This study aims to determine the effectiveness of the use of multimedia in education in improving compliance and quality of life of kidney transplant patients. The method used in writing this article is literature review on three databases ProQuest, PubMed, ScienceDirect using the keywords multimedia, education, adherence AND 'quality of life' and 'kidney transplantation’. The results of a review show that the types of multimedia that can be used in education include animated videos, applications and artificial intelligence chatbots. The conclusion of this study is that the use of multimedia in education effectively improves adherence and quality of life for kidney transplant patients. However, the use of multimedia in education must consider accessibility, the elderly population and patients with visual and/or cognitive impairments. The effect of ADOPT nursing intervention mode on the compliance and quality of life of kidney transplant patients in 1-3 years after surgery

Objective: To explore the effect of ADOPT (Attitude, Definition, Open mind, Planning, Try it out) nursing intervention mode on the living habit compliance, self-monitoring compliance and quality of life of kidney transplant patients in 1-3 years after surgery.Methods: From December 2020 to August 2021, 48 patients in 1-3 years after kidney transplantation who met the inclusion and exclusion criteria and were regularly followed up in the Kidney Transplantation Clinic of the Department of Urology Surgery in Baogang Hospital were selected as the research subjects, and 48 kidney transplant patients were divided into the intervention group and the control group by simple random sampling. The control group was given routine nursing care in the Kidney Transplantation Clinic of the Department of Urology Surgery, and the intervention group was given nursing care measuresformulated on the basis of ADOPT nursing intervention mode. The compliance and quality of life of the two groups were compared with the Kidney Transplant Patient Compliance Scale and the Kidney Transplant Patient Quality of Life Related Rating Scale in 1-3 years after surgery.Results: After 2 months and 4 months of intervention, the scores of living habit compliance and self-monitoring compliance in the intervention group were significantly higher than those in the control group (p < .001), and the scores of quality of life in the intervention group and the total score of quality of life were better than those in the control group, and the difference was statistically significant (p < .001).Conclusions: The implementation of the ADOPT nursing intervention mode for kidney transplant patients in 1-3 years after surgery can improve their compliance level and enhance the quality of life, which is worthy of reference.

Examining the Relationship Between Sleep Quality and Quality of Life in Kidney Transplant Patients

Objective: This study aimed to examine the relationship between sleep quality and quality of life in patients who had a kidney transplant. Materials and Methods: The research has a cross-sectional and descriptive design. The sample of the study consisted of 77 patients who had a kidney transplant at a university hospital organ transplantation centre between August 2020 and August 2021 and were followed up in the outpatient clinic. An individual identification form, the Quality-of-Life Short Form, and the Pittsburgh Sleep Quality Index were used to collect research data. Descriptive statistics (percentage and number), Pearson’s correlation test, Student’s t-test for independent variables, ANOVA, and linear regression analysis were used in the research analysis. Results: In the study, the mean age of the patients was 47.19±11.29 (min: 24, max: 73), and the mean transplantation time was 94.92±75.23 months. The mean PSQI and SF-36 scores of kidney transplant patients were 4.91±3.09 and 65.49±18.18, respectively. There was a statistically significant correlation between sleep quality and quality of life in kidney transplant patients (r=-0.511, p=0.0001). Conclusions: In the study, the quality of life increased as the sleep quality of the patients increased.

Reduction of arteriovenous access blood flow in kidney transplant patients.

Cardiovascular disease is the most frequent cause of death in kidney transplant patients. High-flow arteriovenous fistula (AVF) increases cardiac output and may contribute to hyperkinetic heart failure. AVF follow-up is not implemented in kidney transplant patients. The aim of this study was to reduce AVF blood flow in a group of patients with a high-flow AVF following kidney transplantation to reduce cardiac strain. This prospective study was performed in kidney transplant patients who had a vascular access created before transplantation. The AVF of these patients was examined by ultrasound with a focus on AVF flow and brachial artery size. If high-flow AVF was detected, flow reduction was performed in the indicated group of patients. The study examined 164 patients, of whom 24 had a hyperfunctional AVF (14.8%). A total of 16 AVF flow reductions were performed, which led to an average decrease in cardiac index by 0.77 L/min/m2. Primary patency of the reconstructions was 93.33% after 12 months. All patients experienced a subjective improvement in dyspnea. Reduction in AVF flow leads to an improvement in the quality of life of kidney transplant patients. Maintaining a functional AVF is beneficial, especially for patients after repeated surgeries, where the option of creating an autologous AVF is limited.

Self-Care Education Program as a New Pathway Toward Improving Quality of Life in Kidney Transplant Patients: A Single-Blind, Randomized, Controlled Trial.

Despite the benefits of kidney transplant, kidney recipients are confronting many challenges that affect the quality of their lives. Implementation of the most effective self-care education program based on the needs of patients may address these challenges and improve their quality of life. This study aimed to determine the effects of a self-care education program on the quality of life in kidney transplant patients. In this single-blind, randomized, controlled trial, 59 patients who met inclusion criteria were selected using a convenience sampling technique. Patients were randomly allocated into intervention (n = 29) and control (n = 30) groups. The intervention group received a bedside self-care education program in 3 sessions. The control group only received routine care. Data were collected by demographic and kidney transplant questionnaires before and after the intervention. Measuring the mean score of quality of life was the primary outcome in this study. We used descriptive and inferential statistics to analyze the data. A significant difference was observed in the mean score of quality of life between both groups after intervention (P < .001). The mean score of quality of life increased significantly in the intervention group after the self-care education program (P < .001). A self-care education program is a more effective approach to improve the knowledge and skills of transplant patients. Thus, we suggest an emphasis on teaching self-care knowledge and skills for transplant patients in nursing care education programs. This can ultimately lead to quality of life improvement in kidney transplant patients.

QUALITY OF LIFE AND ASSOCIATED FACTORS OF KIDNEY TRANSPLANT PATIENTS IN ETHIOPIA

Introduction: Kidney transplantation (KT) is the preferred replacement treatment for patients with chronic kidney disease (CKD). It provides better quality of life (QoL) besides many other benefits. World Health Organization defines QoL as an individual’s awareness of their position in life in the context of the culture and value systems in which they live and relate to their goals, hopes, standards and concerns. Several studies on QoL of patients who had KT are done using standardized qualitative and quantitative methodologies around the world. We haven’t come across publication on QoL after KT from Africa. This study was done to determine the quality of life and associated factors of kidney transplant patients in Ethiopia, Eastern Africa. Methodology: The study was conducted at Ethiopian Ministry Health’s National kidney transplant center located at Saint Paul Hospital Millennium Medical College, Addis Ababa Ethiopia. It was an institution-based cross-sectional study design. A validated questionnaire KTQ-25 was used for data collection on 97 kidney transplant patients. The study was conducted from Jan 28, 2019 to April 5, 2019. Descriptive analyses of demographic and QoL variables were conducted. Also demographic and patient characteristic variables included in and bivariate analyses were conducted using student t-test and one way analysis of variance or ANOVA were used to determine relationships between demographics, and QoL. Results and Discussion: The mean of QoL in this study was 6.06 ∓ 0.79, the maximum score was obtained in appearance dimension and the minimum score was obtained in fear dimension. In comparing the quality of life scores based on the socio-demographic factors, no statistically significant difference was observed. In comparing the quality of life scores based on health-related and functional characters a statistical difference was observed between the quality of life score based on the time after transplant (p=0.035) and the duration returning to their job after transplant (p=0.031). P-value < 0.05 is considered as statistical significance. Published studies couldn’t be found about quality of life in kidney transplant patients as many worldwide and couldn’t come across with such studies in Africa. Despite this fact QoL in Ethiopia is good measuring a mean score of 6.068 + 0.79 which is higher than that of Rebollo in Spain (5.9 ∓1.18), Oveilly study in US (4.5 ∓1.2), Iran (4.9 ∓1.27) and Chinese (4.8 ∓1.43) which has been reported with the same questionnaire. Conclusions: Even though Kidney transplantation was started only four years back, Ethiopian kidney transplant patients’ quality of life level is better. Time after transplant and the duration to return to their job after transplant has a statistically significant association with quality of life. Having a counseling session for those who score low in quality of life domains is recommended. Comparison of the Effect of Multimedia and Booklet Methods on Quality of Life of Kidney Transplant Patients: A Randomized Clinical Trial Study.

ABSTRACTBackground: Proper education can improve the quality of life. Multimedia as an interactive educational method and booklet as a traditional one have their own unique effects. This study aimed at compare the interactive multimedia and booklet methods at the time of discharge on the quality of life of kidney transplant patientsMethods: In this single-blind interventional study, 80 patients from Bouali Hospital, Shiraz, from September 2017 until June 2018 were selected by convenience sampling, and then divided into two groups (booklet and interactive multimedia) by block randomization. Intervention for the first group included short and long-term care information in the form of a booklet, and an interactive multimedia CD with the same content for the second group. Quality of life was assessed by kidney transplant questionnaire (KTQ) at the time of discharge and after two months. Adherence to education was verified by phone. The data were analyzed through SPSS v.24 software using independent and paired t-tests. Plt;0.05 was considered significantResults: The mean overall quality of life score in both groups increased significantly, P=0.044 for the booklet group and P=0.039 for the multimedia group. In emotional, fatigue, uncertainty/fear domains, both groups showed improvement after the intervention (P<0.05). However, in physical symptoms and appearance domains, differences were not significant, before and after education. After intervention, no significant difference was observed between the two groups in the overall and specific domains of quality of life score (P=0.437)Conclusion: Both methods had positive impacts on the quality of life and emotional, fatigue and uncertainty/fear domains. Effect of nursing intervention guided with self-management on self-management behavior and quality of life of kidney-transplant patients

Objective To investigate the effect of nursing intervention guided with self-management on self-management behavior and quality of life of kidney-transplant patients .Methods 192 eligible subjects were randomly divided into the experimental group （ n =99 ） and the control group （ n =93 ） .The control group received conventional nursing and follow-up visits while the experimental group adopted nursing intervention guided with self-management in addition , including collective education , group discussion , individual guidance and information.Self-management scale and quality of life scale were used to assess patients ’ self-management behavior and quality of life before and 3 months after intervention .Results There was no statistically significant difference of self-management behavior and quality of life between two groups before intervention （P〉0.05）. But after intervention , there was statistically significant difference of self-management behavior and quality of life between two groups （t=9.25,-6.65, respectively;P〈0.05）.In the experimental group, the score of self-management scale before and after intervention was respectively （93.22 ±3.28） and（100.44 ±4.44）, the score of quality of life was （12.16 ±1.46）and（10.92 ±0.92）, and the differences were statistically significant （t=-9.11,5.01, respectively; P〈0.05）.Conclusions Nursing intervention guided with self-management can improve the self-management level and quality of life of kidney transplant patients and has clinical significance for preventing rejection and increasing survival rate . Health‐related quality of life in kidney transplant patients with diabetes

We sought to assess the disutility associated with diabetes in the kidney transplant population. We enrolled 233 kidney transplant recipients age 18-74 from a Midwestern hospital outpatient department. Recipients with multiple or multi-organ transplants, those with laboratory evidence that suggests acute cellular damage (creatinine-kinase > 200 U/L), or a diagnosis of acute renal failure or acute rejection were excluded from the analysis (n = 33). Participants health-related quality of life (HRQOL) were evaluated using the Euro-QoL-5 Dimension (EQ-5D), Health Utility Index Mark III (HUI-III), and the Short Form-6D (SF-6D), which was calculated from the generic section (SF-12) of the Kidney Disease Quality of Life 36 (KDQOL-36). We estimated health utilities associated with diabetes using general linear modeling after adjusting for demographic, socioeconomic, and clinical characteristics. The adjusted health disutilities associated with diabetes were clinically and statistically significant: EQ-5D (Δ = 0.05; p < 0.01), HUI-III (Δ = 0.09; p < 0.01), and SF-6D (Δ = 0.04, p < 0.01). There was no difference between diabetic patients with good glycemic control (mean serum glucose <126 mg/dL in the three months prior to enrollment) and patients with poor glycemic control. Among kidney transplant patients between the ages of 18-74, non-diabetics have significantly higher HRQOL scores on the EQ-5D, HUI-III, and SF-6D compared with patients with diabetes.

Health-related quality of life in kidney transplant patients and non-renal replacement therapy patients with chronic kidney disease stages 3b–4

Patients with a functioning kidney transplant (Tx) and patients with chronic kidney disease (CKD) not in dialysis report better health-related quality of life (HRQoL) than patients requiring dialysis, but poorer than the general population. HRQoL is associated with kidney function, but it is unknown whether the kidney function per se is the main determinant of HRQoL. The aim of this study was to compare the HRQoL in 2 groups of patients with CKD: 1 group with native kidneys only (non-renal replacement therapy [non-RRT] group) and 1 group with a functioning kidney transplant (Tx group). The study was designed as a paired cross-sectional single-center study including 38 stable Tx patients age- and gender-matched with 38 non-RRT patients with the same kidney function, CKD stages 3b–4. HRQoL was evaluated using the short form-36 (SF-36) and a visual analogue scale (VAS). The multi-item scales and summary scores in SF-36 were not significantly different between the 2 groups of patients or the general Norwegian population. However, the non-RRT group scored significantly better than the Tx group when HRQoL was evaluated by VAS. The main determinants for HRQoL in both groups of patients were depression estimated by Beck depression inventory scores and comorbidity expressed by Davies comorbidity index scores. HRQoL evaluated by SF-36 in a group of stable Tx patients in CKD stages 3b–4 is comparable to that of a group of non-RRT patients. However, HRQoL VAS was better in the non-RRT group, suggesting that VAS and SF-36 may evaluate different aspects in HRQoL in the same group of patients.

The Health Related Quality of Life for Kidney Transplant Patients in Bulgaria—A Pilot Study

ABSTRACTThe aim of the present study was to analyze the pharmacotherapy cost and quality of life of patents after kidney transplantation compared to those with chronic kidney disease in Bulgaria. This retrospective pharmacotherapy cost study and prospective health related quality of life (QoL) study was performed during 2010–2011 at the biggest Sofia hospital serving all transplant patients. The cost of pharmacotherapy was analyzed after reviewing patients' records. The health related quality of life was evaluated with the multi-attributable questionnaire SF-36, which measures 9 health domains.Forty-two percent of the patients with chronic kidney disease, and 26% of those with kidney transplant were female. The average age of the transplanted patients was 39.7 for male patients and 42.2 for female ones, while in the group suffering from chronic kidney disease the average male age was 48.3 and that of female patients, 51.2. No statistically significant difference was observed among the mean monthly cost of therapy per patient during the two-year period. The QoL was with lower values in the role limitations domain for patients with chronic kidney diseases and this difference was statistically significant. We found a correlation among the mean cost of pharmacotherapy and mean QoL in both groups of patients. In the group of patients with kidney transplantation the place of living and general health state were positively correlated. The cost of pharmacotherapy and the health state in comparison with the previous year were also positively correlated.This is the first Bulgarian study of the quality of life of kidney transplant patients. It shows that the SF-36 could be successfully used in this group of patients as well as for comparison with the QoL of patients with chronic kidney disease.

Loss of Renal Function Is Associated With Deterioration of Health-Related Quality of Life in Kidney Transplant Patients

Objective The evaluation of health-related quality of life (HRQOL) is becoming an important measure of the outcomes of kidney transplantation. The aim of this study was to evaluate whether deterioration of renal function was associated with a worse HRQOL in kidney transplant patients (KTP) compared with patients experiencing chronic native kidney insufficiency. Patients and Methods HRQOL was assessed in 128 stable KTP and 102 chronic kidney disease patients (CKDP) using the SF-36 health survey. The 2 groups were matched for age, sex, sociodemographic conditions, and renal function, the only difference being that KTP had experienced hemodialysis treatments before transplantation. Results Overall, KTP revealed a satisfactory HRQOL compared with CKDP. At variance with CKDP, KTP with estimated creatinine clearances >60 mL/min versus <60 mL/min showed higher scores among 7 of 8 SF-36 categories: physical function (PF), role physical (RP), bodily pain (BP), general health (GH), vitality (VT), role emotional (RE), and mental health (MH). Estimated creatinine clearance showed a significant positive correlation with PF ( P = .0004), RP ( P = .008), BP ( P = .01), GH ( P = .0001), VT ( P = .001), RE ( P = .03), and MH ( P = .02), but exclusively in KTP. Multiple regression analysis confirmed in KTP that the scale scores of PF, RP, GH, VT, and RE were significantly dependent on creatinine clearance. Conclusion Our data demonstrated that among KTP deterioration of renal function was associated with a worse HRQOL.

Effect of a DanJeon Breathing Exercise Program on the Quality of Life in Patients With Kidney Transplants

This quasi-experimental study attempted to show that nursing intervention using the DanJeon Breathing Exercise Program (DJBEP) improved the quality of life of recipients after kidney transplantation. DJBEP progressed in three steps. We prospectively included 29 outpatient volunteers: experimental group: n = 15; control group: n = 14. DJBEP derived from the Roy's adaptation model decreased both the stress and the uncertainty of kidney transplantation recipients. It has also been shown to restore serum cholesterol and serum creatinine levels and enhance strength and flexibility. Simultaneously, self-esteem was enhanced, and eventually adaptation was promoted both physiologically and psychologically. The quality of life of kidney transplantation recipients was enhanced. DJBEP played an effective role as a nursing intervention to promote the quality of life of kidney transplant patients by increasing their physiological and psychological status.

The effects of gender and age on health‐related quality of life following kidney transplantation

To evaluate whether gender, age and their interaction affect health-related quality of life and overall health status following kidney transplantation. Some investigators have examined the main effects of gender and/or age on health-related quality of life following kidney transplantation, but the potential interaction effect of these measures on this outcome has not been reported. This was a cross-sectional, single-centre study, based in one US geographic area. Self-report survey data were provided by adult kidney transplant recipients using the SF-36 Health Survey (SF-36) and a visual analogue scale of overall health. SF-36 physical and mental component summary and individual scales and overall health were measured prospectively at one time point post-transplant. All adult patients were eligible to participate and rolling enrolment was employed. Statistical effects were tested using analysis of covariance (controlling for time post-transplant). Subjects (n = 138) included 66 women and 72 men. There were no effects of gender, age group, or their interaction on MCS or overall health scores (all p >or= 0.12). Physical component summary scale data demonstrated: (i) a significant effect of gender (p = 0.025); (ii) a statistically marginal effect of age group (p = 0.068); and (iii) a statistically marginal gender by age group interaction effect (p = 0.066). Women reported poorer scores on the SF-36 physical functioning (p = 0.049), role physical (p = 0.014) and bodily pain scales (p = 0.028). There was an effect of age group on physical functioning (p = 0.005), with younger patients reporting higher scores. Women report lower scores on several physical measures and may experience a greater reduction with age in physical health-related quality of life than men. Physical functioning declines with age following kidney transplantation. Findings may help healthcare professionals to develop gender- and age-specific interventions to optimize health-related quality of life of kidney transplant patients.

Spanish validation of the "Kidney Transplant Questionnaire": a useful instrument for assessing health related quality of life in kidney transplant patients

BackgroundThere is a growing interest in the evaluation of Health Related Quality of Life (HRQoL) among patients undergoing Renal Replacement Therapy. In Spain, no specific questionnaire exists for kidney transplant patients. Here we present the Spanish validation of the first specific HRQoL assessment tool: the kidney transplant questionnaire (KTQ).MethodsProspective study of 31 patients on transplant waiting list who received the first kidney. Patients were evaluated before transplant and after 1, 3, 6 and 12 months, using the KTQ and the SF-36 Health Survey. Feasibility, validity, reliability, and sensibility to change were evaluated.ResultsMean time of administration of the KTQ was 12 minutes. Correlation coefficients among KTQ dimensions range between 0.32 and 0.72. Correlation coefficients of KTQ dimensions with SF-36 PCS were low (r<0.4), and with SF-36 MCS were moderate-high (r>0.4) except for Physical Symptom dimension (r = 0.33). Cronbach's Alpha was satisfactory for all KTQ dimensions (Physical Symptoms = 0.80; Fatigue = 0.93; Uncertainty/Fear = 0.81; Emotional= 0.90) except Appearance (0.69). Intraclass correlation coefficients ranged between 0.63 and 0.85, similar to those of the original KTQ version.ConclusionsResults of validation study show that feasibility, validity, reliability and sensibility to change of the Spanish version of the KTQ are similar to those of the original version.
