Management of Early Post-Operative Complications of Esophageal Atresia With Tracheoesophageal Fistula: A Retrospective Study.

BackgroundEsophageal atresia (EA) with tracheoesophageal fistula (TEF) is a rare congenital malformation of the trachea and the esophagus. While the condition can result in a debilitating clinical picture, its outcomes have significantly ameliorated in recent times. The diminishing mortality associated with the disease can be attributed to a myriad of factors, including surgical advances, specialized anesthetic care, and categorical ventilator provision. These advances have resulted in increased survival rates even in premature infants who present with exceedingly low birth weights. Nevertheless, the mortality surrounding the condition still remains exceedingly high in some parts of the world, including the Middle East and Asia. The aim of the present study is to identify and outline the management of the postoperative complications that are intricately linked with soaring mortality rates.MethodsWe conducted a single-center retrospective study, three years in duration, of all the patients who were operated for esophageal atresia with tracheoesophageal fistula. The exclusion criteria included patients who died before the operation and those who were referred to other centers for management. The study evaluated several factors, including the various postoperative complications, their adept management, and the eventual outcomes. Data pertaining to the patient demographics, treatment, and radiological and laboratory findings was obtained and eventually analyzed using the Statistical Package for Social Sciences (SPSS) version 23.0 (IBM Corp., Armonk, NY, USA) software.ResultsThe present study included a total of 12 cases diagnosed in our hospital during the aforementioned study period. Of these patients, two patients (16.7%) died before operation because of associated severe congenital anomalies such as cardiac pathologies. Three patients were referred to other centers for management. These patients were excluded from our analysis. The remaining seven patients were included in our analysis. In our study, gastroesophageal reflux was the most common postoperative complication and was noted in six patients. Leakage of anastomosis was noted in two patients. Lung collapse was noted in merely one patient and was thus the least common complication. The overall mortality rate hovered around 28.6%.ConclusionsWhile most patients who are surgically managed for esophageal atresia with tracheoesophageal fistula develop postoperative complications, these complications are amenable to conservative management through the means of antibiotics, ventilator support, and total parenteral nutrition.

Redo-laparoscopy in the management of complications after laparoscopic colorectal surgery: a systematic review and meta-analysis of surgical outcomes.

The laparoscopic approach for colorectal surgery has gradually become widely accepted for the treatment of both benign and malignant diseases thanks to its several advantages over the open approach. However, it is associated with the same potential postoperative complications. Some recent studies have analyzed the potential role of laparoscopy in early diagnosis and management of complications following laparoscopic colorectal surgery. The aim of this systematic review was to investigate the outcomes of redo-laparoscopy (RL) for the management of early postoperative complications following laparoscopic colorectal surgery, focusing on length of stay, morbidity and mortality. A systematic review of the literature was performed according to the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines through MEDLINE (PubMed), Embase and Google Scholar from January 1990 to December 2019. The main outcomes examined were conversion rate, length of hospital stay, postoperative morbidity and mortality rates. A meta-analysis of all eligible studies was then conducted and forest plots were generated. A total of 19 studies involving 1394 patients who required reoperation after laparoscopic colorectal resection were included. In 539 (38.2%) of these patients, a laparoscopic approach was adopted. The most common indication for returning to the operating theater was anastomotic leakage (64.4% of all redo-surgeries, 67.7% of RL) and the most common type of intervention performed in RL was diverting stoma with or without anastomotic repair/redo (47.1%). Nine studies were included in the pooled analysis. The mean length of stay was significantly shorter in the RL group than in the redo-open one (WMD = - 0.90; 95% CI - 1.04 to - 0.76; Z = - 12,6; p < 0.001). A significantly lower risk of mortality was observed in the RL cohort (OR = - 0.91; 95% CI - 1.58 to - 0.23; Z = - 2.62; p = 0.009). Laparoscopy is a valid and effective approach for the treatment of complications following laparoscopic primary colorectal surgery thanks to it is well-established advantages over the open approach, which remain noticeable even in redo-surgeries.

Systematic review of the feasibility of laparoscopic reoperation for early postoperative complications following colorectal surgery

Returning to the operating theatre for management of early postoperative complications after colorectal surgery is an important key performance indicator. Laparoscopic surgery has benefits that may be useful in surgical emergencies. This study explored the evidence for the advantages of laparoscopic reoperation. A systematic review was performed to identify publications reporting the outcomes of laparoscopy as a mode of reoperation for the management of early postoperative complications of colorectal surgery. The main outcomes examined were 30-day mortality, 30-day morbidity, length of hospital stay, second reoperation rate, ICU admission and stoma formation at reoperation. After screening 3657 citations, ten non-randomized cohort studies were identified (1137 reoperations). Laparoscopic reoperation was equivalent to or better than open reoperation, with lower rates of 30-day mortality (0-4·4 versus 0-13·6 per cent), 30-day morbidity (6-40 versus 30-80 per cent), length of stay (mean(s.d.) 15·8(2·8) versus 29·1(14·5) days), ICU admission and duration of stay in the ICU. Anastomotic leak was the most common indication, after which more patients received a defunctioning loop stoma instead of an end stoma at laparoscopic than open reoperation. Laparoscopic reoperation is feasible in selected patients, with the advantages of improved short-term outcomes.

Laparoscopy for the Detection and Treatment of Early Complications from Minimally Invasive Urologic Surgery

To evaluate the role of laparoscopy for the detection and management of early postoperative complications after minimally invasive urologic surgery. From October 2003 to September 2008, data were prospectively collected for all patients needing surgical intervention within 21 days after urologic minimally invasive procedures. No patients operated on for a postoperative complication during this period were excluded. Minimally invasive surgical intervention was performed on all hemodynamically stable patients in whom pneumoperitoneum could be established safely. A total of 1962 laparoscopic or robot-assisted urologic procedures were performed. In 14 (0.7%) cases, surgical intervention was necessary for postoperative complications. Two patients underwent exploratory laparotomy because of abdominal distention and hemodynamic instability. Laparoscopic surgical intervention successfully diagnosed and treated the remaining 12 patients. There were no conversions to open surgery. No additional trocars were necessary apart from preexisting sites. Two (14%) patients had minor postexploration complications. Mean estimated blood loss was 70 mL (range 50-100 mL). The mean length of hospital stay after exploration was 2 days (range 5 hours-5 days). With surgical expertise, laparoscopic treatment of intra-abdominal complications after minimally invasive urologic procedures can be successfully and safely performed. The advantages of the minimally invasive approach may be preserved.

Cost‐comparison of Laparoscopic and Open Surgery for Mid or Low Rectal Cancer after Preoperative Chemoradiotherapy: Data from a Randomized Controlled Trial

The purpose of the present study was to compare the direct costs of laparoscopic surgery (LS) and open surgery (OS) in the treatment of mid or low rectal cancer after preoperative chemoradiotherapy in patients in Korea. The records of 130 LS patients and 125 OS patients were reviewed. Hospital stay after surgery and overall complication rates within three months of surgery were not significantly different. The LS group had significantly higher median costs than the OS group ($7,467.30 vs. $5,667.00; P < 0.001). The median hospital costs during hospitalization for surgery were higher in the LS group ($7,436.60 vs. $5,626.60; P < 0.001), but hospital costs for management of early postoperative complications were similar. The higher direct costs of LS were mainly due to the more expensive consumables and equipment needed for LS. Further study is needed to determine whether the higher direct costs of LS for rectal cancer are balanced by advantages of LS over OS, such as better short-term outcomes and cosmetic effect.

Overview of the Perioperative Management of Lung Volume Reduction Surgery Patients

This article reviews management strategies that may improve the outcome of thoracic surgery and particularly lung volume reduction surgery (LVRS) in patients with severe emphysema. Maximal preoperative pharmacologic therapy includes bronchodilators and inhaled corticosteroids to attain peak lung function at the time of surgery. Nonpharmacologic measures include smoking cessation and pulmonary rehabilitation. Mechanical ventilation during the perioperative period should ensure adequate oxygenation, while avoiding dynamic hyperinflation. Keys to successful postoperative care include close monitoring while in the intensive care unit, early extubation, adequate pain control, chest physiotherapy and appropriate chest tube management. Aggressive management of early postoperative complications, including air leaks, respiratory failure, arrhythmias, and hemorrhage, can also be expected to improve outcomes.

Antitumor Necrosis Factor-α Therapy for Early Postoperative Recurrence of Gastrointestinal Behçet's Disease: Report of a Case

Behçet's disease is a chronic, relapsing, systemic inflammatory disease characterized by recurrent oral and genital ulcers, skin lesions, and ocular lesions. Gastrointestinal tract is occasionally involved in Behçet's disease, and the typical gastrointestinal lesions are punched-out, discrete mucosal ulcerations. Corticosteroids, 5-aminosalicylic acid derivatives, thalidomide, and immunomodulators have been used to treat Behçet's disease. Some patients with gastrointestinal Behçet's disease undergo surgical management because of the refractoriness to these medical measures or complications, such as perforation and bleeding. Even after successful surgery, postoperative complications, such as poor healing of the anastomosis site and postoperative ulcer recurrence, are so common that repeated operations are frequently required. However, there have been few reports addressing the most effective way to reduce and treat postoperative complications and recurrences. We describe a patient with Behçet's disease involving the distal ileum, who was treated with infliximab as rescue therapy of an unhealed anastomosis site and early recurrent ulcers after a distal ileocecectomy because of persistent bleeding. Abdominal pain from recurrent ulcers improved by the fifth day after the infliximab infusion and almost disappeared by the second week. An ileocolonoscopy performed 15 days after the infliximab infusion showed near complete healing of the recurrent ulcers. This report suggests that infliximab may be an effective novel therapy for the management of early postoperative complications and recurrences in gastrointestinal Behçet's disease.

Decade-long experience with surgical therapy of myasthenia gravis: early complications of 324 transsternal thymectomies

Background. We studied the incidences and evaluated the management of early postoperative complications after thymectomy for myasthenia gravis. Methods. During the period between 1987 and 1996, 324 thymectomies were performed through median sternotomy access under general anesthesia. Postoperative management was administered according to a standardized protocol of anticholinesterase medication, which was withdrawn for the 48 hours of obligatory postoperative mechanical ventilation. The mean age of patients was 34 years (range, 8 to 71 years). Results. One hundred forty-nine patients made an uneventful recovery; 104 patients had only minor complications, whereas 71 patients had major complications. The mortality rate was 0.6% (2 patients). The major surgical complications were recorded as sternal bleeding (1 patient) and sternal disruption (1 patient). The major general complications were recorded as tracheal stenosis (1 patient), pneumonia (3 patients), heart failure (1 patient), gastric hemorrhage (1 patient), and respiratory insufficiency (71 patients). Forty-six reintubations were performed on 40 patients and 19 tracheostomies (6%) were performed postoperatively. Conclusions. The excessive incidence of respiratory insufficiency and airway-associated morbidity was potentially related, at least partially, to prolonged mechanical ventilation and withdrawal of anticholinesterase medication. Earlier weaning of patients with revision of 48-hour withdrawal of anticholinesterase medication is necessary.

Of 1,703 patients undergoing arterial procedures during a three-year period, 81 (4.7%) required reoperation within 72 hours because of early complications. Thrombosis (58 patients) and hemorrhage (19 patients) were the most frequent complications encountered. Reoperation was successful in salvaging a successful outcome in the majority of patients after both hemorrhagic and thrombotic complications. The results seem to justify a continued aggressive approach to the management of early complications following vascular reconstructive procedures.

The Management of Early Lacrimal Tube Complications

The introduction of a closed loop of suture material through the Jones lacrimal tube, the DCR opening, and the nose at the time of surgery, facilitates the management of early postoperative complications in conjunctivo-dacrocystorhinostomy.
