Understanding complex interactions between neighborhood environment and personal perception in affecting walking behavior of older adults: A random forest approach combined with human-machine adversarial framework

The population aging is a growing problem worldwide. Walking is one of the most important ways of self-management of health for older adults, determined by many factors, such as neighborhood environment (NE) and socio-economic attributes. Although the previous studies have typically predicted elderly walking behavior through NE, they are limited by the methodological system and data collection, resulting in low prediction accuracy. To this end, this study incorporates residents' subjective perceptions of the environment and objective neighborhood environmental attributes into the evaluation system, uses human-machine adversarial framework and machine learning methods to predict elderly walking behavior, and assesses the nonlinear effects of each factor. The results show that (1) combining subjective and objective factors, the prediction accuracy of elderly walking behavior has been effectively improved based on human-machine adversarial framework and machine learning methods. (2) The nonlinear and threshold effects of environmental and perceptual factors on the walking time of the elderly were revealed. (3) The neighborhood attributes were incorporated into the walking behavior prediction, and were found to be of comparable importance to the influence of the NE on the behavior of the elderly. These results provide more reliable qualitative and quantitative auxiliary suggestions for planners.

Associations of neighbourhood environmental attributes and socio-economic status with health-related quality of life in urban mid-aged and older adults: Mediation by physical activity and sedentary behaviour

This study examined the associations of objectively assessed physical features of the neighbourhood environment with physical and mental aspects of health-related quality of life (HRQoL) as measured by the SF-36, and the roles of physical activity and sedentary behaviour in these associations. We used data from a national sample of Australian mid-aged and older adults living in urban areas (N = 4141). Environmental attributes were computed for 1-km-radius areas surrounding participants' residential addresses. Neighbourhood socio-economic status (SES) and average annual concentrations of PM2.5 were the only attributes related to HRQoL in the expected direction in the total- and direct-effect regression models. All other environmental attributes were related to HRQoL via physical activity behaviours and leisure-time sitting. The associations of most environmental features with HRQoL mediated by physical activity and sedentary behaviours were inconsistent, positive through some pathways and negative through others. This study suggests that neighbourhood SES may in part benefit HRQoL by helping promote an active lifestyle. Neighbourhood attributes defining walkability may benefit HRQoL by providing opportunities for walking and resistance training and, through these, by helping reduce leisure-time sitting. However, the same attributes also may limit opportunities for household activities and gardening and negatively impact on HRQoL through these pathways.

How the perceived neighbourhood environment influences active living in older dwellers of an Asian ultra-dense metropolis

The way older adults perceive their neighbourhood environment may determine their levels of physical activity. We examined the associations of perceived neighbourhood environmental attributes with accelerometry-assessed and self-reported physical activity and sedentary time in Hong Kong Chinese older adults. In doing so, we estimated the inter-relationships among perceived environmental attributes, the mediating role of physical activity in the environment-sedentary time nexus and the moderating role of sex. We used data from the Active Lifestyle and the Environment in Chinese Seniors (ALECS) project collected on older adults (N = 909; ≥65 years) living in neighbourhoods varying in walkability and socio-economic status (71 % response rate). Self-reported physical activity and perceived neighbourhood attributes were assessed with validated questionnaires. Accelerometer-based physical activity and sedentary time were collected in 402 participants. Older adults who perceived their neighbourhood to be walkable, safe, aesthetically pleasing and equipped with public sitting facilities engaged in more physical activity and less sedentary time. Curvilinear relations of perceived residential density and activity-friendly urban design features indicated that extreme levels of density may not be optimal for older adults to adopt an active lifestyle because they do not provide sufficient space for sitting facilities and greenery and do not result in better perceptions of neighbourhood walkability when compared to areas with moderate-to-high levels of density. Creative solutions for the incorporation of greenery and public places for sitting in megacities are needed.

Neighborhood environment and quality of life among community-living older adults in Nigeria: The moderating effect of physical activity

Understanding how neighborhood environments are related to older adults’ quality of life (QoL) and physical activity (PA) is important for public health actions on healthy ageing in sub-Saharan Africa. We examined associations of perceived neighborhood environment attributes with QoL among older adults in Nigeria and investigated the moderating effects of PA on these associations. We conducted a cross-sectional study of 353 older adults (mean age = 68.9 ± 9.1 years) selected from 5 high- and low-income communities in Maiduguri, Nigeria. QoL, attributes of the neighborhood environments and PA were self-reported using validated questionnaires. Multi-level models were used to examine the direct associations between neighborhood environment attributes and each of the four domains of QoL (physical health, psychological health, social relationships, and environmental health), as well as the moderating effects of leisure-time and total PA. Seven of nine neighborhood environment features were positively associated with multiple domains of QoL. Residential density, land-use diversity, land-use mix-access, walking infrastructure, traffic safety and ‘overall walkability’ were positively related to both or either physical health and environmental health QoL among those who are physically active. In contrast, walking infrastructure, traffic safety, and ‘overall walkability’ were negatively related to psychological health QoL among those not physically active. Our findings suggest being physically active moderates the association of neighborhood environments with QoL among Nigerian older adults. We suggest that designing age-friendly communities and simultaneously promoting PA may be needed to improve QoL and help prepare the Nigerian society for the predicted increase in the older adult population.

The Role of Social Determinants of Health in Cardiovascular Diseases: An Umbrella Review.

Cardiovascular disease (CVD) is the leading cause of mortality worldwide. Addressing social determinants of health (SDoH) may be the next forefront of reducing the enormous burden of CVD. SDoH can be defined as any social, economic, or environmental factor that influences a health outcome. Comprehensive evidence of the role of SDoH in CVD is lacking, nevertheless. This umbrella review aims to give a comprehensive overview of the role of SDoH in CVD. We searched systematic reviews (with or without meta-analyses) using 8 databases and included review reference lists. Four themes (economic circumstances, social/community context, early childhood development, and neighbourhood/built environment) and health literacy in the health/health care theme were considered. Seventy reviews were eligible. Despite the quality of the included reviews being low or critically low, there was consistent evidence that factors relating to economic circumstances and early childhood development themes were associated with an increased risk of CVD and CVD mortality. We also found evidence that factors in the social/community context and neighbourhood/built environment themes, such as social isolation, fewer social roles, loneliness, discrimination, ethnicity, neighborhood socioeconomic status, violence, and environmental attributes, had a role in CVD. SDoH factors without (or with minimal) evidence synthesis for CVD were also identified. In sum, this umbrella review offers evidence that SDoH, especially economic circumstance and early childhood development, play a significant role in CVD. This calls for the strengthening of nonmedical interventions that address multiple factors simultaneously and the inclusion of SDoH in future CVD risk prediction models. Associations between neighborhood environment and sense of community belonging in urban China: Examining mediation effects of neighborly interactions and community satisfaction.

Improving sense of community belonging (SCB) would help people live longer, happier lives. Although the importance of neighborhood environment on SCB is stressed in the literature, few studies have paid attention to perceived environment, as well as consider mediation effects such as neighborhood social interactions and place satisfaction. Relied on a sample of 1051 respondents in Shanghai in 2018, this study investigates the associations between both objective and perceived neighborhood environment attributes and SCB in urban China, mediated by neighborly interactions and community satisfaction using structural equation modeling. The results suggest that the influence of perceived neighborhood environment on SCB is more prominent than that of objective neighborhood environment. In detail, perceived pedestrian facilities and perceived leisure facilities are vital to SCB, while among objective neighborhood environmental elements, the influence of land use entropy, park density and street greenery are significant. Then, neighborhood environmental attributes can influence SCB by affecting neighborly interactions and community satisfaction. We also identify gender differences in the effects of neighborhood environment upon SCB. Given increasing awareness of the connection, neighborhood environment may prove to be valuable assets to improve individuals' psychosocial constructs such as SCB.

On pathways and agreement: Objective and perceived accounts of neighbourhood attributes and their associations with mental health during pregnancy

There is growing interest in understanding the links between neighbourhood environmental attributes (i.e. greenness, walkability and air pollution) and human health. Recent research has analysed the mediating role of a diverse set of potential factors and studied the agreement between objective and perceived modalities of those attributes. In this study, we explored the connections between objective neighbourhood attributes, their perceived accounts and mental health during pregnancy, using a measure of social cohesion as potential mediator with data from two samples of pregnant women recruited during the 12th week of pregnancy in two Spanish cities (Donostialdea, n = 440; Barcelona n = 364). Besides, we ran analyses on the agreement between objective and perceived measures. We fitted four separate Structural Equation Models and detected associations between objective neighbourhood attributes and mental health occurred only through their perceived counterparts and the strengthening of social cohesion. We also found poor to fair agreement between greenness measures in both cities, walkability measures only in Donostialdea, and were unable to detect any meaningful agreement between air pollution variables. Using rescaled versions of neighbourhood attribute variables and in some instances, we saw that the higher the objective value of a given attribute, the larger the differences between objective and perceived accounts of such attributes.

The neighbourhood environment and profiles of the metabolic syndrome

BackgroundThere is a dearth of studies on how neighbourhood environmental attributes relate to the metabolic syndrome (MetS) and profiles of MetS components. We examined the associations of interrelated aspects of the neighbourhood environment, including air pollution, with MetS status and profiles of MetS components.MethodsWe used socio-demographic and MetS-related data from 3681 urban adults who participated in the 3rd wave of the Australian Diabetes, Obesity and Lifestyle Study. Neighbourhood environmental attributes included area socio-economic status (SES), population density, street intersection density, non-commercial land use mix, percentages of commercial land, parkland and blue space. Annual average concentrations of NO2 and PM2.5 were estimated using satellite-based land-use regression models. Latent class analysis (LCA) identified homogenous groups (latent classes) of participants based on MetS components data. Participants were then classified into five metabolic profiles according to their MetS-components latent class and MetS status. Generalised additive mixed models were used to estimate relationships of environmental attributes with MetS status and metabolic profiles.ResultsLCA yielded three latent classes, one including only participants without MetS (“Lower probability of MetS components” profile). The other two classes/profiles, consisting of participants with and without MetS, were “Medium-to-high probability of high fasting blood glucose, waist circumference and blood pressure” and “Higher probability of MetS components”. Area SES was the only significant predictor of MetS status: participants from high SES areas were less likely to have MetS. Area SES, percentage of commercial land and NO2 were associated with the odds of membership to healthier metabolic profiles without MetS, while annual average concentration of PM2.5 was associated with unhealthier metabolic profiles with MetS.ConclusionsThis study supports the utility of operationalising MetS as a combination of latent classes of MetS components and MetS status in studies of environmental correlates. Higher socio-economic advantage, good access to commercial services and low air pollution levels appear to independently contribute to different facets of metabolic health. Future research needs to consider conducting longitudinal studies using fine-grained environmental measures that more accurately characterise the neighbourhood environment in relation to behaviours or other mechanisms related to MetS and its components.

Physical inactivity and perceived environmental factors: a cross-sectional study among civil servants in Abia State, Southeastern Nigeria.

Introductionphysical inactivity has been identified as the fourth leading risk factor for global mortality due to non-communicable diseases. Prevalence rates of 91.0% and 62.2%, have been documented among civil servants in the northern and southern parts of Nigeria, respectively. There is a paucity of data regarding the relationship between physical inactivity and environmental factors among civil servants in the State. This study assessed the prevalence and perceived environmental factors associated with physical inactivity among civil servants in Abia State Nigeria.Methodswe conducted a cross-sectional study in which we recruited 440 civil servants using a multistage sampling technique. We used an interviewer-administered structured questionnaire to collect data on sociodemographic, physical activity, and neighbourhood environmental attributes. Descriptive, bivariate and multivariate analysis were done. The level of significance was set at 5%.Resultsthe mean age of the respondents was 39.0±9.2 years, and 61% were females. The prevalence of physical inactivity was 48.4% (95%CI: 43.7%-53.2%). The putative environmental factors included perceptions of low residential density areas, perceived absence of neighbourhood sidewalks, perceived unavailability of bicycling facilities and the perception of an unsafe neighbourhood due to night crimes. The predictor of physical inactivity was the perceived absence of neighbourhood sidewalks (aOR=2.02, 95%CI: 1.10-3.73).Conclusionin this study, physical inactivity is prevalent among civil servants in Abia State. The need for the stakeholders in collaboration with the Ministry of Environment to focus on the provision of sidewalk facilities, layout of residential areas and limit security risks in the State to enhance physical activity is highlighted.

From urban neighbourhood environments to cognitive health: a cross-sectional analysis of the role of physical activity and sedentary behaviours

BackgroundThere is a dearth of studies on the effects of the neighbourhood environment on adults’ cognitive function. We examined how interrelated aspects of the built and natural neighbourhood environment, including air pollution, correlate with adults’ cognitive function, and the roles of physical activity and sedentary behaviours in these associations.MethodsWe used data from 4,141 adult urban dwellers who participated in the Australian Diabetes, Obesity and Lifestyle 3 study on socio-demographic characteristics, neighbourhood self-selection, physical activity and sedentary behaviours, and cognitive function. Neighbourhood environmental characteristics included population density, intersection density, non-commercial land use mix, and percentages of commercial land, parkland and blue space, all within 1 km residential buffers. We also calculated annual mean concentrations of NO2 and PM2.5. Generalised additive mixed models informed by directed acyclic graphs were used to estimate the total, direct and indirect effects of environmental attributes on cognitive functions and the joint-significance test was used to examine indirect effects via behaviours.ResultsIn the total effects models, population density and percentage of parkland were positively associated with cognitive function. A positive association of PM2.5 with memory was also observed. All neighbourhood environmental attributes were directly and/or indirectly related to cognitive functions via other environmental attributes and/or physical activity but not sedentary behaviours. Engagement in transportation walking and gardening frequency partially mediated the positive effects of the neighbourhood environment on cognitive function, while frequency of transportation walking mediated the negative effects.ConclusionsIn the context of a low-density country like Australia, denser urban environments with access to parkland may benefit residents’ cognitive health by providing opportunities for participation in a diversity of activities. A more fine-grained characterisation of the neighbourhood environment may be necessary to tease out the negative and positive impacts of inter-related characteristics of urban neighbourhood environments on cognitive function.

Does neighborhood built environment support older adults' daily steps differ by time of day?

BackgroundNeighborhood environmental attributes can promote walking behavior among older adults. Nevertheless, these walkability attributes may affect walking behavior differently depending on the time of the day. This study examined the association between neighborhood environmental attributes and average step counts at different times of the day. MethodsWe included 122 older adults (aged ≥ 60 years, mean age: 70.0 years) from Taiwan in this study. Neighborhood environmental attributes were estimated using a geographic information system. An accelerometer was used to calculate their daily step counts during three time periods: morning (6:00 to 12:00), afternoon (12:01 to 18:00), and night (18:01 to 24:00). In this cross-sectional study, we performed multiple linear regression after adjustment for potential covariates to examine the correlation of five walkability attributes, namely residential density, street connectivity, sidewalk availability, destination accessibility, and public transportation accessibility, with total step counts during different time periods. ResultsSidewalk availability, destination accessibility, and public transportation accessibility demonstrated a positive association with older adults' step counts during morning. No association was found between any neighborhood environment attribute and older adults’ total step count during afternoon and night. ConclusionsNeighborhoods with high sidewalk availability, access to destinations, and public transportation accessibility may encourage older adults' walking behavior during morning. Future research applying longitudinal methods is required to understand how the time of the day affects environment's role in supporting walking behavior.

Associations of neighborhood built and social environments with frailty among mid-to-older aged Australian adults.

Neighborhood environmental attributes are associated with physical activity and health status. We examined cross-sectional associations of built and social environmental attributes with frailty among community-dwelling mid-to-older aged adults in Australia. Data from 3419 adults aged 50-74 years living in 200 neighborhoods in Brisbane, Australia, were used. Frailty status was assessed by a frailty index comprising 32 items. The built environment attributes examined were residential density, street connectivity, land use mix, park area and bus stop density. Self-reported levels of safety and social cohesion were used as social environmental factors. Associations were examined using multilevel logistic regression. The prevalence of frailty was 12%. One standard deviation increment in street connectivity in a 1-km network buffer around participants' homes and in a neighbourhood was associated with 23% and 13% higher odds of being frail, respectively. One standard deviation increment in land use mix at the neighborhood level was associated with 12% lower odds of being frail. Higher levels of safety and social cohesion were consistently associated with a lower odds of being frail. Consistent with previous studies, we found that social environmental attributes were relevant to residents' frailty status, whereas the relationships between built environmental attributes and frailty were not clear (frailty was associated with lower street connectivity but with higher land used mix). Community-level initiatives to enhance safety and social cohesion might be effective to reduce frailty. Further research is required to explain the equivocal findings obtained for built environmental attributes and frailty. Geriatr Gerontol Int 2021; 21: 893-899.

Are Changes in Walking for Transportation in Brazilian Older Adults Associated With Attributes of the Neighborhood Environment?

This study aims to examine the association between neighborhood environment attributes and changes in walking for transportation among older adults. Longitudinal analysis was performed considering a population-based study (EpiFloripa Idoso), carried out in 2009-2010 with follow-up in 2013-2014. Changes in walking, obtained with the International Physical Activity Questionnaire during both waves were associated with data from the environment perception, evaluated using individual items from the Neighborhood Environment Walkability Scale (baseline only) performing multinomial logistic regression. A total of 1,162 older adults (65.2% women, mean age = 73.7 years) participated. Those who reported the presence of parks and squares (OR = 2.44, 95% confidence interval [CI; 1.70, 3.51]), sidewalks (OR = 1.66, 95% CI [1.03, 2.70]), crosswalks (OR = 1.69, 95% CI [1.05, 2.72]), illuminated streets (OR = 2.80, 95% CI [1.24, 6.33]), and safety for day walks (OR = 1.93, 95% CI [1.14, 3.24]) were more likely to remain active or become active when commuting (≥150min/week). Older adults are more active in neighborhoods that present more favorable attributes regarding walking for transportation.

Neighborhood environmental attributes and walking mobility decline: A longitudinal ecological study of mid-to-older aged Australian adults.

ObjectivesCross-sectional studies have found some built environmental attributes to be associated with residents’ lower levels of mobility (functional capacity to walk outside the home). However, less is known about what environmental attributes are related to mobility decline. This longitudinal study examined area-level associations of specific environmental attributes with mid-to-older aged adults’ changes in walking mobility.MethodsData collected from 4,088 adults (aged 46–71 years at baseline) who participated in a cohort study in Brisbane, Australia were used. The outcome was the change in self-reported mobility score (SF-36) from 2013 to 2016, which were aggregated at the neighborhood (N = 156) and suburb (N = 99) levels, due to the known lack of sensitivity in SF-36 subscales to individual changes. Linear regression analysis examined associations of mobility change with seven environmental attributes measured at baseline (residential density, intersection density, land use mix, density of walking/bike paths, park density, bus stop density, density of social incivilities), adjusting for confounding variables.ResultsParticipants on average reported 4% of mobility decline during the 3-year study period. It was found that greater land use diversity was consistently associated with less decline in walking mobility, while greater density of social incivilities was associated with more decline in walking mobility. The latter finding was significant only at the neighborhood level. No consistent associations were observed for residential density, intersection density, density of walking/bike paths, park density, and bus stop density.DiscussionOur findings suggest that mid-to-older aged adults who live in areas with lower land use diversity and more social incivilities may be at risk of developing mobility limitations. Recommended policies to slow residents’ mobility decline and to achieve aging in place include improving these environmental attributes where needed and advising older adults to relocate to safer, mixed-use neighborhoods.

Development of Measures of Perceived Neighborhood Environmental Attributes Influencing, and Perceived Barriers to Engagement in, Healthy Behaviors for Older Chinese Immigrants to Australia.

Environmental correlates, barriers, and facilitators of physical activity, healthy eating, and socializing are understudied in older immigrants to developed countries. This study developed/adapted and validated measures of perceived barriers and neighborhood environmental characteristics related to these health-enhancing behaviors appropriate for older Chinese immigrants to Australia and similar Western countries. Older Chinese immigrants living in Melbourne (Australia) were recruited from neighborhoods varying in walkability and percentage of Chinese residents. Versions of the Neighborhood Environment for Healthy Aging–Chinese Immigrants to Australia (NEHA-CIA) questionnaire (20 subscales) and the Perceived Barriers to Health-Enhancing Behaviors questionnaire (four subscales) were developed from extant validated scales and information collected in formative qualitative research. Thirty-one participants took part in cognitive interviews aimed to pilot-test and refine the questionnaires. The modified questionnaires were administered to 52 participants twice, two weeks apart. Test-retest reliability (intraclass correlation coefficients), internal consistency (Cronbach’s α), and construct validity (associations with theoretically-relevant constructs) were examined. Most items and subscales of both questionnaires had good test-retest reliability and internal consistency, while the NEHA-CIA also showed good construct validity. Future studies need to further examine the construct validity of the questionnaire of perceived barriers and determine the factorial validity of both measures on large representative samples.

Neighborhood Environment and Objectively Measured Sedentary Behavior Among Older Adults: A Cross-Sectional Study.

Background: We examined the relationships between objectively assessed neighborhood environment and the patterns of sedentary behavior among older adults.Methods: A total of 126 community-dwelling older adults (aged 65 years or above) were recruited. Data on neighborhood environmental attributes (resident density, street intersection density, sidewalk availability, accessible destinations, and accessible public transportation), accelerometer-assessed total time and patterns of sedentary behavior (number and duration of bouts), and sociodemographic characteristics were collected. Multiple linear regression models were developed.Results: After adjustment for potential confounders, greater sidewalk availability was negatively related to the number of sedentary bouts (β = −0.185; 95% CI: −0.362, 0.015; p = 0.034) and sedentary bout duration (β = −0.180; 95% CI: −0.354, −0.011; p = 0.037).Conclusions: This study revealed that a favorable neighborhood environment characterized by sidewalk availability is negatively associated with sedentary behavior patterns in Taiwanese older adults. These findings are critical to inform environmental policy initiatives to prevent sedentary lifestyle in older adults.

Correlates of frequency of outdoor activities of older adults: Empirical evidence from Dalian, China

Identifying attributes of neighborhood environment that influence the frequency of participation in type-specific outdoor activities of older adults is instrumental in developing tailored interventions for promoting healthy aging. The aim of this study is to examine the association between neighborhood characteristics and the frequency of type-specific outdoor activities. Data were collected through a survey among 363 older adults aged 60 years and over living in Dalian, China, in 2017. A zero-inflated count modeling approach was applied. Older adults who perceive accessibility to local shops as over 15 min are likely to conduct less utilitarian activities. A distance of less than 800 m to the nearest park has a negative effect on the frequency of participation in sedentary activities for older adults and a distance of 800–1200 m is likely to support those who frequently engage in leisure walking. High social capital is positively associated with the frequency of participation in leisure walking and skill-based leisure activities. The findings of this study are helpful for urban planners and designers and policy makers who wish to create physical activity-supportive and inclusive neighborhood environments for older adults as a heterogeneous population with varied activity patterns.

Urban environments and objectively-assessed physical activity and sedentary time in older Belgian and Chinese community dwellers: potential pathways of influence and the moderating role of physical function

BackgroundMany studies have examined neighbourhood environmental correlates of older adults’ physical activity (PA) but only a few focused on sedentary time (ST). Only a small proportion of these studies used objective measures of PA/ST, such as accelerometer-assessed PA/ST, and only a couple employed accelerometer cut-points appropriate for older adults. Furthermore, although older adults experience declines in physical function as they age, there is a dearth of information on the impact of the neighbourhood environment on PA/ST in individuals with different levels of physical function.MethodsWe used data from two extant cross-sectional studies conducted in Hong Kong (China) and Ghent (Belgium) (N = 829). Participants were recruited from pre-selected administrative units stratified by socio-economic status and walkability. Moderate-to-vigorous PA (MVPA) and ST were assessed for 7 days using accelerometers and cut-points developed for older adults. Objective neighbourhood environmental attributes within 400 m and 1 km buffers surrounding participants’ homes were quantified using Geographic Information Systems data. Lower extremity physical function was objectively assessed. Socio-demographic information was collected via interviews. Total, direct and indirect (mediated) effects of environmental attributes on MVPA and ST were estimated using generalised additive mixed models and the joint-significant test.ResultsCommercial/civic destination density and number of parks within 1 km from home showed positive total and direct effects on MVPA, and public transport density showed negative total and direct effects on ST, which were consistent across cities and physical function levels. The total and direct effects of residential density on MVPA depended on physical function, and those of residential density on ST differed by city. A complex network of potential inconsistent pathways linking all environmental attributes to MVPA and ST in the whole sample or in subgroups of participants was revealed.DiscussionAccess to parks and commercial/civic destinations appear to support older adults’ MVPA in different geographical and cultural contexts and irrespective of their physical function level. By supporting MVPA, these characteristics also contribute to a reduction in ST. The potential effects of public transport, recreational facilities and residential density are less straightforward and point at inconsistent effects that may depend on the geographical context and level of physical function.
