Radar localization of breast and axillary lesions with SCOUT: a prospective single institution pilot study.

Wire-guided localization has been the mainstay of localization techniques for non-palpable breast and axillary lesions prior to excision. Evidence is still growing for relatively newer localization technologies. This study evaluated the efficacy of the wireless localization technology, SCOUT®, for both breast and axillary surgery. Data were extracted from a prospective database (2021-2023) of consecutive patients undergoing wide local excision, excisional biopsy, targeted axillary dissection, or axillary lymph node dissection with SCOUT at a high-volume tertiary centre. Rates of successful reflector placement, intraoperative lesion localization, and reflector retrieval were evaluated. A survey of surgeon-reported ease of lesion localization and reflector retrieval was also evaluated. ACTRN386751. One-hundred-ninety-five reflectors were deployed in 172 patients. Median interval between deployment and surgery was 3 days (range 1-20) and mean distance from reflector to lesion was 3.2 mm (standard deviation, SD 3.1). Rate of successful localization and reflector retrieval was 100% for both breast and axillary procedures. Mean operating time was 65.8 min (SD 33). None of the reflectors migrated. No reflector deployment or localization-related complications occurred. Ninety-eight percent of surgeons were satisfied with ease of localization for the first half of cases. SCOUT is an accurate and reliable method to localize and excise both breast and axillary lesions, and it may overcome some of the limitations of wire-guided localization.

Abstract PO4-18-12: MELODY: A prospective non-interventional multicenter cohort study to evaluate different imaging-guided methods for localization of malignant breast lesions (EUBREAST-4/iBRA-NET, NCT 05559411)

Abstract Background: In the last decades, the proportion of breast cancer patients receiving breast-conserving surgery has increased, reaching 70-80% in developed countries. In case of non-palpable lesions, surgical excision requires some form of breast localization. While wire-guided localization has long been considered gold standard, it carries several limitations, including logistical difficulties, the potential for displacement and patient discomfort, and re-excision rates reaching 21%. Other techniques (radioactive seed or radio-occult lesion localization, intraoperative ultrasound, magnetic, radiofrequency and radar localization) have been developed with the aim of overcoming these disadvantages. However, comparative data on the rates of successful lesion removal, negative margins and re-operations are limited. In the majority of studies, the patient’s perspective with regard to discomfort and pain level has not been evaluated. The aim of MELODY (MEthods for LOcalization of Different types of breast lesions) is to evaluate different imaging-guided localization methods with regard to oncological safety, patient-reported outcomes, and surgeon and radiologist satisfaction. Methods: The EUBREAST and the iBRA-NET have initiated the MELODY study to assess breast localization techniques and devices from several perspectives (NCT05559411, http://melody.eubreast.com). MELODY is a prospective intergroup cohort study which enrolls female and male pts. requiring breast-conserving surgery and imaging-guided localization for invasive breast cancer or DCIS. Multiple or bilateral lesions and neoadjuvant chemotherapy are allowed. Primary outcomes are: 1) Intended target lesion and/or marker removal, independent of margin status on final histopathology, and 2) Negative resection margin rates at first surgery. Secondary outcomes are, among others: rates of second surgery and secondary mastectomy, Resection Ratio (defined as actual resection volume divided by the calculated optimum specimen volume), duration of surgery, marker dislocation rates, rates of marker placement or localization failure, comparison of patient-reported outcomes, rates of “lost markers” and diagnostician/radiologist’s and surgeon’s satisfaction as well as the health economic evaluation of the different techniques. Target accrual: 7,416 patients. Enrollment started in January 2023. The study will be conducted in 30 countries and is supported by the Oncoplastic Breast Consortium (OPBC), AWOgyn, AGO-B and SENATURK. Abstract PO3-17-03: Impalpable breast lesion localisation device satisfaction in UK surgeons and radiologists: results of the iBRA-NET national practice questionnaire

Abstract Introduction A national practice questionnaire in 2020 collected quantitative data from UK breast surgeons on breast localisation device use and found wire to be the most frequently used localisation device for impalpable lesions (Somasundaram, et al., 2020). Wire localisation was found to be associated with significant logistical issues including displacement and delays in theatre scheduling. Localisation device practice has changed greatly in the three years since the last questionnaire (Dave, et al., 2022). This study aimed to assess the change in device use, impact on logistics and to qualitatively assess surgeon and radiologist experience across the range of localisation devices. Methods The questionnaire was designed with identical questions to the 2020 study to allow for direct comparison of change, this collected information on demographics, logistics and device use. A pilot study was conducted to ascertain domains that were important to clinicians in differentiating between the quality of localisation device experience. The qualitative questions asked respondents to assign a Likert scale score of 0-10 for satisfaction in each identified domain. Two national practice questionnaires were created used SurveyMonkey® and were distributed to UK breast surgeons and radiologists via direct email, social media and the Association of Breast Surgery newsletter. The means of the satisfaction responses were compared with the mean responses for wire as the standard using a two-sample t test using Microsoft Excel. Results The surveys were completed between August and December 2022. There were 157 completed questionnaires, with 76 responses being from surgeons and 81 from radiologists. There has been significant change in the use of localisation devices in the UK, from 83% wire, 5% radio-occult lesion localisation (ROLL), 2% radioiodine seed and 9% Magseed® in 2020 to 45% wire, 2% ROLL, 3% radioiodine seed, 30% Magseed®, 5% SAVI SCOUT, 12% Hologic LOCalizer® and 2% Sirius Pintuition® by 2022. In 2020 9% of patients had localisation performed prior to the day of surgery, by 2022 this had increased to 64% (p < 0.05). Changes in localisation techniques also allowed these patients to undergo surgery earlier on a list; 15% of units in 2020 could operate on a localisation patient prior to 9am, by 2022 this was 41%. The satisfaction scores for Magseed® were statistically significantly higher than for wire in six of eight domains for surgeons and seven of nine domains for radiologists (p < 0.05). Surgeons found SAVI SCOUT® and Hologic LOCalizer® to be preferable to wire in five and seven domains respectively (p < 0.05). Discussion The follow-up survey demonstrates a change in practice in the use of newer localisation devices. The outcomes of the change seem to be an ability to place devices in advance, earlier start times in theatre and higher clinician satisfaction. The qualitative data demonstrates that clinicians are able to differentiate between the attributes of localisation devices. Due to a small sample size in some of the newer devices we are unable to directly compare between the newer devices, however, Magseed® scored significantly higher than wires in seven of the nine domains. Evolution of localization methods for non-palpable breast lesions: a literature review from a translational medicine perspective.

With an increasing number of non-palpable breast lesions detected due to improved screening, accurate localization of these lesions for surgery is crucial. This literature review explores the evolution of localization methods for non-palpable breast lesions, highlighting the translational journey from concept to clinical practice. A comprehensive search of PubMed, Embase, and Scopus databases until September 2023 was conducted. Multiple methods have been developed throughout the past few decades. (I) Wire-guided localization (WGL) introduced in 1966, has become a reliable method for localization. Its simplicity and cost-effectiveness are its key advantages, but challenges include logistical constraints, patient discomfort, and potential wire migration. (II) Intraoperative ultrasound localization (IOUS) has shown promise in ensuring complete lesion removal with higher negative margin rates. However, its utility is limited to lesions visible on ultrasound (US) imaging. (III) Breast biopsy marker localization: the use of markers has improved the precision of localization without the need for wire. However, marker visibility remains a challenge despite improvements in their design. (IV) Radioactive techniques: radio-guided occult lesion localization (ROLL) and radioactive seed localization (RSL) offer flexibility in scheduling and improved patient comfort. However, they require close multidisciplinary collaboration and specific equipment due to radioactive concerns. (V) Other wireless non-radioactive techniques: wireless non-radioactive techniques have been developed in recent three decades to provide flexible and patient-friendly alternatives. It includes magnetic seed localization, radar techniques, and radiofrequency techniques. Their usage has been gaining popularity due to their safety profile and allowance of more flexible scheduling. However, their high cost and need for additional training remain a barrier to a wider adoption. The evolution of breast lesion localization methods has progressed to more patient-friendly techniques, each with its unique advantages and limitations. Future research on patient-reported outcomes, cosmetic outcomes, breast biopsy markers and integration of augmented reality with breast lesion localization are needed.

Harnessing the power of radiomics and deep learning for improved breast cancer diagnosis with multiparametric breast mammography

Breast cancer, with its high mortality, faces diagnostic challenges due to variability in mammography quality and breast densities, leading to inconsistencies in radiological interpretations. Computer-aided diagnostic (CAD) systems, while helpful, struggle with accurately interpreting lesion characteristics such as morphology, density, and size. To address this, our study developed advanced deep-learning algorithms to improve the detection, localization, risk assessment, and classification of breast lesions, aiming to reduce false positives on human intervention and tackle slow convergence rates. Key innovations of the approach include preprocessing techniques, advanced filtering, and data augmentation strategies to optimize model performance, mitigating over- and under-fitting concerns. A significant development is the Chaotic Leader Selective Filler Swarm Optimization (cLSFSO) method, which effectively detects breast-dense lesions by extracting textural and statistical features. Additionally, the study adapted deep learning models like modified VGGNet and SE-ResNet152 through transfer learning, significantly enhancing their capability to distinguish between normal and suspicious mammography regions. The study also introduces hybrid deep neural network-based approaches, including CNN+LSTM and CNN+SVM, for diagnosing and grading cancerous polyps from the pre-segmented ROIs. Besides, the transfer learning paradigm is employed to boost the efficacy in classifying breast masses and reducing computing time by modifying the final layer of the proposed pre-trained models. The integration of Grad-CAM techniques further refines our analysis, leading to more accurate assessments and improved diagnosis of breast anomalies. Evaluated using benchmark and private datasets, our algorithms demonstrated a sensitivity of 0.99 and an overall AUC of 0.99, indicating significant improvements in mammogram analysis. These advancements aid radiologists, potentially improving patient outcomes and contributing to medical imaging and AI in healthcare.

Magnetic Seed vs Guidewire Breast Cancer Localization With Magnetic Lymph Node Detection

Guidewires have been the standard for breast lesion localization but pose operative and logistic challenges. Paramagnetic seeds have shown promising results, but to the authors' knowledge, no randomized comparison has been performed. To determine whether the combination of a paramagnetic seed and superparamagnetic iron oxide (SPIO) is equivalent to guidewire and SPIO for breast cancer localization and sentinel lymph node detection (SLND). This was a phase 3, pragmatic, equivalence, 2-arm, open-label, randomized clinical trial conducted at 3 university and/or community hospitals in Sweden from May 2018 to May 2022. Included in the study were patients with early breast cancer planned for breast conservation and SLND. Study data were analyzed July to November 2022. Participants were randomly assigned 1:1 to a paramagnetic seed or a guidewire. All patients underwent SLND with SPIO. Re-excision rate and resection ratio (defined as actual resection volume / optimal resection volume). A total of 426 women (median [IQR] age, 65 [56-71] years; median [IQR] tumor size, 11 [8-15] mm) were included in the study. The re-excision rate was 2.90% (95% CI, 1.60%-4.80%), and the median (IQR) resection ratio was 1.96 (1.15-3.44). No differences were found between the guidewire and the seed in re-excisions (6 of 211 [2.84%] vs 6 of 209 [2.87%]; difference, -0.03%; 95% CI, -3.20% to 3.20%; P = .99) or resection ratio (median, 1.93; IQR, 1.18-3.43 vs median, 2.01; IQR, 1.11-3.47; P = .70). Overall SLN detection was 98.6% (95% CI, 97.1%-99.4%) with no differences between arms (203 of 207 [98.1%] vs 204 of 206 [99.0%]; difference, -0.9%; 95% CI, -3.6% to 1.8%; P = .72). More failed localizations occurred with the guidewire (21 of 208 [10.1%] vs 4 of 215 [1.9%]; difference, 8.2%; 95% CI, 3.3%-13.2%; P < .001). Median (IQR) time to specimen excision was shorter for the seed (15 [10-22] minutes vs 18 [12-30] minutes; P = .01), as was the total operative time (69 [56-86] minutes vs 75.5 [59-101] minutes; P = .03). The experience of surgeons, radiologists, and surgical coordinators was better with the seed. The combination of SPIO and a paramagnetic seed performed comparably with SPIO and guidewire for breast cancer conserving surgery and resulted in more successful localizations, shorter operative times, and better experience. Evaluation of a Surgical Navigation System for Localization and Excision of Nonpalpable Lesions in Breast and Axillary Surgery.

Elucent Medical has introduced a novel EnVisio™ Surgical Navigation system which uses SmartClips™ that generate a unique electromagnetic signal triangulated in 3 dimensions for real-time navigation. The purpose of this study was to evaluate the efficacy and feasibility of the EnVisio Surgical Navigation system in localizing and excising nonpalpable lesions in breast and axillary surgery. This pilot study prospectively examined patients undergoing breast and nodal localization using the EnVisio Surgical Navigation system. SmartClips were placed by designated radiologists using ultrasound (US) or mammographic (MMG) guidance. The technical evaluation focused on successful deployment and subsequent excision of all localized lesions including SmartClips and biopsy clips. Eleven patients underwent localization using 27 SmartClips which included bracketed multifocal disease (n = 4) and clipped lymph node (n = 1). The bracketed cases were each localized with 2 SmartClips. Mammography and ultrasound were used (n = 8 and n = 19, respectively) to place the SmartClips. All 27 devices were successfully deployed within 5 mm of the targeted lesion or biopsy clip. All SmartClip devices were identified and retrieved intraoperatively. No patients required a second operation for margin excision. In a limited sample, the EnVisio Surgical Navigation system was a reliable technology for the localization of breast and axillary lesions planned for surgical excision. Further comparative studies are required to evaluate its efficacy in relation to the other existing localization modalities.

Joint localization and classification of breast masses on ultrasound images using an auxiliary attention-based framework.

Multi-task learning (MTL) methods have been extensively employed for joint localization and classification of breast lesions on ultrasound images to assist in cancer diagnosis and personalized treatment. One typical paradigm in MTL is a shared trunk network architecture. However, such a model design may suffer information-sharing conflicts and only achieve suboptimal performance for individual tasks. Additionally, the model relies on fully-supervised learning methodologies, imposing heavy burdens on data annotation. In this study, we propose a novel joint localization and classification model based on attention mechanisms and a sequential semi-supervised learning strategy to address these challenges. Our proposed framework offers three primary advantages. First, a lesion-aware network with multiple attention modules is designed to improve model performance on lesion localization. An attention-based classifier explicitly establishes correlations between the two tasks, alleviating information-sharing conflicts while leveraging location information to assist in classification. Second, a two-stage sequential semi-supervised learning strategy is designed for model training to achieve optimal performance on both tasks and substantially reduces the need for data annotation. Third, the asymmetric and modular model architecture allows for the flexible interchangeability of individual components, rendering the model adaptable to various applications. Experimental results from two different breast ultrasound image datasets under varied conditions have demonstrated the effectiveness of the proposed method. Furthermore, we conduct comprehensive investigations into the impacts of various factors on model performance, gaining in-depth insights into the mechanism of our proposed framework. A coupling, stabilizing, and shaping strategy for breast ultrasound computed tomography (USCT) with a ring array transducer

Breast ultrasound computed tomography (USCT) has been gradually promoted to clinical application after years of rapid development. Compared with the traditional handheld ultrasound scanning method, the scanning plane of USCT is fixed at the coronal plane, and the scanning path is designed in advance; the acoustic window is not in direct contact with the breast, a lot of coupling medium (usually degassed water is used to fill the gaps between the probe and breast. The clinical application of breast USTC faces challenges: (1) the processes of water degassing, heating, filling, draining, and cleaning prolong the entire scan cycle and reduce patient throughput. (2) The breast is not stabilized and slight movements of the breast may cause motion artifacts in the USCT images. (3) The non-normal incidence of ultrasound into the breast causes reflected and transmitted signals received with a low signal-to-noise ratio (SNR) or even unable to be detected. This article proposes a coupling, stabilizing, and shaping strategy for the clinical application of USCT with a ring array transducer. The solid gel coupling agent (SGCA) is applied for coupling, and a set of SGCA moldings is designed to stabilize and shape the breast during scanning, the breast shape and size which vary from person to person are simplified into several models. The preparation time is reduced to less than 1 min by replacing disposable moldings. The results show that the breast after shaping is close to round in the coronal plane, and slopes of the breast skin are limited in the sagittal and transverse planes, the breast subcutaneous tissue (fat and glands) has a better contrast-to-noise ratio (CNR) and can be better distinguished in the reflection images than that of the breast without shaping. The mean value of the raw beamformed data which represents the reflection signal amplitude of breast subcutaneous tissue after shaping shows 1.5 times that of the breast without shaping, the signal-to-noise ratio (SNR) of the raw transmission signal data after breast shaping is overall higher than that of the breast without shaping. The application of SGCA moldings for breast coupling, stabilizing, and shaping also benefits establishing a standardized scanning process, the standardized diagnosis of the breast lesion, and the localization of breast lesions.

Localization of contrast-enhanced breast lesions in ultrafast screening MRI using deep convolutional neural networks

ObjectivesTo develop a deep learning–based method for contrast-enhanced breast lesion detection in ultrafast screening MRI.Materials and methodsA total of 837 breast MRI exams of 488 consecutive patients were included. Lesion’s location was independently annotated in the maximum intensity projection (MIP) image of the last time-resolved angiography with stochastic trajectories (TWIST) sequence for each individual breast, resulting in 265 lesions (190 benign, 75 malignant) in 163 breasts (133 women). YOLOv5 models were fine-tuned using training sets containing the same number of MIP images with and without lesions. A long short-term memory (LSTM) network was employed to help reduce false positive predictions. The integrated system was then evaluated on test sets containing enriched uninvolved breasts during cross-validation to mimic the performance in a screening scenario.ResultsIn five-fold cross-validation, the YOLOv5x model showed a sensitivity of 0.95, 0.97, 0.98, and 0.99, with 0.125, 0.25, 0.5, and 1 false positive per breast, respectively. The LSTM network reduced 15.5% of the false positive prediction from the YOLO model, and the positive predictive value was increased from 0.22 to 0.25.ConclusionsA fine-tuned YOLOv5x model can detect breast lesions on ultrafast MRI with high sensitivity in a screening population, and the output of the model could be further refined by an LSTM network to reduce the amount of false positive predictions.Clinical relevance statementThe proposed integrated system would make the ultrafast MRI screening process more effective by assisting radiologists in prioritizing suspicious examinations and supporting the diagnostic workup.Key Points• Deep convolutional neural networks could be utilized to automatically pinpoint breast lesions in screening MRI with high sensitivity.• False positive predictions significantly increased when the detection models were tested on highly unbalanced test sets with more normal scans.• Dynamic enhancement patterns of breast lesions during contrast inflow learned by the long short-term memory networks helped to reduce false positive predictions.

The aim of marking the non-palpable breast lesions is to achieve accurate lesion localization, to obtain the better cosmetic result with less tissue loss and to provide negative surgical margin.Aim of the studyIn the current study, we aimed to compare the wire-guided localization (WGL) technique with the radio-guided occult lesion localization (ROLL) technique to assess their accuracy and efficacy in non-palpable breast lesions localization.MethodsThis is a retrospective study conducted at Baheya center for Early Detection and Treatment of Breast Cancer from January 2018 and June2022,where 670 patients with non-palpable breast lesions underwent an excision were enrolled randomly in ROLL group (n = 320) and WGL (n = 350).ResultsBoth the localization time and the time of operation were significantly decreased with the ROLL in comparison to WGL(P < 0.001). Complete lesion excision with clear margins were reported in 119/135(88.2%) of ROLL group and in 130/159 (81.8%) of WGL group and the difference was significant (P < 0.001). Reoperations (re-lumpectomy or mastectomy) were done as a second procedure on 16(11.8%) of the ROLL patients compared with 29(18.2%) in the WGL patients(P < 0.001).ConclusionThis study shows that ROLL is as effective as WGL for non-palpable breast lesions excision. Also, ROLL improve the outcomes by decreasing the duration of surgery, localization time, achieving a higher percentage of clear margin in spite of lower specimen size and scar length.

IntroductionFor decades the standard for preoperative breast lesions’ localization has been wire localization. In recent years the options for nonwired localization have significantly expanded and include radioactive seeds, radar reflectors, radiofrequency identification tags and magnetic seeds. The aim of our study is to evaluate on a large scale the performance of preoperative magnetic seed localization of nonpalpable breast lesions. Material and MethodsWe prospectively collected data on all patients undergoing image-guided magnetic seed localization from September 2019 to December 2022. We analyzed imaging findings, histological results, and type of surgery. The primary outcome was the successful localization rate. Secondary outcomes were the successful placement rate, the ease of percutaneous positioning, the procedural complications, and the reintervention rate. ResultsA total of 1123 magnetic seeds were placed in 1084 patients by 4 radiologists under ultrasound (1053) or stereotactic (70) guidance. All seeds were detectable transcutaneously in all breasts sizes and at all depths by 7 surgeons with a success rate of 100%. A total of 97.5% seeds were correctly placed into the target lesions (only 2.5% were dislocated). All radiologists have shown good compliance during the procedure, and there were no complications or safety issues. The reoperation rate was 5.1%. ConclusionsImage-guided localization with magnetic seeds is an easy, safe, reliable, and effective method for localizing nonpalpable breast lesions. Both radiologists and surgeons agreed that the technology was intuitive to use and that it can be widely applied in preoperative localization in breast units.

Magnetic seed versus skin tattoo localization of non-palpable breast lesions: a single institution cohort study.

The objective of this retrospective study was to investigate the accuracy and feasibility of magnetic seed compared to skin tattoo in preoperative localization of impalpable breast lesions in terms of accuracy of placement, re-excision and positive margins rates, and breast/surgical specimen volume ratio. We retrospectively analyzed 77 patients who underwent breast conservative surgery in our center from November 2020 to November 2021, with previous localization with skin tattoo or magnetic seed. Thirty-seven magnetic seeds were placed in 36 patients (48.6%) and 40 skin tattoos were performed in the remaining cases (51.4%). The seeds were placed correctly at the two-view mammogram acquired after the insertion in 97.6% (36/37) of cases. With both methods, 100% of the index lesions were completely removed and found in the surgical specimen. The reported re-excision rate was 0% for both groups. A significant difference was observed in the volume of breast parenchyma removed between the two groups, inferior in the seed group (p = 0.046), especially in case of voluminous breasts (p = 0.003) and small lesions (dimension < 8mm, p = 0.019). Magnetic seed is a non-radioactive localization technique, feasible to place, recommended in case of non-palpable breast lesions, saving the breast parenchyma removed compared with skin tattoo, without reducing the accuracy. Our findings contribute to the current evidence on preoperative localization techniques for non-palpable breast lesions, highlighting the efficacy of magnetic seed localization for deep and small lesions. • Magnetic seed is a non-radioactive technique for the preoperative localization of non-palpable breast lesions studied in comparison with skin tattoo. • Magnetic seed is feasible to place in terms of post-placement migration and distance from the target lesion. • Magnetic seed is recommended in case of non-palpable breast lesions, saving the breast parenchyma removed without reducing the accuracy.

RFID trial: localization of non-palpable breast lesions using radiofrequency identification tags or wire

BackgroundBreast cancer is the most frequently diagnosed cancer and the leading cause of cancer death in women. Approximately 50% of breast cancers are discovered at an early stage in patients for whom conservative surgery is indicated. Intraoperative localization of non-palpable breast lesions is generally accomplished using a hook wire to mark the area of concern under ultrasound or stereotactic localization. But this technique has several drawbacks (painful, stressful…). We propose the use of a wire-free breast lesion system using miniature radiofrequency identification (RFID) tags. This technique could improve patient comfort and surgical comfort for surgeons. We therefore propose a study to assess the interest of introducing the RFID localization technique at the Jean PERRIN comprehensive cancer center.MethodsThis is a single-center prospective trial designed to assess the interest in introducing the RFID localization technique at the Jean Perrin center. It aims to show the superiority of the RFID technique in terms of patient tolerance compared to the gold-standard (hook wire). A sequential inclusion in time will be performed: 20 inclusions in the gold-standard group, then 20 patients in the RFID group before repeating the inclusion scheme. Any patient requiring preoperative localization will receive a senology consultation. The RFID tag will be placed during this consultation. The hook wire localization will be done the day before the surgery. Patients will fill out a Hospital Anxiety and Depression scale (HAD) questionnaire at the time of inclusion. They will then fill out a satisfaction questionnaire in 2 steps: during the placement of the device (RFID tag or hook wire) or during the postoperative consultation at 1 month. Radiologists and surgeons will fill out a questionnaire to evaluate the localization technique, respectively after the localization and surgery procedures.DiscussionThe RFID study is the first study in France which specifically assesses the interest of the RFID localization in terms of patients comfort. Patient comfort is one of the key elements to take into consideration when managing patients in oncology and new technologies such as RFID tags could improve it.Trial registrationClinicalTrials.gov ID; NCT04750889 registered on February 11, 2021.

Radiofrequency localization of nonpalpable breast cancer in a multicentre prospective cohort study: feasibility, clinical acceptability, and safety

PurposeIn breast conserving surgery, accurate lesion localization is essential for obtaining adequate surgical margins. Preoperative wire localization (WL) and radioactive seed localization (RSL) are widely accepted methods to guide surgical excision of nonpalpable breast lesions but are limited by logistical challenges, migration issues, and legislative complexities. Radiofrequency identification (RFID) technology may offer a viable alternative. The purpose of this study was to evaluate the feasibility, clinical acceptability, and safety of RFID surgical guidance for localization of nonpalpable breast cancer.MethodsIn a prospective multicentre cohort study, the first 100 RFID localization procedures were included. The primary outcome was the percentage of clear resection margins and re-excision rate. Secondary outcomes included procedure details, user experience, learningcurve, and adverse events.ResultsBetween April 2019 and May 2021, 100 women underwent RFID guided breast conserving surgery. Clear resection margins were obtained in 89 out of 96 included patients (92.7%), re-excision was indicated in three patients (3.1%). Radiologists reported difficulties with the placement of the RFID tag, partially related to the relatively large needle-applicator (12-gauge). This led to the premature termination of the study in the hospital using RSL as regular care. The radiologist experience was improved after a manufacturer modification of the needle-applicator. Surgical localization involved a low learning curve. Adverse events (n = 33) included dislocation of the marker during insertion (8%) and hematomas (9%). The majority of adverse events (85%) occurred using the first-generation needle-applicator.ConclusionRFID technology is a potential alternative for non-radioactive and non-wire localization of nonpalpable breast lesions.
