A retrospective analysis of the patient journey for children with an unerupted maxillary central incisor.

Maxillary central incisors (MCI) are the third most impacted teeth. Timely multidisciplinary management is indicated as unerupted incisors can cause functional- and appearance-related distress. To assess the patient journey for children with unerupted MCI, including referral, clinical assessment, surgical management and follow-up treatment, and highlight areas for improvement, as well as identify factors impacting orthodontic treatment burden (OTB). A retrospective service evaluation of children aged 6-16 years who had surgical management of unerupted MCI under general anaesthetic (GA) between 2018 and 2021. Fifty-two children with 62 unerupted MCI were identified. Mean age at referral was 8.8 years. Most children (82.7%) had supernumerary teeth in the anterior maxilla. Mean time between listing for GA and surgery increased from 4.3 to 15.2 months following the COVID-19 lockdown. Mean age at surgery was 10.4 years. Supernumerary removal and incisor exposure and bond was the most common treatment (56.6%). Most patients required hospital orthodontic treatment post-surgery (65.4%). Most children referred were below 9 years (60.1%); these children had reduced OTB compared to those who had been referred at an older age. Stage of root development was also a significant predictor in OTB. An unexpected finding was that conical supernumeraries were found to prevent MCI eruption.

ENDODONTIC MANAGEMENT OF CALCIFIC METAMORPHOSIS IN MAXILLARY CENTRAL INCISOR: A CASE REPORT

Calcific metamorphosis is a pulpal reaction to dental trauma where the root canal gets obliterated by the deposition of hard tissue within the root canal space. Calcified root canal possesses a great challenge to the endodontist as negotiating such canal is very much difficult and tiring job along with risk of iatrogenic root perforation. Negotiation of root canal during endodontic treatment is very much crucial for its success as it could affect the cleaning, shaping, disinfection and finally 3 dimensional obturation of the root canal. This case report demonstrates the clinical management of maxillary central incisor with calcified canal in a 28 years old female patient with a history of trauma 2 years back. The preoperative radiograph shows complete obliteration of the canal up to coronal 2/3rd of the root canal along with widening of the periodontal ligament space. The only way to locate and negotiate such calcified canal was to remove the calcified pulp tissue from the canal while staying centered within the root during negotiation. Control radiograph was taken to rule out any deviation from the root’s centre during negotiation. Once the calcified tissue from the coronal 3rd of the root was removed using fine ultrasonic instruments, a #08 C+ file in a watch winding motion along with alternate use of 5.25% sodium hypochlorite and 17% EDTA was then used to negotiate the remainder of the canal. After negotiation of the calcified canal of tooth #21, it was then successfully cleaned, shaped and obturated till the apical terminus.

Misdiagnosis or Missed Diagnosis? Cone-Beam Computed Tomography-Aided Multidisciplinary Management of Maxillary Central Incisor with Palatogingival Groove

Palatogingival groove is a developmental anomaly that is most often seen on maxillary incisors, with a predilection for lateral incisors. It begins in the central groove and can extend to varying lengths and depths into the root. This groove can communicate with the pulp internally and periodontium externally, thereby establishing a milieu for the development of complex endodontic-periodontic lesion. In this article, we report multidisciplinary management of a maxillary central incisor, which was mistaken to be a vertical root fracture instead of a palatogingival groove. We also discuss the significance of early recognition of this lesion, differential diagnosis, and it's interdisciplinary management with the aid of cone-beam computed tomography.

Management of two-rooted maxillary central and lateral incisors: A case report with a multidisciplinary approach involving CAD/CAM and CBCT technology

A thorough knowledge of the root morphology and variations closely relates with the success of endodontic therapy. Although it is rare, additional roots or canals may exist in maxillary incisors, which is an important variation to consider. This paper describes the multidisciplinary management of a maxillary central incisor and a lateral incisor, both of which presented two roots with aberrant crown morphology that was verified by cone beam computed tomography and restored with prosthetic rehabilitation involving full-contour monolithic zirconia crown after root canal treatment.

Management of maxillary central incisor with an extracanal and periapical cyst using cone-beam computed tomography as a diagnostic aid

The success of endodontic treatment depends on the knowledge of internal and external anatomy of the tooth, the normal supporting tissues of the tooth and any pathology related to it. The a im of the present study is to present the management of maxillary incisor with two canals and periapical lesion. The presence of two canals in maxillary central incisor was confirmed with cone-beam computed tomography. The tooth was cleaned and shaped with hand filing and obturated with lateral condensation technique. Periapical lesion was treated with surgical approach. 1-, 3 and 9-month follow-ups showed good healing of periapical tissue and suffi cient bone formation. The patient remained asymptomatic. Sufficient knowledge of the normal root canal anatomy and its variations, and proper treatment planning aids in successful endodontic treatment.

Extraction socket preservation followed by ovate pontic site preparation and prosthesis for emergence profile in esthetic region

After tooth extraction, the alveolar ridge will commonly decrease in volume and change morphologically. The reasons for socket preservation, there seems to be a consensus that sufficient alveolar bone volume and favorable architecture of the alveolar ridge are essential to achieve ideal functional and esthetic prosthetic reconstruction and also provide esthetically pleasing restorations to our patients without violating the predictability and function of those prostheses. Grafting is one of the most common methods of socket preservation widely used to prevent the collapse of oral tissues. The present case report describes the management of maxillary central incisors, which is vertically fracture using socket preservation using hydroxyapatite bone graft BioGraft (BioGraft HABG Active, manufacturer. IFGL ltd. Kalunga Industrial Estate. U.K) and platelet-rich fibrin followed by ovate pontic site preparation giving emergence profile in esthetic region.
