Minimum 10-Year Survivorship of Mobile-Bearing Unicompartmental Arthroplasty: Single Surgeon, North American Non-Designer Consecutive Series.

Mobile-bearing unicompartmental knee arthroplasty (UKA) provides a durable option for the surgical treatment of monocompartmental knee arthritis. Despite its availability in the United States since 2004, there is only 1 published North American series reporting on the minimum 10-year results of mobile-bearing UKA. The purpose of this study is to determine the survivorship, reasons for failure, and patient-reported outcomes of the Oxford mobile-bearing UKA at minimum 10-year follow-up. One hundred fifty-seven knees were eligible for inclusion in this study based on the date of their index surgery allowing for a minimum 10-year follow-up. The mean follow-up from implantation was 11.4 years (range 10.0-13.8). Failures were reviewed for potential etiologic factors. Survivorship free of reoperation for any reason and free of revision was determined using Kaplan-Meier curves. Functional outcomes were assessed using the Knee Society Knee and Function scores. There were 17 revisions (10.6%). Six were secondary to progression of lateral compartment arthritis, 5 for persistent pain, 3 for femoral component aseptic loosening, 2 polyethylene dislocations, and 1 deep infection. The mean time to revision was 4 years (range 0.1-11). The survivorship free from revision at minimum 10-year follow-up was 85%. At final follow-up, the mean Knee Society Knee Score was 93 (range 66-100) and the mean functional score was 80 (range 30-100). This single surgeon series demonstrated a survivorship of 85% at minimum 10-year follow-up. These results are comparable to published data from North America, but survivorship is lower than European series.

Manipulation of Tibial Component to Ensure Avoidance of Bearing Separation from the Vertical Wall of Tibial Component in Oxford Unicompartmental Arthroplasty.

In Oxford unicompartmental knee arthroplasty, the relationship between the mobile bearing and the vertical wall of the tibial tray is important in preventing bearing dislocation. Separation of the bearing from the vertical wall can cause spinning of the bearing with an increased risk of subsequent dislocation. We report on intraoperative adjustment of the tibial tray performed to prevent the bearing from spinning. After tibial and femoral bone cutting and adjustment of the flexion and extension gap, the trial bearing is inserted and the bearing-vertical wall distance is evaluated before the preparation using the tibial template and bearing trial. In the case of separation, it can be resolved by medialization with or without rotational adjustment. The technique is useful and can be easily performed, it is therefore recommendable for all cases of Oxford mobile-bearing unicompartmental arthroplasty.

Prevention and treatment of bearing dislocation of mobile-bearing unicompartmental knee arthroplasty

Bearing dislocation is a special complication of mobile-bearing unicompartmental arthroplasty, caused by many factors, such as imbalance of the flexion and extension gap, malposition of components, impingement by the remaining osteophytes and cement, damage or delayed chronic laxity of medial collateral ligament, traumatic accident and habitual high knee flexion. It can be reduced by strictly controlling the operation indications before operation, osteotomy and implanting the prosthesis accurately while protecting the medial collateral ligament during operation, actively guiding the appropriate rehabilitation actions and activity intensity of patients after operation. Treatment should be individualized according to the causes and individual conditions of patients.

How does the inclination of the tibial component matter? A three-dimensional finite element analysis of medial mobile-bearing unicompartmental arthroplasty

BackgroundMedial unicompartmental knee arthroplasty (UKA) using Oxford mobile-bearing prosthesis is performed in the treatment of medial compartmental arthritis of the knee. However, little is known about the stress distributions for mobile-bearing UKA on the medial tibial plateau. MethodsIn this study, the stresses on the coronal plane were calculated in a three-dimensional model of the proximal tibia. The features of the stress distribution were investigated when the tibial tray was placed in 15°, 10°, six degrees, and three degrees varus, neutral (0°), and in three degrees, six degrees, 10°, and 15° valgus on the coronal plane of the medial plateau. ResultsThe peak von Mises stress was found on the cortex below the medial plateau while the stresses of cortical bone increased gradually as the inclination of the tibial tray was changed from varus to valgus. The amount of peak stress was almost the same as that in the normal knee model when the tibial tray was placed in six degrees valgus and consistently lower in varus inclination than in the normal knee model. Conversely, the peak stress of soft bone was found at the bottom of the slot. ConclusionsThis study demonstrates that the inclination of the tibial component affects stress distribution in the proximal tibia after UKA. Slight varus inclination of the mobile-bearing tibial component is acceptable as it lowers the peak stress on the medial cortex. Additionally, placing the tibial tray in slight varus avoids a rise in stress between the tip of the keel and the medial tibial cortex.

Medial Mobile-Bearing Unicompartmental Knee Arthroplasty in Young Patients Aged Less Than or Equal to 50 Years

BackgroundContemporary research has shown medial mobile-bearing unicompartmental knee arthroplasty to be an effective treatment in patients younger than 60 years; however, only one other study has specifically investigated unicompartmental arthroplasty outcomes in patients 50 years or younger. The purpose of this study was to determine the clinical outcomes and survivorship of medial mobile-bearing unicompartmental arthroplasty in this younger patient population. MethodsA retrospective review of patients undergoing primary unicompartmental knee arthroplasty using the Oxford partial knee from 2003 to 2014 in a single practice database was performed. Patients were included in the study if they were 50 years of age or younger with a primary diagnosis of anteromedial osteoarthritis and minimum clinical follow-up of 2 years. Patient clinical outcomes, function, and need for revision surgery were assessed. ResultsThe study included 340 knees. Average patient age was 46.5 years, and the mean follow-up was 6.1 years. Patients demonstrated significant improvements (P < .05) in range of motion (114.5 v 116.9), University of California Los Angeles activity score (4.4 vs 5.6), Knee Society clinical (37.3 vs 86.5) and functional scores (58.8 v 79.8). Overall, 20 patients required reoperation, and the predicted survival rate was 96% at 6 years and 86% at 10 years. Aseptic loosening occurred in 7 patients at an average of 5.6 years postoperatively, while 4 patients required conversion to total knee arthroplasty because of arthritic progression at a mean time of 6.6 years. There were no revision procedures required due to polyethylene liner wear or breakage. ConclusionMedial mobile-bearing unicompartmental arthroplasty should be considered as a treatment option in patients younger than 50 years of age suffering from anteromedial osteoarthritis of the knee.
