Association of serum homocysteine with vitamin B12 and folate levels in women with pre-eclampsia in a tertiary health care center in Nepal

BackgroundPre-eclampsia is a syndrome that chiefly includes the development of new-onset hypertension and proteinuria after 20 weeks of pregnancy. Pre-eclampsia is one of the major causes of mortality and morbidity in Nepal. Hyperhomocysteinemia may be a cause of the endothelial dysfunction provoked by oxidative stress in pre-eclampsia. This study was designed to evaluate the association of homocysteine with Vitamin B12 and folate in patients with pre-eclampsia.MethodAn observational cross sectional study was performed in the Gynecology and Obstetrics Department of TUTH involving seventy two subjects with pre-eclampsia. Blood pressure, urinary protein levels, serum homocysteine, Vitamin B12 and folate levels were compared in both mild and severe forms of pre-eclampsia. Concentration of Vitamin B12 and folate were measured using Vitros ECI and homocysteine was measured using CLIA. SPSS 23.0 was used to analyze the data. Tests were performed with Mann Whitney Test and Spearman’s rank correlation test. A p-value < 0.05 was considered statistically significant.ResultsThis study showed no significant difference in age and weeks of gestation in both mild and severe forms of pre-eclampsia. Mean concentration of homocysteine was higher (13.1 ± 6.4 micromol/L) in severe Pre-eclampsia as compared to mild cases (7.6 ± 2.8 micromol/L). Mean concentration of folate was lower in severe cases (35.4 ± 24.1 micromol/L) when compared with mild cases of pre-eclampsia (57 ± 23.4 micromol/L).ConclusionHomocysteine levels were increased in severe Pre-eclampsia when compared with mild pre-eclampsia and this finding can be used to predict and prevent complications in patients with pre-eclampsia.

Plasma Homocysteine, Folic Acid and Vitamin B12 in Abruptio Placentae: A Cross-Sectional Study of Their Role and Feto-Maternal Outcome.

Background Out of the many causes of abruptio placentae, the micronutrient association with its occurrence and severity has not been researched extensively till now. We aim to measure the serum levels of homocysteine, folic acid and vitamin B12 in patients with abruptio placentae in the third trimester of pregnancy and compare the levels with those without the complication. We also propose to compare the feto-maternal outcome between the groups. Methods The cross-sectional study was undertaken in 50 pregnant women with abruption before or during delivery and 50 controls with uncomplicated pregnancy over 28 weeks of gestation. Serum levels of homocysteine, folic acid, and vitamin B12 were determined andfeto-maternal outcome was compared between the groups Results Mean age of the cases and controls are 26.82 ± 5.5 and 28.82 ± 4.88 years respectively. Obstetric characteristics have significant difference between the groups in terms of gravidity, mode of delivery, timing of delivery, proportion of stillbirths and blood transfusion. The mean concentration of homocysteine and vitamin B12 between the groups also have a significant difference . The serum level of homocysteine is significantly correlated with serum vitamin B12 level (Pearson correlation= -0.601, P=0.000). However, folic acid concentration between the groups remains comparable. Conclusion Hence we conclude that vitamin B12 and homocysteine are significant determinants of abruptio placentae in pregnant women. Supplementation with the vitamin in the high-risk Indian population can avert a number of obstetric complications occurring due to raised homocysteine.

Increased Homocysteine Level and Risk of Gestational Hypertension in South East Algerian population

The objective of this study is to target the effect, in the South East Algerian population, of homocysteine level on gestational hypertension (GH) in wom-en. We also aim to study if gestational hypertension and preeclampsia (PE), two types of hypertensive disorders, share the same risk factors. The case-control study has been undertaken in Ouargla maternity hospital. Homocys-teine (Hcy) levels have been measured using an automated chemilumines-cence method. Creatinine and total protein have been determined using a colorimetric method, while an enzymatic technique has been employed to estimate the aspartate aminotransferase (AST) and the alanine aminotrans-ferase (ALT). The mean concentration of homocysteine was significantly higher for PE (14.90±8.54 μmol/l) and GH (11.10±5.13 μmol/l) compared to that of normotensive women (6.85±3.23 μmol/l) p≤ 0.01. Besides, hyperho-mocysteinemia (Hhcy) was detected with 70% of mothers with PE and with 40% of mothers with GH compared to only 11.53 % of controls. These values show the correlation between the rate of Hcy and hypertension during preg-nancy aetiology and severity. The other metabolites showed a significant increase in total protein level in PE and GH women compared to controls (p≤0.01). A significant increase in creatinine level is observed for patients with preeclampsia (p&lt;0.05). There were no differences among patients and controls concerning AST nor ALT. Results show that homocysteine is an im-portant risk factor as for gestational hypertension as for preeclampsia. Com-plications have similar biochemical profiles leading GH to be given as much attention as PE.

Relationship between serum homocysteine and different menopausal stage

Objective: This study aimed to investigate the serum level of homocysteine in menopausal women and to explore the association between menopausal status, blood lipids, and homocysteine concentrations.Methods: The study recruited females aged 40–60 years who were not receiving menopausal hormone therapy. The demographic characteristics and menopausal status of the women were collected in the form of questionnaires. The study analyzed the association between hyperhomocysteinemia and variables using binary logistic stepwise regression.Results: Among 366 enrolled subjects, menopausal status was divided into four stages: premenopause stage (n = 135), menopausal transition stage (n = 91), early postmenopause stage (n = 87), and late postmenopause stage (n = 53). The proportion of hyperhomocysteinemia in the premenopausal stage, menopausal transition stage, and postmenopausal stage was 43%, 26.4%, and 45%, respectively (χ2 = 8.999, p = 0.011). The mean concentration of homocysteine was 9.75 µmol/l. The level of homocysteine was higher in the postmenopause stage than in the other stages (p = 0.043), and the difference between postmenopause and menopausal transition was statistically significant (p < 0.01). In the binary logic analysis, menopausal transition compared with postmenopause (odds ratio = 2.027, 95% confidence interval = 1.117–3.679, p = 0.005).Conclusions: Serum homocysteine levels are associated with menopausal status. Homocysteine concentrations were progressively higher across menopausal stages. The transformation in the female body across the menopause transition stages may cause elevations in the homocysteine level in postmenopausal women.

The comparison of cardiovascular risk factors prevalence among Catholics and Seventh-day Adventists living in southern Poland

Abstract Introduction. A review literature concerning the religious affiliation and that of cardiovascular disease did not show any clear correlations between these variables. Aim. To determine selected cardiovascular risk factors and the risk of a cardiovascular event among Seventh-day Adventists (SDA) and Catholics. Material and Methods. A cross-sectional study was carried out in the years 2014-2015 among 252 people, including 118 Seventhday Adventists and 134 Catholics over 18 years of age, residents of southern Poland. The results of the following were analysed: anthropometric measurements, an interview questionnaire, physical examination and laboratory tests, as well as the SCORE scale. Results. The mean concentration of homocysteine and triglycerides in Catholics was significantly higher than in Adventists. Adventists had significantly higher blood pressure and mean HDL cholesterol concentration than Catholics. On the basis of BMI, overweight and obesity were ascertained in a somewhat greater percentage of Catholics than Adventists, and on the basis of waist circumference, android obesity was found to be more common in Catholics than in Adventists. Conclusions. Based on the SCORE scale, the risk of a cardiovascular event was significantly higher in Catholics than in Adventists.

Variability of total thiol compounds, oxidative and nitrosative stress in uncomplicated pregnant women and nonpregnant women

The objective of this study was to determine the changes in total plasma thiols (homocysteine, cysteine and cysteinylglycine), lipid peroxidation and nitric oxide concentrations during normal pregnancy. These variables were measured in 28 uncomplicated pregnant women at first, second and third trimesters and in 19 nonpregnant women. The mean concentrations of homocysteine, cysteine and cysteinylglycine were significantly lower in all trimesters of pregnancy compared with nonpregnant controls. There was significant elevation in serum lipid peroxidation levels of pregnant women within first and third trimesters compared with nonpregnant women. In spite of increase in mean nitric oxide levels in pregnant women, this increase did not reach statistically significant levels. This study provides information about the changes in plasma levels of many variables having important role in pregnancy complication during all trimesters in uncomplicated pregnancy compared with nonpregnant women.

Association between Helicobacter pylori-infection, C-reactive protein and status of B vitamins

Some investigations, but not all, found that a chronic infection with Helicobacter pylori (Hp) is associated with deficiencies in B vitamins, elevated plasma total homocysteine concentrations (tHcy) and increased plasma levels of proinflammatory acute-phase proteins. It has been suggested that these factors promote atherogenesis and therefore could mechanistically explain why people infected with Hp might have an increased risk for cardiovascular diseases (CVD). Therefore we evaluated the association between Hp-infection, concentration of C-reactive protein (CRP), and status of various B vitamins in apparently healthy subjects. In 69 subjects with proved Hp-infection and 21 healthy control subjects identified in a cross sectional study, blood samples were collected to determine serum folate, serum vitamin B12, serum methylmalonic acid (MMA), serum CRP and plasma vitamin B6 and plasma total homocysteine (tHcy). The mean concentration of CRP was significantly higher in the Hp-positive collective than in controls. Although mean concentrations of vitamin B12, B6, and MMA differed between the groups, statistical significance was missed. However, the mean concentrations of homocysteine and folic acid were nearly the same in both groups. In univariate analysis a significant impact of Hp-status was shown on cobalamin (p=0.028; eta square: 0.055), and in multivariate analysis of variance the Hp-status had an impact on vitamin B12-values (p=0.028; eta square 0.057). In this study Hp-infection shows no significant impact on status of B vitamins, but has a significant influence on CRP concentration. However, this study does not support the hypothesis that Hp-infection is related to CVD via elevated levels of tHcy.

Hyperhomocysteinemia in Patients with Arterial Occlusive Disease

Hyperhomocysteinemia (HHCA) is defined as an independent risk factor for atherothrombotic vascular disease; therefore, screening for HHCA is recommended. However, the incidence and characteristics of HHCA in patients with vascular disease remain unclear. We conducted this study in an attempt to resolve these issues. This nonrandomized prospective study included 56 patients who were admitted with occlusive arterial disease (group I), and 39 control patients without occlusive arterial disease (group II). We recorded all the demographic data of both groups and collected blood samples for fasting homocysteine, vitamin B(12), and folic acid. All of the patients were followed up and the results were compared. The mean concentration of homocysteine was 12.69 +/- 3.82 micromol/l in group I and 10.46 +/- 5.08 micromol/l in group II (P = 0.00048). In group I, the mean homocysteine levels for patients aged > or =70 years and those aged <70 years were 13.74 +/- 3.02 and 11.55 +/- 4.15 micromol/l, respectively (P = 0.021). There was no significant difference in mortality between the patients with HHCA and those with normal homocysteine levels during follow-up. The incidence of hyperhomocysteinemia was higher in the patients with occlusive vascular disease than in the control patients. More evidence of the association with vitamins B(12) and folate and the benefits of homocysteine-lowering therapy is needed since we found no relationship between these vitamins and homocysteine in this study.

Role of homocysteine in the development of the cerebral and cardiovascular pathology in young persons

Cerebral and cardiovascular pathology is the leading cause of the death and invalidity in industrially developed countries. During recent years new data allowed to increase significantly the presentations of the pathogenesis of these diseases. They revealed that hyperhomocysteinemia increases the risk of early development of atherosclerosis and thrombosis of the coronary, cerebral and peripheral arteries independently of the traditional risk factors and it is the prognostic marker of the lethal outcome. The objective of this study was to appreciate the role of homocysteine in the development of the cerebral and cardiovascular pathology (ischemic stroke, myocardial infarction) in young persons. We examined 28 patients during an acute period of the ischemic stroke (IS) and 10 patients with acute myocardial infarction (AMI) (mean age 45.1 ± 1.15 and 40.9 ± 2.34 years). Clinical evaluation of the neurological status of the patients with IS was conducted traditionally and according to the international clinical scales determining neurological deficiency (Bartela, NIH-NINDS, Orgogoso, Scandinavian, Index of the Disorders of Adoptive Activity) at patient's admission. The control group consisted of 15 healthy subjects (mean age 44.7 ± 1.02 years). The homocysteine concentration in the blood was analyzed in all fasting patients on the immunochemoluminescent analyzer 'IMMULITE One' (USA) using the assay kit of 'DPC' production (USA). We revealed the presence of the moderate hyperhomocysteinemia in the patients with IS and AMI. The mean concentration of the serum homocysteine exceeded the mean concentration of this metabolite in healthy subjects by 57% and 48%, respectively. The patients with IS had dependence of the clinical course of the disease on the homocysteinemia level. In mild (n = 7) and moderate (n = 9) neurological deficiency, the mean concentration of homocysteine was 8.1 ± 0.65 and 10.8 ± 1.44 μmol/l respectively. In expressed and gross (n = 11) neurological deficiency, the mean concentration of homocysteine was 16.0 ± 2.44 μmol/l, which exceeded the control value by 85%. In two patients with gross neurological deficiency and the highest values of homocysteine, the disease finished by fatal outcome. All patients with increased homocysteine concentration had atherosclerotic changes of the vessels that were uncharacteristic for their age. All patients had the atherothrombotic subtype of IS. The development of AMI and the severe degree of the atherothrombotic subtype of IS in young persons may thus be conditioned by increased homocysteine concentration provoking early formation of the atherosclerotic changes of the vessels and thrombosis. The obtained data may be useful in the composition of programs of the prophylactic measures.

Neural tube defects and elevated homocysteine levels in amniotic fluid

OBJECTIVE: Our purpose was to study maternal blood and amniotic fluid concentrations of homocysteine and relevant vitamins in relation to neural tube defects. STUDY DESIGN: Concentrations of total homocysteine, folate, and vitamins B 12 and B 6 were measured in maternal blood and amniotic fluid of 27 women carrying a fetus with a neural tube defect and 31 control women carrying a healthy fetus. RESULTS: The mean total homocysteine concentration in amniotic fluid of the study group was significantly higher than that of the control group. The mean concentrations of total homocysteine in blood and the vitamins folate, B 12, and B 6 in, respectively, blood and amniotic fluid were not significantly different between the grous. The mean concentrations of homocysteine and vitamin B 6 were significantly lower in amniotic fluid than in blood groups, whereas vitamin B 12 in amniotic fluid was higher than in blood. CONCLUSION: These results support the hypothesis that at least the cause of a subset of neural tube defects could reside in a primary or secondary maternal or fetal derangement of homocysteine metabolism.
