Integrated immunology in Colorado

Establishment of the Rocky Mountain region as a hub forimmunology research began in the nineteenth and twenti-eth centuries with the arrival of tuberculosis patients sentfor ‘‘clean air and sunshine’’ therapy. Gerald Webb, a pupilof Sir Almroth Wright—an early developer of typhoidvaccines and a strong proponent of immunization—movedto Colorado to be with his wife who had contractedtuberculosis and to complete his medical education at theUniversity of Denver. In June 1913, a small group ofdoctors from the United States and Canada, who hadtrained in London with Wright, met to consider thefounding of a society. Wright had famously said that ‘‘thephysician of the future will be an immunisator.’’ Webb,who had become a clinician–immunologist in ColoradoSprings, insisted that ‘‘immunologist’’ was a more inclu-sive term. Thus, Webb protected us from celebrating the100th birthday of the American Association of Immuni-sators in 2013. He was elected the first President of theAAI, a reflection of the prominence of the Colorado groupeven in the early history of American immunology [1].Many of the Colorado hospitals and research institutesbegan as tuberculosis sanatoria. The physicians who caredfor tuberculosis patients were impressed with how promi-nent a role the immune response played in both the controland the symptomatology of the disease. With the advent ofstreptomycin in the 1940s, the need for specialized sana-toria waned, and some of them were converted into hos-pitals for the treatment of another prominent pulmonaryimmunologic disease, asthma. National Jewish Hospital forConsumptives, the precursor of National Jewish Health,was founded in 1899. The Denver Sheltering Home forJewish Children, the precursor of the National AsthmaCenter, was established in 1907 as a home for the childrenof tuberculosis patients. These two sites merged in 1978and continue to lead in respiratory care and immunologyresearch today. This was where Kimishige and TerukoIshizaka identified IgE as the antibody isotype that medi-ates allergy in 1966 [2]. In that same year, Henry Claman atthe University of Colorado School of Medicine observedthe synergistic interaction between thymocytes and bonemarrow cells for the production of antibody, thus identi-fying for the first time the helper T cell and T-B collabo-ration [3].Both the Ishizakas and Claman were recruited to Col-orado by David Talmage, who was credited as the man whobrought modern immunology to Denver; he formulatedwhat became known as the clonal selection theory in 1957[4, 5]. Talmage arrived at the University of Colorado in1959. He encouraged the growth of immunology at all ofCU’s affiliated institutions, and began the process wherebynew recruits in these institutions would also be appointedas regular CU faculty. This policy of inclusiveness led toremarkable growth of the Denver immunology group inseveral locations, and the eventual separation of the dis-cipline from the Department of Microbiology and the for-mation of the Integrated Department of Immunology in1993. Alexithymia and chronic respiratory disease. A review of current research.

Alexithymia within a chronic respiratory illness population has been given considerable attention in the psychiatric/psychological literature over the course of the past 5 years. Various observations as to the alexithymic trait within the chronic respiratory diseases (asthma, chronic bronchitis/emphysema, and tuberculosis) have revealed significant findings as to the manner in which alexithymia manifests itself and impacts on chronic respiratory disease. The present paper reviews the literature on alexithymia that has emanated from National Jewish Hospital and Research Center/National Asthma Center and raises questions for future research as regards alexithymia in chronic respiratory diseases.

Some 430 physicians are assembled in a Chicago hotel meeting room taking the American College of Chest Physicians' (ACCP) latest course on pulmonary medicine. All are in their seats by 8 AM, and they often are still in those chairs up to ten hours later. It goes on like this for five straight days, complete with evening homework. But what's this? Says course director Reuben M. Cherniack, MD, director, Department of Medicine, National Jewish Hospital, Denver: "I haven't seen anyone nod off. And I've yet to hear a complaint." How so? According to ACCP Executive Director Alfred Soffer, MD, the continuing interest seems to stem from a combination of motivation and participation. Steroid side effects also seen in asthmatics

Physicians using high doses of oral corticosteroids to treat asthmatic patients should be aware that two of the known side effects of these agents may occur in this group of patients as well: weakening of bones and development of posterior subcapsular cataracts. In two recent separate studies of asthmatics taking 10 to 20 mg/day of prednisone, significant loss of trabecular bone and fractures were found in at least 10% of adults and initial cataractous changes in 30% of children. One investigative group reported preliminary findings that cataracts regressed when the children began using inhaled steroids. Some Considerations Bearing on the Doctrine of Self-fulfilling Prophecy in Sex Chromosome Aneuploidy

Urease testing of mycobacteria with BACTEC radiometric instrumentation.

A total of 140 mycobacterial isolates from patients treated at Fitzsimons Army Medical Center or the National Jewish Hospital and Research Center and from animal specimens submitted to the National Veterinary Services Laboratory were tested by using a urease procedure modified for use with a BACTEC model 301. Mycobacterial suspensions were prepared by using Middlebrook 7H10 Tween broth. Of the 98 mycobacteria isolates which were urease positive utilizing standard methodology, all were positive using the radiometric procedures. Similarly, all 42 urease-negative isolates were also negative employing the new methodology. Although maximum radiometric readings were observed at 48 h, all positive strains were readily identified 24 h after inoculation without sacrificing either test sensitivity or specificity. Thus, urease testing of mycobacteria, using the modified BACTEC radiometric methodology, was as sensitive, as specific, and more rapid than conventional methods.

Treatment of disease due to Mycobacterium intracellulare.

During eleven and one-half years, 122 patients with M. intracellulare disease were treated at National Jewish Hospital and Research Center, Denver, Colorado. Of the 81 patients treated with multiple drug chemotherapy, 63 (78%) were considered to be treatment successes. On follow-up (averaging 55 months) 40 of the 63 patients had remained clinically stable or improved and 12 had grown worse. Ten of these 63 patients had relapsed. Of the 63 patients considered to be treatment successes, 23 (36.5%) had died during follow-up, mostly of pulmonary disease. Of the 18 patients who failed to respond to chemotherapy, 2 were later successfully treated with surgery, 5 patients were clinically worsening, and 8 (44%) had died during a follow-up period averaging 43 months. A group of 41 patients had indeterminate results with chemotherapy. Eight had died while in the hospital. Follow-up information after an average of 43 months indicated that 9 patients became treatment successes (5 with chemotherapy, 4 with surgery), 12 were clinically stable or improving, and an additional 13 had died for a total of 21 deaths (51%). The overall success of treatment in the 122 patients was 61%. Fifty-two (43%) died during the study. Many patients will respond to drug therapy and remain clinically well for years. Because of the potential seriousness of this disease in some patients, multiple drug chemotherapy should be used.

Tuberculosis in association with pregnancy

A review was made of the records of 27 patients admitted to National Jewish Hospital who developed or experienced a reactivation of pulmonary tuberculosis during pregnancy or the first 12 months post partum. Sixteen patients had drug-resistant disease, and 11 had drug-susceptible disease. The drug-resistant group had more extensive radiographic abnormalities (p < 0.01), longer sputum conversion times (p < 0.05), and a higher incidence of pulmonary complications and death (p = 0.05). Patients with tuberculosis associated with pregnancy have the same clinical presentation as nonpregnant patients. Tuberculosis during pregnancy should be treated, and the drugs that appear to be safest for the fetus include isoniazid, para-aminosalicylic acid, ethambutol, and rifampin.

Coexistence of Sleep Apnea and Asthma Resulting in Severe Sleep Hypoxemia

ALTHOUGH significant hypoxemia may be present during status asthmaticus, 1 severe hypoxemia is unusual in chronic bronchial asthma. 2 We report the case of a patient with chronic bronchial asthma who had mild hypoxemia while awake, but the patient was unusual in that he had erythrocytosis. During sleep he had severe hypoxemia that was primarily due to obstructive apnea. A tracheostomy was required for effective treatment of the apnea and hypoxemia. Report of a Case A 66-year-old man from Detroit was referred to National Jewish Hospital and Research Center for evaluation and treatment of asthma. The patient had frequent episodes of intermittent dyspnea accompanied by wheezing for 2 1/2 years before admission. He had never been a cigarette smoker and produced minimal sputum. Alterations in Pulmonary Function Following Respiratory Viral Infection

Respiratory viral illness is a major cause of morbidity in both adults and children. This report focuses on both the acute and chronic effects on respiratory function of these ubiquitous infections. Infant airways are particularly vulnerable due to the relatively low conductance in immature peripheral airways. Bronchiolitis, caused predominantly by respiratory syncytial virus, is the most important of these viral illnesses and is emerging as a major risk factor for the subsequent development of obstructive airway disease in adults, possibly by interference with normal alveolar proliferation. The basic pathogenic mechanism involved in adult respiratory viral infection is bronchial hyperreactivity, presumably secondary to epithelial damage and resultant sensitization of rapidly adapting airway receptors. In addition, there may be virus-related alterations in the autonomic and humoral regulation of airway tone. Viral infections may alter the effects of common air pollutants on respiratory function.
