Early Predictors of Recovery From Nonoperatively Treated Achilles Tendon Rupture: 1 Year Follow-Up Study.

To investigate early structural and mechanical predictors of plantarflexor muscle strength and the magnitude of Achilles tendon (AT) nonuniform displacement at 6 and 12 months after AT rupture. Thirty-five participants (28 males and 7 females; mean ± SD age 41.7 ± 11.1 years) were assessed for isometric plantarflexion maximal voluntary contraction (MVC) and AT nonuniformity at 6 and 12 months after rupture. Structural and mechanical AT and plantarflexor muscle properties were measured at 2 months. Limb asymmetry index (LSI) was calculated for all variables. Multiple linear regression was used with the 6 and 12 month MVC LSI and 12 month AT nonuniformity LSI as dependent variables and AT and plantarflexor muscle properties at 2 months as independent variables. The level of pre- and post-injury sports participation was inquired using Tegner score at 2 and 12 months (scale 0-10, 10 = best possible score). Subjective perception of recovery was assessed with Achilles tendon total rupture score (ATRS) at 12 months (scale 0-100, 100=best possible score). Achilles tendon resting angle (ATRA) symmetry at 2 months predicted MVC symmetry at 6 and 12 months after rupture (β = 2.530, 95% CI 1.041-4.018, adjusted R2 = 0.416, p = 0.002; β = 1.659, 95% CI 0.330-2.988, adjusted R2 = 0.418, p = 0.016, respectively). At 12 months, participants had recovered their pre-injury level of sports participation (Tegner 6 ± 2 points). The median (IQR) ATRS score was 92 (7) points at 12 months. Greater asymmetry of ATRA in the early recovery phase may be a predictor of plantarflexor muscle strength deficits up to 1 year after rupture. Calcaneal tendon stiffness is not associated with dynamic time-dependent contractile output.

The ability to rapidly generate muscular torque and velocity is important in specialized activities and daily tasks of living. Tendon stiffness is one factor in the neuromuscular system that influences musculoskeletal torque transmission. Previous studies have reported weak-to-moderate correlations between tendon stiffness and rate of torque development (RTD). However, these correlations have been reported only for isometric contractions, which may not be relevant to contractions involving joint rotation (i.e., dynamic). The purpose was to investigate the effect of calcaneal tendon stiffness on the dynamic RTD and rate of velocity development (RVD) in plantar flexor muscles. Young adult males (n=13) and females (n=2) performed prone isometric- and isotonic-mode maximal voluntary plantar flexion contractions (MVC). Ultrasound imaging was used to quantify tendon morphological characteristics to estimate Young's elastic modulus (YM). Maximal voluntary and electrically evoked (300 Hz) isometric- and isotonic-mode (at 10% and 40% MVC loads) contractions were evaluated for RTD and RVD through a 25° ankle joint range of motion. YM was correlated with isometric RTD, but only for evoked contractions (RTD0-50 ms: r=0.54, p=0.02, RTD0-200 ms: r=0.62, p=0.01). Conversely, YM was not correlated with dynamic RTD (voluntary: r = -0.07-0.41, p=0.06-0.40, evoked: r = -0.2-0.3, p=0.14-0.24) nor RVD (voluntary: r = -0.08-0.24, p=0.27-0.40, evoked: r=0.12-0.3, p=0.14-0.34). These correlations would indicate that calcaneal tendon stiffness is an important factor for rapid isometric torque development, but less so for isotonic contractions. The determinants of dynamic contractile rates are more complex and warrant further study.

Antagonist muscle torque at the ankle interfere with maximal voluntary contraction under isometric and anisometric conditions

While resultant maximal voluntary contraction (MVC) is commonly used to assess muscular performance, the simultaneous activation of antagonist muscles may dramatically underestimate the strength of the agonist muscles. Although quantification of antagonist torque has been performed in isometric conditions, it has yet to be determined in anisometric conditions. The aim of the study was to compare the mechanical impact of antagonist torque between eccentric, isometric and concentric contractions in PF and DF MVCs. The MVCs in dorsiflexion (DF) and plantar-flexion (PF) were measured in isometric, concentric and eccentric conditions (10° s-1) in nine healthy men (26.1 ± 2.7 years; 1.78 ± 0.05 m; 73.4 ± 6.5 kg) through two sessions. Electromyographic (EMG) activities from the soleus, gastrocnemius medialis and lateralis, and tibialis anterior muscles were simultaneously recorded. The EMG biofeedback method was used to quantify antagonist torque. Resultant torque significantly underestimated agonist torque in DF MVC (30–65%) and to a lesser extent in PF MVC (3%). Triceps surae antagonist torque was significantly modified with muscle contraction type, showing higher antagonist torque in isometric (29 Nm) than in eccentric (23 Nm, p < 0.001) and concentric (14 Nm, p < 0.001) conditions and resulting in modification of the DF MVC torque-velocity shape. Estimation of the antagonist torque in isometric or anisometric conditions provides new relevant insights to improve neuromuscular performance assessment and to better design strength training and rehabilitation programs related to the torque applied by agonist and antagonist muscles.

Contribution of Achilles tendon mechanical properties to torque steadiness in persons with transfemoral amputation.

How Achilles tendon mechanics and plantar flexion strength and torque steadiness are altered in the intact leg of persons with trauma-related amputation is unknown. Understanding Achilles tendon mechanics following amputation will further inform rehabilitation approaches to enhance posture, balance, and force control. Conduct a pilot study to quantify plantar flexion maximal voluntary contraction torque, torque steadiness, and Achilles tendon mechanics in persons with unilateral trauma-related transfemoral amputation and controls without amputation. Cross-sectional study. Isometric plantar flexion maximal voluntary contractions were performed with the intact leg of ten males with transfemoral amputation (48 ± 14 years) and the dominant leg of age-matched male controls without amputation. Torque steadiness was calculated as the coefficient of variation in torque over 6 s during submaximal tracking tasks (5%, 10%, 25%, 50%, and 75% maximal voluntary contraction). Achilles tendon elongation and cross-sectional area were recorded with ultrasound to calculate strain, stress, and stiffness. Maximal voluntary contraction and torque steadiness did not differ between persons with amputation (90.6 ± 31.6 N m, 3.7 ± 2.0%) and controls (95.8 ± 26.8 N m, 2.9 ± 1.2%; p > 0.05). Tendon stiffness (21.1 ± 18.2 N/mm) and strain (5.2 ± 1.3%) did not differ between groups (p > 0.05). Tendon cross-sectional area was 10% greater in persons with amputation leading to 29% lower stress (p = 0.021). Maximal voluntary contraction was a predictor of a lower coefficient of variation in torque (R2 = 0.11, p < 0.05). Persons with trauma-related transfemoral amputation do not differ in plantar flexion maximal voluntary contraction and torque steadiness of the intact leg compared with controls without amputation. Larger tendon cross-sectional area reduces stress and enables distribution of force across a greater area.

Importance of Maximal Strength and Muscle-Tendon Mechanics for Improving Force Steadiness in Persons with Parkinson's Disease.

Although plantar flexion force steadiness (FS) is reduced in persons with Parkinson’s disease (PD), the underlying causes are unknown. The aim of this exploratory design study was to ascertain the influence of maximal voluntary contraction (MVC) force and gastrocnemius-Achilles muscle-tendon unit behaviour on FS in persons with PD. Nine persons with PD and nine age- and sex-matched non-PD controls (~70 years, 6 females per group) performed plantar flexion MVCs and sub-maximal tracking tasks at 5, 10, 25, 50 and 75% MVC. Achilles tendon elongation and medial gastrocnemius fascicle lengths were recorded via ultrasound during contraction. FS was quantified using the coefficient of variation (CV) of force. Contributions of MVC and tendon mechanics to FS were determined using multiple regression analyses. Persons with PD were 35% weaker during MVC (p = 0.04) and had 97% greater CV (p = 0.01) with 47% less fascicle shortening (p = 0.004) and 38% less tendon elongation (p = 0.002) than controls. Reduced strength was a direct contributor to lower FS in PD (ß = 0.631), and an indirect factor through limiting optimal muscle-tendon unit interaction. Interestingly, our findings indicate an uncoupling between fascicle shortening and tendon elongation in persons with PD. To better understand limitations in FS and muscle-tendon unit behavior, it is imperative to identify the origins of MVC decrements in persons with PD.

Effects of acute sleep deprivation on H reflex and V wave.

The impact of sleep deprivation on muscular strength and power remains poorly understood. We aimed to determine the acute effects of 24hr of sleep deprivation on H-reflex and V-wave excitability. Fourteen healthy young adults (eight men, six women) were included. Participants visited the laboratory on two different occasions, without and with 24hr of sleep deprivation. In each session, participants were tested for maximal voluntary contraction (MVC) of the plantar flexors and dorsiflexors, soleus H- and M-recruitment curves, and evoked V wave, as well as tibialis anterior/soleus electromyographic co-activation. Twenty-four hours of sleep deprivation did not affect either plantarflexion MVC or soleus electromyographic normalized amplitude (p>.05). Moreover, H-reflex and V-wave peak-to-peak normalized amplitude did not change with sleep deprivation (p>.05). Conversely, we obtained a significant increase in antagonist/agonist level of co-activation during MVC post-sleep deprivation (6.2±5.2%, p<.01). In conclusion, we found that H-reflex and V-wave responses are well preserved after 24hr of sleep deprivation, revealing that descending neural drive and/or modulation in Ia afferent input remains largely unaffected under these circumstances. Yet, sleep deprivation affects motor control by exacerbating the magnitude of antagonist/agonist co-activation during forceful muscle contractions and this is novel.

Effect of ankle joint position on triceps surae motor unit firing rates

The triceps surae is an interesting model to study the effect of muscle length changes on motor unit properties for two reasons. First, the gastrocnemii components show lower motor unit discharge rates (MUDRs) when shortened by knee joint flexion as compared to an extended knee joint, whereas all other muscles studied display either higher or similar MUDRs in a shortened compared to a lengthened position. Second, the soleus receives strong afferent feedback due to its predominantly type I muscle fiber composition indicating that muscle length changes may have a great effect on its motor unit properties. The limited previous studies have not explored how changes in ankle joint position affect the triceps surae muscles. Thus, the purpose of this study was to compare MUDRs of the bi‐articular medial gastrocnemius (MG) and mono‐articular soleus at two ankle joint positions, 30° dorsiflexion and 20° plantar flexion, during the common task of plantar flexion. Four young, healthy men (~24 years old) were seated upright in a leg dynamometer with both the hip and knee joint angles at 90°. Subjects performed two ~3s isometric plantar flexion maximal voluntary contractions (MVC) at each ankle joint position. Tungsten electrodes were then used to record MUDRs from the MG and soleus during separate steady state 3–10s isometric plantar flexion contractions at 25, 50, 75, and 100% MVC at one ankle joint position. After a series of contractions, the ankle joint angle was changed and the subjects performed another series of plantar flexion contractions at each torque level. Initial ankle joint position and the order of contractions performed were randomized prior to each testing session. Subjects visited the lab multiple times to repeat the intervention in order to sample from many motor units. The mean MVC torque was 53% lower in the shortened (20° plantar flexion; 105Nm) compared to the lengthened (30° dorsiflexion; 223Nm) position. A total of 487 motor units from the MG (n=228) and soleus (n=259) were identified. Mean MG MUDRs at 25, 50, 75 and 100% MVC were lower by 8, 0, 17 and 14%, respectively in the dorsiflexed compared to the plantar flexed position. Similarly, mean soleus MUDRs were lower by 8, 12, 25 and 25% at 25, 50, 75 and 100% MVC, respectively in the dorsiflexed compared to the plantar flexed position. These preliminary findings indicate that shortening the MG and soleus by placing the ankle joint in a plantar flexed position leads to higher MUDRs as compared to a dorsiflexed position. This may serve as a compensatory mechanism for the decreased torque producing capability of the triceps surae in a shortened (plantar flexed) position.Support or Funding InformationFunded by NSERC.

Velocity of the muscle tendon unit is sex-dependent and not altered with acute static stretch.

Contraction velocity of a muscle tendon unit (MTU) is dependent upon the interrelationship between fascicles shortening and the tendon lengthening. Altering the mechanical properties of these tissues through a perturbation such as static stretching slows force generation. Females, who have inherently greater compliance compared with males, have slower velocity of MTU components. The addition of a static stretch might further exacerbate this sex difference. The purpose of this study was to investigate the velocity of fascicle shortening and tendon lengthening in males and females during isometric maximal voluntary contraction (MVC) of the plantar flexors prior to and following an acute static stretch. The MTU was imaged with ultrasound and voluntary activation tested with twitch interpolation for the 5-s plantar flexion MVC, which proceeded and followed an acute stretch. For the 3-min stretch the ankle was passively rotated to maximal dorsi-flexion. The males were stronger (128.71 ± 7.88 Nm) than the females (89.92 ± 4.70 Nm) but voluntary activation did not differ. Tendon lengthening velocity (p = 0.001) and fascicle shortening velocity (p = 0.01) were faster in males than females. Tendon velocity was positively and significantly correlated with fascicle velocity, (r2 = 0.307, p = 0.02). Although sex was significant as a predictor (p = 0.05) time was not independently significant. Thus, stretch did not alter this relationship in either sex (p = 0.6). The velocity of the individual components of the MTU is slower in females when compared with males; however, acute stretch does not alter the relationship between these components in males or females.

Triceps surae elasticity modulus measured by shear wave elastography is not correlated to the plantar flexion torque.

Supersonic Shear Imaging (SSI) is a technique which analyses quantitatively the tissue properties in real time. The relation between joint torque and Young's modulus (E) of the agonist muscles is important for obtaining stratification values and ranges of normality. The aim of this study was to evaluate the intra and intersessions reliability of the E values of the Achilles tendon and medial gastrocnemius muscle, bilaterally, during rest, and correlate them with the isometric plantarflexion peak torque. Shear modulus maps were acquired bilaterally in Achilles tendon (AT) and medial gastrocnemius (MG) muscle of 24 healthy male volunteers. Two 5-second plantarflexion maximal voluntary contractions were performed with a 40-seconds interval and correlated with E values. a good intrasession reliability (intraclass correlation coefficient- ICC= 0.821-0.986) and a weak Pearson's correlation was found between E values and peak torque (r= 0.022 to -0.202) for both limbs (P > .05). E values cannot be predictive of the triceps surae force production in untrained men. It could be helpful, otherwise, to monitor a chronic strength adaptation after an exercise intervention or rehabilitation program. IIb, individual cohort study.

Differential impact of visual feedback on plantar- and dorsi-flexion maximal torque output.

The effect of visual feedback on enhancing isometric maximal voluntary contractions (MVC) was evaluated. Twelve adults performed plantar-flexion and dorsi-flexion MVCs in 3 conditions (no visual feedback, visual feedback, and visual feedback with target). There was no significant effect of visual conditions on dorsi-flexion MVC but there was an effect on plantar-flexion. Irrespective of whether a target was evident, visual feedback increased plantar-flexion MVC by ∼15%. This study highlights the importance of optimal feedback to enhance MVC.

Homogeneity of fascicle architecture following repeated contractions in the human gastrocnemius medialis

This investigation sought to determine the effects of fatigue on fascicle architecture across the length of the human gastrocnemius medialis (GM). With institutional ethical approval, fifteen healthy males performed repeated isometric plantar flexion maximal voluntary contractions (MVC) until peak force fell 30% below baseline. Brightness-mode ultrasound was used to determine fascicle length and pennation angle at rest and during MVC prior to and following the fatiguing contractions. The results show a significant increase in fascicle length during MVC in the distal (2.8mm, 8.1%) middle, (4.9mm, 14.1%), and proximal (5.2mm, 14.7%) regions post-fatigue compared to pre-fatigue (p<0.05). There was also a significant decrease of pennation angle during MVC in the distal (3.3°, 8.8%), middle (3.9°, 9.4%), and proximal (2.9°, 6.9%) regions post-fatigue compared to pre-fatigue (p<0.05). These changes, however, were not region specific. These are the first results to show that fascicle shortening within the GM remains homogeneous following fatigue, suggesting that the fascicles were fatigued in a similar pattern throughout the muscle. The significant reduction of fascicle shortening may reflect an additional strategy to maintain an optimal force output in fatigued conditions, although future work is needed to confirm this notion.

Spatial variability of muscle activity during human walking: The effects of different EMG normalization approaches

Human leg muscles are often activated inhomogeneously, e.g. in standing. This may also occur in complex tasks like walking. Thus, bipolar surface electromyography (sEMG) may not accurately represent whole muscle activity. This study used 64-electrode high-density sEMG (HD-sEMG) to examine spatial variability of lateral gastrocnemius (LG) muscle activity during the stance phase of walking, maximal voluntary contractions (MVCs) and maximal M-waves, and determined the effects of different normalization approaches on spatial and inter-participant variability. Plantar flexion MVC, maximal electrically elicited M-waves and walking at self-selected speed were recorded in eight healthy males aged 24–34. sEMG signals were assessed in four ways: unnormalized, and normalized to MVC, M-wave or peak sEMG during the stance phase of walking. During walking, LG activity varied spatially, and was largest in the distal and lateral regions. Spatial variability fluctuated throughout the stance phase. Normalizing walking EMG signals to the peak value during stance reduced spatial variability within LG on average by 70%, and inter-participant variability by 67%. Normalizing to MVC reduced spatial variability by 17% but increased inter-participant variability by 230%. Normalizing to M-wave produced the greatest spatial variability (45% greater than unnormalized EMG) and increased inter-participant variability by 70%. Unnormalized bipolar LG sEMG may provide misleading results about representative muscle activity in walking due to spatial variability. For the peak value and MVC approaches, different electrode locations likely have minor effects on normalized results, whereas electrode location should be carefully considered when normalizing walking sEMG data to maximal M-waves.

Effects of plyometric and pneumatic explosive strength training on neuromuscular function and dynamic balance control in 60–70 year old males

The present study compared neuromuscular adaptations to 12weeks of plyometric (PLY) or pneumatic (PNE) power training and their effects on dynamic balance control. Twenty-two older adults aged 60–70 (PLY n=9, PNE n=11) participated in the study. Measurements were conducted at Pre, 4, 8 and 12weeks. Dynamic balance was assessed as anterior–posterior center of pressure (COP) displacement in response to sudden perturbations. Explosive isometric knee extension and plantar flexion maximal voluntary contractions (MVCs) were performed. Maximal drop jump performance from optimal dropping height was measured in a sledge ergometer. Increases in knee extensor and ankle plantar flexor torque and muscle activity were higher and occurred sooner in PNE, whereas in drop jumping, PLY showed a clearer increase in optimal drop height (24%, p<0.01) after 8weeks of training and soleus muscle activity after 12weeks of training. In spite of these training mode specific adaptations, both groups showed similar improvements in dynamic balance control after 4weeks of training (PLY 38%, p<0.001; PNE 31%, p<0.001) and no change thereafter. These results show that although power and plyometric training may involve different neural adaptation mechanisms, both training modes can produce similar improvements in dynamic balance control in older individuals. As COP displacement was negatively correlated with rapid knee extension torque in both groups (PLY r=−0.775, p<0.05; PNE r=−0.734, p<0.05) after training, the results also highlight the importance of targeting rapid force production when training older adults to improve dynamic balance.

Cortical and segmental excitability during fatiguing contractions of the soleus muscle in humans

Objective The aim of this study was to examine the cortical and segmental excitability changes during fatigue of the soleus muscle. Methods Ten healthy young subjects performed 45 plantar flexion maximal voluntary contractions (MVCs) (7-s on/3-s off) in 9 epochs of five contractions. Motor evoked potentials (MEPs) using transcranial magnetic stimulation and H-reflexes were assessed during the task. Results The torque and the soleus EMG activity both showed the greatest decline during the 1st epoch, followed by a gradual, but significant decrease by the end of the task (∼70% pre-fatigue). The H-reflex sampled at rest after each epoch decreased to 66.6 ± 18.3% pre-fatigue after the first epoch, and then showed no further change. The MEP on 10% pre-fatigue MVC after each epoch increased progressively (252.9 ± 124.2% pre-fatigue). There was no change in the MEPs on the 3rd MVC in each epoch. The silent period on the MVC increased (109.0 ± 9.2% pre-fatigue) early with no further changes during the task. Conclusions These findings support that the motor cortex increases excitability during fatigue, but with a concomitant inhibition. Significance These findings are in contrast to upper extremity muscles and may reflect a distinct response specific to postural, fatigue-resistant muscle.

Gender differences in the passive stiffness of the human gastrocnemius muscle during stretch

The aim of this study was to determine whether muscle stiffness measured in vivo was different between males and females. Distal displacement of the gastrocnemius medialis myotendinous junction was measured directly using ultrasonography during passive dorsiflexion in eight males and eight females (age range 19-28 years). Plantarflexion torque and myotendinous junction displacement were measured at 5° intervals, where 0° was with the foot at right angles to the tibia. Stiffness of the gastrocnemius medialis muscle was calculated between 0° and 25° of dorsiflexion, and defined as passive plantarflexion torque/distal displacement of the myotendinous junction (N m cm(-1)). Relative muscle stiffness was also calculated as distal displacement relative to resting muscle length, and as passive torque relative to plantarflexion maximal voluntary contraction torque. No significant gender difference was observed in passive dorsiflexion torque, or in passive torque/maximal voluntary torque throughout the range of motion. Distal displacement of the gastrocnemius myotendinous junction was 26% more in females than in males (P < 0.05). Myotendinous junction displacement was 5.0 ± 1.4% of resting gastrocnemius medialis length in females, and 3.9 ± 0.6% in males. Over 25° of passive dorsiflexion, gastrocnemius medialis muscle stiffness was greater in males than in females by 44% (P < 0.05). In conclusion, based on the in vivo assessment of myotendinous junction displacement, passive gastrocnemius medialis muscle stiffness is greater in males than in females.

Early structural adaptations to unloading in the human calf muscles

The present study investigated the influence of muscle architectural changes on muscle torque during 3-week unilateral lower limb suspension (ULLS). Plantarflexion maximal voluntary contraction (MVC), soleus (SOL), gastrocnemius medialis (GM) and lateralis (GL) muscle volume (VOL), GL fascicle length (L(f)) and pennation angle (theta), physiological cross-sectional area (PCSA), and electromyographic (EMG) activity were assessed in eight healthy men (aged 19 +/- 0 years) after days 14 and 23 of ULLS. After 14 day of ULLS, MVC and SOL EMG decreased (P < 0.05) by 10% and 29%, respectively, but did not further decline between days 14 and 23. SOL, GM and GL muscle VOL decreased by 5%, 6% and 5%, respectively (P < 0.05), on day 14, and by 7% (SOL), 10% (GM) and 6% (GL) on day 23. In GL, theta and L(f) were reduced by 3% (P < 0.05) and 2% (NS), respectively, on day 14, and by 5% (P < 0.05) and 4% (P < 0.05), respectively, on day 23. Consequently, GL PCSA declined by 3% (P < 0.05) on day 14, but did not further decrease on day 23. Similarly, the 7% (P < 0.05) loss in GL force/PCSA observed on day 14 persisted until the end of the unloading period. These findings suggest that rapid muscle architecture remodelling occurs with lower limb unloading in humans, with changes occurring within 14 days of weight bearing removal. These adaptations, mitigating the decline in muscle PCSA, might protect from a larger loss of muscle force.

Mechanical properties of the triceps surae tendon and aponeurosis in relation to intensity of sport activity

The purpose of the present study was to investigate whether the mechanical properties (i.e. force strain relationship) of the triceps surae tendon and aponeurosis relate to the performed sport activity in an intensity-dependent manner. This was done by comparing sprinters with endurance runners and subjects not active in sports. Sixty-six young male subjects (26±5 yr; 183±6 cm; 77.6±6.7 kg) participated in the study. Ten of these subjects were adults not active in sports, 28 were endurance runners and 28 sprinters. All subjects performed isometric maximal voluntary plantar flexion contractions (MVC) on a dynamometer. The distal aponeuroses of the gastrocnemius medialis (GM) was visualised by ultrasound during the MVC. The results showed that only the sprinters had higher normalised stiffness (relationship between tendon force and tendon strain) of the triceps surae tendon and aponeurosis and maximal calculated tendon forces than the endurance runners and the subjects not active in sports. Furthermore, including the data of all 66 examined participants tendon stiffness correlated significantly ( r=0.817, P<0.001) with the maximal tendon force achieved during the MVC. It has been concluded that the mechanical properties of the triceps surae tendon and aponeurosis do not show a graded response to the intensity of the performed sport activity but rather remain at control level in a wide range of applied strains and that strain amplitude and/or frequency should exceed a given threshold in order to trigger additional adaptation effects. The results further indicate that subjects with higher muscle strength possibly increase the margin of tolerated mechanical loading of the tendon due to the greater stiffness of their triceps surae tendon and aponeurosis.

Effect of different ankle- and knee-joint positions on gastrocnemius medialis fascicle length and EMG activity during isometric plantar flexion

The purpose of this study was to provide evidence on the fact that the observed decrease in EMG activity of the gastrocnemius medialis (GM) at pronounced knee flexed positions is not only due to GM insufficiency, by examining muscle fascicle lengths during maximal voluntary contractions at different positions. Twenty-two male long distance runners (body mass: 78.5±6.7 kg, height: 183±6 cm) participated in the study. The subjects performed isometric maximal voluntary plantar flexion contractions (MVC) of their left leg at six ankle–knee angle combinations. To examine the resultant ankle joint moments the kinematics of the left leg were recorded using a Vicon 624 system with 8 cameras operating at 120 Hz. The EMG activity of GM, gastrocnemius lateralis (GL), soleus (SOL) and tibialis anterior (TA) were measured using surface electromyography. Synchronously, fascicle length and pennation angle values of the GM were obtained at rest and at the plateau of the maximal plantar flexion using ultrasonography. The main findings were: (a) identifiable differences in fascicle length of the GM at rest do not necessarily imply that these differences would also exist during a maximal isometric plantar flexion contraction and (b) the EMG activity of the biarticular GM during the MVC decreased at a pronounced flexed knee-joint position (up to 110°) despite of no differences in GM fascicle length. It is suggested that the decrease in EMG activity of the GM at pronounced knee flexed positions is due to a critical force–length potential of all three muscles of the triceps surae.

Difference in aftereffects following prolonged Achilles tendon vibration on muscle activity during maximal voluntary contraction among plantar flexor synergists

It has been suggested that a suppression of maximal voluntary contraction (MVC) induced by prolonged vibration is due to an attenuation of Ia afferent activity. The purpose of the present study was to test the hypothesis that aftereffects following prolonged vibration on muscle activity during MVC differ among plantar flexor synergists owing to a supposed difference in muscle fiber composition. The plantar flexion MVC torque and surface electromyogram (EMG) of the medial head of gastrocnemius (MG), the lateral head of gastrocnemius (LG), and the soleus (Sol) were recorded in 13 subjects before and after prolonged vibration applied to the Achilles tendon at 100 Hz for 30 min. The maximal H reflexes and M waves were also determined from the three muscles, and the ratio between H reflexes and M waves (H/Mmax) was calculated before and after the vibration. The MVC torque was decreased by 16.6 +/- 3.7% after the vibration (P < 0.05; ANOVA). The H/Mmax also decreased for all three muscles, indicating that Ia afferent activity was successfully attenuated by the vibration in all plantar flexors. However, a reduction of EMG during MVC was observed only in MG (12.7 +/- 4.0%) and LG (11.4 +/- 3.9%) (P < 0.05; ANOVA), not in Sol (3.4 +/- 3.0%). These results demonstrated that prolonged vibration-induced MVC suppression was attributable mainly to the reduction of muscle activity in MG and LG, both of which have a larger proportion of fast-twitch muscle fibers than Sol. This finding suggests that Ia-afferent activity that reinforces the recruitment of high-threshold motor units is necessary to enhance force exertion during MVC.

Effect of muscle fatigue on the compliance of the gastrocnemius medialis tendon and aponeurosis

The aim of the present study was to examine whether or not the compliance of the gastrocnemius medialis (GM) tendon and aponeurosis is influenced by submaximal fatiguing efforts. Fourteen elderly male subjects performed isometric maximal voluntary plantarflexion contractions (MVC) on a dynamometer before and after two fatiguing protocols. The protocols consisted of: (1) submaximal concentric isokinetic contractions (70% isokinetic MVC) at 60°/s and (2) a sustained isometric contraction (40% isometric MVC) until failure to hold the defined moment. Ultrasonography was used to determine the elongation and strain of the GM tendon and aponeurosis. To account for the axis misalignment between ankle and dynamometer, the kinematics of the leg were captured at 120 Hz. The maximum moment decreased from 85.9±17.9 Nm prior fatigue to 79.2±19 Nm after isokinetic fatigue and to 69.9±16.4 Nm after isometric fatigue. The maximal strain of the GM tendon and aponeurosis before fatigue, after isokinetic and after isometric fatigue were 4.9±1.1%, 4.4±1.1% and 4.3±1.1% respectively. Neither the strain nor the elongation showed significant differences before and after each fatiguing task at any 100 N step of the calculated tendon force. This implies that the compliance was not altered after either the isokinetic or the isometric fatiguing task. Therefore it was concluded that the strains during the performed submaximal fatiguing tasks, were too small to provoke any structural changes in tendon and aponeurosis.
