Management of Feeding Problems in Infants with Cleft Palate and Review on Pierre-Robin Syndrome

Pierre Robin syndrome is a congenital disorder with triad of features micrognathia, glossoptosis which result in airway obstruction and feeding difficulties and cleft palate. Cleft palate is the most common congenital anomalies of craniofacial region seen in Pierre-Robin syndrome. Infants with cleft palate have to face various problems. Difficulty in feeding is the most common problem faced by cleft palate neonates that made them difficult to maintain adequate nutrition, result in failure to thrive. There are many methods given in literature to overcome this problem including special type of nursing bottles. Feeding Problems and Long-Term Outcomes in Preterm Infants—A Systematic Approach to Evaluation and Management

Preterm infants are known to have long-term healthcare needs. With advances in neonatal medical care, younger and more preterm infants are surviving, placing a subset of the general population at risk of long-term healthcare needs. Oral feeding problems in this population often play a substantial yet under-appreciated role. Oral feeding competency in preterm infants is deemed an essential requirement for hospital discharge. Despite achieving discharge readiness, feeding problems persist into childhood and can have a residual impact into adulthood. The early diagnosis and management of feeding problems are essential requisites to mitigate any potential long-term challenges in preterm-born adults. This review provides an overview of the physiology of swallowing and oral feeding skills, disruptions to oral feeding in preterm infants, the outcomes of preterm infants with feeding problems, and an algorithmic approach to the evaluation and management of neonatal feeding problems.

Investigation of feeding problems in children with cerebral palsy

Introduction: Eating and swallowing are important for child health and play an essential role in her/his physical, social, emotional and cultural development. In children with cerebral palsy because of neuromuscular disorders, there is high prevalence of feeding and swallowing disorders. Early diagnosis, prevention and management of feeding problems are important process. In this study, feeding problems in different types of cerebral palsy were investigated. Materials and methods: In this cross-sectional descriptive study, 60 children with cerebral palsy that referred to clinical centers were selected using random sampling. Feeding and swallowing skills of these children were investigated with using of Pediatric assessment scale for severe feeding problems. Results: Findings indicated that children with spastic and flaccid cerebral palsy have feeding problems with similar severity. Also children with athetoid cerebral palsy showed lowest severity of feeding problems. Conclusion: Swallowing performance and proper feeding skills plays an important role in children's development. Given the high prevalence of feeding problems in children with cerebral palsy, early diagnosis and intervention should be done to prevent of secondary problems. Food allergy competencies of dietitians in the United Kingdom, Australia and United States of America.

BackgroundA knowledgeable and competent dietitian is an integral part of the food allergy multidisciplinary team, contributing to effective diagnosis and management of food allergic disorders. Little is currently known about the food allergy training needs and preferences of dietitians. The purpose of this paper is to measure and compare self-reported food allergy competencies of dietitians based in the UK, Australia and USA.MethodsA survey of USA-based paediatric dietitians was developed to measure self-reported proficiency and educational needs in the area of food allergy. The survey was modified slightly and circulated online to paediatric and adult dietitians in the UK and Australia. Descriptive statistics and Pearson correlations are presented.ResultsA total of 797 dietitians completed the questionnaire. Competency in “developing food challenge protocols” and “managing feeding problems” were rated the poorest overall across all three settings. A higher level of competency was significantly positively associated with length of practice as a dietitian, percentage of caseload composed of patients with food allergy and training in food allergy. The most popular topics for further training were food additives, pharmacological reactions and oral allergy syndrome.ConclusionsThere is a need amongst dietitians to increase their knowledge in different aspects of food allergy diagnosis and management, specifically the areas of developing food challenge protocols and management of feeding problems. Parental experiences of feeding problems in their infants with Down syndrome.

Although feeding difficulties in infants with Down syndrome are described in the literature, the experiences of parents regarding the feeding problems of their infants are largely omitted. In order to promote closer collaboration with families and speech-language therapists in early communication intervention, the study investigated some experiences of a group of parents concerning the feeding problems of their infants with Down syndrome. A descriptive survey approach was implemented to collect quantitative data. The results revealed the type of feeding problems and associated conditions occurring in the infants of the twenty participants; the type of feeding methods used; the assistance received during the feeding problems; the emotions experienced in relation to the feeding problems in their infants as well as suggestions made by the participants regarding the management of early feeding problems. Numerous feeding problems occurred in the infants and the variety of emotions experienced by the participants indicated that feeding therapy and counseling were required. As a result of the different conditions underlying the feeding problems of the participants' infants, a comprehensive approach, which included therapeutic and medical intervention, was necessary. In order to provide effective early communication intervention services from birth, the results indicated that the knowledge, skills and sensitivity of speech language therapists to identify caregiver needs, to provide appropriate family-focused intervention and to make recommendations regarding the management of feeding problems in infants with Down syndrome is of great importance.

Feeding problems in young children: report of three cases and review of the literature

Summary Background: Feeding problems are common in young children. Standard procedures for identifying, assessing or treating these problems are not available at present. This paper describes three case reports of children attending a joint feeding clinic run by a dietitian and a clinical psychologist. The feeding problems included selective eating, slow eating, difficult behaviour at mealtimes and a preference for food of an inappropriate texture for the children's ages. Results: One of the three children improved after 4 months, one after 9 months, but the third child has not progressed after 2 years. Conclusion: A joint clinic can help the management of feeding problems by standardizing treatment. The management of these children should be audited and the use of a classification system and assessment tool would improve care. A parent support group should be established.
