Intraoral Condylectomy with 3D-Printed Cutting Guide versus with Surgical Navigation: An Accuracy and Effectiveness Comparison.

This study compares the accuracy and effectiveness of our novel 3D-printed titanium cutting guides with intraoperative surgical navigation for performing intraoral condylectomy in patients with mandibular condylar osteochondroma (OC). A total of 21 patients with mandibular condylar OC underwent intraoral condylectomy with either 3D-printed cutting guides (cutting guide group) or with surgical navigation (navigation group). The condylectomy accuracy in the cutting guide group and navigation group was determined by analyzing the three-dimensional (3D) discrepancies between the postoperative computed tomography (CT) images and the preoperative virtual surgical plan (VSP). Moreover, the improvement of the mandibular symmetry in both groups was determined by evaluating the chin deviation, chin rotation and mandibular asymmetry index (AI). The superimposition of the condylar osteotomy area showed that the postoperative results were very close to the VSP in both groups. The mean 3D deviation and maximum 3D deviation between the planned condylectomy and the actual result were 1.20 ± 0.60 mm and 2.36 ± 0.51 mm in the cutting guide group, and 1.33 ± 0.76 mm and 4.27 ± 1.99 mm in the navigation group. Moreover, the facial symmetry was greatly improved in both groups, indicated by significantly decreased chin deviation, chin rotation and AI. In conclusion, our results show that both 3D-printed cutting-guide-assisted and surgical-navigation-assisted methods of intraoral condylectomy have high accuracy and efficiency, while using a cutting guide can generate a relatively higher surgical accuracy. Moreover, our cutting guides exhibit user-friendly features and simplicity, which represents a promising prospect in everyday clinical practice.

Clinical Application of Individualized 3D-Printed Templates in the Treatment of Condylar Osteochondroma.

Osteochondroma (OC) is one of the most common benign tumors of the long bones, but it rarely occurs in the maxillofacial skeleton. However, mandibular condylar OC often leads to severe facial deformity in affected patients, including facial asymmetry, deviation of the chin, and malocclusion. This study aimed to explore the clinical application of individualized 3D-printed templates to accurately and effectively treat condylar OC. A total of 8 patients with mandibular condylar OC were treated from July 2015 to August 2021. The enrolled patients (5 women and 3 men) had a median age of 27 years (range: 21-32 years). All patients exhibited symptoms of facial asymmetry and occlusal disorders preoperatively. The digital software used to virtually design the process consisted of three-dimensional reconstruction, 3D-cephalometry analysis, virtual surgery, individualized templates, and postoperative facial soft-tissue prediction. A set of 3D-printed templates (DOS and DOT) were used in all cases to stabilize the occlusion and guide the osteotomy. Then, pre- and post-operative complications, mouth opening, clinical signs, and the accuracy of the CT imaging analysis were all evaluated. All the measurement data were presented as means ± SD; Bonferroni and Tamhane T2 multiple comparison tests were used to examine the differences between the groups. All patients healed uneventfully. None of the patients exhibited facial nerve injury at follow-up. In comparing the condylar segments with T0p and T1, the average deviation of the condylar segments was 0.5796 mm, indicating that the post-operative reconstructed condyles showed a high degree of similarity to the reconstruction results of the virtual surgical plan. Individualized 3D-printed templates simplified surgical procedures and improved surgical accuracy, proving to be an effective method for the treatment of patients with slight asymmetric deformities secondary to condylar OC.

Evaluation of Mandibular Symmetry in Patients With Condylar Osteochondroma Who Underwent Intro-Oral Condylar Resection and Simultaneous Bimaxillary Orthognathic Surgery.

Mandibular condylar osteochondroma (OC) can result in facial asymmetry, malocclusion, and temporomandibular joint dysfunction. The authors have previously demonstrated a novel method for conservative condylectomy and simultaneous orthognathic surgery for treatment of mandibular condylar OC. The purpose of this study was to evaluate the immediate improvement and long-term stability of mandibular symmetry in the treatment of condylar OC. Fifty-six patients with unilateral mandibular condylar OC combined with secondary facial asymmetry and malocclusion were enrolled in this retrospective study. The computerized tomography (CT) scans were acquired with the mandible in centric relation (CR) before surgery, 1 week and 12 to 18 months after surgery. The images were reconstructed and processed for the analysis with ProPlan CMF 2.1 software. After defining the skeletal landmarks and the reference planes, the chin deviation, chin rotation and mandibular asymmetry index were calculated. The operations and healing were uneventful and the patients showed no signs of recurrence or temporomandibular joint ankylosis during the follow-up. Facial symmetry was greatly improved right after the surgery in the chin deviation (from 9.2 to 1.7 mm, P < 0.01), chin rotation (from 11.2 to 2.3, P < 0.01) and the asymmetry index of three mandibular landmarks (Go, MF and Sg, all P < 0.01). It also showed a stable result after 12 to 18 months follow-up. We also established a novel measurement method, which showed that the combination of conservative condylectomy via the intraoral approach based on intraoperative navigation and simultaneous orthognathic surgery is effective for improving the facial symmetry when treat the mandibular condylar OC.

Imaging of osteochondroma of the temporomandibular joint – Report of two cases

Osteochondroma of mandibular condyle is an uncommon entity. In the temporomandibular region, they usually present as restriction in mouth opening and pain in the joint. They can grow to a large size to impinge upon the cranial fossa. We present the imaging features of two such rare cases, one was giant condylar osteochondroma and the other one was condylar osteochondroma associated with contralateral osteochondroma of the zygomatic process of the temporal bone and congenital block cervical vertebrae. Computed tomography, magnetic resonance imaging of the temporomandibular joint and bone scan findings are discussed in detail along with the follow-up of patients.

Histopathological features of condylar hyperplasia and condylar Osteochondroma: a comparison study

BackgroundBoth mandibular condylar hyperplasia and condylar osteochondroma can lead to maxillofacial skeletal asymmetry and malocclusion, although they exhibit different biological behavior. This study attempted to compare the histological features of mandibular condylar hyperplasia and condylar osteochondroma using hematoxylin-and-eosin (H&E) staining, and immunohistochemistry staining of PCNA and EXT1 with quantitative analysis method.ResultsThe H&E staining showed that condylar hyperplasia and condylar osteochondroma could be divided into four histological types and exhibited features of different endochondral ossification stages. There was evidence of a thicker cartilage cap in condylar osteochondroma as compared condylar hyperplasia (P = 0.018). The percentage of bone formation in condylar osteochondroma was larger than was found in condylar hyperplasia (P = 0.04). Immunohistochemical staining showed that PCNA was mainly located in the undifferentiated mesenchymal layer and the hypertrophic cartilage layer, and there were more PCNA positive cells in the condylar osteochondroma (P = 0.007). EXT1 was mainly expressed in the cartilage layer, and there was also a higher positive rate of EXT1 in condylar osteochondroma (P = 0.0366). The thicker cartilage cap, higher bone formation rate and higher PCNA positive rate indicated a higher rate of proliferative activity in condylar osteochondroma. The more significant positive rate of EXT1 in condylar osteochondroma implied differential biological characteristic as compared to condylar hyperplasia.ConclusionsThese features might be useful in histopathologically distinguishing condylar hyperplasia and osteochondroma.

Digital cutting guide and endoscopically-assisted vertical ramus osteotomy to treat condylar osteochondroma: a long-term study

We have introduced an effective treatment for mandibular condylar osteochondroma with a digital cutting guide and endoscopically-assisted vertical ramus osteotomy (VRO). Eleven patients with unilateral condylar osteochondroma, who did not require orthognathic surgery or had less than 3mm deviation of the chin and a stable occlusion, were treated during the period April 2013–January 2017 with a digital cutting guide and endoscopically-assisted VRO. Clinical data collected included the occlusion, facial contour, and maximum mouth opening (MMO). Computed tomographic (CT) scans were taken before and after operation. Two patients also had additional shaping of the mandibular contour. The pathological diagnosis was confirmed to be osteochondroma in all cases. A mean (range) 19 (12–40) months of follow-up for all 11 cases showed stable postoperative occlusion and facial aesthetics. There were no functional disturbances, recurrence, or condylar absorption. VRO is an alternative to orthognathic surgery for patients with osteochondroma who do not have severe malocclusions. The digital cutting guide and endoscopically-assisted VRO make it possible to achieve precise resection of the tumour and maintain the occlusion with minimal invasion.

Comparison of computed tomography and magnetic resonance imaging for the detection of mandibular condylar osteochondroma*

Abstract Objective The purpose of this study was to compare computed tomography (CT) and magnetic resonance imaging (MRI) for the detection of mandibular condylar osteochondroma. Methods Preoperative CT and MRI of 33 patients with unilateral condylar osteochondroma were reviewed. The morphology, location, continuity with the parent bone, cartilage cap, perichondrium of tumors, and changes in soft and hard tissues adjacent to the lesions were investigated by two reviewers. Data were analyzed using McNemar test. A P value &lt; 0.05 was considered significant. Results Among the 33 condylar osteochondromas, 11 were of the diffuse type, 10 were of the sessile type, and 12 were of the pedunculated type. Continuity with the cortex and marrow of the host condyle was observed on both CT and MRI. Both modalities had identical detection rates of surface reconstruction of the temporal bone joint, condylar dislocation, and pseudarthrosis formation. However, MRI showed significantly higher detection rates of the cartilage cap and perichondrium than CT (P &lt; 0.05). Furthermore, MRI showed ipsilateral and contralateral temporo-mandibular joint (TMJ) disc displacement in 4 cases and 6 cases, respectively, and ipsilateral and contralateral TMJ effusion in 20 cases and 14 cases, respectively. Conclusion CT can intuitively display the morphology and spatial location of condylar osteochondromas through three-dimensional reconstruction. MRI may be superior to CT in the detection of cartilage cap, perichondrium of the condylar osteochondroma, and changes in the TMJ and adjacent soft tissues.

Dentofacial Deformity Caused by Bulky Osteochondroma: Report of an Unusual Case and the Importance of Cone Beam Computed Tomography

Introduction:Osteochondroma of mandibular condyle is a rare benign tumor.Case Report:This case report described clinical, radiographic features, differential diagnosis, histopathologic correlation and treatment of condylar osteochondroma.Conclusion:Conebeam computed tomography (CBCT) is an alternative modality to CT or MRI that should be performed in all cases of suspected osteochondroma of the mandibular condyle.

Ipsilateral sagittal split ramus osteotomy to facilitate reconstruction of the temporomandibular joint after resection of condylar osteochondroma

We report the outcomes of 12 patients with osteochondroma of the mandibular condyle who were treated by condylectomy with sagittal split ramus osteotomy (SSRO) between January 2011 and October 2015. Variables assessed before and after operation were imaging, appearance, maximum mouth opening, maximum mandibular protrusion, lateral excursion, and function of the temporomandibular joint (TMJ). Patients were followed up for a mean (range) of 21 (13 - 30) months. Outcomes were satisfactory with no complications or recurrence. Patients regained good occlusion and facial symmetry, and satisfactory function of the TMJ. Our results suggest that SSRO is a good option for condylar reconstruction after resection of mandibular condylar osteochondroma.

Establishment and application of a digital workflow in the treatment of condylar osteochondroma and secondary dentofacial deformities using image-guided endoscopic navigation combined with orthognathic surgery

Objective: To establish a digital workflow in the treatment of mandibular condylar osteochondroma with secondary dentofacial deformities using navigation and endoscope combined with orthognathic surgery. Methods: Thirty-six patients with unilateral condylar osteochondroma were analyzed retrospectively. Preoperative planning and simulation were carried out on the digital three-dimansional (3D) model in all patients. With the aid of image-guided endoscopic navigation, osteochondroma resection and condylectomy were accurately performed. Secondary dentofacial deformities were simultaneously corrected using orthognathic surgery. All patients were followed up regularly and received postoperative CT scans. The preoperative simulated model and the postoperative actual model were matched using ProPlan CMF 2.0 software. Four corresponding points were marked in the virtual and actual ostectomy plane, respectively. The intersections of mandibular sigmoid notch and posterior ramusrim with condylectomy plane were marked as the anterior point and the posterior point, respectively. The perpendicular bisector of the line from the anterior point to the posterior point was intersected with the lateral and medial margin of condylectomy plane to form the lateral point and the medial point, respectively. The straight-line distances between the corresponding points in the virtual and actual ostectomy plane were respectively measured to analyze the ostectomy discrepancy. Results: All of 36 patients obtained satisfactory clinical effects. Facial symmetry and morphology were greatly improved. Postoperative CT showed that condylar tumors were completely removed. The preoperative simulated model and the postoperative actual model were matched. The average discrepancy between the planned and actual surgical resection was minimal on the anterior points ([0.24 ± 0.17] mm) and the mean error was maximal on the posterior points ([3.86±1.03] mm). The patients showed no signs of tumor recurrence in the 6 to 12 months of follow-up. Conclusions: Endoscope-assisted and navigation-guided tumor resection and condylectomy combined with simultaneous orthognathic surgery has satisfactory clinical effects in the treatment of condylar osteochondroma and secondary dentofacial deformities. The digital management workflow reported in this paper provides us a valuable option for this potentially complicated procedure.

Mandibular condylar osteochondroma (OC) results in asymmetric prognathism with facial morphology and functional disturbances. The aim of this study was to explore the feasibility of computer-assisted surgical planning combined with intraoperative navigation in the treatment of condylar OC. Five patients with mandibular condylar OC were enrolled in this study. Surgical planning and simulation was performed based on a computed tomography reconstruction model using SurgiCase software. Under the guidance of navigation, a condylar OC resection and conservative condylectomy was carried out via intraoral approach. Simultaneous orthognathic surgery was used to correct the facial asymmetry and malocclusion. All patients healed uneventfully. No facial nerve injury or salivary fistula occurred. Facial symmetry and morphology were greatly improved and stable occlusion was obtained in all cases. Good matching between preoperative planning and postoperative results was achieved. Patients showed no signs of recurrence or temporomandibular joint ankylosis during follow-up of 12–30 months. Computer-assisted surgical planning and intraoperative navigation is a valuable option in the treatment of mandibular condylar OC.

Facial symmetry and morphology were greatly improved, and stable occlusion was obtained in all cases. The patients showed no signs of recurrence and temporomandibular joint ankylosis in the 16 to 54 months of follow-up. Endoscope-assisted tumor resection and condylectomy combined with simultaneous orthognathic surgery provide us a valuable option in the treatment of mandibular condylar OC.

Treatment of Osteochondroma in the Mandibular Condyle and Secondary Dentofacial Deformities Using Surgery Combined With Orthodontics in Adults

Osteochondroma is a benign tumor that is rare in the craniofacial region. When it does occur, the condyle and coronoid process have been the most commonly affected sites. The secondary progressive malocclusion and facial asymmetry will be common physical signs in most cases. The traditional surgical treatment of osteochondroma of the mandibular condyle has been condylectomy, with or without reconstruction of the condyle. We believe more attention should be paid to restoring joint function, improving the facial appearance, correcting malocclusion, and re-establishing harmony among them. From January 2000 to March 2013, 27 patients (17 women and 10 men) who had been diagnosed with osteochondroma of a unilateral mandibular condyle underwent condylectomy and condylar reconstruction using a pedicled posterior mandibular border obtained by ramus osteotomy. The secondary dentofacial deformities were simultaneously corrected using orthognathic and facial contouring procedures, followed by orthodontic treatment, when necessary. Occlusion and temporomandibular joint pain and function, including the maximal mouth opening and maximal protrusion, were recorded preoperatively and postoperatively. The patients were followed for an average of 13 months (range 24 to 48). The outcomes of, and feedback information from the patients showed apparent improved joint function, with no cases of osteochondroma recurrence. The secondary dentofacial deformities were corrected significantly. Satisfactory occlusion was achieved with orthognathic and orthodontic treatment. Our data suggest that condylectomy and condylar reconstruction, with simultaneous correction of the secondary dentofacial deformities using orthognathic procedures, might be a better approach to manage osteochondroma accompanied by dentofacial deformities. In addition to facial contouring procedures, orthodontic treatment should be considered for better improvement in facial esthetics and occlusion.

Low Condylectomy and Orthognathic Surgery to Treat Mandibular Condylar Osteochondroma: A Retrospective Review of 37 Cases

To evaluate the outcomes from surgical treatment of mandibular condylar osteochondroma (condylar hyperplasia [CH] type 2) using a specific surgical protocol. CH type 2 is a unilateral benign pathologic condition, with progressive proliferation of osseous and cartilaginous tissues in the condylar head. This causes condylar enlargement, often with exophytic growth, resulting in significant facial deformity, pain, and masticatory and occlusal dysfunction. This was a retrospective cohort study of 37 patients (28 females and 9 males), with an average age of 26.3 years (range 13 to 48), with CH type 2, and associated dentofacial deformity. The condylar pathologic features were confirmed by histologic analysis. All patients were treated with low condylectomy, recontouring of the condylar neck to form a new condyle, repositioning of the articular disc over the condylar stump and repositioning of the contralateral disc, if displaced, and any indicated orthognathic surgical procedures. Postoperative follow-up averaged 48 months (range 12 to 288). Patients were assessed preoperatively and at the longest follow-up point for incisal opening, lateral excursions, pain, jaw function, diet, disability, and occlusal and skeletal stability. The pre- and postoperative assessments were compared using paired t test. At the longest follow-up point, a nonsignificant decrease (2.3 mm) was seen in the maximum incisal opening; however, the excursive movements had decreased significantly an average of 2.5 mm on the right and 2.2 mm on the left. A statistically significant improvement was seen in pain, jaw function, diet, and disability. A stable Class I skeletal and occlusal relationship was maintained in 34 of the 37 patients (92%). Two patients developed relatively minor postoperative malocclusions that were managed with orthodontics. In 1 patient, a high condylectomy was performed, and the tumor continued to grow, causing malocclusion and jaw deformity to recur. A low condylectomy and sagittal split were performed 14 months later, with a stable result at 4 years after surgery. The results of the present study have demonstrated that a low condylectomy procedure with recontouring of the condylar neck to function as a condyle and repositioning of the articular discs, combined with orthognathic surgery, is a viable option for the treatment of osteochondroma of the mandibular condyle and associated jaw deformity.

Using Conservative Condylectomy for Management of a Large Osteochondroma of the Mandibular Condyle With 6-Year Follow-up

Osteochondroma is a hamartomatous proliferation of cartilaginous tissue, which is the most common benign tumor of the long bones, but is relatively rare in the maxillofacial region. Most cases of mandibular condylar osteochondroma manifest with facial asymmetry or malocclusion with limited temporomandibular joint movements. Several approaches for management of this lesion have been proposed, as conservative condylectomy technique. This procedure has been suggested a valid approach to minimize facial asymmetry, contributing to the recovery of occlusion associated with no local tumor recurrence, and without condylar reconstruction procedure. Therefore, this article aims to describe a clinical report of a true osteochondroma of the mandibular condyle in a 35-year-old patient who was successfully treated using conservative condylectomy procedure.

Osteochondroma / osteocartilagenous exostosis is a cartilage-capped exophytic lesion that arises from the cortex of bone. It constitutes 20 - 50% of all benign tumours and 10 - 15% of all bone tumours, but it is actually a developmental lesion rather than a true neoplasm. An osteochondroma in the maxillo- facial region, however, is rare. In this anatomical area, the tumour is most often reported in association with the coronoid process. Osteochondroma of the mandibular condyle is rare and only 37 documented cases have so far been reported in the English language literature. I report a case of osteochondroma of the mandibular condyle and describe the characteristic OPG and CT features of this condition. INTRODUCTION: Osteochondroma (OC) of the mandibular condyle is a relatively rare condition that causes a progressive enlargement of the condyle, usually resulting in facial asymmetry, temporomandibular joint (TMJ) dysfunction, and malocclusion. Radiographically, there is a unilaterally enlarged condyle usually with an exophytic outgrowth of the tumor from the condylar head. I present a case of a left mandibular condylar OC that created a malocclusion, and TMJ Dysfunction. lesion was fairly well circumscribed with thin cortex. Left mandibular condyle was not seen separately. The patient was adviced CT scan for further evaluation. Saggital and coronal CT images revealed a large globular, mixed bone density condylar mass arising from left mandibular condyle (Figures 2 and 3). The lesion was extending superiorly into the articular fossa, but the cortical bone of the fossa was intact. The cortex of the tumor was continuous with that of the normal condylar bone. The pattern of the trabecular bone of the tumor blended with the intramedullary host bone. There was an evidence of pseudoarthosis formation with greater wing of sphenoid bone with thinning of greater wing. The lesion was reaching up to pterygoid process with compression and thinning of pterygoid process. Mandibular condyle was displaced laterally by the lesion. Tumor excision was performed. Histopathologic examination revealed a nodular lesion having cartilaginous cap and mature bone tissue beneath, consistent with osteochondroma. DISCUSSION: Osteochondroma is a benign tumour of the appendicular skeleton arising from the metaphyseal regions of long bones, most commonly seen in the second and third decade of life. Osteochondromas are not common in the mandibular condylar region. Review of the literature indicates that patients with these tumours present mainly in the fourth decade with mean age of 39.7 years and a male to female ratio of 1:1.5. Osteochondroma of mandibular condyle (OMC) is the most common benign lesion of the temporomandibular joint that causes growth of the mandibular condyle, ramus elongation and secondary facial dismorfosis. The diagnosis must be objectified clinical and histopathology, remove the tumor, reevaluate its sequel and plan surgical management due to their diagnosis. We present 2 cases, women, 29 and 50 which debuted consulting for moderate pain in the temporomandibular joint affected. Both have facial asymmetry lateromentonismo, elongation of ramus posterior contralateral overbite and canting of the occlusal plane, with 2 and 3 years of evolution estimated without lymphadenopathy and without collateral diseases. After completing the clinical trial imaging (CT, scintigraphy) and laboratory tests proceed with surgical management deferred to objectify histologic diagnosis (condylectomy mandibular and meniscopexia). After the re-evaluation of the patient and the sequel to the first surgical (condylar stump status, tumor control without recurrence, evaluation healthy condyle, and the new mandibular position) sets the new diagnostics and new therapeutic arises: occlusal stabilization (with prosthetics or orthodontics) with or without orthognathic surgery (to return the skeletal balances subsequent airway expand and improve the aesthetics). In our cases after condylectomy, a patient underwent prosthetic occlusal stabilization and other orthognathic surgery was necessary. After a year of monitoring both patients are in excellent condition. The objective diagnosis is the basis of our surgical management, obtaining diagnostic accuracy (inability quick biopsy) and a more conservative replanning after condylectomy.

Endoscope-assisted conservative condylectomy in the treatment of condylar osteochondroma through an intraoral approach

Mandibular condylar osteochondroma (OC) can result in morphological and functional disturbances, including facial asymmetry and temporomandibular joint (TMJ) dysfunction. The aim of this study was to explore the feasibility of endoscope-assisted tumour resection and conservative condylectomy via an intraoral approach. Seven patients with condylar OC were enrolled in this study. Endoscope-assisted tumour resection and conservative condylectomy were achieved intraorally, and no conventional extraoral incision was needed. Direct vision of the magnified and illuminated operative field was realized with the assistance of an endoscope. No facial nerve injury or salivary fistula occurred in any patient. Stable occlusion was realized through postoperative orthodontic treatment. The patients showed no signs of tumour recurrence or TMJ ankylosis during follow-up (range 18–43 months). Endoscope-assisted condylar OC resection and conservative condylectomy via intraoral approach offers great advantages with no significant complications compared with conventional extraoral incisions. The endoscope provides us with a valuable treatment option for this potentially complicated procedure

Osteochondroma of mandibular condyle: a 5-year follow-up

Introduction: Osteochondroma (OC) is one of the most common benign tumours of the axial skeleton, but is rarely found in the facial bones. There is debate whether this represents a true tumour or an exostosis. Its clinical presentation in mandibular condyle is usually with a combination of preauricular pain, mandibular dysfunction and facial asymmetry.
