Enhanced recovery after surgery-based nursing in older patients with postoperative intestinal obstruction after gastric cancer surgery: A retrospective study.

Gastric cancer-related morbidity and mortality rates are high in China. Patients who have undergone gastric cancer surgery should receive six cycles of chemotherapy according to their condition. During this period, intestinal obstruction is likely to occur. Electrolyte balance disorders, peritonitis, intestinal necrosis, and even hypovolemic shock and septic shock can seriously affect the physical and mental recovery of patients and threaten their health and quality of life (QoL). To quantitatively explore the effects of enhanced recovery after surgery (ERAS)-based nursing on anxiety, depression, and QoL of elderly patients with postoperative intestinal obstruction after gastric cancer. The clinical data of 129 older patients with intestinal obstruction after gastric cancer surgery who were treated and cared for in our hospital between January 2019 and December 2021 were examined retrospectively. Nine patients dropped out because of transfer, relocation, or death. According to the order of admissions, the patients were categorized into either a comparison group or an observation group according to the random number table, with 60 cases in each group. After nursing care, the observation group required significantly less time to eat for the first time, recover bowel sounds, pass gas, and defecate than the comparison group (P < 0.05). No significant difference was noted in nutrition-related indicators between the two groups before care. Before care, the Symptom Check List-90 scores between the two groups were comparable, whereas anxiety, depression, paranoia, fear, hostility, obsession, somatization, interpersonal sensitivity, and psychotic scores were significantly lower in the observation group after care (P < 0.05). The QoL scores between the two groups before care did not differ significantly. After care, the physical, social, physiological, and emotional function scores; mental health score; vitality score; and general health score were significantly higher in the observation group, whereas the somatic pain score was significantly lower in the observation group (P < 0.05). ERAS-based nursing combined with conventional nursing interventions can effectively improve patient's QoL, negative emotions, and nutritional status; accelerate the time to first ventilation; and promote intestinal function recovery in elderly patients with postoperative intestinal obstruction after gastric cancer surgery.

Effects of Early Cardiac Rehabilitation Training on Cardiac Function and Quality of Life in Elderly Patients Undergoing Coronary Artery Bypass Grafting: A Retrospective Study

At T3, the LVEF levels and 6-MWT distance of both groups were sharply lower than those at T1 (p &lt; 0.05). Compared with the levels at T3, the LVEF levels; the 6-MWT; and the global, physical, emotional, and social levels of the C and R groups at T2 and T4 significantly increased (p &lt; 0.05). At T1, the LVEF level; the 6-MWT; and the global, physical, emotional, and social levels of the C group was not statistically significant compared with those of the R group (p &gt; 0.05). At T2, T3, and T4, the LVEF levels; the 6-MWT; and the global, physical, emotional, and social levels of the R group were sharply higher than those of the C group (p &lt; 0.05). Conclusion: Early cardiac recovery training can effectively ameliorate the cardiac function and improve the quality of life of elderly patients undergoing CABG.

Correlation between health literacy and life quality in elderly patients with chronic periodontitis.

This study aims to explore the status quo of health literacy and quality of life in elderly patients with chronic periodontitis and determine their correlation to understand the influencing factors of health literacy and quality of life. A total of 150 elderly patients with chronic periodontitis were investigated using the general information questionnaire, Oral Health Literacy-Adult Questionnaire, and Oral Health Impact Profile for Chronic Periodontitis. The total scores of health literacy and quality of life of elderly patients with chronic periodontitis were 6.59±4.11 and 29.25±9.42, respectively. The total score of health literacy was negatively correlated with the total score of quality of life (r=-0.234, P<0.01). Regression analysis showed that age, place of residence, education level, and disease understanding were the influencing factors of health literacy. Family history, smoking, and education level were the influencing factors of quality of life. The health literacy level of elderly patients with chronic periodontitis is insufficient, and their quality of life needs to be improved. Improving the health literacy of patients is an effective way to improve their quality of life. Nursing staff must formulate corresponding health education strategies to improve the health literacy of patients and improve their quality of life.

The effect of surgical aortic valve replacement on the quality of life of patients with aortic valve stenosis

Background/Introduction The increasing efficacy of transcatheter aortic valve implantation (TAVI) justifies a renewed assessment of quality of life (QOL) after surgical aortic valve replacement (SAVR). Purpose The aim of this study was to evaluate long-term QOL in elderly patients undergoing isolated SAVR. Methodology 60 patients with severe aortic stenosis (AS) enrolled in our study (mean age 70.25±8.76, n=21 women). Clinical and QOL data collected pre surgery and at 1 year. For the assessment of QOL we used the instruments SF-36 (36-Item Short Form Health Survey Questionnaire), Euro-QOL 5 Dimensions (EQ-5D) and the visual analogue scale (VAS). Bivariate and multiple logistic regression analyses were performed. Results QoL is significantly improved 1 year after SAVR according to the three assessment tools. The physical health score (PCS) was 58±10% preoperatively and 80±10% (p&amp;lt;.001) after 1 year. Similarly, mental health score (MCS) improved (55±6% vs 77±9%, p&amp;lt;.001). Multivariate analysis showed that patients with a biological or Perceval valve and a preoperative EF &amp;lt;50% had greater improvement in PCS 1 year after SAVR than patients who received a mechanical valve or had a preoperative EF &amp;gt;50%. Conclusions The perceived QOL of patients with AS improved significantly 1 year after SAVR. The results could be useful in informing future patients and giving them realistic expectations.

Postoperative Limb Function and QOL in Elderly Patients With Malignant Bone Tumor/Soft Tissue Sarcoma.

Malignant bone tumors (MBT) and soft tissue sarcomas (STS) require wide excision. Although the number of elderly patients is increasing, wide excision may decrease limb function and quality of life (QOL) for elderly patients. However, no detailed evaluation of the functional prognosis or QOL of elderly patients with sarcoma has been reported. This study evaluated postoperative limb function and QOL in elderly patients with MBT and STS. This retrospective study included 67 patients aged >70 years with MBT or STS who underwent surgery at a single institution. The Toronto Extremity Salvage Score (TESS), EuroQoL 5-dimension 5-level (EQ-5D-5L) questionnaire, Musculoskeletal Tumor Society (MSTS) score, and psoas muscle index (PMI) were evaluated. We also assessed factors associated with the postoperative TESS and EQ-5D-5L index. Detailed examination of the MSTS items perioperatively revealed significant decline in manual dexterity/walking ability and support but significant improvement in pain and emotional acceptance. The mean PMI decreased significantly from 4.7 to 4.23 perioperatively. The postoperative mean TESS and EQ-5D-5L index was 76.9 and 0.74, respectively. Patients with good performance status and clinical frailty scale scores preoperatively had better postoperative TESS and EQ-5D-5L scores. The current study strongly suggests the possibility of maintaining postoperative limb function, satisfaction, and QOL in patients with MBT and STS by choosing patients in good condition and the appropriate procedure that the patient desires. However, perioperative progression of sarcopenia should be noted.

Improvement of Symptoms and Quality of Life After Successful Percutaneous Coronary Intervention for Chronic Total Occlusion in Elderly Patients.

Background Data regarding the impact of successful chronic total occlusion treated with percutaneous coronary intervention (CTO-PCI) on symptoms and quality of life (QOL) in elderly patients (≥75 years) are unknown. This prospective study aimed to assess whether successful CTO-PCI could improve the symptoms and QOL in elderly patients (≥75 years). Methods and Results Consecutive patients who underwent elective CTO-PCI were prospectively enrolled and subdivided into 3 groups based on age: age<65 years, 65 years≤age<75 years, and age≥75 years. The primary outcomes included symptoms, as assessed with the New York Heart Association functional class and Seattle Angina Questionnaire, and QOL, as assessed with the 12-Item Short-Form Health Survey questionnaire, at baseline, 1 month, and 1 year after successful CTO-PCI. Of 1076 patients with CTO, 101 were age≥75 years (9.39%). Hemoglobin, estimated glomerular filtration rate, and left ventricular ejection fraction levels all decreased with increasing age, and NT-proBNP (N-terminal pro-B-type natriuretic peptide) increased. The proportion of dyspnea and coronary lesions, including multivessel disease, multi-CTO lesion, and calcification were higher in elderly patients. Procedural success rate, intraprocedural complications, and in-hospital major adverse cardiac events were not statistically different in the 3 groups. Importantly, symptoms, including dyspnea and angina, were markedly improved regardless of age at 1-month and 1-year follow-up (P<0.05). Likewise, successful CTO-PCI significantly improved QOL at 1-month and 1-year follow-up (P<0.01). Additionally, the incidence of major adverse cardiac events and all-cause mortality at 1-month and 1-year follow-up was not statistically different in the 3 groups. Conclusions Successful PCI was beneficial and feasible to improve symptoms and QOL in patients ≥75 years of age with CTO.

The article discusses various aspects of chronic pain syndrome in general and chronic back pain in elderly patients. The frequency of chronic pain occurrence in the elderly, according to different authors, is up to 83.8%, chronic back pain ranges from 27 to 86%. The negative impact of chronic back pain on the life quality of elderly patients was shown. Correlations were found between chronic back pain and such conditions as an increased risk of falls, fractures, and the development of dementia. Chronic pain syndrome in people over 60 y. o. contributes to a risk of mortality increase by 57%. The reasons for the chronic back pain development are not only degenerative-dystrophic changes in the spine, but also changes in proprioception and locomotor function. The analysis of the literature substantiates the need for further research on chronic back pain in the elderly.

Patient satisfaction is an important consideration when determining the optimal treatment for non-melanoma skin cancer (NMSC). One critical aspect of patient satisfaction is post-procedural wound care quality of life (QOL), especially as the elderly population grows. This study aimed to evaluate post-procedural wound care QOL in elderly patients undergoing electrodessication and curettage (ED&C) for NMSC in difficult-to-reach areas, namely the posterior shoulder and back. To do so, patient demographics, functionality, co-morbidities, and post-procedural wound care QOL were assessed in twenty elderly patients (age > 65) who underwent ED&C for NMSC at a single academic dermatologic surgery clinic. Independent t-tests were used to evaluate how QOL related to patient age, gender, living situation, relationship status, co-morbidities, and functionality. Patients who lived alone had better-wound care QOL compared to patients who did not live alone (p = 0.04). Patients reported concerns about knocking the wound and did not feel they could care for the wound independently. Patients who were married, female, or had a lower comorbidity score reported poorer QOL, although this finding was not statistically significant. This study indicates that patients' QOL can be negatively affected by post-procedural wounds located in difficult-to-reach areas. As dermatologists strive to improve patient satisfaction, wound care quality of life should be considered when choosing treatment for NMSC.

Influence of psychological nursing and health education on depression, anxiety and life quality of elderly patients with lung cancer.

To explore and analyze the influence of psychological nursing and health education on depression, anxiety and life quality of elderly patients with lung cancer. There were 84 elderly patients with lung cancer who were admitted and treated in our hospital since July 2018 till July 2019, serving as the objects of observation. The selected patients were divided into the control group (n=42) and the observation group in principle of random allocation by computer numbers. Routine care was conducted to the control group, while psychological nursing and health education were implemented to the observation group, so as to compare the improvement effect of depression, anxiety, life quality and compliance on the two groups of patients before/after the two different nursing methods were adopted for intervention. Hamilton Depression Scale and Hamilton Anxiety Scale scores in the observation group were lower than those in the control group, and the levels of various pulmonary function indicators including degree of blood oxygen saturation, forced expiratory volume in 1 second, forced vital capacity and maximum ventilatory capacity and quality of life scores were significantly improved; compared with the control group, the total compliance of the observation group was significantly increased, and the difference between the two groups was statistically significant (P < 0.05). The psychological nursing and health education implemented for elderly patients with lung cancer will improve patients' bad mood effectively, enhance life quality and perfect prognosis for patients, maintaining significant value in clinical application.

Aim: Determine the effect of palliative care program on the quality of life for hepatocellular carcinoma of elderly Patients. Design: the researcher used quasi- experimental research design. Setting: outpatient clinics at Gastrointestinal Surgery Center (GISC) hepatology unit affiliated to Mansoura University, Dakahlia governorate, Egypt. Subjects: The study subjects included a random purposive sampling of 72 HCC elderly patients of both sex diagnosed at least 6 months. Tools: Data collection is done by two tools namely Patients' interview schedule sheet (elderly Demographic and Health Relevant Characteristics) and Functional Assessment of Cancer Therapy- For elderly patients with Hepatobiliary cancer (FACT-Hep) Version4: Results: the age of the studied participants ranged from 60 to above 70 years. The elderly patient in study group total quality of life scores increased significantly in post one and post three follow up after implementation of palliative care program (P=0.000), while no improvement was observed in the control group. Conclusion: the QOL of elderly patients with HCC is improved by implementation of palliative care program. The effect of Tai Chi on the quality of life in the elderly patients recovering from coronavirus disease 2019: A protocol for systematic review and meta-analysis.

Background:coronavirus disease 2019 (COVID-19) is spreading fast starting late 2019. As their cardiopulmonary and immune functions gradually decline, elderly people are prone to COVID-19. Tai Chi has a positive impact on heart function, blood pressure, lung function, blood circulation, and so on, and it's suitable for the elderly. Quality of life (QoL)can reflect of individuals’ physical and mental health, it can also reflects their ability to participate in society. This systematic review and meta-analysis will summarize the current evidence that Tai Chi improve the QoL in the elderly patients recovering from COVID-19.Methods:We will search PubMed, EMBASE, MEDLINE, the Cochrane Library, Chinese National Knowledge Infrastructure, Chinese Biomedical Literature Database, Chinese Science and Technology Periodical Database, Wanfang Database, Clinical Trials and Chinese Clinical Trial Registry. The complete process will include study selection, data extraction, risk of bias assessment and meta-analyses. Endnote X9.3 will be used to manage data screening. The statistical analysis will be completed by Stata/SE 15.1 software.Results:This proposed study will evaluate the effectiveness and safety of Tai Chi for the improvement of QoL in elderly COVID-19 patients during the recovery period.Conclusion:The conclusion of this study will provide evidence to prove the safety and effectiveness of Tai Chi on elderly COVID-19 patients during the recovery period.Ethics and dissemination:This protocol will not evaluate individual patient information or infringe patient rights and therefore does not require ethical approval.Registration:PEROSPERO CRD42020206875

Patients’ versus caregivers’ reports of quality of life in elderly patients with cognitive impairment

AbstractBackgroundA few studies have examined the differences between patient and caregiver reported Quality of Life (QOL) in patients with dementia. The purpose of this study was to investigate the differences between patient and caregiver ratings on QOL in elderly patients with cognitive impairment and assessed the relationship of patient QOL and cognitive function, and depression.MethodForty‐seven elderly patients with cognitive impairment and their caregivers participated. Patient QOL was assessed using by the Korean version of Quality of Life‐Alzheimer's Disease (KQOL‐AD) scale. Each patient and caregiver rated the patient QOL. Korean Mini Mental Status Exam in the Korean version of the CERAD assessment packet(MMSE‐KC) and Short form of the Geriatric Depression Scale(SGDS) were also assessed.ResultThere were no significant differences between patient and caregiver ratings on all QOL items except marriage. There were significant correlations between patient and caregiver reports of patient on all QOL items except marriage and total KQOL‐AD scores. Total KQOL‐AD Scores of patient QOL were positively significant correlated with MMSE‐KC scores and were negatively significant correlated with SGDS scores.ConclusionPatient and caregiver reported QOL in elderly patients with cognitive impairment did not differ. Elderly patients seem to be able to assess their own QOL properly although they have impairment in cognitive function. Depression of elderly patients with cognitive impairment adversely affects patient QOL.

Home Health Care Intervention regarding Quality of Life for Elderly Patients with Chronic Obstructive Pulmonary Disease

Background: Chronic obstructive pulmonary disease represents an important public health challengeand it's a major cause of chronic morbidity and mortality of the elderly people throughout the world.The aim of this study was to evaluate home health care intervention regarding quality of life for elderlypatients with chronic obstructive pulmonary disease in Benha City. Research design: A quasiexperimental design was utilized in this study. Setting: This study was conducted in Outpatient Clinicsof Chest Hospital in Benha City. The sample: Purposive sample 100 elderly patients with chronicobstructive pulmonary disease. Tools: Three tools are used I): A structured interviewing questionnairewhich consists of four parts to assess A): Demographic characteristics of the studied elderly patients, B):Medical history of the studied elderly patients with COPD, C): Knowledge of the studied elderly patientsregarding chronic obstructive pulmonary disease D): Reported practices of the studied elderly patientsregarding COPD, II): Scale to measure quality of life of the studied elderly patients with chronicobstructive pulmonary disease, III): Observational checklist: A- was used to observe practices of thestudied elderly patients, B- was used to observe home of the studied elderly patients. Results: 55% ofthe studied elderly patients had good knowledge post intervention, 81% of the studied elderly patientswere satisfactory practices post intervention, 72% of the studied elderly patients had good total scores ofquality of life post intervention. Conclusion: The home health care intervention succeeded to improveknowledge, practices and quality of life of the studied elderly patients. Recommendations: Continuoushome health care intervention for the elderly patients with chronic obstructive pulmonary disease toincrease their knowledge and improve their practices

Effects of Orem's Self-Care Model on the Life Quality of Elderly Patients with Hip Fractures.

Background Hip fractures of elderly patients are a public health problem worldwide, mostly lying in bed for a long time; therefore, the importance of life quality in such patients is an issue beyond question. Orem's self-care model is a nursing pattern which is introduced with the purpose of improving the self-care ability of individuals, especially the patients suffering from diseases with limits on activity. Objective The aim of this study was to determine the effects of Orem's self-care program on life quality of senile patients with hip fractures. Methods A randomized clinical trial study was conducted on 130 eligible old patients suffering from hip fractures who were selected using easy sampling methods and allocated randomly into two groups of experiment and control. The data were collected through validated questionnaires including visual analogue scale (VAS) and Barthel index for them. The experiment group was treated according to Orem's self-care model, and the control group was treated on the basis of the traditional care model. The data of complications including pneumonia, deep venous thrombosis, urinary infection, wound problem, and bedsore were also gathered. Results As revealed, mean scores of VAS and Barthel index one week after operation in the experiment group were significantly different from the control one (P < 0.05, P ≤ 0.001). The changes of VAS and Barthel index six weeks postoperatively of the two groups were also statistically significant (P < 0.05, P ≤ 0.001). Compared with the control group, the difference of complications reduced significantly in the experiment group (P < 0.05). Accordingly, educational intervention according to Orem's self-care model seemed to be effective in promoting self-care ability for these senile patients. Conclusions According to the obtained results, a self-care program based on Orem's model for elderly patients with hip fractures can improve life quality and reduce perioperative complications significantly. Therefore, it is recommended that this nursing program should be taken into account as a part of treatment measures for these patients.

MON-268 Factors Associated With QoL Impairment In Patients With Acromegaly In The Elderly

Background: With rapid aging in the society and the improvement of prognosis in patients with acromegaly, treatment goals have been changed. In elderly patients with acromegaly, improvement in the quality of life (QoL) is one of the most important treatment goals. However, factors associated with QoL in elderly patients with acromegaly remains unclear. Objective: To clarify the differences in the QoL in patients with acromegaly between the young and elderly, and to identify the factors associated with the impairment in the elderly. Patients and Methods: This is a retrospective cross-sectional single-center study. Eighty patients with acromegaly (male 49%, the mean age 59.6±12.6 years, mean IGF-I SD score 0.7±2.3), who had been followed up in the outpatient clinic at Kobe University Hospital, were enrolled. Patients were divided into following 2 groups; Elderly group (E group); 65 years of age or more (n =34, male 41%, mean age 71.4±4.1 years), or Young group (Y group); younger than 65 years old (n =46, male 50.9%, mean age 50.9±9.2 years). We evaluated QoL using AcroQoL and analyzed the associated factors by multivariate analysis. Results: In the comparison of E and Y group, IGF-I SD score (0.4±1.6 in E group vs. 0.9±2.7 in Y group) and the way of treatment (surgery; 65% vs 72%, current medical therapy; 56% vs 43%, past radiation therapy; 15% vs 9%) were comparable, while disease duration was longer in E than Y group (13.7±10.9 vs 8.3±8.1 years, p =0.02). Arthropathy was more frequent in E group (50% vs 11%, p <0.01).In total, current medical therapy and hydrocortisone replacement were associated with lower total scale (β =-0.27, p =0.01 and β =-0.35, p <0.01). Then, we separately analyzed the associated factors in each E and Y group. In Y group, current medical therapy and hydrocortisone replacement were associated with lower total (β =-0.48, p <0.01, and β =-0.35, p <0.01) and physical scales (β =-0.40, p <0.01, and β =-0.31, p <0.01). Past radiotherapy for the pituitary tumor was also associated with lower mental scale (β =-0.40, p <0.01). On the other hand, in E group, arthropathy and high BMI were associated with lower total scale (β =-0.41, p =0.02, and β =-0.40, p =0.02) and current hydrocortisone replacement was associated with lower physical scale (β =-0.36, p =0.03). Conclusions: AcroQoL score was impaired both in E and Y group in patients with acromegaly. In total, current medical therapy and hydrocortisone replacement were associated with lower QoL scale as previously reported; however, when stratified by age, arthropathy and high BMI were associated with lower QoL scale in the elderly. These data clearly indicate that the factors influence on QoL are different depending on the age. Although causal relationship remains unclarified, these data suggest that when treat the patients, it may be important to avoid these complications.

Mediating effect of self-perceived aging on the relationship between family care and quality of life among elderly patients with type 2 diabetes mellitus

Objective To explore the mediating effect of self-perceived aging between family care and quality of life (QOL) of elderly type 2 diabetes patients, and to provide the evidence for improving the QOL of elderly type 2 diabetes patients. Methods The convenience sampling method was used to investigate 215 elderly patients with type 2 diabetes by using the Brief Ageing Perceptions Questionnaire, the Family APGAR Index and the Diabetes Specific Quality of Life Scale, and statistical analysis was performed using SPSS20.0 software. Results The self-perceived aging score of the elderly patients with type 2 diabetes was (49.90±12.11), the family care score was (7.17±2.53), and the QOL score was (65.89± 12.07). Family care score was negatively correlated with QOL score in elderly patients with type 2 diabetes (r value was -0.480, P<0.01), self-perceived aging score was negatively correlated with family care score (r value was -0.472, P<0.01), and positively correlated with QOL score (r value was 0.688, P<0.01). The mediating effect test showed that self-perceived aging played a complete mediating role between family care and QOL in elderly type 2 diabetes patients, and the mediating effect accounted for 86.51% of the total effect. Conclusions Self-perceived aging is the mediating variable of family care and QOL, and establishing positive self-perceived aging is conducive to improving the QOL of elderly patients with type 2 diabetes.
