EMPODERANDO MÃES: O CRUCIAL PAPEL DO ENFERMEIRO NA PROMOÇÃO E APOIO À AMAMENTAÇÃO – REVISÃO INTEGRATIVA DE LITERATURA APOIO À AMAMENTAÇÃO

Some mothers may face difficulties during the breastfeeding process, which can be influenced by the presence of a hormone called oxytocin, which stimulates the release of breast milk. Furthermore, it is important to emphasize that breastfeeding is considered the ideal nutrition for all children due to its nutrient composition. Regarding methodology, this study is an integrative literature review, allowing for the analysis and subsequent synthesis of existing academic production on breastfeeding in a humanized process. The final sample of this integrative literature review consisted of 5 scientific articles included in this study as they met the previously established inclusion and exclusion criteria and, consequently, answered the research question. Finally, it is highlighted that nurses can be available to address mothers’ doubts and concerns about the subject, offering continuous support throughout the breastfeeding period and helping to resolve issues that may arise, such as low milk production or difficulties with the baby’s latch, as well as providing emotional support during this process.

Turmeric Topical Application: An effective solution for reducing breast milk secretion delays in new mothers

Background: The problem of low coverage of exclusive breastfeeding is partly influenced by insufficient breast milk production. The percentage of exclusive breastfeeding in infants under six months of age in Central Kalimantan in 2020 was 52.98%. This percentage was among the lowest nationally. In 2021, the coverage of exclusive breastfeeding in infants under six months of age reached 56.8%, and in 2022 it did 60.5%. The figure in 2022 exceeded the set target, but several regencies/cities still had low coverage. Insufficient breast milk supply impacts on the nutritional status of the child. Low breast milk production also arises due to several other factors during breastfeeding, namely late initiation of breastfeeding, long breastfeeding time, sore nipples, breast pain and swelling, and inverted nipples. Aims: The study sought to measure the level of success of turmeric topical application in shortening the time it takes to initiate breast milk secretion. Methods: The study uses a true experimental posttest-only control group design. A subject of 50 mothers having full-term spontaneous deliveries and with normal breasts was used in the study. The sample was divided into two groups: control and intervention groups. The sample was selected using a matching process based on age, parity, and early initiation of breastfeeding. Subjects who met the inclusion criteria were offered whether they were willing to be given turmeric compresses after the delivery process. Subjects who were willing were included in the intervention group and those who were not willing were included in the control group. Age and parity in the intervention group were all taken, while the control group was selected to match the intervention group. The intervention group was given a treatment with a turmeric compress on the breasts for six hours. All participants were then observed for time of breast milk secretion initiation. In this experiment, we didn't perform blinding, but we managed it by having the midwife administer the turmeric compress just once during the delivery process before sending the client home. Following this, we recorded the results, specifically the initial breast milk release in both the intervention and the control group. Results: The average time periods it took to initiate breast milk secretion in the intervention and control groups were 509.96 minutes (8.5 hours) and 1573.76 minutes (26.2 hours), respectively. There was a significant difference (p = 0.023) in the time of breast milk secretion initiation of 1063.8 minutes or 17.73 hours between both groups. Conclusion: Applying a turmeric compress on the breasts of postpartum mothers may shorten the time it takes to initiate breast milk secretion. Penerapan Pijat Oksitosin Terhadap Produksi ASI di Ruang Cempaka RSUD Dr.Soehadi Pridjonegoro Sragen

Background: Breast Milk (ASI) is a fatty emulsion in lactose protein and inorganic salts secreted by the mother's mammary gland, useful as food for infants. Breast milk is difficult to achieve because one of them is breast milk that does not come out or does not run smoothly. One of the non-pharmacological arrangements to help smooth the release of breast milk can be done by oxytocin massage. Objective: Knowing the results of the implementation of oxytocin massage to breast milk production at the Cempaka Ward of dr. Soehadi Pridjonegoro Sragen Hospital. Method: This study is a descriptive study in the form of a case study conducted on 2 postpartum mother respondents by massage oxytocin for 2 consecutive days with a frequency of 2x a day for 10-15 minutes. Result: The study of oxytocin massage in both postpartum mothers on the first day has not changed breast milk production. Then on the second day of breast milk production, the two respondents showed an increase, evidenced by the increase in breast milk, empty breasts after catching up, and the breasts looked full before breastfeeding. Conclusion: There is an increase in breast milk production before and after giving oxytocin massage to postpartum mothers in the Cempaka Room of dr. Soehadi Prjonegoro Sragen Hospital.

Oxytocin Massage, Breast Milk Production, and Comfort in Post Partum P0A1 Mothers with Spontaneous Delivery: A Case Study in the Delima Room of RSUD Waled

All pregnant women have the potential to experience complications during pregnancy and the postpartum period. To prevent complications such as bleeding and infection in the postpartum period, comprehensive nursing care is essential, including counseling on postpartum care. This counseling aims to meet the mother's need for knowledge and skills related to self-care, newborn care, family adaptation, and health recovery. This study aims to describe the implementation of nursing care in postpartum mothers through the nursing process approach. A case study design was used on primiparous postpartum mothers with spontaneous delivery in the Delima room, Waled Hospital, Cirebon Regency. The research was conducted on February 12, 2024. The results showed that breast milk production can be accelerated by nonpharmacological measures such as oxytocin massage, which involves massaging the area around the back (vertebrae pars thoracica) to stimulate the release of breast milk. The mother reported feeling satisfied, happy, and confident in her ability to provide breast milk to her baby. Although the problem of ineffective breastfeeding related to inadequate milk supply was not fully resolved, patients reported feeling comfortable after oxytocin massage. The implication of the study suggests that oxytocin massage can be an effective intervention in accelerating milk release and improving the well-being of postpartum mothers. Appropriate counseling and nursing care are crucial in helping postpartum mothers overcome challenges and improve the quality of self-care and newborn care.

Efektifitas Pijat Woolwich Terhadap Produksi Asi di Bidan Praktek Mesra Wilayah Puskesmas Pintu Padang Kecamatan Batang Angkola Kabupaten Tapanuli Selatan

In order to reduce child morbidity and mortality, the United Nations Children's Fun (UNICEF) and the World Health Organization (WHO) recommend that children should only be given breast milk for at least six months (Infodatin, 2020). Adequacy of breast milk prevents hunger and malnutrition, reduces infant mortality, improves mental and cognitive development of infants. Woolwich massage increases the hormones prolactin and oxytocin which play a role in stimulating the release of milk in mothers after giving birth. Woolwich massage. carried out in the area of the lactiferous sinuses exactly 1-1.5 cm above the areola mamae, with the aim of removing the milk that is in the lactiferous sinuses. The purpose of this study is to analyze the effect of the woolwich massage method on increasing breast milk production for the adequacy of babies in postpartum mothers in the Pintu Padang Health Center area, South Tapanuli Regency. Quasiexperimental study Woolwich massage on mammary glands. Population Mother post partum 1-4 days. The total population sample is 20 respondents. Research time June to November 2022. The results of the Pearson correlation test of 0.751 with a significance level (p) of 0.058 indicate a strong correlation and a positive direction where the woolwich massage facilitates the release of breast milk in meeting the needs of the baby. Midwives are motivated to do woolwich massage in the mammary area after assisting in childbirth.

Factors Affecting Breast Milk Output in Post Sectio Caesarea Mothers at RSUD Sultan Sulaiman Kab. Serdang Bedagai Year 2023

Breastfeeding brings joy to new mothers and provides the best start for children. However, this process is always easy, especially for mothers after cesarean delivery (SC). This research explores the factors that influence the release of breast milk (ASI) in post-SC mothers at Sultan Sulaiman Hospital, North Sumatra. In planning a cross-sectional survey, variables such as breastfeeding frequency, breast care, nursing care, diet, and gestational age were evaluated to understand overall breast milk production. The results show that regular breastfeeding frequency and good breast care are associated with smooth breast milk production. However, hospitalisation did not show a significant correlation with breast milk production in mothers after CS.

Breast Care Knowledge and Attitudes in Special Women at the Barru Regional General Hospital, Barru Regency, South Sulawesi

Breast care during the postpartum period is a necessity for mothers who have just given birth. The postpartum period itself is six weeks or 40 days after delivery. During the postpartum period, breast care is a very important action to care for the breasts, especially to facilitate the release of breast milk (ASI). Swollen breasts usually occur in the first days of a baby's birth. Data obtained from Barru Hospital showed five cases of swollen breasts. The aim of this study was to determine the relationship between postpartum mothers' knowledge and attitudes towards breast care at Barru Hospital. This research uses a correlation analytical method with a cross sectional approach, data collection techniques using questionnaires with respondents namely postpartum mothers who receive services at Barru Hospital. Data analysis uses univariate analysis to test respondents' knowledge and attitudes and bivariate analysis to determine the relationship between variables. The results of univariate research with frequency distribution show that some respondents (52.5 %) have less knowledge, the attitude category shows that some respondents (52.5%) have an unsupportive attitude. The results of bivariate analysis with Chi-Square distribution obtained a value of 0.01 (p-value < 0.05). The results of the bivariate analysis showed that very few of the respondents (19%) were 4 people with little knowledge and a supportive attitude. The conclusion of this research is that there is a relationship between postpartum mothers' knowledge and attitudes towards breast care at Barru Hospital. Advice to health workers to carry out continuous outreach and education about breast care.

The Effect of Theory-Based Care on Breastfeeding Self-Efficacy, Anxiety and Breast Milk Release

Objective: The effect of nursing care provided according to Dennis’ breastfeeding self-efficacy theory on breastfeeding self-efficacy, anxiety, and breast milk secretion was evaluated.
 Methods: In this quasi-experimental study, 60 mothers in the first week postpartum were examined. The mothers and their supporters in the Dennis Theory-Based Nursing Care group were trained. Before the training, mothers were given the personal information form and the breastfeeding diary form to be filled out for 15 days. The perception of breastfeeding self-efficacy of mothers was evaluated using the “Postpartum Breastfeeding Self-Efficacy Scale”. Their anxiety was determined using the “State Anxiety Scale”, and breast milk release was assessed using the “Daily Form for Baby’s Adequate Nutrition”. The parameters were measured on the first day and 15 days after the training. Routine hospital standard nursing care was provided to the mothers in the control group.

The Effectiveness of the HYPO Method (Hypnobreastfeeding and Oxytocin Massage) with Breast Milk Expenditure in Postpartum Post-Sc Mothers at RSIA Cahaya Bunda Cirebon City in 2023

With all these factors, it helps mothers and their families, especially their husbands, to be able to do oxytocin massage and hypnosis so that they can release the hormones prolactin and oxytocin to trigger milk release. (Josefa, 2013). The importance of exclusive breastfeeding can be seen from the role of the world, namely in 2006 the World Health Organization (WHO) issued Child Growth Standards which were then applied throughout the world whose contents emphasized the importance of giving only breast milk to babies from birth to 6 months of age. (Prasetyono, 2009). Apart from that, the importance of breastfeeding was also seen in world events, namely the World Breastfeeding Week in August 2008, The World Alliance For Breast Feeding Action (WABA) chose the theme Mother Support: Going For the Gold. The meaning of the theme is a movement to invite everyone to increase support for mothers to provide their babies with gold standard food. This study uses comparative research. That is using a control group design research. In this study there were two groups, namely the control group without treatment and the intervention group with treatment, then the results were measured through observation before being processed in the SPSS test. In the control group without intervention, the results of the majority of breastfeeding were ≤ 20 ml as much as 95%. Meanwhile, in the intervention group with the treatment, there was a majority of > 20 ml of breast milk expenditure, as much as 90%. The average value of breastfeeding was 34.50 ml for the intervention group, 15 ml for the control group. There is an effectiveness between the HIPO method (Hypnobreastfeeding and Oxytocin Massage) and milk production in postpartum post SC mothers at RSIA Cahaya Bunda Cirebon with a significance value of 0.000 <0.05.

The Effect of Giving a Combination of Lavender Aromatherapy Oil and Oxytocin Massage on the Smooth Flow of Breast Milk in Breastfeeding Mothers

<p><strong>Introduction:</strong><strong> </strong>Breast milk is known to have good nutritional content, following WHO recommendations regarding the importance of exclusive breastfeeding for babies during the first six months. The factors inhibiting the release of breast milk are very diverse, and one way to overcome this is by giving a combination of lavender aromatherapy oil and oxytocin massage by massaging the back area of the spine. Combining these two methods can influence the flow of breast milk to increase the release of the hormone oxytocin. The research aimed to identify the effect of a combination of lavender aromatherapy oil and oxytocin massage on the smooth flow of breast milk in breastfeeding mothers.</p><p><strong>Methods:</strong> This type of research is pre-experimental with a one-group pretest-posttest design approach. The number of respondents was 30 breastfeeding mothers using the accidental sampling research method according to the inclusion and exclusion criteria. The independent variable combines lavender aromatherapy oil and oxytocin massage, given three times a week according to the SOP for 15-20 minutes for each respondent. The dependent variable is smooth breastfeeding, conducting interviews and observations before and after the intervention. Data analysis used the Wilcoxon test.</p><p><strong>Result:</strong> The research results before the intervention were that half of the respondents had good breastfeeding fluency, namely 15 people (50%), and after being given the intervention, most respondents had good breastfeeding fluency, namely 20 people (66.7%). The Wilcoxon test results showed a p value of 0.000, which means that there is an effect of giving a combination of lavender aromatherapy oil and oxytocin massage on the smooth flow of breast milk in breastfeeding mothers.</p><strong>Conclusion:</strong> It is hoped that the results of this research can be applied to support exclusive breastfeeding and used as a source of information, reference, and comparison in handling breast milk disorders in breastfeeding mothers for further research

COVID-19 pandemic has made postpartum mothers experience anxiety from mild to fear of crowds and lock themselves at home. So that it affects the expenditure of postpartum mother's milk, because the increase in cortisol makes the hormone oxytocin inhibition in its secretion so that the release of breast milk is inhibited. Knowing the effect of the anxiety of the Covid-19 pandemic on the release of postpartum mother's milk at the Independent Practice Midwife Hj. Faridatul Ampera Performance. This type of research is an analytic survey using a cross sectional design. It is recommended that pregnant women always think positively and seek information about health, especially Covid-19 or do relaxation so that they are not anxious.

THE EFFECT OF BOMB METHOD (BREASTCARE, OXYTOCIN MASSAGE, AND MARMET TECHNIQUE) ON BREAST MILK PRODUCTION IN POSTPARTUM MOTHERS

The smooth process of lactation or the production and release of breast milk is influenced by breast care, frequency of breastfeeding, mother's psychology, mother's health, and contraception. The decrease in milk production is also caused by a lack of the hormones prolactin and oxytocin which play a role in the lactation process. This study aims to determine the effect of BOM method on milk production in post partum mothers. Quasy experimental research design with a Static Group Comparison approach. The population in this study were all post partum mothers in the Working Area of the Melonguane Public Health Center, Talaud Islands Regency in December 2022-January 2023 as many as 34 people. Data collection used purposive sampling, so that a sample of 32 people was obtained. The research instrument used the SOP for the BOM method and a checklist for breast milk production. Data analysis using the Mann Whitney Test. The results showed that all milk production in the intervention group was good, as many as 16 respondents (100%), while in the control group, most were good, as many as 9 respondents (56.3%). The results of the Mann Whitney test showed that the p value (0.000) < ???? Faktor-Faktor Yang Mempengaruhi Ibu Hamil Dalam Melakukan Perawatan Payudara Di Bidan Praktek Mandiri Bena Handayani Desa Lama Kec Hamparan Perak Tahun 2023

BACKGROUND: According to Huliana (2013), breast care is carried out to prevent blockage of the milk ducts thereby facilitating the release of breast milk. This is confirmed by the results of research by Sholichah (2011) which found a relationship between breast care for pregnant women and the smooth production of breast milk. In this study, mothers who had good breast condition with good breast care had good breast milk production. If the condition of the mother's breasts is in poor condition and the care is not good then it can more or less disrupt the lactation process. Breast care is carried out during pregnancy (antenatal) to get the effect of producing breast milk more quickly. This was obtained from the results of research by Astari (2018) that mothers who had breast care during the antenatal period produced breast milk after giving birth faster than mothers who did not have breast care during the antenatal period. OBJECTIVE To analyze the relationship between factors that influence pregnant women in carrying out breast care at the Bena Handayani Independent Practicing Midwife in 2023. METHOD chi square test p value < 0.05 means H0 is rejected and Ha is accepted as a statistical test result of a significant relationship. RESULTS: There is a relationship between education, age, parity, knowledge and attitudes towards breast care at the Bena Handayani independent practice midwife in 2023. Suggestions are given for research and for research sites and for educational institutions regarding breast care.

The Influence of Health Education on Knowledge of Breastfeeding Mothers about the Benefits of Katuk leaves Againts Improvement Breast Milk Production

Katuk leaves play a role in the prolactin reflex and stimulate the alveoli to produce breast milk, because they contain polyphenyls and steroids. It also stimulates the hormone oxytocin so that it stimulates the release of breast milk. Katuk leaves also contain several aliphatic compounds. Katuk leaves which can increase milk production, are thought to originate from the hormonal effects of sterol chemical compounds that are estrogenic. Type of research One Group Pretest Posttest Design with pre-test and post-test design. The sample used was mothers who were breastfeeding with babies less than 2 months old, a total of 30 respondents were taken by total sampling. The research instrument used questionnaires and leaflets. The statistical test uses the Shapiro Wilk test with the SPSS 16.0 program. 50% of respondents had sufficient knowledge and 93.3% of respondents had good knowledge regarding the benefits of katuk leaves in increasing milk production. Based on Shapiro Wilk's statistical test, it was found that p value = 0.000, so that p value = 0.000 < α = 0.05. The influence of health education on health education about the benefits of katuk leaves to increase milk production in nursing mothers. Penyuluhan Kesehatan tentang Pijat Oksitosin Pada Ibu Hamil Trimester II dan III

Milking Machine Settings and Liner Design Are Important to Improve Milking Efficiency and Lactating Animal Welfare—Technical Note

The purpose of milking machines is to harvest milk at optimal quality and speed, while maintaining animal comfort and teat defense mechanisms against invading mastitis pathogens. Therefore, the milking machine is a very important piece of equipment on dairy farms to maintain a long healthy lactation by following the physiological conditions of the udder. The mechanical forces during long-term machine milking processes lead to changes in the teat tissue. This effect is related to the degree of adaptation of the milking machines to the physiological requirements of the individual udder anatomy and the physiological conditions of the lactating animals. If both, milking machine settings and liner design are not suitable for all teats and animals on the farm, some animals will not be fully milked, the teat condition will deteriorate over time and in the end, they may suffer from mastitis. Therefore, maintaining healthy udders and teats during milking is a central key component of an effective milking machine to produce good milk yield with higher quality by preventing mastitis and maintaining animal health and welfare. On large and thick teats, conventional liners often fit too tight, causing a massive mechanical stress load on the tissue. On small teats, however, they often do not adhere sufficiently close to the teat which can cause a considerable air admission and hence liner slips. The new liners, “Stimulor® StressLess” (Siliconform, Türkheim, Germany), have a wave-like lip construction and adapt well to the different teat sizes in a herd, thus ensuring consistent milking of lactating animals. A proper milking machine accommodates all teat sizes and forms, has a low vacuum to effectively open the teat and to stimulate physiological milk release and letdown. In addition, the right pulsation rate will maintain a stable vacuum on the teat area during milking. In conclusion, an ideal milking machine adapts to the morphological, anatomical, and physiological characteristics of the udder and teats of the lactating animals and it should achieve a physiologically ideal milking process that meets high animal welfare standards and increases milk production with a high quality standard.

The Effect of Oxytocin Massage on Discharge Breast Milk for Postpartum Mothers in Batam City

Breast milk (ASI) is the first and best food that must be given to babies because it contains nutrients that are needed in the process of growth and development of children's intelligence. Increasing breast milk production can be done in two ways, namely pharmacologically and non-pharmacologically. One action that can be done to help the release of breast milk is through oxytocin massage which can be done by massaging the area around the back (pars thoratica vertebrae) to stimulate the release of milk. The purpose of this study is to determine the effect of oxytocin footing on breast milk expenditure in postpartum mothers in Batam City in 2023. The type of research used is quasi-experiment with pretest research design and posttest one group design. The data analysis used was Paired t-test and this study was conducted for 5 days. The sampling technique in this study was purposive sampling with 17 respondents. The results of this study based on statistical tests obtained P value = 0.0001 with a difference of 3.06%, which means that it can be a significant difference before and after soy milk consumption. Conclusion: there is a significant influence between oxytocin footing on breast milk production in postpartum mothers in the working area of Batu Aji Health Center Batam City. Researchers hope that postpartum mothers can do oxytocin steps to increase milk production

Acute decrease in mothers' cortisol following nursing and milk expression

The glucocorticoid hormone cortisol is an integral component of signaling pathways related to stress reactivity, energy balance, immune function, and other processes. In animal models, lactation is robustly associated with alterations in glucocorticoid signaling, and limited data suggest that similar changes may occur across human lactation. We asked whether milk letdown/secretion in breastfeeding mothers was associated with changes in cortisol, and whether such effects required presence of an infant. We measured changes in maternal salivary cortisol concentrations before and after nursing, the expression of breastmilk with an electric pump, or control activities. Participants conducted pre-session and post-session sampling (at 30 min) for all conditions, and provided a sample of pumped milk from one session. Both nursing and mechanical expression of breastmilk but not control were associated with equivalent declines in maternal cortisol concentration from pre-session values, indicating an effect of milk letdown on circulating cortisol independent of infant contact. Pre-session maternal salivary cortisol concentration was strongly and positively correlated with cortisol concentration in pumped milk samples, indicating that cortisol ingested by offspring provides a signal of maternal cortisol levels. Self-reported maternal stress was associated with higher pre-session cortisol concentrations, as well as with a larger drop in cortisol following nursing or pumping. These findings demonstrate that milk release—in the presence or absence of a suckling infant—regulates cortisol in mothers, and supports the potential for maternal signaling through breastmilk.

EFEKTIVITAS PIJAT OKSITOSIN TERHADAP PENGELUARAN ASI PADA IBU POST PARTUM DI WILAYAH KERJA PUSKESMAS KARYA PENGGAWA TAHUN 2021

Breast milk is a source of life for children which is very important in the first life of a child. Expression of breast milk can be accelerated by non-pharmacological action, namely through oxytocin massage. The purpose of this study was to determine the effectiveness of oxytocin massage on breastfeeding in Post Partum mothers at Public Health Center Karya Penggawa, Pesisir Barat Regency in 2021.The design of this study used True Experimental with a Randomize Control Trial (RCT) research design. The samples in this study were 30 respondents, 15 control groups and 15 intervention groups with purposive sampling. The instruments used were the observation sheet and SOP. The statistical test used the Chi Square test. The results obtained indicate that oxytocin massage has an effect on breastfeeding. Mothers who received oxytocin massage were 3.2 times more likely to express breast milk than mothers who received warm water compresses. Those who received oxytocin massage had an intensity of expressing breastmilk faster than those who did not. Mothers should pay more attention to the need for breast milk for their children and how to facilitate the release of breast milk as well as family support, especially husbands, which are very much needed.
