The pharmacist plays a crucial role in healthcare by helping to reduce prescription and medication errors, as well as issues related to pharmacotherapy that could harm hospitalized patients. The aim of this study was to analyze, based on the literature, the contribution of the clinical pharmacist in the hospital setting, focusing on intervention and reducing the incidence of medication-related prescription errors. By working in collaboration with the multidisciplinary team, the clinical pharmacist provides guidance to professionals on the safe and rational use of medications, which enhances the quality of patient therapy. The impact of the clinical pharmacist is significant, as they are capable of reducing the occurrence of avoidable adverse effects by 66%, which can be controlled not only through the discontinuation of medication combinations but also through dose adjustment. It is important to highlight the central role of the pharmacist in medication dispensing, emphasizing the importance of technical and clinical analysis of prescriptions and medication releases in hospital settings, thereby promoting the rational use of medications.
