Small cell lymphocytic variant of marginal zone lymphoma: A distinct form of marginal zone lymphoma derived from naïve B cells as a cutaneous counterpart to the naïve marginal zone lymphoma of splenic origin.

Abstract Primary cutaneous marginal zone lymphoma most commonly represents an indolent form of cutaneous B cell lymphoma. However, epidermotropic marginal zone lymphoma, blastic marginal zone lymphoma and B cell dominant variants without isotype switching can be associated with extracutaneous dissemination. The presumptive cell of origin is a post germinal center B cell with plasmacytic features. In the extracutaneous setting, however, a naive B cell origin has been proposed for a subset of marginal zone lymphomas, notably splenic marginal zone lymphoma. The author encountered 11 cases of atypical lymphocytic infiltration of the skin primarily occurring in older individuals with an upper arm and head and neck localization; there was a reproducible pattern of diffuse and nodular infiltration by small monomorphic-appearing B cells. Phenotypically, the infiltrate was one predominated by B cells exhibiting CD23 and IgD positivity without immunoreactivity for CD38 and there were either no plasma cells or only a few without light chain restriction. In cases presenting with a solitary lesion complete excision and/or radiation led to successful disease remission in all cases without recurrence or metastatic disease. Of three cases with multiple initial lesions, evidence of extracutaneous disease was seen in two cases and recurrence occurred in one case. No patients have died of lymphoma. Longer term follows up and additional cases are needed to determine if this subset of marginal zone lymphoma is associated with a worse prognosis.

Differentiation of Jessner's Lymphocytic Infiltration of the Skin from Various Chronic Cutaneous Lupus Erythematosus Subtypes by Quantitative Computer-Aided Image Analysis

Background: The distinction between chronic cutaneous lupus erythematosus and Jessner's lymphocytic infiltration of the skin is still not clear. Objective: To investigate the difference between chronic cutaneous lupus erythematosus and Jessner's lymphocytic infiltration of the skin in an objective and quantitative manner. Methods: Clinical photographs were examined for erythema quantification using two erythema parameters (a* and erythema dose), and histological images were assessed by computer-aided image analysis. Results: The difference between normal and erythematous skin was more prominent using erythema dose than a*, but no significant differences among the three diseases were observed. The correlation between inflammatory dermal cell infiltration and epidermal thickness, as well as that between erythema parameters and histopathological features, was not significant. Conclusion: This is the first attempt to use computer-aided image analysis for the differentiation of similar skin diseases. However, the differentiation between these diseases using quantitative computer-aided image analysis was still not possible.

INTRODUCTION: Jessner's lymphocytic infiltration of the skin (JLIS) is a skin condition of unknown aetiology characterized by erythematous papules and plaques located on the head, neck and upper back. The eruption resolves spontaneously after months or a few years but can recur for several years. A variety of empirical treatments has been tried with limited success. Here we report a case of jessners lymphocytic infiltration of skin successfully treated with hydroxychloroquine and prednisolone.

Erythema migrans arciforme et palpabile

A 39-year-old man presented with an annular clearly palpable erythema on the décolleté. Based on clinical and histopathological findings, palpable migratory arciform erythema was diagnosed. This skin condition is classified as a rare type of T-cell pseudolymphoma. It is still a matter of debate whether palpable migratory arciform erythema is a specific entity or a clinical variant of lymphocytic infiltration of the skin (Jessner-Kanof). Topical corticosteroids or oral antibiotics are generally used. In our patient, UV-A1 therapy led to a complete regression of the lesions.

We report the case of a 47-year-old woman, who presented multiple erythematous infiltrated papules and plaques interesting the face, the forearms and hands. Skin biopsy confirmed the diagnosis of Jessner’s lymphocytic infiltration of the skin. The evolution was favorable with antimalarial treatment.

Jessner lymphocytic infiltration of the skin is an uncommon skin disorder characterized by benign accumulations of lymphocytes by a “coat-sleeve”-like perivascular infiltration in the skin [1]. These small lesions are solid, pink or red in colour and appear on the face, neck and back. The lesions may remain unchanged for several years and then spontaneously disappear, leaving no scars [2]. Prognosis is good because lymphocytic infiltration of the skin may resolve spontaneously and require no treatment. Treatments include cosmetic camouflage, photoprotection, excision of small lesions, topical steroids, hydroxy chloroquine, intralesional steroids, systemic steroids and cryotherapy [3]. We report here on a case of Jessner lymphocytic infiltration associated with an atrophic scar due to cutaneous leishmaniasis from 5 years before.

Photodynamic therapy: new treatment for refractory lymphocytic infiltration of the skin

Lymphocytic infiltration of the skin (LIS) is a relatively uncommon skin condition, first described by Jessner and Kanof in 1953. LIS presents mainly on the face, in particular on the cheeks and earlobe, but also on the neck, upper trunk or proximal limbs of middle-aged adults. LIS is often resistant to treatment. Photodynamic therapy (PDT) refers to the process of applying a topical prodrug, such as 5-aminolaevulinic acid (ALA) or methyl 5-aminolaevulinic acid (MAL), followed by irradiation with visible or ultraviolet light. The prodrug is converted by target tissue to photoactive porphyrins, which lead to local tissue destruction when activated by light. We describe a 48-year-old woman with refractory LIS, who was treated with MAL-PDT.

Jessner's lymphocytic infiltration of the skin (LIS) is a chronic, benign T-cell infiltrative disorder, usually manifesting as erythematous papules or plaques on the face, neck and back. five patients presented LIS with numerous skin lesions on the face and back characteristic of this disease. Histological examination showed a lymphocytic infiltrate in the dermis without any modification of the epidermis. Direct immunofluorescent study was negative in all cases. Response to dermocorticoids proved inconsistent or negative in all patients. An excellent outcome was achieved in all five patients with pulsed dye laser. In one case, further skin lesions appeared at 1 year and responded to the same treatment. Following a single treatment session with 6-8 J/cm(2), three of five patients showed normal skin. Regression occurred in the other two cases after two to three sessions. Pulsed dye laser appeared to be the best treatment for Jessner-Kanof disease for three patients at 4-8 years of follow-up. only one case of Jessner-Kanof disease treated by pulsed dye laser has been reported. Pulsed dye laser has been used in cutaneous lupus and annular granuloma. Selective photothermolysis allows photocoagulation of dilated vessels. pulsed dye laser at 595nm could offer a valuable therapeutic alternative, and even a first-line treatment with no side effects.

Primary cutaneous T-cell lymphoma: Two rare presentations

Cutaneous T-Cell Lymphoma (CTCL) is a subset of non-Hodgkin's T-cell lymphomas which presents with malignant lymphocytic infiltration of the skin. The disease spectrum is characterized by clonal proliferation of helper T-cells of the CD4 phenotype in the skin. Mycosis Fungoides (MF), the commonest variant of CTCL, is characterized clinically by an indolent clinical course with subsequent evolution of patches, plaques andtumours and histologically by the infiltration of the epidermis by medium-sized to large atypical T-cells with cerebriform nuclei [1]. Although not very rare worldwide, it is commonest among Africans and relatively uncommon among Asians. Hypopigmented MF is a rare variant of patch stage MF in the dark skinned. Folliculotropic MF is an uncommon variant characterized by folliculotropic T-cell infiltrates with or without mucinous degeneration of the hair follicles.

Lymphocytic infiltration of the skin (Jessner-Kanof) but not reticular erythematous mucinosis occasionally represents clinical manifestations ofBorrelia-associated pseudolymphoma

Lymphocytic infiltration of the skin (LIS) and reticular erythematous mucinosis (REM) are characterized histologically by an inflammatory cutaneous lymphocytic infiltrate similar to the histological appearance of pseudolymphoma. To re-evaluate a large cohort of patients with the clinical and/or histological diagnosis or differential diagnosis of LIS and REM and to assess the evidence for infection with Borrelia. Sixty-nine cases of LIS and 34 cases of REM were retrospectively investigated. Haematoxylin and eosin sections were re-examined, and histological diagnoses were specified and confirmed by clinicopathological correlation. Evidence for Borrelia infection was assessed by immunohistochemistry and focus-floating microscopy (FFM). LIS appeared to serve as a collective term for two main clinicopathological reaction patterns: (i) (tumid) lupus erythematosus (LE) (32 of 69, 46%) and (ii) pseudolymphoma (31 of 69, 45%). Other diagnoses (five of 69, 7%) included polymorphic light eruption, arthropod bite reaction, spongiotic dermatitis, drug eruption and urticaria. Spirochaetes were detected by FFM in 24 of 31 (77%) cases with a pseudolymphomatous reaction, while all nonpseudolymphomatous reactions were negative. Of the cases initially considered as REM, 21 of 34 (62%) were classified as LE, four of 34 (12%) as pseudolymphoma (three of four positive for Borrelia), and five of 34 (15%) as other diagnoses (folliculitis, morphoea, seborrhoeic dermatitis, prurigo and arthropod bite reaction). The diagnosis of Borrelia-associated pseudolymphoma was made significantly more often in those cases where LIS was considered as initial differential diagnosis than REM (P < 0.05). LIS and REM seem to represent clinicopathological reaction patterns. Our results confirm that, after accurate clinicopathological correlation, most cases of both conditions constitute hidden variants of LE. Furthermore, LIS, in contrast to REM, frequently comprises pseudolymphomatous reactions including borrelial lymphocytoma.

Abstract While the majority of T cells expressing self-reactive T cell receptors (TCRs) are eliminated in the thymus during development, a fraction emerge into periphery. We have created the SFZ70 TCR transgenic mouse to better understand the in vivo behavior of these peripheral auto-reactive T cells. The SFZ70 TCR was cloned from the SFZ70 hybridoma, which was generated from a CD4+ T cell isolated from the skin-draining peripheral lymph nodes (PLN) of a scurfy mouse. The SFZ70 hybridoma responds specifically to dendritic cells (DC) isolated from the PLN of wild-type mice but not to DC isolated from other tissues, and therefore appears to recognize a cutaneous antigen. SFZ70+RAG1-/- mice contain both peripheral CD4+Foxp3+ regulatory T cells (Tregs) and CD4+Foxp3- T cells and do not exhibit inflammation in the skin or other organs. To examine whether SFZ70 Tregs are required for maintenance of peripheral tolerance, we generated SFZ70 mice lacking functional Tregs by crossing SFZ70 mice to scurfy mice (SFZ70+RAG1-/-Foxp3sf/Y). SFZ70+RAG1-/-Foxp3sf/Y mice develop severe autoimmune disease characterized by lymphocytic infiltration of the skin and liver. Additionally, CD4+Foxp3- T cells from these mice show increased proliferation and display a CD62LloCD44hi activated phenotype. Our results show that in the absence of Tregs, an individual scurfy T cell-derived TCR is capable of inducing multi-organ autoimmune disease.

A 41-year-old woman presented with a 3-year history of erythematous papules and plaques on both cheeks. The lesions were asymptomatic, without burning or pruritus, but cosmetically disfiguring. The patient denied onset or exacerbation of the eruption following sun exposure. Previous treatment included cryotherapy, pulsed-dye laser and potent topical steroids with little or no response. Skin examination revealed multiple erythematous infiltrated papules and plaques with no epidermal changes on both cheeks (Fig. 1A). She had a few hypopigmented scars after earlier laser- or cryotherapy. Comparison of histopathologic–clinical characteristics of Jessner's lymphocytic infiltration of the skin and lupus erythematosus tumidus: Multicenter study of 46 cases

We sought to identify criteria able to distinguish between Jessner's lymphocytic infiltration of the skin (JLIS) and lupus erythematosus tumidus (LET). The following characteristics were recorded in a retrospective, multicenter analysis of patients with JLIS and LET: clinical features (number, size, type, and localization of lesions; photosensitivity; extracutaneous signs), histologic findings, phototesting, lupus serology, treatment, and outcome. Available histologic slides were reviewed blinded to the initial diagnosis using a pre-established grid. Univariate analysis of data from patients with JLIS (15 women, 17 men; mean age: 35 years) and LET (13 women, one man; mean age: 31 years) showed the following significant (P < .05) differences: more frequent back involvement and annular lesions in JLIS, as opposed to female predominance, more frequent face involvement, and plaques in LET. Phototesting, especially ultraviolet B, induced lesions in 18 of 26 patients with JLIS and all 4 with LET. The blinded histologic review (33 samples) only found slight epidermal atrophy and focal thickened dermoepidermal junction more frequent and perivascular lymphocyte infiltrations less dense in LET. The two groups of patients reclassified according histopathologic features (18 LET and 11 JLIS) showed only slight clinical differences (more frequent nasal bridge lesions in LET and annular lesions in JLIS). The retrospective nature of the study is a limitation. JLIS and LET in this population showed more similarities than differences, supporting a continuous spectrum covering these two entities.

Lymphocytic infiltration of the skin (Jessner): a T-cell lymphoproliferative disease.

Recent studies have suggested that Jessner 's lymphocytic infiltration of the skin and lymphocytoma cutis both represent pseudo-B-cell lymphomas. We have now done immunohistochemical studies on frozen skin sections of six patients with lymphocytic infiltration of the skin. In all cases a predominance of T lymphocytes was observed and B lymphocytes were sparse or completely absent. These results indicate that Jessner 's lymphocytic infiltration of the skin is a T-cell lymphoproliferative disease which, unlike lymphocytoma cutis, cannot be regarded as a pseudo-B-cell lymphoma.

Could Jessner’s Lymphocytic Infiltrate of the Skin Be a Dermal Variant of Lupus Erythematosus? An Analysis of 210 Cases

Background and Objective: The objective of this study was to evaluate the relationship between Jessner’s lymphocytic infiltration of the skin (JLI) and lupus erythematosus (LE), which has been the subject of debate since its initial description in 1953. Material and Methods: This is a retrospective study including all patients with a histopathologically ascertained diagnosis of JLI performed at the Laboratoire d’Histopathologie Cutanée of the Strasbourg University Hospital between 1993 and 2003. Information about patient characteristics and follow-up data were retrieved between 2004 and 2005. Special attention was paid to features indicative of LE. Results: 210 consecutive patients (102 women and 108 men) with a mean age 42 years were diagnosed with JLI in the reference period. 175 patients (83%) had multiple lesions and 32 patients (15%) had only a single lesion at the time of diagnosis (data not available in 3 patients). The head, neck and upper part of the thorax were involved in 171 patients (81%). An annular or arciform configuration and/or arrangement were present in 111 patients (53%). Lesions consisted of red (100%) papules or plaques (98%). Mean follow-up was 4 years. Sixteen patients (7.6%) had proven LE. Only 2 patients (1%) developed >4 ACR criteria of systemic LE. Furthermore, 1 patient had antiphospholipid antibody syndrome and 2 patients had rheumatoid arthritis. Conclusions: This high frequency of patients with typical features of LE strongly argues that JLI could be a dermal variant of LE and not an autonomous entity. It might be the cutaneous marker of a subset of LE patients with excellent prognosis.

Thalidomide: dermatological indications, mechanisms of action and side-effects

Thalidomide was first introduced in the 1950s as a sedative but was quickly removed from the market after it was linked to cases of severe birth defects. However, it has since made a remarkable comeback for the U.S. Food and Drug Administration-approved use in the treatment of erythema nodosum leprosum. Further, it has shown its effectiveness in unresponsive dermatological conditions such as actinic prurigo, adult Langerhans cell histiocytosis, aphthous stomatitis, Behçet's syndrome, graft-versus-host disease, cutaneous sarcoidosis, erythema multiforme, Jessner-Kanof lymphocytic infiltration of the skin, Kaposi sarcoma, lichen planus, lupus erythematosus, melanoma, prurigo nodularis, pyoderma gangrenosum and uraemic pruritus. This article reviews the history, pharmacology, mechanism of action, clinical uses and adverse effects of thalidomide.
