PORT CHEMOTHERAPY IN CANCER PATIENT : OUR EXPERIENCE AND REVIEW

Port implant plays an important role in the management of cancer chemotherapy. A cancer patient needs long term chemotherapy, frequent intravenous medication, parenteral nutrition, blood transfusion and blood extraction for blood test. For many reasons peripheral lines are not available as patients have already received chemotherapy. The port is an excellent way to administer chemotherapy on a long term basis but it should be with at most care. It not only improves the quality of life in cancer patients but also reduces the burden on nursing care. Proper placement and care of the port implant is very important to avoid complications. Methodology: This is a retrospective study in which collected data of patients who received the chemotherapy for cancer through Chemo port at SLV cancer center, Coimbatore, India. Aims And Objectives: The main objective of this study is to analyze the complication and outcomes related to port implant. Results: The Chemoport has been implanted in 84 patients in a period of three year (2017-2020) in which solid malignancy cases are (n =76) and hematological malignancy cases are (n=8). Among the solid malignancies cases, Breast cancer (n=48) was the most common malignant disease followed by Ovarian cancer (n=12) and Lymphoma (n=8). Of the 84 patients in the study group, for the first 24 patients, we started chemotherapy on the eighth day of the port implant and for the next 60 patients we started chemotherapy on the first postoperative day of the port implantation. The various complications developed in this study group are given in the descending order as follows: Infection(n=4), septicemia(n=3),catheter blockage due to blood clot(n=3), subclavian vein thrombosis(n=1), port site skin necrosis(n=1), catheter kink(n=1), pneumothorax(n=1) and pulmonary embolism and death(n=1).

Risk Factors for Skin Toxicities Associated with Bendamustine-Based Chemotherapy in Patients with Non-Hodgkin Lymphoma.

Bendamustine plays an especially important role as a treatment for non-Hodgkin lymphoma (NHL). However, patients administered bendamustine alone or in combination with rituximab (BR) may experience drug-associated skin toxicities that can profoundly impact their health-related QOL through both physical discomfort and psychological distress. Moreover, worsening skin symptoms may lead to dose reduction or termination in the management of cancer chemotherapy. We retrospectively investigated patient backgrounds and pretreatment characteristics from medical records of NHL patients treated with bendamustine alone or BR therapy and identified predictive factors for skin toxicities at the start of chemotherapy. Patients were eligible for the study if they were 20 years older, diagnosed with NHL, and received bendamustine alone or BR therapy at the Department of Hematology, Kobe City Medical Center General Hospital, between April 1, 2011, and March 31, 2018. This study included 95 patients with newly diagnosed or refractory or relapsed NHL. Multivariate stepwise logistic regression analysis with backward selection revealed that baseline non-prior chemotherapy (odds ratio (OR), 15.72; 95% confidence interval (CI), 4.24-83.13, p < 0.001) was a significant factor influencing the occurrence of skin toxicity. Our results demonstrated that non-prior chemotherapy was a significant risk factor for skin toxicities in patients with NHL receiving bendamustine alone or BR therapy. No patient experience serious side effects of grade 3 or higher and that bendamustine is very useful as a first-line treatment.

Design, Biological Evaluation, and Molecular Modeling of Tetrahydroisoquinoline Derivatives: Discovery of A Potent P-Glycoprotein Ligand Overcoming Multidrug Resistance in Cancer Stem Cells.

P-Glycoprotein is a well-known membrane transporter responsible for the efflux of an ample spectrum of anticancer drugs. Its relevance in the management of cancer chemotherapy is increased in view of its high expression in cancer stem cells, a population of cancer cells with strong tumor-promoting ability. In the present study, a series of compounds were synthesized through structure modulation of [4'-(6,7-dimethoxy-3,4-dihydro-1 H-isoquinolin-2-ylmethyl)biphenyl-4-ol] (MC70), modifying the phenolic group of the lead compound. Among them, compound 5b emerged for its activity against the transporter (EC50 = 15 nM) and was capable of restoring doxorubicin antiproliferative activity at nontoxic concentration. Its behavior was rationalized through a molecular modeling study consisting of a well-tempered metadynamics simulation, which allowed one to identify the most favorable binding pose, and of a subsequent molecular dynamics run, which indicated a peculiar effect of the compound on the motion pattern of the transporter.

PSY2-2 - Role of Medical Oncologist in Local Medical Service and Medical Oncology Education

ABSTRACT Overall cancer treatment should be coordinated among a radiation oncologist, surgeon, and medical oncologist, where appropriate. Unfortunately, there were few medical oncologists in Japan mainly because of lack of the academic society like JSMO and divisions in medical schools and hospitals for medical oncology. Therefore, the multidisciplinary approach in treating cancers was inappropriate. Since 2006, more than 700 cancer drug therapy specialists (diplomates, Subspecialty Board of Medical oncology, JSMO) were fostered in Japan, and the medical oncologists with the diplomate are gradually distributed in local cancer core hospitals. The current role of the medical oncologists in the hospitals is diverse and it includes treatment and management of cancer chemotherapy, such as treatment of both common and rare advanced cancers, management of outpatient chemotherapy center, organization of the regimen review committee and tumor board, education of their medical staffs and patients etc. The expanding role of the medical oncologists in the management of cancer patients may contribute to the spread of standard cancer chemotherapy. However, the number of medical oncologist is still limited. Many challenges still exist, both within the hospital and in translating oncological, technological and societal change into the context of local medical society. Because the number of cancer patient is increasing in Japan, wide spread advances in oncology by oncology specialists is urgent matter. In this symposium, current and future roles of the medical oncologists in local cancer core hospitals and their role in medical oncology education should be discussed.
