Barriers for Using Electronic Health Records (EHRs) by Nurses in Qatar: A Cross Sectional Study

Introduction: Electronic Health Records (EHR) have been implemented by multiple hospitals all around the world. EHR initiatives tend to be driven by the promise of availability of patient data and enhanced system integration by the need to improve efficiency and cost-effectiveness and by a changing doctor-patient relationship towards where care is divided among a multidisciplinary members of health care professionals and/or by the need to deal with a more complex and rapidly changing environment. The aim of this research is to determine the barriers faced by nurses in Qatar in using EHRs in terms of knowledge, skills, and attitude (KSA).
 Methods: A survey was sent to approximately 12,000 nurses from different hospitals under Hamad Medical Corporation in Qatar through the corporation's official e-mail from January 1, 2021 to May 31, 2021. The questionnaire was developed and validated by the researcher of the study (α=0.877). The Statistical Package for the Social Sciences (SPSS) version 25.0 was used to analyze the data.
 Results: A total of 262 nurses participated in the study. Majority of the nurses who participated are females (69%) and ages 31-35 years old (50.4%). Among the respondents 98.5% are staff nurses with various educational levels: bachelor’s degree in Nursing (43.5 %), diploma in Nursing (27%), and Master of Arts in nursing (3.8%). Only 23.3% of the respondents have a length of service of less than 4 years; 76.7% have been with their institution more than 4 years. The majority of respondents came from the inpatient areas (37%), followed by the outpatient nurses (15.3%), and operating room nurses (14.1%). Around 91% of the respondents claimed to have a 0-3 number of trainings in the past 3 years. A significant correlation was found between age (r=-0.124, p=0.045), length of service (r=0.193, p=0.002), and area assignment (r=0.129, p=0.037) with the skill on EHR.
 Conclusion: Results of the study showed that the dimension of knowledge, skills, and attitude towards the use of EHR is a barrier, but only to a moderate extent. The nurses in Qatar are highly educated, well experienced and are mostly millennials. This opens a great opportunity of acceptance and amplification of EHR in Qatar with the proper training and support to nurses.

BACKGROUND Coronavirus was noticed in the mid-1930’s and the first case of infection in humans was reported as a cold in 1960. In December 2019, clusters of pneumonia cases caused havoc in China. In order to eliminate the infection in the current pandemic, World Health Organisation (WHO) has furnished several recommendations, online courses, and training sessions to increase knowledge, prevention, and control among the health care workers (HCWs). It is crucial to guarantee the safety of not only the health care professionals but also the people who are in close contact with them. We wanted to assess the attitude and safety practices of family members of health care professionals dealing with Covid-19 patients and if the increasing number of cases in India have altered their reaction. METHODS A cross-sectional survey-based study was conducted from July - August 2020 on HCWs dealing with Covid-19 patients and any close contact personnel such as family members of HCWs in order to avoid the spread of Covid-19 among individuals aged 14 - 60 years. RESULTS A total of 368 respondents participated in the study. Majority of the respondents had a positive attitude. In terms of relation, children had a negative attitude towards having their family member deal with Covid-19 patients (46.2 %; N = 46) whereas, 51.5 % (N = 35) spouses had a more positive outlook and 49.2 % (N = 181) respondents reported that the increasing cases in India had altered their initial reaction. CONCLUSIONS Majority of the participants from our study reported a positive attitude towards a family member working in close contact with a Covid-19 patient. More than 50 % of the participants had undertaken various precautions to restrict the spread of the disease. Significado dos Cuidados Paliativos para a Qualidade da Sobrevivência do Paciente Oncológico

Introdução: Os avanços e as melhorias no diagnóstico e tratamento do câncer têm proporcionado, no Brasil e no mundo, o aumento do número de sobreviventes. Embora promissor, o aumento de sobreviventes resulta em mais pessoas convivendo com os efeitos crônicos e tardios do câncer e seus tratamentos, requerendo uma nova reflexão acerca dos impactos sociais, da formação de políticas públicas e dos modelos assistenciais voltados para a oferta de cuidados paliativos em oncologia. Objetivo: Compreender as relações de interdependência e complementaridade entre a aplicação do conceito de paliação e qualidade da sobrevivência do paciente oncológico, na perspectiva da equipe multidisciplinar. Método: Pesquisa qualitativa com base na hermenêutica dialética, realizada em hospital de referência em oncologia no Rio de Janeiro, com participação de nove profissionais de saúde integrantes da equipe multiprofissional, com dados produzidos em dois grupos focais, entre julho e agosto de 2015. Resultados: Quatro amplas categorias empíricas foram identificadas: noção de sobrevivência em oncologia; sobrevivência, paliação e linha de cuidados em oncologia; paliação e qualidade da sobrevivência em oncologia; e sobrevivência e avaliação do modelo de paliação institucional. Conclusão: As necessidades de paliação dos sobreviventes ao câncer demandam políticas específicas de controle de câncer que envolvam a formação de um modelo de planejamento de assistência orientado por uma definição de sobrevivência mais ampliada, fundada em estudos acerca das necessidades físicas, emocionais, econômicas e sociais desse crescente grupo.

Validating a Web-based Diabetes Education Program in continuing nursing education: knowledge and competency change and user perceptions on usability and quality.

BackgroundNurses as the members of health care professionals need to improve their knowledge and competencies particularly in diabetes mellitus through continuing nursing education programs. E-learning is an indirect method of training that can meet nurses’ educational needs. This study is aimed at validating a web-based diabetes education program through measurement of nurses’ knowledge and clinical competency in diabetes and nurses’ perception about its usability and quality.MethodsThis Quasi-experimental research was conducted on a single group of 31 nurses employed in hospitals affiliated with Shiraz University of Medical Sciences. We used a 125 MCQ knowledge test and Objective Structured Clinical Exam (OSCE) to measure knowledge and clinical competency of nurses in diabetes before and after intervention. A Learning Management System (LMS) was designed to provide educational content in the form of 12 multimedia electronic modules, interactive tests; a forum and learning activities. Nurses were trained for two months in this system after which the post-test was administered. Each nurse completed two questionnaires for measurement of their perceptions on usability and quality. We used descriptive statistics for demographic and descriptive data analysis. Paired t-test was used to compare pre- and post-data using SPSS.ResultsThe findings showed significant differences in knowledge scores (p < 0.001), total score of clinical competencies (p < 0.001), and all ten assessed clinical competencies. The range of ratings given by participants varied on the six usability variables of Web-based training (2.96-4.23 from 5) and eight quality variables of Web-based training (3.58-4.37 from 5).ConclusionWeb-based education increased nurses’ knowledge and competencies in diabetes. They positively evaluated Web-based learning usability and quality. It is hoped that this course will have a positive clinical outcomes.

Exploring sources of knowledge utilized in practice among Jordanian registered nurses

Understanding sources of knowledge used in everyday practice is very helpful in improving the quality of health care services. There is a consensus in the literature that nurses mostly relied in their practice on experiential knowledge gained through their interactions with other members of health care professionals and patients. The general aim of this study is to explore the sources of knowledge Jordanian registered nurses use during their practice. A descriptive correlational design was used to collect data from 539 Jordanian registered nurses from 10 hospitals using a self-administered questionnaire. The mean year of experience of the sample was 7.08 years. Of the 615 questionnaires distributed, 555 were returned. This yields a response rate of 87.6%. Results revealed that the top five ranked sources used by Jordanian registered nurses include: the information that nurses learned during nursing education, personal experience in nursing over time, what was learned through providing care to patients, information gained through discussion between physicians and nurses about patients, and information from policy and procedure manuals. Jordanian registered nurses recognize the value of research and that research utilization (RU) is an important issue and must not be ignored. The study has many implications for practice, education and research. Health care managers and decision makers need to play a more visible and instrumental role in encouraging RU to improve patients' quality of life.
