Profile of a disabled person with HIV/AIDS 18 years and older from the standpoint of the International Classification of Functioning

The spread of HIV infection continues not only in the Russian Federation, but throughout the world. With the onset of secondary lesions in HIV infection, patients develop persistent dysfunctions of various organs and body systems, leading to a limitation in the main categories. The International Classification of Functioning and Disabilities and Health ICF, as a tool for describing the life of an individual, is gradually being introduced into medical practice, most often used to assess the effectiveness of the rehabilitation process. By now HIV infection/AIDS does not have a basic comprehensive description in terms of the ICF. The authors, in the article, provide an analysis of the existing problems from the standpoint of the ICF in patients and disabled people with HIV infection/AIDS. As a result of the analysis, the authors propose a set of domains that correspond to the most common disorders in patients and disabled people with HIV infection and characterize limitations in daily activities (self-service and domestic life) and in the main areas of life (education, work and employment).

Using the provisions of the International Classification of Functioning in the assessment of disability in people with HIV infection aged 18 years and older

To date in Russia continues the spread of HIV/AIDS. At the stage of secondary lesions in HIV infection, the patient has persistent violations of the functions of various organs and systems of the body, which reduce the quality of life, lead to a restriction of human life, which requires assistance from society and the state. The Russian Federation has established a system of economic, legal, and social measures to support disabled people, as well as all citizens with HIV infection. The establishment of a disability group, its causes, timing, time of onset, and the needs of a disabled person in various types of social assistance is assigned to federal institutions of medical and social expertise.
 A retrospective analysis of 100 protocols of medical and social examination of citizens with HIV infection was carried out by random sampling/AIDS in federal state institutions of medical and social expertise of St. Petersburg and other regions of Russia in the period from 2017 to 2020. The purpose of our study was the possibility of including the provisions of the International Classification of Functioning, disability and Health in the assessment of categories of disability in people with HIV-AIDS. The frequency and severity of disability, designated in the form of domains of the international classification of functioning Activity and participation in the group of patients under consideration, were studied, while the most striking or generalizing domain was taken from each section, characterizing the restriction of each category of vital activity established in a disabled person with HIV infection (d599 unspecified Self-care; d450 Walking; d230 Daily routine; d350 Conversation; d710 Basic interpersonal interactions; d859 Work and Employment, other specified and unspecified).
 According to the results obtained, the most common functional disorders lead to restrictions in daily activity (self-care and everyday life) and in the main areas of life (education, work and employment). Violations of physical and cognitive functions as the process progresses have a significant impact on the ability to work of AIDS patients, limiting their choice of occupations and leading to termination of employment.
 The International Classification of Functioning, Disability and health is a convenient tool that reflects the level of functioning of an individual at various stages of the disease.

Clinical approach to lung diseases in HIV-infected patients (review)

The review presents literature data on therapeutic and diagnostic management of pulmonary diseases in HIV-infected patients. The article provides different practical approaches in the diagnosis of HIV-associated lung lesions and the spectrum of probable diseases, depending on the degree of immunodeficiency and the severity of clinical manifestations. The reference clinical signs of the most common opportunistic diseases with possible lung involvement are presented. The literature review highlights the feasibility of accurate etiological and invasive diagnosis of lung lesions in different clinical settings, as well as the need to designate various options of empirical antimicrobial therapy. The features of antimicrobial therapy and, in particular, the use of fluoroquinolones during the differential diagnosis of pulmonary lesions in HIV infection are discussed. The difficulties of verifying of HIV-associated lung diseases indicate the need of unified therapeutic and diagnostic approaches for use by various physicians (general practitioners, pulmonologists and phthisiologists).

Dermatological aspects of HIV infection: personal clinical observation

Skin and mucous membrane lesions are significant clinical manifestations of HIV infection. Dermatoses associated with HIV infection are characterized by persistent, increasing severity. Atypical symptoms may occur. These dermatoses are resistant to therapy. They are often associated with lymphadenopathy. These criteria are sufficient grounds for supposing the presence of HIV infection, especially when taking into account visceral and systemic abnormalities, as well as the general status of the patient.Up to 92 % of HIV-infected people suffer from various dermatoses. In 72 % of patients, two or more skin diseases occur simultaneously. Skin and mucous membrane lesions in patients with HIV are often one of the first manifestations of HIV infection. Hence, the role of the dermatovenerologist consists in the timely detection of HIV infection. The spectrum of skin lesions in HIV infection is extremely wide. Conventionally, there are three main groups of dermatoses: infectious, non­infectious and neoplastic.Clinical examples were given of skin pathologies manifestation in HIV­infected patients as the first HIV infection display in the patients.Examination on HIV infection was conducted in the clinicodiagnostic laboratory of regional communal establishment «Kryvyi Rih Dermatovenerologic dispensary» with making rapid immunochromatographic tests «Wantai Bio­Pharm» and carrying out immunoassay studies using immunoassay systems for the determination of antibodies to the human immunodeficiency virus of the first and second types «Recombinant-HIV 1,2-MBA MedBioAlliance, Ukraine».HIV-­status in the inspected patients was confirmed by the reference­laboratory of Kryvyi Rih AIDS Prevention and Cont­rol Centre. Clinical cases of HIV infection in patients with various dermatoses were considered, in particular, in a patient with seborrheic dermatitis and disseminated streptoderma, in a patient with pustular psoriasis, in a patient with rosacea. Cases of HIV associated with toxicoderma and urticaria were described.The presence of various opportunistic infections in HIV-infected patients, as well as various dermatological manifestations, requires greater vigilance of dermatologists and family doctors as first-time doctors in order to improve timely diagnosis and treatment effectiveness.

KIDNEY LESIONS IN HIV-INFECTED PATIENTS

Introduction. HIV prevalence is one of the most important issues of contemporary medicine. Over a 30-year history of this disease more than 75 million people have been infected with HIV, nearly 30 million adults and children of died. In the future decades, its significance in world premature mortality rates continues to rise. The objective of the study was to establish clinical and laboratory features of kidney lesions in HIV infection.
 Methods. The study involved 292 HIV-infected patients, who were managed outpatiently at the Chernivtsi Regional AIDS Center. Taking into account the main markers of kidney lesions: persistent proteinuria and glomerular filtration rate <60 mL/min/1.73 m2, 48 persons were diagnosed with chronic kidney disease (CKD), which was very frequently accompanied by dysfunction of these organs.
 Results. Increasing proteinuria rate is accompanied by a significant renal dysfunction and more frequently is combined with arterial hypertension as well as hematuria without significant differences in the incidence of opportunistic diseases. The mean reciprocal correlation between the levels of proteinuria and glomerular filtration rate (r=-0.562, p<0.01), as well as between the levels of proteinuria and hemoglobin (r=-0.596, p<0.01) have been established as well.
 Conclusions. Kidney lesions in HIV-infected are most often characterized by tubulointerstitial lesions. At the same time, glomerular kidney lesion, which is much less common, is accompanied by a significantly higher level of HIV RNA.

Morphological characteristics of brain lesions in HIV infection.

Currently, diseases of the central nervous system are a common cause of death of patients with HIV infection. The article presents results of retrospective analysis of etiotropic structure of 186 lethal cases and typical pathological changes of brain tissue of patients aged 21 to 61 years who had confirmed diagnosis of HIV and died of diseases associated with CNS damage in the Municipal infectious hospital in the city ofDnepr(Ukraine) in the period from 2011 to 2016. The most common causes of death in patients with HIV infection and CNS infection were cerebral tuberculosis (75.0%) including co-infection (n = 35) and meningoencephalitis that developed as a result of yeast fungal (15.6%). In the study of neuropathological processes in the autopsy material of the brain tissue of patients with HIV, various multifocal and diffuse morphological changes have been identified that can be presented in this work in the form of 4 groups: 1) Multiple nonspecific changes associated with both direct action of HIV and co-factors : infection, inflammation, hypoxia, etc., 2) Viral-induced vasculitis with hemorrhages, 3) Subacute aseptic encephalitis, 4) HIV-associated opportunistic infections. General pathomorphological changes in brain tissue are revealed that can cause similarities of clinical manifestations, can complicate differential diagnosis of cerebral lesions of various etiologies, and lead to a divergence of pre and postmortal diagnoses.

Evaluation of caregivers’ knowledge, beliefs and practices regarding oral lesions in HIV-patients: A pilot study

Seventy to ninety percent of HIV-infected individuals have oral lesions that affect their quality-of-life. Caregivers play an important role in complementing the healthcare team. It is essential that the caregivers have optimal knowledge, beliefs and practices toward the management of care regarding oral-lesions in HIV-infected patients in caregiving institutions in Johannesburg, South Africa. To measure the knowledge, beliefs and practices relating to oral lesions in HIV infection amongst caregivers in caregiving institutions in Johannesburg. This cross-sectional pilot study conducted during 2005 and 2006 involved caregivers in palliative care institutions in Region 8, Johannesburg Metropolitan area (n = 52). Using a questionnaire, information obtained included demographics, knowledge, beliefs and practices with regard to oral care for HIV-positive patients. Principal component analysis was used to extract questionnaire items of the knowledge domain. The knowledge domain displayed excellent internal consistency (Cronbach α-score of > 0.91). Of the caregivers, 65.4% had received training in oral care. Compared to caregivers not trained in oral care (NTOC), those caregivers trained in oral care (TOC) were more likely to be older (31 yrs vs. 44 yrs; p= 0.001), female (72% vs. 94%; p = 0.03) and were less likely to be employed on a full-time basis (94% vs. 68%; p = 0.03). Compared with NTOCs, TOCs had more knowledge regarding HIV-specific lesions and ‘routine’ oral-care for the patients (p < 0.005). Similarly, TOC caregivers had more positive beliefs about oral care for those living with HIV (p < 0.05). Oral manifestations of HIV/AIDS in China: a review

The purpose of this study is to review the Chinese-language medical and dental literature from 1982 to 2008 on oral manifestations (OMs) of patients with HIV/AIDS for introducing the spectrum of OMs of the patients in China. All data were extracted from 18 references which had used diagnostic criteria for HIV/AIDS. Four of the references had used the EC-Clearinghouse classification for oral lesions in HIV infection. The feasible overall rate and 95% confidence interval (95%CI) of the data on OMs were calculated. Risk group analysis revealed that, of 203 patients, 64.3% were men and 35.7% were women (age range, 5 months to 64 years; mean age in three studies, 34.0, 34.3, and 36.1 years). Of these patients, 22.2% were infected by sexual contacts, 11.8% by intravenous drug use (IDU), 59.6% by blood or its products, 2.9% by mother to child transmission, and 3.4% were unclear. In 203 patients, oral candidiasis (OC) was the most common lesion (66%, 95%CI = 59.48-72.52%), followed by herpes simplex (HS) (22.2%, 95%CI = 16.48-27.92%), ulcerative stomatitis (14.8%, 95%CI = 9.92-19.68%), salivary gland disease (11.3%, 95%CI = 6.94-15.66%), oral hairy leukoplakia (OHL) (9.8%, 95%CI = 5.71-13.89%), necrotizing gingivitis (5.9%, 95%CI = 2.66-9.14%), Kaposi's sarcoma (2.9%, 95%CI = 0.59-5.21%), other malignant tumors (2.9%, 95%CI = 0.59-5.21%), and linear gingival erythema (2.0%, 95%CI = 0.07-3.93%). The spectrum of OMs reported from China is similar to that described in the international literature. Present data are useful to supplement international resources of HIV/AIDS research.

Assessment of risk behaviours and factors associated with oral and peri-oral lesions in adult HIV patients at Tikur Anbessa Specialized Hospital, Addis Ababa, Ethiopia

Background: HIV patients have several habits that can produce oral lesions. However the association of oral habits and risk behaviors with oral and peri-oral lesions in HIV patients has not been studied in Ethiopia to date. Objective: To assess the relationship of oral and peri-oral lesions with oral habits risk behaviors socio-demographic factors and clinical stages of disease among HIV infected adult Ethiopians. Methodology A cross-sectional study was conducted on consecutive adult ART naive HIV patients at ART clinic of Tikur Anbessa Specialized University Hospital Addis Ababa Ethiopia during December 2005 to July 2006. The EC-clearing houses classification and diagnostic criteria for oral lesions in HIV infection was used. Written informed consent was obtained before enrollment to the study. Results A total of 384 patients 126 (33%) males and 258 (67%) females were evaluated. The mean + SD age of males was 39.9 +10.56 and females was 33.17 + 8.85 years. Fifty seven (15%) of the unit of analysis were smokers 178 (46%) alcohol consumers and 217(57.1%) khat chewers use tooth sticks for dental cleaning. Of which 132 (34.7%) clean their teeth at least once a day and 302 (79%) gave history of recent significant weight loss. Linear regression analysis showed association of oral and peri-oral lesions with less frequent cleaning (p=0.038 p=0.03 respectively) means of cleaning (p=0.004; p=0.001) broad-spectrum antibiotic use (p=0.019 p=0.008) advanced stage of HIV (p=0.016 p=0.001) and CD4 level below 200 Cells/mm3 (p=0.001 p=0.01). The oral lesions were seen more frequently among smokers (p=0.028) heavy alcohol consumers (p=0.041) those with age above 40 years (p=0.016) and recent weight loss (p=0.001). At the level of binary logistic regression analysis smoking (p=0.005) CD4 count less than 200 (p=0.039) and stage 3 (p=0.022) and stage 4 disease (p=0.046) were found to be independent risks while daily dental cleaning habit was found to be protective (p=0.021). Conclusion Oral and peri-oral diseases among HIV/AIDS patients and are significantly associated with correctable risk factors like smoking and poor oral cleaning habits. Lesions are also frequent with evidence of advanced HIV disease and low CD4 count. These findings demonstrate the need for oral health examination and education to improve on risk behaviors and oral care in HIV/AIDS patients.

Oral Lesions in HIV Infection in Developing Countries: an Overview

HIV infection is a major global health problem affecting developing and developed countries alike. Oral lesions that are associated with this disease are important, since they affect the quality of life of the patient and are useful markers of disease progression and immunosuppression. Oral lesions in HIV infection have been well-documented in developed countries, but there are fewer reports on oral lesions from developing countries. Oral candidiasis is the most common opportunistic infection seen in all continents. Kaposi's sarcoma has been reported only from Africa and Latin America, while histoplasmosis and penicilliosis were reported in patients with advanced disease from Thailand. HIV-associated salivary gland disease has a high prevalence in Africa and Latin America, especially in the pediatric group. It is clear that there are considerable regional variations in the oral manifestations of HIV infection, depending both on the populations studied and on the clinical expertise available, among other factors. Well-designed and -documented studies are necessary for the correct assessment of the nature and magnitude of the problem in developing countries, if oral health measures are to be effectively formulated for the HIV-infected.

Access to oral health care for HIV patients in Nigeria: Role of attending physicians

Objective: To assess the knowledge of attending physicians on oral manifestations of HIV infection and their attitude and practice towards dental referral for HIV patients. Method: A self-administered questionnaire survey of attending physicians in outpatient clinics of two teaching hospitals in Lagos and the National hospital, Abuja. Results: seventy-eight (78) doctors returned questionnaires. 16 of them (20.5%) could list correctly three oral lesions in HIV infection, 40 (51.3%) listed two, 18 (23.1%) listed one while 4 (5.1%) could not list any. In managing oral health complaints by patients, 59 (75.5%) agreed that their patients had complained of oral symptoms, of these, only 18 had ever referred a patient for dental care. Majority (84.6%) agreed that HIV patients should have regular dental check ups. Conclusion: The education of both patients and health providers in Nigeria on the importance of good oral health in the quality of life of the HIV patient is necessary. The significance and management of periodontal lesions in HIV infection.

It is now almost 20 years since we reported the existence of a previously unknown lesion, oral hairy leukoplakia, and its unexpected nature as the only human disease in which there is prolific replication of the gamma-herpesvirus, Epstein-Barr virus (EBV). Since then, it has become clear that, in the HIV-infected population, oral lesions are of particular significance. Their presence in individuals of unknown HIV serostatus is highly suggestive of HIV infection, while in people who are known to be HIV-infected, the development of oral candidiasis or hairy leukoplakia--often the very first lesions to occur--indicates that the battle between HIV virion production and destruction of immunologically important cells on, versus replacement of those cells has shifted in favour of HIV. These observations have led to the almost universal inclusion of oral lesions in staging and classification schemes for HIV infection. Recently, lower frequencies of oral disease have been seen in those on HIV therapy, except that oral warts may become more common as the viral load falls and CD4 count rises. To review the significance and management of periodontal lesions seen in HIV infection. Traditional review. HIV-associated periodontal lesions may be categorized as unusual forms of gingivitis, necrotizing periodontal diseases and exacerbated periodontitis. These lesions are significant in the extent to which they mark the underlying HIV disease and have service planning implications. Only limited data are available to inform guidelines for the management of individual patients. Research of the effectiveness of interventions for HIV-associated periodontal lesions is needed.

Classifications of oral lesions in HIV infection.

Manifestations of immunosuppression may take the form of opportunistic infection, and neoplasia. While this paper has focused on gingival and periodontal manifestations. these tissues cannot be evaluated in isolation. The presence of involvement of other oral tissues such as the cheek or tongue with manifestations associated with HIV such as hairy leukoplakia, Kaposi's sarcoma at these sites, and candidiasis in addition to periodontal manifestations may further increase the clincal suspicion of underlying immunosuppression and/or progression of the immunosuppressive state. The periodontist plays an essential r le in identifying the periodontal status of an individual and has an important r le to play in early recognition of signs and symptoms of HIV disease or progression of the medical condition. Only through such recognition can appropriate definitive diagnostic testing be conducted, and appropriate therapeutic intervention for the oral condition and the systemic condition be considered.

Theoretical basis for the activity of thalidomide.

The revival of thalidomide began shortly after the drug was withdrawn from the market because of its teratogenic properties. Therapeutic effects of thalidomide were found accidentally in leprosy patients with erythema nodosum leprosum (ENL). Subsequent research widened the understanding of the activity of thalidomide, and with improved methodology and the augmented background knowledge of immunology it was possible to interpret the properties of thalidomide more coherently. Effects on tumour necrosis factor-alpha (TNFalpha) release play an important role in the ability of thalidomide to affect the immune system. Alteration of synthesis and release of cytokines such as interleukin (IL)-1, IL-2, IL-4, IL-6, IL-8, IL-10, IL-12 and interferon-gamma is involved in the complex mechanisms of thalidomide. Thalidomide targets leucocytes, endothelial cells and keratinocytes, affecting them in a different manner and at different cellular levels. Changes in the density of adhesion molecules alter leucocyte extravasation and the inflammatory response in the tissue involved. Several mechanisms for the teratogenic action of thalidomide are currently under review, but this mode of action of the drug still remains unclear and we review evidence-based hypotheses for the teratogenicity of thalidomide. Thalidomide shows significant clinical impact in several diseases such as ENL in lepromatous leprosy, chronic graft-versus-host disease, systemic lupus erythematosus, sarcoidosis, aphthous lesions in HIV infection, wasting syndrome in chronic illness, inflammatory bowel disease, multiple myeloma and some solid tumours. In 1998 the US Food and Drug Administration approved thalidomide exclusively for the treatment of ENL, and strict conditions were stipulated for its use in order to prevent teratogenic adverse effects. However, despite the promising findings of thalidomide at the molecular level, namely its anti-TNFalpha properties and its intercalation with DNA, and activity in clinical trials, there is still a great need for more intensive research.

Some unusual oral lesions in HIV infection: comments on the current classification.

Of the numerous oral lesions reported in HIV-infected patients, four entities will be discussed in this paper: linear gingival erythema; cystic lymphoid hyperplasia of the parotid gland; oral non-Hodgkin's lymphoma; and oral squamous cell carcinoma. Based on the literature and the author's personal experience, it appears that linear gingival erythema is perhaps a specific HIV-associated periodontal lesion, but that insufficient data are available in the literature yet to classify this periodontal lesion as a lesion that is strongly associated with HIV infection. On the other hand, cystic lymphoid hyperplasia of the parotid gland has been rather widely reported and has characteristic histopathological features that would justify consideration of this lesion as highly indicative of an underlying HIV infection. Oral non-Hodgkin's lymphoma, at present regarded as a lesion 'strongly associated with HIV infection', is less characteristic in this respect and should merely be classified as 'a lesion seen in HIV infection'. Oral squamous cell carcinomas, not included in the most recent International Classification of Oral Manifestations of HIV infection, should be listed in Group 3 of that classification, representing 'lesions seen in HIV infection'. Further criteria should be developed that will enable distinctions to be made between lesions 'strongly associated with', 'less commonly associated with', and 'seen in' HIV infection.

Prognostic value of oral candidosis and hairy leukoplakia in 111 Mexican HIV-infected patients.

A follow-up study was carried out to evaluate the prognostic value of hairy leukoplakia (HL) and oral candidosis (OC) in a cohort of 111 asymptomatic Mexican HIV infected patients. Oral exams were performed at baseline and every 6 months, from September 1989 to March 1994. Chi-square contingency table test, the Kruskall-Wallis one-way analysis of variance, the Kaplan-Meier product-limit method and the log rank test were used for the analysis. Univariate and multivariate Cox's proportional hazards analysis were also performed. Fifty-four patients (51%) progressed to AIDS (initially 36 CDC-II and 18 CDC-III). Individuals with HL and/or OC, showed faster development to AIDS than subjects without lesions or other HIV-related manifestations (P = 0.008). The presence of OC, HL or both always remained significant despite adjustment for total lymphocytes, CDC stage, zidovudine therapy or its combinations. Oral lesions in HIV infection may be regarded with other clinical and laboratory studies as markers of HIV disease progression and as indicators to begin antiretroviral treatment.

A consensus has been reached on the classification of the oral manifestations of HIV infection and their diagnostic criteria, based on presumptive and definitive criteria. The former relate to the initial clinical appearance of the lesion and the latter are often the result of special investigations. Candidiasis, hairy leukoplakia, specific forms of periodontal disease [linear gingival erythema, necrotising-(ulcerative) gingivitis and necrotising(ulcerative) periodontitis], Kaposi's sarcoma and non-Hodgkin's lymphoma are strongly associated with HIV infection. Lesions less commonly associated with HIV infection and lesions seen in HIV infection, but not indicative of the disease, are also listed.
