Resonance Frequency Analysis to Assess the Stability of Immediate Implants: A Retrospective Clinical Trial.

Adequate implant primary stability is a key factor to obtain osseointegration and can be measured at insertion by insertion torque (IT) and at different timepoints with resonance frequency analysis (RFA), expressed as an implant stability quotient (ISQ). This retrospective study investigated the correlation between ISQ and IT at implant insertion. All patients who were eligible for this single-cohort retrospective clinical trial were treated with an immediate implant. IT parameters were recorded at implant insertion, and ISQ values were recorded at insertion and at 2-, 4-, and 12-month follow-ups. The study comprised 23 patients who received 32 implants. The mean IT value was 46.87 ± 9.66 Ncm (range: 25 to 65 Ncm), and the mean ISQ value at implant insertion was 71.45 ± 4.24 (range: 63 to 78); these values showed a statistically significant correlation (P < .0001). According to the present data and considering the implant design used in this trial, there is a statistically significant and positive correlation between IT and ISQ values. Thus, ISQ can be used as a reliable method to measure implant stability over time.

The clinical efficacy of the allogenic demineralized dentin matrix graft for implant placement: a systematic review.

The Allogenic Demineralized Dentin Matrix (Allo-DDM) has been introduced as a new grafting material, and several investigations were conducted, showing its efficacy in bone augmentation. The objective of this systematic review was to evaluate the available literature on the Allo-DDM, revealing its clinical performance when used for implant placement procedures. This study is registered in the PROSPERO database (number: CRD42021264885, date: 30/7/2021). Four databases, as well as the grey literature, were searched, selecting human studies where the Allo-DDM was applied to augment implant-recipient sites. Six articles were included. The total number of implants placed in Allo-DDM-grafted sites was 149. Mean implant stability quotient values (ISQ) were 60.4 and 68.67 for the primary and secondary ISQ, respectively (data from one study). Buccal marginal bone loss around implants was approximately 1.46 mm after 24 months of prosthetic loading (data from one study). Two studies documented a total of 6 cases of dehiscence of the grated area, however, not affecting early implant success. Histological findings from all studies showed new bone formation around the graft particles. A low number of publications are available, presenting only preliminary results, thus the long-term success/survival of implants still needs to be further explored. Moreover, the possibility of bony dehiscence with the use of this material should be investigated. Within these limitations, the Allo-DDM could be a possible alternative to other grafting materials used for bone augmentation and implant placement. Nevertheless, considering this limited evidence, future studies are necessary to confirm this conclusion.

Immediate vs. Delayed Placement of Immediately Provisionalized Self-Tapping Implants: A Non-Randomized Controlled Clinical Trial with 1 Year of Follow-Up.

This study aimed to examine the clinical and esthetic outcomes of immediately provisionalized self-tapping implants placed in extraction sockets or healed edentulous ridges one year after treatment. Sixty patients in need of a single implant-supported restoration were treated with self-tapping implants (Straumann BLX) and immediate provisionalization. The implant stability quotient (ISQ) and insertion torque were recorded intraoperatively. After one year in function, the implant and prosthesis survival rate, pink esthetic score (PES), white esthetic score (WES), and marginal bone levels (MBL) were assessed. Sixty patients received 60 self-tapping implants. A total of 37 implants were placed in extraction sockets and 23 in edentulous ridges, and then all implants were immediately provisionalized. All implants achieved a high implant stability with a mean insertion torque and ISQ value of 58.1 ± 14.1 Ncm and 73.6 ± 8.1 Ncm, respectively. No significant differences were found between healed vs. post-extractive sockets (p = 0.716 and p = 0.875), or between flap vs. flapless approaches (p = 0.862 and p = 0.228) with regards to the insertion torque and ISQ value. Nonetheless, higher insertion torque values and ISQs were recorded for mandibular implants (maxilla vs. mandible, insertion torque: 55.30 + 11.25 Ncm vs. 62.41 + 17.01 Ncm, p = 0.057; ISQ: 72.05 + 8.27 vs. 76.08 + 7.37, p = 0.058). One implant did not osseointegrate, resulting in an implant survival rate of 98.3%. All implants achieved PES and WES scores higher than 12 at the 1-year follow-up. The clinical use of newly designed self-tapping implants with immediate temporization was safe and predictable. The implants achieved a good primary stability, high implant survival rate, and favorable radiographic and esthetic outcomes, regardless of the immediate or delayed placement protocols.

The Effects of Injectable Platelet-Rich Fibrin on Implant Stability.

To investigate the effects of injectable platelet-rich fibrin (i-PRF) on implant stability. A total of 40 implants (BEGO Semados RS/RSX implants, BEGO Implant System) were surgically placed in 15 patients between the ages of 25 and 67 years who had mandibular edentulous areas. After the implant sockets were prepared with the appropriate protocol, i-PRF was applied to the implant surface and socket with the help of a 5-cc sterile syringe in the study group, and implants were placed without i-PRF in the control group. In the research process, the resonance frequency analysis (RFA) method was used to measure implant stability. The implant stability quotient (ISQ) values were determined during the time of the operation and at the first, second, and fourth weeks. The results obtained after the stability measurement periods showed that the decrease in the mean ISQ values in the control group was statistically significant in the first week. Evaluations made in the following weeks were not statistically significant. The study group showed an increase in ISQ values during the measurement periods, and the increases in the second and fourth weeks were statistically significant. I-PRF had positive effects on early implant stability, and i-PRF can be safely used in dental implant surgery and promotes bone healing around dental implants.

Resonance Frequency Analysis for Immediately Placed Dental Implants Replacing Periapically Infected Teeth

Aims: There is a controversy regarding the indication of implant insertion into the sockets of infected teeth. This study aimed to evaluate the outcome of immediate implants replacing periapically infected teeth utilizing Resonance Frequency Analysis (RFA) method. Materials and Methods: Preoperatively, clinical, and radiographic examination accomplished for the patients. After anesthetizing the surgical area, the accused tooth extracted, and the socket curetted by surgical curette to remove the periapical lesion then irrigated by normal saline solution. The implant inserted into its prepared site. Beta-tricalcium phosphate (β-TCP) (combined by collagen membrane) used to fill gaps ≥ 2 mm and to repair bone defects. Implant stability quotient (ISQ) values were measured for the implants during surgery and after 16 weeks. Postoperative clinical and radiographic evaluation were conducted for each patient. Results: Fourteen implants out of 41 immediate implants (34.1%) had been inserted in the extraction sockets of teeth have chronic periapical lesions (infected sites), T-test showed no significant difference in implant stability (ISQ value) between implant placed in infected and non-infected sites neither at baseline nor at 16-weeks. Paired t-test showed highly significant increase in implant stability (ISQ value) of implants placed in infected sites while in the other implants the mean ISQ value increased with no significant difference during the healing period. The success rate was 100% after 4 years of implantation. Conclusions: Presence of periapical lesion is not a contraindication to place immediate implant when properly managed and can provide similar survival rate to that of implants inserted into healthy sockets.

Clinical and radiographic outcomes of dental implant after maxillary sinus floor augmentation with rhBMP-2/hydroxyapatite compared to deproteinized bovine bone.

ObjectivesThis study aimed to evaluate the clinical and radiographic outcomes of early implant placement and functional loading in maxillary sinus floor augmentation (MSFA) using recombinant human bone morphogenetic protein 2/hydroxyapatite (rhBMP-2/HA) and to compare these outcomes with those of the conventional protocol in MSFA using deproteinized bovine bone (DBB).Materials and methodsThe rhBMP-2/HA and DBB groups consisted of 14 and 13 patients who underwent MSFA with BMP and DBB, respectively. After placement of 22 implants and 21 implants in the rhBMP-2/HA and DBB groups, respectively, abutment connections were performed 3 months after implant placement for the rhBMP-2/HA group and 6 months after implant placement for the DBB group. Changes in grafted sinus height (GSH), marginal bone loss (MBL), and implant stability were evaluated up to one year after functional loading.ResultsSurvival rates for the rhBMP-2/HA and DBB groups after one year of functional loading were 90.9% and 90.5%, respectively. Both groups exhibited no significant time-course changes in GSH until one year of functional loading (rhBMP-2/HA, p = 0.124; DBB, p = 0.075). Although significant MBL occurred after one year of functional loading for both groups (rhBMP-2/HA, p < 0.001; DBB, p < 0.001), there were no significant differences in time-course changes in MBL between the two groups (p = 0.450). The mean implant stability quotient values in the rhBMP-2/HA and DBB groups were 75.3 and 75.4 after one year of functional loading, respectively, and there were no significant differences between the two groups (p = 0.557).ConclusionsMSFA using rhBMP-2/HA allowed implant rehabilitation with early implant placement and functional loading and led to a comparable survival rate and implant stability after 1 year of functional loading with acceptable MBL and stable maintenance of GSH compared to the MSFA using DBB with 6 months of healing after implant placement.

The influence of implant design on implant stability in low-density bone under guided surgical template in inexperienced surgeons: A pilot randomized controlled clinical trial using resonance frequency analysis.

There are several factors that greatly influence implant stability with implant design being a paramount factor; however, few studies investigate its impact. To investigate the implant stability in relation to two different implant designs, a cylindrical shaped single-threaded design (CS/ST) and a tapered shaped double-threaded design (TS/DT) using resonance frequency analysis over the first 8 weeks after implantation. Twenty-eight implants were randomly allocated using computer-generated random numbers into two groups and were placed as single tooth implant in the posterior arch in human jaw as specimens. iCAT™ CBCT scan (Hatfield, PA) was used to determine the bone density according to Misch's Bone classification. The osseotomy sites were prepared and implants were placed with guided surgical template by inexperienced surgeons which were prepared with the same implant planning software (3shape® implant studio). The implant stability was measured using the resonance frequency analysis Osstell® ISQ (Osstell AB, Sweden) on the implant level over the first 8 weeks at three different time intervals. A mean implant stability quotient (ISQ) value was recorded at each measurement time points. The first ISQ of each implant recorded at the time of implant placement were considered as baseline and were the so-called primary stability. All 28 dental implants were analyzed. A similar pattern of implant stability changes was observed in both implant designs. A significant decreased was found at the first 4 weeks after implantation (p <0.05) before ascending to maximum cumulative stability by the eighth week (p <0.05). Between the two groups, TS/DT group had a higher mean ISQ values than that of the CS/ST group at all three observation periods but did not reach statistical significance (p=0.69). Regarding different types of bone, TS/DT showed a significant difference in mean ISQ values in D4 bone. To date, all 28 implants are in function with no failure/and or complications. The difference in implant design did not significantly influence the implant stability. TS/DT shows superiority over CS/ST when placed in D4 bone and offer a significant advantage due to their positive bounce back of the ISQ values by the eighth week after implant installation. The stability of tetracalcium phosphate/titanium implants: A short-term follow-up study

Background/purposeBioceramic tetracalcium phosphate (TTCP) is used as a surface modifier on the implant surface and the clinical studies on this surface modification are still limited. The objective of this clinical study was to investigate short-term implant stability of titanium implant surfaces being modified through sandblasting and acid etching (SLA), followed by TTCP sintered bioceramic anchoring.Materials and methodsA total of 20 patients who had single tooth space were included in this study. Surface modification by SLA plus with TTCP on Ti implants with a diameter of 4.0 mm and lengths of 10 and 11.5 mm were placed. Implant stability quotient (ISQ) value was measured immediately (ISQ0) and one month (ISQ1), two months (ISQ2), three months (ISQ3), and four months (ISQ4) after implantation. Subgroup analysis was defined to location (maxilla, mandible) and bone density (soft or hard bone). Statistical analysis was performed using Friedman test and Mann–Whitney U test.ResultsThe mean ISQ values with standard deviation at the different time points of ISQ0 to ISQ4 were 60.03 ± 14.12, 53.48 ± 15.24, 58.91 ± 14.43, 63.14 ± 12.22, and 63.50 ± 13.61, respectively. The results showed significant differences between the ISQ1 and ISQ3 groups and between the ISQ1 and ISQ4 groups. On the other hand, there was no statistical differences between the maxilla and mandible as well as between soft and hard bone types in all implant groups.ConclusionTTCP/titanium implant showed favorable stability in short-term ISQ values over 4 months. The locations and bone types demonstrated no effect on implant stability.

Evaluation of the effect of the lateralized inferior alveolar nerve isolation and bone grafting on the nerve function and implant stability. (Randomized Clinical Trial).

The inferior alveolar nerve lateralization (IANL), although allows for an implant full-length mandibular height engagement, coincides with depleting the buccal bone support and sensory deficits. This study aims to assess whether interposing a bone graft coupled with securing a collagen membrane separation between the inferior alveolar nerve (IAN) and the underlying dental implants would preserve the nerve function, enhance the implant stability, and minimize the radiographic marginal bone loss. Eighteen patients with 30 atrophic mandibular edentulous ridges were subjected to IANL after being randomly assigned to two treatment modalities which consisted of 15 patients each. The (control group) utilized conventional IANL in direct contact with 20 implants. The (test group) implemented the IAN collagen-membrane wrapping and interposing bone graft to overlay 23 implants. The neural function, the radiographic marginal bone loss, and the implant stability quotient were assessed and compared 6 months postoperatively. All the patients regained their full neurosensory function after 6 months, with statistically nonsignificant differences between both groups throughout the follow-up period. The mean marginal bone loss in the test group was (0.42 ± 0.09) mm versus (0.38 ± 0.14) mm for the control group, which was statistically similar (P= 0.401). The 6-month postoperative mean implant stability quotient values of the test group recorded (74.73 ± 2.68) versus (74.73 ± 1.79) for the control group, which was statistically nonsignificant with a value of P= 0.626. The interposed bone graft, coupled with the collagen membrane isolation, neither subsided the neural disturbances nor enhanced the secondary implant stability and marginal bone loss.

Comparison of Reliability of 3 Resonance Frequency Analysis Devices: An In Vitro Study.

The purpose of the present study was to investigate the intraobserver and interobserver reliability of 3 resonance frequency analysis (RFA) devices and to compare the implant stability quotient (ISQ) values according to implant macro design and diameter in 2 different bone densities. A total of 64 implants (Neoss ProActive; Neoss; Harrogate, UK) of varying diameters (3.5 and 4.0 mm) and implant macro design (tapered and straight) were placed in 2 artificial bone blocks (the density of type 2 and 3). The implant primary stability was measured using Osstell IDx (Osstell; Göteborg, Sweden), Osstell Beacon and Penguin RFA (Integration Diagnostics; Göteborg, Sweden). The ISQ value of each implant was measured by 2 observers and recorded 5 times in 2 directions. The intraobserver and interobserver reliability of RFA devices were evaluated. In addition to that, mean ISQ values were calculated for each RFA device to evaluate the effect of implant diameter, implant macro design, and bone density on ISQ values. ISQ values were significantly higher for implants placed within the type 2 bone than for the type 3 bone. The 4.0-mm diameter implants presented higher ISQ values than 3.5-mm diameter implants. The intraclass correlation coefficient (ICC) values for intraobserver reliability were above 0.85 for each observer and the ICC values for interobserver reliability were 0.94, 0.93, and 0.98 for Osstell IDx, Osstell Beacon, and Penguin RFA, respectively. Although there was excellent interobserver reliability with 3 RFA devices, the intraobserver reliability of Osstell Beacon and Penguin RFA were slightly better than Osstell IDx. Bone density and implant diameter were parameters affecting the primary stability of implants.

Alveolar ridge preservation in defect sockets in the maxillary aesthetic zone followed by single-tooth bone level tapered implants with immediate provisionalization: a 1-year prospective case series

BackgroundClinical studies of single-tooth replacement in compromised bone using bone level tapered implants in the aesthetic zone are scarce.AimTo assess clinically, radiographically and aesthetically over 1 year the performance of a bone level tapered implant in the maxillary aesthetic zone in sites after alveolar ridge preservation.Material and methodsThirty patients (16 male, 14 female) with a failing tooth and large bone defect after removal received alveolar ridge preservation. After 3 months, implants were placed with immediate provisionalization. Definitive restorations were placed after 3 months. The treatment was evaluated 1 year following the definitive restoration.ResultsAll the patients attended the 1-year follow-up. One implant was lost (96.7% implant survival rate). The mean implant stability quotient value was 68.9 ± 8.74 at implant placement. The mean marginal bone level change was minor (− 0.07 ± 0.12 mm). The mean mid-buccal mucosa changed with + 0.01 ± 0.45 mm. The median Pink Esthetic Score and White Esthetic Score after 1 year were 6 [4; 7] and 8 [7; 9], respectively. The patients’ mean overall satisfaction (0–100 VAS scale) was 86.6 ± 10.3.ConclusionBone level tapered implants with immediate provisionalization perform well after alveolar ridge preservation in the maxillary aesthetic zone, according to implant stability, clinical, radiographic, aesthetic and patient-centred outcomes.Trial registrationNTR, NL8755. Evaluation of variation in implant stability quotient at different time intervals after immediate loading in the posterior mandible using penguin resonance frequency analysis system

Purpose: The aim of this in vivo study is to compare the variation in implant stability quotient (ISQ) values of implants using resonance frequency analysis (RFA) to access the immediately loaded (within 24 h) implants at different time intervals using Penguin RFA system. Materials and Methods: Ten Nobel Replace Tapered RP implants were placed in posterior mandible region of patients. Patients were included in the study after fulfilling the inclusion criteria. Prior to enrolling in the study, thorough information about the procedure was provided to the patients, and written consent was obtained for the same. Immediate loading was done within 24 h using tooth-colored self-cure acrylic resin. Penguin RFA System was used to measure the ISQ of dental implants. The ISQ values were recorded immediately after implant placement, after 4 weeks, after 8 weeks, and after 12 weeks. Results: The mean ISQ values of the immediately loaded implants at the time of placement were 82.60, after 4 weeks were 80.60, after 8 weeks were 83.70, and after 12 weeks were 87.10. There was a statistically significant difference between the ISQ values at the time of placement of implants and after 12 weeks. Conclusion: This study concluded that immediate loading in posterior mandible single tooth region is a safe and predictable option.

Comparison of Measurements of Implant Stability by Two Different Radio Frequency Analysis Systems: An In Vitro Study

Aim:To compare and evaluate the implant stability quotient (ISQ) measurements by two different radio frequency analysis (RFA) machines, Osstell Mentor and PenguinRFA.Materials and Methods:Twenty bone-level implants (3.7 × 10 mm2) were placed on a bovine bone in this in vitro study. The ISQ measurements were performed with the PenguinRFAand Osstell Mentor machines using the multipeg of the Penguin system. The measurements were repeated three times for each direction and an average ISQ value was calculated. The results were averaged (mean ± SD) and the intraclass correlation coefficient (ICC) was calculated to assess the relationship between the measurements.Result:The mean ISQ values for the Osstell and Penguin machines were 77.60 + 2.11 and 78.05 + 2.04, respectively. The ICC was 0.958. The ISQ values obtained from the Osstell and Penguin machines were significantly compatible ( P &lt; .05).Conclusion:Both of the RFA devices provided similar and reliable ISQ measurements.

Facial Gingival Changes With and Without Socket Gap Grafting Following Single Maxillary Anterior Immediate Tooth Replacement: One-Year Results.

This 1-year prospective study evaluated horizontal and vertical facial gingival tissue changes after immediate implant placement and provisionalization (IIPP) with and without bone graft in the implant-socket gap (ISG). During IIPP, 10 patients received bone graft material in the ISG (G group), while the other 10 patients did not (NG group). The implants were evaluated for implant stability quotient (ISQ), modified plaque index (mPI), modified bleeding index (mBI), marginal bone level (MBL), facial gingival level (FGL), and facial gingival profile (FGP) changes. The mean ISQ value at 9-month follow-up was statistically significantly greater than on the day of implant surgery (P < .05). The mPI and mBI scores demonstrated that patients were able to maintain a good level of hygiene. There were no statistically significant differences in the mean MBL changes between the G and NG groups (P > .05). There were statistically significant differences in FGL changes between the G (-0.77 mm) and NG (-1.35 mm) groups (P = .035). There were no statistically significant differences in FGP changes between the G and NG groups (P > .05). However, statistically significant differences were noted in FGP change between the 3-12 and 0-12 month intervals in both groups (P < .05). Within the limitations of this study, although no significant differences were noted in FGP changes between groups, G group experienced significantly less FGL changes than NG group. Bone graft material placement into ISG seems to be advantageous for tissue preservation during IIPP. However, future long-term studies, with larger sample size, are needed to validate the efficacy of such procedure.

Is a two-month healing period long enough to achieve osseointegration? A prospective clinical cohort study

The aim of this study was to evaluate the effectiveness of loading at 2 months after the surgical placement of implants. A tapered implant was used. Implant stability was determined using resonance frequency analysis at implant insertion (T0; primary stability), after 2 months (T1), and at the 6-month follow-up (T2). Descriptive statistics and the t-test were used. Significance was at P ≤ 0.05. A total of 268 implants were inserted in 142 patients. No patient dropped out and no implant had failed at the 6-month follow-up. The mean implant stability quotient value (ISQ) increased over time, with a statistically non-significant difference for T0 vs. T1 (P = 0.08) and a statistically significant difference for T1 vs. T2 (P = 0.0018) and T0 vs. T2 (P = 0.000). Only 99 implants, characterized by an extremely high mean primary stability value (80.34), did not have a recorded increase in ISQ at T1. A 2-month healing period allowed the implant to achieve secondary stability, confirming the effectiveness of the loading protocol at 2 months.

Alveolar Ridge Expansion by Osseodensification-Mediated Plastic Deformation and Compaction Autografting: A Multicenter Retrospective Study.

Osseodensification preserves bone bulk, facilitates compaction autografting, and deforms trabecular bone in an outward strain, which result in alveolar ridge plastic expansion. The aim of this retrospective study was to evaluate ridge expansion after osseodensification. Patients treated with implant placement through osseodensification were evaluated. The alveolar ridge width was measured at the level of the crest and 10 mm apical to the crest before and after osseodensification. Insertion torque and implant stability quotient (ISQ) values were recorded at implant placements. Expansion values were grouped into the following 3 groups according to the initial alveolar ridge width: group 1: 3 to 4 mm (n = 9), group 2: 5 to 6 mm (n = 12), and group 3: 7 to 8 mm (n = 7). Twenty-one patients who received 28 implants were included. Twenty-six implants were integrated, resulting in a survival rate of 92.8%. There was a significant difference in the mean expansion value at the coronal aspect of the ridge between group 1, group 2, and group 3 (2.83 ± 0.66 mm, 1.5 ± 0.97 mm, 1.14 ± 0.89 mm, P < 0.05). The mean torque and ISQ values were 61.2 ± 13.9 Ncm and 77 ± 3.74. Osseodensification can alter ridge dimensions and allow for ridge expansion. Greater expansion can be expected at the crest in narrow ridges with adequate trabecular bone volume.

A Clinical Resonance Frequency Analysis of Implants Placed at Dehiscence-type Defects with Simultaneous Guided Bone Regeneration During Early Healing.

To investigate the implant stability quotient (ISQ) values of implants placed in bone with and without dehiscence bone defects over 12 weeks and to compare the ISQ values between the two groups. Twenty-two patients with edentulous area at posterior mandible were enrolled. Thirty OsseoSpeed EV Astra Tech implants (Dentsply Sirona), 4.2 mm in diameter, were placed. Twenty implants were placed without bone regeneration (control group), while 10 presented with favorable bone defects and received simultaneous guided bone regeneration with dental implant placement (study group). At the time of implant placement, 2, 4, 8, and 12 weeks, resonance frequency analysis (RFA) was utilized. The changes in ISQ values within group were analyzed with repeated-measures ANOVA, and the mean ISQ values between the control and study groups were compared using unpaired t tests. All implants were successfully integrated without complication. The control group demonstrated mean ISQ values of 74.30 ± 6.01, 69.58 ± 5.30, 71.10 ± 5.80, 75.08 ± 3.93, and 77.85 ± 3.18 at baseline, 2, 4, 8, and 12 weeks, respectively. The study group demonstrated mean ISQ values of 69.85 ± 7.00, 63.40 ± 8.47, 59.90 ± 10.23, 72.55 ± 3.10, and 76.20 ± 2.68 at baseline, 2, 4, 8, and 12 weeks, respectively. The study group showed significantly lower mean ISQ values at 2 weeks (P = .021) and at 4 weeks (P = .007) after implant placement compared to those of the control group. Within the 12-week healing period, all implants demonstrated successful osseointegration and achieved stability in favorable bone defects. Nevertheless, clinicians should consider that significantly lower implant stability can occur in the first month.

Evaluating cortico-cancellous ratio using virtual implant planning and its relation with immediate and long-term stability of a dental implant- A CBCT-assisted prospective observational clinical study

Primary and long-term implant stabilities are crucial in predicting the success of dental implants. We aimed to evaluate corticocancellous ratio (CCR) around virtual implant using cone beam computed tomography (CT) and assess its relationship with immediate and long-term stability of the implants placed. A total of 135 image records of posterior mandibular implant sites planned for dental implant were included in our study. CCR was calculated using CT images and implants were placed after stent preparation. Implant stability was calculated immediately, 4 months later, and 2 years later. Pearson's correlation test showed a significant correlation (P and lt; 0.001) between CCR and implant stability. ANOVA and post-hoc Tukey tests showed a significant difference in implant stability between groups with different CCRs at all follow-up timepoints. No significant difference was found between mean implant stability quotient values for low CCR at 2-year follow-up and high CCR immediately after implant placement. Implant stability is improved with greater CCR. Cortical bone seems to be crucial factor for immediate and long-term stability of a dental implant. Virtual planning using CT can assess implant stability. Further histological studies are required to confirm the relation between CCR and implant stability. The escalating demand of the implant treatment in the dental practice necessitates measuring the several predictors of procedure success. This study introduces a novel predictor (CCR) around virtual implant for detecting the immediate and long-term stability of a dental implant.

Impact of Hyperbaric Oxygen on the Healing of Bone Tissues Around Implants

The impact of hyperbaric oxygen (HBO) on the healing of bone tissues around implants was studied using an animal model. A total of 32 beagle dogs were selected and randomly divided into the HBO group and the blank group. The dogs in the HBO group were subjected to 90 minutes of HBO therapy. X-ray, cone-beam computerized tomography imaging, implant stability quotient (ISQ) values, histological observation, quantitative analysis of bone histomorphometry, and hematoxylin-eosin (HE) and Masson staining were evaluated. In this study, at 4 weeks after the surgery, the mean ISQ value in the HBO group was higher than that in the blank group, and the difference had statistical significance (P < 0.05). At week 4 and 8, the mean values of bone ingrowth fraction (BIC%) and the percentage of bone area (BA%) in the HBO group were both higher than those of the blank group. Decalcified paraffin sections were stained with HE and Masson staining showed that the bone tissue around the implant in the HBO group has more osteoblasts than control group, and many irregular marrow cavities and Haversian bone plates were observed in the new bone tissue. This study showed that after implantation, early osteogenesis was better in the HBO group than in the blank group. On one hand, the healing time of the bone tissue around the implants was reduced.

Clinical efficacy of the graft free slit-window sinus floor elevation procedure: A 2-year randomized controlled clinical trial.

Slit-window technique is a lateral approach sinus floor elevation (SFE) procedure that utilizes a rigid resorbable membrane in conjunction with stabilizing bony slits to tent the Schneiderian membrane without addition of grafting materials. The aim of this randomized clinical trial was to compare clinical and radiographic outcomes of the Slit-window SFE with those of SFE with bone grafting. Thirty-three patients with 38 edentulous sites were included. Edentulous sites were randomly allocated to the Slit-window SFE or SFE with bone grafting. Simultaneous with SFE, each site received two dental implants. Implant stability quotient (ISQ) values were recorded at baseline and monthly visits for 8months. The bone coverage and bone height gain at implant sites were recorded on cone beam computed tomography (CBCT) scans 6 and 24months postoperatively. There were no significant differences in the mean ISQ values between the groups at any time point (p>0.05). All implants in both groups were found to have complete bone coverage. Significant increases in the bone height were found 6 and 24months postoperatively in both groups (p<0.001). Within limitations of the present study, it can be concluded that the Slit-window SFE technique without the need for bone grafting resulted in sufficient bone height gain around implants placed simultaneous with the SFE procedures. The bone height around these implants was stable during the 2-year follow-up period. The treatment outcomes for these implants were comparable to those placed simultaneous with lateral approach SFE in conjunction with bone grafting.
