Impact of Covid-19 on patient safety culture in hospital wards: A comparative study

BackgroundThe current coronavirus pandemic has profoundly impacted healthcare services across all dimensions. Patient safety culture stands as a pivotal element in enhancing safety and the quality of patient care. This study aimed to compare the patient safety culture in the COVID-19 ward with that in other inpatient wards. MethodsIn this cross-sectional study, 418 nurses were selected through a quota sampling method from an educational hospital in Western Iran. For data collection, the Hospital Survey on Patient Safety Culture (HSOPSC), developed by AHRQ, was utilized. Data were analyzed using descriptive statistics, including mean and standard deviation (SD), as well as analytical statistics such as independent-samples t-test, Mann-Whitney test, Spearman's correlation test, and Kruskal-Wallis test. SPSS 25 was used for analysis, and the significance level was set at p ≤ 0.05. ResultsThere was no statistically significant difference between the two groups (p > 0.05), but there were variations in the mean scores within different dimensions. The overall mean score of patient safety culture in the coronavirus ward was 117 ± 20.91, indicating a moderate to low level of patient safety culture in this group. In contrast, in other inpatient wards, the mean score was 122.66 ± 23.47. ConclusionIn the coronavirus department, due to the heightened patient severity and increased nursing workload, the patient safety culture was rated as weak. Therefore, it demands specific attention from management to provide additional training for healthcare staff, aiming to enhance their performance in coping with stressful and unpredictable conditions.

Nurses’ Perceptions of Patient Safety Culture and Evaluation of Organizational Commitment Level

This research was carried out to determine nurses' perceptions of patient safety culture and organizational commitment levels, as well as to show the relationship between these two variables.
 In this descriptive and correlational study, data were collected between May and June 2018 with the participation of 299 nurses working in a public hospital. Turkish versions of surveys titled Hospital Survey on Patient Safety Culture and Organizational Commitment Scale were used in order to collect data. The data collected was summed using frequencies, percentages, means, and standard deviation. Pearson correlation in comparing categorical data between groups were benefitted in comparative analyses. 
 Nurses’ perception of patient safety culture was at a level that needed improvement (Total average percentage of positive responses 54%). The standards of organizational commitments of the participants were found to be high (4.09±0.85). The association between nurses’ perceptions of patient safety culture and their organizational commitment degrees (r=0.41) were found statistically significant (p≤0.01). As the level of patient safety culture of nurses increases their level of commitment to their institutions also increases.
 The patient safety culture level of nurses is below the desired level and needs to be improved.. Enhancing the safety culture in hospitals will improve their organizational commitment.

Perceived patient safety culture and its associated factors among clinical managers of tertiary hospitals: a cross-sectional survey

BackgroundPatient safety is a global challenge influenced by perceived patient safety culture. However, limited knowledge exists regarding the patient safety culture perceived by hospital clinical managers and its associated factors. This study aims to investigate the perceptions of patient safety culture and associated factors among clinical managers of tertiary hospitals in China.MethodsA cross-sectional survey was conducted from June 19 to July 16, 2021, involving 539 clinical managers from four tertiary hospitals in Changsha City of Hunan Province. The Hospital Survey on Patient Safety Culture (HSOPSC) was utilized to assess perceived patient safety culture. Bivariate, multivariable linear regression, and logistic regression analyses were performed.ResultsThe mean score for the total HSOPSC was 72.5 ± 7.6, with dimensional scores ranging from 62.1 (14.9) to 86.6 (11.7). Three dimensions exhibited positive response rates (PRRs) < 50%, indicating areas that need to be improved: “nonpunitive response to errors” (40.5%), “staffing” (41.9%), and “frequency of events reported” (47.4%). Specialized hospitals (β = 1.744, P = 0.037), female gender (β = 2.496, P = 0.003), higher professional title (β = 1.413, P = 0.049), a higher education level (β = 1.316, P = 0.001), and shorter time delays per shift (β=-1.13, P < 0.001) were correlated with higher perceived patient safety culture. Education level, work department, “teamwork within a unit”, “management support for patient safety”, “communication openness”, and “staffing” dimensions were associated with patient safety grades (all P < 0.05). Years worked in hospitals, occupation, education level, work department, hospital nature, professional title, “communication openness”, and “handoffs & transitions” were associated with the number of adverse events reported (all P < 0.05).ConclusionsOur study revealed a generally low level of patient safety culture perceived by clinical managers and identified priority areas requiring urgent improvement. The associated factors of patient safety culture provide important guidance for the development of targeted interventions in the future. Promoting patient safety by optimizing the patient safety culture perceived by clinical managers should be prioritized.

Patient safety culture predictors and outcomes for sustainable oncology nursing practice: A cross-sectional correlational study.

To examine the predictors and outcomes of patient safety culture (PSC) among oncology nurses working in public Saudi hospitals according to participant characteristics and evaluate the relationship between PSC domains. PSC is crucial in healthcare systems, particularly in oncology and chemotherapy units, and its assessment can enhance the standard service provided and cancer care quality. There is currently limited research on the status, predictors and outcomes of PSC in cancer care settings in developing countries, including Saudi Arabia. A cross-sectional correlational study. A convenience sample of 101 oncology nurses working in two large Saudi tertiary care hospitals participated in this study. The Hospital Survey on Patient Safety Culture validated instrument and demographic and work surveys were completed by the participants. The study methods were compliant with the STROBE checklist. Descriptive statistics and multiple linear regressions were used to analyse the data. The areas of PSC strength were related to organizational learning-constant improvement, feedback and communication about errors, and transitions and handoffs. Manager/supervisor actions and expectations, hospital management support, communication openness, experience in the current unit and oncology unit/area were the predictors of PSC. In terms of PSC outcomes, the oncology nurses reported either no or one to two adverse events and a substantially good patient safety rating. The level of PSC was lower than expected. Communication openness, experience in the current unit and oncology unit/area were the strongest predictors of PSC. Investing in oncology nursing practice that addresses these concerns and prioritizes patient safety is critical in Saudi cancer care settings to increase patient safety. The findings contribute to a better understanding of the predictors and outcomes of PSC, which should be considered when establishing effective nursing interventions or strategies for PSC in cancer care settings. Investigating the relationship between patient safety culture and self-efficacy of operating room: A cross-sectional study on 228 perioperative nurses in Iran

BackgroundHospitals should implement structural changes as well as a culture of patient safety among their staff in order to increase the quality and safety of healthcare. One of the traits that can have an impact on mental health and the provision of high-quality services and care for these people is self-efficacy. The goal of the current study was to link operating room nurses' self-efficacy to the culture of patient safety. MethodsThis cross-sectional study was done in 2022 on a group of 228 operating room nurses who were employed by hospitals in the province of East Azerbaijan. In this research, 228 operating room nurses were included in the study by random sampling method. Data collection tools included demographic questionnaire, patient safety culture questionnaire and general self-efficacy questionnaire Sherer. Software SPSS version 27 was used to examine the data that had been gathered. P value less than 0.05 is regarded as significant. ResultsThe results of data analysis showed that the average score of patient safety culture in operating room nurses is 131.2 ± 14.5, which is in the middle level. Also, the average self-efficacy score is 51.7 ± 5.5, which is in the middle level. The self-efficacy of operating room nurses and patient safety culture are directly and significantly correlated (P<0.001). Therefore, increasing the self-efficacy of operating room nurses leads to the promotion of patient safety culture. ConclusionBased on the results of the study, the more self-efficacy operating room nurses have, the higher the level of patient safety culture will be. Considering the critical responsibility of operating room nurses in providing safe and quality services to patients, people with higher self-efficacy provide better safe services and patients in hospitals with a more positive safety culture experience fewer injuries.

Patient safety culture and associated factors among health care providers in government and private hospitals, Bahir Dar City Northwest, Ethiopia, 2022: a comparative cross-sectional study

BackgroundPatient safety in a healthcare setting is now a major global concern. Millions of people suffer disabling injuries or death directly related to medical care errors, particularly in developing countries. Evidence about patient safety culture in Ethiopia is limited. Therefore, this study was designed to assess the level of patient safety culture and associated factors among healthcare providers in government and private healthcare providers.Methods and materialsInstitution based cross-sectional study was conducted from May to June 30, 2022. Self-administered hospital survey on Patient Safety Culture (HSOPSC) tool was used to select 448 study participants. Epi Data version 4.6 and SPSS version 26 were used for data entry and analysis. Chi-square test, Bi-variable, and multivariable logistic regressions were done to determine the association between the independent and outcome variable.ResultA total of 448 healthcare providers with a response rate of 99.6% participated. The prevalence of good patient safety culture was 50.9%( 95%CI: 46.2, 55.6%). Patient safety culture difference was observed between government and private healthcare providers (× 2 = 22.6, df = 1, p = 0.000). Type of hospitals (AOR = 0.37(95% CI:(0.21, 0.68), profession (AOR = 2.16 (95% CI:(1.02,4.62), job satisfaction (AOR = 0.19,95%CI:(0.12,0.30), participated in patient safety programs(AOR = 2.69:(95%CI:1.53,4.75), providing necessary equipment and materials (AOR = 2.05(95%CI: 1.18,3.55%), and work shift (AOR = 0.47( 95%CI: 0.25,0.93) were found significantly associated with good patient safety culture among healthcare providers.ConclusionThe prevalence of good patient safety culture was relatively low. Patient safety culture difference is observed between government and private healthcare providers. Type of hospitals (public or private), profession, job satisfaction, participation in patient safety programs, providing necessary equipment and materials, and work shifts were associated factors for patient safety culture. Therefore, it is better to design patient safety improvement strategies for both government and private healthcare providers.

Patient safety culture among healthcare professionals in Bhutan

Introduction: There is dearth of information regarding the status of patient safety culture in Bhutanese healthcare system. The aim of the study was to assess the baseline patient safety culture among the healthcare professionals working in different levels of hospitals in Bhutan. Methods: A cross-sectional study was conducted using the Hospital Survey on Patient Safety, version 2 (HSOPS2) questionnaire in 2021 in six District Hospitals (DHs), two Regional Referral Hospitals (RRHs), and one National Referral Hospital (NRH) in Bhutan. A total of 782 healthcare professionals were recruited into the study. EpiData version 3.0 and Statistical Package for Social Science (SPSS) version 28.0 were used to process data and conduct the statistical analysis on survey data including descriptive statistics and validity and reliability of survey. The internal consistency reliability of each composite measure was assessed by calculating Cronbach’s alpha score. Results: The overall level of patient safety culture was 61.28% (95% CI: 113.26, 115.27) and the percentage scores for the ten patient safety dimensions ranged from 45.13% to 82.65%. The dimension, “Teamwork within the hospital units” had the highest positive score while the dimensions, “Staffing and work pace” and “Reporting patient safety events” had the lowest positive scores. Conclusions: The healthcare professionals working in hospitals in Bhutan had average patient safety culture. To improve patient safety culture, more emphasis should be given on human resource management, patient safety event reporting system, hospital management support for patient safety and quality improvement initiatives and creating an environment that promote just and learning culture.

Level of patient safety culture among public healthcare professionals in Pretoria.

Patient safety culture (PSC) norms set within an organisation prevent harm during medical care. This study assessed the level of PSC among public healthcare professionals (HCPs) in Pretoria, South Africa. A multi-centre cross-sectional study conducted in three hospitals and 25 clinics in regions 1 and 2 of Pretoria, using a self-administered questionnaire adapted from the Hospital Survey on Patient Safety Culture. Using the Raosoft online sample size formula, from 1238 public HCPs identified, the sample size was calculated at 294; this expanded to 319 as a result of respondents' willingness to participate in the study. Of the 319 respondents with a mean age of 39.9 years, the minimum and maximum ages were 22 and 66 years, respectively. The age group of 30-39 years had the highest participation rate (17.6%). Most respondents (41.1%) came from the Odi district hospital and there were more women (78.1%) and nurses (49.2%). Positive attitudes were found for all PSC components, with staff education and training scoring highest (98.7%). Patient safety culture received a satisfactory rating from HCPs from the targeted facilities. This study showed that public HCPs in Pretoria's regions 1 and 2 have a good PSC, particularly among nurses, professionals with more experience, and at primary care level.Contribution:To maintain or increase awareness of this concept among HCPs, the study advocates a PSC programme as well as ongoing education that can be supported by district and facility managers.

Evaluation of patient safety culture as perceived by nurses during the COVID-19 pandemic

Abstract Objectives: A good patient safety culture (PSC) is linked to a reduced risk of patient problems and minimal undesirable occurrences. This study investigated the PSC levels from nurses’ perspectives during the COVID-19 pandemic. Methods: A descriptive cross-sectional design was applied. The Hospital Survey on Patient Safety Culture (HSOPSC) questionnaire was administered to 315 nurses working at 2 major hospitals in Jeddah, Saudi Arabia. The data were analyzed using descriptive statistics, a t-test, and a one-way ANOVA test. The statistical significance of the correlation was determined at the 0.05 level. Results: PSC was rated as medium overall according to the nurses, with a weighted mean of 2.88–0.76 and a relative weight of 57.57%. In addition, all PSC composites were rated from medium to high, except organizational learning, which was rated low. The correlation between sociodemographic variables as well as PSC levels was investigated using the t-test and one-way ANOVA test. The association is statistically significant when P ≤ 0.05. The findings revealed a statistically significant correlation between nurse nationality (t = −4.399, P = 0.000), age (F = 7.917, P = 0.000), experience in years (F = 3.760, P = 0.024), and hospital (t = −0.401, P = 0.689). Conclusions: The nurses in this study had a medium overall PSC level, and all PSC composites ranged from a medium to a high level, except organizational learning, which had a low level. In addition, the findings showed that there is a significant relationship between PSC levels, nurses’ nationalities, experience in years, and the hospital itself.

Exploring the well-being of community pharmacy professionals, turnover intention and patient safety: Time to include operational responsibility.

The COVID-19 pandemic added significant occupational pressures on community pharmacists. The objective of this research project was to investigate the level of distress and burnout among community pharmacy professionals and its association with their retention within their occupation as well as patient safety outcomes. We conducted a cross-sectional study on 722 community pharmacy professionals from all Canadian provinces using an online survey, including scientifically validated measures. The data were analyzed using multiple regression analysis. In Canada, 85% of community pharmacy professionals reported their mental health had suffered since the COVID-19 pandemic. Younger pharmacy professionals and those paid hourly reported a worsening level of mental health and an increasing level of turnover intention. Pharmacists with more dynamic/disrupted work schedules and those working for a large pharmacy chain (more than 25 pharmacies in Canada) reported lower levels of mental health quality. Pharmacy professionals working in pharmacies that are open more than 70 hours a week reported a lower level of patient safety culture. Pharmacists' mental health was the significant predictor of their turnover intention, implying a heightened risk to professional effectiveness and retention. Compassion satisfaction was positively associated with patient safety culture and safety behaviour, while compassion fatigue and secondary traumatic stress were significantly associated with pharmacists' level of risk-taking behaviours. This study emphasized the importance of prioritizing the mental health and well-being of community pharmacy professionals and demonstrated individual and systemic factors predicting the well-being and turnover intention of community pharmacists, as well as patient safety culture within their pharmacy. This research makes a case to consider actions to shift the monitoring focus from community pharmacists (also known as "individual responsibility") to community pharmacies (also known as "operational responsibility") for managing patient safety. Additionally, community pharmacists should be provided with the professional autonomy to affect their working conditions and alleviate the stress that has the potential to negatively affect the delivery of care.

Patient safety culture in the operating room: a cross-sectional study using the Hospital Survey on Patient Safety Culture (HSOPSC) Instrument

BackgroundCredible evidence has established a link between the level of patient safety culture in healthcare environments and patient outcomes. Patient safety culture in the operating room has received scant attention despite the burden of adverse events among surgical patients. We aimed to evaluate the safety culture in our operating rooms and compare with existing data from other operating room settings.MethodsWe investigated the patient safety culture in the operating rooms of our hospital as perceived by the surgeons, nurse anaesthetists and perioperative nurses using the Hospital Survey on Patient Safety Culture (HSOPSC) instrument. IBM Statistical Package for Social Science software, version 25, was used for data entry and analysis. Differences were considered significant when p < 0.05.ResultsOnly 122 completed surveys were returned out of a survey population of 132 frontline staff, yielding a response rate of 92.4%. The overall average composite score was 47%. The average composite scores ranged from 17–79.6% across the 12 dimensions of the HSOPSC, with teamwork within units being the only dimension with demonstrable strength. Non-punitive response to error, communication openness, feedback and communication about error”, frequency of events reported”, handoffs and transition and staffing need improvement. The perceived safety culture varied according to work areas and professional roles with nurse anaesthetists having the highest perception and the surgeons the least.ConclusionPatient safety culture in our operating rooms is adjudged to be weak, with only one of the twelve dimensions of HSOPSC demonstrating strength. This is notwithstanding its comparative strengths relative to other operating room settings.

The impact of hospital accreditation on patient safety culture in the health services implementation at Sanglah General Hospital, Bali

Background and purpose: Hospital accreditation is carried out to evaluate hospitals’ performance in providing patients with excellent and safe health services. One aspect of service quality is patient safety culture. This study aims to determine the impact of hospital accreditation on patient safety culture in health service implementation at the Sanglah General Hospital in Denpasar City, Bali Province. Methods:This study used a qualitative research design. In-depth interviews were conducted to 11 informants consisting of hospital management, clinicians and nurses which were chosen purposively since they were directly related to the assessment and supervision of health service providers at Sanglah Hospital. The interview guide was based on the Modified Manchester Patient Safety Framework (MaPSaF). Data were analyzed with a thematic approach. Results: The interview results show that the ten dimensions of patient safety culture had been implemented well but still need to be improved in the dimensions of giving priority to safety, system errors and responsibilities, recording incidents and best practices, learning and influencing change, also education and training of staff and teamwork. The maturity level of patient safety culture at Sanglah Hospital is at generative culture in four dimensions and at proactive culture on the six dimensions of patient safety culture. Conclusion: Accreditation had a positive impact on the maturity level of patient safety culture at a proactive level, mainly changes in knowledge, attitudes, and behavior in health service providers. However, in its implementation, there were still constrained on infrastructure, time, and energy.

Patient safety culture awareness among healthcare providers in a tertiary hospital in Riyadh, Saudi Arabia.

Patient safety is a serious concern in the health care industry. To enhance patient safety, healthcare providers are expected to minimize accidental harm to patients and enhance the quality of patient-centered care. The main objective of this study is to explore the awareness of the patient safety culture among healthcare providers. It is further intended to assess key fields and factors that hinder patient safety adoption and determine the effects of demographic factors on healthcare providers' awareness of patient safety culture. This study applied a cross-sectional quantitative design. It was conducted in a tertiary hospital in Riyadh, Saudi Arabia. The participants consisted of all healthcare providers working in a specific tertiary hospital in Saudi Arabia. A random sampling technique was applied in this study. The study sample size was 409 participants. A valid and reliable questionnaire was used to collect the required data. The T-tests, ANOVA. And regression was used. The study found that there is a moderate level of patient safety culture awareness among healthcare providers. Moreover, the findings also revealed that the age group “31–40” showed statistically different awareness levels as compared to the “more than 50 years' age group” (p = 0.012). Also, this study has found that gender and education have a significant influence on the awareness level of patient safety culture while position and work area have no influence on the awareness level of patient safety culture among healthcare providers. Managers in healthcare institutions should develop speeder response plans and make them part of the patient safety culture. Institutions offering bachelor's degrees and postgraduate in nursing should pay more attention to the subject of patient safety. The government healthcare sector, together with the private healthcare sector, should continuously train healthcare providers on patient safety procedures to improve the patient safety culture. Healthcare providers should be encouraged to report errors made during diagnosis or treatments to avoid them in the future.

Comparative Assessment of the Level of Patient Safety Culture between Surgical and Nonsurgical Units in Bulgarian Hospitals

Patient safety culture is a key component of the organizational culture and a critical measure of the quality of healthcare. The aim of this study was to gain an insight into the problems concerning patient safety culture, based on the analysis of data, collected after interviewing healthcare specialists working in surgical and nonsurgical units in selected Bulgarian hospitals. This was a cross-sectional online study using a web-platform and the Bulgarian Version of Hospital Survey on Patient Safety Culture. It was conducted among healthcare workers (n = 620) in 2021. The B-HSOPSC incudes 42 scales grouped in 12 different domains. We compared the percentage of positive ratings and outcome dimensions between surgical and other hospital departments with the nonparametric Mann–Whitney U test, χ2 tests, Fisher’s Exact Test, and OR. The results showed that there are no statistically significant differences between the ratings on Patient Safety Culture given by the surgical and the nonsurgical staff except for the dimension “Hospital management support for patient safety”. Results from the study highlighted that the most important aspect of hospital patient safety is the shortage of medical staff in both surgical and nonsurgical hospital units. Communication, work shift organization, handoffs and transitions between shifts and among different hospital units, as well as communication with line managers were rated as satisfactory in Bulgarian hospitals.

Perception of medical-surgical wards’ patients from safety culture: A cross-sectional and multicenter study

Background and aims: Identifying the strengths and weaknesses of safety culture in hospitals can be the basis for developing plans to improve the safety status of patients. This study aimed to determine the level of patient safety culture (PSC) from their view in medical-surgical wards of five teaching hospitals in Zanjan. Methods: This cross-sectional study was performed on 245 patients admitted to medical-surgical wards of five teaching hospitals in the Zanjan, Iran, from 2020-Agust-22 to 2020-November-21. Poisson time sampling method was used. Data collection was performed using a demographic information questionnaire and PSC scale. The data were analyzed using SPSS 18 and descriptive statistics, independent t test, and one-way analysis of variance. Results: The mean of total PSC was 3.63±0.60 out of 5. The highest mean of PSC was related to organizing and planning for care (3.87±0.71) and communication and teamwork (3.80±0.70). The lowest mean was related to information dissemination (3.31±0.60). Conclusion: Safety culture or, in other words, patients’ perception of the safety situation prevailing in studied wards and hospitals was moderate. Therefore, it needs to be upgraded and improved. Among the various dimensions of patient-perceived safety, the manner and extent of information sharing was worse. It is suggested that plans be made to direct actions to improve the patient’s safety culture in general and in the field of information dissemination in particular.

Concern for Patient Safety Culture of ECMO Team in Emergency Department: A Cross-Sectional Survey.

To evaluate the patient safety culture status of the ECMO team in the emergency department of a tertiary care hospital. A cross-sectional survey was conducted in the emergency department of Xiangya Hospital from September 1st to 30th, 2021. The Chinese version of HSOPSC electronic questionnaire was administered to all staff involved in ECMO management and initiation. A total of 152 ECMO team members completed the survey. Among the 12 dimensions of patient safety culture, 4 dimensions recorded relatively high positive response rates (>50%): organizational learning-continuous improvement (87.1%), teamwork within units (86.8%), feedback and communication about errors (58.5%), and supervisor/manager expectations and actions promoting patient safety (55.6%). 8 dimensions recorded relatively low positive response rates (<50%): nonpunitive responses to errors (24.1%), hospital handoffs and transitions (27.1%), staffing (28.6%), the frequency of event reporting (32.4%), teamwork across units (33.2%), communication openness (39.7%), management support for patient safety (48.7%), and overall perceptions of patient safety (49.3%). The overall level of patient safety culture was measured at 47.6%. The ECMO team should immediately address issues of nonpunitive responses to errors, hospital handoffs and transitions, and staffing to improve the safety of ECMO. Going forward, the head of the ECMO team should regard patient safety culture as a top priority, improve staff satisfaction, standardize the transfer and handover process, and create a blame-free environment to facilitate event reporting.

PATIENT SAFETY CULTURE ASSESSMENT IN TERTIARY CARDIAC CARE CENTRE IN WESTERN ZONE OF INDIA: A CROSS SECTIONAL STUDY

Background: Patient safety culture is an essential part of quality health care delivery and is now a major global concern. Patient safety culture is defined as the attitudes, values, beliefs and perspective staff share within the organization. Therefore, present study aimed to evaluate patient safety culture and associated causes among the health care workers. Methods: A cross sectional study was carried out using a predesigned questioner hospital survey on patient safety culture (HSOPSC) from June to July, 2021. A total 1590 employees submitted the questioner form. Results: The overall positive patient safety culture was 77.52 percent. Ninety-six percent of staff agreed about staff help each other during busy timings. On logistic regression analysis, regularly work review (β=0.14,95%, P=0.000), patient safety culture affects when work place is rushed (β= 0.32,95%, P=0.000), staff feel like their mistakes are held against them (β= 0. 14, P=0.000), HOD takes action to address patient safety concerns that are brought in attention (β=0.15, P=0.000), staff speak up when they see something that negatively affects (β=0.08, P=0.04), good corporation and coordination with hospital’s other department (β=0.11, P=0.02), during the shift changes important information is left out (β= 0.14, P=0.000) and fall preventive measures are taken while transferring patients (β=0.08,P=0.01) were significantly associated with the level of patient safety culture. Conclusion: Patient safety culture assessments, helps the healthcare organizations to become aware of present status of the organization in regards to areas which needed to urgent attention, weaknesses, existing present safety problems, the gap which creates the patient safety issues and helps the organization to improve their errors.

The impact of hospital accreditation on patient safety culture in the health services implementation at Sanglah General Hospital, Bali

Background and purpose: Hospital accreditation is carried out to evaluate hospitals' performance in providing patients with excellent and safe health services. One aspect of service quality is patient safety culture. This study aims to determine the impact of hospital accreditation on patient safety culture in health service implementation at the Sanglah General Hospital in Denpasar City, Bali Province.Methods: This study used a qualitative research design. In-depth interviews were conducted to 11 informants consisting of hospital management, clinicians and nurses which were chosen purposively since they were directly related to the assessment and supervision of health service providers at Sanglah Hospital. The interview guide was based on the Modified Manchester Patient Safety Framework (MaPSaF). Data were analyzed with a thematic approach.Results: The interview results show that the ten dimensions of patient safety culture had been implemented well but still need to be improved in the dimensions of giving priority to safety, system errors and responsibilities, recording incidents and best practices, learning and influencing change, also education and training of staff and teamwork. The maturity level of patient safety culture at Sanglah Hospital is at generative culture in four dimensions and at proactive culture on the six dimensions of patient safety culture.Conclusion: Accreditation had a positive impact on the maturity level of patient safety culture at a proactive level, mainly changes in knowledge, attitudes, and behavior in health service providers. However, in its implementation, there were still constrained on infrastructure, time, and energy.

Improving the culture of safety among healthcare workers: Integration of different instruments to gain major insights and drive effective changes.

The Safety Attitude Questionnaire (SAQ) and Manchester Patient Safety Framework (MaPSaF) are known as effective tools to assess patient safety culture and climate and develop targeted strategies. However, they are seldom applied in an integrated way. The aim of this study was to conduct an implementation project through a novel use of both instruments to gain unique insights. The Italian version of MaPSaF and SAQ were administered to 1,759 healthcare workers from three Italian hospitals (response rate: MaPSaF 70.5%, SAQ 61.6%). MaPSaF evaluation proved an overall bureaucratic level of patient safety culture. SAQ scores showed a predominance of neutral scores (75.99%). The dimension perception of management gained the lowest mean score (53.32), while Stress recognition obtained the highest (75.17). Safety climate perception differed significantly among groups: working in a small hospital, in a medical department, and being a physician were associated with the most positive results. The majority (67.1%) of responders to both MaPSaF and SAQ considered the two instruments as providing with different and complementary information. Overall, results showed that an integrated approach in the evaluation of an organisation's safety culture may result useful for an in‐depth analysis of the criticalities and the adoption of appropriate improvement strategies.

Patient Safety Culture in a Teaching Hospital in Eastern Province of Saudi Arabia: Assessment and Opportunities for Improvement.

PurposeThe objective of the present study is to explore the perceptions of patient safety culture (PSC) among King Fahd University Hospital’s (KFUH) employees and to develop recommendations to overcome the factors that impede the integration of PSC in the study setting.MethodsThis is a cross-sectional study that assessed the level of PSC at KFUH. This study used the Hospital Survey on Patient Safety Culture tool from all KFUH healthcare workers (n=900) in 2018.FindingsThe response rate of the study was 67%. Findings show that KFUH excelled in three PSC composites: continuous organizational learning, feedback and communication about error, and frequency of events reported. In contrast, staffing, teamwork within units, and non-punitive response to error yielded low composite scores.Originality/ValueThe strength of the present study was the use of a valid questionnaire that has been used widely in the literature with a large sample size, which yielded valid results. It is, to our knowledge, the first research study that analyzes health workers’ perceptions on patient safety culture in a teaching hospital in Eastern Province in KSA and compares it with Agency for Healthcare Research and Quality (AHRQ) and Saudi Hospital Survey on Patient Safety Culture (HSPSC). Results from the study highlight the need to employ an adequate number of workers, implement continuous patient safety training programs, and adopt safety programs and policies.
