Utility of UV Signature Mutations in the Diagnostic Assessment of Metastatic Head and Neck Carcinomas of Unknown Primary.

Metastatic carcinoma of unknown primary origin to the head and neck lymph nodes (HNCUP) engenders unique diagnostic considerations. In many cases, the detection of a high-risk human papillomavirus (HR-HPV) unearths an occult oropharyngeal squamous cell carcinoma (SCC). In metastatic HR-HPV-independent carcinomas, other primary sites should be considered, including cutaneous malignancies that can mimic HR-HPV-associated SCC. In this context, ultraviolet (UV) signature mutations, defined as ≥ 60% C→T substitutions with ≥ 5% CC→TT substitutions at dipyrimidine sites, identified in tumors arising on sun exposed areas, are an attractive and underused tool in the setting of metastatic HNCUP. A retrospective review of institutional records focused on cases of HR-HPV negative HNCUP was conducted. All cases were subjected to next generation sequencing analysis to assess UV signature mutations. We identified 14HR-HPV negative metastatic HNCUP to either the cervical or parotid gland lymph nodes, of which, 11 (11/14, 79%) had UV signature mutations, including 4 (4/10, 40%) p16 positive cases. All UV signature mutation positive cases had at least one significant TP53 mutation and greater than 20 unique gene mutations. The management of metastatic cutaneous carcinomas significantly differs from other HNCUP especially metastatic HR-HPV-associated SCC; therefore, the observation of a high percentage of C→T with CC →TT substitutions should be routinely incorporated in next generation sequencing reports of HNCUP. UV mutational signatures testing is a robust diagnostic tool that can be utilized in daily clinical practice.

Carcinoma hemorrhagiectoides: case report of an uncommon presentation of cutaneous metastatic breast carcinoma.

In most cases, cutaneous metastases develop after the diagnosis of the primary internal malignancy has been established, but sometimes they can be discovered earlier or simultaneously. We describe a case of a 90-year-old woman who presented to the emergency room in poor general condition, with cutaneous lesions characterized by hot, infiltrated, violaceous and erythematous plaques involving the left chest wall. The clinical and histopathological findings were consistent with the recently described variant of inflammatory cutaneous metastatic carcinoma named carcinoma hemorrhagiectoides. Microscopic examination demonstrated extensive infiltration of the dermis by tumor cells as well as intralymphatic involvement by neoplastic cells. This is a very rare presentation of cutaneous metastasis from breast cancer.

Carcinoma en cuirasse as an initial manifestation of inflammatory breast cancer.

Carcinoma en cuirasse is an uncommon clinical manifestation of metastatic cutaneous carcinoma characterized by diffuse sclerodermoid induration of the skin. The name was given in the first description by Velpeau in 1838 based on its resemblance to the steel breastplate of cuirassier (cavalry soldier) [1]. Other terms for this distinctive morphological variant of cutaneous metastasis include armoured cancer, Panzerkrebs, scirrhous carcinoma and Acarcine eburnee [2, 3]. Usually carcinoma en cuirasse occurs in patients with local tumour relapse after mastectomy, albeit in some subjects it develops as a presenting feature of primary breast cancer (BC) [4]. Rarely, this form of cutaneous metastasis was associated with other adenocarcinomas (e.g. carcinoma of the lung, kidney or gastrointestinal tract) [5].

Epigenetic alterations in metastatic cutaneous carcinoma

Squamous cell carcinoma (SCC) and basal cell carcinoma (BCC) are the 2 most common cutaneous carcinomas. Molecular profiles predicting metastasis of these cancers have not been identified. Epigenetic profiles of 37 primary cases of cutaneous SCC and BCC were quantified via the Illumina Goldengate Cancer Panel. Differential protein expression by metastatic potential was analyzed in 110 total cases by immunohistochemical (IHC) staining. Unsupervised hierarchical clustering analysis revealed that metastatic BCCs had a methylation profile resembling cutaneous SCCs. Metastatic cutaneous SCCs were found to be hypermethylated at FRZB (median methylation: 46.7% vs 4.7%; p = 4 × 10(-5) ). Metastatic BCCs were found to be hypomethylated at MYCL2 (median methylation: 3.8% vs 83.4%; p = 1.9 × 10(-6) ). Immunohistochemical staining revealed few differences between metastatic and nonmetastatic cancers. Metastatic primary BCCs and cutaneous SCCs had distinct epigenetic profiles when compared to their nonmetastatic counterparts. Epigenetic profiling may prove useful in future diagnosis and prevention of advanced nonmelanoma skin cancers.

Cutaneous Carcinoma: Beyond Mohs Surgery

Program Description: This miniseminar is intended to be a discussion of how best to manage patients with cutaneous carcinomas with aggressive growth features, with local invasion and metastasis that may not be controlled with either traditional extirpative surgery or Mohs micrographic resection. This presentation will be primarily focused on issues related to aggressive growth situations with basal cell and squamous cell carcinomas, particularly those with perineural invasion. How far to go with extirpative surgery versus considerations for other treatments will be discussed. Educational Objectives: 1) Recognize cutaneous carcinomas with aggressive growth features, including best tumor mapping and staging. 2) Interpret available treatment strategies when confronted with cutaneous carcinomas that are not likely to be controlled with surgical extirpation alone. 3) Define current concepts with use of targeted medical therapies in the treatment of advanced and metastatic cutaneous carcinomas.

Nasal Tip Cutaneous Metastases Secondary to Lung Carcinoma: Three Case Reports and a Review of the Literature

Cutaneous metastatic carcinoma of the nose is a rare presentation associated with lung cancer. We report here 3 cases of cutaneous metastatic carcinoma of the nose that originated from lung cancer. Two men, age 61 and 76 years, with lung cancers were referred for evaluation of a tumour on the tip of the nose. The third patient, a 57-year-old man, had developed a rosacea-like tumour on the tip of the nose; although he had no history of internal cancer, whole-body positron-emission tomography-computed tomography revealed a primary lung cancer. Skin biopsies of all 3 cases showed metastatic squamous cell carcinoma, and all primary lung cancers were squamous cell carcinomas. Only 3 patients are described here, and further reports are needed to substantiate this interesting phenomenon. When an elderly patient presents to dermatology with a tumour on the nose with or without known internal cancer, it is necessary to approach the diagnosis with caution.

Cutaneous Metastasis of Endometrial Carcinoma: A Case Report and Literature Review

Cutaneous metastases from endometrial adenocarcinoma are rare with poor prognosis. Despite large autopsy studies quoting a frequency of cutaneous metastases from endometrial adenocarcinoma of under 1 %. The most common presentation of cutaneous metastases is rapidly developing nodules or tumors. The diagnosis of cutaneous metastatic carcinoma hinges on histopathologic evaluation of the involved skin lesion. Cutaneous Metastatic Carcinoma

This study was to highlight the diagnostic and therapeutic values of cutaneous metastatic carcinoma. Presented are patients with histologic diagnosis of cutaneous metastatic carcinoma seen in the University of Calabar Teaching Hospital, Calabar, Nigeria from 2000 to 2006. Six patients (2 men and 4 women) with cutaneous metastatic carcinoma whose ages ranged from 37 to 55 years presented during the study period and accounted for 6.5% of total cutaneous cancer and 0.6% of total malignancy. Cutaneous metastatic carcinoma exhibited a pattern of clinical significance. Clinicians are urged to renew interest in cutaneous nodules, chronic discharging sinuses, and ulcers.

Metastatic Cutaneous Breast Carcinoma: A Case Report and Review of the Literature

Cutaneous metastatic carcinoma, also known as carcinoma erysipeloides, is an unusual clinical finding. In women, it is most frequently found in association with breast cancer rather than other visceral malignancies. Breast cancer patients can present with cutaneous manifestations of breast disease at the time of their initial diagnosis; however, cutaneous metastases more often present well after the initial diagnosis and treatment of the breast disease. Prompt recognition of skin metastases by the plastic surgeon, especially during the delayed setting of reconstructive surgery following mastectomy, allows for earlier intervention in treating the systemic spread of the disease. A clinical case of carcinoma erysipeloides presenting in this delayed setting is outlined in the present report, along with a review of the literature.

子宮頚癌が指端に転移した稀な皮膚癌の症例を報告した。49歳女性。右第4指指端部の腫脹を主訴に来院し救急外来にて膿瘍を疑われ排膿処置をくりかえされたが改善しないため皮膚科受診となった。患者は皮膚科初診の15ヵ月前子宮頚癌(stage I b)に対し切除術を受けたが, その後の局所再発に対し放射線·化学療法が行われていた。病理組織にて表皮と非連続性に腫瘍細胞の胞巣状の増殖を認め, 原発巣との組織学的検討により子宮頚癌の皮膚転移と診断した。過去の報告より手指への癌転移の特徴をまとめた。

Diagnostic immunohistochemistry of cutaneous metastatic breast carcinoma: A statistical analysis of the utility of gross cystic disease fluid protein-15 and estrogen receptor protein

Background: For accurate classification of cutaneous metastatic carcinoma, reliable tumorspecific immunohistochemical markers would be valuable. Objective: Our purpose was to analyze the sensitivity, specificity, and positive predictive value of gross cystic disease fluid protein-15 (GCDFP-15) and estrogen receptor protein (ERP) for diagnosing cutaneous metastatic breast carcinoma. Methods: Tissue sections from 68 consecutive cases of cutaneous metastatic carcinoma were stained for GCDFP-15 and ERP. Hematoxylin-eosin-stained slides and immunostained slides were reviewed in a blinded fashion before retrospective chart review to ascertain the underlying primary tumor type. Results: Of 42 cases of cutaneous metastatic breast carcinoma, 30 were positive for GCDFP-15. Of 41 cases of metastatic breast carcinoma, 30 were positive for ERP. Calculated sensitivity, specificity, and positive predictive value for GCDFP-15 for diagnosing cutaneous metastatic breast carcinoma were 71%, 91%, and 94%, respectively, and 73%, 100%, and 100% for ERP, respectively. Combined values of these indices for both stains were 83%, 91%, and 95%, respectively. Conclusion: GCDFP-15 and ERP are valuable markers for cutaneous metastatic breast carcinoma and should be used in combination.

Uterine Adenocarcinoma Metastatic to the Skin Responsive to Megestrol Acetate

Although endometrial carcinoma is the most common gynecological cancer, it only rarely metastasizes to the skin, with a reported prevalence of 0.8%. We report a case of an elderly lady who recently developed cutaneous metastases from an endometrial primary diagnosed 7 years earlier. Our purpose was to determine if the oral administration of megestrol acetate would be effective in the treatment of uterine adenocarcinoma metastatic to the skin. The index case was given oral megestrol acetate over a period of 9 months. Pre- and posttreatment skin biopsies were performed and analyzed. During treatment with megestrol acetate, the cutaneous metastases in the abdominal skin decreased in area to less than 10% of the pretreatment surface area correlating with a marked histopathologic decrease in adenocarcinomatous glandular structures. Megestrol acetate can be successfully utilized in the treatment of endometrial adenocarcinoma metastatic to the skin as well as ostensibly in the treatment of other progesterone-sensitive metastatic cutaneous carcinomas.

Secondary cutaneous and subcutaneous metastases originating from a visceral carcinoma are relatively uncommon and are associated most frequently with mammary, pulmonary, renal, and colonic cancers. Cutaneous and subcutaneous deposits from cancer of the prostate must be classified as extremely rare, e.g., only one solitary cutaneous metastatic carcinoma of prostatic origin was seen among 136,017 surgical and postmortem specimens submitted to the department of pathology at UCLA Hospital from male patients thirty-five years of age and older during the years between 1955 and 1980. Recently we observed a second case. A discussion of these 2 cases and a review of the English literature are reported.

METASTATIC CUTANEOUS CARCINOMA FROM THE BREAST

CARCINOMA of the breast can produce cutaneous metastases which show a highly diversified clinical picture. 1 Consequently, many descriptive terminologies have been used, especially before the etiology and pathology of the lesions were known or understood. Probably the most common type of metastatic cutaneous carcinoma from the breast is inflammatory carcinoma. According to Lee and Tannenbaum 2 who have reported 28 cases, this particular type occurs in 1.3% of all mammary cancers. Clinically, the skin over and around the breast is red, warm, and slightly brawny, with well-demarcated borders similar to erysipelas. There is usually no ulceration. Pathologically, the dermal lymphatics are invaded by malignant tumor cells, and there is an associated hyperemia and inflammation of the corium. The primary breast tumor can be any histologic type of carcinoma. Other authors have reported similar cases but have used slightly different names.
