Metaplastic Carcinoma Breast in Polycythemic Octogenarian – Lessons Learnt: Case Report

An 80-year-old lady presented with lump Left breast of 8 months duration. Clinically was aT4b lesion, sonomammogram showed BIRADS V lesion and core needle breast biopsy features were suggestive of metaplastic carcinoma breast of adenosquamous type, ER, PR-Positive, Her2Neu-Negative. Patient received preoperative hormonal therapy followed by modified radical mastectomy and postoperative adjuvant chemotherapy and hormonal therapy. Post operative patient showed good clinical recovery and was recurrence free at 6 months follow up although patient had all the poor prognostic factors with large size, LN involvement and high Ki 67. This article is a rare example of metaplastic breast carcinoma presenting in geriatric age group and showing good recovery despite presence of all poor prognostic factors. Hence this report is to enlighten the medical caregivers about the existence of this rare entity and challenges involved in management of this entity.

Metaplastic breast carcinoma: Analysis of clinical and pathologic features, a five-year study.

Metaplastic carcinoma breast (MCB) is a rare tumor comprising of both glandular and non-glandular patterns with epithelial and or mesenchymal components. Due to their varied clinicomorphological features, diagnosis has been challenging. To study the clinicopathological and histomorphology of cases of metaplastic carcinoma breast diagnosed in a tertiary care hospital along with literature review. This is a retrospective study including data of 11 patients who were diagnosed with MCB either on trucut or mastectomy specimens conducted between January 2014 and December 2018. The study includes 11 patients, out of which 10 were diagnosed on mastectomy while one on trucut specimen. All the patients were women with the mean age of presentation being 43.8 years. The most common presentation was palpable breast lump with mean tumor size of 7.3 cm in greatest dimension. Skin involvement was seen in 36.3% of cases. While eight cases (72.7%) were classified as epithelial, three (27.2%) were classified as mixed. Amongst epithelial variety, in eight cases, squamous component was seen along with infiltrating ductal carcinoma (IDC) while one was pure squamous type. In mixed variety, one case showed spindled areas along with squamous areas and areas of IDC. Other two showed focal sarcomatous and cartilaginous areas in one and angiosarcomatous, bone and cartilage formation admixed with areas of IDC in other case. MCB are rare breast tumors with aggressive course and are characterized by their large size and rapid growth rate. Recently, there has been an upsurge in the cases of MCB due to increase in recognition of this entity. It has to be distinguished from other tumors by the pathologists so as to guide proper treatment.

Metaplastic carcinoma breast in male – A case report and review of literature

Male breast cancers are rare and account for 1% of all breast cancer cases reported annually. Incidence of metaplastic breast cancer in females is less than 1% thus metaplastic breast cancers in males are even rarer. Larger tumour size and higher grade differentiate them from invasive carcinoma. They are also more aggressive than invasive carcinoma. On IHC these are typically oestrogen- and progesterone-receptor, Her 2 neu negative. Here we report a rare case of a 35-year-old male patient who presented with left chest wall swelling for six months, histopathological examination showed it to be metaplastic carcinoma breast. The patient underwent left MRM and adjuvant chemotherapy.

Clinicopathological and Radiological Features with Long Term Follow Up of Metaplastic Carcinoma Breast in India.

Objective:Metaplastic breast carcinoma (MBC) is a heterogeneous group of invasive carcinomas with squamous and/or mesenchymal differentiation. Because of their rare occurrence, the information regarding the clinical behaviour of metaplastic carcinomas is limited. The purpose of our study was to delineate the clinicopathological and radiological features, treatment outcomes, prognostic factors, and survival of patients with MBC. Methods:Ambispective observational study with prospective recruitment was done from 1st January 2019 to 31st August 2020. Retrospective data included between 1st January 2009 and 31st December 2018. In the retrospective group surgical database of our department was searched and those with MBC diagnosis on post-operative histopathology recruited. In prospective group patients with MBC on core biopsy were followed and those operated were included. The patients followed up at our breast cancer clinic (BCC) and their demographic, clinical, pathological radiological and treatment details noted. Results:Forty patients formed the study population. The mean age of the patents was 42 years. Ipsilateral axillary lymph node metastasis was present in 22.5%. The pathological median tumor size was 5.4 (range 2.1 to 22 cm). The most common differentiation was cartilaginous (35%) followed by squamous (32.5%). The most common mammographic grading was BIRADS 4 (Breast Imaging Reporting and Data system). Magnetic resonance imaging was T2 hyperintense with peripheral rim enhancement and restriction on DWI. The median overall (OS) and disease-free survival (DFS) was 42 and 40 months, respectively. Fifteen patients (37.5%) had disease related mortality. A subgroup analysis revealed that, type of differentiation, histopathology and tumor size > 5cm affected both OS and DFS significantly. Conclusion:Metaplastic breast cancer in our setup presents in young patients with aggressive large tumors at a higher stage and diverse histopathology and with comparable overall and disease-free survival. The histological subtype, tumor differentiation and tumor size are prognostic factors.

Metaplastic carcinoma breast: Three year histopathological analysis at a tertiary care centre of Kashmir

Background: Metaplastic breast carcinomas (MBCs) comprise a rare but aggressive subtype of breast cancer and represent less than 5% of invasive breast carcinomas. Histologically, metaplastic breast carcinoma is characterized by the presence of divergent cellular differentiation and heterologous elements, including squamous, spindled, sarcomatoid /pleomorphic, chondroid, and osseous differentiation. Material and Methods: Histopathologic characteristics of 11 cases of metaplastic carcinoma of breast encountered over a period of three years from May 2016 to May 2019 in our institute. Results: We encountered 11 cases of metaplastic breast cancers out of 226 primary breast malignancies. Two were breast conservation surgeries and the rest were modified radical mastectomies. The histopathological patterns encountered included squamous, chondroid, sarcomatoid and mixed histologic subtypes. Median tumor size was 3.5 cm, majority were triple-negative for estrogen receptor, progesterone receptor, and human epidermal growth factor receptor 2. All except two patients had lymph nodal metastasis. Conclusion: Metaplastic breast carcinoma is a rare type of breast cancer associated with a poor prognosis. Most patients in this series had high-grade, triple-negative tumors and were treated with optimal therapy.

Coexistence of metaplastic carcinoma of the breast with tuberculous axillary lymphadenopathy: A rare occurrence

Metaplastic breast carcinoma (MBC) is a rare malignant neoplasm of the breast which is highly aggressive and is known for its early recurrence and poor prognosis. Concurrent detection of a MBC and tuberculosis of the axillary lymph node is a rarity. To the best of our knowledge, there is no such case report in the literature. A 35-year-old premenopausal woman was diagnosed as a case of left carcinoma breast with a left axillary lymph node. Core-needle biopsy of the breast was suggestive of squamous cell carcinoma. The patient underwent modified radical mastectomy with a latissimus dorsi myocutaneous flap. The postoperative biopsy of the breast and axilla revealed MBC and tuberculous lymphadenitis of the axillary lymph node. The patient received antitubercular therapy and radiotherapy, followed by chemotherapy and had no recurrence on follow-up. This case report emphasizes that metaplastic carcinoma breast can coexist with tuberculous axillary lymphadenopathy.

Metaplastic Carcinoma of Breast with Osteosarcomatous Differentiation: A Rare Case Diagnosed by Fine Needle Aspiration Cytology

Metaplastic carcinoma is a rare form of breast cancer. It constitutes a distinct aggressive form of invasive breast cancer with histological evidence of epithelial to mesenchymal transition towards spindle, chondroid or osseous cell type. Osseous metaplasia is an exceptionally rare component in metaplastic breast carcinoma. We present a case of 40 yr old female who was diagnosed with Metaplastic carcinoma of breast on fine needle aspiration cytology (FNAC) and subsequently confirmed by histopathological examination. The patient reported to our institution with complaints of painless left breast lump of 5 yr duration. USG findings revealed large infiltrative hypoechoic mass in left upper quadrant of breast suggestive of malignancy. FNAC of lump showed loosely cohesive clusters of malignant spindle cells along with multinucleated giant cell in background of chondroid stroma. A tentative diagnosis of metaplastic carcinoma breast was made. Modified radical mastectomy was performed. Histopathological examination of specimen revealed presence of osteoid lined by malignant osteoblasts like cells interspersed with multinucleated giant cells. A diagnosis of metaplastic carcinoma of breast with osteosarcomatous differentiation was confirmed on histopathological examination. Metaplastic breast carcinoma are heterogenous group of tumors constituting 0.2% of breast carcinomas. Age group and clinical features and radiographic features are similar to other invasive breast carcinomas. Cases is presented for its rarity and utility of FNAC in diagnosing such cases having divergent histological features.

Metaplastic carcinoma of the breast: A report of two cases along with review of literature

Metaplastic carcinomas of breast are uncommon, representing 0.3% of invasive carcinomas of breast. We are reporting two cases of metaplastic carcinoma breast, one revealing rhabdomyoblastic and spindle cell differentiation and other revealing osteoclast like giant cells. Both were estrogen receptor, progesterone receptor and Her2u negative on immunohistochemistry. Early diagnosis of metaplastic carcinomas is essential as these tumors are usually triple negative and hence, do not respond to transtuzumab. Metaplastic carcinomas typically express HER1/epidermal growth factor receptor (EGFR), thus majority of metaplastic carcinomas are treated with EGFR inhibitors such as geftinib and cetuximab. Also, metaplastic carcinomas are associated with a worse prognosis with a disease free survival rate being 78.1% in metaplastic breast carcinomas compared with 91% in infiltrating duct carcinomas.
