Using the Nasopalatine Canal for Enhanced Distribution in Severely Atrophic Maxilla With Immediate Loading of Zygomatic Implants: An 8-Year Retrospective Cohort Study.

This study presents retrospective 8-year clinical performance data from a patient cohort treated with immediately loaded maxillary full prostheses supported by zygomatic implants combined with implants placed in the nasopalatine canal, as a treatment for severely resorbed edentulous ridges. A retrospective analysis was conducted on data of maxillary edentulous patients with severe bone atrophy. All of them received zygomatic implants in combination with an implant placed in the nasopalatine foramen and an immediately loaded fixed temporary prosthesis in the first 24 hours. The frequency of major and minor adverse events was presented according to the time of occurrence. Fifteen patients were included in the study, without cases of implant loss or sensitivity in the nasopalatine region were observed up to 8 years. Only 1 major complication was observed, whereas minor complications were more frequent. In conclusion, this 8-year follow-up study demonstrated that the nasopalatine canal can be considered a viable implant site, with no major risks, to improve biomechanics by enabling a polygonal distribution in severely atrophic maxillae treated with zygomatic implants and immediate loading.

Influence of scanning pattern on accuracy, time, and number of photograms of complete-arch implant scans: A clinical study

ObjectivesTo measure the influence of scanning pattern on the accuracy, time, and number of photograms of complete-arch intraoral implant scans. MethodsA maxillary edentulous patient with 7 implants was selected. The reference implant cast was obtained using conventional methods (7Series Scanner). Four groups were created based on the scanning pattern used to acquire the complete-arch implant scans by using an intraoral scanner (IOS) (Trios4): manufacturer's recommended (Occlusal-Buccal-Lingual (OBL)), zig-zag (Zig-zag), circumferential (Circumf), and novel pattern that included locking an initial occlusal scan (O-Lock group) (n = 15). Scanning time and number of photograms were recorded. The linear and angular measurements were used to assess scanning accuracy. One-way ANOVA and Tukey tests were used to analyze trueness, scanning time, and number of photograms. The Levene test was selected to assess precision (α=0.05). ResultsStatistically significant differences in trueness were detected among OBL, Zig-zag, Circumf, and O-Lock regarding linear discrepancy (P<0.01), angular discrepancy (P<0.01), scanning time (P<0.01), and number of photograms (P<0.01). The O-Lock (63 ± 20 µm) showed the best linear trueness with statistically significant differences (P < 0.01) with Circumferential (86 ± 16 µm) and OBL (87 ± 19 µm) groups. The O-Lock (93.5 ± 13.4 s, 1080 ± 104 photograms) and Circumf groups (102.9 ± 15.1 s, 1112 ± 179 photograms) obtained lower scanning times (P < 0.01) and number of photograms (P < 0.01) than OBL (130.3 ± 19.4 s, 1293 ± 161 photograms) and Zig-zag (125.7 ± 22.1 s, 1316 ± 160 photograms) groups. ConclusionsThe scanning patterns tested influenced scanning accuracy, time, and number of photograms of the complete-arch scans obtained by using the IOS tested. The zig-zag and O-Lock scanning patterns are recommended to obtain complete-arch implant scans when using the selected IOS.

Esthetic analysis of upper lip morphology variation after the cross-arch fixed restoration of maxillary implant-supported prostheses via radiographic methods

Objective: To measure and analyze upper lip morphology variation before and after the cross-arch fixed restoration of the maxillary implant-supported prostheses using cone- beam CT (CBCT) to provide an esthetic objective reference for maxillary edentulous patients. Methods: There were 32 maxillary edentulous patients selected in the Department of Dental Implantology, The Affiliated Stomatological Hospital of Nanjing Medical University from January 2010 to December 2023. The CBCT data of patients with maxillary full-arch implant-supported fixed prostheses were retrospectively collected before implantation (T0), at the time of radio-guide insertion (T1), and after final repair (T2). The length and thickness of the upper lip were measured and analyzed at each time point, and the correlation between the upper lip contour data and facial esthetic parameters was analyzed. Results: After the final prosthodontic treatment, the length of the upper lip was significantly increased from (21.72±2.84) mm to (24.98±2.93) mm (t=-8.13, P<0.001) compared with that before implant treatment. The widths of the middle and vermilion of the upper lip (Sm-Hm/Ls-UP), were reduced from (13.24±1.41), (12.81±1.67) mm to (11.36±1.67), (10.21±1.69) mm, with significant differences (t=7.79, P<0001; t=9.37, P<0.001). The lower face height (Sn-Gn) was increased from (54.52±4.95) mm to (58.70±4.42) mm, with significant differences (t=-11.05, P<0.001). However, the nasolabial angle reduced significantly from 95.35°± 7.70°to 90.53°±7.28°(t=7.68, P<0.001). The width of the middle of the upper lip with a radiation guide was (10.94±1.24) mm, and it increased significantly compared with that after the final prosthesis treatment (t=-0.76, P<0.05). The proportion of straight upper lip profiles accounted for 59% (19/32), and the proportion of concave upper lip profiles accounted for 41% (13/32) after the final prosthesis treatment. In addition, the results of correlation analysis showed that the nasolabial angle variation was weak and negatively correlated with residual bone height (r=-0.37, P=0.490). Conclusions: After the treatment of the maxillary full-arch implant-supported fixed prosthesis, the length of the upper lip increases, and the thickness of the upper lip becomes significantly thin. The maxillary full-arch implant-supported fixed prostheses support upper lips to improve the patients' side appearances.

Survival Rate of Implants and Mechanical Complications of 64 Implant-Supported Complete Arch Prostheses in Maxillary Edentulous Patients with a Follow-Up of up to 12 years: A Cross Sectional Analytical Study.

This study aimed to identify the technical complications associated with maxillary complete-arch implant-supported fixed prostheses (ISFPs) and to evaluate the survival rates of both implants and prostheses. Additionally, we sought to analyze the impact of the presence of cantilevers, the type of prosthetic abutment, and the number of implants on the occurrence of complications. Data were obtained from standardized clinical assessments carried out in a single appointment by two calibrated professionals using information from dental records. The sample consisted of 64 patients rehabilitated with ISFPs classified as hybrid prostheses (metal-resin). The survival rate for implants was 98.6% after 1 to 12 years (5.2 years ± 36.6 months) and was 100% for the prostheses after a follow-up period of 12 to 144 months (mean 48.1 ± 33.94 months). The most frequent complications were occlusal wear (82.8% of patients) and loosening of prosthetic screws (16.6%) and abutments (8.1%). Screw loosening frequency was significantly greater in 30-degree abutments compared with straight abutments (P=0.008), but no significant difference was found between 17-degree and 30-degree abutments. Prostheses with a higher occurrence of screw loosening had a greater average CL/AP spread ratios (0.9 􀀀 0.5). In addition, abutments installed on distal implants were more likely to loosen (P<0.001). The absence of mutually protected occlusion did not significantly influence the loosening of screws and prosthetic abutments. Implantsupported fixed maxillary complete-arch rehabilitations have high survival rates but are subject to technical complications.

To evaluate radiographic changes around six implants (Two posterior zygomatic and four conventional implants) used with fixed detachable restorations in patients with severe maxillary posterior resorption with either Polyether ether ketone (PEEK) or titanium frameworks. Twelve maxillary edentulous patients were rehabilitated with six implants, including two posterior zygomatic and 4 conventionally placed implants for each patient following all-on-six distribution, and opposing mandibular all-on-four implant retained fixed detachable prostheses. A two-stage surgical unloaded healing protocol was performed. According to framework and teeth materials, there were two groups; Group I: CAD-CAM milled framework from modified BioHPP PEEK, bonded to polymethylmethacrylate crowns. Group II: screw-retained CAD-CAM milled framework from titanium, bonded to zirconium crowns. Bone loss evaluation for six implants was performed using cone beam computed tomography at the time of insertion (T0), after one year (T1), and after three years (T2). There was a statistically significant difference in bone loss between PEEK and titanium groups. Less bone loss in PEEK groups around anterior conventional implants at buccal surfaces (P =0.01), at mesial surfaces (P <0.001), at distal surfaces (P <0.001), and zygomatic implants at buccal surfaces (P =0.004), and at palatal surfaces (P =0.003). Within the limitations of the study, the full-arch PEEK and titanium frameworks of fixed-detachable prostheses used with an All-on-Six distribution using zygomatic implants for rehabilitation of maxillary edentulous atrophied posterior arches was a promising treatment approach. Less bone loss was observed with the PEEK framework restorations combined with the PMMA crowns group.

Clinical and patient-centered outcomes following rehabilitation of atrophic edentulous maxilla using six implants placed simultaneously with bilateral maxillary sinus augmentation: A retrospective case series

ObjectivesTo evaluate treatment success, patient satisfaction, and oral health-related quality of life (OHRQoL) after rehabilitation of atrophic edentulous maxilla (AEM) with six implants placed simultaneously with bilateral maxillary sinus floor augmentation (MSFA). Materials and methodsThe selected patients were fully edentulous with atrophic maxillary posterior residual ridges and rehabilitated with six implants placed simultaneously with bilateral MSFA and immediate All-on-4 provisional fixed prosthesis (PFP). After 7–12 months of implant surgery, all patients have received an All-on-6 definitive fixed prosthesis (DFP). After at least one year of function with DFP, clinical and radiographic examinations were performed. Patient satisfaction was assessed through a visual analog scale (VAS). The OHRQoL before treatment (T0), during provisional (T1), and after definitive prosthesis (T2) was evaluated using OHI-14. Results20 maxillary edentulous patients were rehabilitated with 120 implants, 20 immediate All-on-4 PFP, and 20 All-on-6 DFP. Of those, 12 patients have passed at least a year follow-up after DFP insertion and were eligible to be included in the assessment of treatment success. After a mean of 20 (12–36) months follow-up, the implant and prosthesis survival rates were 100%. The success rate at the implant level was 98.6%. The mean marginal bone loss (MBL) was 0.09 ± 0.06 mm. No prosthetic or postoperative complications, and the mean general satisfaction was (91.75 ± 7.06). There was a significant improvement in all OHIP-14 domains during the final All-on-6 prosthesis (T2) (P < 0.01). ConclusionsRehabilitation of atrophic edentulous maxilla using six implants with simultaneous bilateral MSFA and immediate All-on-4 PFP is a successful treatment approach associated with minimal postoperative complications and significant improvement in OHRQoL.

Modified transcrestal sinus floor elevation with concomitant implant placement in edentulous posterior maxillae with residual bone height of 5 mm or less: a non-controlled prospective study

The aim of this study was to describe a modified transcrestal sinus floor elevation (mTSFE) technique and to evaluate its clinical effectiveness and reliability when residual bone height is severely reduced. Forty-three maxillary edentulous patients who met the inclusion criteria were enrolled. All patients underwent the mTSFE technique; 66 dental implants were inserted simultaneously. Patient-reported outcomes were assessed 2 weeks after surgery. Prosthetic crowns were placed 6 months after surgery. Radiographic analyses and clinical analyses were conducted to assess the clinical effectiveness and feasibility of mTSFE during a follow-up period of 2–8 years. The mean vertical bone increase after surgery was 8.09 mm, and it decreased to 6.56 mm at 6 months after surgery. Two cases of membrane perforation occurred during surgery and one implant was lost in the third year after surgery; the survival rate at the implant level was 98.48%. No severe postoperative complication was reported and the subjective feeling of patients was acceptable. This mTSFE technique could simplify the operative procedure and might be helpful to reduce intraoperative trauma, as well as to alleviate postoperative discomfort.

This happens due to excessive masticatory forces from the natural dentition against the opposing arch. To overcome this problem, stainless steel can be added either in the form of mesh, bar or plate. In this present case report described the prosthetic rehabilitation of a complete maxillary edentulous and partial mandibular edentulous patient with stainless steel mesh reinforced maxillary removable complete denture and conventional removable partial denture to improve the impact strength of the denture and also at the same time to reduce the chances of alveolar ridge resorption.

Morphometry and displacement analysis of the upper lips following maxillary full-arch implant-supported fixed prostheses: a 3D morphometric study

BackgroundWith the emergence of three-dimensional (3D) integration technology, analysis of soft tissue displacement and morphological changes after maxillary full-arch implant-supported fixed prostheses can be performed. The aim of this study was to verify the feasibility of the 3D integration method for constructing the relative position of the prostheses and facial soft tissue, evaluate the displacement and morphological variation of the upper lips after maxillary full-arch implant-supported fixed prostheses.MethodsTwenty-five maxillary edentulous patients were recruited in this study. At the time of final prosthesis delivery, the 3D prostheses data and three 3D facial profiles were integrated. After method validation, the 3D position changes of seven soft tissue landmarks were used to reflect the 25 upper lips. The variation of four morphological distances were analyzed to reflect the morphological alteration of the upper lips. Two pairs of dentofacial landmarks were used to analyze the sagittal relative position of the prostheses and soft tissue. The included patients were also grouped to determine the impact of sex, upper lip thickness, and length on lip support changes.ResultsThe average distance of the two matched relative reliable forehead regions was only 0.32 mm. The sagittal shifts of labrale superius (LS), stomion (STO), crista philtri left (CPHL) and crista philtri right (CPHR) were 3.44 ± 1.39 mm, 2.52 ± 1.38 mm, 3.04 ± 1.18 mm, and 3.12 ± 1.21 mm, respectively. With the exception of the decrease in the length of subnasale (SN)-LS, the length of cheilion right (CHR)-cheilion left (CHL), CPHR-CPHL, and LS-STO significantly increased. The two pairs of dentofacial landmarks had strong positive movement correlations along the sagittal direction. Patients with thinner and longer lips showed more lip support than those with thicker and shorter lips by a clinically insignificant amount.ConclusionsThe integration method of 3D facial and dental data showed high repeatability in constructing the dentofacial relative position. The linear equations reflecting dentofacial relative position could aid clinicians in evaluating the restoration effect and estimate the upper lip variation.

3D Virtual Patient Representation for Guiding a Maxillary Overdenture Fabrication: A Dental Technique.

This report describes a technique to obtain a 3D virtual representation of a maxillary edentulous patient guided by an additively manufactured intraoral scan body. The intraoral scan body incorporated a custom tray and occlusion rim which facilitated the acquiring of a digital definitive cast, maxillary occlusion rim position, interocclusal registration, and guided the integration of the facial scans. The technique simplified the design and manufacturing of the maxillary overdenture.

New Rehabilitation Concept for Maxillary Edentulism: A Clinical Retrospective Study of Implant Crown Retained Removable Partial Dentures.

There have been no studies of implant-crown-retained removable partial dentures (IC-RPD) for the treatment of maxillary edentulism. The purpose of this study was to perform clinical and radiographic evaluations of implants in IC-RPD compared to implant overdentures (IOD) in maxillary edentulous patients. Twenty IC-RPDs with 74 splinted implant crowns and 18 IODs with 71 implants retained with magnet attachments were observed in 38 patients. We statistically analyzed survival rates and marginal bone loss (MBL) of implants based on multiple variables including first year pathologic condition, location of placed implant, age, and sex in both treatments. Patient reported oral measurements (PROMs) regarding functional/esthetic improvement after IC-RPD or IOD treatments and prosthetic complications were also statistically analyzed. After a median observation period of 47.1 months (up to 147 months), we observed 97.3% implant survival rates for IC-RPD and 70.4% for IOD (p < 0.001). Among variables, first year pathologic condition (p < 0.001) and sex (p = 0.027) influenced implant survival rates. The MBL of implants for IC-RPD and IOD groups at the final check-up were 1.12 ± 1.19 mm and 3.31 ± 1.71 mm, respectively (p < 0.001). In both groups, patients with peri-implantitis (p < 0.001) and patients older than 65 years (p = 0.029) showed significantly higher implant MBL regardless of treatment modality. Functional and esthetic satisfaction were significantly improved (p < 0.001) after both treatments. The IOD group showed more frequent prosthetic complications compared to the IC-RPD group. Within the limitations of a retrospective study, we concluded that RPD with few splinted implant crowns is a feasible alternative treatment modality for maxillary edentulous patients with anatomical limitations.

Comparison of four-implant-retained overdentures and implant-supported fixed prostheses using the All-on-4 concept in the maxilla in terms of patient satisfaction, quality of life, and marginal bone loss: a 2-year retrospective study.

The purpose of the present study was to compare and assess maxillary edentulous patients treated with four-implant-retained overdentures and with implant-supported fixed prostheses using the All-on-4 concept in terms of patient satisfaction, oral health-related quality of life, and marginal bone loss. Thirty-three patients treated with either maxillary four-implant-retained overdentures (n = 15) or with maxillary implant-supported fixed prosthesis using the All-on-4 concept (n = 18) were compared in terms of patient satisfaction, quality of life, and marginal bone loss. Independent sample t test was used to compare the two groups in terms of satisfaction, OHIP-14 scores, and marginal bone loss. The paired sample t test was used to compare bone loss at different time intervals. The comparison of marginal bone loss with regard to implant location was accomplished with the one-way ANOVA test. P values of < .05 were considered as statistically significant. No statistically significant differences were found between the two groups' OHIP-14 scores (P > .05). When patient satisfaction scores were evaluated, hygiene maintenance (P = .001) and pain scores (P = .001) were significantly higher in the implant-retained overdenture group than in the implant-supported fixed prosthesis group (P < .05). No statistically significant difference was observed between axial and tilted implants at both the 12-month (P = .59) and 24-month (P = .77) follow-up periods in the implant-supported fixed prosthesis group. Four-implant-supported fixed prostheses with the All-on-4 concept and four-implant-retained overdentures present similar marginal bone loss and quality of life scores after 2 years of function. However, patients found overdentures easier to clean but more painful in comparison with the fixed prosthesis.

Eight to eleven-year follow-up of immediately loaded implants placed in edentulous maxillae with compromised bone volume and poor bone quality: A prospective cohort study.

There is a lack of long-term evaluations of immediately loaded implants in patients with compromised maxillary bone. To evaluate long-term survival and success of immediately loaded implants in subjects with poor maxillary bone quality and volume. Follow-up was performed on subjects who received six implants loaded within 24 hours with screw-retained fixed prostheses. Twenty-five subjects with limited bone (Lekholm and Zarb, quantity 3 and 4, and quality C and D) were included in the study. Nineteen participated in the radiographic examination and of these 17 participated in the clinical examination at the last visit. Evaluations of marginal bone loss were performed at 1, 3, 5, and 8-11 years. The last clinical examination included removal of the prosthesis followed by registration of: plaque, pus, pocket depth, bleeding upon probing, mobility, and percussion testing. The mean follow-up was 9 years and 2 months (101-131 months) after surgery and showed a cumulative implant survival rate of 81.9% and success rate of 74.7%. Mean marginal bone loss was 1.29 mm (SD 2.47 mm range 0-11 mm) with a mean pocket depth of 3.1 mm (SD 2.4 mm, range 1.5-13.5 mm). 42.5% of the implants showed plaque retention and 72.2% showed bleeding on probing. This nonaugmenting immediate loaded implant protocol for maxillary edentulous patients is a satisfactory solution for selected patients.

Patient-Reported Outcomes of Maxillary Edentulous Patients Wearing Overdentures Retained by Two Implants from Insertion to 4 Years.

This cohort study evaluated patient satisfaction for maxillary implant-retained overdentures (IODs) on two implants up to 4 years and assessed the treatment effect over time. Patients encountering problems with their conventional dentures were included and received maxillary IODs on two titanium-zirconium implants and ball anchors in the canine area. Patient satisfaction was assessed using the oral health impact profile (OHIP-20E) questionnaires both for dentures and IODs. Two months after insertion of IODs (baseline), the patients chose the preferred overdenture design with full or reduced palatal coverage. OHIP-20E questionnaires were followed according to the individual choice at 1 and 4 years, and outcomes were compared with baseline. Sixteen out of 21 patients were evaluated at a mean follow-up of 4 years (range: 2.4 to 4.8 years). There was no significant difference in the OHIP domains for IODs at 1 year (OHIP_total_1y: 9.5, SD: 13.0) and 4 years (OHIP_total_4y: 14.2, SD: 19.1) compared with baseline (OHIP_total_BL: 12.4, SD: 14.7). Patients were most satisfied with social disability both for IODs (OHIP_BL: 6.0, SD: 7.6; OHIP_1y: 3.4, SD: 5.4; OHIP_4y: 5.7, SD: 9.5) and dentures (OHIP_CD_old: 28, SD: 29.7; OHIP_CD_new: 25.4, SD: 28.67). Patients were least satisfied with functional limitation both for IODs (OHIP_BL: 6.0, SD: 7.6; OHIP_1y: 3.4, SD: 5.4; OHIP_4y: 5.7, SD: 9.5) and dentures (OHIP_CD_old: 28, SD: 29.7; OHIP_CD_new: 25.4, SD: 28.67). Patient satisfaction with maxillary IODs on two implants did not change from baseline to 4 years and was high at 4 years of function.
