Objective. To characterize pregnant women of advanced maternal age and, in an exploratory manner, assess the factors associated with the presentation of complications in this population, at a Peruvian maternal institute in 2022. Materials and Methods. onducted at the National Maternal Perinatal Institute during the year 2022. A total of 191 medical records that met the inclusion and exclusion criteria were reviewed. Analyses were subsequently performed using IBM SPSS software, version 25.0. Descriptive and bivariate statistical analyses were carried out using the Chi-square test. Crude prevalence ratios with their 95% confidence intervals were also calculated using Poisson regression with robust variances. Results. It was found that the prevalence of cesarean sections was 87% among elderly pregnant women, with the main indications for cesarean being severe preeclampsia at 25.9%, the most frequent pathology being intrauterine growth restriction (IUGR) at 24.7%, and fetal distress syndrome (FDS) at 23.5% in the patients. The bivariate analysis showed a statistically significant association between severe preeclampsia and gestational age (RRa: 0.881; 95% CI: 0.837 - 0.926). Conclusion. 20% of the pregnant women treated at a Peruvian institute had advanced maternal age.
