Differences in Graft Tear Configurations in Medial and Lateral Meniscal Allograft Transplantation: Objective Evaluations Using Magnetic Resonance Imaging.

Few studies have compared the characteristics of meniscal allograft tears between medial and lateral meniscal allograft transplantation (MAT) using bone fixation. To investigate the prevalence, location, and patterns of allograft tears after MAT with the bone fixation technique and compare tear patterns between medial and lateral compartments according to the time elapsed after surgery. Cohort study; Level of evidence, 3. The charts of consecutive patients who underwent primary medial or lateral MAT between December 1996 and June 2019 were retrospectively reviewed. The location, pattern, and postoperative periods during which allograft tears occurred were evaluated by reviewing all series of follow-up magnetic resonance imaging (MRI) scans, which were performed at 6 weeks, 3 months, 6 months, and 1 year postoperatively and every 2 years thereafter with the patient's agreement. Postoperative periods for allograft tears were defined as the time between surgery and the follow-up MRI scan in which the meniscal tear was first confirmed. Allograft tears were compared between the medial and lateral MAT groups. A total of 327 consecutive patients who underwent MAT (55 medial, 272 lateral) with a minimum 2-year follow-up were retrospectively reviewed. The incidences of allograft tears after medial and lateral MATs were 32.7% and 30.9%, respectively. The mean times for tears were 80.1 ± 81.1 months and 48.9 ± 46.3 months in the medial and lateral MAT groups, respectively (P = .130). In both the medial and lateral MAT groups, allograft tears were observed mainly in the posterior horn, with complex tears being the most commonly identified tear type. In medial MATs, root tears were the second most common at 27.8%, with a significantly higher proportion than the lateral MATs (P = .014). On the other hand, in lateral MATs, meniscocapsular separation and radial tears were the second most common at 15.5% each, albeit not significantly more common than in medial MATs (P = .123 and P = .454, respectively). All root tears in medial MATs and meniscocapsular separations in lateral MATs were observed within 1 year postoperatively. Significant differences in allograft tear patterns were identified between the medial and lateral MAT groups. The proportion of root tears in medial MATs was higher than that in lateral MATs; conversely, the proportion of meniscocapsular separation was more common in lateral MATs. Such tear patterns, which may require surgical repair or graft resection, were observed only within 1 year of surgery. Therefore, close observation and regular follow-up in the earlier postoperative period are necessary after medial or lateral MATs.

Clinical and Radiological Outcomes of Medial Meniscal Allograft Transplantation Combined With Realignment Surgery.

The outcomes of medial meniscal allograft transplantation (MMAT) combined with high tibial osteotomy (HTO) compared with isolated MMAT remain unclear. To compare the clinical and radiological results of MMAT combined with HTO and isolated MMAT. Cohort study; Level of evidence, 3. This retrospective study included 42 consecutive patients, who were divided into group M (isolated MMAT; n = 22) and group H (MMAT combined with HTO with a varus angle >3°; n = 20). Group differences in subjective knee scores, isokinetic muscle strength test, and radiological outcomes (Kellgren-Lawrence grade, mechanical axis, graft extrusion, graft status, and articular cartilage loss) were compared. The mean follow-up period was 29.2 ± 4.9 months and 27.4 ± 5.3 months for groups M and H, respectively. The Lysholm score improved from 55.4 ± 9.5 to 81.3 ± 9.7 and from 52.6 ± 8.9 to 84.2 ± 10.2 in groups M and H, respectively (both P < .001). The International Knee Documentation Committee subjective score improved from 51.4 ± 10.3 to 79.6 ± 9.4 and from 49.3 ± 11.4 to 81.4 ± 8.3 in groups M and H, respectively (both P < .001). Both groups showed no significant differences in subjective knee scores and isokinetic extensor strength at the final follow-up. The rate of preoperative and postoperative high International Cartilage Regeneration & Joint Preservation Society grade (≥3) did not differ between the 2 groups. Group M showed greater coronal graft extrusion than did group H (3.3 ± 0.7 mm vs 2.7 ± 0.8 mm; P = .014); the rate of pathologic graft extrusion (≥3 mm) was not higher in group M (40.9%) than in group H (20%) with the number of patients available (P = .143). Both groups showed no significant difference in the graft status. Graft tears were observed in 2 patients (9%) in group M and 1 patient (5%) in group H (P = .607). Clinical scores significantly improved after isolated MMAT and MMAT combined with HTO compared with preoperative values, and their short-term outcomes were similar. Postoperative graft extrusion was greater in patients who underwent isolated MMAT, implying that active correction of varus alignment during MMAT may help in intra-articular biomechanics.

Clinical Outcomes of Medial Meniscal Allograft Transplantation With or Without High Tibial Osteotomy: A Case-Control Study Up to 8 Years of Follow-up

Background: Satisfactory clinical results of meniscal allograft transplantation (MAT) have been reported in recent years. However, it remains unclear whether the clinical outcomes of MAT when combined with an osteotomy are inferior to those of isolated MAT. Purpose: To compare the survival rates and clinical outcomes of patients who received isolated medial MAT with those of patients undergoing medial MAT combined with high tibial osteotomy (HTO). Study Design: Cohort study; Level of evidence, 3. Methods: A total of 55 patients underwent arthroscopic medial MAT using the soft tissue technique and HTO (mean age, 41.3 ± 10.4 years; 9 female); after fuzzy case-control matching on demographics, 55 controls who underwent isolated medial MAT were also included. Survival analyses were performed using the Kaplan-Meier method with surgical failure, clinical failure (Lysholm score, <65), and reoperation as endpoints. Subjective clinical scores were collected preoperatively and at the final follow-up. Results: The mean follow-up time was 5.4 years, up to 8 years. All outcomes significantly improved at the last follow-up (P < .001). No differences were identified between MAT and MAT + HTO groups preoperatively and at the last follow-up (P > .05). At the final follow-up, 8 of 55 (14.5%) of the MAT + HTO patients and 9 of 55 (16.4%) of the MAT patients had a Lysholm score <65 (P = .885). Overall, 90% of the patients declared they would repeat the surgery regardless of the combined procedure. Surgical failure was identified in 6 of 110 (5.5%) patients: 5 of 55 (9.1%) in the MAT + HTO group and 1 of 55 (1.8%) in the MAT group (P = .093). Clinical failure was identified in 19 of 110 (17.3%) patients: 11 of 55 (20%) in the MAT + HTO group and 8 of 55 (14.5%) in the MAT group (P = .447). A significantly lower survivorship from surgical failure was identified in the MAT + HTO group (hazard ratio, 5.1; P = .049), while no differences in survivorship from reoperation and clinical failure were identified (P > .05). Conclusion: Patients undergoing medial MAT + HTO showed similar clinical results to patients undergoing isolated medial MAT at midterm follow-up, and thus a surgically addressed malalignment does not represent a contraindication for medial MAT. However, the need for a concomitant HTO is associated with a slightly higher failure rate over time.

Isolated meniscus allograft transplantation with soft-tissue technique effectively reduces knee laxity in the presence of previous meniscectomy: In-vivo navigation of 18 consecutive cases

ObjectivesAlthough meniscal allograft transplantation (MAT) is a well-established procedure with satisfactory clinical results, limited in vivo kinematic information exists on the effect of medial and lateral MAT performed in the clinical setting. The purpose of the present study was to evaluate the biomechanical effect of arthroscopic isolated medial and lateral MAT with a soft-tissue fixation on pre- and post-operative knee laxity using a surgical navigation system. Methods18 consecutive patients undergoing MAT (8 medial, 10 lateral) were enrolled. A surgical navigation system was used to quantify the anterior-posterior displacement at 30 and 90 degrees of knee flexion (AP30 and AP90), the varus-valgus rotation at 0 and 30 degrees of knee flexion (VV0 and VV30) and the dynamic laxity on the pivot-shift test (PS), which was determined through the anterior displacement of the lateral tibial compartment (APlat) and posterior acceleration of the lateral tibial compartment during tibial reduction (ACC). Data from laxity before and after MAT were compared through paired t-test (p ​< ​0.05). ResultsAfter medial MAT, there was a significant decrease in tibial translation of 3.1 ​mm (31%; p ​= ​0.001) for AP30 and 2.3 ​mm (27%; p ​= ​0.020) for AP90, a significant difference of 2.5° (50%; p ​= ​0.002) for VV0 and 1.7° (27%; p ​= ​0.012) for VV30. However, medial MAT did not determine any reduction in the PS kinematic data. Lateral MAT determined a significant decrease in the tibial translation of 2.5 ​mm (38%; p ​< ​0.001) for AP30 and 1.9 ​mm (34%; p ​= ​0.004) for AP90 as well as a significant difference of 3.4° (59%; p ​< ​0.001) for VV0 and of 1.7° (23%; p ​= ​0.011) for VV30. There was also a significant reduction of the PS of 4.4 ​mm (22%; p ​= ​0.028) for APlat and 384.8 ​mm/s2 (51%; p ​= ​0.005) for ACC. ConclusionMAT with soft-tissue fixation results in a significant laxity reduction in an in-vivo setting. Medial MAT improved knee kinematics by determining a significant reduction with particular emphasis on AP translation and VV manoeuvre. Conversely, Lateral MAT determined a massive reduction of the PS and a mild decrease of the AP translation and VV manoeuvre. Study designControlled laboratory study.

Tear patterns and locations are different between lateral and medial compartments in patients with early anatomical failure after meniscal allograft transplantation.

To investigate the characteristics of anatomically failed grafts within 1year after meniscal allograft transplantation (MAT) and compare the differences between lateral and medial MATs. The records of consecutive patients with anatomically failed grafts within 1year after primary MAT between 2005 and 2018 were reviewed. Anatomical failure was defined as a tear covering > 50% of the allograft or an unstable peripheral rim. The pattern and location of the graft tears were analyzed using magnetic resonance imaging or arthroscopy. A total of 21 patients were included. All 21 patients had anatomical failure with tears involving > 50% of the allograft, whereas 15 had an unstable peripheral rim of the allograft. The mean failure time was 6.6 ± 3.6months in all patients (lateral MAT, n = 15; medial MAT, n = 6). In the lateral MAT group, meniscocapsular separation was the most common pattern (n = 10, 66.7%), followed by complex (n = 3, 20.0%), radial (n = 1, 6.7%), and longitudinal (n = 1, 6.7%) tear. In the medial MAT group, a root tear was the most common pattern (n = 5, 83.3%), followed by a complex tear (n = 1, 16.7%). Meanwhile, in the lateral MAT, the midbody was the most frequently affected location (n = 9, 60.0%), followed by the posterior (n = 5, 33.3%) and anterior (n = 1, 6.7%) areas; in the medial MAT group, the posterior (n = 5, 83.3%) was the most frequently affected location, followed by the anterior area (n = 1, 16.7%). Significant differences in the pattern (P = 0.002) and location (P = 0.043) of the graft tears were found between lateral and medial MATs. In patients with early failure after MAT, meniscocapsular separation in the midbody of the lateral compartment and root tears in the posterior area of the medial compartment were the most common. Thus, surgeons are encouraged to pay extra attention to these vulnerable areas during the early period after MAT. IV.

Minimum 10-Year Clinical Outcomes and Survivorship of Meniscal Allograft Transplantation With Fresh-Frozen Allografts Using the Bridge-in-Slot Technique.

Meniscal allograft transplantation (MAT) has been shown to provide clinical benefits in patients with symptomatic meniscal deficiency in the short term and midterm. There is, however, a paucity of data regarding long-term outcomes after MAT using fresh-frozen allografts and the bridge-in-slot technique. To report clinical outcomes and revision rates after primary MAT with fresh-frozen allografts and the bridge-in-slot technique in a large case series of patients at a 10-year minimum follow-up. Case series; Level of evidence, 4. A retrospective review of prospectively collected data was performed on patients undergoing primary MAT between 2001 and 2012. Lysholm, International Knee Documentation Committee subjective form, and Knee injury and Osteoarthritis Outcome Score subscales were collected preoperatively and at 1-, 2-, 5-, and minimum 10-year follow-ups. Cox proportional hazards modeling was used to identify variables associated with reoperation and failure, defined as revision MAT or conversion to arthroplasty. Reoperation was defined as a subsequent surgical intervention on the transplanted meniscus, including partial or total meniscectomy, meniscal repair, or failure as defined in the previous sentence. A total of 174 patients undergoing MAT met the inclusion criteria and were followed for a mean of 12.7 ± 2.7 years (range, 10.0-21.0 years). The mean age at surgery was 28.3 ± 10.1 years. The patients were predominantly female (n = 92; 53%), and medial MAT was the most commonly performed procedure (n = 91; 52%). Concomitant procedures were performed in 115 patients (66%), with the most common procedure being osteochondral allograft transplantation (n = 59; 34%). Patients demonstrated statistically significant postoperative improvements at all time points for all patient-reported outcome measures (P≤ .0001). A total of 65 patients (37%) underwent a meniscal reoperation at a mean time of 6.6 ± 5.5 years (range, 0.3-16.7 years) postoperatively. A total of 40 patients (23%) met the criteria for failure at a mean time of 7.3 ± 5.0 years (range, 1.0-17.4 years) after MAT, with 22 of these patients having undergone a previous meniscal reoperation. At the final follow-up, 13 patients (7%) had undergone revision MAT and 27 (15%) had converted to arthroplasty. The MAT survival rates free of meniscal reoperation and failure were 73% and 85% at 10 years and 60% and 72% at 15 years, respectively. At the time of the final follow-up, 86% of patients reported that they were satisfied with their overall postoperative condition. Primary MAT demonstrates efficacy and durability with high rates of patient satisfaction at a minimum 10-year follow-up. Patients should be counseled that although reoperation rates may approach 40% at 15 years, rates of overall revision MAT and conversion to arthroplasty remain low at long-term follow-up.

Biomechanical Analysis Evaluating Meniscal Extrusion After Knotless Suture Anchor Fixation for Segmental Medial Meniscal Allograft Transplantation.

Segmental medial meniscal allograft transplantation (MAT) has been shown to restore knee biomechanics; however, stable fixation of the transplantation is critical to avoid extrusion and maximize healing. To evaluate the degree of meniscal extrusion and biomechanical function of segmental medial MAT performed with meniscocapsular sutures versus repair augmentation with knotless suture anchors. Controlled laboratory study. Segmental midbody medial meniscectomy and subsequent segmental medial MAT were performed on 10 fresh-frozen cadaveric knees. The knees were then loaded in a dynamic tensile testing machine to 1000 N for 60 seconds at 0°, 30°, 60°, and 90° of flexion, and 4 conditions were tested: (1) intact, (2) segmental defect, (3) inside-out segmental repair, and (4) anchor plus inside-out segmental repair of the medial MAT. Meniscal extrusion was measured using high-fidelity ultrasound imaging. The mean contact area and the mean and peak contact pressures were assessed with submeniscal pressure-mapping sensors. Data from testing conditions were compared with 2-way repeated-measures analysis of variance, with pairwise comparison using the Bonferroni method. At 90° of flexion, the segmental defect state showed a higher degree of meniscal extrusion compared with all other states (P ≤ .012). There was no difference in the degree of meniscal extrusion between the intact state and the inside-out repair or anchor plus inside-out segmental repair states at all knee flexion angles (P > .05). There was no significant difference in the mean and peak contact pressures among the 4 states at all flexion angles except that at 0° of knee flexion there was significantly lower peak contact pressure at the medial compartment after anchor plus inside-out segmental repair compared with the segmental defect state (P = .048). Meniscal extrusion was not significantly increased at any flexion angle after segmental resection. The addition of knotless anchors did not improve meniscal extrusion or contact pressures/area compared with capsular repair alone. The addition of knotless anchors did improve contact mechanics from the segmental defect state, but only at 0° of flexion. The addition of knotless suture anchors to segmental meniscal transplantation increased stabilization of the meniscus at full extension compared with repair with sutures alone. This increased stabilization may lead to better long-term outcomes.

Revision and Conversion to Arthroplasty Are Low Among Adolescents Undergoing Meniscal Allograft Transplantation Using the Bridge-In-Slot Technique at Midterm Follow-Up

To report midterm outcomes after primary medial and lateral meniscal allograft transplantation (MAT) withfresh-frozen allografts implanted with the bridge-in-slot technique in the adolescent patient population. Adolescent patients less than 18 years old at the time of primary MAT from 1999 to 2016 were retrospectively identified. International Knee Documentation Committee (IKDC) subjective form, Lysholm, and Knee Injury and Osteoarthritis Outcome Score (KOOS) subscales scores were collected before surgery and at 1-year, 2-year, and a minimum 5-year follow-up. Thresholds for achieving clinically significant outcomes were calculated, and the proportion of patients achieving minimal clinically important difference (MCID), patient-acceptable symptomatic state (PASS), and substantial clinical benefit (SCB) was determined. Meniscus reoperation (partial, subtotal, or total meniscectomy, repair, or failure) and failure (revision MAT or conversion to arthroplasty) rates were determined. Forty-four (female n= 33; male n= 11) of 62 identified patients met inclusion criteria and were followed for a mean of 9.5 ± 3.8 years (range, 5.0-17.7). Lateral MAT was performed in most patients (n= 35/44 [80%]). Isolated MAT was performed in 27 (61%) patients. Common concomitant procedures included osteochondral allograft transplantation (32%), autologous chondrocyte implantation (18%), and anterior cruciate ligament reconstruction (14%). MCID, PASS, and SCB were achieved by patients at a minimum 5-year follow-up for IKDC (62%; 76%; 31%), Lysholm (62%; 79%; 23%), and KOOS questionnaires (Pain [65%; 81%; 41%], Symptoms [58%; 81%; 47%], Activities of Daily Living [53%; 77%; 35%], Sport [86%; 75%; 50%], and Quality of Life [59%; 81%; 59%]), respectively. Fourteen patients (32%) underwent reoperation at an average of 5.0±4.3 years (range, 0.8-14.0) after MAT. Three (7%) patients met criteria for failure, requiring revision MAT an average of 3.8 ± 1.1 years (range, 2.8-4.9) after transplantation. No patients underwent arthroplasty. Overall survival free from failure at 1, 2, 5, and 10 years was 100%, 100%, 93%, and 93%, respectively. At the time of final follow-up, 80% of patients reported satisfaction with their current physical status. Primary MAT in adolescent patients resulted in significant and durable functional improvements at mid- to long-term follow-up. At an average of 9.5 years after surgery, meniscal reoperation rate was 32% whereas graft survival free of revision MAT was 93%. Adolescents undergoing MAT demonstrated similar functional outcomes and graft survivability when compared to available adult MAT literature. Level IV, retrospective case series.

Poster 286: Biomechanical Analysis Evaluating Meniscal Extrusion Following Knotless Suture Anchor Fixation for Segmental Medial Meniscus Allograft Transplantation

Objectives: Segmental medial meniscus allograft transplantation (MAT) has been shown to restore knee biomechanics, however, stable fixation of the transplant is critical to avoid extrusion and maximize healing. The purpose of this study was to evaluate the degree of meniscal extrusion and biomechanical function of segmental MAT performed with meniscocapsular sutures versus repair augmentation with knotless suture anchors. Methods: Ten fresh frozen cadaveric knees underwent central midbody medial meniscectomy (15-mm defect) and subsequently segmental medial MAT. Knees were loaded in a dynamic tensile testing machine to 1000 N for 20 seconds at 0°, 30°, 60°, and 90° of flexion. Four conditions were tested: (1) intact, (2) segmental defect, (3) inside-out segmental repair, and (4) anchors plus inside-out segmental repair of the MAT. Meniscal extrusion was measured for each test state using high-fidelity ultrasound imaging by three independent observers. Submeniscal medial and lateral pressure-mapping sensors assessed the mean contact pressure, peak contact pressure, mean contact area, and pressure mapping. Assessment of inter- and intraobserver reliability of measurements were done using intraclass correlation coefficients. A two-way repeated measures ANOVA was computed in order to evaluate whether there is a significant interaction between meniscal states and knee flexion angles on the outcome variables. If the P value was ≤ 0.05, a pairwise comparison of the means was determined using the Bonferroni method. Results: At 0° of knee flexion, the segmental defect demonstrated the statistically significant higher degree of meniscal extrusion (2.08 mm ± 0.78 mm) compared with intact (1.02 mm ± 0.86 mm, P = 0.039) and anchor plus inside-out segmental repair (1.44 mm ± 0.76 mm, P = 0.044), but not for an inside-out segmental repair (1.55 mm ± 0.49 mm, P = 0.08). The segmental defect state also showed a higher degree of meniscal extrusion compared with all other states at 90° of knee flexion (P &lt; 0.01). There was no difference in the degree of meniscal extrusion between the intact state and the inside-out repair or anchor plus inside-out segmental repair in all knee flexion angles (P &gt; 0.05). Three observers showed good-to-strong intra-rater and moderate-to-strong inter-rater reliabilities for extrusion measurements. There was no significant difference in the mean contact pressure and peak contact pressure among four states in all knee flexion angles for both medial and lateral compartments except for that at 0° of knee flexion, there was a significantly lower peak pressure at the medial compartment after anchor plus inside-out segmental repair compared with the segmental defect state (P = 0.048). The medial contact area of the intact state was significantly higher than the other states at 0° of knee flexion (P &lt; 0.05). However, the lateral contact area after anchor plus inside-out repair showed no significant difference compared with the intact state (P = 0.13). Conclusions: Segmental meniscus transplantation mechanics may be improved at full extension (0° of knee flexion) with the addition of knotless anchors. However, in terms of meniscus extrusion, the knotless anchors provided limited benefit over inside-out repair. Overall, a robust repair is achieved of a segmental meniscus with sutures and knotless anchors. Although, additional work is needed to assess long-term in vivo results.

Poster 341: Clinical Outcomes of Concomitant Arthroscopically Assisted Anterior Cruciate Ligament Reconstruction and Meniscal Allograft Transplantation

Objectives: Single-stage meniscus allograft transplantation (MAT) with concomitant anterior cruciate ligament reconstruction (ACLR) is a technically challenging procedure for management of knee pain and instability in younger patients. However, there is limited data on specific outcomes of this procedure, such as return to work and functional status relative to the U.S. population at large. The purpose of this study was to assess surgical and patient-reported outcomes following concomitant ACLR and MAT. Methods: We conducted a retrospective review of patients who underwent concomitant ACLR and MAT at our institution from 2010 to 2021. We included patients who underwent either concomitant primary or revision ACLR and medial or lateral MAT with a minimum follow-up of 8 months. Complications, graft failures, and reoperations were abstracted from patient records and Visual Analog Scale (VAS) pain, satisfaction, Lysholm Knee Scale, return to sport, and return to work outcomes were assessed using an email survey. Patient-Reported Outcomes Measurement Information System (PROMIS) Pain Interference, Pain Intensity, and Physical Function Scores were used to measure patients’ functional status relative to the U.S. population. All p-values less than 0.05 were considered significant. Results: The cohort consisted of 18 knees of 17 individual patients (16 unilateral, 1 bilateral). The cohort were majority male (15 patients; 83%) with mean age of 31.6 years (range 19-49) and mean BMI of 27.6 (range 22.2-53.3). Mean follow-up time was 47.3 months (range 8-101). A majority underwent revision ACLR (12 patients; 67%) with medial MAT (18 patients; 92%). Reoperation rate was high (5 patients; 28%) but only one patient experienced recurrence of painrequiring TKA. Patient-reported outcomes (n = 14) were generally positive with low VAS pain (mean 2.2), high satisfaction (mean 77.9%), and fair Lysholm score (mean 81.1) though satisfaction was lower among revision ACLR patients versus primary ACLR patients (mean 67% vs 100%; p = 0.04). Return to work rate was high (13 patients; 93%) while return to sport rate was low (6 patients; 43%). Postoperative PROMIS scores were comparable or superior to the national average and correlated significantly with patient satisfaction (p &lt; 0.05). Conclusions: The concomitant ACLR and MAT procedure is associated with excellent knee pain and functional outcomes, low rates of complications and meniscus graft failure, and high rate of return to work after surgery. However, rate of return to sport is low and satisfaction is worse among revision ACLR patients. Overall, the single-stage ACLR and MAT procedure appears to be a safe and efficacious treatment option for patients with concomitant ACL and meniscus deficiency.

Acceptable clinical outcomes despite high reoperation rate at minimum 12-month follow-up after concomitant arthroscopically assisted anterior cruciate ligament reconstruction and medial meniscal allograft transplantation

The purpose of this study was to assess surgical and patient-reported outcomes following concomitant ACLR and medial MAT.MethodsWe conducted a retrospective case series of patients who underwent medial MAT with concomitant primary or revision ACLR at our institution from 2010 to 2021 and had minimum 12-month follow-up. Complications, reoperations, visual analog scale (VAS) pain, satisfaction, Lysholm score, return to sport, and return to work outcomes were assessed. Patient-Reported Outcomes Measurement Information System (PROMIS) Pain Interference, Pain Intensity, and Physical Function Scores were used to measure patients’ functional status relative to the US population. P-values < 0.05 were considered significant.ResultsThe cohort consisted of 17 knees of 16 individual patients. The cohort was majority male (82.4%) with mean age of 31.9 years (range 19–49 years) and mean body mass index (BMI) of 27.9 kg/m2 (range 22.5–53.3 kg/m2). Mean follow-up time was 56.8 months (range 13–106 months). Most patients underwent revision ACLR (64.7%). The 1-year reoperation rate was high (23.5%), with two patients (11.8%) tearing their meniscus graft. Patient-reported outcomes indicated low VAS pain (mean 2.2), high satisfaction (mean 77.9%), and fair Lysholm score (mean 81.1). Return to work rate was high (92.9%), while return to sport rate was low (42.9%). Postoperative PROMIS scores were comparable or superior to the national average and correlated significantly with patient satisfaction (p < 0.05).ConclusionsThe concomitant ACLR and MAT procedure is associated with excellent knee pain and functional outcomes and high rate of return to work after surgery, though the 1-year reoperation rate is high and rate of return to sport is low.Level of evidence: IV.

Effect of ICRS Lesion Grade on Graft Survival After Medial Meniscal Allograft Transplantation: MRI-Based Objective Evaluation

Background: Data are lacking regarding the survival rate after medial meniscal allograft transplantation (MAT) alone. Furthermore, little information is available about prognostic factors for graft survival that affect the outcomes of medial MAT. Purpose: To investigate the prognostic factors and survival rate of allograft after medial MAT. Study Design: Case-control study; Level of evidence, 3. Methods: The records of 78 consecutive patients who underwent primary medial MAT between 1996 and 2018 were reviewed. Kaplan-Meier survival analysis was performed to analyze the anatomic and clinical survival rates. Anatomic failure was defined as a tear covering >50% of the allograft or unstable peripheral rim. Clinical failure was considered Lysholm score <65 or need for additional surgery such as meniscal repair, revision MAT, realignment osteotomy, and meniscectomy for >50% of the allograft. Patient factors affecting anatomic and clinical failure were analyzed. Results: The mean follow-up period was 6.9 ± 5.3 years (range, 2-21 years). Anatomic failure was noted in 19 patients (24.4%), and none of these patients had a persistent poor Lysholm score of <65; of these, 2 patients who underwent meniscal repair also had clinical failure. Clinical failure was noted in 7 patients (9.0%); 4 patients had Lysholm score <65, 2 patients underwent meniscal repair, and 1 patient underwent realignment osteotomy. The estimated 10-year anatomic and clinical survival rates were 73.89% and 87.90%, respectively. Anatomic survival was significantly associated with only high-grade International Cartilage Regeneration & Joint Preservation (ICRS) lesion (ICRS grade 3 or 4) (hazard ratio, 3.171; 95% CI, 1.124-8.944; P = .029). However, the clinical survival rate was not significantly associated with any factors. Patients with low-grade ICRS lesion (ICRS grade 0, 1, or 2) showed a higher estimated 10-year anatomic survival rate compared with patients with high-grade ICRS lesions (87.6% vs 63.3%, respectively; P = .022). Conclusion: Low-grade ICRS lesion was associated with higher anatomic survival rate after medial MAT. In patients with high-grade ICRS lesions, the clinical outcome might be good; however, the status of an allograft might be poor. The surgeon should be aware of this and explain to the patient that close observation is necessary.

Assessment of Anatomic Restoration and Clinical Outcomes Between Medial and Lateral Meniscal Allograft Transplantation.

Background:Proper anatomic restoration is an important consideration for meniscal allograft transplantation (MAT), even with the different anatomica characteristics between the medial meniscus and lateral meniscus.Purpose/Hypothesis:The purpose of this study was to assess the accuracy of anatomic restoration in medial and lateral MAT (MMAT and LMAT) procedures and to compare their outcomes. We hypothesized that (1) the anatomic differences between the medial and lateral menisci will mean a less accurate anatomic restoration for MMAT and (2) clinical outcomes after MMAT will be inferior compared with LMAT.Study Design:Cohort study; Level of evidence, 3.Methods:We retrospectively evaluated 20 patients who underwent MMAT using the bone plug technique and 21 patients who underwent LMAT using the keyhole technique at a single institution from July 2014 to June 2019. Demographic data, previous surgeries, and concomitant procedures were recorded, as were lower limb alignment and osteoarthritis grade on radiographs. Using preoperative and follow-up magnetic resonance imaging, the meniscal position, rotation, extrusion, and intrameniscal signal intensity were evaluated. Clinical outcomes were evaluated using the International Knee Documentation Committee and Lysholm scores.Results:The mean follow-up was 41.15 ± 18.86 and 45.43 ± 21.32 months for the MMAT and LMAT patients, respectively. Concomitant procedures were performed in 90% of MMATs and 15% of LMATs. There was no significant difference between the native and postoperative root positions after LMAT; however, for MMAT, the position of the anterior root was located significantly posteriorly (P = .002) and medially (P = .007) compared with preoperatively. In addition, the allograft medial meniscus was restored in a more internally rotated position (P = .029). MMATs also exhibited significantly increased meniscal extrusion compared with LMATs (posterior horn, P < .001; midbody, P = .027; anterior horn, P = .006). However, there was no significant difference between the 2 groups at final follow-up in intrameniscal signal intensity or clinical scores.Conclusion:LMAT showed higher accuracy than MMAT in restoring meniscal position and rotation, and there was less meniscal extrusion. However, clinical scores improved after both LMAT and MMAT compared with preoperative values, and midterm clinical outcomes were similar. The small anatomical errors seen in the MMAT technique were not clinically relevant at midterm follow-up.

Paper 33: Serum and Urine Biomarkers for Treatment Monitoring after Meniscal Allograft Transplantation in a Preclinical Canine Model

Objectives:To analyze serum and urine biomarker concentrations for their capabilities in delineating differences in assessments of pain and functional outcomes after meniscus allograft transplantation (MAT) in a preclinical canine model. The study was designed to test the hypothesis that serum and urine biomarkers could be used for prognostic (1- and 3-month post-surgical time points) and diagnostic (6-month time point) assessments, based on strong associations with clinically relevant pain and function outcomes after MAT.Methods:Twelve adult, purpose-bred research hounds were included and underwent medial meniscal release (MR) to induce medial compartment gonarthrosis. Three months following MR, medial MAT was performed using: fresh-frozen meniscus (n=4), fresh meniscus (n=4), or fresh menisco-tibial OCA (n=4). Serum and urine from all dogs were collected preoperatively and at 1, 3, and 6 months following MAT. Validated outcome measures for pain, function, effusion, knee range of motion, and gait kinetics were collected at these same timepoints. To develop a prognostic panel of biomarkers, biomarker data from the 1-month and 3-month post-MAT surgery timepoints were used to model 6-month outcomes. A diagnostic panel of biomarkers was developed using biomarker data from the 6-month post-MAT surgery to model 6-month outcomes.Results:Across prognostic biomarker panels, serum biomarkers were mainly represented. A panel including serum IL-6, IL-8, IL-10, and IL-18 had strong model fit for prognostication of the gait kinetics outcome, operated limb %Total Pressure Index (%TPI) (R2=.45). Whereas, a biomarker panel including serum CTX-II and OPG had strong model fit for prognostication of the primary pain-related outcome, VAS pain (R2=.52). Across diagnostic biomarker panels, a panel including serum MMP-1 and MMP-3 and urine PINP and TIMP-1 had strong model fit as diagnostic biomarkers for %TPI (R2=.86). Whereas, a panel including urine CTX-I, CTX-II, IL-8, MMP-2, and TIMP-1 had strong model fit as diagnostic biomarkers for VAS pain (R2=.44).Conclusions:Biomarker panels of selected serum and/or urine proteins highly correlate with clinically-relevant metrics for pain and function outcomes in a preclinical model of meniscus allograft transplantation.

Concomitant Meniscotibial Ligament Reconstruction Decreases Meniscal Extrusion Following Medial Meniscus Allograft Transplantation: A Cadaveric Analysis

To compare meniscal extrusion (ME) following medial meniscus allograft transplantation (MMAT) with and without meniscotibial ligament reconstruction (MTLR). Ten cadaveric knees were size-matched with meniscus allografts. MMAT was performed via bridge-in-slot technique. Specimens were mounted in a testing system and ME was assessed via ultrasound anterior, directly over, and posterior to the medial collateral ligament at the joint line under 4 testing conditions: (1) 0° flexion and 0 newtons (N) of axial load, (2) 0° and 1,000 N, (3) 30° and 0 N, and (4) 30° and 1,000 N. For each condition, "mean total extrusion" was calculated by averaging measurements at each position. Next, MTLR was performed using 2 inside-out sutures through the remnant allograft meniscotibial ligament and secured to the tibia using anchors. The testing protocol was repeated. Differences in ME between MMAT alone versus MMAT+ MTLR were examined. Within-group differences between the measurement positions, loading states, and flexion angles also were assessed. "Mean total extrusion" was greater following MMAT alone (2.56 ± 1.23 mm) versus MMAT+ MTLR (2.14 ± 1.07 mm; P= .005) in the loaded state at 0° flexion. ME directly over the MCL was greater following MMAT alone (3.51 ± 1.00 mm) compared with MMAT+ MTLR (2.93 ± 0.79 mm; P= .054). Posteriorly, in the loaded state at 0°, ME was greater following MMAT alone (2.43 ± 1.10 mm) compared with MMAT+ MTLR (1.96 ± 0.99 mm; P= .010). In all conditions, ME was greater in the loaded state versus the unloaded state. Following MMAT, the addition of MTLR significantly reduced overall ME when compared with isolated MMAT during loading at 0° of flexion in a cadaveric model; given the small absolute values of change in extrusion, clinical significance cannot be gleaned from these findings. During medial meniscus allograft transplantation, augmentation with meniscotibial ligament reconstruction may limit meniscal extrusion and improve the biomechanical milieu of the knee joint following transplant.

Biomechanical Analysis of Segmental Medial Meniscal Transplantation in a Human Cadaveric Model

Background: Meniscal deficiency has been reported to increase contact pressures in the affected tibiofemoral joint, possibly leading to degenerative changes. Current surgical options include meniscal allograft transplantation and insertion of segmental meniscal scaffolds. Little is known about segmental meniscal allograft transplantation. Purpose: To evaluate the effectiveness of segmental medial meniscal allograft transplantation in the setting of partial medial meniscectomy in restoring native knee loading characteristics. Study Design: Controlled laboratory study. Methods: Ten fresh-frozen human cadaveric knees underwent central midbody medial meniscectomy and subsequent segmental medial meniscal allograft transplantation. Knees were loaded in a dynamic tensile testing machine to 1000 N for 20 seconds at 0°, 30°, 60°, and 90° of flexion. Four conditions were tested: (1) intact medial meniscus, (2) deficient medial meniscus, (3) segmental medial meniscal transplant fixed with 7 meniscocapsular sutures, and (4) segmental medial meniscal transplant fixed with 7 meniscocapsular sutures and 1 suture fixed through 2 bone tunnels. Submeniscal medial and lateral pressure-mapping sensors assessed mean contact pressure, peak contact pressure, mean contact area, and pressure mapping. Two-factor random-intercepts linear mixed effects models compared pressure and contact area measurements among experimental conditions. Results: The meniscal-deficient state demonstrated a significantly higher mean contact pressure than all other testing conditions (mean difference, ≥0.35 MPa; P < .001 for all comparisons) and a significantly smaller total contact area as compared with all other testing conditions (mean difference, ≤140 mm2; P < .001 for all comparisons). There were no significant differences in mean contact pressure or total contact area among the intact, transplant, or transplant-with-tunnel groups or in any outcome measure across all comparisons in the lateral compartment. No significant differences existed in center of pressure and relative pressure distribution across testing conditions. Conclusion: Segmental medial meniscal allograft transplantation restored the medial compartment mean contact pressure and mean contact area to values measured in the intact medial compartment. Clinical Relevance: Segmental medial meniscal transplantation may provide an alternative to full meniscal transplantation by addressing only the deficient portion of the meniscus with transplanted tissue. Additional work is required to validate long-term fixation strength and biologic integration.

Meniscal allograft transplantation shows a mismatch between anatomic and clinical failures.

Clinical results of meniscal allograft transplantation (MAT) are not always consistent with graft status. This study aimed to investigate (1) the degree and pattern of mismatch between anatomic and clinical failures in MAT and (2) preoperative factors associated with the mismatch. Two hundred and ninety-eight consecutive patients who underwent primary medial or lateral MAT during 2004-2015 were reviewed. Anatomic failure was defined as an allograft showing meniscal tear involving > 50% of the graft or unstable peripheral rim. Clinical failure included poor Lysholm score of < 65 and any requirement for re-operations such as arthroplasty, realignment osteotomy, revision MAT, and meniscectomy (more than 50% of the graft or to the zone of meniscocapsular junction). Failure cases were categorised according to the type of failure as follows: (1) type 1, anatomic failure followed by clinical failure; (2) type 2, anatomic failure did not lead to clinical failure; and (3) type 3, clinical failure without anatomic failure. Preoperative factors including age, sex, body mass index, MAT compartment, time from previous meniscectomy, alignment, cartilage status, and accompanying procedures were analysed according to the failure type. Forty (13.4%) patients showed anatomical or clinical failure during the median (25th-75th percentile) follow-up duration of 47 (30-72) months (range 24-178months). Eleven (3.7%) patients showed both anatomical and clinical failure (type 1 failure). Seventeen (5.7%) patients showed anatomic failure that did not lead to clinical failure (type 2 failure). Twelve (4.0%) patients failed clinically without meniscal tear (> 50% of graft) or unstable peripheral rim (type 3 failure). Comparative analyses among failure types found a significant difference in MAT compartment (p = 0.01). In particular, the incidence of type 3 failure was higher in medial than in lateral MAT (p = 0.003). A notable number of failure cases of MAT showed a mismatch between anatomic and clinical failures. Even with anatomic failure, MAT did not always lead to poor clinical scores or re-operations, whereas MAT could have poor results without substantial allograft problems. Therefore, both anatomic and clinical aspects should be considered when evaluating MAT. In particular, type 3 failure occurred more frequently in medial than in lateral MAT. III.

Use of contralateral lateral meniscus for medial meniscal allograft transplantation: a cadaveric study.

Meniscal allografts are a preferred alternative to menisectomy in cases of irrepairable meniscal tears in young patients. Biological meniscal allograft transplantation requires a cadaveric donor, limiting its availability for transplantation. We are exploring the possibility of using contralateral lateral meniscus for medial meniscal allograft transplantation, as it can be easily procured from proximal tibial cuts from total knee replacement. Ten paired knees from five formalin-fixed Indian male cadavers were dissected. Outer and inner circumferences of the medial and meniscus, area of the articular surface of the medial tibial plateau covered by the native medial meniscus and transplanted lateral meniscus were noted. Measurements were taken using software ImageJ (National Institute of Health). The mean of the recordings from two independent observers was taken as the final value. Inter-observer and intra-observer reliability were also calculated. The mean inner circumference of the medial meniscus was significantly larger than the lateral meniscus (p < 0.0001). However, outer circumferences were not significantly different from each other (p = 0.1). Area of the tibial plateau covered by the native medial meniscus was smaller than the area covered by the transplanted lateral meniscus, though the difference was not statistically significant. Inter-observer reliability and intra-observer reliability were good (ICC 0.904 and 0.927, respectively). Based on measurements of the outer circumference of medial and lateral menisci, lateral meniscal allograft can be matched for transplantation on the contralateral medial side from the donor with same dimensions of the tibial plateau. Further clinical studies are necessary to prove the clinical significance of this cadaveric study. Diagnostic study.

Medial Meniscal Allograft Transplantation

Background: This technique video reviews medial meniscal allograft transplantation (MAT) using a representative case example. Indications: Medial meniscal allograft transplantation is indicated in symptomatic patients with a deficient medial tibiofemoral compartment that has not progressed to arthritic changes. Concomitant procedures to address focal cartilage defects, ligamentous laxity, and/or limb malalignment should be performed prior to MAT or in the same surgical setting. Technique Description: Preoperative workup includes magnetic resonance imaging (MRI), prior arthroscopic pictures, and sizing radiographs. The Pollard radiographic method measures for the appropriate cryopreserved allograft size. Arthroscopic instruments remove residual meniscal tissue to a 1 mm base around the capsule. Percutaneous lengthening of the medial collateral ligament (MCL) at its femoral attachment aids visualization/instrumentation. This technique employs 8-mm bone plugs for anterior and posterior meniscal root fixation. Tunnels size 8.5 mm diameter and 10 mm depth are created. Once the meniscal allograft is placed in the joint, inside-out sutures are placed throughout the meniscal body. Sutures from the meniscal roots are secured with an anchor in the anterior proximal tibia. Results: There are numerous outcomes studies of meniscal allograft transplantation with a reported overall graft survivorship of roughly 70% at 10 years and 60% at 15 years follow-up. Discussion/Conclusion: Meniscal allograft transplantation is a temporizing measure that provides good midterm clinical results, although long-term failure rates increase incrementally. Most studies suggest return to sport is possible although activity modification is recommended.

Meniscal Allograft Transplantation After Anterior Cruciate Ligament Reconstruction Can Improve Knee Stability: A Comparison of Medial and Lateral Procedures

Background: The purpose of this study was to evaluate the clinical score and stability after meniscal allograft transplantation (MAT) after a previous anterior cruciate ligament (ACL) reconstruction. Hypothesis: Medial MAT would improve anteroposterior stability, and lateral MAT would improve rotational stability. Study Design: Cohort study; Level of evidence, 3. Method: We retrospectively investigated 31 cases of MAT after a previous total or nearly total meniscectomy and ACL reconstruction between November 2008 and June 2017. Cases were divided into medial (16 cases) and lateral (15 cases) MAT groups. The patients were assessed preoperatively and at the 2-year follow-up. Results: In the medial MAT group, the International Knee Documentation Committee, Lysholm, Lysholm instability, and Tegner scores improved significantly at the 2-year follow-up, and there were also significant improvements in the anterior drawer, Lachman, and pivot-shift tests. In the lateral MAT group, the Lysholm and Tegner scores improved significantly at the 2-year follow-up, as had the anterior drawer and Lachman tests but not the pivot-shift test. The medial MAT group showed significant improvement in side-to-side difference on Telos stress radiographs, from 6.5 mm (preoperatively) to 3.6 mm (2-year follow-up) (P = .001), while the lateral MAT group showed no significant change. There was no progression of arthritis in either group. Conclusion: Medial MAT improved not only anteroposterior stability but also rotational stability in the meniscus-deficient ACL-reconstructed knee. Lateral MAT showed improvements in the anterior drawer and Lachman tests but not in the pivot-shift test or side-to-side difference on Telos stress radiographs in meniscus-deficient ACL-reconstructed knees. Instability and pain are indications for MAT in meniscus-deficient ACL-reconstructed knees.
