When insurance is not a barrier: psychological factors predicting whether bariatric candidates undergo surgery

BackgroundDespite the effectiveness of bariatric surgery, utilization rates have increased only marginally over the last 2 decades; candidates who are eligible for bariatric surgery regularly fail to undergo surgery. The Minnesota Multiphasic Personality Inventory-2 Restructured Form (MMPI-2-RF) has previously been used to assist in identification of those who will not move forward with surgery after being identified as eligible. However, medical insurance has been identified as a significant barrier to surgery; research in those who have universal health care may yield different results. ObjectivesDetermine if MMPI-RF scales are associated with failure to undergo bariatric surgery in patients eligible to have the procedure. SettingLarge military hospital in the Northwestern U.S. MethodsThis study used archival data for 279 patients psychologically screened for eligibility for bariatric surgery. All assessments took place between January 2017 and December 2019. T-tests and chi-square tests were used to compare groups of patients who did and did not have surgery on relevant medical and demographic variables. Profile analyses of patient MMPI-2-RF scores were conducted to examine scale associations with undergoing surgery. ResultsA total of 86 bariatric surgery candidates (30.8%) did not undergo surgery. Results showed that sex, age, employment status, and arthritis were different between groups. Additionally, MMPI-2-RF scales were different between groups, including somatic complaints, neurological complains, cynicism, and helplessness/hopelessness. ConclusionsMMPI-2-RF scales were associated with not having bariatric surgery, although not all scales exceeded clinical cut-offs. Findings indicate psychological and psychosocial differences, rather than psychopathology per se, may play a role in who undergoes bariatric surgery.

Perrotta Integrative Clinical Interviews‐3 (PICI‐3): Development, regulation, updation, and validation of the psychometric instrument for the identification of functional and dysfunctional personality traits and diagnosis of psychopathological disorders, for children (8–10 years), preadolescents (11–13 years), adolescents (14–18 years), adults (19–69 years), and elders (70–90 years)

AbstractThe Perrotta Integrative Clinical Interview, second version (PICI‐2) requires structural and functional updates, based on clinical and academic experience, especially in terms of functional traits and interpretation of psychopathological disorders. The Perrotta Integrative Clinical Interviews‐3 (PICI‐3) was created and structured into four sections, dedicated to dysfunctional traits in children and pre‐adolescents (PICI‐C‐3, 8–13 years) and in adolescents, adults, and the elderly (PICI‐TA‐3, 14–90 years), to common secondary disorders (PICI‐DS‐3, 8–90 years) and functional traits (PICI‐FT‐3, 8–90 years), with the identification of all functional elements and structural aspects of personality according to the model underlying the PICI (IPM). Selecting 1732 subjects, between 8 and 90 years old, the statistical analysis showed that the psychometric test has a well‐defined and stable construct, with the variables well represented and positively correlated with other constructs already validated. In particular: (a) the PICI‐TA‐3 (Section A) was compared with the Minnesota Multiphasic Personality Inventory‐2‐Restructured Form (MMPI‐2‐RF), obtaining 99.3% compatibility of results, with a Pearson's coefficient (R) of 0.999 and p < 0.001; (b) the PICI‐C‐3 (Section B) was compared with the Child Behavior Checklist (CBCL), obtaining 94.1% compatibility of results, with a Pearson coefficient (R) of 0.969 and p < 0.001; (c) the PICI‐FT‐3 (Section D) was compared with the Big Five Personality Test (Big5), obtaining 89.4% compatibility of results, with a Pearson coefficient (R) of 0.797 and p < 0.001. The PICI‐3 is a valid, efficient, and effective psychometric tool to identify the functioning or dysfunction of personality traits for psychopathological diagnosis.

Six-year postoperative associations between the Minnesota Multiphasic Personality Inventory – 3 (MMPI-3) and weight recurrence, eating behaviors, adherence, alcohol misuse, and quality of life

BackgroundMetabolic and bariatric surgery (MBS) leads to beneficial and sustained outcomes. However, many patients evidence weight recurrence and psychosocial functioning may be associated with weight recurrence. The Minnesota Multiphasic Personality Inventory – 3 (MMPI-3) is validated for use in presurgical MBS evaluations and likely has clinical utility in understanding weight recurrence and other aspects of postoperative functioning. SettingCleveland Clinic Bariatric and Metabolic Institute. MethodsA sample of 163 participants consented to take a battery of self-report measures related to psychological functioning, eating behaviors, adherence, alcohol misuse, and quality of life along with their postoperative weight. MMPI-3 scale scores were prorated from the Minnesota Multiphasic Personality Inventory – 2 – Restructured Form (MMPI-2-RF) or scored from the Minnesota Multiphasic Personality Inventory – 2 – Restructured Form – Expanded (MMPI-2-RF-EX). ResultsWeight recurrence was quite variable in this sample. Postoperative MMPI-3 scales related to emotional/internalizing dysfunction were modestly associated with higher weight recurrence. Postoperative MMPI-3 scale scores also demonstrated associations with other postoperative outcomes including measures of eating behaviors, adherence, alcohol misuse, and quality of life. ConclusionsPostoperative psychosocial functioning as assessed by the MMPI-3 was associated with weight recurrence and a number of other problematic psychological outcomes beyond weight recurrence.

12 The Effect of Adverse Childhood Experiences on Minnesota Multiphasic Personality Inventory-2-Restructured Form Symptom Reporting Among Adult Neuropsychological Referrals

Objective:Adverse childhood experiences (ACEs) is a broad construct that refers to negative events one may experience during childhood, including, but not limited to, childhood maltreatment, household dysfunction, and trauma. ACEs have consistently shown to be associated with negative physical and mental health outcomes. Although researchers have investigated the effects of trauma and abuse on personality measures, few studies have examined differences between those with high ACEs, low ACEs, and no ACEs on measures of personality in the context of neuropsychological evaluations.Participants and Methods:The current study included 128 consecutive adult patients referred for outpatient neuropsychological evaluation of attention-deficit/hyperactivity disorder. The sample was 39.8% non-Hispanic White, 21.9% non-Hispanic Black, 16.4% Hispanic, 10.9% Asian/Pacific Islander, and 10.9% other race/ethnicity, with a mean age of 27.9 years (SD=6.3) and mean education of 16.1 years (SD=2.2). Multivariate analyses of variance were performed to evaluate differences on the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) between individuals who experienced high levels of ACEs (>4/10 on the Adverse Childhood Experiences Questionnaire), low levels of ACEs (1-3/10), and no ACEs (0/10).Results:When analyzing Higher-Order (H-O) scales, there was a significant group difference in mean elevation on the Behavioral/Externalizing Dysfunction (BXD) scale, F(2,113)=3.124, p < .05, such that individuals in the high ACEs group evidenced higher scores than those in the low ACEs group (p < .05). Additionally, there were significant differences on several Restructured Clinical (RC) scales. Specifically, there were group differences on the Low Positive Emotions (RC2) scale, F(2,113)=3.427, p < .05, such that those in the low ACEs group evidenced higher scores than those in the no ACEs group (p < .05). The Antisocial Behavior (RC4) scale also had significant differences, F(2,113)=13.703, p < .001, such that those in the high ACEs group had higher scores than those in the low and no ACEs groups (p < .001). Finally, the Ideas of Persecution (RC6) scale yielded significant group differences, F(2,113)=4.793, p < .05, such that those in the high ACEs group evidenced higher scores than those in the low and no ACEs groups (p < .05).Conclusions:In sum, this study demonstrated that ACEs, particularly high levels of ACEs, are related to higher difficulties with problems with under-controlled and rule-breaking behaviors, low positive emotional responses, and beliefs that others pose a threat. As such, assessment of ACEs may serve an important role in characterizing patients’ psychological status as part of a comprehensive neuropsychological evaluation.

A - 90 Assessment Methods for Anxiety in Epilepsy.

Anxiety is disproportionately common in people with epilepsy, however, the optimal tool to screen for assessment of anxiety in epilepsy has not been definitively established. This study aims to assess if a long questionnaire such as the Minnesota Multiphasic Personality Inventory-2 Restructured Form (MMPI-2-RF) is necessary for identifying and quantifying anxiety symptoms in patients with epilepsy or if the Beck Anxiety Inventory (BAI) will suffice as a screening measure. Retrospective data were analyzed from a neuropsychology registry of patients with epilepsy (N = 317; 136 male, 181 female). As proposed by Kenfack et al, 2022, patients that scored ≥16 on the BAI were cross tabulated with patients identified by the MMPI-2-RF as having T-scores ≥65 on scales EID (emotional/internalizing dysfunction), RC7 (dysfunctional negative emotions), STW (stress/worry), AXY (anxiety), and RC1(somatic complaints). Multiple chi square tests with alpha set at.05 were performed to examine the relationship between BAI and MMPI-2-RF indices in identifying anxiety in patients with epilepsy. As shown in Table 1, scores above the cutoff on the BAI were significantly associated with scores falling above the cutoffs for all of the MMPI-2-RF anxiety indices (EID, RC1, RC7, STW, & AXY). The diagnostic value of the BAI is similar to that of MMPI-2-RF subscales commonly used to assess anxiety. The findings provide support for using the BAI for screening anxiety symptoms in patients with epilepsy undergoing neuropsychological assessment. Keywords: Anxiety, Epilepsy, MMPI-2-RF, BAI.

B - 50 The Relationship between Objective and Subjective Measures of Attention.

The relationship between objective and subjective measures of attention is understudied. This study aimed to evaluate the convergence between commonly used measures in the assessment of attention-deficit hyperactivity disorder (ADHD) and to determine whether there is a significant difference between individuals with and without ADHD on subscales related to ADHD symptoms. This study involved an archival data set of Veterans (N = 504) who completed the Minnesota Multiphasic Personality Inventory-2 Restructured Form (MMPI-2-RF) or MMPI-3, the Barkley's Adult ADHD Rating Scale or a local DSM-5/TR ADHD diagnostic interview, the Connor Continuous Performance Test 3 (CCPT-3) or the Integrated Visual & Auditory 2, and the Trail Making Test as part of a clinical assessment in a specialty ADHD clinic. Nonparametric comparisons between those with and without ADHD revealed no statistically significant differences across the cognitive measures. Self-reported symptoms of inattention (z = 12.07, p < 0.001, r = 0.57, large effect), hyperactivity (z = 4.70, p < 0.001, r = 0.22, small effect), and impulsivity (z = 5.52, p < 0.001, r = 0.26, small effect), significantly differentiated participants with and without ADHD. Pearson correlations between the two types of measures showed that omission errors on the CCPT-3 were significantly correlated to self-reported inattention symptoms (r = 0.15, p = 0.039). All other correlations between objective and subjective measures were not significant. These results suggest that neuropsychological measures are not strongly correlated to self-reported symptoms of inattention, and as such may be less sensitive to ADHD.

The role of obesity severity in Minnesota multiphasic personality inventory-2-restructured form prediction of 12-month bariatric surgery outcomes

BackgroundThe Minnesota Multiphasic Personality Inventory-2 Restructured Form (MMPI-2-RF) has been widely used in bariatric surgery samples. However, questions remain regarding its utility when predicting changes in body mass index over time following bariatric surgery. ObjectivesExamine whether MMPI-2-RF scales differentially predict 12-month changes in body mass index (BMI) following bariatric surgery when comparing patients with Class III or higher versus Class II or lower obesity. SettingMilitary hospital in the Northwestern United States. MethodsThis retrospective study evaluated data from 193 bariatric surgery patients who completed the MMPI-2-RF as part of presurgical evaluation requirements. Hierarchical linear modeling was used to predict body mass index over a 12-month postsurgical period based on MMPI-2-RF scales. Loss to follow-up rate was 30% during this period. ResultsAmong patients with Class II or lower obesity, the Symptom Validity, Adjustment Validity, Response Bias, Cynicism, Aggression, Stress/Worry, and Anger Proneness scales showed a significant relationship to BMI after bariatric surgery. Among patients with Class III or higher obesity, the Infrequent Psychopathology Responses, Emotional / Internalizing Dysfunction, Ideas of Persecution, Multiple Specific Fears, and Inefficacy scales showed a significant relationship to body mass index after bariatric surgery. ConclusionsCertain MMPI-2-RF scales may have better utility in predicting bariatric surgery outcomes based on the patient’s obesity severity. The interaction of metabolic and personality factors may play a significant role in weight change following bariatric surgery.

MMPI-2-RF Profiles of Treatment-Seeking Veterans in a VA Pain Clinic and Associations with Markers of Physical Performance.

Chronic pain is a debilitating condition for many military Veterans and is associated with posttraumatic stress disorder (PTSD). This study examined the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) in 144 Veterans (88.2% male, mean age = 57.95years) recruited from a VA outpatient pain clinic and associations with self-reported pain severity, pain-related interference in daily activities, prescription opioid use, and objective metrics of physical performance on tasks impacted by pain (walking, stair climbing, grip strength, indexed by a single latent variable). Among the cohort with valid responses on the MMPI-2-RF (n = 117) and probable PTSD, mean Somatic Complaints (RC1) and Ideas of Persecution (RC6) scores were clinically elevated. All MMPI-2-RF scales were more strongly correlated with self-reported pain interference than severity. Regressions revealed associations between self-rated pain interference (but not pain or PTSD severity) and physical performance scores (β = .36, p = .001). MMPI-2-RF overreporting Validity and Higher-Order scales contributed incremental variance in predicting physical performance, including Infrequent Psychopathology Responses (β = .33, p = .002). PTSD severity was associated with prescription opioid use when accounting for the effects of over-reported somatic and cognitive symptoms (odds ratio 1.05, p ≤ .025). Results highlight the role of symptom overreporting and perceptions of functional impairment to observable behaviors among individuals with chronic pain.

The Relationship between the Structures of Personality Inventory for the Diagnostic and Statistical Manual, Fifth Edition (PID-5) and Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF).

Objective: The Personality Inventory for the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (PID-5), is a trait-based measure of pathological personality designed to assess Criterion B of an alternative diagnostic system for personality disorders (PDs). In this study, we aimed to evaluate the relations among the PID-5 and the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF); a commonly used self-report instrument with a hierarchical structure. Method : We examined the joint structure of the PID-5 scales along with levels of the MMPI-2-RF hierarchy to understand whether conceptually expected structures tend to be loaded with each other. Data were collected from 536 participants from the general population of Iran. Results: Findings of Pearson's correlation analyses exhibited the generally expected patterns between the two mentioned measures on most scales, with some divergences. Similarly, although applying a set of joint exploratory structural equation modeling (ESEM) exhibited some factor loadings for PID-5 facets within the hierarchical framework of MMPI-2-RF scales that were different to what was theoretically expected, both measures were generally loaded in a conceptually expected way, indicating that they have a similar dimensional structure. Conclusion: Our findings provide support for adequate convergence of maladaptive personality traits and psychopathology structures, as well as for utilizing MMPI-2-RF to measure personality psychopathology from a dimensional perspective. The implications of these results are discussed by the authors.

Minnesota Multiphasic Personality Inventory-2-Restructured Form Profiles Among Adults With Attention-Deficit/Hyperactivity Disorder: Examining the Effect of Comorbid Psychopathology and ADHD Presentation.

Despite widespread use of the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF), it is surprisingly understudied among adults with attention-deficit/hyperactivity disorder (ADHD). This is significant as ADHD is a frequent referral for neuropsychological evaluation; however, the core symptom of attention difficulty is a nonspecific sequela of many psychological disorders. This study aimed to characterize MMPI-2-RF profiles among adults with ADHD and examine the effect of comorbid psychopathology. A large, demographically diverse sample of 413 consecutive adults referred for neuropsychological evaluation to assist with differential diagnosis of ADHD who completed the MMPI-2-RF was examined. Profiles of the 145 patients diagnosed with ADHD-only were compared to 192 with ADHD and a comorbid psychological disorder and a 55-patient non-ADHD psychiatric comparison group. Among the ADHD-only group, profiles also were compared based on ADHD-presentation type (Predominantly Inattentive vs. Combined presentation). The ADHD/psychopathology and psychiatric comparison groups scored higher than the ADHD-only group across nearly all scales with widespread clinical elevations. Conversely, the ADHD-only group displayed an isolated elevation on the Cognitive Complaints scale. Comparison between ADHD presentations revealed several small-moderate significant differences, the largest of which occurred on the Externalizing and Interpersonal scales. Adults with ADHD alone, and no other psychopathology have a unique MMPI-2-RF profile characterized by isolated elevation on the Cognitive Complaints scale. These results support use of the MMPI-2-RF in assessment of adults with ADHD as it can help distinguish ADHD alone from ADHD/comorbid psychopathology and identify relevant psychiatric comorbidities that may be contributing to patients' inattention complaints.

Psychological Profiles and 12-Month Weight Outcomes of Women Evaluated for Reoperative Bariatric Surgery.

The use of psychological screening instruments prior to bariatric surgery has been well established. However, there is currently no specific literature on psychological screening of candidates for reoperative bariatric surgery. This study evaluated archival data for 40 women who were candidates for reoperative bariatric surgery and completed the Minnesota Multiphasic Personality Inventory-2 Restructured Form (MMPI-2-RF). Profile analysis examined differences between patient groups who did and did not undergo reoperative surgery. MMPI-2-RF profiles for reoperative patients then were compared to previous samples of preoperative and postoperative patients. Hierarchical linear modeling was used to predict BMI following reoperative surgery over a 12-month period based on MMPI-2-RF scales while controlling for age and initial BMI. Profile analysis results showed no significant differences on MMPI-2-RF scale scores between reoperative candidates who did and did not undergo a second surgery. With some minor differences attributed to minimization of symptoms, there were no systematic differences in MMPI-2-RF scale scores for reoperative surgery patients compared to preoperative and postoperative patient groups. BMI outcomes over a 12-month period showed that age and initial BMI were significantly better predictors than MMPI-2-RF scores. Administration of MMPI-2-RF for reoperative bariatric surgery patients likely is equivalent to its typical use in preoperative screening. Only the MMPI-2-RF Disconstraint scale showed any relationship to BMI outcomes over time following reoperative surgery.

Mental Health and Personality Characteristics of University Students at Risk of Smartphone Overdependence.

The purpose of this study was to verify the relationship between the risk of smartphone dependence, mental health, and personality traits in university students using the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF), and to identify the MMPI-2-RF scales that can predict the risk of smartphone dependence. Of the 772 university students who participated in the study, 163 were in the smartphone overdependence group, accounting for 21.1% of the total survey respondents, which was one in five of those surveyed. High T-scores on the measure indicate greater psychopathology. The smartphone overdependence group had significantly higher T-scores than the general user group on all but three of the MMPI-2-RF scales, and the degree of smartphone overdependence was positively correlated with scores on these scales. There was no difference between the dependent and non-dependent groups on the interpersonal passivity, aesthetic-literary interest, and aggression scales, and scores on these three were not correlated with smartphone dependence. Among the MMPI-2-RF scales, those found to predict the risk of smartphone overdependence were the emotional/internalizing problems, behavioral/externalizing problems, antisocial behavior, cognitive complaints, helplessness/hopelessness, inefficacy, juvenile conduct problems, aggression, interpersonal problems, disconstraint, negative emotionality/neuroticism, and introversion/low positive introversion/low positive emotionality scales. Based on these findings, we propose that effective prevention and intervention for smartphone overdependence must be comprehensive and holistic rather than focusing on specific aspects of mental health or personality. The implications of the findings are discussed.

The structure of dimensions of psychopathology in normative and clinical samples: Applying causal discovery to MMPI-2-RF scales to investigate clustering of psychopathology spectra and p-factors.

We applied a Bayesian Constraint-based Causal Discovery method (BCCD) to examine the hierarchical structure of the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) Restructured Clinical (RC) scales. Two different general psychopathology super spectra (p-factor) scales were extracted from (1) all RC scales and (2) all RC scales except the RCd (Demoralization) scale. These p-factor scales were included in separate models to investigate the structure of dimensions of psychopathology in a normative (n = 3,242) and clinical (n = 2,466) sample, as well as the combined normative/clinical sample (N = 5,708), by applying the BCCD algorithm to obtain a data-driven reconstruction of the internal hierarchical structure of the MMPI-2-RF. Research on the underlying structure of the MMPI-2-RF has clinical relevance as well as conceptual relevance in the context of the HiTOP model. Results demonstrated that the syndromes measured with the RC-scales-in presence of a p-factor-cluster into six spectra: internalizing, disinhibited-externalizing, antagonistic-externalizing, thought disorder, detachment, and somatoform. These results may support a super spectrum construct, as it was necessary for obtaining a bottom-up reconstruction of this six-spectrum structure. We found support for superiority of a broad super spectrum with additional variance over and above demoralization, as it resulted in the clearest structure (i.e., clustering of the RC scales). Furthermore, our results indicate independent support for the bifactor structure model of psychopathology.

Comprehensive Analysis of MMPI-2-RF Symptom Validity Scales and Performance Validity Test Relationships in a Diverse Mixed Neuropsychiatric Setting.

The utility of symptom (SVT) and performance (PVT) validity tests has been independently established in neuropsychological evaluations, yet research on the relationship between these two types of validity indices is limited to circumscribed populations and measures. This study examined the relationship between SVTs on the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) and PVTs in a mixed neuropsychiatric setting. This cross-sectional study included data from 181 diagnostically and demographically diverse patients with neuropsychiatric conditions referred for outpatient clinical neuropsychological evaluation at an academic medical center. All patients were administered a uniform neuropsychological battery, including the MMPI-2-RF and five PVTs (i.e., Dot Counting Test; Medical Symptom Validity Test; Reliable Digit Span; Test of Memory Malingering-Trial 1; Word Choice Test). Nonsignificant associations emerged between SVT and PVT performance. Although the Response Bias Scale was most predictive of PVT performance, MMPI-2-RF SVTs generally had low classification accuracy for predicting PVT performance. Neuropsychological test performance was related to MMPI-2-RF SVT status only when overreporting elevations were at extreme scores. The current study further supports that SVTs and PVTs measure unique and dissociable constructs among diverse patients with neuropsychiatric conditions, consistent with literature from other clinical contexts. Therefore, objective evidence of symptom overreporting on MMPI-2-RF SVTs cannot be interpreted as definitively indicating invalid performance on tests of neurocognitive abilities. As such, clinicians should include both SVTs and PVTs as part of a comprehensive neuropsychological evaluation as they provide unique information regarding performance and symptom validity.

An Examination of the Treatment Utility of the MMPI-2-RF: Prediction of Post-Treatment Depressive Symptoms and Increased Understanding of the Therapeutic Alliance in an Inpatient Mood Disorder Treatment

The study examined the ability of Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) substantive scales to predict depressive symptoms in a psychiatric inpatient setting. The indirect effect of patient-rated alliance with their treatment team on these relationships was also investigated. Participants included 678 (52.5% female, 97.1% White) inpatients diagnosed with a mood disorder. MMPI-2-RF scales, Patient Health Questionnaire (PHQ-9) at intake and discharge, and Working Alliance Inventory-Short at discharge were used to test study hypotheses regarding MMPI-2-RF predictive utility and the influence of alliance. Jacobson and Truax’s (1991) reliable change index (RCI) was calculated to identify those who did (74% of the sample) and did not (24%) make reliable and clinically significant depressive symptom change, and the predictive utility of MMPI-2-RF scores in distinguishing these groups was examined. MMPI-2-RF scales assessing internalizing and somatic dysfunction accounted for an additional 2% to 8% of the variance in depressive symptoms reported at discharge, above and beyond depressive symptoms reported at intake. Somatic scales were also able to differentiate groups based on clinically significant change on the PHQ-9 (small-sized effect). The relationship between MMPI-2-RF scales and depressive symptoms at discharge was indirect through alliance in 64% of models. Clinical implications are discussed.

An Exploratory Study of MMPI-2-RF Personality and Psychopathology Profiles of Adults with Autism Spectrum Disorder without Intellectual Disability.

More empirical research is needed to disentangle the phenotypes of autism spectrum disorder (ASD) and cluster C personality symptomatology (CCPD), as both show similarities in their clinical presentation. We explored personality and psychopathology dimensions as conceptualized in contemporary dimensional taxonomies (i.e., hierarchical taxonomy of psychopathology; HiTOP) in adults with ASD without intellectual disability operationalized by the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF). Applying secondary analytic processes using clinical data, we cross-examined the MMPI-2-RF profiles of adults with ASD (n = 28) compared to adults with Cluster C personality disorders (CCPD; n = 28) and a control group (n = 28) by conducting nonparametric tests and assessing effect sizes. The profiles of the ASD and CCPD groups evidenced to be similar, and both average clinical profiles diverged from the average control group profiles by elevated levels of demoralization, internalizing, and somatization symptomatology. There were small differences between the average profiles of adults with ASD and adults with CCPD. Additional research using dimensional measures of psychopathology could elucidate the dimensional phenotypes of ASD and CCPD. Based on the results in this study, the MMPI-2-RF may not meaningfully discriminate between the two clinical presentations, with the exception of various externalizing scales.

Detecting feigned ADHD in college students using the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF)

Objective: To investigate the utility of the validity scales of the Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) for detecting feigned Attention-Deficit Hyperreactivity Disorder (ADHD), we utilized a simulation design. Method: We examined group differences across the Restructured Clinical (RC) and validity scales as well as the classification ability of the validity scales across three cut scores. Analyses were conducted across five simulation groups (N = 177) and a standard instruction group (N = 32). Results: Across most of the RC and validity scales, those feigning ADHD produced significantly higher scores than the standard instruction group, but generally no significant differences between the feigning groups were demonstrated. The most promising scales for detecting feigned ADHD were F-r, Fp-r, and Fs at cut scores in the 70 T to 80 T range, respectively. Conclusions: Results support the use of the MMPI-2-RF in ADHD evaluations with scores on F-r, Fs, and Fp-r being particularly useful in detecting feigned ADHD in college students. However, there was no evidence to support the feigning of distinct ADHD symptoms presentations.

The Minnesota Multiphasic Personality Inventory–2–Restructured Form (MMPI-2-RF) and Patient-Reported Outcomes Measurement Information System–29 (PROMIS-29) Comparison Study: Assessing for PROMIS-29 Depression and Anxiety Psychopathologic Cutoff Values Amongst Patients Undergoing Elective Complex Spine Procedures

The Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) and the Patient-Reported Outcomes Measurement Information System-29 (PROMIS-29) are used to assess patient psychology, pain, and quality of life. As psychological factors, such as depression and anxiety, are associated with poor perioperative outcomes, we aim to translate MMPI-2-RF values to PROMIS-29 scores and establish cutoff values for PROMIS-29 anxiety and depression domains that might warrant attention preoperatively. Seventy adult patients scheduled for an elective spinal surgery between July 2018 and February 2020 who completed both the MMPI-2-RF and PROMIS-29 preoperatively at a single institution were reviewed. Patients with MMPI-2-RF scores of 65 or greater (the cutoff for psychopathology) in the emotional/internalizing dysfunction scale (4.29%) had an average PROMIS-29 depression score of 14.33, which is significantly higher than the control group's (<65 score) 8.49 score (P= 0.04). Similarly, those demonstrating psychopathology on the demoralization (4.29%) and helplessness/hopelessness (4.29%) scales had average PROMIS-29 depression scores significantly higher than the control group's averages (15.33 vs. 8.45, P= 0.02 and 14.33 vs. 8.49, P= 0.04, respectively). Patients with an MMPI-2-RF score of 65 or greater on the emotional/internalizing dysfunction (4.29%), stress/worry (10.00%), and anxiety (7.14%) scales had average PROMIS-29 anxiety domain scores of 15, 15, and 15, respectively, which were significantly greater than that of the control group's scores (8.94, P= 0.04; 8.75, P= 0.004; and 8.55, P < 0.001, respectively). PROMIS-29 scores of 15 or greater on the depression and anxiety domains may have psychopathologies that warrant addressing, given their increased likelihood of having poor outcomes.

Examination of the Generalizability of Underreporting Detected by the MMPI-2-RF in a Correctional Sample.

It is relatively unclear if underreporting (UR) detected by Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF) L-r (Uncommon Virtues) and K-r (Adjustment Validity) scale scores generalizes to score distortion on criterion measures differently based on test-takers having scored highly on L-r, K-r, or both. Thus, this study reanalyzed the correctional sample of 632 men previously presented by Forbey and colleagues and defined UR groups as high scores on L-r alone (n = 89), K-r alone (n = 45), or both (n = 29). Groups were compared on selected MMPI-2-RF and criterion scales assessing internalizing, externalizing, and thought dysfunction difficulties using analysis of variance (ANOVA) and post hoc tests. Results suggested that UR generalized to score distortion on collateral measures, with K-r outperforming L-r in the prediction of this generalizability on all but measures of specific externalizing difficulties. However, having elevations on both L-r and K-r resulted in the strongest pattern of generalizability.
