Microbiology of wound infection after oral cancer surgery

IntroductionThe aim of present study is to investigate the most common infection pathogen found in the postoperative wounds, following surgical treatment of oral and oropharyngeal cancer, in order to identify the most suitable antibiotic treatment. Patients and methodsWe analyzed patients with squamous cell cancer of oral and oropharyngeal region. In patients who developed postoperative wound infection, wound swabs were taken from three different sites: the cannula, wounds on the neck and wounds in the oral cavity. ResultsIn total 195 patients were included. The postoperative wound infection was detected in 115 patients (59%). In average, the swabs were taken 8 days after the surgery. The similar bacterial species from all three sites were detected in 24 patients (12,3%). In comparison, we found that there was statistically significant difference in the bacteria abundance from all three sites (p=0,031). There were significantly more bacteria in the wounds of the neck than cannula (p=0,007) and in the wounds in the oral cavity than cannula (p=0,002). No statistically significant difference between the wound on the neck and in the oral cavity was found. The most frequently isolated bacterial family was Enterobacteriaceae. Other more commonly isolated bacteria species were Staphylococcus spp. (G+), Pseudomonas aeruginosa (G-), Corynebacteruim spp. (G +) and Acinetobacter baumanii (G-). ConclusionBased on the most commonly isolated groups of pathogens we concluded that probably the best empiric antibiotic treatment of wound infections until antibiogram is completed might be achieved from the group of aminoglycosides or quinolones. Antibiotic therapy should be reviewed if necessary when antibiogram is completed.

Risk factors and microbiology of wound infections following cesarean delivery: Experience of a single institution.

To determine the prevalence of wound infection following cesarean delivery, risk factors, common bacterial pathogens and their antibiotic sensitivity. The study population consisted of 5787 cesarean deliveries. All of the patients received 2g doses of cephazolin perioperatively for antibiotic prophylaxis. Patients with wound infection who had two doses of 1g cephazolin postoperatively and who were continued on oral preparations of 500mg of cephuroxime twice daily for 5 days after hospital discharge were included in Group A. Patients with wound infection whose postoperative antibiotics and antibiotics after discharge were omitted were included in Group B. Patient related variables, gestational age, co-morbidities, cesarean section indications, neonatal intensive care requirements were assessed. Risk factors were evaluated according to the type of the procedure (elective or emergent) and administered antibiotic protocol. The incidence of wound infection following cesarean section was 0,37% in elective operations and 5,4% in emergency cases. On the other hand, wound infection rate was found to be 1,35% in antibiotic receiving group (Group A) and 1,12% in the group not receiving antibiotics (Group B). Increased rate of wound infections were remarkable in emergency cases and postoperative antibiotics did not have a major impact in reducing the rate of wound infection following cesarean section.

Microbiology of Wound Infections Among patients of a Tertiary Hospital in Benin City, Nigeria.

Wound infections are associated with increased morbidity and mortality. Etiologic agents of wound infections vary with geographical locations. This study was conducted to assess the prevalence of wound infections, the effect of age and gender on the prevalence as well as the etiologic agents and their susceptibility to antibacterial agents. Wound swabs collected from 2061 patients were processed to investigate etiologic agents using standard technique. Disc susceptibility of bacterial agents was determined. The overall prevalence of wound infections was 64.8%. The prevalence of wound infections was not significantly affected by gender but was significantly affected by age (P<0.001). The prevalence of wound infections was minimum among age group of <5 years old (20.0%) and maximum among the age group of 36-40 years old (77.5%). Staphylococcus aureus was the most prevalent etiologic agent (21.5%). β-lactams, fluoroquinolones and gentamicin were the most effective antibacterial agents. The prevalence of wound infections was high and was only affected by age. Staphylococcus aureus was the most predominant etiologic agent and β-lactam antibiotics are recommended.

Microbiology of wound infections among hospitalised patients following the 2005 Pakistan earthquake

After a devastating earthquake hit Northern Pakistan in October 2005, we retrospectively analysed the microbiology records of patients admitted with trauma wounds to Shifa International Hospital, Pakistan. For comparison, we included the records of hospitalised patients with wound infections before, and after, the earthquake. Age and hospital stay were entered as continuous variables and mean +/- SD was reported. Gender, type of injuries, micro-organisms isolated, susceptibilities and type of growth (single/polymicrobial) were entered as categorical variables and frequency reported. Relative risk with 95% confidence interval was shown for the two groups. Mean +/- SD age of patients was 35.15+/-11.11 years with female preponderance (58.4%). Fractures were the commonest injuries (55.9%). Gram-negative bacteria were the commonest organisms found (89%), with a predominance of polymicrobial infections (59.6%) and multidrug-resistant organisms (61.5%). Pseudomonas aeruginosa, Enterobacter spp. and Acinetobacter spp. were the commonest isolates, and most of these were multidrug resistant.

Microbiology of Wound Infection after Head and Neck Cancer Surgery

Specimens of pus were obtained from 24 patients who developed postoperative wound infection after head and neck cancer surgery. Aerobic bacteria only were isolated in two instances (8%), anaerobic bacteria only in one (4%), and mixed aerobic and anaerobic bacteria in 21 (88%). A total of 146 isolates were recovered (66 aerobic and 80 anaerobic), an average of six isolates per specimen (2.7 aerobic and 3.3 anaerobic). The most frequently recovered isolates were Peptostreptococcus sp, Staphylococcus aureus, Bacteroides sp, Fusobacterium, and enteric gram-negative rods. Twenty-two isolates recovered from 17 wounds (71%) produced beta-lactamase. These included all five isolates of S aureus and nine of 17 (53%) of the Bacteroides melaninogenicus group. The polymicrobial aerobic/anaerobic nature of postoperative wound infections after head and neck cancer surgery and the presence of beta-lactamase-producing bacteria may have important implications for the management of these infections.
