Interesting surgical cases of mesenteric cystic lymphangioma

Mesenteric cystic lymphangioma involving the gastrointestinal tract is uncommon in infants and children. It is a benign tumor due to abnormal formation lymphatic system. These tumors exhibit symptoms varying in site and size of localization. Usually, symptoms are manifested by acute abdominal pain, intestinal obstruction, rising body temperature, and diarrhea. Lymphangioma can easily misdiagnose with other conditions like intestinal obstruction, acute appendicitis, or ovarian cyst in a female child. Therapeutic interventions such as fibrin glue, steroids, bleomycin, OK-432, and laparoscopic-assisted surgery are available. From 2018 to 2022, the Regional Paediatrics Hospital of Grodno, Belarus has recorded two cases of mesenteric lymphangioma. Both were emergency cases. In this scientific paper, we have described a surgical approach with improved outcomes. In both cases, surgical resection of lymphangioma that was rooted alongside the ileum was done. We used laparotomy in our surgical procedure, however, the approach was different, one being lower median laparotomy and another being a transrectal approach. Post-operatively patients were in satisfactory condition. Even though mesenteric lymphangioma is a benign condition, it should be immediately diagnosed and surgically managed to avoid morbidity, and mortality due to an increase in size that can fatally compress the adjacent organs impairing the function of an organ and complications like intestinal obstruction and bleeding. Our approaches with case-1 and case-2, both resulted in effective surgical and post-surgical outcomes. But more data is required to make a population-based influence depending on our approach.

A rare cause of acute abdomen in childhood: coexistence of mesenteric cystic lymphangioma and acute appendicitis

Abdominal cystic lymphangiomas are benign congenital anomalies of the mesenteric and retroperitoneal lymphatics, which are not common in the literature. Diagnosis is usually made by physical examination, ultrasonography, and computed tomography. If a significant portion of adjacent organs is not to be sacrificed, complete surgical removal of the mass is the therapeutic option for symptomatic abdominal cystic lymphangiomas. In this study, a patient with mesenteric cystic lymphangioma who underwent surgical treatment is presented. A result of the radiological examinations performed on a 17-year-old female patient who applied for intermittent abdominal pain and a mass in the abdomen as a result of the well-defined, septated mesenteric cystic lymphangioma and acute appendicitis were detected. The patient underwent a diagnostic laparoscopic procedure and then an open surgical treatment, and excision and resection anastomosis were performed on the mass surrounding the jejunum approximately 100 cm distal to the ligament of Treitz. The patient did not have any postoperative problems and was discharged with recovery on the 5th day after surgery. Mesenteric cystic lymphangiomas should be kept in mind in patients with intermittent abdominal pain and abdominal mass, and surgical treatment should be applied in selected cases, as it may rarely cause serious complications.

Abdominal cystic lymphangiomas in pediatric cases

Mesenteric cystic lymphangiomas are benign but rare neoplasms, primarily seen in children. Among internal organs, these masses are commonly found in the ileal mesentery. In this case series, we presented nine pediatric patients with abdominal lymphangiomas admitted to the surgery ward during 2010 and 2020, of which 6 were boys and three girls. Symptoms varied from abdominal distention to chronic and acute abdominal pain, anorexia and vomiting, and a scrotal mass in our patients. Considering various presentations of these patients and possible critical complications, surgeons need to have lymphatic malformations in mind as a differential diagnosis in such cases to improve the outcome.

Case of Abdominal Cystic Lymphangioma Seen at The University Hospital of Antananarivo HJRA

Cystic lymphangioma is a benign malformations of the lymphatic system. Most of them are found in the head and neck. The abdominal localization represents 2 to 5% of cases and poses a preoperative diagnostic difficulty. Most of the diagnosis is made following laparotomy indicated because of signs of complications. We report one case of mesenteric cystic lymphangioma in children. The revealing manifestation was chronic vomiting with repercussions on the general condition. The diagnosis was not established until after the histopathology examination of the resection piece. Resection was complete. No recurrence was observed. The quality of resection determines the postoperative outcome and prognosis of abdominal cystic lymphangiomas.

Mesenteric Cystic Lymphangioma Induced Recurrent Abdominal Pain And Signs of Intestinal Obstruction: A Case Report

Mesenteric cystic lymphangioma is an uncommon benign abdominal mass. This is a case of a 40-year-old female who presented to the hospital with recurrent abdominal pain and distention. Computed Tomography and magnetic resonance imaging tests suggested a mesenteric cyst. The patient underwent laparotomy which confirmed the presence of a mesenteric cyst along with small bowel narrowing at the site of the cyst caused by multiple adhesive bands released during the procedure. The cyst was found and excised approximately 200 cm away from the ileocecal junction, and was pathologically confirmed to be a mesenteric cystic lymphangioma. These findings support that mesenteric cystic lymphangioma could cause chronic abdominal pain and intestinal obstruction.

Infected mesenteric cystic lymphangioma as a cause of pediatric acute abdomen

Mesenteric cystic lymphangiomas are rare intraabdominal tumors, most often seen in the pediatric population. These tumors manifest in a variety of ways and can present as an acute abdomen secondary to infection, volvulus, or ischemia. We report a case of a two-year-old presenting with peritonitis resulting from infection of multiple mesenteric cystic lymphangiomas and discuss changing management strategies as seen in the literature.

Mesenteric cystic lymphangioma – A rare entity with unusual presentation

Lymphangiomas are a rare benign proliferation of the lymphatic system. Intra-abdominal lymphangioma is rare and accounts for <5% of lymphangioma cases. Cystic mesenteric lymphangioma is extremely rare and accounts for <1% of abdominal lymphangiomas. Abdominal swelling, pain, vomiting, and constipation are common clinical presentations. We report a case of a 5-year-old boy with no significant medical history of illness who presented with acute abdomen. Preoperative accurate diagnosis by radiological modalities is very difficult. Surgical excision is the choice of treatment. Definitive diagnosis requires histopathological examination.

Cystic lymphangioma is a benign congenital malformation of the lymphatic vessels. At the National Center for Pediatric Surgery, Gezira State, Sudan; 2021; a female baby of 15 months old age presented complaining of abdominal distention of 10 days duration. Patient Looks unwell, not febrile, and not pale or jaundice; abdomen was distended with full flanks, there was a palpable mass in the epigastric region, no area of tenderness and no palpable organs. Abdomino-pelvic U/S showed very huge abdomino –pelvic cystic mass with multiple thick septa. CT abdomen showed huge abdomino-pelvic mass with multiple cysts that having variable sizes measuring up to 6*7 cm. An elective explorative laparotomy was done. Intra operative findings: Multiple cystic masses arise from the root of the small bowel mesentery matted together with multiple enlarged mesenteric lymph nodes involving segment of distal jejunum with multiple adhesions. Release of adhesion, resection of the masses along with 57 cm of the involving jejunum, anastomosis was done, and homeostasis was secured. Histopathology result showed dilated cystic spaces; lined by flat endothelial cells and multiple reactive lymph nodes, no evidence of malignancy. Features are in line with lymphangioma and associated mesenteric lymphadenitis. Discussion and Conclusion: Our case was 15 months old which is the age of presentation as mentioned internationally. Mesenteric cystic Lymphangiomas are more common among male unlike our case. Ultrasonography and CT abdomen were highly recommended to achieve early diagnosis and treatment of such cases.

Rare mesenteric cystic lymphangioma presenting as acute intestinal obstruction in a 10-year-old boy

A 10-year-old previously healthy boy with no prior surgical history presented with three days of generalized abdominal pain, nausea, and emesis. Physical exam revealed a diffusely tender and distended abdomen with pain out of proportion to exam. Abdominal x-ray revealed findings consistent with small bowel obstruction. He underwent emergent abdominal exploration finding a volvulized portion of small bowel due to a small bowel mass. A 25cm × 18 cm mass associated with small bowel was removed and small bowel anastomosis performed. Post-operative pathology revealed a mesenteric cystic lymphangioma. The patient was subsequently discharged without post-operative complications to date. A review of the literature revealed less than 200 reported cases of mesenteric cystic lymphangioma. We encourage accurate reporting of this unique pathology for the benefit of surgical work-up and planning, epidemiologic tracking and outcomes.

Case Report: Cystic lymphangioma of the omentum

BACKGROUND: Mesenteric lymphangioma is a rare, benign congenital cystic tumor of the lym-phatic vessels, which occurs in less than 5% pf the cases at the abdomen. They appear most often during childhood. This tumor should be considered as a differential diagnosis of acute abdomen and other abdominal masses. CASE REPORT: A 6-year-old boy presented with abdominal pain and peritonism. He presented a low-grade fever, a positive rebound sign, as well as leukocytosis and neutrophilia. Ultrasono-graphy showed abundant free fluid in abdomen and pelvis. EVOLUTION: Exploratory laparotomy was performed, finding a cystic mass dependent on the greater omentum, this mass was resected. Histopathology was negative for malignancy and compatible with cystic lymphangioma, immunohistochemistry was positive for D2-40 marker. The patient evolved favorably without surgical complications and without evidence of recurrence up to one year of follow-up. CONCLUSION: Mesenteric cystic lymphangioma can present with symptoms of acute abdomen, it can also have a chronic evolution. Complete resection is the treatment of choice, currently minimally invasive procedures are performed with favorable results. Ultrasound is useful for long-term follow-up.

Mesenteric Cystic Lymphangioma in Pediatric Patient: A Rare Intra-Abdominal Tumor Management in Rural Country Case Report

Introduction: Mesenteric Cystic Lymphangioma (MCL) is a benign malformation of the lymphatic system. MCL seldomly occurs with a very rare incidence of around 1: 250,000. This neoplasm is more common in pediatric patients. Symptoms can appear with various clinical features ranging from asymptomatic abdominal mass to acute abdomen.&#x0D; Case description: Here we report a patient with MCL: that presented with bilious vomiting, abdominal pain, and abdominal mass. The patient underwent explorative laparotomy, mass excision, and jejunal resection.&#x0D; Discussion: Although benign, MCL can cause other symptoms such as bleeding, torsion, or lymphangioma rupture. Therefore, MCL should be considered as one of the differential diagnoses in acute abdominal and abdominal mass cases in children.&#x0D; Conclusion: In conclusion, although sometimes asymptomatic and found incidentally, the MCL should be resected because it has the potential to grow and invade vital organs and cause life-threatening complications.

Unusual fetal ascites and spontaneous bladder rupture in a female fetus: a case report

BackgroundFetal bladder rupture causing urinary ascites is uncommon. It is generally related to invasive fetal medicine procedures or obstructive disorders such as in posterior urethral valves in male fetuses. An exceptional case of spontaneous bladder rupture in a female fetus occurred in a pregnant woman treated with high doses of opiates in an intensive care unit. This unusual obstetric situation leads to discussion of the possible causes of fetal bladder rupture, its management, and the pediatric prognosis.Case presentationWe report the case of a 30-year-old nulliparous black woman with a history of mesenteric cystic lymphangioma and multiple bowel resections leading to chronic malabsorption. During her pregnancy, our patient presented with an occlusive syndrome and major bilateral renal dilation. Urinary derivation resulted in iatrogenic bilateral ureteral perforation. Our patient thus presented with major uroperitoneum, bilateral pleural effusion and acute renal failure, treated by thoracic drainage and bilateral nephrostomy. Postoperative pain required treatment with level III analgesics. In this context, 5 days after morphine treatment introduction an enlarged fetal bladder was observed, followed 3 days later by voluminous fetal ascites. The diagnosis of spontaneous bladder rupture was suspected. After multidisciplinary discussion, expectant management was decided. At 31 weeks and 4 days gestation, our patient went into spontaneous labor with a subsequent vaginal delivery. The infant required resuscitation and paracentesis of ascites at birth. Her neonatal course was favorable with a simple urethral bladder drainage. Cystography at day 9 was normal. At 2 years of follow-up, the mother and the child have a normal course.ConclusionsAn iatrogenic origin of megacystis in a female fetus must be evoked in the event of maternal administration of high doses of opiates in the second part of her pregnancy. In our case, the megacystis was followed by spontaneous bladder rupture at 30 weeks of gestation, with a favorable maternal fetal issue.

Abstract Lymphangioma accounts for 5% of benign pediatric tumors and less than 1% is located in the abdomen. Indeed mesenteric cystic lymphangioma is a rare benign abdominal mass and its pathogenesis is still unknown. Mesenteric cystic lymphangioma is often diagnosed incidentally. Nevertheless, in some cases its clinical presentation can mimic several common surgical pathologies. Here we describe a case of mesenteric cystic lymphangioma mimicking an ileo-colic intussusception.

Cystic lymphangioma in the peripheral jejunal mesentery in an adult and excision with laparoscopic-assisted surgery: a case report

BackgroundLymphangiomas are uncommon congenital malformations that present mainly in the head, neck, and axillar regions in pediatric patients. Mesenteric cystic lymphangiomas (MCLs), which occasionally present with substantial growth and the invasion of adjacent vital structures, are rarely reported in adults. We report a case of MCL in an adult who was treated with laparoscopic-assisted excision.Case presentationA 40-year-old Japanese man visited his family physician for prolonged periumbilical pain. Plain computed tomography (CT) showed a low-density mass in his left abdomen, and he was referred to our hospital 2 weeks later. His abdomen was flat and soft, and no mass was felt upon palpation. Routine laboratory data showed no abnormalities in the blood cell counts. The levels of tumor markers, such as carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA19-9), and cancer antigen 125 (CA125), were within normal ranges. Contrast-enhanced CT was performed, and a low-density mass was observed with an irregular outline and poor contrast, as well as involvement of the peripheral mesenteric artery and partial compression of the adjacent jejunum without dilatation of the oral side of the bowel. The patient was diagnosed with lymphatic cysts and observed for 1 month without symptom exacerbation. Follow-up CT showed no increase in the size of the mass but showed apparent invasion of the jejunal wall without bowel obstruction. Magnetic resonance imaging (MRI) showed intermediate intensity on T1-weighted imaging (T1WI) and high intensity on T2-weighted imaging (T2WI). The coronal view on T2WI clearly showed an accumulation of cystic lesions. We performed tumor excision with partial resection of the jejunum in a laparoscopic-assisted manner. Pathological examination showed multicystic lesions with an attenuated endothelial lining, surrounding rich adipose tissue and scattered smooth muscle fibers; the patient was diagnosed with MCL.Immunohistochemical assays supported this diagnosis.ConclusionsThis is rare case of MCL presenting in an adult who underwent successful laparoscopic-assisted resection. Mesenteric lymphangioma (ML) should be considered in the differential diagnosis of patients with intraabdominal cysts. Radical excision is optimal, even when the patient is asymptomatic. Laparoscopic-assisted tumor resection is a suitable surgical method for treating MLs located in the peripheral mesentery.

Giant mesenteric cystic lymphangioma: a rare cause of intra-abdominal catastrophe

Lymphangiomas are uncommon benign tumours and occur mainly in children with the most common sites being head and neck followed by the axilla and mediastinum. Intra-abdominal lymphangiomas are rare. Clinical presentation is diverse ranging from incidentally discovered abdominal mass to symptoms of acute abdomen. Life threatening complications are more likely to develop in children. We here present a rare case of giant mesenteric cystic lymphangioma causing intra-abdominal catastrophe in an adult.
