The Influence of Lid-Parallel Conjunctival Folds and Conjunctivochalasis on Dry Eye Symptoms with and Without Contact Lens Wear: A Review of the Literature.

Lid-parallel conjunctival folds (LIPCOF) and conjunctivochalasis (CCH) are similar conditions that seem to be related to dry eye severity. In addition, there is a lack of studies on the topic of LIPCOF and CCH on dry eye symptoms in non-contact lens and contact lens (CL) wearers. Therefore, the aim of this study is to review the relationship of LIPCOF and CCH with dry eye symptoms in non-CL and CL wearers, as well as to report the treatment of dry eye disease (DED) in non-CL and CL wearers who present LIPCOF or CCH. A literature review of full-length original studies in two databases, PubMed and Scopus, was performed. The search period included observational studies in humans published between April21, 2009 and March20, 2023. A total of 26 studies were included. The studies suggest that LIPCOF and CCH are significantly related to dry eye symptoms in non-CL and CL wearers. However, the impact of CL wear on LIPCOF and CCH may be complex and may vary depending on individual factors such as lens type, lens care regimen, and pre-existing ocular conditions. Regarding LIPCOF management, tear substitutes seem to significantly reduce LIPCOF and dry eye symptoms in non-CL wearers, while vectored thermal pulsation (VTP) and microblepharoexfoliation (MBE) are suggested as promising treatment for LIPCOF and dry eye symptoms in CL wearers. Regarding CCH management, surgical interventions may be effective in reducing CCH when medical treatments have no response. In addition, an ocular examination by slit lamp is necessary to distinguish both conditions. Tear substitutes, VTP, MBE, and fitting CLs with low coefficient of friction (CoF) seem to reduce and prevent LIPCOF. However, surgical treatment options seem to be more effective in the complete elimination of CCH. An adequate evaluation and differentiation between LIPCOF and CCH are important, and they should be considered by practitioners in managing dry eye symptoms in non-CL and CL wearers.

Clinical characterisation of contact lens discomfort progression

PurposeThis study aimed to assess the subjective and objective differences among the steps of the contact lens discomfort (CLD) progression classification established by the Tear Film & Ocular Surface Society (TFOS) using questionnaires and clinical signs, and to propose a simplified classification. MethodsContact lens (CL) wearers were evaluated in a single visit. The Contact Lens Dry Eye Questionnaire (CLDEQ)-8, the Contact Lens Discomfort Index, and Visual Analog Scales for discomfort and dryness were administered. The non-invasive break-up time, the tear film lipid layer thickness, conjunctival hyperaemia and papillae, lid-parallel conjunctival folds, the fluorescein tear film break-up time, corneal and conjunctival staining, lid wiper epitheliopathy, and the Schirmer test were assessed. Sign and symptom scores were compared among TFOS CLD progression steps using analysis of variance or the Kruskal–Wallis H test. Steps 1 and 2 (reduced comfort), and steps 3 and 4 (reduced wearing time) of the TFOS classification were combined to obtain a simplified classification, and the same comparison was performed. A p-value ≤ 0.05 was considered statistically significant. ResultsOne hundred-fifty CL wearers (97 women and 53 men) aged 34.4 ± 12.6 years were included. In the TFOS classification, there were significant differences between step 0 (no CLD) and the rest of the severity steps for the scores obtained in all questionnaires (p ≤ 0.015). All steps were differentiated (p ≤ 0.032) based on the simplified classification for all questionnaires, except steps 1 and 2 for the CLDEQ-8 and dryness VAS (p = 0.089 and p = 0.051, respectively). There were no differences (all p ≥ 0.06) between the sign scores among the steps of either classification. ConclusionCLD management is encouraged from its first appearance. Simplifying the phases of CLD severity may allow a more accurate classification and a better awareness of the problem by clinicians and CL wearers by using more straightforward simple messages.

Ocular Surface Evaluation in Immunoglobulin G4–Related Ophthalmic Disease

To evaluate the functional and structural changes of the meibomian glands and ocular surface in immunoglobulin G4-related ophthalmic disease (IgG4-ROD) patients. Cross-sectional, matched case-control comparison study. This study included 64 patients with biopsy-proven IgG4-ROD (aged 63.4 ± 12.2 years, 39 male) and 64 sex- and age-matched healthy controls. Patients were managed by hospitals covering the publicly funded ophthalmology service in Hong Kong. Outcome measures included anterior segment examination and keratographic and meibographic imagings. A total of 64 worst-affected eyes of the 64 IgG4-ROD patients were analyzed. Corneal fluorescein staining (P=.0187), lid margin telangiectasia (P=.0360), lid-parallel conjunctival folds (P=.0112), papillae (P=.0393), meibomian gland plugging (P=.0001), meibomian gland expressibility (P=.0001), and meibum quality (P=.0001) were more significant in IgG4-ROD patients compared with healthy controls. Both upper and lower meibomian gland dropouts (P=.001 and .0003), and tear meniscus height (P=.0001) were higher in IgG4-ROD patients. Non-invasive tear break-up time (NITBUT) (P=.0166) and Schirmer test results (P=.0243) were lower in IgG4-ROD patients. Upper (r=0.336, P=.0140) meibomian gland dropouts and NITBUT (r=-0.293, P=.0497) were positively and negatively correlated with the IgG4-ROD onset age, respectively. The number of extraocular organ involvement was negatively correlated with the Schirmer test(r=-0.341, P=.0167). Lower NITBUT was found in IgG4-ROD eyes with lacrimal gland enlargement than in IgG4-ROD eyes without lacrimal gland enlargement radiologically (P < .0001). IgG4-ROD patients showed features of both aqueous tear deficiency and evaporative dry eye disease. We recommend ocular surface evaluation to all patients newly diagnosed with IgG4-ROD. Further studies are warranted to clarify the mechanism of IgG4-related dry eye disease.

Evaluation and correlation analysis of ocular surface disorders and quality of life in autoimmune rheumatic diseases: a cross-sectional study

ObjectiveThis cross-sectional study aimed to reveal the association between ocular surface disorders and psychological, physiological situations among autoimmune rheumatic patients.MethodsNinety autoimmune rheumatic patients (180 eyes) hospitalized in the Department of Rheumatology, The Second Xiangya Hospital, Central South University and 30 controls (60 eyes) were enrolled in the study. All participants were assessed for ocular surface disorders including dry eye disease (DED) by the Ocular Surface Disease Index (OSDI) for symptoms evaluation, and slim lamp examinations for tear break-up time (TBUT), meibomian gland secretion, symblepharon and corneal clarity, Schirmer I test, corneal fluorescein staining (CFS), lid-parallel conjunctival folds (LIPCOF). Systematic conditions were evaluated using the Short Form 36-Health Survey (SF-36) for health-related quality of life, Hospital Anxiety and Depression Scale (HADS) for anxiety and depression, Health Assessment Questionnaire-Disability Index (HAQ-DI) for difficulties in activities of daily living, and Pittsburgh Sleep Quality Index (PSQI) for sleep quality. Pearson and spearman’s analysis were conducted to examine the relationship between systematic conditions and ocular surface conditions.ResultsThe analyses were controlled for age and sex. 52.22% of eyes (94 in 180) of autoimmune rheumatic patients and 21.67% of eyes (13 in 60) of controls were diagnosed with DED. The autoimmune rheumatic patients showed significant higher OSDI score, fewer basal tear secretion, more severe CFS and conjunctivochalasis than controls. There were no statistically significant differences in TBUT, meibomian gland secretion, symblepharon, and corneal clarity between the two groups. For systematic conditions, autoimmune rheumatic patients had significantly lower SF-36 scores, higher anxiety scores, and HAQ-DI scores than controls. No statistically significant differences were detected in depression scores and PSQI between the two groups. Among autoimmune rheumatic patients, OSDI scores were moderately correlated with quality of life, anxiety, depression and sleep quality.ConclusionFactors including quality of life, anxiety, depression, and sleep quality are associated with ocular surface conditions, especially DED symptoms. Management of systemic conditions and psychotherapy should also be considered as part of the treatment among autoimmune rheumatic patients.

Meibomian Gland Assessment in Routine Ophthalmology Practice.

This cross-sectional study aimed to investigate the connection between meibomian gland (MG) excreta quantity and quality after MG expression (MGX), dry eye disease (DED) symptoms, and objective DED signs and to clarify the relationship between dry eye and MG function in DED pathophysiology. The study included 200 subjects, 100 with and 100 without dry eye symptoms. Schein questionnaire was used to determine the severity of dry eye symptoms and self-reported skin type for facial skin dryness self-evaluation. Objective dry eye signs were assessed by monitoring conjunctival hyperemia, lid parallel conjunctival folds (LIPCOF), tear break-up time (TBUT), fluorescein surface staining and digital MGX. Subjects with DED symptoms had significantly lower MG quantity scores than healthy controls (p < 0.001). Meibum quality and quantity scores significantly correlated with female gender (p = 0.002), Schein questionnaire score (p < 0.001), fluorescein corneal staining score (p = 0.019), self-reported skin type (p < 0.001), TBUT (p < 0.001) and LIPCOF (p = 0.041). After adjustment for age and gender in a logistic regression analysis, dry eye was independently and significantly associated with self-reported skin type (OR 0.73, p < 0.001), LIPCOF (OR 1.04, p < 0.001), fluorescein corneal staining (OR 1.05, p = 0.019), TBUT (OR 0.77, p < 0.001) and meibum quantity score (OR 0.59, p < 0.001). Dry eye symptoms and objective signs correlated well in this study. MGX discriminated between the subjects with and without DED symptoms and was associated with other objective DED signs. Results showed a significant association between meibum quality and quantity, MG function, DED and facial skin dryness self-perception. This paper established a correlation between dry eye symptoms caused by MG dysfunction and dry skin, which can help general health practitioners consider dry eye as a cause of chronic eye complaints with patients who report dry skin.

Ocular surface disorders affect quality of life in patients with autoimmune blistering skin diseases: a cross-sectional study

BackgroundAutoimmune blistering skin diseases (AIBD) are a group of rare chronic autoimmune diseases which are associated with ocular surface diseases especially dry eye disease. This study is designed to investigate the relationship between ocular surface disorders and quality of life among patients with autoimmune blistering skin diseases.MethodsTwenty-four AIBD patients (18 pemphigus and 7 pemphigoid) and twenty-five non-AIBD controls were included. Ocular surface disease index (OSDI), ocular surface evaluation, including slit-lamp examination, Schirmer I test, tear break-up time, corneal fluorescein staining, lid-parallel conjunctival folds, meibomian gland evaluation, presence of symblepharon and corneal opacity were assessed. Life quality was evaluated by multiple questionnaires, including Medical Outcomes Study 36-Item Short Form Questionnaire (SF-36), Hospital Anxiety and Depression Scale (HADS), Pittsburgh Sleep Quality Index (PSQI) and Health Assessment Questionnaire-Disability Index (HAQ-DI). Ocular surface tests and quality of life were compared between AIBD patients and non-AIBD controls. In the AIBD patients, the associations between ocular surface parameters and quality of life were also evaluated.Results92% of AIBD patients and 87.5% of age- and sex-matched non-AIBD controls were diagnosed with dry eye in this study. Compared with non-AIBD controls, AIBD patients reported lower SF-36 scores (P < 0.05) and severer OSDI, Schirmer I test, tear break-up time, corneal fluorescein staining, presence of symblepharon and corneal opacity measures (P < 0.05). OSDI, Schirmer I test were correlated with SF-36 composite scores or scores on the SF-36 subscales.ConclusionsAIBD patients experience reduced quality of life and more severe ocular surface disorders including dry eye, symblepharon and corneal opacity. Early treatments of dry eye and collaborations among multidisciplinary physicians are necessary in patients with AIBD.

Dry eye examination - benefits of Ocular Surface Disease Index (OSDI) questionnaire with clinical testing

Introduction/Objective. Dry eye is a multifactorial disease with incidence up to 50% in the general population. It is characterized by a loss of homeostasis of the tear film and accompanied by ocular symptoms. Ocular Surface Disease Index (OSDI) questionnaire is designed to provide a rapid assessment of the symptoms. The aim of this study was to evaluate the diagnostic capacity of OSDI. Methods. A prospective, randomized and observational study was conducted at the Clinic for Eye Disease, University Clinical Center of Serbia, between December 2018 and February 2019. The OSDI questionnaire was used to rate the severity of dry eye disease. Schirmer I test, tear break-up time test (TBUT), Rose Bengal test and lid-parallel conjunctival folds (LIPCOF) test were performed as a clinical proof of the symptoms. Results. A total of 27 patients, 15 male (55.4%) and 12 female (44.6%), with mean age of 60 ? 15 years were included in the study. The average value of OSDI score was 26.37 ? 23.98 (0?80). Schirmer I test and Rose Bengal test for the right and the left eye, as well as the TBUT test for the left eye were positively correlated with OSDI score (Spearman correlation coefficient). Conclusion. OSDI questionnaire is a fast, reliable, and inexpensive test. In our study we have found a correlation between the OSDI score and other clinical tests, except with LIPCOF test. At this moment, the questionnaire that could be the gold standard for dry eye disease diagnosis does not exist, therefore further studies concerning this topic are needed.

Dry eye and dry skin - is there a connection?

ABSTRACT Aim To enquire whether patients with dry eye symptoms also report dry skin, whether their perception could be corroborated with objective measurement, and whether dry eye disease might be suspected based on patients’ complaints. Methods This cross-sectional study included 50 subjects, 25 with and 25 without dry eye symptoms. Schein questionnaire was used to determine the severity of dry eye symptoms. Ocular signs were assessed by monitoring conjunctival hyperemia, ocular surface staining, meibomian gland expression, tear film lipid layer thickness, tear break-up time, lid parallel conjunctival folds, Schirmer test, and meibometry. Skin dryness was assessed by noting patients’ self-perception of their facial skin dryness and measured by sebumeter. Results Subjects without dry eye symptoms had self-reported oilier facial skin than those with dry eye symptoms (p < .001). Sebumetry scores measured on the forehead and cheek were significantly higher in subjects without dry eye symptoms than dry eye subjects (p = .003). After adjustment for age and gender in a logistic regression analysis, dry eye was independently and significantly associated with dry skin (AOR 0.69, p = .040), higher LIPCOF score of both eyes (AOR 2.28, p = .028), lower sebumetry score of the forehead (AOR 0.98, p = .041) and cheek (AOR 0.98, p = .041), and shorter TBUT score after gland expression (AOR 0.90, p = .018). Conclusion This study showed that ocular dryness was subjectively and objectively positively correlated to facial skin dryness. Patients reliably described their skin condition. People with dry facial skin also had drier eyes.

BCLA CLEAR - Effect of contact lens materials and designs on the anatomy and physiology of the eye

This paper outlines changes to the ocular surface caused by contact lenses and their degree of clinical significance. Substantial research and development to improve oxygen permeability of rigid and soft contact lenses has meant that in many countries the issues caused by hypoxia to the ocular surface have largely been negated. The ability of contact lenses to change the axial growth characteristics of the globe is being utilised to help reduce the myopia pandemic and several studies and meta-analyses have shown that wearing orthokeratology lenses or soft multifocal contact lenses can reduce axial length growth (and hence myopia).However, effects on blinking, ptosis, the function of Meibomian glands, fluorescein and lissamine green staining of the conjunctiva and cornea, production of lid-parallel conjunctival folds and lid wiper epitheliopathy have received less research attention. Contact lens wear produces a subclinical inflammatory response manifested by increases in the number of dendritiform cells in the conjunctiva, cornea and limbus. Papillary conjunctivitis is also a complication of all types of contact lenses. Changes to wear schedule (daily disposable from overnight wear) or lens materials (hydrogel from SiHy) can reduce papillary conjunctivitis, but the effect of such changes on dendritic cell migration needs further study. These changes may be associated with decreased comfort but confirmatory studies are needed. Contact lenses can affect the sensitivity of the ocular surface to mechanical stimulation, but whether these changes affect comfort requires further investigation.In conclusion, there have been changes to lens materials, design and wear schedules over the past 20+ years that have improved their safety and seen the development of lenses that can reduce the myopia development. However, several changes to the ocular surface still occur and warrant further research effort in order to optimise the lens wearing experience.

Manual interferometric device for routine non-invasive tear film break-up time assessment

ABSTRACT Purpose: To determine the feasibility of non-invasive tear break-up time (NIBUT) assessment using the Handheld tear lipid layer thickness assessment instrument and compare it with the standard tear break-up time (TBUT) test and other dry eye tests. Subjects and methods: 108 subjects were enrolled, 56 with and 52 without dry eye symptoms. Schein questionnaire was used to determine the severity of dry eye symptoms. Ocular signs were assessed by NIBUT, TBUT, lipid layer thickness (LLT), lid-parallel conjunctival folds (LIPCOF), conjunctival hyperemia, and corneal staining. Results: Median NIBUT and TBUT, and other clinical test values significantly differed among the dry eye symptoms group and control group. NIBUT yielded the most significant difference between the groups (NIBUT: 7 sec vs. 17.5 sec, p < .001, Z = 5.94; TBUT: 5 sec vs. 10 sec, p < .001, Z = 4.38; LLT: p = .007; LIPCOF: p < .001, conjunctival hyperemia: p < .047, corneal staining: p < .010). Spearman’s test showed a significant correlation between NIBUT and TBUT (p < .001), NIBUT and LLT (p = .001), NIBUT and LIPCOF (p = .019), NIBUT and conjunctival hyperemia (p = .002), and NIBUT and corneal staining (p = .012) in the dry eye symptoms group. NIBUT did not significantly differ among the three measurements in both groups of patients (p = .061, p = .096), while TBUT values did in the control group (p < .001). Short NIBUT values were the main predictors and indicators of dry eye complaints (AOR = 0.87, p < .001), superior to TBUT (AOR = 0.88, p = .008) and other tests even after adjustment for age and gender. Conclusion: NIBUT measured by Handheld instrument is a simple, accessible, practical, and, most of all, reproducible and objective method that might allow NIBUT assessment on a regular basis.

Diagnostic value of three simple and rapid dry eye tests: Lid parallel conjunctival folds, tear meniscus height and tear ferning

Introduction/Objective. The objective of this paper was to assess the diagnostic value of three simple dry eye (DE) tests: lid parallel conjunctival folds (LIPCOF), tear meniscus height (TMH), and tear ferning (TF). Methods. LIPCOF, TMH, and TF diagnostic DE tests were performed in 100 patients. Eighty of them were referred to us by rheumatologists and general practitioners either during evaluation for Sjogren?s syndrome, or because of DE symptoms. The control group was composed of 20 patients, with no DE-related symptoms. Ocular Surface Disease Index questionnaire was used for DE symptoms? evaluation. Results of LIPCOF, TMH, and TF tests were compared with results of the Copenhagen criteria DE tests i.e., tear fluorescein breakup time, Schirmer I and Rose Bengal tests. Ability of the tests to recognize DE in various grades according to Dry Eye Work Shop (DEWS) report score system was assessed. Results. Compared to the Copenhagen criteria, sensitivity of LIPCOF and TMH was high (92.8% and 83.5%, respectively), while specificity was low (34.4% and 49.2%, respectively). TF had low sensitivity (59.1%) but high specificity (82.7%). Mean values of both LIPCOF and TMH differed significantly (F = 7.222, p &lt; 0.001 and F = 11.802, p &lt; 0.001, respectively) between the control group and all DEWS grades, but not among different grades of DE. Conclusion. TMH and LIPCOF diagnostic tests showed high sensitivity, which makes them excellent screening DE tests. Low sensitivity of TF suggests that it is not truly a good screening test on its own, but its high specificity is of definite value.

Dust exposure: a novel environmental risk factor for conjunctivochalasis?

Objective:This study aimed to determine the relationship between clinical symptoms and findings of patients diagnosed with conjunctivochalasis in our clinic and the risk factors reported in the literature, to determine new possible etiological factors, and to compare routine tear assessment methods with novel strip meniscometry in these patients.Materials and Methods:The study included patients who presented to our clinic between March 2019 and June 2019 and were diagnosed with conjunctivochalasis. The Ocular Surface Disease Index was used to assess ocular surface symptoms. Anterior segment examination included tear break-up time, corneal staining, localization of conjunctivochalasis, and the presence of pinguecula or pterygium. For each patient, tears were measured using both Schirmer and strip meniscometry. Patients with conjunctivochalasis were evaluated as lid-parallel conjunctival folds grade 1, 2, or 3.Results:There was a total of 57 participants, including 31 women (54.4%) and 26 men (45.6%). Lid-parallel conjunctival folds grade was found to be statistically significantly associated with dust exposure (p = 0.034). Multiple regression analysis revealed that dust exposure was an independent predictor of being diagnosed with lid-parallel conjunctival folds grade 2 versus lid-parallel conjunctival folds grade 1 (odds ratio = 3.515, p = 0.029). Moreover, Schirmer’s test and strip meniscometry results were positively correlated in patients with conjunctivochalasis (r = 0.302 and p = 0.022).Conclusion:We conclude that external factors are prominent in patients with conjunctivochalasis, especially dust exposure. In addition, strip meniscometry may be preferred as a more practical and noninvasive alternative to investigate dry eye symptoms in patients with conjunctivochalasis.

Gender-Specific Differences in Signs and Symptoms of Dry Eye Disease

ABSTRACT Introduction Dry eye disease (DED) is a multifactorial disease of the ocular surface characterized by loss of homeostasis of the tear film. Epidemiological studies suggest it occurs more frequently in women than men. The aim of this retrospective analysis was to determine whether patients with DED show gender-specific differences in symptoms and signs. Material and Methods A retrospective analysis of 107 patients was conducted at the Department of Ophthalmology of the University of Düsseldorf – Germany. After completing the OSDI questionnaire, the patients underwent objective refraction and were asked about the following symptoms: burning, itching, foreign body sensation, epiphora, sticky eyes, pain, red eyes and swollen eyelids. Furthermore, they underwent an evaluation of the following signs: lipid layer thickness, non-invasive break-up-time (NI-BUT), conjunctival hyperemia, lid parallel conjunctival folds, meibography, Schirmer test and tear meniscus height, ocular surface staining, expressibility of meibomian glands. Results Of the 107 patients (56.2 ± 17.3 years) 75 were women and 32 men. Women reported significantly more often eyelid swelling (p = .03) and showed a tendency to complain more about red eyes (p = .051), while men tended to complain more often about epiphora (p = .053). In an age-matched sample taken from the cohort, significant differences were found for OSDI-scores (p = .025) showing women reporting more symptoms compared to men, while men showed more conjunctival hyperaemia (p = .004) than women. Women showed a significant thicker lipid layer (p = .0009) in the full cohort, but not in the age-matched sample (p = .43). Conclusion Although the majority of the investigated parameters did not show gender-specific differences, women reported higher OSDI scores. These findings could be explained by an increased frequency of neuropathic symptoms or ocular surface sensitivity in women or higher resilience of men to consult an ophthalmologist. Sex may have an effect on the clinical characteristics of DED, but further studies are needed to confirm these findings.

Assessment of Langerhans Cells in the Central Cornea as Tool for Monitoring Inflammatory Changes in Patients with Keratoconjunctivitis sicca under Topical Therapy with Cyclosporine A 0.05% Eye Drops

Keratoconjunctivitis sicca is one of the most common ocular diseases and is associated with a considerable decrease in the quality of life. It is accompanied by symptoms of discomfort, tear film instability, hyperosmolarity of the tear film, inflammation of the ocular surface and neurosensory abnormalities. It is still not clear yet if the density of Langerhans cells in the central cornea can be used for objective evaluation of the inflammation in the tear film and the ocular surface. 47 patients (age 21 - 85 years, 13male and 34 female patients) with moderate to severe keratoconjunctivitis sicca (according to the DEWS criteria severity level 2 - 3) were enrolled in this prospective, clinical trial. Patients were treated for 3 months with topical anti-inflammatory therapy with cyclosporine 0,05% eye drops twice daily. The following parameters were examined: OSDI Score, tear break-up time (TBUT), Schirmer test with anaesthesia, lid-parallel conjunctival folds (LIPCOF), in-vivo confocal microscopy with HRT II and RCM for evaluation of Langerhans cell density in the central cornea, visual acuity, tear film osmolarity and Meibomian gland dysfunction (MGD). After anti-inflammatory treatment, there was a significant reduction in the density of Langerhans cells (p ≤ 0.001**), an increase in TBUT (p ≤ 0.001**), as well as a significant decrease in tear film osmolarity (p ≤ 0.05*) and OSDI Score (p ≤ 0 001**). Visual acuity, MGD and Schirmer test did not change significantly. The results of this clinical trial show that topical anti-inflammatory therapy with cyclosporine A 0.05% eye drops in patients with moderate to severe keratoconjunctivitis sicca is effective in reducing inflammation of the ocular surface and the tear film, as well as in increasing the quality of life in these patients. Evaluation of Langerhans cell density in the central corneal epithelium by in vivo confocal microscopy is an effective objective diagnostic feature in monitoring anti-inflammatory therapy in patients with dry eye disease or other ocular surface pathologies.

Substance P Level in Tears as a Potential Biomarker for Contact Lens Discomfort

ABSTRACT Purpose: To analyze the effects of contact lens (CL) wear, time of the day, and CL discomfort (CLD) on clinical signs, tear inflammatory mediators and substance P. Methods: Thirty symptomatic and 30 asymptomatic CL wearers attended two visits (morning and afternoon) on two days (non-CL and CL wearing days). Comfort, meniscus area, noninvasive breakup time (NIBUT), tear collection, hyperemia, lid parallel conjunctival folds, fluorescein staining, and sensitivity were performed. The tear levels of 23 inflammatory mediators and substance P were measured. Results: Comfort, meniscus area, NIBUT, and MMP-9 were lower while conjunctival staining and EGF higher (p ≤ 0.015) on the CL wearing day. Comfort, IL-8/CXCL8, and VEGF were lower while EGF, IP-10/CXCL10, and MCP-1/CCL2 higher (p ≤ 0.047) in the afternoon. Comfort was lower and substance P higher (p ≤ 0.006) in symptomatic wearers. Conclusion: Substance P may be implicated in CLD etiology; its role and potential application as a biomarker should be further studied.

The Effect of Microblepharon Exfoliation on Clinical Correlates of Contact Lens Discomfort.

Microblepharon exfoliation improved eyelid signs and tear film characteristics after a single in-office treatment in symptomatic contact lens wearers. The purpose of this study was to assess the effect of two eyelid hygiene treatments-microblepharon exfoliation and a hypoallergenic foam cleanser (LidHygenix)-on clinical signs of the eyelids, meibomian glands, and tear film in contact lens discomfort. A randomized, interventional, unmasked, crossover trial was conducted on 30 experienced daily-wear soft contact lens wearers. Assessment of clinical signs of the eyelid margin, meibomian gland morphology and secretion, and tear film biophysical properties was performed (baseline 1), and participants were randomly assigned to receive one of the two treatments (microblepharon exfoliation or foam cleansing using LidHygenix) as a single in-office procedure. Symptoms were evaluated using the Contact Lens Dry Eye Questionnaire-8 immediately after treatment, and assessment of all the study variables was repeated at the follow-up visit 7 to 10 days after treatment. After 28 to 30 days of washout, participants returned for reassessment of the study variables (baseline 2) and were crossed over to receive the alternate treatment. Follow-up was repeated 7 to 10 days after the second treatment. Seven to 10 days after treatment with microblepharon exfoliation, symptomatic wearers showed significant improvement in anterior blepharitis (mean difference, 0.60; P = .04), lid wiper staining (0.50; P = .06), and lid-parallel conjunctival folds (0.68, P = .02) along with orifice capping (median difference, 0.65; P < .001), foam (0.90; P < .001), secretion volume (0.69; P < .001), quality (0.74; P < .001), and expressibility (0.49; P = .002), which were also clinically significant changes. However, in tear properties, significant improvements were observed in tear volume (LidHygenix, -1.25 mm; microblepharon exfoliation, -1.62 mm), break-up time (-0.14 seconds; -0.14 seconds), tear evaporation rate without contact lenses (21.52 g m h; 45.43 g m h), and lipid layer thickness (-20.61 nm; -25.13 nm) after both treatments but in symptomatic lens wearers only (P < .05). Microblepharon exfoliation improved eyelid signs and tear film characteristics in symptomatic contact lens wearers after a single in-office treatment.

Comparative study of tear lipid composition in two human populations with different exposure to particulate matter in La Plata, Argentina.

To identify the changes in the lipid profile of the tear film in two human populations exposed to different levels of particulate material, and its relationship with dry eye, by gas chromatography with mass spectrometry (GC-MS) detection. A panel study involving 78 volunteers, who live and work in two locations in Argentina with different pollution levels: urban zone (n = 44) and industrial zone (n = 34). We measured the mean levels of particulate matter (PM) exposure. The tear samples were analyze by gas GC-MS detection and the dry eye was diagnose using Schirmer test, fluorescein breakup time, vital staining with fluorescein and lissamine green, and lid parallel conjunctival folds (LIPCOF). Statistical analysis was performed using Chi-Square, Bartlett's, Mann-Whitney tests, and Multiple Correspondence Analysis. PM10 level was significantly higher in industrial zone than in urban area (p < 0.05). Subjects exposed to higher levels of PM10 in outdoor air presented more presence of fatty acids (FA) of long chain, a higher proportion of saturated fatty acids (SFA), and lower unsaturated fatty acids (UFA), showing a differentiated profile, which may be associated with a PM level. The incidence of dry eye was greater in the industrial zone (p < 0.001), showing in both populations for this pathology higher FA ω-6 levels, which are responsible for the inflammation process. The lipid profile in populations exposed to higher levels of PM10, like the industrial zone, shows a differentiated profile of FA and more incidence of dry eye with higher FA ω-6 levels, which are responsible for the inflammation process.

Correlation between conjunctival scraping cytology and other clinical dry eye metrics in determination of dry eye related inflammation

Background/Aim. New and improved definition of dry eye disease (DED) emphasized that hiperosmolarity and inflammation with initial tear film instability play etiological role. The aim of this study was to explore relation of some commonly used clinical tests to dry eye disease (DED) related inflammation measured by conjunctival scraping cytology. Methods. We examined 100 subjects, 80 of them having DED. We performed Schirmer without anesthesia (Schirmer I), Fluorescein Tear Break Up Time (FTBUT), Rose Bengal (RB), Lid Parallel Conjunctival Folds (LIPCOF), Tear Meniscus Height (TMH) and Tear Ferning (TF) and compared the values to scraping scores of tarsal conjunctiva. Results. FTBUT had the best sensitivity (93.6%). The highest specificity was found with RB (93.2%), but it was also high with Schirmer I, TF and FTBUT (respectively 89.8%, 84.5%, 78.0%). RB and FTBUT had the highest correlation with conjunctival scraping score (r = 0.707, p &lt; 0.001; r = -0.507, p &lt; 0.001). Conclusion. In our study, FTBUT, though often used in many combinations of the DED tests, showed a remarkably high sensitivity and specificity on its own, as well as good correlation with DED related inflammation detected with conjunctival scraping cytology.

Lid-parallel conjunctival fold (LIPCOF) morphology imaged by optical coherence tomography and its relationship to LIPCOF grade.

Lid-parallel conjunctival folds (LIPCOF) are a well-accepted clinical sign in dry eye diagnosis. Commonly, LIPCOF is classified by grading the number of folds observed by slit-lamp microscope. This study investigated the relationship between subjective grading scale and LIPCOF morphology imaged by optical coherence tomography (OCT). Temporal and nasal LIPCOF of 42 subjects (mean age 27.3 ± 8.4 (SD) years; 13 M, 29 F) were observed and classified by an experienced optometrist using the Pult LIPCOF grading scale (0: no permanent, lid-parallel conjunctival fold; 1: one fold; 2: two folds, 3: three or more folds). Additionally, LIPCOF cross-sectional area (LIPCOF-A) and fold count (LIPCOF-C), as imaged by OCT (Cirrus HD; Carl Zeiss Meditec, Jena, Germany), were analysed with ImageJ 1.50 (http://rsbweb.nih.gov/ij). Correlations between subjective grading and LIPCOF-A and LIPCOF-C were analysed by Spearman correlation, differences between subjective grading and LIPCOF-C were analysed by Wilcoxon test. For temporal and nasal sectors, mean subjective LIPCOF grade was 1.43 ± 0.86 grade units and 0.57 ± 0.80 grade units, mean LIPCOF-C was 1.67 ± 0.82 folds and 0.69 ± 0.78 folds, and mean LIPCOF-A was 0.0676 ± 0.0236mm2 and 0.0389 ± 0.0352 mm2, respectively. Subjective temporal and nasal LIPCOF grade was significantly correlated to LIPCOF-C (r = 0.610, p < 0.001 and r = 0.645, p < 0.001, respectively), and to LIPCOF-A (r = 0.612, p < 0.001 and r = 0.583, p < 0.001, respectively). LIPCOF-C was not statistical different to subjective LIPCOF grade (p = 0.07 and p = 0.239; temporal and nasal sectors, respectively). OCT allows for better imaging of finer details of LIPCOF morphology, and especially of LIPCOF area. OCT evaluation of LIPCOF area correlated well with subjective grading and appears to be a promising objective method for LIPCOF classification.

Lid-Parallel Conjunctival Folds and Their Ability to Predict Dry Eye.

The observation of lid-parallel conjunctival folds (LIPCOF) is reported to be useful in dry eye diagnoses. Around 70% of the European clinicians use this test in a dry eye clinic. This study investigated the sensitivity and specificity of LIPCOF to predict dry eye. Ocular Surface Disease Index (OSDI) outcome, noninvasive break-up time (NIKBUT) using the Keratograph (Oculus, Wetzlar, Germany), and LIPCOF of 148 randomized selected patient reports (88 females; mean age=37.0±12.9 years) from 3 different sites were analyzed. Subjects were divided into dry eye and non-dry eye groups by OSDI only, named symptomatic dry eye (OSDI scores ≥15) and by a composite score (OSDI scores ≥15 and NIKBUTaverage of ≤9 sec). Lid-parallel conjunctival folds (temporal, nasal, Sum [=temporal+nasal]) were significantly correlated to OSDI and to NIKBUTaverage (Spearman; r>-0.185, P<0.013). NIKBUTaverage (r=-0.322; P<0.001) was significantly correlated to OSDI scores. Based on the OSDI questionnaire, 37.2% of the subjects were symptomatic and 14.7% were positive for dry eye based on the composite score. Lid-parallel conjunctival folds were a significant discriminator between healthy and dry eye subjects (P≤0.001). The areas under the receiver operating characteristic curve for temporal LIPCOF, nasal LIPCOF, and LIPCOF Sum were 0.716, 0.737, and 0.771, respectively, for the symptomatic dry eye group and 0.771, 0.719, and 0.798, respectively, for the composite dry eye group. As LIPCOF demonstrated high predictive ability of dry eye; it appears to be a promising test in the diagnoses of dry eye.
