Effect of Radiotherapy and Taper of Root Canal Preparation on the Biomechanical Behavior of Mesial Roots of Mandibular Molars

IntroductionThis study aimed to investigate the effect of radiotherapy and taper of root canal preparation on the biomechanical behavior of mesial roots of mandibular molars. MethodsEighty mandibular molars with 2 canals in the mesial root were randomly allocated into 2 groups (n = 40): one group underwent irradiation (60 Gy), while the other did not. Subsequently, the mesial roots were sectioned and each group was subdivided into 5 subgroups (n = 8), according to the preparation taper: no preparation (control); 25.03; 25.04; 25.06; and 25.08. All groups were considered homogeneous regarding their dimensions, weight, and morphology. The prepared specimens were embedded in cylindrical plastic molds and subjected to a cyclic fatigue test. A failure analysis was performed according to the extension and course of the fractures. Two-way ANOVA, Tukey's post-hoc, Fisher's exact, and Kaplan–Meier tests were conducted to evaluate the obtained data (α = 5%). ResultsFatigue resistance decreased as the taper of the preparation increased (P < .05). Preparation 25.03 presented significantly higher values only than 25.08 (P < .05), while 25.04, 25.06, and 25.08 were considered similar (P > .05). Irradiation significantly reduced the biomechanical performance (P < .05). Survival analysis corroborated these findings. There were no differences in the distribution of fracture types among the groups (P > .05). ConclusionThe biomechanical behavior of the mesial roots of the mandibular molars decreased significantly in the face of irradiation and as the taper of the preparation increased.

The effect of root canal preparation tapers on planktonic bacteria and biofilm reduction in the apical third: A correlative microtomography and microbiological laboratory study.

To evaluate the influence of different preparation tapers on the reduction in planktonic bacteria and biofilms of Enterococcus faecalis and Candida albicans in the apical third (4 mm) of the mesial roots of mandibular molars, correlating decontamination with canal shape. After microtomography analysis for morphological standardization of the canals, 48 mandibular molar roots, each containing two canals (96 canals), were contaminated with E. faecalis and C. albicans and divided into four groups (n = 11) for canal instrumentation using ProDesign Logic 2 files with different tapers G (.03): # 25.03; G (.04): # 25.04; G (.05): # 25.05; and G (.06): # 25.06 and irrigation with 2.5% sodium hypochlorite. Four roots were examined under a scanning electron microscope (SEM) to qualitatively assess biofilm formation. Eight roots were used as the negative control group (samples were not contaminated). Bacteriological samples were taken exclusively from the apical third of the roots before and after chemical-mechanical preparation and bacterial counts were determined (CFU/mL). The final micro-CT scan was used to quantify the volume variation and unprepared canal area in the apical third. Statistical analysis was performed using the Kruskal-Wallis, Student-Newman-Keuls and Wilcoxon tests for analysis of microbiological data. anova and the Tukey or Games-Howell test were used for analysis of micro-CT data and Spearman's test for correlations (α = 5%). All groups showed a significant reduction in bacteria (p < .05), with no statistically significant difference between groups. There was no significant difference in per cent volume increase between groups. The unprepared area (Δ%) was affected by the file used (p = .026) and was significantly lower for G (.06) compared to G (.03). There was no statistically significant correlation among bacterial reduction, volume and unprepared area (p > .05). The different preparation tapers influenced root canal shaping in the apical third but did not improve decontamination in this region.

Efficacy of the GentleWave System in the removal of biofilm from the mesial roots of mandibular molars before and after minimal instrumentation: An exvivo study.

To compare the efficacy of Enterococcus faecalis biofilm removal using the GentleWave System (GWS) (Sonendo Inc, CA) on non-instrumented versus minimally instrumented root canal systems. Thirty-four mandibular molars were autoclaved and allocated to four groups: Negative control (n = 5); positive control (n = 5); Group 1: non-instrumentation + GWS (NI + GWS) (n = 12); and Group 2: minimal instrumentation + GWS (MI + GWS) (n = 12). Of 34 samples, 24 samples with Vertucci type 2 configuration within the mesial root of each sample were allocated to Groups 1 and 2 and then matched based on the working length and root canal configuration. After inoculation of samples with E. faecalis for 3 weeks, the GWS was used on Group 1 without any instrumentation and Group 2 after instrumentation of mesial canals until size 20/06v. CFU and SEM analysis were used. Log10 (CFU/mL) from the positive control, and Group 1 and 2 were 7.41 ± 0.53, 3.41 ± 1.54, and 3.21 ± 1.54, respectively. Both groups showed a statistically significant difference in the reduction of viable E. faecalis cells compared to the positive control (Group 1 [p = .0001] and Group 2 [p < .0001]), whilst showing no significant difference between the two tested groups (p < .05). The use of GWS on the non-instrumented root canal system could be an effective disinfection protocol in removing the biofilm without dentin debris formation.

Is the assessment of the mandibular molar danger zone affected by field of view and voxel size in cone beam computed tomography examinations?

ObjectiveTo verify if assessment of the danger zone (DZ) in the mesial root of mandibular molars is affected by field of view (FOV) and voxel sizes in cone beam computed tomography (CBCT) scans. Study DesignForty mandibular molars were scanned by micro-computed tomography, creating the reference standard. The teeth were then submitted for CBCT scans with FOVs of 10 × 5.5 cm and 5 × 5.5 cm and voxel sizes of 0.4, 0.2, 0.15, and 0.075 mm3. The smallest dentin thickness in the DZ from the mesiobuccal and mesiolingual canals was measured at 2, 4, and 6 mm apical to the root furcation. Descriptive statistics, paired t-tests, and intraclass correlation coefficients were used for statistical analysis with significance established at P<0.05. ResultsAll CBCT measurements overestimated the DZ dentin thickness (P<0.001) compared to the reference standard. The greatest overestimation occurred in the 5 × 5.5 cm FOV with 0.4 mm3 voxels (P = 0.007). Dentin thickness measured with the 5 × 5.5 cm FOV and 0.075 mm3 voxels was significantly smaller and produced the best ICC value with the reference standard (0.936). ConclusionsCBCT overestimates the dentin thickness of the DZ regardless of FOV and voxel sizes. The 5 × 5.5 cm FOV showed the best performance with the 0.075 mm3 voxel size, but it performed poorly with 0.4 mm3 voxels.

How Does Intentional Apical Foraminal Enlargement Affect the Foramen and Root Canal Morphology?

IntroductionThis study assessed the effect of intentional foraminal enlargement on the foramen and the apical root canal morphology. MethodSixty mesial roots of mandibular molars were scanned by micro-computed tomography. Their apical foramina were photographed with a stereomicroscope before and after preparation. Three groups were formed (n = 20) according to the working length (WL). G-1: foramen - 1 mm; G0: foramen; and G+1: foramen + 1 mm. Each group originated 2 subgroups (n = 10): G-1: Buchanan's patency (size 10 K-type file) and foraminal debridement (sizes 20, 25 and 30 K-type files); G0 and G+1: rotary foraminal enlargement (ProDesign S size 25/.08) or reciprocating foraminal enlargement (R25). The area, perimeter, transportation, and noninstrumented walls of the foramen were evaluated. The root canal transportation and the centering index of preparation at 1, 3 and 5 mm from the foramen were also assessed. Data were compared statistically (α = 5%). ResultsThe instruments used at the foramen and 1 mm beyond promoted foraminal enlargement and transportation. Regarding NIW, there was no difference between mechanized foraminal enlargements performed at the foramen or 1 mm beyond, similar to the manual foraminal debridement group (P > .05). There was no difference in transportation and centralization at 1-, 3-, and 5-mm apical levels, regardless of the instrumentation systems. ConclusionMechanical preparation at the foramen, or 1 mm beyond, resulted in foraminal enlargement, transportation and were not able to touch all root canal walls that delimit the foramen.

Evaluation of the Quality of Canal Obturation Using the Continuous Wave Method after Shaping with Different NiTi Rotary Instruments by Micro-CT

Abstract Objective The aim of this article was to evaluate the influence of mesial root canal preparation of mandibular molars with the reciprocal systems (RCP) and self-adjusting file (SAF) in the incidence of voids spaces in endodontic obturation performed by modified continuous wave compaction (MCWC) technique, through images obtained in micro-computed tomography (micro-CT). Materials and Methods Mesial roots of 15 mandibular molars, with two independent root canals, were divided into three groups (n = 5/roots), according to the endodontic instrumentation strategy (RCP, SAF, and twisted file [TWF]). After the root canals (n = 10, for each instrumentation system) were filled with MCWC, images of each specimen were obtained by micro-CT followed by three-dimensional reconstruction. The percentage of voids was obtained (in mm3) in relation of the total volume, cervical–middle, and apical thirds. Statistical Analysis The data obtained were submitted to statistical analysis by Kruskal–Wallis and Dunn tests (p &lt; 0.05). Results With respect to the total volume of root canal, TWF demonstrated a lower percentage in comparison to SAF (p &lt; 0.05). No significant difference was obtained between TWF and RCP or RCP and SAF (p &gt; 0.05). In the apical third, RCP showed a smaller percentage of voids when compared with SAF (p &lt; 0.05); however, TWF demonstrated similarity with RCP and SAF (p &gt; 0.05). The middle–cervical third exhibited no statistically difference between TWF and RCP or RCP and SAF (p &gt; 0.05), whereas TWF was lower than SAF (p &lt; 0.05). Conclusion Root canal preparation of mesial root of mandibular molars with TWF provides a lower incidence of voids in the endodontic filling performed by MCWC.

Micro-CT Assessment of Filling Removal Effectiveness in the Apical Third of Curved Canals with Different Types of Anatomy.

The aim of this study was to assess the effectiveness of filling removal material from the apical third of curved mesial root canals of mandibular molars. Reciprocating instrumentation followed by additional rotary instrumentation with instruments made of alloys with different heat treatments was evaluated. Thirty-six mesial roots of mandibular molars were divided into two groups: Group Class IV consisted of 16 roots with two independent canals, and Group Class II consisted of 20 roots with two canals that merged into one at their apical level. Each of these two groups were further divided into two subgroups, according to the additional rotary instrument used after the reciprocating instrumentation: Group RH and Group RM for Hyflex and Mtwo, respectively. After each procedural step, the roots were scanned by micro-tomography. After each step of filling removal, the Wilcoxon matched pair test and the Mann-Whitney test were used for the evaluation between groups. The significance level adopted was 5%. Significant differences were observed between groups with different Class II and Class IV anatomies, regarding filling removal after Reciproc (P<0.05). After the use of an additional rotary instrumentation, no differences were observed between the two groups (P>0.05). In the apical third of mesial roots of mandibular molars with Class II anatomy, an additional rotary instrumentation was shown to be necessary for improving the removal of filling material after using the single-file reciprocating instrumentation technique.

Exploring the Efficacy of 6 Preparation Systems for Achieving Minimal Root Canal Transportation: A Micro-CT Investigation

IntroductionThis study aimed to assess the canal transportation with 6 preparation systems. MethodsSixty curved mesial roots of mandibular molars were scanned, and their root canals (n = 20 per group) were prepared with XP-endo Shaper, BioRace, ProTaper Next, Twisted File Adaptive, Reciproc, or Reciproc Blue systems. After preparation, a new scan was conducted, and transportation was determined by comparing the geometric center of the canal in 13,542 cross-sectional slices. The transport direction frequencies were recorded, and linear transport were compared using a Univariate GLM model and Tukey honestly significant difference tests (α = 5 %). ResultsTransportation was affected by the preparation system and root third (P < .05), with no significant effect observed for the root canal (P > .05). The Twisted File Adaptive had the highest transport (P < .05), and the lowest transports were observed with XP-endo Shaper, ProTaper Next, and BioRace (P < .05). Reciproc Blue and Reciproc showed intermediate results (P < .05). The apical and coronal third exhibited the lowest and highest transportation, respectively (P < .05). A significant interaction was observed between the preparation system and root third (P < .05). In the coronal third, transportation was mostly toward the disto-inside direction, whereas in the middle and apical thirds, transportation was predominantly toward the mesio-outside direction. ConclusionsXP-endo Shaper had the lowest mean linear transportation followed by the ProTaper Next, BioRace, Reciproc Blue, and Reciproc systems. The Twisted File Adaptive system had the worst performance. The direction of transportation was generally toward the inner aspect of the canal at the coronal third and toward the outer aspect of the curve at the middle and apical thirds.

Comparative Evaluation of Different Irrigant Agitating Devices for Debris Removal from the Mesial Roots of Mandibular Molars: An In Vitro Study.

To evaluate the efficiency of debris removal from the mesial root canals and isthmus of mesial roots of mandibular molars after final irrigant agitation with XP-endo Shaper, EndoVac (EV), plastic finishing file, and conventional needle irrigation. Forty extracted human mandibular first molar teeth with the isthmus between the mesial roots were selected, access cavities were prepared and distal roots were decoronated. All the mesial root specimens were mounted in acrylic, sectioned at the coronal, middle, and apical thirds, along with the resin block, and examined for debris in the canals and isthmus before the start of the procedure using a stereomicroscope and were mounted back in a custom-made jig. After determination of the working length, they were allocated to the following four different irrigant activation groups (n = 10): Group I-XP-endo Finisher; group II-EndoVac; group III-plastic finishing file; and group IV-standard needle irrigation. After biomechanical preparation (BMP) till F3, final agitation was done for debris removal with the respective agitating devices, and all the samples were examined under a stereomicroscope for the amount of debris in the canal after final agitation. Of all the irrigation devices used, the XP-endo Finisher and EV showed more debris removal when compared to other groups (p < 0.05). The plastic finishing file in continuous rotation was more efficient in cleaning debris compared with conventional needle irrigation (p < 0.05). The XP-endo Finisher and EV showed a significant difference in cleaning efficacy than the plastic finishing files. So, these can be used as adjunctive for agitating the final irrigant. The irrigant agitation devices promote the penetration of irrigants into the intricate root canal anatomy and increase their antibacterial and tissue-dissolving effectiveness thereby improving the success rate and treatment outcome.

Efficacy of different irrigant activation techniques for cleaning root canal anastomosis

ObjectiveThis study compared in vitro the anastomosis cleaning efficacy of different irrigant activation techniques at different levels; control group non-activation (NA), passive ultrasonic irrigation (PUI) using Irrisafe, and EDDY sonic activation.MethodsSixty anastomosis-containing mesial roots of mandibular molars were mounted in resin, sectioned at 2, 4, and 6 mm from the apex. Then reassembled and instrumented in a copper cube. For the irrigation technique roots were randomly divided into 3 groups (n = 20): group 1: NA, group 2: Irrisafe, group 3: EDDY. Stereomicroscopic images of anastomoses were taken after instrumentation and after irrigant activation. ImageJ program was used to calculate the percentage of anastomosis cleanliness. The percentage of cleanliness was calculated before and after final irrigation within each group and were then compared using paired t-tests. Intergroup and intragroup analyses were performed to compare between different activation techniques at the same root canal level (2, 4 and 6 mm) (intergroup) and to evaluate if each technique had different cleanliness efficacy according to the root canal level (intragroup) using one-way analysis of variance and post hoc tests (p < 0.05).ResultsAll three irrigation techniques significantly improved anastomosis cleanliness (p < 0.001). Both activation techniques were significantly better than the control group at all levels. Intergroup comparison revealed that EDDY significantly achieved the best overall anastomosis cleanliness. The difference between EDDY and Irrisafe was significant in favor to EDDY at 2 mm and insignificant at 4 and 6 mm. The intragroup comparison showed that improvement in anastomosis cleanliness (i2-i1) in the needle irrigation without activation group (NA) was significantly higher in the apical 2 mm level compared to the 4 & 6 levels. While the difference in anastomosis cleanliness improvement (i2-i1) between levels in both Irrisafe and EDDY groups was insignificant.ConclusionsIrrigant activation improves anastomosis cleanliness. EDDY was the most efficient in cleaning anastomoses located in the critical apical part of the root canal.Clinical relevanceCleaning and disinfection of the root canal system followed by apical and coronal sealing is the key for healing or prevention of apical periodontitis. Remnants of debris and microorganisms retained within the anastomoses (isthmuses), or other root canal irregularities may lead to persistent apical periodontitis. Proper irrigation and activation are essential for cleaning root canal anastomoses.

Dentin Thickness of the Danger Zone in the Mesial Roots of the Mandibular Molars: A Cone Beam Computed Tomography Analysis

Mandibular first molars appear to be the most commonly tooth subjected to a root canal treatment, therefore a better understanding of the anatomy critical zones for resistance of this teeth may decrease the treatment's failure rate. So, this study was conducted to evaluate the dentin thickness of the danger zone in mesial roots of mandibular first molars using cone beam computed tomography in an Iranian population. In this cross-sectional study, 210 Cone Beam Computed Tomography acquisition of the mandibular first molars were collected from a radiology center in Qazvin. The dentin thickness of the mesial roots (mesiobuccal and mesiolingual canals) was measured from the furcation to 5 mm below. The relationship between the dentin thickness in the danger zone and parameters, like age, gender, placement side, root length, the curvature of the canal, canal type, presence of middle mesial canal, and distance between the orifices of the mesial canals was investigated. Frequency, mean and standard deviation for variables were calculated, and data analysis was done by SPSS using simple and multiple linear regression and Pearson correlation coefficient. Also, two-sample t-test was used to compare mesiobuccal and mesiolingual on two sides. The significant level was also considered at (p < 0.05). The average minimum thickness of danger zone dentin was found to be 0.885 ± 0.259 mm in the mesiobuccal canal and 0.906 ± 0.206 mm in the mesiolingualcanal. Also, the minimum thickness of dentin in the mesiobuccal and mesiolingual canals in the range of 0 to 1 mm from the furcation was more than in other areas. There was no significant relationship between the minimum dentin thickness of the danger zone with gender, placement side, root length, canal type, and mesial canal entrance distance. But with increasing age, the thickness of dentin in the danger zone in the mesiolingualcanal increased significantly (p = 0.008). It was also observed that with the increase in the curvature of the canal, the thickness of the dentin in the danger zone in the mesiobuccal canal decreased (moderately curved (p = 0.008), severely curved (p = 0.046)). In addition, the thickness of the dentin in the mesiobuccal and mesiolingual canal was less in the samples with the middle mesial canal (p = 0.047, 0.044). Less dentin thickness in the danger zone in the mesial roots of mesiolingual mandibular first molars was seen in younger patients in mesiolingual canal, with a greater degree of canal curvature in the mesiobuccal canal and teeth with a middle mesial canal. Therefore, it is suggested that large taper instruments should be used with more precision to prevent complications.

Diamond-coated ultrasonic tip decreases debris and uninstrumented surface after preparation of curved canals with isthmus.

The aim of this study was to evaluate root canal preparation with nickel titanium rotary instruments and complementary preparation with ultrasonic tip in curved canals of mandibular molars with isthmus. Twenty-eight mesial roots of mandibular molars with curvature between 20° and 40° and presence of isthmus throughout the entire extension of the root canals were prepared using ProDesign Logic CM (PDL) up to size 40.05, or HyFlex EDM (HFEDM) up to size 40.04. Complementary preparation was performed in the isthmus region using the ultrasonic insert E18D (Helse, Istmo Diamantada). The root canals were scanned using micro-CT (SkyScan 1176) at 9 µm voxel size before and after each preparation step. Transportation, percentage of increase in volume, debris and uninstrumented surface (UNS) were evaluated. Mann Whitney, Wilcoxon, paired and non-paired t-tests were used for statistical analysis (α = 0.05). The canals prepared with PDL and HFEDM obtained similar results for all the variables assessed before using E18D (p > 0.05). E18D significantly decreased the percentage of debris and UNS values in both Groups (p < 0.05). The complementary preparations with E18D caused a smaller quantity of debris in the isthmus of the canals previously prepared with PDL in comparison with HFEDM (p < 0.05). PDL and HFEDM provided similar root canal preparation. PDL promoted a smaller quantity of Debris in the isthmus than HFEDM after using E18D. E18D significantly improved cleaning, and reduced Debris and UNS.

Prevalence of Separated Instruments in Routine Endodontic Practice and its Overall Impact on the Procedure: A Questionnaire Study

Background Fracture of endodontic instruments during routine root canal treatment and retreatment procedures has high prevalence. This unfortunate mishap requires certain expertise to diagnose and manage without any further complications.Objectives The objective of the present questionnaire study was to assess the knowledge of the clinician to diagnose an instrument separation during routine endodontic procedure and the steps carried out to manage this unfortunate mishap.Methods An online questionnaire comprising of 26 questions was circulated on social media groups to be filled by undergraduate postgraduate students practicing dental surgeons and specialists. All the responses were recorded and descriptive analysis of all the variables was done using frequency and proportion for categorical variables and Chi square test was used to compare the proportions. The level of significance was set at p lt0.05.Results The results showed that 75.8 of the participants could diagnose instrument separation. About 73.6 noticed instrument separation in the apical third of the root canals and 48.1 of the separated instruments were found in the mesial root of mandibular molars. Around 59.3 of the respondents opined that the separation was because of over usage of the instruments. 61.3 of them could bypass and complete obturation. Multiple visits were preferred for the management of separated instrument by 68.9 of the respondents.Conclusion Within the limitations of the study we conclude that most clinicians could diagnose the instrument separation and manage it by either removing or bypassing the instrument.

The Evaluation of Debris and Smear Layer Generated by Three Rotary Instruments Neo NiTi, 2Shape and Revo_S: An Ex-vivo Scanning Electron Microscopic Study.

This study compared the cleaning effectiveness of NeoNiTi, 2Shape and Revo_S rotary instruments. Fifty mandibular molar mesial roots were selected with an angle of curvature less than 20 degrees divided into three groups (n=15). Five samples were selected as negative control group. In all three systems, the final file was 25, 6%. The score of debris and smear layer in three thirds (coronal, middle and apical) of the root canal walls were evaluated using scanning electron microscopic (SEM) magnification. The data were analyzed using the Kruskal Wallis and Mann Whitney U tests for intergroup comparison (P≤0.05) and Freidman and Wilcoxon signed-rank test was employed for intragroup comparison (P≤0.05). Residual debris of the 2Shape system in the apical region was significantly higher than the other two systems (P=0.039). Revo_S and 2Shape groups had significantly higher quantities of debris in the apical than the coronal region (P=0.029 and P=0.02, respectively). In the 2Shape group, the amount of mid-region debris was significantly higher (P=0.005) than the coronal. In inter-group comparison there was no significant difference in residual smear layer between the systems. In intra-group comparison in all three systems, the amount of smear layer in the coronal third was significantly higher than in the other two areas. (P=0.017, P<0.001 and P=0.032, respectively). 2Shape left the highest amount of debris in the apical region. The amount of debris in Revo_S and 2Shape groups in the apical region was significantly higher than in the coronal. The amount of smear layer in all three groups in the coronal area was higher than the middle and apical areas.

Radicular dentinal microcracks during biomechanical root canal preparation using four minimally invasive endodontic files.

The aim of the study was to compare the dentinal microcrack formation on human mandibular molars using 4 different minimally invasive file systems at 3, 6 and 9 mm from apex using a stereomicroscope. 125 mesial roots of mandibular molars were randomly divided into 5 groups and instrumented (n=25). Group 1: Control Group (CG), Group 2: Self Adjusting File (SAF), Group 3: XP-Endoshaper (XP), Group 4: TRUShape (TS), Group 5: V Taper 2H (VT). After instrumentation was completed, the roots were sectioned at 3 mm, 6 mm, and 9 mm from apex using a slow-speed circular saw. Digital images were captured using a 24× stereomicroscope by using a digital camera. Two operators independently checked each specimen for the presence of dentinal defects. Statistical comparison between the file systems was done using Chi-square (p<0.05). There was no statistically significant difference in the frequency of microcracks between the groups of file systems (X2=4.509, p>0.05). VT file system exhibited higher microcracks (25%) but was not statistically significantly higher than XP (12%), TRUShape (12%), and SAF (4%) endodontic file systems. Within the limitations of this study, it can be concluded that all the file systems used in our research are minimally invasive files and produced few dentinal microcracks. VT files produced maximum while SAF produced the least number of micro-cracks.

Effect of Additional Apical Preparation on Retreatment of Curved Root Canals Filled with Different Sealers.

This study assessed the influence of additional apical preparation on material removal during retreatment of curved root canals filled with different sealers. Twelve mesial roots of mandibular molars with two separate canals and curvature between 25 and 35 degrees were selected. The working length was established 1-mm short of the apical foramen, and all the root canals were prepared using ProDesign R (PDR) 25/0.06. After preparation, the root canals were divided in two groups (n = 12) and filled by continuous wave condensation technique with NeoMTA Plus or AH Plus. All the root canals were retreated using rotary ProDesign Logic RT 25/0.08, reciprocating PDR 25/0.06 and apical preparation with PDR 35/0.05. Additional apical preparation was performed with ProDesign Logic (PDL) 50/0.01. The samples were scanned using a Skycan 1176 micro-computed tomography (micro-CT), voxel size 8.74 µm, before and after the retreatment procedures. Percentages of remaining filling material were evaluated. The data were submitted to paired and unpaired t-tests (α = 0.05). Percentage of remaining filling material was similar between the root canals filled with NeoMTA Plus or AH Plus sealer after retreatment using PDR 35/0.05, and after additional apical preparation using PDL 50/0.01 (p > 0.05). However, PDL 50/0.01 significantly decreased the percentage of remaining filling material in the apical third after the additional apical preparation for both sealers (p < 0.05). NeoMTA Plus or AH Plus did not influence the retreatment of curved root canals. ProDesign Logic 50/0.01 promoted greater filling material removal in the apical third.

Evaluation of Remaining Dentin Thickness of the Distal Portion of the Mesial Root of Mandibular Molar with Protaper Gold, Neoendo and Stainless Steel (SS) Hand K Files Using Cone Beam Computed Tomography for Analysis.

Aim: To evaluate the remaining dentin thickness of the distal portion of the mesial root of mandibular molar with stainless steel (SS) hand K files, ProTaper Gold and NeoEndo using cone beam computed tomography for analysis.&#x0D; Materials and Methods: Forty-five extracted human mandibular molars were collected. Teeth were randomly assigned to three groups (n=15). Samples were stabilized on blocks of wax. The working length for each sample was determined at 1 mm short from the apical foramen by using a ISO size #15 K file. Preparation was carried out according to the manufacturer’s instructions and canals were instrumented upto apically for each group. After each instrumentation, root canals were irrigated with 5.25% sodium hypochlorite solution followed by 17% EDTA solution. Final irrigation was done with normal saline. Post cleaning and shaping, cone beam computed tomography scans of all samples in the three groups were collected.&#x0D; Result: Protaper Gold and NeoEndo removed more dentin than SS hand file. As the mean remaining dentin thickness for SS K file, Neoendo and Protaper was 1.30, 1.05 and 0.80mm respectively&#x0D; &#x0D; Conclusion: Protaper Gold, NeoEndo and SS Hand files showed statistically significant difference in remaining dentin thickness of root canal length. Therefore, it was concluded that Protaper Gold file system showed a greater amount of removal of dentin as compared to NeoEndo file system &amp; SS Hand K files.

Multispecies biofilm removal by a multisonic irrigation system in mandibular molars.

The aim of the study was to assess biofilm removal efficacy of GentleWave System and passive ultrasonic irrigation (PUI). Twenty-two human mandibular molars with Vertucci's type II configuration in the mesial root were selected. Teeth were autoclaved, inoculated with dental plaque and incubated in a CDC biofilm reactor for two weeks. The mesial roots were instrumented up to 20.06 file (V-Taper) for the GentleWave group and up to 35.04 file (Vortex Blue) for PUI group. Irrigation was performed using GentleWave and PUI irrigation protocols (n=11). Dentine debris on paper points samples were obtained for quantitative real-time polymerase chain reaction (qPCR) and 16S ribosomal RNA gene sequencing (next-generation aequencing-NGS). For qPCR, a non-parametric test (α=0.05) was used. Next-generation sequencing data were analysed using mothur, with alpha diversity calculated as the Shannon and Chao1 indices and Bray-Curtis dissimilarities were used for beta diversity. Differences in alpha diversity and abundances of genera were evaluated using Kruskal-Wallis test. Differences in community composition were evaluated using analysis of similarity with Bonferroni correction for multiple comparisons. Quantitative real-time polymerase chain reaction results showed that the reduction estimated in percentages for both groups was equivalent (p > .05). NGS analysis showed that both techniques promoted a significant reduction in reads and OTUs number (p < .05). Shannon alpha diversity and Chao1 index showed no differences between pre- or post-treatment samples for both groups (p > .05). Additionally, pre-treatment communities differed from post-treatment samples in both groups regarding bacterial taxa reduction (ANOSIM R=0.50 and 0.55, p< .001). Bacterial reduction in mesial roots of mandibular molars prepared to 35.04 with PUI was similar to those prepared to 20.06 with a multisonic irrigant activation system.
