Treatment of multiple superficial basal cell carcinomas with topical imiquimod

Basal cell carcinoma is the most common malignancy of man. The use of the topical imiquimod 5% cream offers a noninvasive, nonsurgical, and an effective option for the treatment of primary types superficial basal cell carcinoma (sBCC). We present a case series of patients who used imiquimod 5% cream with good therapeutic effect. The medication had a good tolerability profile and there were no ocular adverse events. Imiquimod 5% cream could be considered the first choice treatment for BCCs in older patients who are poor candidates to surgery.

Successful Treatment of Multiple Superficial Basal Cell Carcinomas with Topical Imiquimod

BACKGROUND The use of imiquimod 5% cream for the treatment of superficial basal cell carcinoma (BCC) has been described. Its clinical efficacy was first shown in a study in patients with biopsy-proven superficial and nodular BCC. OBJECTIVE To evaluate the efficacy of topical imiquimod in a patient with multiple superficial BCCs unresponsive to topical 5-fluorouracil and to review the literature on this treatment modality for BCCs. RESULTS After 12 weeks of topical imiqmod, the patient's lesions had resolved. The patient was evaluated three months after discontinuation of treatment. There was no clinical evidence of any recurrent tumor. CONCLUSION Imiquimod 5% cream appears to be effective for multiple, superficial BCCs unresponsive to other modalities.

Nonsyndromic type of hereditary multiple basal cell carcinoma

An autosomal dominant phenotype characterized by multiple superficial basal cell carcinomas (BCC) without associated anomalies is postulated on the ground of the following data. There are several reports on multiple BCC occurring in two generations of a family, including male-to-male transmission. There are also three reports of a strictly unilateral manifestation of multiple superficial BCC, suggesting mosaicism. The father of one of the patients with unilateral involvement was affected with multiple disseminated BCC, which suggests a type 2 segmental manifestation in the son, reflecting loss of heterozygosity that occurred at an early stage of embryogenesis. The three cases of unilateral arrangement would be difficult to explain without the assumption that multiple nonsyndromic superficial BCC may occur as a distinct mendelian trait.

Many of the standard forms of therapy for large or multiple superficial basal cell carcinomas are limited by significant postoperative pain, excessive scarring, and prolonged wound healing time. Combining traditional curettage with carbon dioxide laser vaporization creates a procedure that allows excellent visualization, due to the bloodless surgical field produced by the laser, minimal nonspecific thermal damage, rapid healing, and diminished postoperative pain. In addition, the speed and ease with which this procedure can be performed allow successful treatment of many lesions in a single outpatient session. We wish to report our results using this technique for the treatment of 52 patients with 370 superficial basal cell carcinomas.
