Rates and predictors of reimplantation of matrix-induced autologous chondrocyte implantation following first stage cartilage harvest: A cohort study

PurposeTo assess the reimplantation rate and predictors of patients requiring second-staged matrix-induced autologous chondrocyte implantation (MACI) reimplantation after initial first stage cartilage biopsy. MethodsA retrospective review was performed from 2018 to 2022 among patients who underwent only phase I MACI biopsy procedure (biopsy group) or both phase I with transition to phase II implantation of chondrocytes (implantation group) at a single tertiary center. Demographic, qualitative, and quantitative measurements were recorded, and univariate and multivariate regression analysis was performed to assess predictors of ultimately requiring second stage MACI implantation. ResultsA total of 71 patients (51% female, age 27.7 ± 10.6 years (range 12–50)) were included in this study. Eventually, 25 of 71 patients (35.2%) experienced persistence of symptoms after initial MACI biopsy and other concomitant procedures, requiring second-stage implantation. Univariate analysis showed the implantation group compared to the biopsy group had a greater lesion size (5.2 cm2 ± 3.3 vs. 3.3 cm2 ± 1.4, p = 0.024), a higher proportion patients ≥ 26 years of age (76% vs. 43%, p = 0.009), a medial femoral condyle lesion more commonly (33% vs 11%, p = 0.005), were more often female (72% vs. 39%, p = 0.008), and had less often soft tissue repair at time of biopsy (32% vs. 61%, p = 0.020). Backward multivariate logistic regression analysis revealed that size of the lesion (OR 1.43, p = 0.031) and age ≥ 26 years old at time of biopsy (OR 3.55, p = 0.042) were independent predictors of not responding to initial surgery and requiring implantation surgery. ConclusionThis study found that 35% of patients undergoing MACI phase I biopsy harvest eventually required autologous implantation. Independent risk factors for progressing to implantation after failed initial surgery were larger defect size and older age.Level of Evidence: III, Cohort Study.

Do reconstructive techniques for osteochondritis dissecans of the skeletally mature knee work?A systematic review and meta-analysis.

Osteochondritis dissecans (OCD) is a common cause of knee pain. Management for adult-onset OCD (AOCD) usually involves surgery. Surgical treatments include palliative, reparative and reconstructive techniques. The aim of this systematic review and meta-analysis is to evaluate the efficacy of reconstructive techniques for the treatment of OCD in skeletally mature knees. A systematic search was carried out on four databases up to November 2023 (Medline, Embase, Cochrane Library, Web of Science). The study was registered on international prospective register of systematic reviews and performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines. Clinical studies on skeletally mature patients were included, which utilised reconstructive techniques such as autologous chondrocyte implantation (ACI), matrix-induced autologous chondrocyte implantation, osteochondral allograft transplantation surgeryor bone marrow-derived cellular transplantation. Demographical data, patient-reported outcome measures and postoperative complications were recorded. Quantitative outcome measures that were comparable across studies were pooled for meta-analysis. A random effects model was used. Heterogeneity was assessed using the I2 statistic and Cochran's Q test. Statistical significance was set at p < 0.05. Risk of bias was assessed using the risk of bias in non-randomised studies - of interventions tool for nonrandomised studies. Sixteen studies were included with 458 OCD lesions in 432 patients. The average age was 24.9, and 62.6% were male. The mean follow-up time was 61.5 months. At 36 months follow-up, International Knee Documentation Committee (IKDC) subjective, Tegner and EuroQol-visual analogue scale (EQ-VAS) scores improved from 42.4 to 78.6 (standard mean difference [SMD]: 2.47; p < 0.001), 2.27-4.99 (SMD: 2.363; p = 0.002) and 30.4-57.5 (SMD: 2.390; p < 0.001), respectively. Overall complication rate was 8.9%. Smaller OCD lesion sizes resulted in a greater improvement in IKDC subjective (SMD: 2.64 vs. 2.01; p = 0.038), EQ-VAS (SMD: 3.16 vs. 0.95; p = 0.046) and Tegner scores (SMD: 3.13 vs. 1.05; p = 0.007) and had a lower complication rate (p = 0.008). Males showed a larger improvement in IKDC subjective scores than females (SMD: 2.56 vs. 1.56; p = 0.029), while younger patients had a larger improvement in IKDC subjective scores (SMD: 2.71 vs. 2.12; p = 0.045) and fewer complications than older patients (p = 0.003). There were no significant differences between cohorts treated with ACI and those treated with non-ACI reconstructive techniques. Publication bias was not detected (n.s.). Reconstructive techniques used to treat OCD in the skeletally mature knee resulted in significant improvements in clinical and functional outcomes, with a low overall complication rate. Since a younger age leads to a greater improvement in IKDC subjective score and a lower complication rate, surgical intervention should not be delayed, especially in AOCD lesions which are more likely to follow a progressive and unremitting clinical course. New horizons in cartilage repair: update on treatment trends and outcomes

IntroductionArticular cartilage plays a crucial role in preserving the knee joint's structural integrity. Damage can manifest as focal chondral defects or progressive osteoarthritis. Treatment options for knee cartilage damage are multifaceted, encompassing surgical and nonsurgical interventions to repair defects, alleviate symptoms, restore function, and slow disease progression. ObjectivesGain insight and summarize the latest available evidence of knee joint preservation techniques. MethodsThe indications, outcomes, and biological characteristics of operative and nonoperative knee cartilage repair and joint preservation therapies were summarized. ResultsOsteochondral autologous transplantation and osteochondral allograft transplantation offer durable solutions with excellent long-term outcomes. Autologous chondrocyte implantation and matrix-induced autologous chondrocyte implantation are advanced, cell-based therapies effective for larger defects, especially in younger, active individuals. Non-operative therapies such as hyaluronic acid provide symptomatic relief for knee osteoarthritis. Bone marrow aspirate concentrate and platelet-rich plasma show efficacy in alleviating symptoms and improving joint function, presenting a minimally invasive approach with promising results for knee preservation and cartilage repair. ConclusionThe use of minimally invasive and nonsurgical interventions in addressing focal cartilage defects, osteoarthritis, and overall joint preservation has yielded varied results. Although recent literature has exhibited encouraging findings, the existing body of high-level evidence remains limited. The emerging evidence inspires optimism for the future, as medical practitioners shift focus to the next wave of knee cartilage repair and joint preservation methodologies. However, comprehensive research is essential to establish conclusive insights into outcomes, safety profiles, and comparative effectiveness among the diverse available treatments.

10-Year Prospective Clinical and Radiological Evaluation After Matrix-Induced Autologous Chondrocyte Implantation and Comparison of Tibiofemoral and Patellofemoral Graft Outcomes

Background: Long-term outcomes in larger cohorts after matrix-induced autologous chondrocyte implantation (MACI) are required. Furthermore, little is known about the longer-term clinical and radiological outcomes of MACI performed in the tibiofemoral versus patellofemoral knee joint. Purpose: To present the 10-year clinical and radiological outcomes in patients after MACI and compare outcomes in patients undergoing tibiofemoral versus patellofemoral MACI. Study Design: Case series; Level of evidence, 4. Methods: Between September 2002 and December 2012, 204 patients who underwent MACI were prospectively registered into a research program and assessed preoperatively and at 2, 5, and 10 years postoperatively. Of these patients, 168 were available for clinical review at 10 years, with 151 (of a total of 182) grafts also assessed via magnetic resonance imaging (MRI). Patients were evaluated using the Knee injury and Osteoarthritis Outcome Score, a visual analog scale for pain frequency and severity, satisfaction, and peak isokinetic knee extensor and flexor strength. Limb symmetry indices (LSIs) were calculated for strength measures. Grafts were scored on MRI scans via the MOCART (magnetic resonance observation of cartilage repair tissue) system, with a focus on tissue infill and an overall MRI graft composite score. Results: All patient-reported outcome measures improved (P < .0001) up to 2 years after surgery. Apart from the significant increase (P = .004) in the peak isokinetic knee extensor LSI, no other patient-reported outcome measure or clinical score had changed significantly from 2 to 10 years. At the final follow-up, 92% of patients were satisfied with MACI to provide knee pain relief, with 76% satisfied with their ability to participate in sports. From 2 to 10 years, no significant change was seen for any MRI-based MOCART variable nor the overall MRI composite score. Of the 151 grafts reviewed via MRI at 10 years, 14 (9.3%) had failed, defined by graft delamination or no graft tissue on MRI scan. Furthermore, of the 36 patients (of the prospectively recruited 204) who were not available for longer-term review, 7 had already proceeded to total knee arthroplasty, and 1 patient had undergone secondary MACI at the same medial femoral condylar site because of an earlier graft failure. Therefore, 22 patients (10.8%) essentially had graft failure over the period. At the final follow-up, patients who underwent MACI in the tibiofemoral (vs patellofemoral) joint reported significantly better Knee injury and Osteoarthritis Outcome Score subscale scores for Quality of Life (P = .010) and Sport and Recreation (P < .001), as well as a greater knee extensor strength LSI (P = .002). Even though the tibiofemoral group demonstrated better 10-year MOCART scores for tissue infill (P = .027), there were no other MRI-based differences (P > .05). Conclusion: This study reports the long-term review of a prospective series of patients undergoing MACI, demonstrating good clinical scores, high levels of patient satisfaction, and acceptable graft survivorship at 10 years. Patients undergoing tibiofemoral (vs patellofemoral) MACI reported better long-term clinical outcomes, despite largely similar MRI-based outcomes.

Epigenetic Priming Enhances Chondrogenic Potential of Expanded Chondrocytes.

Expansion of chondrocytes presents a major obstacle in the cartilage regeneration procedure, such as matrix-induced autologous chondrocyte implantation. Dedifferentiation of chondrocytes during the expansion process leads to the emergence of a fibrotic (chondrofibrotic) phenotype that decreases the chondrogenic potential of the implanted cells. We aim to (1) determine the extent that chromatin architecture of H3K27me3 and H3K9me3 remodels during dedifferentiation and persists after the transfer to a three-dimensional (3D) culture; and (2) to prevent this persistent remodeling to enhance the chondrogenic potential of expanded bovine chondrocytes, used as a model system. Chromatin architecture remodeling of H3K27me3 and H3K9me3 was observed at 0 population doublings, 8 population doublings, and 16 population doublings (PD16) in a two-dimensional (2D) culture and after encapsulation of the expanded chondrocytes in a 3D hydrogel culture. Chondrocytes were treated with inhibitors of epigenetic modifiers (epigenetic priming) for PD16 and then encapsulated in 3D hydrogels. Chromatin architecture of chondrocytes and gene expression were evaluated before and after encapsulation. We observed a change in chromatin architecture of epigenetic modifications H3K27me3 and H3K9me3 during chondrocyte dedifferentiation. Although inhibiting enzymes that modify H3K27me3 and H3K9me3 did not alter the dedifferentiation process in 2D culture, applying these treatments during the 2D expansion did increase the expression of select chondrogenic genes and protein deposition of type II collagen when transferred to a 3D environment. Overall, we found that epigenetic priming of expanded bovine chondrocytes alters the cell fate when chondrocytes are later encapsulated into a 3D environment, providing a potential method to enhance the success of cartilage regeneration procedures.

Minimum 10-Year Outcomes of Matrix-Induced Autologous Chondrocyte Implantation in the Knee: A Systematic Review.

Matrix-induced autologous chondrocyte implantation (MACI) is an established cell-based therapy for the treatment of chondral defects of the knee. As long-term outcomes are now being reported in the literature, it is important to systematically review available evidence to better inform clinical practice. To report (1) subjective patient-reported outcomes (PROs) and (2) the rate of graft failure, reoperation, and progression to total knee arthroplasty (TKA) after undergoing MACI of the knee at a minimum 10-year follow-up. Systematic review; Level of evidence, 4. A comprehensive search of Ovid MEDLINE and Epub Ahead of Print, In-Process & Other Non-Indexed Citations and Daily; Ovid Embase; Ovid Cochrane Central Register of Controlled Trials; Ovid Cochrane Database of Systematic Reviews; and Scopus from 2008 to September 15, 2022, was conducted in the English language. Study eligibility criteria included (1) full-text articles in the English language, (2) patients undergoing a MACI within the knee, (3) clinical outcomes reported, and (4) a minimum 10-year follow-up. In total, 168 patients (99 male, 69 female; mean age, 37 years [range, 15-63 years]; mean body mass index, 26.2 [range, 18.6-39.4]) representing 188 treated chondral defects at a minimum 10-year follow-up after MACI were included in this review. Significant and durable long-term improvements were observed across multiple PRO measures. Follow-up magnetic resonance imaging (MRI), when performed, also demonstrated satisfactory defect fill and an intact graft in the majority of patients. The all-cause reoperation rate was 9.0%, with an overall 7.4% rate of progression to TKA at 10 to 17 years of follow-up. At a minimum 10-year follow-up, patients undergoing MACI for knee chondral defects demonstrated significant and durable improvements in PROs, satisfactory defect fill on MRI-based assessment, and low rates of reoperation and TKA. These data support the use of MACI as a long-term treatment of focal cartilage defects of the knee.

MACI Sandwich Technique With Autologous Bone Graft

Background: Osteochondral lesions of the knee are difficult to manage. Such lesions can be effectively treated with the matrix-induced autologous chondrocyte implantation (MACI) “sandwich” technique using autologous chondrocytes on porcine collagen membrane in conjuction with autologous cancellous bone grafting (ABG). Few studies have examined this technique to restore the osteochondral unit. Indications: The MACI “Sandwich” procedure is indicated when osteochondral lesions are larger than 2 cm2 in size and deeper than 8 to 10 mm, with symptoms consistent with the location of the lesion. Technique Description: The chondral defect is radically debrided back to healthy bone and cartilage tissue. The base of the bony cavity is drilled to enhance the vascular supply and promote healing. Next, the bony cavity is prepared to slightly undermine the subchondral bone surface around the articular margins, with the depth of the cavity wider than the opening, similar to a dental amalgam. The autologous cancellous bone is subsequently morselized and impacted up to the level of the native subchondral bone plate. The first MACI membrane with the cells facing up is placed directly onto the bone graft site and compressed with a neural patty. The neural patty is removed, and the second MACI membrane is then placed with the cells facing down. The edges are then micro-sutured to ensure stability. Results: The “sandwich” technique has superior survival rates compared with autologous bone grafting alone, with patients reporting decreased pain, improved function, and high satisfaction scores over a mid-term to long-term follow-up. Discussion/Conclusion: The MACI “sandwich” technique is an effective surgical intervention to restore the osteochondral unit and preserve the patient's native knee joint. Return to Play After Knee Articular Cartilage Restoration: Surgical Options, Rehabilitation Protocols, and Performance Outcomes.

Numerous cartilage restoration techniques have proven to be effective in the treatment of articular cartilage defects. The ultimate goal of these procedures is to improve pain and function, thereby increasing the likelihood of a patient's return to physical activity. Postoperative rehabilitation is a key component for a successful and expedient return to activities. The purpose of this article is to review the current literature regarding common surgical options, rehabilitation protocols, and performance outcomes after operative treatment of articular cartilage defects. Studies have demonstrated improved short- to long-term outcomes in a majority of techniques. However, the clinical benefits of microfracture are short-lived, which has led to the use of alternative procedures. Rehabilitation protocols are not standardized, but emphasis has been placed on bracing, weightbearing, early continuous passive range of motion, and strengthening to improve function. There is growing evidence to suggest that accelerated rehabilitation after matrix-induced autologous chondrocyte implantation may result in superior outcomes compared to delayed rehabilitation. Overall, most techniques result in satisfactory rates of return to play, though existing comparative studies typically include patients with heterogeneous pathology, complicating effective synthesis of outcomes data. In appropriately selected patients, cartilage restoration procedures after articular cartilage injury result in favorable patient-reported clinical outcomes and high rates of return to play. While studies emphasize the critical role that rehabilitation plays with respect to outcomes after surgery, there are substantial inconsistencies in protocols across techniques.

Engineered biochemical cues of regenerative biomaterials to enhance endogenous stem/progenitor cells (ESPCs)-mediated articular cartilage repair.

As a highly specialized shock-absorbing connective tissue, articular cartilage (AC) has very limited self-repair capacity after traumatic injuries, posing a heavy socioeconomic burden. Common clinical therapies for small- to medium-size focal AC defects are well-developed endogenous repair and cell-based strategies, including microfracture, mosaicplasty, autologous chondrocyte implantation (ACI), and matrix-induced ACI (MACI). However, these treatments frequently result in mechanically inferior fibrocartilage, low cost-effectiveness, donor site morbidity, and short-term durability. It prompts an urgent need for innovative approaches to pattern a pro-regenerative microenvironment and yield hyaline-like cartilage with similar biomechanical and biochemical properties as healthy native AC. Acellular regenerative biomaterials can create a favorable local environment for AC repair without causing relevant regulatory and scientific concerns from cell-based treatments. A deeper understanding of the mechanism of endogenous cartilage healing is furthering the (bio)design and application of these scaffolds. Currently, the utilization of regenerative biomaterials to magnify the repairing effect of joint-resident endogenous stem/progenitor cells (ESPCs) presents an evolving improvement for cartilage repair. This review starts by briefly summarizing the current understanding of endogenous AC repair and the vital roles of ESPCs and chemoattractants for cartilage regeneration. Then several intrinsic hurdles for regenerative biomaterials-based AC repair are discussed. The recent advances in novel (bio)design and application regarding regenerative biomaterials with favorable biochemical cues to provide an instructive extracellular microenvironment and to guide the ESPCs (e.g. adhesion, migration, proliferation, differentiation, matrix production, and remodeling) for cartilage repair are summarized. Finally, this review outlines the future directions of engineering the next-generation regenerative biomaterials toward ultimate clinical translation.

Paper 54: Long Term Clinical and Radiological Review of Matrix-Induced Autologous Chondrocyte Implantation

Objectives: Long-term outcomes following matrix-induced autologous chondrocyte implantation (MACI) are required to better demonstrate later-stage clinical outcomes, patient satisfaction and longevity of repair tissue. This prospective study sought to present the long-term (minimum 10 year) clinical and radiological outcomes of the largest prospectively assessed MACI dataset yet to be reported and compare outcomes in patients undergoing tibiofemoral (TF) versus patellofemoral (PF) MACI. Methods: Between September 2002 and December 2012, 204 patients that underwent MACI were prospectively registered into a research program and assessed pre-surgery and at 2, 5 and 10 years. Of these, 168 patients (182 grafts) were available for clinical review at a minimum 10 years (range 10 to 16 years), of which 151 also underwent MRI. Included in the 182 MACI grafts with minimum 10-year review were: 83 medial femoral condyle (MFC), 32 lateral femoral condyle (LFC), 35 trochlea and 32 patella. Clinically, patients were evaluated with the Knee Injury and Osteoarthritis Outcome Score (KOOS), the 36-item Short Form Health Surveys (SF-36), a visual analogue scale for pain frequency (VAS-F) and severity (VAS-S), satisfaction and peak isokinetic knee extensor and flexor strength. Limb symmetry indices (LSIs) were calculated for strength measures. Grafts were scored on MRI via the magnetic resonance observation of cartilage repair tissue (MOCART) system, with a focus on tissue infill and an overall MRI graft composite score. Results: All PROMs significantly improved (p&lt;0.0001) over the pre- and post-operative period to 2-years post-surgery (Figure 1), with no statistical change (p&gt;0.05) from 2-years to final 10-year follow-up. While the peak isokinetic knee extensor LSI improved significantly over time (p=0.016), at final follow up the mean LSI for peak isokinetic knee flexor and extensor strength was 99.1% and 92.8%, respectively. At final follow-up, 93% of patients were satisfied with MACI to provide knee pain relief, 77% were satisfied with their ability to participate in sports and 89% were satisfied overall. A statistically significant decline was observed from 5 years post-surgery to final follow-up in the MOCART variable of subchondral lamina (p=0.002), while a non-statistical decline was observed in tissue infill (p=0.066) and the overall MRI composite score (p=0.072) (Figure 2), with the MRI-based progression of one patient over the period demonstrated in Figure 3. Of the 151 grafts reviewed via MRI at 10 years or beyond, 14 (9.3%) had failed as defined by graft de-lamination or a graft bed devoid of any repair tissue. Furthermore, of the 36 patients (out of the prospectively recruited 204) that were not available for longer term review, 7 had already proceeded to total knee arthroplasty (TKA) and 1 patient had undergone secondary MACI at the same MFC site due to earlier graft failure. Therefore, a total of 22 patients (10.8%) had essentially failed at or before the final review time. At final follow-up, patients that underwent MACI in the TF (versus PF) joint reported significantly better KOOS sub-scale scores for Pain (p=0.013, TF mean 86.5, PF mean 80.1), Sport (p&lt;0.001, TF mean 71.5, PF mean 60.5) and Quality of Life (p=0.010, TF mean 65.8, PF mean 58.7), as well as a greater knee extensor strength LSI (p=0.002, TF mean 96.0%, PF mean 85.8%). While the TF group demonstrated better 10-year MOCART scores for tissue infill (p=0.027, TF mean 3.2, PF mean 2.9), there were no other differences in MRI-based scores including the overall MRI composite score (p=0.481, TF mean 3.0, PF mean 3.1). Conclusions: This study reports the largest prospective series of patients with long-term review, demonstrating good clinical scores, levels of patient satisfaction and graft survivorship beyond 10 years. Patients undergoing TF (versus PF) MACI reported better long-term clinical outcomes, despite largely similar MRI-based outcomes.

Poster 144: No Difference in Chondrocyte Viability Using Scissor as Compared to Custom Cutter for Matrix-Induced Autologous Chondrocyte Implantation Membrane (MACI)

Objectives: Matrix-induced autologous chondrocyte implantation (MACI) is a well-established method for knee cartilage restoration that uses autologous chondrocytes seeded onto a porcine membrane. Preserving chondrocyte viability while ensuring the correct size and contour of the MACI membrane is critical to the outcome. Traditional manual preparation has been performed using scissors (Sc). More recently, custom cutters (CC) have made preparation easier and more efficient. However, the effect of the MACI preparation method on cell viability is currently unknown. The purpose of this study was to determine the difference in chondrocyte viability between MACI membranes prepared with either the Sc or CC technique. Our hypothesis was that there would be no difference in overall cell viability between groups. Methods: The study was approved by the Institutional Review Board (IRB), and all the patients were consented prior to surgery. The remnant MACI membrane following surgical implantation (N=5 patients) was utilized for study purposes. A board-certified and fellowship-trained surgeon prepared each membrane on the back table using sterile technique. Preparation included an untouched region (control), a hand-cut 15 mm circular region (Sc), and a custom-cut 15mm circular region (CC). The samples were carefully transported in media to the laboratory setting. In the laboratory, The Sc and the CC regions were further divided into three distinct zones depending on their proximity to the cutting area (i.e., impact, adjacent, and central zones), and tested using confocal laser scanning microscopy with 3D Spot Segmentation to quantify the percentage of Live/Dead cells within the various zones (percentage of live cells and cell density were presented in x105/cm2). The Impact Zone, which was the closest to the cutting edge, was defined by the boundary of high cell density. This was followed by the Adjacent Zone between the Impact Zone and the most inner Central Zone. The size of the Impact Zone was calculated relative to the size of the entire sample. The increase of dead cells in the affected zones were calculated relative to the total cell count and represented as a percentage. Results were analyzed statistically. Results: The cell viability was lower in the Impact Zone of both the Sc (40.94% ± 2.85, p &lt; 0.005) and CC (36.42% ± 3.85, p &lt; 0.005) groups when compared to the Adjacent Zone (74.17% ± 2.8 and 77.69% ± 2.97, respectively). The cell viability remained high in the Central Zone in all samples, with no significant differences between the Sc and CC groups. (77.18% ± 1.38 to 79.95% ± 1.99, p&gt;.05). The average cell density in both Impact and Adjacent Zones was 5.84 ± 0.26 to 6.49 ± 0.34 x 105/cm2 (p&gt;.05) respectively. The total size of the Impact Zone in both Sc and CC groups was 9.22% ± 0.79 for the CC group and 10.17% ± 1.59 for the Sc group, p&gt;.05. Similarly, the percentage of non-viable cells resulting from the cutting preparation was 3.75% ± 0.38 and 3.97% ± 0.22 for the custom cut and hand-cut groups, respectively, without significant differences between the groups. Conclusions: MACI membrane prepared with either the Sc or CC technique demonstrated a significant reduction in cell viability in the Impact Zone (i.e. periphery) as compared to the Central Zone of the MACI transplant. The Impact Zone was estimated to be approximately 10% of the overall membrane with roughly 4% increased cell death attributed to membrane cutting in each group. There was no difference in the overall chondrocyte viability or size of the Impact Zone when comparing Sc to CC groups. Based on these study results, surgeons may consider using either technique when performing the MACI procedure.

Demonstration of the Medial Subvastus Knee Exposure for MACI Implantation

Background: There are several techniques for knee exposure in treating chondral defects with restorative procedures. The medial subvastus knee exposure is often overlooked when choosing the surgical approach. When properly performed, the medial subvastus approach can allow for the treatment of a wide range of pathology while preserving the extensor mechanism. Indications: We present a case of an active 40-year-old man with persistent left knee pain noted to have a full-thickness medial femoral condylar articular cartilage defect in an otherwise healthy appearing knee. Technique Description: The rationale and considerations for the medial subvastus knee exposure in treating a chondral defect with an autologous chondrocyte implant are discussed. Careful consideration of the approach, intra-articular exposure, and closure are emphasized to achieve optimal outcomes. Results: At 18 months postoperative, the patient reported no knee pain with improvements made in Knee injury and Osteoarthritis Outcome Score (KOOS), Lysholm, and International Knee Documentation Committee (IKDC) scores. These results are consistent with our institutions larger cohort of 26 patients undergoing matrix-induced autologous chondrocyte implantation (MACI) using the subvastus approach. These improvements in patient-reported outcomes are maintained at 2 years postoperative and are consistent with other published outcomes of the MACI procedure. Discussion/Conclusion: Excellent results in addressing chondral pathology about the knee can be achieved by selecting the appropriate surgical access. The subvastus approach has several advantages such as preserving the quadriceps mechanism, improving postoperative quadriceps muscle strength, conservation of the patellar genicular blood supply and possibly reducing postoperative pain that can result in a faster rehabilitation. When performed systematically and carefully, complete exposure of the knee can be achieved to facilitate a wide variety of surgical interventions. In patients undergoing cartilage restoration procedures, the medial subvastus approach should be considered in the surgeon’s armamentarium. Poster 294: Patellar Tendon Insertion is Not Distalized following Tibial Tubercle Osteotomy with Distalization

Objectives: Patella alta is a common anatomic risk factor in patients with patellofemoral instability and/or patellofemoral cartilage lesions, occurring in nearly 25% of these patients. Distalization tibial tubercle osteotomy (TTO) is effective in patients with patella alta in association with patellofemoral instability and cartilage lesions. Prior studies have suggested that increased patellar tendon length itself is a risk factor for recurrent patellofemoral instability, and have proposed the addition of a patellar tendon tenodesis via placement of a suture anchor proximal to the distalization TTO. However, concerns regarding increased patellofemoral cartilage stresses with addition of patellar tendon tenodesis have limited its adoption. The purpose of this study was to evaluate patellar tendon length and alignment parameters on preoperative and postoperative MRI and to evaluate patient reported outcomes (PROs) following distalization tibial tubercle osteotomy (TTO) without patellar tendon tenodesis in patients with patella alta. Methods: We conducted a retrospective review of 20 adult patients who underwent distalization TTO for patellar alta at our institution (December 2014-August 2021). All patients underwent preoperative and postoperative magnetic resonance (MR) imaging of the affected knee. Distalization TTO utilized a wedge-shaped osteotomy measuring ˜5cm in length and was fixed with two 4.5mm bicortical fully threaded screws following TT transfer. Caton-Deschamps (CDI), axial and sagittal TT-TG indices1, patellar tendon length and distances from tibial plateau to patellar tendon insertion and tibial tubercle were assessed (Figure 1). PROs included IKDC Subjective Knee Evaluation, KOOS Quality of Life Form, Kujala Anterior Knee Pain From, and Veterans RAND 12 Item Mental (VR12-MH) and Physical Health (VR12-PH) Surveys. Clinical data, including demographics, complication and perioperative data, was obtained from preoperative and postoperative charts in our electronic medical record. Results: Average age at time of surgery was 27.4 years (range: 14 – 42 years) and all patients were female. Average BMI was 24.9 +/- 4.4 kg/m2. Eleven patients (55%) were diagnosed with isolated patellofemoral cartilage lesions, four patients (20%) were diagnosed with patellofemoral instability without associated cartilage lesions and five patients (25%) were diagnosed with both instability and cartilage lesions. At the time of TTO distalization, eleven patients (55%) underwent matrix-induced autologous chondrocyte implantation (MACI), nine patients (45%) underwent MPFL reconstruction, four patients (20%) underwent lateral release/lengthening, and three patients (15%) underwent osteochondral allograft (OCA) implantation. Fourteen patients (70%) underwent anteromedialization of the tibial tubercle in addition to distalization. Radiographic parameters demonstrated improved patellar height (CDI decreased from 1.36 to 1.11 (p &lt; 0.001), improved rotational alignment (axial TT-TG decreased from 14.3mm to 11.1mm (p = 0.018)) and anteriorization of the TT (sagittal TT-TG increased from -4.3mm to -1.4mm (p = 0.037)) following distalization TTO. Distance from the tibial plateau to the PT insertion decreased from 20.1mm preoperatively to 17.9mm postoperatively (p&lt; 0.020) and patellar tendon length decreased from 53.4mm preoperatively to 50.0mm postoperative (p &lt; 0.001). Average distalization was 6.4mm. IKDC scores increased from 37.1 to 60.5 (p = 0.011), KOOS QOL scores increased from 19.5 to 42.1 (p &lt; 0.001), Kujala scores increased from 50.9 to 70.6 (p = 0.018), VR12-MH scores increased from 52.7 to 59.2 (p = 0.024) and VR12-PH scores increased from 35.1 to 41.7 (p = 0.009). Of the patients with preoperative patellofemoral instability, there were no cases of recurrent instability postoperatively. Ten patients (50%) underwent elective removal of hardware of TTO screws at an average of 10.7 months. There were four (20%) complications in this cohort, with two cases (10%) of postoperative arthrofibrosis requiring manipulation under anesthesia, one case (5%) of postoperative MRSA infection of the MPFL graft and one case (5%) of tibial tubercle delayed union requiring bone grafting and revision fixation. Conclusions: Distalization TTO without patellar tendon tenodesis is an effective surgical technique for improving patellar height in patients with patella alta and patellofemoral instability and/or patellofemoral cartilage lesions, with improvements in radiographic parameters and PROs postoperatively. There were no cases of recurrent instability in this cohort. The patellar tendon insertion does not appear to be significantly distalized following distalization TTO, likely related to scarring of the patellar tendon just proximal to the osteotomy site postoperatively. Given the complexity of risk factors associated with patellofemoral instability and patellofemoral cartilage lesions, distalization TTO provides an additional tool for patient-specific surgical management, without the need for patellar tendon tenodesis.

Cartilage Restoration for Isolated Patellar Chondral Defects: An Updated Systematic Review.

Patellar chondral lesions can be particularly challenging to manage in younger and more active populations. To synthesize, organize, and summarize the results and complication rates of various patellar cartilage restoration techniques. Systematic review; Level of evidence, 4. We performed this systematic review according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines using the Medline, Embase, Scopus, and Cochrane databases. Studies were included that reported on surgical treatment of patellar chondral defects with ≥5 patients and 12 months of follow-up data. Relevant demographic data from the included studies were extracted, and patient-reported outcome scores, visual analog scale for pain results, return-to-sport rate, complications, and concomitant procedures were documented. There were 24 studies that met the inclusion criteria, with a total of 575 patients (male, n = 239; female, n = 336). In total, 6 surgical techniques were utilized. In 9 studies, the surgical procedure of choice was osteochondral autograft transplantation (OAT); 8 studies evaluated autologous chondrocyte implantation (ACI); 3 evaluated advanced microfracture/autologous matrix-induced chondrogenesis; 1 evaluated osteochondral allograft transplantation (OCA); 1 evaluated particulate juvenile articulated cartilage; and 2 evaluated a synthetic osteochondral graft. No uniform functional outcome score or assessment was utilized across studies. OAT was predominantly used for smaller chondral lesions (<2 cm2) and demonstrated minimal complication rates and satisfactory outcome scores. Advanced microfracture techniques showed promise, with improvement in outcome scores and zero complications. Matrix-induced ACI consistently exhibited higher mean improvement in the measured outcome scores and resulted in fewer complications when compared with previous generations of ACI. OAT and ACI were the most studied procedures for isolated patellar chondral defects. Advanced microfracture techniques showed promise, but indications (ie, size) and variability in techniques need to be elucidated in higher-level studies. Further prospective studies comparing OCA and matrix-induced ACI for larger patellar defects are necessary to determine the superior technique.

Rate of Conversion to Matrix-Induced Autologous Chondrocyte Implantation After a Biopsy: A Multisurgeon Study

Autologous chondrocyte implantation (ACI) and matrix-induced autologous chondrocyte implantation (MACI) are performed to treat focal chondral defects (FCDs); both are 2-step procedures involving a biopsy, followed by transplantation. There is little published research evaluating ACI/MACI in patients who undergo a biopsy alone. To determine (1) the value of ACI/MACI cartilage biopsies and concomitant procedures in patients with FCDs of the knee and (2) the conversion rate to cartilage transplantation as well as the rate of reoperation. Case series; Level of evidence, 4. A retrospective review was performed of 46 patients (63% female) who underwent a MACI (or ACI) biopsy between January 2013 and January 2018. Preoperative data, intraoperative data, and postoperative outcomes were assessed at a minimum of 2 years after the biopsy. The conversion rate from a biopsy to transplantation and the reoperation rate were calculated and analyzed. Among the 46 patients included, 17 (37.0%) underwent subsequent surgery, with only 12 undergoing cartilage restoration surgery, for an overall transplantation rate of 26.1%. Of these 12 patients, 9 underwent MACI/ACI, 2 underwent osteochondral allograft transplantation (OCA), and 1 underwent particulated juvenile articular cartilage implantation at 7.2 ± 7.5 months after the biopsy. The reoperation rate was 16.7% (1 patient after MACI/ACI and 1 patient after OCA) at 13.5 ± 2.3 months after transplantation. Arthroscopic surgery with debridement, chondroplasty, loose body removal, meniscectomy/meniscal repair, and other treatment approaches of knee compartment abnormalities at the time of a biopsy appeared to be sufficient in improving function and reducing pain in patients with knee FCDs.

Assessment of Patellar Vascularity after Patellar Cartilage Restoration via Lateral Parapatellar Approach: Analysis Using Dynamic Contrast-Enhanced Magnetic Resonance Imaging.

Patellofemoral cartilage restoration procedures, including osteochondral allograft, particulated juvenile cartilage, and matrix-induced autologous chondrocyte implantation, have been shown to be effective treatments for patellofemoral cartilage lesions. However, concerns exist regarding disruption of the patellar vascular supply and secondary stabilizers of the patellofemoral joint during medial parapatellar approaches, especially when combined with a lateral release. A lateral parapatellar approach affords the possibility of avoiding disruption of the medial blood supply to the patella, while also allowing laterally-based soft tissue stabilization procedures. The purpose of this study was to investigate in vivo changes in patellar vascularity following patellofemoral cartilage restoration procedures performed via a lateral parapatellar approach via use of dynamic contrast-enhanced magnetic resonance (MR) imaging. This study is a prospective case series of 5 adult patients undergoing patellofemoral cartilage restoration procedures via a lateral parapatellar approach with pre-operative and post-operative dynamic contrast-enhanced MR imaging to assess changes in patellar vascularity. Secondary outcomes included knee range of motion, need for revision surgery, and complications. There was no significant post-operative difference in patellar vascularity in patients undergoing patellofemoral cartilage restoration procedures via a lateral parapatellar approach, as evaluated by qualitative MR imaging. Our results suggest that a lateral parapatellar approach for cartilage restoration procedures may preserve patellar vascularity, while also allowing for lateral release to be performed through the same incision.

Mid- to Long-Term Clinical Outcomes of Cartilage Restoration of Knee Joint with Allogenic Next-Generation Matrix-Induced Autologous Chondrocyte Implantation (MACI).

Cartilage defect is a common pathology still lacking a unified treating option. The purpose of this retrospective study is to evaluate the safety, efficacy, and clinical and radiological outcome of cartilage restoration of knee joint with allogenic next-generation Matrix-Induced Autologous Chondrocyte Implantation (MACI) for the first time, as well as the correlation between postoperative clinical and radiological outcomes and preoperative patient history and demographics. From July 2014 to August 2020, 15 patients who went through cartilage restoration with allogenic next-generation MACI were included in this study. Patient demographics and PROM including the International Knee Documentation Committee (IKDC) subjective knee score, Lysholm score, Tegner Activity Scale (TAS), and Knee Injury and Osteoarthritis Outcome Score (KOOS) were obtained preoperatively, at 3, 6, 12months postoperatively and the last follow-up using an online questionnaire platform. MOCART 2.0 score was calculated at the last follow-up. Analysis of variance (ANOVA) was used to compare PROM pre- and post-operation, with two-tailed p < 0.05 defined as statistical significant. Pearson correlation coefficient was used to evaluate correlation between the PROM and MOCART 2.0 score at the last follow-up with patients demorgraphics. All patients were followed for an average of 66.47± 24.15months (range, 21-93). All patients were satisfied with the outcome of the surgery and no complication was reported at the end of the study. No significant improvement was observed until 1 year after the implantation, except for IKDC score at 6 months. All PROM showed significant improvement 1 year post-op except for Lysholm score and TAS, which also increased significantly at the time of the last follow-up. Pearson correlation coefficient showed that the size of the defect, before or after debridement, was significantly negatively correlated with final KOOS-Pain (before debridement: r=-0.57, p < 0.05; after debridement: r=-0.54, p < 0.05) and KOOS-Symptoms score (before debridement: r=-0.66, p < 0.05; after debridement: r=-0.67, p < 0.05). The MOCART 2.0 score was found significantly and negatively correlated with BMI (r=-0.60, p < 0.05), and significantly and positively correlated with Lysholm score (r=0.70, p < 0.05). The next generation MACI with autologous chondrocyte and allogenic chondrocyte ECM scaffold could be used to treat focal articular cartilage defect in the knee joint safely and efficiently with lasting promising outcomes for more than 5 years. The size of the defects should be considered the most negatively correlated parameters influencing the postoperative clinical outcomes.

Cell transplantation techniques for cartilage restoration

BackgroundFocal articular chondral lesions are a common finding among patients presenting with knee pain. If symptomatic and unresponsive to conservative treatment, cell transplantation techniques offer a unique solution to address larger defects by engineering chondrocytes to integrate within the subchondral bone of the lesion and regenerate cartilage. ObjectivesThe purpose of this article is to review the evaluation of, and clinical decision making for, patients being considered for cell transplantation and the available techniques an orthopaedic surgeon has at their disposal. MethodsA review of recent literature regarding cartilage defects in the knee and cell transplantation techniques was performed to provide strategies for evaluating and treating chondral defects with matrix-induced autologous chondrocyte implantation (MACI) or particulated juvenile allograft cartilage (PJAC) procedures. ResultsChondral defects in the knee can be treated with patients’ own chondrocytes embedded into a collagen membrane as a MACI procedure, or minced autologous donor cartilage that is then implanted onto a scaffold as a PJAC procedure. These cell transplantation techniques offer advantages compared to bone marrow stimulation or mosaicplasty, and have shown clinically significant improvements in outcome scores with low rates of complications. ConclusionCell transplantation techniques such as MACI and PJAC offer treatment options that can effectively address large full-thickness chondral defects in the tibiofemoral joint or patellofemoral joint that may respond poorly to bone marrow stimulation or mosaicplasty.
