Role of muscle ultrasound in frailty assessment in older adults with type 2 diabetes mellitus

BackgroundFrailty is a geriatric syndrome that is characterized by increased vulnerability to intrinsic and extrinsic stressors due to decreased biologic reserves. Muscle ultrasound (US) is a valid and reliable method for assessing muscle quantity in older adults. The study aims to examine the relationship between frailty definitions and US-derived muscle parameters.MethodsWe conducted a cross-sectional study with type 2 diabetes mellitus outpatients in a tertiary hospital, and all participants underwent a comprehensive geriatric assessment. For frailty assessment, the Fried Frailty Phenotype (FFP), the Clinical Frailty Scale (CFS), and the Edmonton Frailty Scale (EFS) were performed. Muscle US measurements included Gastrocnemius Medialis (GM) muscle thickness, GM fascicle length, GM pennation angle, Rectus Femoris (RF) muscle thickness, Rectus Femoris cross-sectional area (RFCSA), Rectus Abdominis (RA) muscle thickness, External Oblique (EO) muscle thickness, Internal Oblique (IO) muscle thickness, and Transverse Abdominis (TA) muscle thickness.ResultsIn all, 373 participants were included in the study. The median age of participants was 72.7 ± 5.9 years, and 64.6% of them were female. According to the FFP, 18.2% of the participants were living with frailty, 56% of them were pre-frail; 57.4% of them were living with frailty according to the CFS; 25.2% of them were living with frailty, and 20.6% of them were pre-frail according to the EFS. The FFP, CFS, and EFS scores were related to muscle thickness of GM, RF, and RA, fascicle length of GM, and pennation angle of GM and RFCSA. Particularly, GM pennation angle, RF muscle thickness, and RFCSA were associated with an increased risk of frailty. Besides muscle thickness of GM, RF, and RA, fascicle length of GM, pennation angle of GM, and RFCSA were significant for predicting the presence of frailty.ConclusionsUS-derived regional muscle measurements are associated with frailty definitions (in both physical, cumulative deficit, and multidimensional models) in a diabetic geriatric population.

Effects of gait retraining using minimalist shoes on the medial gastrocnemius muscle-tendon unit behavior and dynamics during running.

The effects of a 12-week gait retraining program on the adaptation of the medial gastrocnemius (MG) and muscle-tendon unit (MTU) were investigated. 26 runners with a rearfoot strike pattern (RFS) were randomly assigned to one of two groups: gait retraining (GR) or control group (CON). MG ultrasound images, marker positions, and ground reaction forces (GRF) were collected twice during 9 km/h of treadmill running before and after the intervention. Ankle kinetics and the MG and MTU behavior and dynamics were quantified. Runners in the GR performed gradual 12-week gait retraining transitioning to a forefoot strike pattern. After 12-week, (1) ten participants in each group completed the training; eight participants in GR transitioned to non-RFS with reduced foot strike angles; (2) MG fascicle contraction length and velocity significantly decreased after the intervention for both groups, whereas MG forces increased after intervention for both groups; (3) significant increases in MTU stretching length for GR and peak MTU recoiling velocity for both groups were observed after the intervention, respectively; (4) no significant difference was found for all parameters of the series elastic element. Gait retraining might potentially influence the MG to operate at lower fascicle contraction lengths and velocities and produce greater peak forces. The gait retraining had no effect on SEE behavior and dynamics but did impact MTU, suggesting that the training was insufficient to induce mechanical loading changes on SEE behavior and dynamics.

Cross-Education Effects of Isokinetic Eccentric Plantarflexor Training on Flexibility, Strength, and Muscle-Tendon Mechanics.

Large increases in joint range of motion (ROM) have been reported after eccentric resistance training, however limited data exist describing the associated mechanisms or potential cross-education effects in the contralateral limb. Therefore, the effects of a 6-week isokinetic eccentric plantarflexor training program were examined in 26 participants. Before and after the training program, dorsiflexion ROM, plantarflexor strength, and muscle-tendon unit (MTU) morphology and mechanics were measured in control (n = 13) and experimental (n = 13) young adult groups. Training consisted of 5 sets of 12 maximal isokinetic eccentric plantarflexor contractions twice weekly on the right limb. Significant (P < 0.05) increases in dorsiflexion ROM (4.0-9.5°), stretch tolerance (40.3-95.9%), passive elastic energy storage (47.5-161.3%), and isometric (38.1-40.6%) and eccentric (46.7-67.0%) peak plantarflexor torques were detected in both trained and contralateral limbs in the experimental group. Significant increases in gastrocnemius medialis (GM) and soleus thickness (5.4-6.1%), GM fascicle length (7.6 ± 8.5%), passive plantarflexor MTU stiffness (30.1 ± 35.5%) and Achilles tendon stiffness (5.3 ± 4.9%) were observed in the trained limb only. Significant correlations were detected between the changes in trained and contralateral limbs for dorsiflexion ROM (r = 0.59) and both isometric (r = 0.79) and eccentric (r = 0.73) peak torques. No significant changes in any metric were detected in the control group. Large ROM increases in the trained limb were associated with neurological, mechanical, and structural adaptations, with evidence of a cross-education effect in the contralateral limb being primarily driven by neurological adaptation (stretch tolerance). The large improvements in ROM, muscle size, and strength confirm that isokinetic eccentric training is a highly effective training tool, with potential for use in athletic and clinical populations where MTU function is impaired and current therapies are ineffective.

Habitually wearing high heels may improve user walking economy in any footwear.

The habitual use of high-heeled footwear may structurally remodel user leg muscle tendons, thereby altering their functional capabilities. High heels set users' ankles in relatively plantarflexed positions, causing calf muscle tendons to operate at relatively short lengths. Habitually operating muscle tendons at relatively short lengths induces structural remodeling that theoretically affects muscle metabolism. Because structural changes occur within the body, the user's locomotor metabolism may change in any footwear condition (e.g., conventional shoes, barefoot). Here, we studied the influence of habitual high-heel use on users' leg muscle-tendon structure and metabolism during walking in flat-soled footwear. We tested eight participants before and after 14 wk of agreeing to wear high heels as their daily shoes. Overall, participants who wore high heels >1,500 steps per day, experienced a 9% decrease in their net metabolic power during walking in flat-soled footwear (d = 1.66, P ≤ 0.049), whereas participants who took <1,000 daily steps in high heels did not (d = 0.44; P = 0.524). Across participants, for every 1,000 daily steps in high heels, net metabolic power during walking in flat-soled footwear decreased 5.3% (r = -0.73; P = 0.040). Metabolic findings were partially explained (r2 = 0.43; P = 0.478) by trending shorter medial gastrocnemius fascicle lengths (d = 0.500, P = 0.327) and increased Achilles tendon stiffness (d = 2.889, P = 0.088). The high-heel intervention did not alter user walking stride kinematics in flat-soled footwear (d ≤ 0.567, P ≥ 0.387). While our limited dataset is unable to establish the mechanisms underlying the high-heel-induced walking economy improvement, it appears that prescribing specific footwear use can be implemented to alter user muscle-tendon properties and augment their function in any shoes.NEW & NOTEWORTHY Habitually wearing high-heeled footwear structurally remodels leg muscle tendons and improves user walking economy, regardless of worn attire.

Running 40 Minutes under Temperate or Hot Environment Does Not Affect Operating Fascicle Length.

Muscle mechanics is paramount in our understanding of motor performance. However, little is known regarding the sensitivity of fascicle dynamics and connective tissues stiffness to exercise duration and ambient temperature during running, both increasing muscle temperature. This study aimed to determine gastrocnemius medialis (GM) fascicle dynamics in vivo during running in temperate and hot conditions, as well as muscle-tendon unit responses. Using ultrafast ultrasound, 15 participants (8 men, 7 women; 26 ± 3 yr) were tested before, during (2 and 40 min), and after a running task (40 min at 10 km·h -1 ) in temperate (TEMP; ~23°C) and hot (HOT: ~38°C) conditions. Although core, skin temperatures, and heart rate increased from the beginning to the end of the exercise and in a larger extent in HOT than TEMP ( P < 0.001), the physiological stress elicited did not alter running temporal parameters and GM fascicle operating lengths, with similar behavior of the fascicles on their force-length relationship, over time (2 vs 40 min) or across condition (TEMP vs HOT; P ≥ 0.248). Maximal voluntary force production did not reported statistical changes after exercise ( P = 0.060), and the connective tissues stiffness measured (i.e., passive muscle and stiffness of the series-elastic elements) did not show neither time ( P ≥ 0.281), condition ( P ≥ 0.256) nor time-condition interaction ( P ≥ 0.465) effect. This study revealed that prolonged running exercise does not alter muscle-tendon unit properties and interplay, which are not influenced by ambient temperature. These findings may rule out potential detrimental effects of heat on muscle properties and encourage further investigations on longer and more intense running exercise.

The effects of 6-week home-based static stretching, dynamic stretching, or eccentric exercise interventions on muscle-tendon properties and functional performance in older women

BackgroundJoint inflexibility is acknowledged as a significant contributor to functional limitations in the older adult, with lengthening-type exercises identified as a potential remedial approach. Nevertheless, the responses to eccentric exercise in female older adults have not been extensively studied especially in home-based environment. Here, we aimed to assess the effectiveness of home-based static stretching (ST), dynamic closed-chain stretching (DCS), or eccentric exercise (ECC) interventions on flexibility, musculotendinous architecture, and functional ability in healthy older women. MethodsWe randomly assigned 51 healthy older women (age 65.9 ± 3.4 years) to one of three interventional exercise groups: DCS (N = 17), ECC (N = 17), or ST (N = 17). The training was performed 3 times a week for 6 weeks. The participants’ musculotendinous stiffness, fascicle length, eccentric strength, and functional capacities were measured before the intervention, after 6 weeks of exercise, and at a 1-month follow-up. ResultsThe results showed that all three interventions improved hamstring flexibility and passive ankle dorsiflexion (p < 0.001), with increased biceps femoris and medial gastrocnemius fascicle length (p < 0.01). However, there was no significant change in musculotendinous stiffness. The ECC intervention produced a greater improvement in knee flexor and calf eccentric peak torque (p < 0.05), and gait speed (p = 0.024) than the other two interventions. The changes in flexibility and knee flexor strength remained for up to 4 weeks after detraining. ConclusionIn conclusion, the present study suggests that home-based ECC may be more beneficial in enhancing physical capacities in older women compared with either DCS or SS interventions.

In vivo human medial gastrocnemius fascicle behaviour and belly gear during submaximal eccentric contractions are not affected by concentric fatiguing exercise

Changes in muscle geometry and belly gearing during eccentric contractions influence fibre strain and susceptibility to muscle damage. They are modulated by the interaction between connective tissues and intracellular-intrafascicular fluid pressures and external pressures from neighbouring structures. Fatiguing exercise triggers fluid shifts (muscle swelling) and muscle activation changes that may influence these modulators. Our purpose was to measure medial gastrocnemius (MG) geometric changes in vivo during eccentric contractions before and after maximal concentric muscle work to test the hypothesis that fatigue would reduce fascicle rotation and muscle gear and provoke greater fascicle strain. Submaximal eccentric plantar flexor contractions at 40% and 60% of maximal eccentric torque were performed on an isokinetic dynamometer at 5°.s−1 before and immediately after the fatiguing exercise. MG fascicles and muscle–tendon junction were captured using ultrasonography during contractions, allowing quantification of geometric changes, whole-MG length, and belly gear (Δmuscle length/Δfascicle length). Triceps surae (TS) activation was estimated using surface electromyography and the distribution of activations between synergistic muscles was then determined. After exercise, concentric torque decreased ∼39% and resting muscle thickness increased by 4%, indicating muscle fatigue and swelling, respectively. While soleus (Sol) activation and the Sol/TS ratio increased, no changes in MG, MG/TS ratio or fascicle rotation during the contraction were detected. Thus, fascicle lengthening and belly gear remained unaltered. Changes in muscle thickness during contraction was also similar before and after exercise, suggesting that changes in muscle shape were relatively unaffected by the exercise. Consequently, the muscle maintained mechanical integrity after the fatiguing muscle work.

ISB clinical biomechanics award winner 2023: Medial gastrocnemius muscle and Achilles tendon interplay during gait in cerebral palsy

BackgroundThe interplay between the medial gastrocnemius muscle and the Achilles tendon is crucial for efficient walking. In cerebral palsy, muscle and tendon remodelling alters the role of contractile and elastic components. The aim was to investigate the length changes of medial gastrocnemius belly and fascicles, and Achilles tendon to understand their interplay to gait propulsion in individuals with cerebral palsy. MethodsTwelve young individuals with cerebral palsy and 12 typically developed peers were assessed during multiple gait cycles using 3D gait analysis combined with a portable ultrasound device. By mapping ultrasound image locations into the shank reference frame, the medial gastrocnemius belly, fascicle, and Achilles tendon lengths were estimated throughout the gait cycle. Participants with cerebral palsy were classified into equinus and non-equinus groups based on their sagittal ankle kinematics. FindingsIn typically developed participants, the Achilles tendon undertook most of the muscle-tendon unit lengthening during stance, whereas in individuals with cerebral palsy, this lengthening was shared between the medial gastrocnemius belly and Achilles tendon, which was more evident in the equinus group. The lengthening behaviour of the medial gastrocnemius fascicles resembled that of the Achilles tendon in cerebral palsy. InterpretationThe findings revealed similar length changes of the medial gastrocnemius fascicles and Achilles tendon, highlighting the enhanced role of the muscle in absorbing energy during stance in cerebral palsy. These results, together with the current knowledge of increased intramuscular stiffness, suggest the exploitation of intramuscular passive forces for such energy absorption.

American Society of Biomechanics Journal of Biomechanics Award 2021: Exploring the Functional Boundaries and Metabolic Consequences of Triceps Surae Force-Length Relations during Walking

The relationship between individual muscle dynamics and whole-body metabolic cost is not well established. Here we use biofeedback to modulate triceps surae (TS) activity during walking to test the following hypotheses based on basic principles of muscle physiology: (1) increased TS activity would increase metabolic cost via shorter muscle fascicle lengths and thus reduced force capacity and (2) decreased TS activity would decrease metabolic cost via longer muscle fascicle lengths and thus increased force capacity. 23 young adults walked on an instrumented treadmill at 1.25 m/s using electromyographic (EMG) biofeedback to match targets corresponding to ±20 and ±40% TS activity during push-off (late stance). B-mode ultrasound imaged the medial gastrocnemius (MG). Participants increased net metabolic power up to 85% and 21% when targeting increased and decreased TS activity, respectively (p < 0.001). At the instant of peak gastrocnemius force, MG fascicle length was 7% shorter (p < 0.001) and gastrocnemius force was 6% larger (p < 0.001) when targeting + 40% TS activity. Fascicle length was 3% shorter (p = 0.004) and force was 7% lower (p = 0.010) when targeting −40% TS activity; participants were unable to achieve decreased activation targets. MG fascicle length and activity mediated 11.7% (p = 0.036) and 57.2% (p = 0.006) of the changes in net metabolic power, respectively. MG force did not mediate changes in net metabolic power (p = 0.948). These findings suggest that changes in the functional operating length of muscle, induced here by volitional changes in TS activity, mediated changes in the metabolic cost of walking, relatively independently of force. Thus, shifts to shorter fascicle lengths (e.g., aging) may mediate activity-induced increases in metabolic cost.

Muscle fascicle and sarcomere adaptation in response to Achilles tendon elongation in an animal model.

Permanent loss of muscle function seen after an Achilles tendon rupture may partly be explained by tendon elongation and accompanying shortening of the muscle. Muscle fascicle length shortens, serial sarcomere number is reduced, and the sarcomere length is unchanged after Achilles tendon transection (ATT) and these changes are mitigated with suturing. Controlled laboratory study. Two groups of rats underwent ATT on one side with a contralateral control (CTRL): A) ATT with 3 mm removal of the Achilles tendon and no suturing (substantial tendon elongation), and B) ATT with suture repair (minimal tendon elongation). The operated limb was immobilized for 2 weeks to reduce load. Four weeks after surgery the rats were euthanized, and hindlimbs were analyzed for tendon length, gastrocnemius medialis (GM) muscle mass, length, fascicle length, sarcomere number and length. No differences were observed between the groups, and in both groups the Achilles tendon length was longer (15.2%, P<0.001), GM muscle mass was smaller (17.5%, P<0.001), and muscle length was shorter (8.2%, P<0.001) on the ATT compared to CTRL side. GM fascicle length was shorter (11.2%, P<0.001), and sarcomere number was lower (13.8%, P<0.001) on the ATT side in all regions. Sarcomere length was greater in the proximal (5.8%, P<0.001) and mid (4.2%, P=0.003), but not distal region on the ATT side. In this animal model, regardless of suturing, ATT resulted in tendon elongation, loss of muscle mass and length and reduced serial sarcomere number, which resulted in an "overshoot" lengthening of the sarcomeres.

Is the muscle–tendon architecture of non-athletic Kenyans different from that of Japanese and French males?

BackgroundIn endurance running, elite Kenyan runners are characterized by longer thigh, shank, and Achilles tendon (AT) lengths combined with shorter fascicles and larger medial gastrocnemius (MG) pennation angles than elite Japanese runners. These muscle-tendon characteristics may contribute to the running performance of Kenyans. Furthermore, these specific lower-leg musculoskeletal architectures have been confirmed not only in elite Kenyan runners but also in non-athletic Kenyans since early childhood. However, it remains questionable whether the differences in muscle-tendon architecture between Kenyans and Japanese differ from those of European Caucasians. Therefore, this study aimed to compare anthropometry and muscle–tendon architecture of young non-athletic Kenyan males with their Japanese and French counterparts.MethodsA total of 235 young non-athletic males, aged 17–22 years, volunteered. The anthropometric measures, thigh, and shank lengths, as well as AT and MG muscle architecture, were measured using ultrasonography and a tape measure. Inter-group differences in anthropometry and muscle-tendon architecture were tested using one-way ANOVA and ANCOVA analyses controlling for shank length and muscle thickness.ResultsThe anthropometric and muscle-tendon characteristics of the non-athletic French were closer to those of the Kenyans than to those of the Japanese. However, the ultrasonography analysis confirmed that the non-athletic Kenyans had the longest AT as well as the shortest MG fascicles and the largest pennation angle compared to the French and Japanese, even after controlling for shank length and muscle thickness with ANCOVA, respectively.ConclusionsThese results confirmed the specificity of the muscle-tendon architecture of the triceps surae in Kenyans in comparison to their Japanese and French counterparts in non-athletic adults. This study provides additional support to the fact that Kenyans may have musculotendinous advantages in endurance running.

Medial gastrocnemius muscle fascicle function during heel-rise after non-operative repair of Achilles tendon rupture

BackgroundTo better understand muscle remodelling in dynamic conditions after an Achilles tendon rupture, this study examined the length of medial gastrocnemius muscle fascicles during a heel-rise at 6- and 12-months after non-operative ATR treatment. MethodsParticipants (15 M, 3F) were diagnosed with acute Achilles tendon rupture. Medial gastrocnemius subtendon length, fascicle length and pennation angle were assessed in resting conditions, and fascicle shortening during bi- and unilateral heel-rises. FindingsFascicle shortening was smaller on the injured side (mean difference [95% CI]: −9.7 mm [−14.7 to −4.7 mm]; −11.1 mm [−16.5 to −5.8 mm]) and increased from 6- to 12 months (4.5 mm [2.8–6.3 mm]; 3.2 mm [1.4–4.9 mm]) in bi- and unilateral heel-rise, respectively. The injured tendon was longer compared to contralateral limb (2.16 cm [0.54–3.79 cm]) and the length decreased over time (−0.78 cm [−1.28 to −0.29 cm]). Tendon length correlated with fascicle shortening in bilateral (r = −0.671, p = 0.002; r = −0.666, p = 0.003) and unilateral (r = −0.773, p ≤ 0.001; r = −0.616, p = 0.006) heel-rise, at 6- and 12-months, respectively. In the injured limb, the change over time in fascicle shortening correlated with change in subtendon length in unilateral heel-rise (r = 0.544, p = 0.02). InterpretationThis study showed that the lengths of the injured tendon and associated muscle can adapt throughout the first year after rupture when patients continue physiotherapy and physical exercises. For muscle, measures of resting length may not be very informative about adaptations, which manifest themselves during functional tasks such as unilateral heel-rise.

Increased muscle force does not induce greater stretch-induced damage to calf muscles during work-matched heel drop exercise

PurposeTo investigate the effect of muscle force during active stretch on quantitative and qualitative indicators of exercise-induced muscle damage (EIMD) in the medial gastrocnemius (MG) muscle.MethodsTwelve recreationally active volunteers performed two trials of an eccentric heel drop exercise. Participants performed a single bout of low-load (body weight) and high-load (body weight + 30% body weight) exercises on separate legs. The total mechanical work output for each condition was matched between legs. Before, two hours and 48 h after each bout of eccentric exercise, electrically stimulated triceps surae twitch torque, muscle soreness, MG active fascicle length at maximum twitch torque and muscle passive stiffness were collected. Triceps surae electromyographic (EMG) activity, MG fascicle stretch and MG muscle–tendon unit (MTU) length were measured during the eccentric tasks.ResultsThe high-load condition increased triceps surae muscle activity by 6–9%, but reduced MG fascicle stretch (p < 0.001). MTU stretch was similar between conditions. The greater muscle force during stretch did not give rise to additional torque loss (5 vs 6%) or intensify muscle soreness.ConclusionsAdding 30% body weight during eccentric contractions has a modest impact on exercise-induced muscle damage in the medial gastrocnemius muscle. These results suggest that muscle load may not be an important determinant of stretch-induced muscle damage in the human MG muscle. The muscle investigated does exhibit large pennation angles and high series elastic compliance; architectural features that likely buffer muscle fibres against stretch and damage.

Maximal fascicle shortening velocity measurements in human medial gastrocnemius muscle in vivo

This study evaluated the maximal fascicle shortening velocity under near-no-load conditions. In addition, we determined whether the rate of torque development during ballistic contraction was related to maximal fascicle shortening velocity. Under passive and active conditions, the medial gastrocnemius muscle fascicle shortening velocity was measured using ultrasonography at 300, 400, 500, 600, 700, 800, 900, and 1000 °s-1 . The maximal fascicle shortening velocity was defined as the fascicle shortening velocity under the lowest angular velocity that satisfied the following two conditions; (1) the difference in torque values between passive and active conditions was below 2.4Nm and (2) the difference in fascicle shortening velocities between passive and active conditions was below 10 mm s-1 . The rate of torque development was analyzed during the periods of 32, 48, 96, 152, and 200 ms after the onset of contraction during ballistic contraction. At the angular velocity (678.6± 147.7°s-1 ) that satisfied the two previously mentioned conditions, the exerted torque and the maximal fascicle shortening velocity were 1.4± 1.3Nm and 251.0± 40.5 mm s-1 . No significant correlations were found between the maximal fascicle shortening velocity and the rate of torque development at each time point. In conclusion, the maximal fascicle shortening velocity was quantified when the angular velocity satisfied the two conditions. Furthermore, the rate of torque development, often used as an indicator of muscle velocity, did not represent the maximal fascicle shortening velocity.

Personalisation of Plantarflexor Musculotendon Model Parameters in Children with Cerebral Palsy

Neuromusculoskeletal models can be used to evaluate aberrant muscle function in cerebral palsy (CP), for example by estimating muscle and joint contact forces during gait. However, to be accurate, models should include representative musculotendon parameters. We aimed to estimate personalised parameters that capture the mechanical behaviour of the plantarflexors in children with CP and typically developing (TD) children. Ankle angle (using motion capture), torque (using a load-cell), and medial gastrocnemius fascicle lengths (using ultrasound) were measured during slow passive ankle dorsiflexion rotation for thirteen children with spastic CP and thirteen TD children. Per subject, the measured rotation was input to a scaled OpenSim model to simulate the torque and fascicle length output. Musculotendon model parameters were personalised by the best match between simulated and experimental torque–angle and fascicle length-angle curves according to a least-squares fit. Personalised tendon slack lengths were significantly longer and optimal fibre lengths significantly shorter in CP than model defaults and than in TD. Personalised tendon compliance was substantially higher in both groups compared to the model default. The presented method to personalise musculotendon parameters will likely yield more accurate simulations of subject-specific muscle mechanics, to help us understand the effects of altered musculotendon properties in CP.

Three‐dimensional architecture of human medial gastrocnemius fascicles in vivo: Regional variation and its dependence on muscle size

Fascicle architecture (length and pennation angle) can vary regionally within a muscle. The architectural variability in human muscles has been evaluated in vivo, but the interindividual variation and its determinants remain unclear. Considering that within-muscle non-uniform changes in pennation angle are associated with change in muscle size by chronic mechanical loading, we hypothesized that the regional variation in fascicle architecture is dependent on interindividual variation in muscle size. To test this hypothesis, we reconstructed fascicles three-dimensionally along and across the whole medial gastrocnemius in the right lower leg of 15 healthy adults (10 males and 5 females, 23.7 ± 3.3 years, 165.8 ± 8.3cm, 61.9 ± 11.4kg, mean ± standard deviation) in neutral ankle joint position with the knee fully extended, using magnetic resonance diffusion tensor imaging and tractography. The 3D-reconstructed fascicles arose from the deep aponeurosis with variable lengths and angles both in sagittal and coronal planes. The fascicle length was significantly longer in the middle (middle-medial: 52.4 ± 6.1 mm, middle-lateral: 52.0 ± 5.1 mm) compared to distal regions (distal-medial: 41.0 ± 5.0 mm, distal-lateral: 38.9 ± 3.6 mm, p < 0.001). The 2D pennation angle (angle relative to muscle surface) was significantly greater in distal than middle regions, and medial than lateral regions (middle-medial: 26.6 ± 3.1°, middle-lateral: 24.1 ± 2.3°, distal-medial: 31.2 ± 3.6°, distal-lateral: 29.2 ± 3.0°, p ≤ 0.017), while only a proximo-distal difference was significant (p < 0.001) for 3D pennation angle (angle relative to line of action of muscle). These results clearly indicate fascicle's architectural variation in 3D. The magnitude of regional variation evaluated as standard deviation across regions differed considerably among individuals (4.0-10.7 mm for fascicle length, 0.9-5.0° for 2D pennation angle, and 3.0-8.8° for 3D pennation angle), which was positively correlated with the muscle volume normalized to body mass (r=0.659-0.828, p ≤ 0.008). These findings indicate muscle-size dependence of the variability of fascicle architecture.

Faster early rate of force development in warmer muscle: an in vivo exploration of fascicle dynamics and muscle-tendon mechanical properties.

Although heat exposure has been shown to increase the skeletal rate of force development (RFD), the underlying processes remain unknown. This study investigated the effect of heat on gastrocnemius medialis (GM) muscle-tendon properties and interactions. Sixteen subjects performed electrically evoked and voluntary contractions combined with ultrafast ultrasound under thermoneutral [control (CON): 25.8 ± 1.8°C, core temperature 37.0 ± 0.3°C, muscle temperature 34.0 ± 1.1°C] and passive heat exposure [hot (HOT): 47.4 ± 1.8°C, core temperature 38.4 ± 0.3°C, muscle temperature 37.0 ± 0.8°C] conditions. Maximal voluntary force changes did not reach statistical significance (-5.0 ± 11.3%, P = 0.052) whereas voluntary activation significantly decreased (-4.6 ± 8.7%, P = 0.038) in HOT. Heat exposure significantly increased voluntary RFD before 100 ms from contraction onset (+48.2 ± 62.7%; P = 0.013), without further changes after 100 ms. GM fascicle dynamics during electrically evoked and voluntary contractions remained unchanged between conditions. Joint velocity at a given force was higher in HOT (+7.1 ± 6.6%; P = 0.004) but the fascicle force-velocity relationship remained unchanged. Passive muscle stiffness and active tendon stiffness were lower in HOT than CON (P ≤ 0.030). This study showed that heat-induced increases in early voluntary RFD may not be attributed to changes in contractile properties. Late voluntary RFD was unaltered, possibly due to decreased soft tissues' stiffness in heat. Further investigations are required to explore the influence of neural drive and motor unit recruitment in the enhancement of explosive strength elicited by heat exposure.

Muscle-tendon architecture in Kenyans and Japanese: Potential role of genetic endowment in the success of elite Kenyan endurance runners.

The specificity of muscle-tendon and foot architecture of elite Kenyan middle- and long-distance runners has been found to contribute to their superior running performance. To investigate the respective influence of genetic endowment and training on these characteristics, we compared leg and foot segmental lengths as well as muscle-tendon architecture of Kenyans and Japanese males (i) from infancy to adulthood and (ii) non-athletes versus elite runners. The 676 participants were divided according to their nationality (Kenyans and Japanese), age (nine different age groups for non-athletes) and performance level in middle- and long-distance races (non-athlete, non-elite and elite adult runners). Shank and Achilles tendon (AT) lengths, medial gastrocnemius (MG) fascicle length, pennation angle and muscle thickness, AT moment arm (MAAT ), and foot lever ratio were measured. Above 8years old, Kenyans had a longer shank and AT, shorter fascicle, greater pennation angle, thinner MG muscle as well as longer MAAT , with lower foot lever ratio than age-matched Japanese. Among adults of different performance levels and independently of the performance level, Kenyans had longer shank, AT and MAAT , thinner MG muscle thickness, and lower foot lever ratio than Japanese. The decrease in MG fascicle length and increase pennation angle observed for the adult Japanese with the increase in performance level resulted in a lack of difference between elite Kenyans and Japanese. The specificity of muscle-tendon and foot architecture of elite Kenyan runners could result from genetic endowment and contribute to the dominance of Kenyans in middle- and long-distance races.

Altered Gastrocnemius Contractile Behavior in Former Achilles Tendon Rupture Patients During Walking.

Achilles tendon rupture (ATR) remains associated with functional limitations years after injury. Architectural remodeling of the gastrocnemius medialis (GM) muscle is typically observed in the affected leg and may compensate force deficits caused by a longer tendon. Yet patients seem to retain functional limitations during—low-force—walking gait. To explore the potential limits imposed by the remodeled GM muscle-tendon unit (MTU) on walking gait, we examined the contractile behavior of muscle fascicles during the stance phase. In a cross-sectional design, we studied nine former patients (males; age: 45 ± 9 years; height: 180 ± 7 cm; weight: 83 ± 6 kg) with a history of complete unilateral ATR, approximately 4 years post-surgery. Using ultrasonography, GM tendon morphology, muscle architecture at rest, and fascicular behavior were assessed during walking at 1.5 m⋅s–1 on a treadmill. Walking patterns were recorded with a motion capture system. The unaffected leg served as control. Lower limbs kinematics were largely similar between legs during walking. Typical features of ATR-related MTU remodeling were observed during the stance sub-phases corresponding to series elastic element (SEE) lengthening (energy storage) and SEE shortening (energy release), with shorter GM fascicles (36 and 36%, respectively) and greater pennation angles (8° and 12°, respectively). However, relative to the optimal fascicle length for force production, fascicles operated at comparable length in both legs. Similarly, when expressed relative to optimal fascicle length, fascicle contraction velocity was not different between sides, except at the time-point of peak series elastic element (SEE) length, where it was 39 ± 49% lower in the affected leg. Concomitantly, fascicles rotation during contraction was greater in the affected leg during the whole stance-phase, and architectural gear ratios (AGR) was larger during SEE lengthening. Under the present testing conditions, former ATR patients had recovered a relatively symmetrical walking gait pattern. Differences in seen AGR seem to accommodate the profound changes in MTU architecture, limiting the required fascicle shortening velocity. Overall, the contractile behavior of the GM fascicles does not restrict length- or velocity-dependent force potentials during this locomotor task.

The effect of small changes in rate of force development on muscle fascicle velocity and motor unit discharge behaviour

When rate of force development is increased, neural drive increases. There is presently no accepted explanation for this effect. We propose and experimentally test the theory that a small increase in rate of force development increases medial gastrocnemius fascicle shortening velocity, reducing the muscle’s force-generating capacity, leading to active motor units being recruited at lower forces and with increased discharge frequencies. Participants produced plantar flexion torques at three different rates of force development (slow: 2% MVC/s, medium: 10% MVC/s, fast: 20% MVC/s). Ultrasound imaging showed that increased rate of force development was related to higher fascicle shortening velocity (0.4 ± 0.2 mm/s, 2.0 ± 0.9 mm/s, 4.1 ± 1.9 mm/s in slow, medium, fast, respectively). In separate experiments, medial gastrocnemius motor unit recruitment thresholds and discharge frequencies were measured using fine-wire electromyography (EMG), together with surface EMG. Recruitment thresholds were lower in the fast (12.8 ± 9.2% MVC) and medium (14.5 ± 9.9% MVC) conditions compared to the slow (18.2 ± 8.9% MVC) condition. The initial discharge frequency was lower in the slow (5.8 ± 3.1 Hz) than the fast (6.7 ± 1.4 Hz), but not than the medium (6.4 ± 2.4 Hz) condition. The surface EMG was greater in the fast (mean RMS: 0.029 ± 0.017 mV) compared to the slow condition (0.019 ± 0.013 mV). We propose that the increase in muscle fascicle shortening velocity reduces the force-generating capacity of the muscle, therefore requiring greater neural drive to generate the same forces.
