Abstract 10951: Clinical Outcome of Maze Procedure for Atrial Fibrillation Combined with Mitral Valve Disease and Tricuspid Regurgitation

Introduction: The surgical strategy for atrial fibrillation (AF) combined with mitral valve (MV) disease are still controversial whether biatrial or left atrial maze procedure should be performed concomitantly. Hypothesis: Our retrospective study aimed to evaluate the long-term results of two kinds of maze procedure combined with MV surgery and identify the factors involved in AF recurrence. Methods: We enrolled 88 patients from January 2010 to December 2020 who underwent MV surgery with maze procedure concomitantly. The patients were divided into two groups; biatrial maze, group F: n=42 and left atrial maze, group L: n=46). Primary endpoint was AF recurrence and we also evaluated early and late complication and preoperative and operative factors related to AF recurrence. The average follow-up term was 2.8 years. Results: Preoperative factors of long-standing AF and TR grade were significantly higher in group F than group L (P=0.004, 0.02, respecively). Consequently, tricuspid annuloplasty (TAP) was performed significantly frequent in group F than group L (P=0.002). We encountered 2 hospital deaths (4.4%) due to postoperative cerebral hemorrhage and AMI in group L. There was 1 case (2.2%) required pacemaker implantation (PMI) during hospitalization in group L and 3 cases (7.1%) requiring PMI in Group F in late-period. The AF recurrence rates at 1 and 5-year follow-ups between Group F and Group L were not significantly different (24.4% and 28.4% vs. 22.9% and 32.4%, P =0.79). However, the AF recurrence rate in group L with TAP is significant high than those of the other groups (group F 24.4% and 28.4%, group L with TAP 44.3% and 56.7%, and group L without TAP 7.7% and 14.3% at 1 and 5 years, respectively p =0.005). Subject with group L with TAP were associated with a high risk of AF recurrence, with an adjusted OR of 2.4 (95% CI: 1.31-4.45, P =0.005). Conclusions: Our study demonstrated that both concomitant maze procedure was effective for preventing AF recurrence. However, it is also revealed that left atrial maze with TAP significantly increased AF recurrence. It was suggested that preoperative right atrial load was involved in primary occurrence of AF and left atrial maze alone might not control AF recurrence in such situation.

Launching minimally invasive stand-alone maze procedure for atrial fibrillation

Minimally invasive Maze procedure via right mini-thoracotomy approach is reportedly a promising option for paroxysmal and non-paroxysmal atrial fibrillation (AF), although it is not widely performed. This study aimed to validate feasibility and safety of minimally invasive stand-alone Maze procedure in an institutional first case series. This study enrolled an institutional consecutive series of 20 cases who underwent minimally invasive Maze procedure between November 2018 and January 2021. Concomitant tricuspid annuloplasty was performed in five cases who showed moderate tricuspid regurgitation preoperatively. Minimally invasive Maze procedure using cryo-energy source was successfully accomplished with sinus rhythm being restored at the intensive care unit entry in all cases without conversion to the sternotomy approach. All cases were discharged home, while one case was complicated with stroke postoperatively. Nineteen cases (95%) showed sinus rhythm at the last follow-up, whereas one case, who had large left atrium, showed recurrent persistent AF despite optimum medical therapy. Catheter ablations were performed for residual conductions at box lesions and/or mitral/tricuspid isthmus in three cases, who showed medically refractory atrial tachycardia post-Maze procedure. Consequently, these three cases showed sinus rhythm restoration at the last follow-up Conclusions: Minimally invasive stand-alone Maze procedure with or without tricuspid annuloplasty was feasible and safe for AF in the institutional first case series. Catheter ablations for recurrent AF post-Maze procedure would be warranted.

The long-term safety and efficacy of concomitant Cox maze procedures for atrial fibrillation in patients without mitral valve disease

ObjectiveNewly published guidelines made the highest level recommendation for surgical treatment for atrial fibrillation. However, the number of patients without a mitral valve procedure with atrial fibrillation who are treated with concomitant surgical ablation is still low (15%-25%), because surgeons are reluctant to perform procedures in patients who would not otherwise require left atriotomy. The purpose of this study was to compare the outcomes of concomitant Cox maze with and without mitral valve procedures. MethodsPatients who underwent concomitant Cox maze procedures were prospectively followed since September 2005. Of the 711 patients, 238 did not receive mitral valve surgery. Propensity score matching was conducted to balance preoperative characteristics between patients with and without mitral valve procedures (164/group after matching). ResultsBefore matching, patients in the mitral valve group were younger (65 vs 67 years, P = .047) and had higher euroSCORE II (European System for Cardiac Operative Risk Evaluation; 3.2% vs 2.6%, P = .002), larger mean left atrial size (5.3 vs 4.8 cm, P < .001), and shorter median atrial fibrillation duration (19 vs 25 months, P = .064). Early outcomes were similar for the matched groups. Cumulative 5-year freedom from stroke did not differ between matched mitral valve and non–mitral valve groups (96.1% vs 96.6%, P = .667). At each time point, the proportion in sinus rhythm off antiarrhythmic medications was similar for the matched groups, including 5 years after surgery (68% vs 63%, P = .492). ConclusionsThe Cox maze procedure is safe and effective with comparable outcomes when performed concomitant to mitral valve or non–mitral valve surgery. Surgeons should base the decision to perform surgical ablation procedures on atrial fibrillation pathophysiology and the benefit to patients, not on the type of concomitant procedure.

Long-term outcome following concomitant mitral valve surgery and Cox maze procedure for atrial fibrillation

ObjectiveAtrial fibrillation (AF) is associated with increased early and long-term morbidity/mortality following valve surgery. This study examined long-term influence of concomitant full Cox maze (CM) and mitral valve procedures on freedom from atrial arrhythmia and stroke. MethodsThis sample comprised patients who underwent CM with a mitral valve procedure (N = 473). Data on rhythm, medication status, and clinical events captured according to Heart Rhythm Society guidelines at 6, 9, 12, 18, and 24 months and yearly thereafter up to 7 years. ResultsMean age was 65 years, mean left atrium size was 5.3 cm, and 15% had paroxysmal AF. Perioperative stroke occurred in 2 patients (0.4%) and operative mortality was 2.7% (n = 13). Return to sinus rhythm regardless of antiarrhythmic drugs at 1, 5, and 7 years was 90%, 80%, and 66%. Sinus rhythm off antiarrhythmic drugs at 1, 5, and 7 years was 83%, 69%, and 55%. Freedom from embolic stroke at 7 years was 96.6% (0.4 strokes per 100 patient-years) with a majority of patients off anticoagulation medication. Greater odds of atrial arrhythmia recurrence during 7 years was associated with longer AF duration (odds ratio [OR], 1.07; P = .001), whereas lower odds were associated with cryothermal energy only (OR, 0.64; P = .045) and greater surgeon experience (OR, 0.98; P = .025). ConclusionsThis study suggests that the addition of CM to mitral valve procedures, even with a high degree of complexity, did not increase operative risk. In long-term follow-up, the CM procedure demonstrated acceptable rhythm success, reduced AF burden, and remarkably low stroke rate. Individual surgeon experience and training may notably influence long-term surgical ablation for AF success.

A single center's experience with pacemaker implantation after the Cox maze procedure for atrial fibrillation

ObjectiveThe Cox maze procedure (CM) is safe and effective for all atrial fibrillation (AF) types. A recent randomized trial found alarming rates of pacemaker implantation (PMI) during hospitalization after CM. The purpose of this study was to assess the rate of PMI and its impact on outcomes after CM. MethodsIncidence of PMI was captured for all CM patients (2005-2015; N = 739). Data were collected prospectively. Multivariable logistic regression was conducted to determine risk factors for PMI. Propensity score matching was conducted between concomitant CM patients and patients without surgical ablation since 2011. ResultsFifty-two patients (7.0%) had in-hospital PMI after CM. Most common primary indication for PMI was sick sinus syndrome (67%), followed by complete heart block (23%) and sinus bradycardia (10%). The only risk factor for in-hospital PMI was type of procedure (P = .020). Patients with multiple valve procedures were at greatest risk (P = .004-.035). STS-defined perioperative outcomes were similar for patients with and without in-hospital PMI. Sinus rhythm off antiarrhythmic drugs were similar by PMI. After propensity score matching (n = 180 per group), in-hospital PMI was similar in CM patients and those without surgical ablation (5% vs 4%, P = .609). ConclusionsThis study demonstrated lower incidence of PMI after CM procedures than recently reported. When indicated, PMI was not associated with increased short- or long-term morbidity or inferior freedom from atrial arrhythmia. Efforts to increase surgeon training with the CM procedure and postoperative management awareness are warranted to improve rhythm outcome and minimize adverse events and PMI.

Anatomical Variations in the Sinoatrial Nodal Artery: A Meta-Analysis and Clinical Considerations.

Background and ObjectiveThe sinoatrial nodal artery (SANa) is a highly variable vessel which supplies blood to the sinoatrial node (SAN). Due to its variability and susceptibility to iatrogenic injury, our study aimed to assess the anatomy of the SANa and determine the prevalence of its anatomical variations.Study DesignAn extensive search of major electronic databases was performed to identify all articles reporting anatomical data on the SANa. No lower date limit or language restrictions were applied. Anatomical data regarding the artery were extracted and pooled into a meta-analysis.ResultsSixty-six studies (n = 21455 hearts) were included in the meta-analysis. The SANa usually arose as a single vessel with a pooled prevalence of 95.5% (95%CI:93.6–96.9). Duplication and triplication of the artery were also observed with pooled prevalence of 4.3% (95%CI:2.8–6.0) and 0.3% (95%CI:0–0.7), respectively. The most common origin of the SANa was from the right coronary artery (RCA), found in 68.0% (95%CI:55.6–68.9) of cases, followed by origin from the left circumflex artery, and origin from the left coronary artery with pooled prevalence of 22.1% (95%CI:15.0–26.2) and 2.7 (95%CI:0.7–5.2), respectively. A retrocaval course of the SANa was the most common course of the artery with a pooled prevalence of 47.1% (95%CI:36.0–55.5). The pooled prevalence of an S-shaped SANa was 7.6% (95%CI:2.9–14.1).ConclusionsThe SANa is most commonly reported as a single vessel, originating from the RCA, and taking a retrocaval course to reach the SAN. Knowledge of high risk anatomical variants of the SANa, such as an S-shaped artery, must be taken into account by surgeons to prevent iatrogenic injuries. Specifically, interventional or cardiosurgical procedures, such as the Cox maze procedure for atrial fibrillation, open heart surgeries through the right atrium or intraoperative cross-clamping or dissection procedures during mitral valve surgery using the septal approach can all potentiate the risk for injury in the setting of high-risk morphological variants of the SANa.

Modified Maze Procedure for Atrial Fibrillation as an Adjunct to Elective Cardiac Surgery: Predictors of Mid-Term Recurrence and Echocardiographic Follow-Up.

The radiofrequency maze procedure achieves sinus rhythm in 45%-95% of patients treated for atrial fibrillation. This retrospective study evaluates mid-term results of the radiofrequency maze-performed concomitant to elective cardiac surgery-to determine sinus-rhythm predictive factors, and describes the evolution of patients' echocardiographic variables. From 2003 through 2011, 247 patients (mean age, 64 ± 9.5 yr) with structural heart disease (79.3% mitral disease) and atrial fibrillation underwent a concomitant radiofrequency modified maze procedure. Patients were monitored by 24-hour Holter at 3, 6, 12, and 24 months, then annually. Eighty-four mitral-valve patients underwent regular echocardiographic follow-up. Univariate and multivariate analysis for risk factors of maze failure were identified. The in-hospital mortality rate was 1.2%. During a median follow-up of 39.4 months, the late mortality rate was 3.6%, and pacemaker insertion was necessary in 26 patients (9.4%). Sinus rhythm was present in 63% of patients at the latest follow-up. Predictive factors for atrial fibrillation recurrence were arrhythmia duration (hazard ratio [HR]=1.296, P=0.045) and atrial fibrillation at hospital discharge (HR=2.03, P=0.019). The monopolar device favored maze success (HR=0.191, P <0.0001). Left atrial area and indexed left ventricular end-diastolic volume showed significant decrease both in sinus rhythm and atrial fibrillation patients. Early sinus rhythm conversion was associated with improved left ventricular ejection fraction. Concomitant radiofrequency maze procedure provided remarkable outcomes. Shorter preoperative atrial fibrillation duration, monopolar device use, and prompt treatment of arrhythmia recurrences increase the midterm success rate. Early sinus rhythm restoration seems to result in better left ventricular ejection fraction recovery.

Anesthetic Considerations for a Patient With Hereditary Hemorrhagic Telangiectasia (Osler-Weber-Rendu Syndrome) Undergoing a Five-Box Thoracoscopic Maze Procedure for Atrial Fibrillation.

Hereditary hemorrhagic telangiectasia (HHT) is an autosomal dominant genetic disorder involving the abnormal communication of vascular structures. HHT typically presents with recurrent epistaxis and telangiectasis of the nasal and buccal mucosa, tongue, and lips. More serious manifestations of this disease include cerebral, pulmonary, gastrointestinal, and hepatic arteriovenous malformations. This case report details a 55-year-old male with HHT undergoing a five-box maze procedure for curative treatment of atrial fibrillation. Particular anesthetic considerations are described to reduce morbidity and mortality in this patient population.

Prediction of sinus rhythm in patients undergoing concomitant Cox maze procedure through a median sternotomy

One of the challenges that exists when discussing the Cox maze procedure for atrial fibrillation (AF) with patients is predicting the success for a given patient. The purpose of the present study was to develop a scoring system using well-established clinical factors to predict the probability of sinus rhythm (SR) after surgery. The data from patients 1 and 2 years postoperatively were analyzed using logistic regression to predict SR, including the most accepted variables associated with failure (age, left atrium size, AF duration, AF type). Regression models were applied using hypothetical patients to examine the predicted probability of SR. The predictors of 1-year SR were a shorter AF duration and greater surgeon experience performing surgical ablation. The predictors at 2 years were a shorter AF duration and smaller left atrium. The 1-year prediction model applied to hypothetical data found a 1-cm increase in left atrial size associated with a 0.4% reduction in SR probability, a 5-year increase in AF duration associated with a 0.8% reduction, and a reduction by 50 cases of surgeon experience associated with a 1.0% reduction. The 2-year model found a 1-cm increase in left atrial size associated with a 1.0% reduction in SR probability, a 5-year increase in AF duration associated with a 0.8% reduction, and a reduction by 50 cases of experience associated with a 0.2% reduction. Our findings are the first step in establishing a risk scoring system to better predict the outcomes after surgical ablation for AF and improve the ability to discuss the risk and benefits with patients.

Efficacy of the Maze Procedure for Atrial Fibrillation Associated with Atrial Septal Defect

BackgroundAtrial fibrillation (AF) is a common complication in elderly patients with atrial septal defect (ASD). The purpose of this study was to examine the efficacy of the maze procedure in these patients.Materials and MethodsBetween February 2000 and May 2011, 46 patients underwent the maze procedure as a concomitant operation with ASD closure. Three patients who underwent a right-sided maze were excluded, and one patient was lost to follow-up. The mean follow-up duration was 3.2±2.5 years. Electrocardiography was performed 1 month, 3 months, 6 months, and 1 year after surgery, and checked annually after that.ResultsAF persisted in 4 patients after surgery. One year after surgery, among 38 patients, 55.3% remained in sinus rhythm without antiarrhythmic drugs. However, when including the patients who took antiarrhythmic drugs, 92.1% were in sinus rhythm. Freedom from AF recurrence at 3 months, 6 months, 1 year, 2 years, 3 years, and 5 years after surgery were 97.4±2.6, 94.4±3.8, 91.2±4.9, 87.8±5.8, 79.5±7.6, and 68.2±12.4, respectively. There was no early mortality after operation.ConclusionConcomitant treatment with the maze procedure and ASD closure is safe and effective for restoring the sinus rhythm.

Transforming growth factor β1-mediated atrial fibrotic activity and the recovery of atrial mechanical contraction after surgical maze procedure

BackgroundThe maze procedure for atrial fibrillation (AF) is effective in restoring sinus rhythm; however, a significant proportion of patients failed to recover atrial mechanical contraction (AMC). We hypothesized that preoperative atrial fibrotic activity would be related to the AMC recovery. MethodsIn atrial tissues of 128 consecutive patients who underwent mitral valve and combined maze surgery using cryoablation for persistent AF, preoperative mRNA levels of various biomarkers were measured including transforming growth factor-β1 (TGF-β1), atrial natriuretic peptide, brain natriuretic peptide (BNP), C-reactive protein, connective tissue growth factor, matrix metalloproteinase, N-terminal prohormone BNP, and tissue inhibitor of metalloproteinase. Presence of AMC was assessed using Doppler echocardiographic measurement of the transmitral A-wave velocity. ResultsAt 1-year follow-up, patients without AMC (n=62) showed higher preoperative TGF-β1 mRNA expression (0.42 versus 0.28, P=0.01) than those with AMC (n=66). There was no significant difference between the two groups regarding the other biomarkers. Recovery rate of AMC and the increment of A-wave velocity were reduced with the increase of preoperative TGF-β1 mRNA level. Multiple logistic regression analysis revealed that TGF-β1 mRNA levels were independently associated with the absence of AMC (odds ratio 7.47, 95% CI 1.63 to 34.4, P=0.01). ConclusionsTGF-β1-mediated atrial fibrotic activity might exert a detrimental effect on the reversibility of atrial mechanical contraction after the maze and combined mitral valve surgery for long-standing persistent AF. Earlier surgical intervention might be needed to preserve atrial mechanical function.

The association between early atrial arrhythmia and long-term return to sinus rhythm for patients following the Cox maze procedure for atrial fibrillation†

Observational studies identified increased age, left atrial (LA) size, type and duration of atrial fibrillation (AF) as independent predictors for failure of AF surgical ablation. Rhythm at discharge following the Cox maze (CM) procedure for AF was never considered a significant predictor of success. The purposes of this study were to: (i) Determine the association of sinus rhythm (SR) at discharge and maintenance of SR. (ii) Identify the variables associated with discharge rhythm. (iii) Identify the effects of discharge rhythm on health-related quality of life (HRQL) post-CM procedure. A prospective study (n = 534) of CM III/IV patients. Rhythm captured during hospitalization, discharge, 3, 6, 12 and 24 months and verified by electrocardiogram and 24-h holter. Subsequent SR interventions were captured. Logistic regression identified predictors of discharge rhythm. HRQL (Short-Form 12; AF symptom frequency and Severity Checklist V3) obtained during follow-up. Eighty-eight percent were discharged in SR (n = 469). LA size (OR = 1.36, CI: 1.02-1.82, P = 0.035) and long-standing AF type (OR = 2.68, CI: 1.31-5.50, P = 0.007) were the only independent predictors of non-SR at discharge. Lower rates of SR at 2 years were found in patients discharged in non-SR (75 vs 91%, P = 0.01). Patients discharged in SR had fewer perioperative morbidities [prolonged ventilation >24 h (6 vs 14%, P = 0.03), renal failure requiring dialysis (0.9 vs 5%, 0.04) and pneumonia (2 vs 9%, P = 0.005)]. During follow-up (mean = 43 ± 27 months), patients discharged in SR had fewer cardioversions (15 vs 29%, P = 0.006), similar percutaneous catheter ablations (6 vs 5%, P = 1.00) and no difference in late embolic strokes (1.5 vs 1.1%, P = 0.54). In this large prospective cohort study, rhythm at discharge was found to be clinically significant with predicting SR at 24 months. Surgeons should be aware that the prognosis of non-SR patients at discharge remains excellent with high rates of SR at 24 months if managed appropriately using rhythm rather than rate control strategies.

Arrhythmia surgery for atrial fibrillation associated with atrial septal defect: Right-sided maze versus biatrial maze

Although it has been inferred that a biatrial maze procedure for atrial fibrillation in left-sided heart lesions may lead to better outcomes compared with a limited left atrial lesion set, it remains controversial whether the biatrial maze procedure is superior to the right atrial maze procedure in right-sided heart lesions. A retrospective review was performed for 56 adults who underwent surgical closure of atrial septal defect and various maze procedures for atrial fibrillation between June 1998 and February 2011. The median age at operation was 59 years (range, 34-79 years). Clinical manifestations of atrial fibrillation were paroxysmal in 8 patients, persistent in 15 patients, and long-standing persistent in 33 patients. A right atrial maze procedure was performed in 23 patients (group 1), and a biatrial maze procedure was performed in 33 patients (group 2). Treatment failure was defined as atrial fibrillation recurrence, development of atrial flutter or other types of atrial tachyarrhythmia, or implantation of a permanent pacemaker. The Cox proportional hazards model was used to identify risk factors for decreased time to treatment failure. During the median follow-up period of 49 months (range, 5-149 months), there was no early death and 1 late noncardiac death. On Cox survival model, group 1 showed a significantly decreased time to treatment failure in comparison with group 2 (hazard ratio, 5.11; 95% confidence interval, 1.59-16.44; P = .006). Maintenance of normal sinus rhythm without any episode of atrial fibrillation recurrence at 2 and 5 years postoperatively was 57% and 45% in group 1, respectively, and 82% and 69% in group 2, respectively. Left-sided ablation in addition to a right atrial maze procedure leads to better electrophysiologic outcome in atrial fibrillation associated with atrial septal defect.

Abstract 18398: Long-term Surgical Outcome of Mitral Valve Repair for Degenerative Mitral Regurgitation with Chronic Atrial Fibrillation: The Impact of Maze Procedure on Long-term Survival and Cardiac Function

Introduction: It is unclear whether maze procedure for atrial fibrillation (AF) would positively affect long-term survival, stroke and cardiac function. The aim of this study is to investigate the long-term surgical outcomes of mitral valve repair for degenerative mitral regurgitation with chronic AF and to determine the impact of concomitant maze procedures on long-term survival, cardiac events, stroke and cardiac function, comparing the outcomes of patients who underwent maze procedure and patients who did not undergo maze procedure. Methods: We retrospectively investigated 159 patients who underwent mitral valve repair for degenerative mitral regurgitation with chronic AF between 1991 and 2010. The mean age was 63.1±10.5 years old. We started Cox-maze IV procedure in 2002. As a result, 65 patients underwent concomitant maze procedure (maze(+) group) and 94 patients did not undergo maze procedure (maze(-) group). Follow-up was completed in 156 patients (98.1%). The median follow-up duration was 7.6 years (max 21 years). Results: There was one operative death (0.63%). The survival rate was significantly better in the maze(+) group than maze(-) group (5-year survival; 100% vs 85.2±4.0%, p&lt;0.001). The actual freedom from atrial fibrillation in the maze(+) group was 86.4±4.5% at 1 year and 81.1±5.6% at 5 years. The actual freedom from cardiac events (cardiac death, cardiac-related re-admission and re-operation) was not different between the maze(+) group and maze(-) group (5-year freedom rate; 81.6±5.9% vs 88.9±3.5%, p=0.113). The rate of freedom from stroke was better in the maze(+) group than maze (-) group (5-year freedom rate; 92.1±3.9% vs 82.6±4.2%), though the difference of them was not statistically significant (Log-rank p=0.081). The latest NYHA functional class was worse in the patients who did not covert to sinus rhythm (AF group) than who did (SR group)(1.4±0.5 vs 1.1±0.3, p&lt;0.001). The systolic and diastolic diameter of left ventricle gradually enlarged in AF group, which became significant difference during the long-term period. Conclusions: Maze procedure can improve long-term survival, rate of freedom from stroke and cardiac function in the patients with degenerative mitral regurgitation with chronic atrial fibrillation.

Results of clinical application of the modified maze procedure as concomitant surgery

Atrial fibrillation is the most common cardiac arrhythmia and is associated with significant morbidity and mortality. The classic cut-and-sew maze procedure is successful in 85-95% of patients. However, the technical complexity has prompted modifications of the maze procedure. The objective of this study was to retrospectively evaluate the clinical safety and efficacy of the maze treatment performed at our institution. From March 2001 until February 2009, 169 patients underwent a modified maze procedure for atrial fibrillation at the Erasmus MC, Rotterdam. Patient characteristics, surgical procedure and follow-up data were obtained by reviewing the medical charts and consulting with the referring physicians. The efficacy of the procedure as measured by AF recurrence was analysed with a repeated measurements model. The quality of life of the patients was assessed with the SF-36 (a short-form health survey with 36 questions) questionnaire and compared with that of the general Dutch population. Of the 169 patients who underwent a modified maze procedure, 163 had their maze procedure as a concomitant procedure. The 30-day mortality rate was 4.7% (n = 8). The rate of post-procedural AF recurrence varied significantly over time (P < 0.0001). Decreased left ventricular function, increased age and higher preoperative creatinine levels were predictors of AF recurrence. Quality of life, as measured with the SF-36 questionnaire, was comparable with that of the Dutch population for all health domains. Concomitant maze is a relatively safe treatment that eliminates atrial fibrillation in the majority of patients, although the probability of recurrent AF increases with the passage of time. Decreased left ventricular function, increased age and higher preoperative creatinine levels are associated with an increased risk of AF recurrence.

Left atrial and left ventricular diastolic function after the maze procedure for atrial fibrillation in mitral valve disease: degenerative versus rheumatic.

The present study was aimed to compare the left atrial and left ventricular diastolic functions amongst the rheumatic and degenerative mitral valve disease patients in atrial fibrillation who reverted to normal sinus rhythm following Cox-maze procedure. We prospectively investigated the left atrial and left ventricular function with Doppler echocardiography, by dividing into the rheumatic (N = 105) and the degenerative group (N = 47). Over the follow-up period (mean: 4.4 ± 1.2years in the rheumatic group, 4.8 ± 1.3years in the degenerative group), the rheumatic group showed statistically significant decrease in A' velocity and E' velocity, on contrary to degenerative group (A' velocity: mean decrease of 0.43 ± 0.13cm/s in the rheumatic group, mean increase of 0.57 ± 0.11cm/s in the degenerative group, p = 0.029, E' velocity: mean decrease of 0.23 ± 0.17cm/s in the rheumatic group, mean increase of 0.21 ± 0.15cm/s in the degenerative group, p = 0.031). In addition, the rheumatic group showed statistically significant increase in E/E' ratio than the degenerative group (mean increase of 4.49 ± 1.98 in the rheumatic group, mean increase of 1.74 ± 1.52 in the degenerative group, p = 0.047). Despite successful sinus rhythm restoration, the progressive loss of LA function as well as LV diastolic function is more prominent in the rheumatic group than the degenerative group. Therefore, differentiated strategies for postoperative surveillance are needed according to the pathology of mitral valve disease.

Breast Implants and Minimally Invasive Cardiac Surgery

The number of women affected by valvular heart disease and the number of women with breast implants are both on the rise. Minimally invasive heart surgery using a limited thoracotomy offers many potential benefits including reduction in blood loss, shorter hospital stay, faster recovery time, decreased pain, and improved cosmesis. Minimally invasive heart surgery often requires access to the second, third, or fourth intercostal space of the anterior chest wall. The presence of a breast implant may interfere with the surgeon's ability to gain adequate exposure for entry to the appropriate intercostal space. We present a case series of 5 women with breast implants who successfully underwent minimally invasive cardiac valve surgery. A retrospective review was conducted of all patients with breast implants who underwent minimally invasive cardiac valve surgery at the University of Southern California University Hospital. In each patient, an inframammary incision was performed, facilitating removal of the implant, performance of the cardiac operation, and reimplantation of the implant. Five women with breast implants who underwent minimally invasive cardiac valve surgery were identified; of these, 4 (80%) patients underwent repair of the mitral valve for mitral regurgitation, whereas 1 (20%) underwent an aortic valve replacement for aortic stenosis. Two patients underwent a concomitant maze procedure for atrial fibrillation during the same operation. The median follow-up time was 7.4 months, and the follow-up period ranged from 2 to 12 months. There were no significant postoperative complications such as infection, hematoma, or need for reoperation. Our series of 5 patients demonstrates that minimally invasive heart surgery performed through an inframammary incision can be safely performed in those with breast implants.

Plasma C-Reactive Protein Is Not Related to Sinus Non-Conversion by Maze Procedure Adjunct to Mitral Valve Surgery

In Framingham cohort study, C-reactive protein was not associated with incident atrial fibrillation (AF) after adjustment for left atrial size. This study examined whether levels of plasma inflammatory markers would be significant risk factors for failed maze procedure for AF. This study enrolled 88 patients with mitral valve disease undergoing valve surgery (n=32, sinus control group) or concomitant maze procedure for persistent atrial fibrillation (AF) (n=56, AF group). The mean follow-up in the AF group was 55.0 ± 17.5 months. The AF and sinus control groups did not differ in preoperative levels of C-reactive protein (p=0.636). In the AF group receiving maze procedure, the sinus conversion (n=37) and non-conversion (n=19) groups did not significantly differ in preoperative levels of interleukin-6 (p=0.607) and tumor necrosis factor-α (p=0.379). In multivariate analysis after adjustment for preoperative plasma inflammatory markers, independent factors associated with sinus conversion were AF duration (p=0.003), and left atrial area (p=0.014). In conclusion, plasma inflammatory markers are not associated with sinus non-conversion by radiofrequency maze procedure.
