses were conducted using the Kaplan-Meier method and were compared by a log-rank test. NRM and CIR were calculated through a competing risk model, with NRM serving as a competing risk for CIR and vice versa. Univariate analyses were performed via Cox proportional hazards model. Patients alive at the end of the study were censored at their last follow-up date. Results: 69 unique patients were included, and their baseline characteristics are described in Table 1. 36 patients were included in cohort A, 10 in cohort B, and 28 in cohort C, 5 of whom underwent autoHCT in CR2+. The median follow-up times of cohorts A, B, and C were 67, 73, and 52 months, respectively. The 2-year OS from the time of diagnosis in cohort A was 94.3% (95% CI, 86.9-100) vs 90.0% (95% CI, 73.2-100) in cohort B (P=0.42), and 84.0% (95% CI, 70.7-99.8) in cohort C (P=0.072). The 2-year PFS from the time of transplant in cohorts A vs B were 91.2% (95% CI, 82.2-100) vs 67.5 (95% CI, 43-100) (P=0.008). The 2-year PFS from the time of diagnosis in cohorts A vs C were 91.4% (95% CI, 82.6-100) vs. 80.5% (95% CI, 66.4-97.6) (P=0.0016) (Figure 1). The 2-year NRM and CIR for cohorts A vs B were 5.8% vs 21.2% (P=0.179) and 2.9% vs 11.3% (P=0.037), respectively. The 2-year NRM and CIR for cohorts A vs C were 5.8% vs 3.6% (P=0.74) and 2.9% vs 16.0% (P<0.001) respectively. Univariate analyses including all 69 unique patients were performed for OS and PFS, and there were no significant associations based on age <60 vs ≥60 years, gender, ECOG performance status ≤1 vs ≥2, center of origin, and thiotepa/busulfan/cyclophosphamide (TBC) vs thiotepa/carmustine (TT-BCNU) conditioning. Conclusion: Although limited by the sample size and retrospective nature, whether autoHCT for PCNSL was performed in CR1 or CR2+ did not impact OS in our analysis, but CIR was higher and PFS was lower when autoHCT was performed in CR2+. Supporting the Alliance trial, autoHCT in CR1 produced longer PFS than NMA-C and patients deemed appropriate candidates for MAC consolidation in CR1 should be considered for autoHCT in this setting. Outcomes of Patients with Heavily Pretreated Relapsed/Refractory Multiple Myeloma Receiving Salvage Cytotoxic Therapy with Supportive Stem Cell Boost

Background Autologous hematopoietic stem cell transplant (ASCT) remains a cornerstone of therapy for eligible patients with multiple myeloma (MM) and can be incorporated as part of the initial therapy (early ASCT) or at the time of first relapse (delayed ASCT) [1]. Current guidelines recommend that adequate hematopoietic stem cells (HSC should be collected for a potential second ASCT, with a minimum goal of 2 million CD34+ cells/kg per transplant [2]. The stored HSC may be used for either a planned tandem transplant or future salvage ASCT. Less commonly, the previously stored HSC may be used for stem cell "boosts” following either dose intensive, non-myeloablative chemotherapy or for patients with "burned out” bone marrows secondary to extensive prior therapies. [3]. To our knowledge, there is a paucity of data on the effectiveness and survival outcome of patients with multiple relapsed MM who receive a stem cell boost following dose intensive chemotherapy. This study analyzes the clinical outcomes of multiply relapsed MM patients receiving multi-agent chemotherapy followed by a stem cell boost. Outcomes evaluated were overall response rate, proportion of patients who received a subsequent line of therapy, and overall survival. Methods This is a retrospective chart review performed at 2 institutions (Hackensack University Medical Center and MedStar Georgetown University Hospital). Fifty-nine (59) patients with MM aged ≥18 years old who underwent first ASCT from 1999 to 2021 and who collected sufficient s to allow for either tandem or salvage transplant or potential future stem cell boost were included in this study. Response to therapy was defined as stable disease or better on evaluation 1 month following stem cell boost. Patients who received another line of therapy less than 1 month post boost but did not have evidence of disease progression at that time, were excluded from ORR analysis. Overall survival (OS) was defined as the time from stem cell boost to date of death or last follow up. Statistical analysis was performed using STATA v16.0 (Stata Corp, TX, USA). Survival analysis was carried out using the Kaplan-Meier method and the log-rank test was used to compare survival curves. Results The patient characteristics who received a stem cell boost are shown in Table 1 (n=59): male gender (61 %), median age at diagnosis was 60 (range 37-81), median number of lines of therapy 6.3 5 Range 1-16). The patient who received only 1 prior line of therapy had aggressive relapse less than 100 days post melphalan-conditioned ASCT which necessitated use of urgent dose intensive chemotherapy. The stem cell dose used for the boost ASCT ranged from 0.4 to 9.8 X 10^6 CD34+ cells/kg (median 2.3). 1.6. Of the 49 evaluable patients, 28 (57.1%) had response to therapy 1 month post stem cell boost. Overall, 37 (62.7%) patients successfully received a subsequent line of therapy post stem cell boost. The median overall survival for the entire cohort was 129 days (range 1 - 1462 days) (Figure 1). OS was significantly longer in responding patients compared to those who did not respond (182 days vs 82 days, p=0.0081). Conclusions Dose intensive, non-myeloablative chemotherapy can improve survival in heavily pretreated, usually penta-refractory MM that require disease for refractory relapse. The administration of a stem cell boost utilizing previously collected HSCs may assist with hematopoietic recovery following dose intensive chemotherapy and in this subset of patients who have poor marrow reserve from prior therapy. This study serves to demonstrate the clinical outcomes of patients treated with multiagent chemotherapy followed by a stem cell boost. With an ORR of 57.1%, this approach can be effective in the appropriate clinical setting and significantly improves OS in patients who do respond to therapy (Figure 2). The goal of such therapy is to achieve disease control, and often serves as a "bridge” for patients to receive subsequent myeloma-directed therapy that may not be readily available at the time of relapse, such as clinical trial or chimeric antigen receptor T-cell (CAR-T) therapy. This data allows for treating oncologists to appropriately implement and inform patients of the therapeutic uses and clinical outcomes of treating such patients with dose intensive multiagent chemotherapy followed by a stem cell boost.

Bridging advanced myeloma patients to subsequent treatments and clinical trials with classical chemotherapy and stem cell support.

Rapidly progressing relapsed/refractory multiple myeloma (RRMM) patients with compromised marrow have limited treatment options. Thus, non-myeloablative chemotherapy with a stem cell boost (SCB) may provide disease control and hematopoietic improvement as bridge to subsequent therapies. We identified 96 patients who received a SCB between January 2011 and December 2019 at the Mount Sinai Hospital. Patients had a median age of 64 years, received a median of 7 prior lines of therapy and 68 and 42% were triple-class and penta-drug refractory, respectively. Chemotherapy included melphalan (MEL) (n = 16), melphalan + carmustine (BCNU/MEL) (n = 52) or a variant of DCEP (dexamethasone, cyclophosphamide, etoposide, cisplatin) (n = 28). Median time to neutrophil recovery was 10 days and was significantly lower with DCEP (8 days) compared to MEL and BCNU/MEL (10-11 days) (p = 0.0047). Time to progression, progression-free survival and overall survival were 3.19, 2.7 and 8.38 months, respectively. The BCNU/MEL group had the highest response rate of 85% (p = 0.05), clinical benefit rate of 94% (p = 0.0014), progression-free survival of 3.3 months (p = 0.4) and overall survival of 8.7 months (p = 0.5). Sixty-six patients (69%) were bridged to new lines of therapy, including clinical trials. Non-myeloablative chemotherapy with SCB provides rapid disease control and marrow recovery with potential to receive further therapy.

Hematopoietic Stem Cell Transplantation for Acute Myeloid Leukemia: An Overview of Systematic Reviews.

Background Hematopoietic stem cell transplantation (HSCT) has become the main treatment for acute myeloid leukemia (AML) and has been studied in many systematic reviews (SRs), but strong conclusions have not been drawn yet. Objective This study aimed to summarize and critically evaluate the methodological and evidence quality of SRs and meta-analysis on this topic. Methods PubMed, Embase, the Cochrane Library, and Web of Science were searched for SRs/meta-analyses regarding HSCT for AML. Two reviewers assessed the quality of SRs/meta-analyses in line with AMSTAR-2 and evaluated the strength of evidence quality with the grading of the evaluation system (GRADE) for concerned outcomes independently. Results 12 SR/Meta articles were included, and the AMSTAR-2 scale showed that the quality grade of all articles was low or very low. GRADE results showed 29 outcomes, 2 of which were high, 12 were moderate, and 15 were low. Limitations and inconsistency were the most important factors leading to degradation, followed by imprecision and publication bias. Allo-SCT had better OS and DFS benefits than auto-SCT and significantly reduced the relapse in intermediate-risk AML/CR1 patients. Auto-SCT was associated with lower TRM than allo-SCT but generally had higher relapse. The results should be confirmed further for the low or moderate evidence quality. Conclusion Current SRs show that allo-SCT in the treatment of AML might improve the OS, RFS, and DFS. Auto-SCT has significantly lower TRM but higher RR. Whether bone marrow transplantation is superior to nonmyeloablative chemotherapy remains to be evaluated. Meanwhile, the quality of methodology needs to be further improved. The intensity of evidence was uneven, and the high-quality evidence of outcomes was lacking. Considering the limitations of our overview, more rigorous and scientific studies are needed to fully explore the efficacy of different interventions of HSCT in AML, and clinicians should be more cautious in the treatment.

A phase 1 adoptive cell therapy using drug-enhanced, tumor-infiltrating lymphocytes, DeTIL-0255, in adults with advanced malignancies.

TPS5602 Background: Tumor-infiltrating lymphocytes (TIL) are a heterogeneous population of T cells that recognize multiple endogenous tumor antigens but may have developed an exhausted phenotype due to the tumor microenvironment. While existing TIL therapies produce durable responses in patients with melanoma, cervical, and head and neck cancers, poor in vitro cell expansion, limited short-lived in vivo persistence, and diminishing potency restrict this approach’s broader application. DeTIL-0255 (drug-enhanced TIL [DeTIL]) is an autologous adoptive cell therapy (ACT) derived from a patient’s tumor and expanded ex vivo with NX-0255, a small-molecule inhibitor of the E3 ligase, Casitas B-lineage lymphoma proto-oncogene B (CBL-B). CBL-B is expressed in T cells, where it functions as a regulator of immune cell activation, in part by requiring CD28 co-stimulation in addition to T-cell receptor activation. Desirable properties enhanced by DeTIL-0255 when compared with TIL include increased number of stem-like CD39-/CD69-, and CD8+ T cells associated with persistence, as well as enhanced cytolytic function. ACT with T cells expanded ex vivo using NX-0255 demonstrated increased anti-tumor activity, longer survival, increased stem-like phenotype, and persistence of tumor antigen-specific T cells in mouse tumor models. Adoptive cell transfer of DeTIL-0255, therefore, may exhibit broader functional activity than conventional TIL, potentially conferring improved anti-tumor activity and response. Methods: NX-DeTIL-0255-201 is a Phase 1 multicenter, open-label study of DeTIL-0255 administered with systemic high-dose IL-2 following nonmyeloablative lymphodepleting chemotherapy in patients with advanced gynecological malignancies for whom standard therapy with proven clinical benefit does not exist, is no longer effective, or is inappropriate. The primary objectives are to evaluate the safety, tolerability, and preliminary antitumor activity of DeTIL-0255. A safety run-in will consist of 3 to 6 patients treated with DeTIL-0255 and evaluated for dose-limiting toxicity (DLT). The DLT period starts with DeTIL-0255 infusion and ends after a total of 28 days. The safety run-in will investigate DeTIL-0255 at a dose range of 1 to 150 x 109 CD3+ T cells (exact dose varying based on expansion potential of DeTIL-0255 from tumor biopsies). Following the safety run-in, cohort expansion will further evaluate the safety and antitumor activity of DeTIL-0255 in patients with recurrent/persistent platinum-resistant epithelial ovarian cancer, cervical carcinoma, and endometrial cancer. Key eligibility criteria include measurable disease, a resectable lesion for TIL harvest, ≥2 prior lines of therapy, and an Eastern Cooperative Oncology Group performance status 0 or 1. Non-Myeloablative Chemotherapy as Consolidation Strategy After High-Dose Methotrexate-Based Chemoimmunotherapy in Patients With Primary CNS Lymphoma: A Retrospective Single Center Study in China.

Primary central nervous system lymphoma (PCNSL) remains a disease with poor outcome and high recurrence rate. We retrospectively analyzed the clinical data of 243 immunocompetent patients with PCNSL in Beijing Tiantan Hospital. The median age of PCNSL patients was 57 years (range 10-95 years). For induction therapy, 94.7% of patients received high-dose methotrexate (HD-MTX) containing regimens, and 59.3% received rituximab, which increased over time. The overall response rate was 72.8%, with 58.8% achieving complete response. With a median follow-up of 27.0 months (95% confidence interval 23.6-30.4), the median progression-free survival (PFS) time was 14.0 months (95% CI 9.45-18.55), and the 2-year PFS rate was 33.2%. The median overall survival (OS) was not reached (NR), with an estimated overall survival rate at 4 years of 61.6%. Among 95 patients who completed sequential consolidation chemotherapy with either pemetrexed or etoposide plus cytarabine, the median PFS was 28 months (95% CI 17.11-38.89), and the estimated overall survival at 4 years was 78.7%. In conclusion, HD-MTX based induction chemotherapy with non-myeloablative sequential consolidation chemotherapy is an alternative feasible treatment option.

A narrative review of consolidation strategies for young and fit patients with newly-diagnosed primary central nervous system lymphoma

Introduction The modern treatment of patients with primary central nervous system lymphoma (PCNSL) consists of two phases: induction, currently represented by a high-dose-methotrexate-based polychemotherapy, and consolidation. The optimal consolidation therapy has not been defined yet, but several strategies, such as whole-brain radiotherapy (WBRT), high-dose chemotherapy supported by autologous stem cell transplantation (HDC/ASCT) or non-myeloablative chemotherapy, have been addressed in important randomized trials. Areas covered This review provides an overview of the current role of consolidation strategies in young and fit patients with newly-diagnosed PCNSL. Publications in English language, peer-reviewed, from high-quality international journals, edited from 2003 to 2021 were identified on PubMed. Expert opinion Consolidation treatment significantly improved outcomes of PCNSL. Radiotherapy had represented for years the only choice in the consolidation therapy, but large randomized trials have demonstrated that HDC/ASCT is equally effective and associated with lower neurotoxicity risk in patients younger than 65-70 years. Encouraging results have been obtained using reduced-dose WBRT, while a recent randomized trial failed to demonstrate that consolidation with non-myeloablative chemotherapy is more effective than HDC/ASCT in PCNSL patients. A personalized consolidation treatment, driven also by a response prediction model based on radiological and molecular details, may improve the management of PCNSL patients.

Advances in treatment of elderly primary central nervous system lymphoma.

The management of older individuals (≥60years) with primary central nervous system lymphoma remains a clinical challenge. Identification of optimal therapy and delivering adequate dose intensity are two of the major issues in treating elderly patients. Premorbid performance status and comorbidities influence individualised treatment approaches and geriatric assessment tools are increasingly utilised. Optimal induction treatment remains high-dose methotrexate-based immunochemotherapy, delivery is feasible in the majority of patients and the goal of treatment remains achieving complete remission. Consolidation strategies are also relevant in the elderly, aiming to maximise duration of response and quality of life (QoL). Potential options include high-dose therapy with haematopoietic stem cell consolidation, non-myeloablative chemotherapy and whole-brain radiotherapy. Efficacy of novel agents, such as Bruton tyrosine kinase inhibitors and lenalidomide, have been reported; these represent an alternative for elderly patients unfit for chemotherapy. Prognosis remains poor, improvement of outcomes in this age group is urgently needed.

Abstract LB150: Clinical Feasibility and Treatment Outcomes with Unselected Autologous Tumor Infiltrating Lymphocyte Therapy in Patients with Advanced Cutaneous Melanoma

Abstract Introduction: A minority of patients (pts) with advanced melanoma achieve long-term survival with immunotherapy and those who relapse following immune checkpoint inhibition (ICI) or BRAFi have limited treatment options. The intrinsic anti-tumor activity and broad neoantigen-specific TCR repertoire of unselected autologous TIL may provide advantages over other treatments in solid tumors, including ICI-refractory melanoma. Methods: This is a retrospective analysis of a single-center experience of TIL for treatment of advanced cutaneous melanoma. Unselected autologous young TIL derived from digested tumor tissue were manufactured under a MHRA Manufacturing Specials license. Pts with advanced cutaneous melanoma and no other treatment options received non-myeloablative lymphodepleting chemotherapy (cyclophosphamide 60 mg/kg/d x 2d, fludarabine 25 mg/m2/d x 5 d [Cy/Flu]), followed by TIL infusion and post-TIL high-dose (HD) IL-2 (600,000-720,000 IU/Kg) on a compassionate use basis. Efficacy for 15 imaging-evaluable pts was reported by investigator assessment of CT/MRI per RECIST 1.1; 6 additional pts were followed using non-RECIST 1.1 imaging (PET) and clinical monitoring. Clinically significant adverse events (AEs) with onset post-TIL infusion were reported for all treated pts. Data cutoff date: 31-DEC-2019. Results: Between OCT-2011 and AUG-2019, 21 pts with advanced cutaneous melanoma were treated with Cy/Flu, TIL (median 31.9 x109 cells infused), and HD IL-2 (median 8 doses). All had high-risk metastatic disease (median 4 sites, stage IV M1d [38%]), an average of 3 prior therapies (any ICI [91%], PD-1 [57%], BRAFi [52%], and MEKi [24%]). With a median follow up of 52.2 mos, the response rate in imaging-evaluable pts (n=15) was 53% with a CR rate of 13%; DCR was 73% including 3 (20%) pts with stable disease. Additional durable responses were observed in the 6 pts followed by PET and clinical monitoring. Responses were generally consistent across subgroups including age, number of disease sites, tumor burden, brain metastases, number of prior lines of therapies, prior PD-1 blockade, prior BRAFi, and prior MEKi. For all treated pts, the median survival was 21.3 mos. Toxicity was generally self-limited and consistent with Cy/Flu and HD IL-2. Common AEs (≥20%, any grade) were thrombocytopenia (62%), pyrexia (57%), rigors (43%), neutropenia (29%), tachycardia (29%), pulmonary edema (24%), and vascular leak (24%); no treatment-related deaths were observed. Conclusion: The high response rate observed in this series exceeds the 41% ORR estimated for TIL in advanced cutaneous melanoma (meta-analysis by Dafni et al. Ann Oncol., 2019) and highlights the successful bench-to-bedside application of unselected autologous TIL to address unmet medical need in advanced melanoma. Use of tumor digests as starting material for manufacturing of TIL demonstrates feasibility of this approach. A multicenter Phase 2 trial of this therapy in advanced melanoma is planned for 2021. Myeloablative versus non-myeloablative consolidative chemotherapy for newly diagnosed primary central nervous system lymphoma: Results of CALGB 51101 (Alliance).

7506 Background: Optimal consolidative therapy for primary central nervous system lymphoma (PCNSL) is not defined. Avoidance of whole brain radiation may reduce risk of neurotoxicity. Non-radiation consolidative options include myeloablative chemotherapy with autologous stem cell transplantation (HDT/ASCT) or non-myeloablative chemotherapy. Methods: This is a randomized phase 2, National Clinical Trials Network study of induction methotrexate (MTX) (8 g/m2 days 1, 15), temozolomide (TMZ) (150-200 mg/m2 D7-11), and rituximab (RTX) (C1 D3, 10, 17, 24 and C2 D3, 10 ) in four 28-day cycles followed by one cycle of cytarabine (ARA-C) (2 g/m2 BID, D1, 2) (MTRA). After induction, patients (pts) received consolidation with thiotepa (5 mg/kg BID, D -5, -4) plus carmustine (400 mg/m2, day -6) and ASCT (Arm A) or one cycle of ARA-C (2 g/m2 BID, D1-4) plus infusional etoposide (40 mg/kg over 96h) (Arm B). Pts were stratified on age and performance status and randomized 1:1 before induction. The primary endpoint was progression-free survival (PFS) from randomization. With 110 pts, there was 84% power to detect an improvement in PFS using a log-rank test (1-sided α= 10%), assuming a median PFS of 3 months for pts who progress during induction, and a median PFS of 2 years (yrs) for Arm B and 4.5 yrs for Arm A consolidation. This report includes the results for the primary endpoint analysis. Results: 113 pts (median age 61 yrs, range 33-75) were randomized (Arm A: 57, Arm B: 56) across 27 centers. 108 eligible pts who received induction therapy were included in the primary endpoint analysis (Arm A: 54, Arm B: 54). 72/108 pts started consolidation and 70/72 completed consolidation per protocol (Arm A: 36, Arm B: 34). With a median follow-up of 3.8 years, median PFS from randomization was 6 yrs (95% CI 3.9-not reached) in Arm A vs 2.4 yrs (95% CI 0.6-not reached) in Arm B (p = 0.02). However, more pts randomized to Arm B went off treatment before consolidation due to progression or death (28% vs 11%, p = 0.05). PFS landmarked at start of consolidation demonstrated a trend for improved PFS favoring Arm A (HR 0.58, 95% CI 0.25-1.36; p = 0.21). Median OS was not reached in either arm, and 3-yr estimates were 83% (95% CI 69-91; Arm A) vs 72% (95% CI 57-82; Arm B). Toxicities were similar between arms with no treatment-related mortality during consolidation. Conclusions: MTRA induction followed by myeloablative consolidation (Arm A) had improved PFS vs MTRA induction followed by non-myeloablative consolidation (Arm B), though more progressions or deaths leading to treatment discontinuation prior to consolidation in Arm B were noted. Bone marrow toxicity and immune reconstitution in melanoma and non-melanoma solid cancer patients after non-myeloablative conditioning with chemotherapy and checkpoint inhibition

Background aimsLymphodepletion with non-myeloablative (NMA) chemotherapy is currently a prerequisite for adoptive cell therapy (ACT). ACT based on tumor-infiltrating lymphocytes has long been used in malignant melanoma (MM), but with the advance of ACT into new cancer diagnoses, the patient predisposition will change. The authors here evaluate the bone marrow (BM) toxicity of NMA in combination with checkpoint inhibition and a priori risk factors in a wide range of cancer diagnoses. MethodsThirty-one non-MM and MM patients were included from two different clinical trials with ACT. The treatment history was extracted from the medical records, together with the hematology data. Immune monitoring with flow cytometry was performed before and at several time points after therapy. ResultsNMA induced reversible myelosuppression in all patients. No significant differences in BM toxicity between MM and non-MM patients were found. The overall hematology counts were reconstituted within 3–6 months but with great individual heterogeneity, including eight patients who developed a second phase of neutropenia after hospital discharge. A performance status >0 was found, and shorter overall survival and sex were statistically associated with longer duration of anemia. By contrast, high expression of co-stimulatory markers CD28+ and CD27+ on T cells at baseline was significantly correlated with shorter duration of neutropenia (P = 0.010 and P = 0.009, respectively), anemia (P = 0.001 and P = 0.001, respectively) and thrombocytopenia (P = 0.017 and P = 0.030, respectively). In addition, following NMA, the authors saw a significant differentiation of T-cell phenotype associated with old age. ConclusionsACT with NMA and checkpoint inhibition is tolerable in patients with multiple cancer diagnoses and therapy backgrounds but comes with substantial transient BM toxicity that is comparable in both non-MM and MM patients. Baseline T-cell CD28/CD27 expression level is predictive of duration of BM toxicity. Furthermore, NMA conditioning induces changes in the immune system that may affect a patient's immunocompetence for many months following therapy.

Survival in patients with high-risk neuroblastoma treated without autologous stem cell transplant or dinutuximab beta

The majority of patients with high-risk neuroblastoma (HR-NB) in low- and middle-income countries (LMIC) do not have access to autologous stem cell transplant (ASCT) and dinutuximab. Consolidation with nonmyeloablative chemotherapy is not well-defined, and the outcomes are variable. We report a single-center outcome of patients with HR-NB, treated with nonmyeloablative consolidation. A tabulated compilation of similar reports is included. A retrospective chart review of patients with HR-NB was performed from January 2009 till June 2016. Patients were treated on the backbone of HR-NBL1/SIOPEN protocol. Treatment included induction with rapid-COJEC, surgery, followed by consolidation. Consolidation involved 4 cycles of topotecan, vincristine, and doxorubicin (TVD) instead of ASCT. Infusion of vincristine and doxorubicin were modified for ease and to enable administration in the clinic. Subsequent treatment included radiotherapy to the primary tumor and differentiation therapy with isotretinoin. Over 7½ years, 28 patients with HR-NB were treated. Two (7%) patients had therapy-related mortality. A relapse or disease progression occurred in 11 (39%) patients at a median duration of 17 months (IQR: 5, 18). Treatment abandonment was observed in 4 (14%) patients. The median follow-up of disease-free patients was 49 months (IQR: 45, 79). Patients with relapse were not treated further. A 4-year EFS of 29.3% was observed when 4-cycles of TVD were administered instead of ASCT in patients with HR-NB. The study and the review will aid decision-making for care of patients in LMIC while considering the options of treatment for HR-NB if access to ACST and dinutuximab is lacking.

SS-5 Current management of primary central nervous system lymphoma

Primary CNS Lymphomas (PCNSLs) is a highly aggressive malignant tumor with poor prognosis and increasing incidence in elderly patients. High-dose methotrexate (HD-MTX) followed by whole-brain radiation therapy (WBRT) improves survival in PCNSLs. Several HD-MTX–based regimens, in combination with alkylating agents and rituximab, have been developed that can achieve high and durable complete response rates in patients with newly diagnosed PCNSL. In Japan, the R-MPV regimen using rituximab, HD-MTX, procarbazine, and vincristine has been recognized as the standard treatment for initial induction for newly diagnosed PCNSL. The optimal consolidative therapy for patients with disease responsive to induction chemotherapy is not yet defined. WBRT at standard dose (30–45 Gy) has a risk of neurotoxicity. To minimize the effects of delayed neurotoxicity, high-dose chemotherapy supported by autologous stem cell transplantation, reduced dose WBRT (23.4Gy), non-myeloablative chemotherapy, and maintenance chemotherapy have been addressed in large randomized trials. Gene expression profiling has provided insights into the pathogenesis of PCNSL. Recent insight into the pathophysiology of PCNSL has led to the investigation of targeted agents in the treatment of recurrent disease. In March 2020, Tirabrutinib (TIR), a second-generation oral Bruton’s tyrosine kinase inhibitor, was approved for relapsed or refractory PCNSL based on the results of the phase I/II study in Japan. Seventeen of 44 patients treated with TIR at 480 mg fasted QD, an approved dose, had overall response rate of 52.9%, median progression-free survival of 5.8 months, and time to response as short as 0.92 months. The most common adverse event at any grade was rash (32%). The skin-related disorders were manageable with appropriate skin treatments. However, greater attention and management is needed the case of more rare adverse events such as severe skin-related disorders and pneumocystis pneumonia. This lecture aims to present the recent development in treatment for PCNSL.

Grade 3 or 4 febrile neutropenia occurred in 12 pts (11.1%) during induction. Dose modifications of MTX were required in 75% of pts and 63.3% of cycles, mainly due to renal adjustments. Dose delays of MTX were required in 52.8% of pts and 22.2% of cycles. Overall response rate (CR, CRu, PR) at the end of induction was 65.7% (95% CI, 56%, 74.6%). Conclusions: While MTRA is feasible and active a significant proportion of pts did not receive consolidation, supporting the need to develop more effective induction strategies.
