What Happened During the Insertion of a Superior Vena Cava Cannula and Pulmonary Artery Catheter for Minimally Invasive Robot-Assisted Cardiac Surgery?

A 59-YEAR-OLD woman weighing 52.5 kg and 158.7 cm tall was admitted with a diagnosis of mitral regurgitation and atrial fibrillation. A routine laboratory examination produced no significant preoperative findings except for severe rheumatic mitral regurgitation, mild mitral stenosis, and atrial fibrillation. Surgery was scheduled for mitral valvuloplasty and the Maze operation using a robotically controlled camera (AESOP3000; Computer Motion, Santa Barbara, CA) for minimally invasive robot-assisted thoracotomy, with a preoperative plan to perform single-lung ventilation, continuous monitoring of mixed venous saturation and cardiac output with the pulmonary artery (PA) catheter, and insertion of a superior vena cava (SVC) cannula via the right internal jugular vein.

Technological advances in oximetry have enabled the development of a pulmonary artery catheter which allows the continuous monitoring of a patient's mixed venous oxygen saturation (SvO2), which is recognised as a valuable indication of cardiac output and the balance between oxygen supply and demand (delivery and consumption). This paper briefly explains the rationale for monitoring SvO2 and highlights possible areas where the data it provides could influence the assessment and implementation of nursing interventions undertaken within intensive care units, and enable those decisions to be made more safely.

The Risks of Pulmonary Arterial Catheterization

Pulmonary arterial catheters have been widely accepted as useful for monitoring the critically ill patient. New modifications allowing transvenous cardiac pacing, Hiss bundle electrocardiography and continuous monitoring of mixed venous saturation promise to increase the acceptance and use of these catheters. The 52 per cent combined major and minor complication rate documented in this study suggest that enthusiasm for pulmonary arterial catheterization should be tempered with caution. Serious complications and death do occur with these devices and may be more frequent than previously recognized. Use of a pulmonary arterial catheter should be undertaken for defined indications, with a reasonable expectation of benefit from the data obtained and with a clear understanding of the risks involved.
